The Kouros image in Archaic Greek art has never been perceived as expressing emotions; and nor have his eyes been a focus of research. Rather, the gaze of most of the Kouroi has been perceived as reflecting a sort of denial or cancellation of expression and emotion. However, the opposite of emotion is in itself an emotion and, indeed, once a human figure is portrayed its expression always conveys some sort of emotional message, no matter how indifferent it may seem. Moreover, emotions in Ancient Greece were perceived differently from their conception today. The present study focuses on the meanings expressed by the gaze of the Kouros type, and examines the essence of this expression as it was understood by the beholder in Antiquity. Two kinds of gaze reflected from the eyes of Kouroi can be discerned: a seemingly hollow and emotionless gaze; and a glowing and radiant gaze. The argument presented here is that both these kinds of gaze are manifestations of eudaemonia - happiness as a reflection of social customs and religious practices.

The hymn to the Greatest Kouros from Palaikastro, Crete, has occasioned considerable debate among students of choral lyric and ancient Greek religion since its discovery in 1904. The god invoked as the greatest kouros has been identified with Zeus Diktaios in whose sanctuary at Palaikastro the hymn was discovered. The hymn as we have it is a second or third century AD copy of a late fourth or third century BC composition. As is so often the case in Cretan studies, Minoan antecedents for the cult of Zeus Diktaios at Palaikastro and for the hymn have been suggested and explored.

Abstract This article examines and discusses a new Carian inscription found on the left leg of a kouros statue. The statue was found in the Temple of Zeus Lepsynos in the ancient Carian city of Euromos during excavations carried out in 2021. The four-line inscription is very difficult to interpret, but nonetheless constitutes a highly relevant contribution to our knowledge of the evolution of the Carian alphabet.

L’exemple de l’atelier samien du kouros d’Ischès permet de mettre en évidence diverses caractéristiques de l’organisation de la production statuaire, ainsi que certaines habitudes dans le comportement des commanditaires.

In recent years several inscriptions, including a deme decree (IGii21181) thought to have been found at Sounion, have been reassigned to Rhamnous. The community of Sounieis dedicated a kouros to Zeus even before it had been incorporated in the Athenian state as a deme. The sole remaining deme decree (IGii21180) suggests that the deme centre must have been about 4 km N of the cape, though there was another centre of population at the cape in classical times. Over time the deme sought to protect its resources, possibly against state mining activities but more certainly against inroads by shepherds or goatherds from other demes. Indeed, this was a surprisingly widespread activity, with some eight series of rupestral boundary inscriptions now known. In some cases, as at Sounion, there seems to have been cooperation between demes or reciprocal hostile responses to incursions by grazing animals. The attribution of this burst of protective activity to the 4th century BC is strengthened by the thorough survey of the region in which one deme (Atene) was situated; this shows that the area was uninhabited between the end of that century and the 4th century AD.

Abstract Depression among older adults is a public health issue, and a large literature highlights the importance of close relationships as both a risk and protective factor for depression. Research in U.S. samples suggests that one spouse’s depressive symptoms can increase their partner’s depressive symptoms, especially for women (Kouros & Cummings, 2010; Tower & Kasl, 1996). Little is known about interpersonal associations in depression, mitigating factors, and the role of gender among older couples in Mexico. This study examined (1) the effects of an individual’s depressive symptoms on their spouse’s symptoms and 2) whether living close to family buffered depression associations using data from the Mexican Health and Aging Study (n=4,071 dyads, age 50+ at initial interview). Depressive symptoms were measured in 2001, 2003, 2012, 2015, and 2018 using a modified 8-item version of the Center for Epidemiologic Studies-Depression Scale. Multilevel modeling was used to fit a dual-intercept growth model (centered at 2012) of husbands’ and wives’ depressive symptoms over time, controlling for age and education. Results showed a partner effect for husbands and wives, such that having a spouse with greater depressive symptoms in 2001 was associated with greater subsequent depressive symptoms, but not with rate of change in symptoms, in 2012. There was also a moderation effect such that the deleterious effect of husbands’ depressive symptoms on wives’ symptoms, as well as rate of increase in symptoms, was higher when family lived nearby, suggesting family may potentially exacerbate depression associations among spouses rather than a buffering them as hypothesized.

<p><strong>Abstract.</strong> The renewed interest, in the context of international museological studies, for historical installations such as museographic devices (able to give shape to a space duly dialoguing with the user), is due today to their communicative immediacy. In recent years the availability of libraries and tools for the use of augmented reality (AR) content has undergone an important increase. AR innovation represents a new method for enhancing the presence visitors into the museum industry despite its use leads to an increase in costs and instrumental investments. In this regard, in a wider work of valorisation and dissemination of archaeological heritage, we are working on the development of an app for tourism purposes. The aim of this paper is the application of some techniques (3D modelling, spectroscopy, virtual reality) to study, record and make available information about archaeological artefacts preserved in the Regional Museum of Lipari (clay masks) and in the National Archaeological Museum of Reggio Calabria (the statue Kouros and the Amphoras). The three-dimensional models produced are well suited to applications of augmented reality for the promotion and dissemination of information on the archaeological /architectural/cultural heritage.</p><p>Moreover, the integration of the information about characterization of materials are essential in order to identify both geographic area of provenience and manufacturing historic period. Some pigments or medium, and in general all materials, can be indeed connected to a determinate historic time or to a specific artist; to this particular purpose spectroscopic methods are valid approach for their non-destructive nature.</p>

The water quality status of the Kouris river in Cyprus was examined in order to fulfil the requirements for ecological quality as defined by the Water Framework Directive-2000/60/EC. Nitrate concentration (mean value) was increased in the Limnatis (2.8 mg L−1) tributary in comparison with the Kryos (2.1 mg L−1) and Kouris (1.0 mg L−1) tributaries depicting the influence of anthropogenic activities. The total maximum daily nutrients loads (TMDLs) based on the flow duration curves approach, showed that nutrients loads exceeded threshold values (33.3–75.6% in all hydrologic condition classes in the Kouris tributary, and 65–78% in the Limnatis tributary) especially under low flow conditions. The TMDL graph is intended to guide the temporal schedule for chemical sampling in all hydrologic classes. Kouris reservoir is an oligotrophic system, strongly influenced by the river’s flash-flood character but also by the implemented management practices. Kouris river outflow, which was reduced to one-tenth in the post dam period altered the wetland hydrologic network and contributed to the decrease of aquifer thickness. Continuous evaluation and update of the River Basin Management Plans will be the basis for the sustainable development of the Kouris basin.

This paper was based on some of the key provisions of Directive 2000/60, which are intended to establish a framework for water protection. Senior environmental objective of Directive 2000/60 is to achieve good status for all waters by 2015. To achieve this goal, some steps followed as required by the Directive. This work was originally to determine the watershed of the river Kouris, which is the study area, located west of Limassol city (Cyprus), in an area calculated about 357km2.The main features of the river basin are indicated, such as the climatic data, geology, agriculture and livestock and also the human interventions relating to the construction of the Kouris dam and river diversion tunnel Diarizos. Additionally, there is a classification of land cover, according to the Corine land cover 2006. A description is made too, for the protected areas, covered in the study area. An application of the typology, is made, to identify the types and the water bodies as defined by the Directive 2000/60. Of the two systems which it lays down (A and B), the system B is selected because is more flexible than the system A and allows the use of additional descriptors. Due to the construction of the Kouris dam, of the impoundment of water upstream and of the absence of feeder in the downstream region, the river basin of Kouris is divided finally into two independent river basins. So it seems appropriate to apply a typology for the watershed of Kouris River in its natural state and one for the two independent river basins, which are resulting from the construction of the dam. Also two typologies are applied, for the descriptors. In one case as descriptors are selected, the altitude, the size of the river basin, the geology, and as an optional descriptor, the gradients. In the second case the selected descriptors, as applied in Cyprus, are the flow regime (continuous or non-continuous) and the annual volume of rainfall resulting from the rainfall and the size of the river basin. The results obtained for the Kouris watershed in its natural state , are 8 types and 62 water bodies, in the case of four descriptors and 3 types and 53 water bodies, in the case of the descriptors like applied in Cyprus. According to the analysis, it is obvious that System B with the use of the four descriptors defines with more accuracy the water bodies than the one applied in Cyprus.

BackgroundAMG 160 is an HLE BiTE® (half-life extended bispecific T-cell engager) that binds PSMA on prostate cancer cells and CD3 on T-cells and induces redirected T-cell lysis of PSMA-expressing cells. This mechanism may allow the BiTE molecule to be active in settings where other targeted or immune therapies have failed. Here, we evaluated the activity of AMG 160 in mouse models of advanced prostate cancer that are resistant to 177Lu-PSMA-617, a PSMA-targeted radioligand therapy that has emerged as a promising treatment modality for metastatic castration-resistant prostate cancer (mCRPC).MethodsTwo prostate cancer models were tested in 6–8-week-old male NCG mice: one cohort had established subcutaneous C4-2 TP53-/- tumors (C4-2 cells with TP53 knockout), and the other cohort had established systemic C4-2 TP53wt/wttumors that mimic metastatic lesions (intracardiac injection). PSMA levels in both models (~255,000 PSMA/cell) are sufficient for tumor growth inhibition with 177Lu-PSMA-617. Mice were administered a single intravenous (IV) infusion of human T-cells. Three days later, mice were treated with 1 cycle of 177Lu-PSMA-617 (30 MBq, IV), or 3 weekly doses of AMG 160 (1 mg/kg, IV) or of a control HLE BiTE molecule (1 mg/kg, IV; target not expressed on C4-2 cells). Therapeutic efficacy was assessed by tumor burden measurements, time to progression (TTP), and survival.ResultsIn both prostate cancer models, AMG 160 treatment significantly improved disease control (figure 1). Median TTP was not reached in the AMG 160 group (p<0.0001), whereas it was 31d (177Lu-PSMA-617) and 23.5d (control) in the subcutaneous model, and 68d (177Lu-PSMA-617) and 50.5d (control) in the systemic model. Median survival was not reached in the AMG 160 group (p<0.0001); it was 39d (177Lu-PSMA-617) and 26.5d (control) in the subcutaneous model, and 77d (177Lu-PSMA-617) and 61d (control) in the systemic model. Following treatment with AMG 160, 2/10 mice with subcutaneous and 7/9 mice with systemic tumors had not progressed at the end of the observation period (>100 days). In contrast, all mice in the 177Lu-PSMA-617 and control groups succumbed to progressive disease.Abstract 717 Figure 1AMG 160 treatment extended survival in mouse models of advanced prostate cancerConclusionsOur study demonstrates potent antitumor activity of AMG 160 monotherapy in models of metastatic CRPC that are resistant to PSMA-targeted radioligand therapy with 177Lu-PSMA-617. Abstract The number of new cases and deaths from prostate cancer (PCa) is highest for Black men compared with other racial and ethnic groups, and Black PCa patients have a shorter average survival as well as a greater risk of tumor recurrence than men of other racial and ethnic groups. However, recent studies have shown that Black PCa patients have a better response to certain therapeutic regimens than White PCa patients. This study focuses on addressing the critical need to determine novel relationships between ancestry-related genetic variation and PCa aggressiveness and response to secondary hormonal therapy in metastatic castration-resistant PCa (mCRPC). We conducted correlative science in a DoD Prostate Cancer Clinical Trials Consortium (PCCTC) prospective multicenter study of secondary hormonal therapy in mCRPC stratified by race, Abi Race. This study enrolled 50 self-reported Black and 50 self-reported White mCRPC patients, and such patients received abiraterone and prednisone until disease progression or adverse event. We performed metabolic profiling using serum samples from fasting patients at baseline and after treatment. In addition, we performed genome-wide genotyping using genomic DNA from whole blood specimens from patients at baseline. We then identified race- and ancestry-related metabolite and SNP variations that associated with outcome using a penalized Cox model approach. In addition, we used Ingenuity Pathway Analysis (IPA) and Lasso Analysis to further study race-related metabolites and SNPs. From these analyses, we identified sphingolipids such as ceramide as race-related metabolites associated with outcome as well as SNPs in Sphingosine Kinase Type 1-Interacting Protein (SKIP) associated with outcome. In addition, our analyses suggest that sphingolipids such as ceramides and SKIP may regulate cancer-related biofunctions differently in Black and White mCRPC patients undergoing abiraterone treatment. Both sphingolipids and SKIP are components of the Sphingosine Rheostat, the regulatory component of sphingolipid cellular metabolism often exploited by various cancers, in which ceramides displays a pro-apoptotic role whereas sphingosine-1-phosphate (S1P) is associated with an anti-apoptotic role and is indirectly regulated by SKIP via regulating the activity of Sphingosine Kinase (SPK1). Evaluation of the function of these sphingolipids and SKIP in PCa cell drug response and aggressiveness are currently underway. Kounis syndrome is a condition where inflammatory cells (mostly mast cells with the contribution of macrophages and T-lymphocytes) cause an acute coronary syndrome. Kounis syndrome comes in four variants: type I in patients with normal coronary arteries; type II in patients with inactive pre-existing atheromatous disease; type III in patients with pre-existing coronary artery stenting; type IV in patients with a pre-existing coronary artery bypass. Recently, we came across a case of recurrent type I Kounis syndrome in our clinical practice. The purpose of the paper is to present our case and conduct a review using the Pubmed scientific database about the most relevant cases of recurrent Kounis syndrome. This review shows that recurrent Kounis syndrome is a rare condition and is mostly associated with Kounis syndrome type III. Recurrent Kounis syndrome may be also triggered by vaccination and it could be associated with chronic spontaneous urticaria. In the last condition, therapy is represented by second-generation anti-histamines and corticosteroids, but also by an anti-IgE monoclonal antibody (omalizumab) in the recalcitrant cases.

Kounis syndrome is an acute coronary syndrome occurring in a context of anaphylaxis, it still an underdiagnosed syndrome. The prognosis of Kounis syndrome remains better than a conventional acute coronary syndrome, we describe 3types of Kounis syndrome with different kind of management, however further management guidelines are still needed to avoid misdiagnose or make the prognosis worse. We report a case of 24 years old Kounis syndrome after a sting of bee, its management in our ED.

Kounis syndrome is defined as the coincidental occurrence of an acute coronary syndrome with hypersensitivity reactions following an allergic event. The three reported variants of Kounis syndrome are vasospastic allergic angina, allergic myocardial infarction and stent thrombosis with occluding thrombus. The syndrome is caused by various inflammatory mediators. The pathophysiological characteristics of Kounis syndrome involve coronary artery spasm and/or atheromatous plaque erosion or rupture during an allergic reaction. Several causes have been described to induce Kounis syndrome, and their number is increasing rapidly. The haemodynamic effect of the syndrome complicated by cardiogenic shock seems to combine allergic shock with extensive peripheral vasodilation and myocardial suppression with the characteristics of cardiogenic shock. Treatment of Kounis syndrome is challenging because it needs management of both cardiac and allergic manifestation simultaneously. We present a case report of type I Kounis syndrome, with coronary spasm secondary to cefuroxime injection complicated with cardiogenic shock. A brief review of the literature on the various facets of this condition is also provided.

Kounis syndrome is a rare anaphylactic reaction leading to coronary spasm, acute plaque rupture, or intrastent stenosis. Many types of medicine or environmental factors can potentially trigger Kounis syndrome by mast cell allergic reactions. In severe Kounis syndrome, reduced blood pressure or cardiac arrest may be accompanying symptoms. The treatment strategy for Kounis syndrome is usually difficult due to both cardiac dysfunction and allergic reactions. The delay to intervention to break the vicious circle may cause catastrophic complications. Therefore, early diagnosis is critical for physicians to outline detailed etiology for prevention and treat the cardiac and allergic symptoms in a timely manner. Here, we describe a case presenting rare severe Kounis syndrome with cardiac arrest which occurred after the administration of a contrast media.

Kounis syndrome is a rare type of acute coronary syndrome (ACS) that occurs as a result of an allergic or anaphylactic reaction. Kounis syndrome can be induced by various medications including antibiotics, proton pump inhibitors, antihypertensive medications, corticosteroids, and antineoplastic medications. Additionally, cases of Kounis syndrome associated with lansoprazole and pantoprazole have been previously reported in the literature. In this report, we present a case of Kounis syndrome associated with omeprazole use, and discuss the need for a high index of suspicion as it is often underrecognised.

Background: Kounis syndrome (KS) is defined as the concurrence of acute coronary syndromes associated with mast-cell and platelet activation in the setting of allergic or anaphylactic insults. Prevalence of KS is considered rare, estimated 1.1% of hospitalized allergic reaction patient, with mortality rate 7%. Case Report: Case of 39 years old male without history of coronary artery disease, having dyspnea, chest discomfort, and pruritus after eating skipjack tuna. 12 lead electrocardiogram showing ST depression segment on lead II, III, and aVF. Patient diagnose as Kounis syndrome variant type I. Treatment of Kounis Syndrome begins with cessation of the causative agent. After therapy using epinephrine and intravenous corticosteroid, clinical improvement was achieved, and ST segment depression on electrocardiogram resolved. Conclusion: Kounis syndrome is a rare disorder which need careful assessment and swift management. The diagnosis confirmed with sign and symptoms of allergic along with disorder in cardiovascular system. Latar Belakang: Kounis Syndrome (KS) didefinisikan sebagai sindrom koroner akut yang terjadi bersamaan dengan aktivasi sel mast dan trombosit dalam keadaan alergi atau anafilaksis. Prevalensi KS tergolong jarang, diperkirakan 1,1% dari pasien reaksi alergi yang dirawat di rumah sakit, dengan angka kematian 7%. Laporan Kasus: Laki-laki 39 tahun tanpa riwayat penyakit jantung koroner, sesak nafas, rasa tidak nyaman di dada, dan pruritus setelah makan ikan cakalang. Elektrokardiogram 12 sadapan menunjukkan segmen depresi ST pada sadapan II, III, dan aVF. Diagnosis pasien sebagai sindrom Kounis varian tipe I. Pengobatan Sindrom Kounis dimulai dengan penghentian agen penyebab. Setelah terapi menggunakan epinefrin dan kortikosteroid intravena, perbaikan klinis dicapai serta depresi segmen ST pada elektrokardiogram teratasi. Simpulan: Kounis syndrome adalah kelainan langka yang membutuhkan penilaian yang cermat dan penanganan yang cepat. Diagnosis ditegakkan dengan adanya tanda dan gejala alergi disertai gangguan pada sistem kardiovaskular.

The most common cause of death in the world is acute disorders of coronary blood circulation, and allergic diseases, which are progressively increasing, are recognized as a «disease of civilization». Kounis syndrome is a pathology characterized by coronary disorders against the background of hypersensitivity caused by various conditions (medications, environmental influences, food, coronary stents, etc.). Vasospastic allergic angina, allergic myocardial infarction, and stent thrombosis with occlusive thrombus infiltrated with eosinophils and/or mast cells are three variants of this syndrome. In Ukraine, clinical cases of Kounis syndrome have not been described, which is explained by low awareness and, accordingly, insufficient alertness of the medical community. Purpose - to acquaint the medical community with the etiology, pathogenetic mechanisms, features of clinical manifestations and modern diagnostic methods of Kounis syndrome. The article presents a review of modern literature sources that describe the prevalence, most common causes, mechanisms of development, clinical manifestations, and management of various variants of Kounis syndrome. Conclusions. Kounis syndrome is a common pathology characterized by the development of coronary blood circulation disorders against the background of allergic diseases. In the presence of any allergic pathology (especially during an exacerbation), a search for signs of coronary circulation disorders (еlectrocardiography, determination of the level of troponins and other cardiac enzymes) should be conducted for the purpose of timely diagnosis and treatment of Kounis syndrome. To predict the risk of development and recurrence of Kounis syndrome, it is recommended to study the allergic profile of the patient and cardiological risk factors already in childhood (conducting the multicomponent Alex test, determining the lipid profile, еlectrocardiography after allergic provocation), studying the genome. To prevent the occurrence and recurrence of Kounis syndrome, patients with allergic pathology should follow basic therapy to achieve control over inflammation. Kounis syndrome is a ubiquitous condition, representing a natural paradigm between coronary artery vasospasm, plaque rupture and anaphylactoid reactions. Kounis syndrome is underdiagnosed, as the inflammatory cells releases, in both allergic and non-allergic acute coronary syndrome, are the same. The syndrome is induced by various causes such as drugs, foods, environmental exposure, stents. There are three variants of Kounis syndrome: vasospastic allergic angina, allergic myocardial infarction, and stent thrombosis. All patients with systemic allergic manifestations correlated with clinical, laboratory and electrocardiography findings of acute myocardial infarction must be investigated for Kounis syndrome. The therapeutic management of Kounis syndrome represents a challenge, as it is necessary to treat both the cardiac and allergic events. After the acute event, a complete cardiological follow-up is mandatory, skin tests, verification of allergies to food, insect stings, drugs and environment agents. The awareness of etiology, epidemiology, pathogenesis, signs and symptoms is important for its diagnosis, treatment, prognosis and prevention.

The term allergic angina, introduced for the first time by Nicholas Kounis in 1991, initially referred to the coexistence of acute coronary syndromes with allergy or hypersensitivity. At present, it is believed that Kounis syndrome is a particular case of systemic disease, with multiorgan arterial involvement generated during immediate hypersensitivity reactions. Myocardial bridging (MB), a condition that can induce coronary artery spasm, has long been regarded as a benign condition. Since both pathologies are associated with arterial spasm, Kounis syndrome and MB are considered to be confounding pathologies for acute coronary syndromes, and their association is quite a rare finding. To date, there are no precise data on the epidemiology, and the population affected by Kounis syndrome seems to be highly heterogeneous. Since this is a rare disease, even less is known about possible different phenotypes, including MB overlap. We report a case of type I variant Kounis syndrome associated with MB with no evidence of coronary artery disease, occurring as late presentation, following a severe systemic reaction (anaphylaxis) induced by a Hymenoptera sting. At present, only two other cases of type I and one case of type II Kounis syndrome occurring in patients with myocardial bridging have been described.

Abstract Introduction: Fusosomes are viral vectors pseudotyped with modified paramyxovirus envelopes targeting specific cell types. A CD8-targeted fusosome delivering CD19CAR transgene has the potential to provide an off-the-shelf therapeutic approach to generate CD19-directed chimeric antigen receptor (CAR) T cells in patients after direct intravenous delivery or extracorporeal delivery (ECD), i.e., a short-term exposure of apheresis product to fusosome before re-infusion into the patient. Here, we demonstrate that an improved manufacturing process resulted in higher fusosome titer and quality, increasing the potency of CAR T generation from resting CD8 T cells. This potency is also observed when fusosomes are evaluated in a clinically relevant mock ECD approach. Methods: Fusosome titer was quantified by ddPCR directly for physical titer (vector genomes, GQA) or after infections of SupT1 cells for functional titer (IU/mL). Transduction of CD8 T cells was measured by flow cytometry and vector copy number analysis after short-term (1-4 hour) incubation of PBMCs with fusosome followed by wash and culture in resting or CD3/CD28-activated conditions. To assess CD19CAR T cell generation at a mock clinical setting and scale, a healthy donor was apheresed to collect PBMCs, fusosome was administered into the apheresis bag and PBMCs were sampled from the bag over a 2.5-hour time course for further analysis. In vivo anti-tumor efficacy of the fusosome was measured in immune-deficient NSG mice engrafted intravenously with Nalm6, then PBMCs followed by fusosome one day later. Tumor growth was monitored by bioluminescence imaging. Results: Fusosomes produced by the improved process had both a higher functional concentration (&gt;14-fold higher IU/mL) and better infectivity (&gt;10-fold higher IU/GQA) in comparison to fusosomes produced by the original process. Exposure of PBMCs to fusosomes produced by the improved process led to increased fusosome binding to CD8 T cells at high IU/PBMC doses and higher transduction efficiency in CD8 T cells at the same calculated dose per cell compared to fusosomes produced by the original process (&gt;3-fold higher mean CAR+ percentage after 2-hour exposure to 2 IU/PBMC, n=8). Furthermore, fusosomes produced by the improved process generated higher frequencies of CAR+ CD8 T cells and resulted in increased ability to control tumor growth at lower doses in vivo in a Nalm6 model. Importantly, the ECD approach translated well to a mock clinical setting, resulting in successful generation of CAR T cells (12.6% of CD8 T cells after a 2.5-hour incubation). Conclusion: CD8-targeted fusosome encoding a CD19CAR transgene represents a potential novel therapeutic approach for cancer. This study demonstrates that improvements in the manufacturing process can significantly increase the potency of the fusosome in T cells, and we intend to move forward to clinical trials using this improved process. Anaphylactic insults that cause cardiovascular signs and symptoms have been defined as Kounis syndrome, which has been associated with specific triggered anaphylactic reactions. Kounis syndrome has not been described in patients with no evidence of coronary artery disease (type I Kounis) in a scenario of idiopathic anaphylaxis. We reported a case of a 65-year-old white woman with no evidence of coronary artery disease who experienced two myocardial infarctions on separate occasions attributable to idiopathic anaphylaxis.

Two cemeteries associated with the early iron age occupation of Kourion have been excavated at the localities of Bamboula and Kaloriziki. In his publications of these cemeteries Benson attempted to demonstrate continuity of occupation at Kourion in the transitional bronze–iron age. A reanalysis of the pottery from both cemeteries has instead shown that the supposed continuity of occupation at Kourion is a result of Benson's erroneous identification of the final Late Cypriot and Initial Cypro-Geometric wares.

Kounis syndrome is an allergic acute coronary syndrome (ACS) characterised by coronary artery spasm, plaque erosion/rupture or stent thrombosis caused by mast cell and other interacting cell activation. Although intracoronary imaging modalities can detect those ACS mechanisms, Kounis syndrome due to plaque rupture has rarely been reported using intracoronary imaging. We present the case of a woman in her 70s who developed Kounis syndrome as a result of plaque rupture detected with optical coherence tomography (OCT). She had non-ST-segment elevation ACS as a result of anaphylaxis to cefazolin. Coronary angiography revealed severe stenosis in the left anterior descending artery; angiographically undetectable plaque rupture was detected using OCT. OCT also revealed intraplaque neovascularisation, suggesting that the culprit plaque had been vulnerable. OCT can aid in understanding the underlying mechanisms of Kounis syndrome.

The combination of acute coronary syndromes, illnesses associated with mast cell activation, interconnected and interacting inflammatory cells, allergic or hypersensitive reactions, and anaphylactic or anaphylactoid episodes is known as the Kounis syndrome. Non-steroidal anti-inflammatory drugs (such as ibuprofen, alclofenac, diclofenac, and naproxen) are the most frequently observed medications that cause Kounis syndrome, followed by antibiotics like ampicillin, azithromycin, ampicillin/sulfactam, amoxicillin, amikacin, cefazolin, cefoxitin, cefuroxime, cephradine, cinoxacin, lincomycin, penicillin, cefopera (5-fluorouracil, capecitabine, carboplatin, denileukin, interferons, paclitaxel, vinca alkaloids). There are three main types of Kounis syndrome: type 1 is allergic vasospastic angina, which is caused by endothelial dysfunction and is one of the causes of non-obstructive myocardial infarction; type 2 is allergic myocardial infarction; and type 3 is allergic stent thrombosis with an occluding thrombus (subtype a) or stent restenosis (subtype b). The first step in treating Kounis syndrome is to stop the allergic reaction, then use medicinal or interventional methods to stabilize the coronary arteries. Strong immunosuppressive and anti-inflammatory drugs, corticosteroids play a significant part in the management of allergic responses. Kounis syndrome is a form of acute coronary syndrome caused by allergic reactions. No cases of cardiac arrest caused by Kounis syndrome that arose after the administration of sugammadex have been reported. A 71-year-old female suffered two cardiac arrests. The first occurred after sugammadex was administered at the end of an operation for a right radial distal fracture. The patient was resuscitated and transferred to our intensive care unit. She was subsequently discharged home. Five months later, she suffered a second cardiac arrest after sugammadex was administered at the end of an operation for a right femoral neck fracture at our hospital. Urgent coronary angiography revealed multiple coronary spasms. Kounis syndrome was diagnosed based on the patient’s elevated serum trypsin levels and a positive result in a skin allergy test of sugammadex. In cases of cardiac arrest with unclear etiologies, Kounis syndrome should be considered.

Kounis syndrome is associated with mast cell activation resulting in acute coronary syndrome secondary to an allergic insult. Various drugs such as antibiotics, analgesics, and environmental exposures such as bee, wasp sting, and poison ivy are known to induce Kounis syndrome. A 68-year-old man admitted with a cobra bite on both hands to emergency care unit and sustained cardiorespiratory arrest. Electrocardiogram, taken 6 hours after the cardiac arrest, showed ST elevations in leads V2 to V5 suggestive of anterior ST elevation myocardial Infarction (STEMI). Serum Troponin was 10 ng/ml (control= <0.5). Serum IgE levels were significantly high (19155IU/ml, baseline 100). 2-Dimensional echocardiogram showed anterior and apical-septal hypokinesia with left ventricular ejection fraction of 30-35%. Coronary angiogram was normal. He remained hypotensive requiring inotropic and vasopressor support during ICU stay. This was a case of Kounis syndrome leading to cardiogenic shock secondary to Cobra (naja naja) bite. This is the only reported case of cobra bite causing Kounis syndrome and cardiogenic shock. Identification of the cause of myocardial infarction in snake envenomation is useful in the management as some of the drugs like adrenaline, morphine, and beta blockers may worsen the clinical syndrome if it is due to Kounis syndrome.

Kounis syndrome is one of the rare variants of acute coronary syndrome that occurs as a result of an allergic reaction. Its main pathophysiological mechanism is coronary vasospasm. Clinically, Kounis syndrome is manifested by symptoms of both an allergic reaction and an acute coronary syndrome, and its treatment is not an easy task, since both cardiac and allergic manifestations must be treated at the same time. In the given article we describe a clinical case of diagnosis and successful treatment of Kounis syndrome caused by cold urticaria in a 58-year-old patient who was hospitalized to the Grodno Regional Clinical Cardiology Center.

AbstractCoronary vasospasm can result from silent myocardial ischaemia to sudden death. There are many precipitant factors including different pharmacological agents. Kounis syndrome is defined by acute coronary syndromes associated with anaphylactic or anaphylactoid reactions. We report, to the best of our knowledge, the first paediatric case of Kounis syndrome due to intravenous atropine.

Drug-induced disease is a common clinical entity. Drug-inducing anaphylaxis is a serious adverse effect. Several cases of allergic acute coronary syndrome or Kounis syndrome were reported. Premature ventricular contractions are the most frequent cardiac arrhythmia with or without structural heart diseases. A premature ventricular contraction is a sign of decreased oxygenation to the myocardium and anxiety but is also found in a healthy heart. A middle-aged married male patient presented to the physician outpatient clinic with syncope within one hour after ingested one tablet of diclofenac potassium (50mg). Diclofenac potassium-induced anaphylaxis and Kounis type I syndrome with premature ventricular contractions. Electrocardiography, oxygenation, monitoring for vital signs, and echocardiography were the done interventions. The dramatic disappearance of anaphylactic shock, Kounis type I syndrome, coronary artery spasm, and premature ventricular contractions after the traditional treatment of anaphylaxis had happened. Complete clinical and electrocardiographic recovery had achieved. The identi􀏐ication of drug-induced disease is a pivotal step in the diagnosis decision making of any medical problems. Diclofenac potassium can induce anaphylactic shock, Kounis type I syndrome, coronary artery spasm, and premature ventricular contractions. Kounis type I syndrome, coronary artery spasm, and premature ventricular contractions can be reversed with treatment of the cause without using anti-ischemic or ant-arrhythmic measures. Reassurance was the recommended regards diclofenac potassium-induced both coronary artery spasm and premature ventricular contractions that accompanied by anaphylaxis.

Introduction: Kounis Syndrome was first described in 1991 by Kounis and Zavras as “the concurrence of chest pain during an allergic reaction, accompanied by clinical laboratory findings of classical angina pectoris caused by inflammatory mediators released during the allergic insult” [1]. The mechanism of Kounis Syndrome most likely involves the release of cytokines through mast-cell degranulation, which leads to coronary vasospasm and atheromatous plaque erosion or rupture following the allergic reaction to an allergen.[2] The treatment is specific to acute coronary syndrome and anaphylaxis, with the added complication that the drugs used, while indicated in each of the two disorders separately, may present contradictions when administered jointly in one patient.[3] The purpose of this review is to briefly revise the existing literature regarding its overlooked diagnosis and contradictory joint management of anaphylaxis and acute coronary syndrome. We will conduct a brief review of the current literature on Kounis Syndrome while describing a suspected case of a female patient presented with both anaphylaxis symptoms and angina pectoris. Conclusions: Kounis syndrome is defined as the co-incidental occurrence of an acute coronary syndrome with hypersensitivity reactions following an allergic reaction. Treatment of allergic reactions may be sufficient in type I KS. In contrast, coronary intervention is needed in the other two types, accompanied by vasodilator drugs, including nitrates and calcium antagonists, each of which may have contradictory effects.

Kounis syndrome is a multisystem and multidisciplinary disease affecting the circulatory system that can be manifested as spasm and thrombosis. It can occur as allergic, hypersensitivity, anaphylactic, or anaphylactoid reactions associated with the release of inflammatory mediators from mast cells and from other interrelated and interacting inflammatory cells, including macrophages and lymphocytes. A platelet subset with high- and low-affinity IgE surface receptors is also involved in this process. Whereas the heart, and particularly the coronary arteries, constitute the primary targets of inflammatory mediators, the mesenteric, cerebral, and peripheral arteries are also vulnerable. Kounis syndrome is caused by a variety of factors, including drugs, foods, environmental exposure, clinical conditions, stent implantation, and vaccines. We report a unique case of a 60-year-old male with a past medical history of allergy to human albumin, alcoholic cirrhosis, and esophageal varices, who was admitted due to multiple episodes of hematemesis. Due to low hemoglobin levels, he was transfused with 3 units of red blood cells and fresh frozen plasma without any adverse reactions. On the third day of hospitalization, severe thrombocytopenia was observed and transfusion of platelets was initiated. Immediately following platelet infusion, the patient developed chest discomfort, skin signs of severe allergic reaction, and hemodynamic instability. The electrocardiogram revealed ST segment elevation in the inferior leads. Given the strong suspicion of Kounis syndrome/allergic coronary spasm, the patient was treated with anti-allergic treatment only, without any anti-platelet therapy. The clinical status of the patient gradually improved and the electrocardiographic changes reverted to normal. Based on these findings, Kounis hypersensitivity-associated acute coronary syndrome, specifically, type I Kounis syndrome, was diagnosed. Although platelet transfusion can be a life-saving therapy, each blood transfusion carries a substantial risk of adverse reactions. The aims of this report are to expand the existing knowledge of patient responses to blood transfusion and provide information on the incidence of various severe transfusion reactions to all blood components and especially to platelets. To the best of our knowledge, Kounis syndrome induced by platelet transfusionhas never been previously reported. Hypersensitivity to platelet external membrane glycoproteins in an atopic patient seems to be the possible etiology. Despite that Kounis syndrome remains an under-diagnosed clinical entity in everyday practice, it should always be considered in the differential diagnosis of acute coronary syndromes.
