Texto completo da fonteIntroduction: The term colorectal cancer (CRC) means cancer of the colon and rectum.Colorectal cancer is the third leading cause of death worldwide with statistically rising mortality and affects thousands of people every year. The disease is investigated with diagnostic imaging where the most common is colonoscopy but also with computed tomography (CT) if the colonoscopy is incomplete or if the patient is unable to undergo a colonoscopy. The examination requires accurate bowel cleansing as well as a limited food intake, which leads to physical as well as psychological concern in the patient. Adequate information before, during and after the examination is necessary to facilitate the patient's mental and physical condition. Aim: The purpose of the study was to compile and illustrate the state of knowledge regarding patients' experiences before and during a CT colon (CTC) examination. Method: The literature study was conducted as a general literature review. Thirteen articles with both qualitative and quantitative approaches were included in the study after a quality review of all.Results: Four main findings were identified in the result. These consisted of physical efforts, concern and anxiety, embarrassment and reluctance for the study related to ethnicity and gender, and the acceptance of discomfort with the study itself. Conclusion: Many of the patients experienced pain during the examination which was related to bowel dilatation. Since many of the patients experienced that the examination was embarrassing and physically stressful, it is very important to have adequate information and a good dialogue. This can reduce their concerns, provide security and increase the manageability of the investigation. It is of utmost importance not to violate the patient's integrity and to provide encouragement during ongoing examination as well as after.

Texto completo da fonteABSTRACT Background The number of people suffering from colorectal cancer increases every year and is the third most common cancer disease in Sweden. Therefore the demand increases for the examination methods in order to be able to make a correct diagnosis and start treatment. CT Colonography is one of the methods often used as a secondary method. Aim The purpose of this study is to examine what significance CT Colonography has in the investigation of colorectal cancer. Method This study is a systematic review. The collection of data were made from the databases PubMed and CINAHL. Studies which fulfil the inclusive criteria and answered the aim of this study were quality reviewed and analyized. Result CT Colonography can show colonic findings and is particularly useful to locate extracolonic findings. The method is important in the preoperative evaluation because of its exact localisation rate and is a good compliment to colonoscopy. CT Colonography is a reliable method with a sensitivity at 94–99.9 % which increases when CAD is used as a reading method, especially in the detection of pathology >10 mm. Conclusion CT Colonography is a valuable method in the investigation of colorectal cancer and other pathologies in the abdomen, colon and rectum. It’s a reliable method with a high sensitivity which increases with CAD as a reading method. A strong advantage of this method is that the whole abdomen is visible in the images so the localisation of extracolonic findings can be detected. Texto completo da fonteBackground: Many patients who participate in CT-colonography find the preparations and the examination itself to be hard. A plain and well written information letter, as well as good communication and patient-centered care from the hospital staff could make the patient experience better. Aim: The aim of the study is to investigate how patients from two different hospitals perceive both the information letter and the oral information from the staff and compare to each other. Method: The study is a qualitative study. Patients have been asked to fill out a survey at the end of their examination. The patients have been strategically selected, inclusion- and exclusion criterias have been adapted. The result of the survey has been analyzed in SPSS. Results: In general the patients were pleased with the information letter. The patients at hospital B were to some extent more satisfied than the patients at hospital A. The majority of the patients from both hospitals were satisfied with the information from the hospital staff. Although the result showed some percentage difference between the hospitals, no significant difference was shown. Conclusion: Overall the participants of the study were satisfied with both the information letter and the information from the hospital staff. Although there were no significant differences, the result showed that the patients from hospital B were more satisfied than the patients from hospital A in all senses.

There are many different ways in order to improve the profession internationally. One can, for example, take advanced practicing to become a reporting radiographer. The advanced practicing has exclusively showed positive results. Nowadays the patients are changing in conditions rapidly that the healthcare industry needs to adapt itself. Even the international development for radiographers seems bright, the need is yet fully realized in Sweden. Purpose: The purpose of this study is to find out which needs and possibilities are available for Swedish radiographers to improve and develop their own professional skills. Method: A qualitative interview study with semi-structured questions. The participants were selected by inclusions and exclusions. The interviews were conducted by telephone or physical meetings and the interviews were recorded and analyzed using qualitative content analysis manifest. Results: The responses from the participants have been varied. Lack of radiographers is palpable. All participants expressed that they had opportunities and possibilities for their radiographers to receive an advanced practicing in skills development even though the supply varied. The result shows that skills development is needed in all areas. Despite this, all hospitals do not express that skill development is important for their radiology department. Conclusion: Advanced skills development for radiographers is an interesting topic. There are more to figure out valid skill development. There is a need to develop radiographers in order to improve individually and together as X-ray department.

Texto completo da fonteA study of the beam distribution characteristics for the clinical linac-based radiosurgical techniques is presented. Two spiral linac-based radiosurgical techniques are developed: the uniform dose-rate spiral irradiation and the dose-rate-weighted spiral irradiation. Both exhibit the same spiraling beam entry trace over the patient's head; however, they differ in their beam distribution along the spiral. The dose-rate-weighted spiral irradiation provides a uniform beam distribution over the 2pi solid angle available in radiosurgery.
The currently existing techniques and the spiral techniques are then compared using the cumulative dose-volume histogram (CDVH) tools available with the McGill Treatment Planning System (MPS). The dose-rate-weighted spiral technique leads to lower dose inhomogeneities within the target volume and better dose conformity within the target. Moreover, it also encompasses smaller volumes of tissue at all isodose levels with larger differences at low isodose levels. A conclusion is reached that the dose-rate-weighted spiral irradiation technique offers interesting advantages over the currently used clinical linac-based techniques.

Texto completo da fonteComputer tomography (CT) - colonography is an examination which is on the rise since coloncancer is more common and acknowledged in todays’ community. In conjunction with a CT-colonography the preparations are demanding and requires the patients' full compliance to get a successful examination. This means that the patient needs a comprehensive information leaflet to understand the extent of the preparations. This study's purpose was to examine if the patient information was sufficient enough in conjunction with the CT-colonography and determine if the experience of the patient information differ between the two hospitals. Also examine if there were and differences genders between each other as well as age. The study will also overhaul if there were any improvement proposals. This study's design followed a quantitative survey with own constructed questions. The number of participants were 51 which filled the survey anonymously during the period March to April 2018. The majority of the participants had a good impression of the information. Between the men and the women there were no differences of the information, the survey answers also showed that the ages was no differnce based on age of the subjects. Few participants had any opinions about improvements but the ones who had wanted to evolve the information in a way that decreased the amount of information. The participants were pleased with the information and there was no significant difference between the two hospitals, gender or age.

Texto completo da fonteAbstract: Prostate cancer is most often diagnosed by a prostate biopsy led by a transrectal ultrasound. Magnetic resonance tomography can also be used to find prostate cancer, and a diagnosis can then be made from the images. More commonly MRI is used as a basis for a biopsy done with TRUS where the MRI is used to localise suspected pathological areas in the prostate and in this way a suitable biopsy spot is chosen. Purpose: To examine the sensitivity of the MRI-camera for prostate cancer, both for imaging alone and for MRI-led biopsy. Method: A systematic literature review was performed where 12 articles were examined and compiled into a result presented as a chart. Result: Shows a high sensitivity with MRI for finding both unsignificant and significant prostate cancer. Conclusion: The results from this study shows that MRI has a high sensitivity for prostate cancer and can with great accuracy discover pathological areas in the prostate. The MRI images can advantageously be used as an effective diagnosing tool for general and significant prostate cancer, a specially combined with transrectal ultrasound.

Texto completo da fonteBackground: Since Wilhelm Conrad Röntgen’s discovery of X-rays in 1896, radiology has undergone great changes. In Sweden, the process of digitalisation radiology was initiated in the mid 1990s. Aim: To describe the organisational changes that took place in five radiol- ogy clinics in Sweden in connection with the digitalisation of radiology. Study questions: How did the number and type of radiological examinations change when comparing two years before with two, four and six years after digitalisation? How did the number and distribution of hours worked change among different staff categories when comparing two years before with two, four and six years after digitalisation? How did the duties change after digitalisation within and between different staff categories at the radiology clinics? How do staff members in key positions describe the implementa- tion of digitalisation and any changes in their professional prac- tice after digitalisation? Method: Quantitative and qualitative methods were combined in order to answer the study questions. Five Swedish radiology clinics took part in the study, and production outcome data were gathered from each clinic. A total of 22 individuals were interviewed, with three to six interviews per radiology clinic. In order to gain an understanding of the positive results at one of the radiology clinics in terms of the increased number of exami- nations and reduction in the number of hours worked after digitalization a case study was conducted at this clinic, in which two questions were posed: How was the digitalisation carried out? How were workflow, im- plementation and changes followed up in detail after digitalisation? Ele- ments of Bramson and Bramson ́s (2005) perspectives and Kotter and Cohen’s (2002) success factors were applied. Findings: All radiology clinics had a small increase in the number of radio- logical examinations, while computed tomography and magnetic reso- nance imaging examinations increased significantly during the period un- der investigation. Thus, more complex examinations were carried out post digitalisation, a fact considered to have led to qualitatively better and safer diagnoses. The total working time was reduced at three clinics, while two exhibited an increase. The total number of hours worked increased for radiologists and radiographers but decreased for secretaries and assistant nurses. In general, radiologists and radiographers performed more tasks post digitalisation, and many of the traditional duties of secretaries and assistant nurses were eliminated or taken over by the computer. Digitalisa- tion enabled greater access to radiological images and patient data in addi- tion to improved quality and diagnostics of the radiological examination. Digitalisation also allowed a better-controlled workflow, as well as im- proving ergonomics, the environment and the coordination with other clinics. The case study interviews revealed themes describing: information, super users, education, clear work flows and routines, staff needs and motiva- tion as well as clear leadership throughout the change process. According to Bramson and Kotter, these themes are important for successful change. Conclusion: None of the individuals interviewed wished to return to the analogue mode of working. The digitalisation led to great changes for the different staff categories, with the advantages outweighing the disad- vantages. To succeed with change, Bramson’s perspectives and Kotter’s success factors should be taken into consideration.

A good introduction to the new graduated radiographer is important for a quick and easy access to the workplace. The aim of the introduction is to feel safe and independent in the workassignments. Few studies highlight the subject of "introduction on a new workplace” and even fewer studies highlight ”new graduated radiographers’ views on the introduction”. A request from a number of radiology departments has led the authors to write this thesis. The purpose of this study is to examine factors new graduated radiographer considered important for a good introduction on a new workplace. A qualitative study was conducted with seven female informants in southern and central Sweden. The collected data was analyzed with a content analysis and three main categories were formed: Experiences of the introduction; Communication and social relations; Suggestions for improvement. Something that was appreciated by our participants was constructive feedback and continuity of supervisors to increase self-confidence. The content in this study has an improvement potential for managers and future colleagues.

Texto completo da fonteBackground: Covid-19 was classified as a pandemic in the beginning of 2020. This forced health care professionals within elderly care, emergency departments, intensive care units, and infection wards with focus on care of patients with covid-19 to work under high pressure, which has been highlighted by various media. Despite playing an important role within covid-healthcare, radiographers have not been acknowledged to the same extent. Aim: To highlight radiographers’ experiences of their role in the covid-healthcare when performing computed tomography examinations of patients who are suspected or confirmed to have covid-19. Material and method: This study has a qualitative design and is based on semi-structured interviews with five radiographers from two radiology departments in Sweden. Results: Covid-19 has led to a change in the work situation for radiographers where a new patient group requires new routines, including more planning and time. The radiographers’ role in covid-healthcare is perceived to be unseen by other departments. Conclusion: The participants' experiences of stress, uncertainty and time spent due to covid-routines affect their work, but have changed during the pandemic as these routines have become part of their daily work. Radiographers are providing patients with good care in connection to the examination, which has been and still is a challenge due to protective equipment feeling obstructive.

Texto completo da fonteIntroduction: All x-ray examinations with ionizing radiation must be justified and optimized. This requires radiology referral of good quality. Aim: The aim of the study was to describe factors that affect the process of justification in radiology referrals. Method: A general literature review based on quantitative studies that have been verified based on its credibility. Result: The results are presented in five categories: justified and unjustified x-ray examinations, lack of clinical information, use of guidelines, introduction of intervention and the design of the radiology referral. Conclusion: The quality of the radiology referral has significance in the process of justification. Number of unjustified x-ray examinations are reduced through increased knowledge, which makes education for referring physicians a keyfactor. This leads to safer healthcare where patients are not exposed to unnecessary ionizing radiation.

Texto completo da fonteBackground: Missed appointments is a problem that effects the hospital economics and the patients’ health, No- show patients is a term for patients thats without informing don't come to their booked appointment.This patient’s group has a higher risk of falling ill urgently or aggravate the already existing disease. A patient cost too the health care system is bigger when they miss booked appointments. Patient’s who decline or select not to participate in screening also increases their risk of falling ill. the Screening patients that’s participate decreases risk of sickness or finding it early. Purpose: The purpose of this study was to examine factors to no show patient’s in radiology. Method: A literature review was conducted. Articles searches using databases PubMed and Cinhal resulted in 15 articles that’s matched the purpose of the study. Result: Six different factors/reasons was found, fear, practical factors, aged, administrations, family and social status. Conclusions: Similar results was found in study’s in other departments of the hospital and there for a generalization was possible. More research needs to be done in to how the seven main branches actually effects missed appointments and no show patient’s. It is of importance to the radiology nurse to know the reasons so they can work towards a positive change in both hospital economics and patient’s health.

Texto completo da fonteBackground Claustrophobia and anxiety are common causes of MRI examinations being interrupted or cancelled. This is not only problematic for the individual patient but also causes a negative impact on the workflow, where the examinations may take up more time than allotted or even cancelled at the last minute. The latter causing a negative financial impact for the hospital and as for the patient it may result in a late diagnosis, or a lack thereof. Aim The purpose of this study was to investigate what methods are used to facilitate patients with claustrophobia or anxiety associated with MRI examinations and to explore related scientific developments. Method The study is a systematic literature study where relevant data was collected by searching relevant databases according to specific selection criteria which was then reviewed, analyzed and compiled into a result. Result The results of the studies showed that strategies applied both before and during the examination may be important for the patient's perceived level of anxiety and could therefore affect the implementation and the diagnostic quality of the MRI examination. Conclusion Various methods, like patient information, sedatives and communication are used to manage patients' anxiety and claustrophobia when undergoing MRI examinations. Good communication and information have proven to be important for the patient's experience. Several alternatives have been studied, such as VR, hypnosis, information films and studies on patients' experiences of the design of the magnetic camera. However, more research is needed.
