Background: Drug utilization is defined as the marketting, distribution, prescribing and use of drugs in society. Various factors like quality, quantity, and cost of drugs plays an important role in selection of drug therapy. Hence we wanted to observe that drug utilization and prescribing pattern of antidepressants.Methods: A prospective observational study were conducted in a psychiatric department at RMMCH for a period of 6 months from November 2019 to April 2020. Sixty patients were enrolled and analysed to study the pattern and the world health organisation (WHO) prescribing indicators.Results: From 60 prescriptions, the most widely prescribed antidepressant drug escitalopram 21 (42%) as a combination drug citalinforte 30 (50%), linotril 21 (42%) lorazepam 12 (20%), haloperidol 6 (10%) and quitipin 3 (5%). Most commonly, depression was seen in age group between 31-40 years and males were more to depression than females. Most drugs were prescribed under brand names (55%) than generic drugs (45%). Monotherapy was the preferred drug pattern for the mild depression followed by dual therapy and triple therapy for moderate and severe depression respectively. The number of drugs which were prescribed from the WHO and national list of essential medicine (NLEM) was haloperidol (10%) and escitalopram (85%) respectively.Conclusions: Escitalopram was the commonly prescribed drug for depression followed by lorazepam, linotril and haloperidol. Monotherapy was most preferred, because of increased medication adherence, low ADRs, minimum cost and lesser drug interactions compared to other therapies.

Le diabète de type 2 (DT2) est une pathologie caractérisée par une hyperglycémie chronique résultant d'une diminution de la production d'insuline et d'une résistance à cette hormone. La diminution de la production d'insuline est causée par une réduction du nombre de cellules β pancréatiques, qui meurent par apoptose, et par une altération fonctionnelle de leur capacité à produire de l'insuline. Préserver ces cellules et maintenir leur fonction représente donc une stratégie prometteuse pour la gestion du DT2. L'alimentation joue un rôle déterminant dans la gestion du DT2. L'acide alpha-linolénique (ALA), un acide gras polyinsaturé oméga-3 essentiel présent dans certaines plantes et son dérivé, l'acide docosahexaénoïque (DHA), présentent des propriétés anti-inflammatoires et cytoprotectrices. Ils exercent également des effets bénéfiques dans le contexte du DT2 en améliorant la sensibilité à l'insuline et la sécrétion d'insuline. Dernièrement, la Protectine DX (PDX), un dérivé oxygéné du DHA produit par une double oxygénation catalysée par la lipoxygénase 15 (15-LOX), a été mise en lumière pour ses effets anti-inflammatoires, neuroprotecteurs et son rôle dans l'amélioration de la sensibilité à l'insuline. PDX pourrait donc présenter un intérêt thérapeutique pour le DT2. L'ALA peut également être oxygéné par la 15-LOX, ce qui conduit à la formation de linotrines (LiNOs). Bien que leur structure soit proche de celle du PDX, les effets des LiNOs restent encore peu explorés. Des études récentes suggèrent qu'elles possèdent des propriétés anti-inflammatoires. Grâce à une collaboration avec le Dr. Thierry Durand, la première synthèse chimique stable de LiNOs a été réalisée, ouvrant ainsi la voie à l'étude de leurs effets potentiels.Mon projet de thèse vise à étudier, pour la première fois, les effets des LiNOs dans le DT2 in vitro, en les comparant à leur précurseur ALA ainsi qu'à PDX et son précurseur DHA. Ce travail s'articule autour de trois objectifs principaux :1. Évaluer la capacité d'ALA, DHA, PDX et des LiNOs à protéger les cellules β-pancréatiques contre la mort cellulaire.2. Comparer leurs effets sur la production d'insuline en réponse au glucose.3. Tester leur impact sur la sensibilité à l'insuline des cellules musculaires.Ces résultats obtenus in vitro serviront de base pour des études in vivo, ouvrant ainsi la voie à des recherches précliniques sur leur intérêt thérapeutique chez l'humain
Type 2 diabetes (T2D) is a condition characterized by chronic hyperglycemia resulting from a decrease in insulin production and resistance to this hormone. The reduction in insulin production is caused by both a decrease in the number of pancreatic β cells, which undergo apoptosis, and a functional impairment of their ability to produce insulin. Preserving these cells and maintaining their function therefore represents a promising strategy for managing T2D. Diet plays a crucial role in the management of T2D. Alpha-linolenic acid (ALA), an essential omega-3 polyunsaturated fatty acid found in certain plants, and its derivative, docosahexaenoic acid (DHA), exhibit anti-inflammatory and cytoprotective properties. They also provide beneficial effects in the context of T2D by improving insulin sensitivity and secretion. Recently, Protectin DX (PDX), an oxygenated derivative of DHA produced by a double oxygenation catalyzed by lipoxygenase 15 (15-LOX), has garnered attention for its anti-inflammatory and neuroprotective effects and its role in enhancing insulin sensitivity. Therefore, PDX may hold therapeutic potential for T2D. ALA can also be oxygenated by 15-LOX, leading to the formation of linotrins (LiNOs). Although their structure is similar to that of PDX, the effects of LiNOs remain largely unexplored. Recent studies suggest that they possess anti-inflammatory properties. Through a collaboration with Dr. Thierry Durand, the first stable chemical synthesis of LiNOs has been achieved, paving the way for the study of their potential effects.My doctoral project aims to investigate, for the first time, the effects of LiNOs in T2D in vitro, comparing them to their precursor ALA as well as PDX and its precursor DHA. This work revolves around three main objectives:1. To evaluate the ability of ALA, DHA, PDX, and LiNOs to protect pancreatic β cells from cell death.2. To compare their effects on insulin production in response to glucose.3. To assess their impact on insulin sensitivity in muscle cells.The results obtained in vitro will provide a foundation for in vivo studies, thereby opening the door to preclinical research on their therapeutic potential in humans
