✓ Vincent van Gogh's life, letters, and art are the framework for this existential speech about the nature of alienation, as well as its threat to humanity and to the artful practice of medicine. The honest, human voice expressed in van Gogh's art stands in opposition to alienation, which occurs when we divide the world into two parts: the “perfect” world of science versus the “flawed” world of human experience. Bridging this divide allows for an “authentic” life, one which honestly defines itself and faces difficult human truths. The most difficult truth relates to our own mortality, but it must be faced if we are to understand the value of existence. Film clips from Woody Allen's Hannah and Her Sisters illustrate how an artist's portrayal of these issues can be both profound and humorous, and how art brings us closer to our own humanity and to the essence of medicine. Neurosurgeons are warned about the lure of science and technology as a substitute for purpose and meaning, both as physicians and as human beings. The role of the American Association of Neurological Surgeons is explored and found to offer neurosurgeons a bridge away from alienation and toward a neurosurgical community. Neurosurgeons are urged to find meaning through service to their profession and to find the voice and art of medicine. [Note: Actual film clips were used when this address was delivered. Unofficial transcripts of the clips have been included in this article so that the integrity of the speech would not be compromised.]

Anamnesis (Greek: anamnese - memory) is a conversation with the patient in order to gather all the information that is essential for discovering the true nature of the disease and making an accurate diagnosis. The oft-quoted saying "Listen to your patient; he will tell you what his diagnosis is" shows the value of the anamnesis in diagnosis. It is believed that the anamnesis has the greatest importance in establishing the correct diagnosis. 50%-70% of the diagnosis is made already on the basis of the anamnesis. Proper history taking is a medical skill that requires from the examiner: good knowledge, a lot of time and patience. In addition, cultural behavior and certain personal qualities and skills of the doctor are also important. But taking an anamnesis is not only a science, but also a skill and an art, as it requires interpretation and clarification of the conversation with the patientAgood clinical assessmentcomes first, and it is reached after many years of practice. Patients with the same diseases can express their symptoms differently, so the main characteristic of medicine as an art is how the doctor interprets different descriptions of the same phenomenon. In the process of taking an anamnesis, doctors are detectives, and the patient (and family or companions) are witnesses. If we ask the right questions and make sure we really understand what the patient has experienced, we are much more likely to arrive at an accurate diagnosis quickly. The point of contact between art and science in medicine is where the doctor feels the emotion that brought the patient to the office. The doctor must not only listen to the words the patient uses, but clarify their meaning. When he learns this, he becomes an artist of the best kind.

Background There has been an unprecedented explosion in the popularity of herbal preparations during the last few decades. Herbal medicines are commonly used by HIV/AIDs clients. There is limited data on the prevalence of herbal medicine and correlating factors of herbal medicine use in Malawi. This study establishes prevalence and factors contributing to the use of herbal medicine among HIV/AIDS clients attending the ART clinic at QECH, Blantyre Malawi.Methods A cross-sectional study design was used to interview 211 conveniently sampled clients at QECH ART clinic. The questionnaire addressed socio-demographic, clinical characteristics, NCD-HIV comorbidity, and herbal medicine utilization. The main outcome of the study was herbal medicine use since the initiation of ART. Logistic regression analysis was done in Stata version 16. Both unadjusted and adjusted models were fitted for potential confounders. ResultsThe prevalence of use of herbal medicine was reported in 17.5% (n=37) of the ART clients. The adjusted logistic regression analysis showed that urban residence was statistically associated with reduced use of herbal medicine (adjusted Odds Ratio –AOR: 0.04, 95% CI: 0.169, 0.976).ConclusionThere is a high prevalence of use of herbal medicine among clients taking ART. Herbal Medicine has the potential to cover the gaps in health coverage in rural communities.
