Vectors of malaria, filaria, dengue, chikungunya, zika, japanese encephalitis, and kala-azar play an important role in the disease transmission in different eco-settings with variable climatic conditions. Ticks, mites, and fleas also pose a threat to new emerging and re-emerging vector-borne diseases, i.e. Kyasanur Forest Disease (KFD), Crimean Congo Hemorrhagic Fever (CCHF), scrub typhus, and other rickettsioses. Now, the time has come that field oriented entomological work has to shift from open, field based towards closed medical entomological laboratories for undertaking molecular research and pathogen/ virus detection among vector species handling them by minimising human risks. It is imperative to note that there is a strong need for a standard protocol for effective medical entomological laboratory practices while handling the pathogen carrying vector species under laboratory conditions. This may help to prevent the transmission of pathogens/ viruses in case of accidental release of vectors carrying pathogens/ viruses from the entomology laboratories. Such protocols would always help the scientists to minimise risks working in closed conditions. Though, there are guidelines/ procedures available for developing medical entomology laboratory, having facilities for insect rearing, its handling and equipment, however, no specific published protocol or guidelines exist presently in the Indian context. In the present manuscript, the need for a standard protocol for arthropod containment levels (ACLs 1- 4) along with the appropriate bio-safety levels based on the risk potential of pathogen carried by the vector species has been discussed for its application at the ground by the respective health authorities/ institutions.The presence of Standard Operating Procedures (SoPs) and guidelines on Good Medical Entomology Laboratory Practices (GMELP) would help the professionals working in a medical entomology laboratory to minimise risks. There is a need to develop and follow Good Medical Entomology Laboratory Practices (GMELP) for handling the vectors (Arthropods) carrying the pathogens/ viruses at the national/ state/ district level as well as by the research institutes, medical colleges, and universities. The present concept note shall help to provide a guiding principle to develop standard operating procedure (SoP)/ Guidelines for GMELP.

This is a translation of the paper “Recommendations for the application and follow-up of quality controls in medical biology laboratories” published in French in the journal Annales de Biologie Clinique (Recommandations pour la mise en place et le suivi des contrôles de qualité dans les laboratoires de biologie médicale. Ann Biol Clin (Paris). 2019;77:577-97.). The recommendations proposed in this document are the result of work conducted jointly by the Network of Accredited Medical Laboratories (LABAC), the French Society of Medical Biology (SFBC) and the Federation of Associations for External Quality Assessment (FAEEQ). The different steps of the implementation of quality controls, based on a risk analysis, are described. The changes of reagent or internal quality control (IQC) materials batches, the action to be taken in case of non-conform IQC results, the choice of external quality assessment (EQA) scheme and interpretation of their results as well as the new issue of analyses performed on several automatic systems available in the same laboratory are discussed. Finally, the concept of measurement uncertainty, the robustness of the methods as well as the specificities of near-patient testing and rapid tests are described. These recommendations cannot apply for all cases we can find in medical laboratories. The implementation of an objective alternative strategy, supported with documented evidence, might be equally considered.

SummaryDuring the 20th century understanding for quality has changed and international and national requirements for quality have been published. Therefore also medical branches started to establish quality management systems. Quality assurance has always been important for medical laboratories. Certification according to the standard ISO 9001 and accreditation according to the standard ISO 17025 have been the proof of fulfilling quality requirements. The relatively new standard ISO 15189 is the first standard for medical laboratories. This standard includes technical and management requirements for the medical laboratory. The main focus is the proof of competence within the personnel. As this standard is accepted throughout the European Union an increase in accreditations of medical laboratories is predictable.

Accreditation is an instrument which confirms competence according to the international standards. Accreditation Body of Serbia and Montenegro (JUAT) is a federal body which manages accreditation system in Serbia and Montenegro. Within its activities of determining competency of organizations for carrying out certification of conformity assessment it also carries out the accreditation of testing laboratories. Within this kind of accreditation very important place belongs to the accreditation of medical laboratories. Generally, requirements for the competence of testing laboratories are given in standard JUS ISO IEC 17025 which cover testing process. A the medical laboratories have broader field of activity than the usual one for testing laboratories and as they include preanalytical, analytical and postanalytical phase of activity, it is necessary for general requirements to be adapted to the additional specifically requirements related to the activities of medical laboratories. It is those additional requirements that are recognized by the standard ISO/IEC 15189 Quality Management in the Medical Laboratory. For these reasons JUAT has established a commission for generating the accreditation scheme for laboratories in the field of medicine with the task to build and develop accreditation scheme for medical laboratories. In this project for accreditation scheme of medical laboratories development, JUAT has included known and respectful experts in medical science, who will contribute by their expertise, experience and competency in establishing and implementation of requirements necessary at accreditation of medical laboratories.

Abstract In 1975 the Centers for Disease Control, in cooperation with the American Association for Clinical Chemistry Cholesterol Reference Method Study Group, began an investigation to develop a reference method for total cholesterol. Five potential reference methods were compared with the definitive method developed by the National Bureau of Standards before the chemical method of Abell et al. (J Biol Chem 1952;195:357-66) was selected as the recommended reference method. Because acceptance of a proposed reference method depends so greatly on the method's capability for transfer to other laboratories by written specifications and instructions, a transferability testing study was designed and conducted with 14 laboratories. The study consisted of preliminary testing of readiness of equipment, reagents, and personnel followed by transferability testing with eight runs on 10 serum pools. Laboratoires that did not meet readiness specifications had higher CVs in the transferability testing. The study demonstrated that the proposed method permits laboratories to attain a CV of less than 1.5% for one laboratory and of less than 3.0% among laboratories. The mean percent bias value was less than 1.0% for six of the 14 laboratories, less than 1.5% for 12, and less than 3.0% for all 14 laboratories.

Staphylococcus aureus is a Gram positive bacterium that is frequently encountered on surfaces such as benches and skin. This organism being a normal flora of the skin is usually nonpathogenic but becomes pathogenic when found outside its normal flora where it can cause varying number of infections such as nosocomial infections and sepsis which may even lead to death if left untreated. This research was therefore carried out to determine the prevalence of Staphylococcus aureus on noncritical surface areas including: Furniture, Doors and Windows of the laboratories of Federal College of Medical Laboratory Science & Technology Jos, Nigeria. Samples were processed, examined and analyzed accordingly. It was observed that out of the 120 samples examined, 8 were gram positive cocci in clusters under Gram staining examination Coagulase and catalase tests were positive indicating specific biochemical tests to identify the organisms in the 8 organisms as Stapylococcus aureus showing a prevalence rate of 6.67%. The 17 working Benches examined, 3(2.50%) were positive. Out of 75 Chairs examined, 5(4.17%) were positive. The 18 Windows examined, 10 Doors examined showed no growth for Staphylococcus aureus. Distribution according to laboratory sections showed that, out of the 95 samples in main laboratory, 6(5.00%) were positive. Out of the 14 samples examined in side laboratory one, 2(1.67%) were positive while none 0(0.0%) was positive out of 11 samples examined in side laboratory two. The isolation of Stapylococcus aureus from these locations stresses the urgent need for public enlightenment campaigns by the appropriate authorities to educate the laboratory workers as well as the masses on the etiological agents, the possible risk factors, routes of transmission and health implication of Stapylococcus aureus infection and ways of preventing these bacteria in the Laboratories especially in Federal College of Medical Laboratory Science and Technology Jos Nigeria.

Syfte: Att undersöka och jämföra effekten av olika LED-lampor genom mätning av konversionsgrad, härdningsdjup och mikro-hårdhet på en universal nano-komposit. Detta utifrån frågeställningarna: - Stämmer härdningslampornas verkliga irradians överens med den som uppges i produktdatabladen?- Kan härdningsprocessen effektiviseras genom att ljushärda med högre irradians och kortare härdningstid?Material & Metod: Materialegenskaperna - konversionsgrad, härdningsdjup och mikro-hårdhet mättes för kompositen Filtek Supreme XTE (3M ESPE) med fem LED-härdningslampor och olika härdningstider (kort, medellång, lång tid). LED-lamporna som användes i studien var L.E.Demetron II (Kerr), Mini LED (Satelec), D-Light Pro (GC), Epilar Deepcure-S (3M ESPE) och Flashmax P3 (CMS Dental). LED-lampornas spektralfördelning erhölls med en CCD-spektrometer (Avantes Inc.) kopplad till en integrerande sfär (Gooch & Housego) med en fiberoptisk kabel (Avantes Inc.). Data analyserades med ensidig och tvåsidig variansanalys (ANOVA-test) följt av Tukey-test. Resultat: En kort härdningstid och en härdning med låg irradians gav en lägre konversionsgrad, härdningsdjup och mikro-hårdhet jämfört med härdning med längre tid och hög irradians.Slutsats: Härdningslampornas verkliga irradians överensstämmer med irradiansen som uppges i produktdatabladen. En tillräcklig polymerisering uppnås med alla härdningslampor vid samtliga härdningstider. Härdningsprocessen kan effektiviseras och/eller förbättras genom att tillföra mer energi på kortare tid. Att förlänga härdningstiden utöver tillverkarens rekommendationer har en positiv effekt på materialegenskaperna – ökad konversionsgrad, härdningsdjup och mikro-hårdhet.
Aim: To investigate and compare the effect of different LED curing devices by measuring degree of conversion, curing depth and micro-hardness of a universal nanocomposite, based on the questions:- Does the actual irradiance of the curing lamps match the one specified in the product data sheets?- Can the curing process be done more efficiently by light curing with a higher irradiance and a shorter curing time?Materials & Methods: The material properties degree of conversion, curing depth and microhardness were measured for the Filtek Supreme XTE (3M ESPE) composite with different LEDs and after curing at different curing times (short, medium, long). The light curing units used in the study were L.E.Demetron II (Kerr), Mini LED (Satelec), D-Light Pro (GC), Epilar Deepcure-S (3M ESPE) and Flashmax P3 (CMS Dental). The spectral irradiance of the LEDs was obtained with a CCD spectrometer (Avantes Inc.) connected to an integral sphere (Gooch & Housego) with a fiber optic cable (Avantes Inc.). Data were analyzed with one- and two-way ANOVA test followed by Tukey test. Results: Short curing times and low irradiance results in lower values of the material properties compared to longer curing times and higher irradiance.Conclusion: The actual irradiance of the curing lamps is consistent with that stated in the product data sheets. Sufficient polymerization was achieved with all curing lamps at all curing times. The curing process can be made more efficient and/or be improved by adding more energy in less time. Extending the curing time has a positive effect on the material properties – an increased degree of conversion, depth of cure and micro-hardness.

The high quality of modern medical care is built upon the creation of scientific knowledge generated from medical research. While the role of management practices has been rigorously explored across various industries, little is known about management in medical research. I collected data surveying principal investigators of medical research laboratories at the Harvard Medical School to examine the relationship of management practices and research outputs. I find that principal investigators with more effective management practices are associated with higher-impact research (measured by citations). This effect is stronger and more significant in younger compared to older laboratories and remains robust after using different controls. This study helps to increase the understanding of management in a scientific setting and should start a new discussion about the relevance of management in medical research.

A survey on the quality of service provided by state laboratories in the Limpopo Province was conducted during the period 2000 2002. The focus was on accuracy and precision. The service quality evaluation was based on standard deviation index, % deviation and % clinically rejectable results. The scores obtained were evaluated in terms of internationally pre determined cut off limits. Education and training were cited by 97% of the interviewees as the major contributory factors to the poor performance. Subsequent to the discussion, a course in quality assurance was designed, presented and evaluated, as above. Although the post intervention performance results did not meet the international performance standard at the given time yet; they were much closer to norm and significantly better than the pre course performance results. The conclusion is that newly designed course will contribute in reaching the international cut off standard for clinical laborato ries.

As the business evolves, organisations are faced with challenges as today’s customers have changed, and demand a value for the products and services purchased. Customer satisfaction is a key factor to business excellence. The health sector and its service providers are faced with such demands, having to ensure customer satisfaction with limited resources. The aim of the study was to investigate a business model that could be implemented within the public sector to render pathology services. Particular reference was given to the National Health Laboratory Service as a provider of choice for rendering medical laboratory services within public health institutions and related departments. The data has been collected using a self constructed questionnaire which has been distributed to the NHLS centers within the Nelson Mandela Metropolitan Municipality. 45 percent of the employees, in the region took part in the survey. The study tested the perceptions of the respondents with respect to the business model, specifically the determinants of a business model and its effects. The results of the empirical study revealed that a relationship exists between the business model, its determinants and customer satisfaction. Correlation analysis was done between the business model, customer relation management, continuous improvement, talent management, business process reengineering and customer satisfaction. Conclusions and recommendations have been drawn from the study. The NHLS needs to change its approach to business, invest in people education, training and development, improve its communication strategies with customers and realign its processes in order to maintain business excellence and provide a better service within the public sector

With 30 million citizens gaining access to U.S. healthcare through the Affordable Care Act by 2016, healthcare managers need preanalytic employees to ensure quality healthcare services can be provided. The purpose of this qualitative single case study explored strategies used to retain preanalytic employees. The target population consisted of 10 clinical laboratory managers in a single community-based clinical laboratory in the mid-southern United States, selected because of prior success in improved employee retention strategies. The conceptual framework grounding this study was the theory of work adjustment. Data triangulation occurred from using semistructured interviews and company documents. All interpretations from the data were subjected to member checking to ensure the trustworthiness of findings. Using the modified van Kaam method for data analysis, 3 themes included increasing employee training to enhance their roles as stakeholders, provide employee recognition and reward programs, and emphasizing to managers the importance of quality patient care. The application of the findings may contribute to social change by providing healthcare managers with new insights and strategies to retain preanalytic employees, reduce organizational costs associated with recruiting, replacing, and training of new employees, and potentially trained staff that provide quality healthcare services to community citizens.

Coronavirus infections are characterized by a high risk of spreading in medical institutions, and health workers may become infected with coronavirus while performing work duties. This article examines the prevalence of the occurrence and persistence of a number of symptoms in medical workers after acute COVID‑19, and examines the impact of the consequences on the quality of life of those who have been ill. The main, primary complaints during hospitalization were: fever, cough, general weakness, sore throat, headache, shortness of breath, sweating, low saturation, which was confirmed by a positive polymerase chain reaction test for SARS‑CoV‑2. The probability of pneumonia of varying severity in medical workers increased in the presence of a history of chronic diseases of the cardiovascular system (OR=2.13/0.5:12.7), respiratory system (OR=1.34/0.2:17.4) or pyelonephritis (OR=3.18/0.5:38.2). In 19.3±4.2% of the ill medical workers, there was a general decrease in the quality of life after the end of the disease, which was expressed in the persistence of chest and joint pain, shortness of breath, asthenia, weight loss, memory impairment, which are the basis for referral to rehabilitation in specialized clinics for post-COVID.

Abstract The importance of medical diagnostics, and specifically, tests for infectious agents such as viruses, is well recognized, and in fact, are a crucial component of efforts to control pandemics and mitigate their effects on public health. A long-established trend is the development of low-cost, easy-to-use point-of-care (POC) diagnostics to provide pervasive, timely testing independent of laboratories and other medical infrastructure. The technology of POC tests is widely accessible to engineering students, and there are many opportunities and avenues for innovation, including Smartphone-based platforms and integration with the Internet of Medical Things (IoMT). We discuss the design, demonstration, and testing of POC virus testing, including tests applicable to COVID-19, as Senior Design Projects for undergraduate mechanical, electrical, and manufacturing engineering majors.

Natural compounds with indolizine scaffolds have demonstrated numerous biological activities and have found use in medical research laboratories. Thys, the unique indolizine scaffold became an important system for the development of new drug candidates in medicinal chemistry. [1,2] Several indolizines with excellent anticancer activity and tubulin polymerization inhibitory potency have been reported recently, and our group contributed also to the field. [1,3] The goal of this study was the design, synthesis and anticancer evaluation of several new derivatives with symmetrical or unsymmetrical substituted 7,7’-(ethene-1,2-diyl)bisindolizine structure, and several new 6-, 7- or 8-substituted indolizine derivatives. Mono and bisindolizines were synthesized in good yields via [3+2] dipolar cycloaddition of the in situ generated ylides, from corresponding N-pyridinium salts to ethyl propiolate. Part of the new derivatives were tested for their anticancer activity by screening against NCI’s 60 human tumour cell lines panel and the results are presented herein. In vitro experiments regarding tubulin interaction were performed for the active compounds.

The objective of the study is to outline the vision of the Russian-Ukrainian war in Renmin Ribao, the main newspaper of the People’s Republic of China. The source base of the research is the content of the Renmin Ribao website during 2022 in English, Spanish, French, Russian, German, Italian, and Portuguese languages. The material was selected using the keywords «Ukraine», «Russia» (and other derivatives), analyzed using induction, vocabulary analysis, classification analysis, and content analysis. Renmin Ribao rarely uses the term “war” to refer to events in Ukraine, resorting to streamlined formulations such as “situation”, “issue”, “crisis”, “conflict” and even “Russian military operation”. The newspaper sees the United States, not Russia, as responsible for the events in Ukraine. Rather, Moscow is a victim of many years of intrigues on the part of the United States, which manifested itself in efforts to restrain and weaken Russia, in particular with the help of Ukraine. The newspaper often reproduces Russian narratives and Russian fakes, disseminates messages typical of Russian propaganda (for example, about biological laboratories in Ukraine), reports on referendums in the occupied Ukrainian territories from the evidence of the Russian RT television channel, about the annexation of four Ukrainian regions from the testimony of Chairman State Duma Vyacheslav Volodin, about the attack on the Crimean bridge from the evidence of the FSB. Renmin Ribao is inclined to the opinion of the harmfulness of anti-Russian sanctions and the impracticality of supplying weapons to Ukraine, sees a priority way out of the Russian-Ukrainian war in an abstract “peace”, and not the victory of Ukraine. The issue in which Renmin Ribao sharply diverges from the position of official Moscow is the identification of the situation in Ukraine and the situation in Taiwan. Drawing parallels between Taiwan and Ukraine is popular in both the Russian and the Western press. However, when the war began to look less and less victorious for Russia, these parallels became unacceptable to both Renmin Ribao and official Beijing.
