La lamina propria des cordes vocales, décrite par Hirano, est l’élément clé des compétences vibratoires des cordes. Cette structure spécifique correspond au plan de glissement de la muqueuse de recouvrement sur le muscle vocal sous-jacent. Les cordes vocales, de part leur fonction de phonation, sont la cible de traumatismes minimes, constants, mais peuvent aussi subir des traumatismes plus importants dont les origines sont multiples. En réponse à ces traumatismes, il existe une réactivité responsable d’une réparation tissulaire. Cette réparation tissulaire peut se faire sur plusieurs modes : soit une réparation ad integrum des tissus avec un retour à l’état initial, soit une réparation aberrante conduisant à la formation de lésions pathologiques ou encore une réparation aboutissant, à la fin des mécanismes mis en jeu, à une cicatrice. Les lésions ou les cicatrices présentes au niveau de la lamina propria sont donc responsables de perturbations vibratoires et ainsi de dysphonies. La première partie de ce travail (Etude I), a permis d’étudier deux lésions bénignes et particulièrement communes des cordes vocales : le polype et l’œdème de Reinke, dont l’apparence macroscopique c’est à dire le phénotype peut varier ou être similaire. Nous avons été capables, grâce à l’étude de l’ADNc par Microarray, d’établir le profil d’expression génétique de ces deux lésions de façon à les différencier, mais aussi à établir les processus de réactivité tissulaire et moléculaire responsable de leur développement. L’œdème de Reinke est caractérisé par les changements relatifs au stress oxydatif avec la présence de mécanismes de défense et de protection cellulaire contre le stress oxydatif, évitant probablement toute évolution vers une transformation maligne. Le polype est le siège de mécanismes de remodelage de la matrice extracellulaire et de réparation tissulaire ; il est apparu comme une lésion inflammatoire avec un nombre élevé de fibroblastes qui assistent le processus de réparation. Il est régulé par un blocage de la progression cellulaire et n’apparaît pas comme une lésion induite par le stress oxydatif. En nous basant sur les résultats de l’étude I, nous avons, dans cette première partie, initié l’étude II, de façon à sélectionner les gènes d’intérêt au sein des lésions malignes impliqués dans les processus de développement et de réactivité tissulaire et moléculaire de ces lésions. Ces gènes jouent un rôle dans la prolifération cellulaire, l’angiogenèse et l’invasion tumorale. L’objectif final de cette étude II, est de mettre en évidence les mécanismes de développement et d’évolution des lésions bénignes et malignes des cordes vocales, en comparant le niveau d’expression des gènes sélectionnés par l’étude I et II. En effet, ces lésions bénignes et malignes, malgré une exposition chronique à des facteurs de risque « identiques » (tabac, stress oxydatif, phono-traumatisme), présentent des processus de réparation différents. Nous avons poursuivi l’étude des phénomènes de réparation tissulaire par l’étude des caractéristiques d’une matrice extracellulaire synthétique. La matrice extracellulaire de la lamina propria possède en effet des caractéristiques spécifiques responsables des caractéristiques vibratoires de l’ensemble de la lamina propria. Lors des phénomènes de réparation notamment, il peut apparaître un tissu cicatriciel très gênant pour la vibration et nous avons étudié à l’aide des procédés de la biologie moléculaire, la réparation tissulaire des cordes vocales sur un modèle animal in vivo. Nous avons ainsi montré l’intérêt de l’adjonction d’une matrice extracellulaire synthétique pour diminuer les risques de cicatrice fibreuse. L’acide hyaluronique qui est un des composants de la matrice extracellulaire est d’un intérêt majeur dans la réparation tissulaire. L’étude III a permis d’évaluer une matrice extracellulaire synthétique (MECs) à base d’acide hyaluronique et de gélatine (Carbylan TM-GSX) dans le but d’optimiser la réparation tissulaire et les propriétés biomécaniques des cordes vocales. L’injection de Carbylan TM-GSX 5% (5% de gélatine et 95% d’acide hyaluronique) a fourni un meilleur environnement moléculaire et a permis d’améliorer la réparation tissulaire et par conséquent les propriétés viscoélastiques des cordes vocales. Par la suite, nous avons été capables dans l’étude IV, d’identifier les mécanismes moléculaires impliqués et responsables de la réparation tissulaire à un stade précoce et de l’amélioration de celle ci. Dans l’étude V, nous avons évalué la réponse inflammatoire à l’injection de Carbylan TM-GSX 5% au niveau de la corde vocale et avons montré qu’elle n’avait aucune conséquence pathologique pour le tissu hôte.

Cancer is a dangerous disease causing the most deaths in the world today and around 550,000 deaths in America per year (American Cancer Society). Larynx cancer data was recorded by Kardaun (1983). The data was collected at a Dutch hospital during 1970 – 1978. Ninety male adults with cancer of larynx were involved into the study. Each patient was divided into one of four groups depending on his or her illness condition. The data also recorded their age, lifetime, and year of entering the research. These are common factors as factors of other cancer data. The purpose of this thesis is to apply proportional hazard regression model, additive hazard regression model, censored quantiles regression model, and censored linear regression model to analyze the above larynx cancer data and find the best regression model of data by using each method. Comparison and suggestion for which method should be used in specific situation are also made. Some related topics are also mentioned so we can have resource for future study. Key words: right censoring, proportional risk model, additive risk model, quantiles regression model, linear regression model.

Producing an accurate model of the human voice has been the goal of researchers for a very long time, but is extremely challenging due to the complexity surrounding the way in which the voice functions. One of the more complicated aspects of modelling the voice is the fluid dynamics of the airflow, by which the process of self-oscillation of the vocal folds is sustained. This airflow also provides the only means by which the ventricular bands (two vocal fold-like structures located a short distance above the vocal folds) are driven into self-oscillation. These have been found to play a significant role in various singing styles and in voice pathologies. This study considers the airflow and flow-structure interaction in an artificial up-scaled model of the human larynx, including self-oscillating vocal folds and fixed ventricular bands. As the majority of any significant fluid-structure interaction takes place between structures found within the larynx, this thesis is limited only to examining this component of the voice organ. Particle Image Velocimetry (PIV) has been used to produce full field measurements of the flow velocity for the jet emerging from the oscillating vocal folds. An important advance in this study is the ability to observe the glottal jet from the point at which it emerges from the vocal folds, thus permitting a more complete view of the overall jet geometry within the laryngeal ventricle than in previous work. Ensemble-averaged PIV results are presented for the experimental model at different phase steps, both with and without ventricular bands, to examine their impact on the dynamics of the human larynx and the glottal jet. Finally, the three-dimensional nature of the glottal jet is considered in order to further understand and test currently held assumptions about this aspect of the jet dynamics. This was achieved by undertaking PIV in a plane perpendicular to that already considered. It is shown that the ventricular bands have an impact on the flow separation point of the glottal jet and on the deflection of the jet centreline. Furthermore, the dynamics of the vocal folds alters when ventricular bands are present, but the glottal jet is found to exhibit similar three-dimensional behaviour whether or not ventricular bands are present.

A study was performed to establish the appearance of normal equine laryngeal cartilages using magnetic resonance imaging. Specimens were acquired from clinically normal horses that were euthanized for reasons other than respiratory disease. Three in situ and 5 ex vivo larynges were imaged using a 0.3 Tesla system. Images were obtained in the transverse plane using T1-weighted 3D spin echo, T2-weighted 3D spin echo, T2-weighted gradient echo, short tau inversion recovery (STIR), and proton density spin echo sequences. Five ex vivo larynges were also imaged in the transverse plane using a 1.5 Tesla system, sequences included T1-weighted 3D spin echo, T2-weighted 3D turbo-spin echo, turbo inversion recovery (TIRM), and proton density spin echo sequences. A frozen gross laryngeal specimen was sliced in 5-mm transverse sections for comparison to the MR images. Excellent correlation was found between MR images and the gross transverse sections. Successful imaging was accomplished using both imaging systems; however, the 1.5 Tesla system yielded superior image resolution. The 0.3 Tesla imaging system would accommodate the intact equine head, which was not possible using the 1.5 Tesla MRI system. The internal morphology of the laryngeal cartilages was clearly identified in all imaging sequences obtained. Cartilages were found to differ in signal intensity based on the tissue composition and imaging sequences performed. MRI was determined to be a useful imaging modality for evaluating the cartilage morphology of the equine larynx. Further investigation is required to document pathologic morphology.<br>Master of Science

Cette thèse porte sur le rôle de l'attention et des indices de groupement perceptif de fréquence fondamentale et de longueur de tractus vocal dans le traitement de messages co-occurents. Nous abordons ce problème à l'aide du paradigme d'amorçage par identité en écoute dichotique , développé par Dupoux et al. (2003) en évitant de dégrader la clarté perceptive du méssage non focalisé. Les participants doivent détecter un mot cible, appartenat à une catégorie cible, dans une liste de mots présentés rapidement à l'oreille focalisée. L'amorce est présentée à l'oreille non focalisée. Elle est insérée dans une série de phrases ineptes et elle est présentée juste avant la cible. Nous montrons un effet d'amorçage significatif moyen de 24 ms lorsque la voix du message non focalisé a une fréquence fondamentale différente (4,2 dT) de celle du message focalisé. Lorsque les messages ont une longueur de tractus vocal différente (15 ou 30 %) l'effet d'amorçage est de 13 ms. Il est de 32 ms lorsque les voix des messages diffèrent sur ces deux dimensions (4,2 dT et 15% de différence de longueur de tractus vocal). Enfin, l'effet d'amorçage disparait lorsque les messages sont prononcés avec la même voix et il est restauré lorsque l'attention est portée sur l'amorce (94 ms), ce qui est cohérent avec les résultats de Dupoux et al. (2003). Cette série d'expériences suggère que un effet d'amorçage par identité peut être produit par de mots insérés dans de la parole continue présentée simultanément à un message focalisé lorsque des indices de groupement, tels que la fréquence fondamentale et la longueur de tractus vocal, sont disponibles pur éviter une dégradation de la clarté perceptive du message non focalisé<br>This thesis investigates the role of attention and grouping cues in the processing of concurrent dichotic messages using a paradigm based on the repetition priming paradigm developed by Dupoux et al. (2003). Thier paradigm is modified in order to avoid deterioration of perceptual clarity of the prime. Participants detected a target word belonging to a specific category presented in a rapid list of words in the attended ear. In the unattended ear, concatenated sentences were presented, some containing a repetition prime presented just before a target word. There was a significant average priming effect of 24ms on detection reaction time when the unattended message voice had a different fundamental frequency (4,2 semitones) from the attended message voice. When the messages had similar fundamental frequencies but different vocal tracts lengths (15 or 30 %) the priming effect was 13 ms. When both fundamental frequency and vocal tract length differed, the priming effect was 32 ms. The priming effect disappeared when the messages had the same vocal tract and fundamental frequency range. However priming was restored (94 ms) when attention was focused on the prime, in line with the results of Dupoux et al. (2003). This set of experiments shows that repetition priming can be produced by words from unattended continuous speech presented simultaneously with an attended message, provided that grouping cues such as fundamental frequency and vocal tract length are present to prevent degradation of the perceptual clarity of the unattended message

Analyses in the past have revealed that flute sound production is a complex procedure involving internal movements of the body. The larynx and pharynx in particular play a crucial role. Some authors, advocate for the abdominal muscles, while others, draw attention to the lips and the oral cavity. However, despite the various studies, the physiology of flute playing is limited by lack of empirically-derived information about what happens when a player carries out musical tasks. The present study is an empirical investigation of the role of the larynx and pharynx in flute playing from the perspective of a flute specialist. Specifically, the study aims to determine vocal fold involvement in vibrato, epiglottis movements, arytenoids and epiglottis involvement in articulation, and glottal aperture in tone and dynamics production (soft to loud, loud to soft). A detailed analysis of the relationship between standard flute techniques and the larynx and pharynx mechanisms is presented, making a significant contribution to the flute pedagogical literature. Two male and three female experienced players (referred to in the study as “the participants”) participated in a video-nasendoscopy procedure. The behaviour of the participants’ larynxes and pharynxes while playing a performance protocol specifically designed for this research was observed by a qualified speech pathologist. Specifically, the observer analysed true vocal fold adduction, false vocal fold adduction, laryngeal height, pharyngeal space, epiglottis movement, and arytenoid adduction. This study reveals that laryngeal/pharyngeal participation in flute playing is not limited to vibrato production or specific techniques, such as singing and playing or flutter tongue (tongue rolling effect while playing). The larynx plays a major role in producing flute tone and dynamics and should be considered by pedagogues, performers and health specialists. These findings can assist flute professionals in their daily practice, performances and teaching. Knowing the role of the larynx in vibrato, articulation, pitch control, and dynamics control, reveals a new tool for flute players to use with assurance when preparing any piece of the standard repertoire, or teaching a student.

This thesis presents a method for fluid-structure interaction in a simplified 2D model of the human larynx using the arbitrary Lagrangian–Eulerian (ALE) approach and a strictly stable high order finite difference method. The ALE method is first tested for the fluid solver with a prescribed boundary movement, and then the method is extended to a two-way coupled explicit fluid-structure interaction where the vocal folds interact with the airflow in the larynx. In each case, the fluid is treated as a Newtonian fluid obeying the perfect gas law and laminar flow is always assumed. Since the interest is ultimately phonation, the compressible Navier–Stokes equations are solved in order to resolve both the flow field and the acoustic waves. Characteristic-based non-reflecting boundary conditions are used so that no unphysical reflections occur at the outflow boundary of the limited computational domain. The finite difference method relies on the summation by parts (SBP) technique which allows energy estimates to be made for the discretized equations in an analogous way as for the continuous problem. In the interior, the difference operator corresponds to the standard sixth order explicit difference method and is third order accurate near the boundaries. The classical explicit fourth order Runge–Kutta method is used for time integration. For the structure field, the linear elastic wave equation is formulated as a first order system. The spatial derivatives are discretized by the same high order difference operator as employed for the flow equations. To implement boundary conditions for displacement or traction, a simultaneous approximation term (SAT) method is derived. Verification proves that the method is nearly fourth order accurate. The linear model is then extended to a nonlinear hyperelastic model based on a neo-Hookean constitutive relation. The strict energy estimate is only valid for the linear equation, but the SAT approach provides a consistent way to implement the traction boundary condition also for the nonlinear equations. Fluid-structure interaction simulations are performed with model parameters corresponding to the real geometry of the human larynx and physical properties of the human vocal folds. Results for the vortex dynamics are investigated and preliminary acoustic results are obtained.

Regionäre Metastasten verschlechtern die Prognose von Patienten mit einem Kopf-Hals-Karzinom deutlich. So verringert sich die 5-Jahresüberlebensrate bei einer Metastasierung in die regionären Lymphknoten um etwa 50%. Die Daten von 672 Patienten mit einem Plattenepithelkarzinom des Oropharynx, Hypopharynx oder Larynx der Jahre 2005 bis 2009 wurden retrospektiv ausgewertet. Bei 270 Patienten mit einem Primärtumor der Tonsille (n=82), des Oropharynx (excl. Tonsille) (n=70), des Hypopharynx (n=33), der Supraglottis (n=42) oder der Glottis (n=43) wurde eine ipsilaterale (n=47) oder bilaterale (n=223) Neck dissection im Rahmen der Primärtherapie durchgeführt. Dabei wurden bei 61% der ipsilateralen und 49% der kontralateralen Neck dissections die Level en bloc reseziert und konnten im Falle einer Metastasierung nicht in die levelbezogene Auswertung eingeschlossen werden. Die Level, in welche die Tumoren bevorzugt metastasieren, wurden evaluiert und tumorspezifische Parameter (Tumorlokalisation, pT-Kategorie) auf ihren möglichen Einfluss auf die Parameter der Metastasierung (pN-Kategorie, Seitenlokalisation, Anzahl, Größe und Level der Lymphknotenmetastasen) getestet. In allen untersuchten Tumorlokalisationen lagen die meisten Lymphknotenmetastasen im ipsi- und kontralateralen Level II. Bei Angabe der Sublevel lagen die Metastasen vor allem in Level IIA. Ipsilateral konnten Lymphknotenmetastasen in allen fünf Leveln und kontralateral in den Leveln II, III und V nachgewiesen werden. Verschiedene Metastasierungswege und Levelkombinationen der Lymphknotenmetastasen zeigen, dass die regionäre Metastasierung keinen starren Gesetzmäßigkeiten folgt. Die Metastasierungsrate und die Seitenlokalisation der Lymphknotenmetastasen unterschieden sich signifikant je nach Lokalisation des Primärtumors. Die Anzahl der ipsilateralen histopathologisch nachgewiesenen Lymphknoten zeigte eine statistisch signifikante Korrelation zum Nachweis von Lymphknotenmetastasen. Eine zunehmende Metastasierung mit steigender pT Kategorie wurde bei den Glottistumoren nachgewiesen, nicht aber bei den Tumoren der anderen Lokalisationen. Die Ausdehnung des Primärtumors (pT-Kategorie) lässt keine Rückschlüsse auf die Parameter regionärer Metastasen (Seitenlokalisation, Anzahl, Größe und Level der Lymphknotenmetastasen) zu.

<p> Every year, many thousands of people worldwide lose the ability to speak due to receiving a laryngectomy, typically for treatment of cancer. At some point in their recovery, most will use an electrolarynx to recover their ability to speak. Typical electrolarynxes utilize a piston to strike a disc pressed to the patient's neck which delivers a pressure wave into the soft tissue. This pressure wave mechanically couples with the vocal tract and generates the fundamental frequency necessary for creating vowels without which speech is not possible. </p><p> Commonly available electrolarynxes suffer from poor frequency control due to the nonlinear character of their impulse driver. They also create a great deal of "self-noise" which is distracting to listeners and makes using voice communication systems difficult. </p><p> We propose a novel electrolarynx implementation which utilizes two interfering ultrasonic waves to generate a fundamental frequency in the vocal tract required for speech restoration. The device is light weight, compact, inexpensive, and offers excellent control of all aspects of the output waveform. In addition, as the primary waveforms are above human hearing, there is little "self-noise" that can be heard by listeners and most communications devices filter such noise as part of their standard digitization process. </p><p> This device offers the potential to greatly improve the lives of those who have lost their voices and must rely on technology to allow them to communicate in the most efficient manner.</p>

A dynamic mechanical model of the human larynx and vocal tract has been developed to investigate its acoustic and fluid dynamic behaviour during sustained vowel production. The model comprises a cylindrical duct, open at one end, with a controlled air flow introduced at the other. The flow entering the duct is modulated by the periodic opening and closing of pair of electro-mechanically driven shutters. Far field measurements of the radiated pressure have shown that the model generates sound which has a spectral distribution that corresponds, at low frequencies, to that of an open vowel. However the spectral amplitudes were somewhat lower than voiced speech sounds normally generated at the same low rate. The addition of an orifice plate to reduce the duct exit area was found to increase the level of the radiated sound and to modify the spectral distribution somewhat. The flow distribution throughout the model duct has been measured using hot wire anemometry. The velocity distribution measured in the model was found to correspond to that measured in the oral cavity of four live subjects. Calibrate pressure measurements at the duct wall have been used define the associated pressure field within the duct. The pressure distribution found within the model corresponded to that measured in vivo by other researchers. The velocity within the duct was shown to be associated with contributions from three separate velocity fields, the rotational acoustic particle velocity and a rotational velocity field due to vortex development at the exit to the shutters. It was shown that the rotational velocity disturbance convected along the duct at approximately the local mean flow velocity. Comparison of prediction with measurement of the radiated sound fields showed that the presence of a rotational velocity field at the duct exit made a significant contribution to the radiated sound pressure level. A discussion is included as to whether acoustic sources, associated with the rotational flow, exists at area discontinuities in the vocal tract in addition to the generally accepted acoustic source due to fluctuating mass flow at the glottal exit. The influence of the Rotheberg Mask on the flow and acoustic behaviour of the model was investigated in some detail. Measurements show that the velocity field was almost unaltered, but the fluctuating pressure amplitudes were greatly reduced. Corresponding reductions were found in the radiated acoustic power with further reductions apparently due to the suppression of the rotational flow sources by the mask.

A partir de larynx humains autopsiques de foetus à terme non malformés, un modèle numérique a été élaboré. Avec ces mêmes organes placés sur un banc expérimental et placés en condition de débit physiologique, un son a été produit qui a été comparé par une analyse acoustique à des premiers cris recueillis en salle de naissance. Une étude en caméra rapide a également été menée sur ces organes dans des conditions identiques. L'ensemble des données expérimentales, et tout particulièrement celles issues de l'utilisation de la caméra rapide, ainsi que toutes les données numériques tendent à démontrer que le rôle des cordes vocales n'est pas de générer le son par elles-mêmes. Voice production is the result of the fluid-structure-interaction (FSI) of airflow through glottis and the vocal folds. The physics of the voice production consists of the geometry of the vocal folds and larynx, the aerodynamics of the airflow within larynx, and mechanical properties of the vocal folds. Through these physical aspects of voice production, engineering analyses can contribute to voice research. Creation of patient-specific computer models of the vocal folds and larynx may be considered as one of the main contributions of engineering studies in voice research. Person-to-person differences in voice parameters are considered in patient-specific models and as a result, they have high potential for clinical applications. In this dissertation, the aforementioned physical aspects of voice production were studied through the creation of patient-specific geometries, numerical analyses on glottal airflow, and investigation of the vocal folds' strain-stress field during oscillation. To address the geometry parameters, the patient-specific three-dimensional (3D) geometries of the vocal folds, larynx, and laryngeal cartilages (i.e., thyroid, cricoid and arytenoid) were built. Morphometric measurements were performed and the obtained results were compared to the existing cadaver data. Low discrepancies between these measurements indicate the feasibility of using Computed Tomography (CT) scan data for creation of 3D models. General laryngeal cartilage models were created while preserving all the biomechanically important morphometric features. A methodology to create accurate geometries of the vocal folds with desired glottal angles was introduced based on mapping high-speed digital imaging (HSDI) data with that from Magnetic Resonance Imaging (MRI). To address the fluid dynamics of voice production, the quasi-steady approximation in glottal airflow simulations was investigated in a two dimensional (2D) driven model. The surface area of the model was kept constant to avoid volume variations. Two-dimensional dynamic simulations of airflow through an idealized glottal orifice with moving walls were performed. A series of steady flow simulations was then performed using vocal fold configurations and boundary conditions that instantaneously coincide with data from the dynamic simulations. Aerodynamic parameters such as fluid flow rate, normal pressure on the vocal folds, shear stress on the glottal walls, and orifice discharge coefficients were compared between the dynamic and static simulations and similarities and differences were discussed. Three-dimensional simulations of the glottal airflow within patient-specific geometries were performed. One of the models was for a healthy subject and the second model was a postsurgical geometry from a patient with cancer. The effects of the geometry differences on the aerodynamic parameters such as orifice discharge coefficient and flow resistance were shown and discussed. To investigate the 3D nature of the glottal airflow, a 2D simulation was conducted in a computational domain made from a frontal CT scan image. The 2D results were compared to the corresponding slice in the 3D simulation. Significant differences between 2D and 3D simulations were observed in orifice discharge coefficients, jet flow deflection, and secondary flow, highlighting the importance of using realistic 3D models for glottal airflow simulations.To address the mechanical properties of the vocal folds, the deformation field on the superior visible surface of the vocal folds was captured in the excised larynx experiments using Digital Image Correlation (DIC). The initial pre-phonatory strain was measured using a dissection protocol and added to the relative strain obtained during the vocal folds' vibration to calculate the net strain. Kinematics of the vocal folds' oscillation was compared to that of synthetic models and similarities and differences were discussed.<br>La production de la voix résulte de l'interaction fluide-structure entre l'écoulement d'air dans la glotte et les cordes vocales. La physique de la production de la voix comporte trois aspects principaux que sont respectivement la géométrie des cordes vocales et du larynx, l'aérodynamique de l'écoulement d'air et les propriétés mécaniques des cordes vocales. Ces trois aspects de la production vocale se situent dans le cadre de l'ingénierie. La création de modèles numériques de cordes vocales et de larynx spécifiques à l'individu peut être vue comme une contribution majeure pour l'analyse technique dans la recherche sur la voix. Ces modèles ont beaucoup d'intérêt pour les applications médicales et thérapeutiques parce qu'ils prennent en compte la variation des propriétés des tissus et de la géométrie entre chaque individu. Dans cette thèse, trois aspects de la production vocale ont été pris en compte : la création de modèles géométriques propres à chaque individu, l'analyse numérique de l'écoulement glottique et la recherche des relations entre les déformations et les contraintes au sein des cordes vocales au cours de la vibration.Afin d'obtenir des paramètres géométriques, des modèles numériques de cordes vocales, du larynx et des cartilages laryngés ont été conçus en trois dimensions et pour chaque patient. Des mesures morphométriques ont été réalisées et comparées à des données ex-vivo. La faible divergence entre ces mesures a permis de valider l'utilisation de la tomographie par rayons X pour la création de modèles 3D. Des modèles standards de cartilages laryngés furent créés tout en conservant les caractéristiques biomécaniques importantes. Une méthode basée sur la mise en correspondance de données d'imageries haute vitesse (HSDI) et d'Imageries par Résonnance Magnétique (IRM), a été introduite pour créer des géométries précises des cordes vocales avec le contour glottique souhaité. La dynamique des fluides lors de la production vocale a été étudiée en utilisant une approximation quasi-statique de l'écoulement glottique dans un modèle 2D. La surface du modèle était fixée constante afin d'éviter des variations de volume et éliminer ainsi la création de sources monopôles parasites. Les simulations dynamiques à deux dimensions de l'écoulement de l'air ont alors été réalisées à travers une glotte idéale aux parois mobiles. Une série de simulations d'écoulement statique étaient alors réalisées en utilisant les configurations de cordes vocales ainsi que les conditions aux limites qui coïncidaient avec les données provenant des simulations dynamiques au même instant. Les paramètres aérodynamiques tels que le débit du fluide, la pression normale aux cordes vocales, la contrainte de cisaillement sur les parois glottiques, le coefficient de perte de charge ont été comparés et discutés pour les simulations dynamiques et statiques.Des simulations 3D de l'écoulement glottique ont été réalisées pour chaque modèle individuel. Une des géométries étaient considérée comme saine et la seconde correspondait à la géométrie post-chirurgicale pour un patient atteint d'un cancer. Les effets des différences géométriques sur les paramètres aérodynamiques tels que le coefficient de perte de charge dû à un changement de section et la résistance à l'écoulement ont été mis en valeur et discutés.Afin de caractériser les propriétés mécaniques des cordes vocales, le champ de déformation de la surface supérieure et visible des cordes vocales d'un larynx excisé a été mesuré à l'aide d'une technique de corrélation d'image numérique (DIC). La déformation initiale de pré-phonation a été mesurée à l'aide d'un récent protocole de dissection et ajoutée à la mesure de déformation relative obtenue pendant la vibration des cordes vocales afin de calculer la contrainte totale. La cinématique de l'oscillation de ces cordes vocales a alors été comparée à celle de modèles synthétiques afin d'observer les similitudes et les différences.

An excised animal larynx model has been used in many studies to better understand the physiological and anatomical properties of the human larynx. One difference between an ex vivo model and an in vivo model is that ion loss occurs postmortem. To compensate for this in the excised model, researchers most commonly use a preservation method that includes completely submerging the specimen in isotonic saline (0.9% Na+Cl-) and then flash freezing it in liquid nitrogen. The flash freezing method allows researchers to maintain the integrity of the structures while also being able to gather specimens as they become available. Not enough research has been done to understand the effects of a preservation method on the outcomes of the study. Additionally, no common method has been established for preservation across studies to ensure that results are not being influenced by this variable. This prospective, mixed experimental design study includes three groups, a control group and two experimental groups. The control group consisted of 10 bench-mounted porcine larynges that were soaked in isotonic saline and flash frozen with liquid nitrogen. Prior to the experiment, the frozen larynges were thawed overnight before trials. The other two groups consisted of 10 bench-mounted porcine larynges each; these larynges were soaked in either isotonic saline or Ringer<'>s solution, a balanced fluid used in vivo to counteract dehydration. Larynges from these two groups were kept fresh and stored in a refrigerator overnight before trials. On the day of experimentation, each larynx was mounted on a bench top setup including three micropositioners to stabilize, adduct, and elongate the vocal folds. All the larynges were connected to a pseudolung via the trachea and humidified air was passed through to the vocal folds until phonation was achieved. Phonatory trials consisted of brief phonation followed by 5-minute desiccation intervals until phonation was no longer achieved. Phonation threshold flow (PTF), defined as the flow observed at the onset of phonation, was observed during each phonation trial; and flow values were compared within and between groups. Statistically significant differences were found between the Ringer<'>s group and the fresh saline group as well as between the Ringer<'>s group and the frozen saline group, indicating that PTF is influenced by the larynx preservation method.

Il s’agit de la première étude qui compare la voix de deux sujets atteints de paralysie récurrentielle (PR) avec leur propre voix avant la survenue de la paralysie. Ce travail montre que les mesures basées sur le spectre sont adaptées pour caractériser la voix avant et après deux types de traitement chirurgical. Une augmentation du débit aérien phonatoire caractérise la PR. Ce débit est corrélé à l’énergie dans les fréquences élevées et à l’amplitude du pic cepstral. Le débit aérien phonatoire et la pression intra-orale sont corrélés à la perception de voix soufflée. Un modèle mathématique du conduit vocal permet d’expliquer les modifications des fréquences des formants par l’effet des résonances trachéales et d’un comportement supraglottique adducteur. Les anomalies du voisement conduisent à une confusion des consonnes occlusives voisées en faveur de leurs homologues non-voisées. La PR conduit à des modifications des pauses dans la parole avec des stratégies de compensation<br>This is the first study comparing the voice of two patients after paralysis with their own voices before the paralysis. Jitter and shimmer are not adapted for this type of dysphonia. Measurements based on the spectrum—high- and mid-frequency energy and cepstral peak prominence—are adapted to characterizing these voices and to documenting improvements after surgery. A high phonatory airflow rate is characteristic. Airflow is correlated with high-frequency spectral energy and with cepstral peak amplitude. Intra-oral pressure is increased. Both parameters are correlated with the perception of breathy voice. Modifications of vowel formant frequencies can be explained using a mathematical model of the vocal tract showing the acoustic effects of tracheal coupling and of supraglottic adduction. Voicing abnormalities lead to perceptual confusions of voiced stops mistaken for their unvoiced homologues. Temporal modifications of a read text are accompanied by compensatory mechanisms

провідні позиції онкологічних захворювань верхніх дихальних шляхів, за даними Національного канцер-реєстру у 2015 році було зареєстровано 1967 нових випадків раку гортані на Україні. Серед основних етіологічних чинників: паління, шкідливі чинники навколишнього середовища, інфікування папілома-вірусом. Спостерігається прогресивна тенденція до зростання захворюваності на рак гортані, що спонукає до пошуку нових методик лікування та післяопераційної реабілітації.

La simple suture d'un nerf laryngé ne permet pas la réhabilitation fonctionnelle d'un hémilarynx paralysé. C'est la neurotisation séparée des muscles laryngés qui permet expérimentalement de parvenir à cet objectif. Le transplant nerveux phrénique est adapté à la réinnervation du muscle abducteur de la corde vocale (CAP). Dans ce travail nous avons étudié ses possibilités d'utilisation, tout en préservant la fonction diaphragmatique. Après avoir fait le point de la littérature sur la réhabilitation des fonctions laryngées (réinnervation et stimulation électrique) ainsi que sur la transplantation de cet organe , nous avons détaillé le rôle des différentes portions du diaphragme et les conséquences de sa dénervation unilatérale. Puis nous avons présenté nos travaux sur ce sujet. Après avoir réalisé chez le chien la réinervation expérimentale d'un hémilarynx pas e nerf phréniqe et une branche du nerf grand hypoglosse (XII) , nous avons étudié chez le lapin queles étaient les conséquences fonctionnelles de la dénervation diaphragmatique partielle entrainée par la résection d'une ou plusieurs des racines cervicales du phrénique. Pour ce faire, nous nous sommes d'abord intéressé à l'anatomie cervicales du nerf phrénique au cou, et plus particulièrement à celle de ses racines pouvant servir de transplant nerveux. Puis nous avons validé la méthode de stimulation du nerf phrénique, utilisée dans les travaux ultérieurs pour déterminer la force résiduelle du diaphragme. Nous avons alors étudié les conséquences de la résection d'une ou plusieurs racines du nerf phrénique : cartographie de l'innervation résiduelle puis conséuences fonctionnelles de ces résections. Nous avons ensuite appliqué ce transplant à la réinnervation des muscles sous-hyoidiens chez le lapin, en le comparant à la branche du XII. Puis nous avons utilisé chez le chien la racine supérieure d'un ou des deux nerfs phréniques pour réinnerver les deux CAP, en réalisant simultanément la réinervation bilatérale des adducteurs par la branche du XII destinée au thyrohyoidien. Les effets diaphragmatiques ont été parallèlement analysés. Nous avons ainsi montré que la racine supéieure d'un phrénique permettait, dans plus d'un cas sur deux, la réinervatio fonctionnelle d'un des deux CAP sans altérer notablement la fonction diaphragmatique. En conclusion, nous proposons l'application clinique de ce transplant pour la réinervation d'un larynx paralysé ou transplanté.
