Formal education has always been the principal model through which societies educate their young. Today, however, schools are seen as non-analytical and are being replaced by logic-based approach of competitive examinations. In India, the growth of shadow education, which is preponderant at the level of secondary education, is providing informal avenues through which the youth garner knowledge and skills. One such avenue is the private coaching for the Joint Entrance Exam to enter into the premier Indian Institute of Technology(s). The article draws on an exploratory study carried out in the city of Delhi and examines the credentialing strategies of IIT aspirants through Joint Entrance Exam coaching and how the pedagogical practices of coaching institutes influence the perceptions and proclivities of aspirants and their families towards school education. I also discuss the policy implications of such practices for formal schooling.

Purpose The purpose of this paper is to identify the coaching structures that aspiring principals associate with developmentally consequential coaching interactions; identify structural features/functions/attributes that shape a structure’s developmental utility and use; and consider how a multifarious coaching structure might advantage the learning experiences of aspiring principals. Design/methodology/approach This qualitative study included multiple interviews with two cohorts of aspiring principals (n=20) from one preparation program and with their leadership coaches (n=5) and was framed using the theories of social capital and networks, situated learning, and distributed cognition. Findings The authors identified eight coaching structures that aspirants identified as consequential to their learning and development. The authors identified four structural features/functions/attributes that shape a structure’s developmental utility. The authors identified three factors that contribute to the developmental utility of this multifarious coaching model. Research limitations/implications This study includes a relatively small participant sample –70 percent of the aspiring principals from two cohorts within one preparation program. Data do not include direct observations of coaching interactions within the context of individual coaching structures. Practical implications The findings suggest that the structuring of leadership coaching is a critical consideration for those designing leadership coaching programs. This multifarious structuring of leadership coaching created three developmental affordances. Originality/value This paper generates new knowledge for the field of principal preparation related to the structuring of leadership coaching and ways in which structuring can shape aspirant learning experiences. These findings are likely to also be instructive to those interested in coaching more generally.

This article explores the judicialisation of party primaries in contemporary Nigeria, which is a defining feature of the country’s electoral politics. Since the inception of the Fourth Republic, the lack of internal democracy within the parties has been the source of protracted crises during nomination, and this often gravitates to the serenity of the court(s). Dominant disquisitions in legal theory contend that disputed primaries are internal party affairs; hence, they are non-justiciable. Drawing on primary and secondary data – YouTube interviews, the Constitution, the Electoral Act, judicial ruling, media reports, and personal observation – this article argues that to the extent that political parties are juridical entities, disputed primary elections are justiciable, hence a legal question to be resolved by the judiciary. To validate our argument, the article draws on Raphael’s (1970) notion of universal and compulsory jurisdiction. Our enquiry reveals that the failure of the internal mechanisms of the parties to resolve disputed party primaries accounts for aggrieved aspirants’ reliance on legal redress. While this approach has been questioned from a legalistic point of view, the constitutionality of seeking legal redress has its provenance in the change of legal regime regulating party primaries, which has shaped, reshaped, and positively impacted electoral democracy in Nigeria.

Critical thinking, as opposed to technical or professional abilities, enhances intellectual capacities and promotes the development of multi perspective thinking. It promotes the growth of the mind's analytical and creative regions. Enhancing critical thinking abilities in higher education constructs an excellent platform for aspirant engineers. To analyze complex problems, investigate various techniques, and design solutions, critical thinking skills are required. The most vivid imaginations are born to artists and musicians. Teaching artists how to use software and graphics tools is relatively simple; however, transforming engineers into artists is far more difficult. Students who use visualization in their studies are encouraged to look at the world from various angles. Sports and academics complement each other well. There are also links between the arts, logic, and athletics. It is beneficial to understand the relationship between critical thinking and academics. This study covers some strategies related to puzzle solving, problem-based learning, design thinking, picture analysis, and experiential learning in order to develop critical thinking and practical problem-solving skills. Whole exome sequencing was used to determine the causative gene in patients with B cell defects of unknown etiology. A homozygous premature stop codon in exon 6 of PIK3R1 was identified in a young woman with colitis and absent B cells. The mutation results in the absence of p85α but normal expression of the p50α and p55α regulatory subunits of PI3K. Bone marrow aspirates from the patient showed <0.1% CD19+ B cells with normal percentages of TdT+VpreB+CD19− B cell precursors. This developmental block is earlier than that seen in patients with defects in the B cell receptor signaling pathway or in a strain of engineered mice with a similar defect in p85α. The number and function of the patient’s T cells were normal. However, Western blot showed markedly decreased p110δ, as well as absent p85α, in patient T cells, neutrophils, and dendritic cells. The patient had normal growth and development and normal fasting glucose and insulin. Mice with p85α deficiency have insulin hypersensitivity, defective platelet function, and abnormal mast cell development. In contrast, the absence of p85α in the patient results in an early and severe defect in B cell development but minimal findings in other organ systems.

Visceral leishmaniasis (VL) is a life-threatening disease characterized by uncontrolled parasitization of the spleen, liver, and bone marrow. Interleukin (IL)-10 has been implicated in the suppression of host immunity in human VL based on the elevated levels of IL-10 observed in plasma and lesional tissue, and its role in preventing clearance of Leishmania donovani in murine models of VL. The aim of this study was to identify the cellular source of IL-10 in human VL and determine if CD4+CD25+ (Foxp3high) regulatory T (T reg) cells are associated with active disease. We analyzed surface marker and gene expression in peripheral blood mononuclear cells and splenic aspirates from Indian VL patients before and 3–4 wk after treatment with Amphotericin B. The results did not point to an important role for natural CD4+CD25+ (Foxp3high) T reg cells in human VL. They did not accumulate in and were not a major source of IL-10 in the spleen, and their removal did not rescue antigen-specific interferon γ responses. In contrast, splenic T cells depleted of CD25+ cells expressed the highest levels of IL-10 mRNA and were the predominant lymphocyte population in the VL spleen. The elevated levels of IL-10 in VL plasma significantly enhanced the growth of L. donovani amastigotes in human macrophages. The data implicate IL-10–producing CD25−Foxp3− T cells in the pathogenesis of human VL.

¡AUDITIO se renueva! Llega el Día Mundial de la audición y no podíamos haber elegido mejor momento para dar la bienvenida a la nueva AUDITIO. Tras muchos meses de trabajo, AEDA ha conseguido renovar de manera única su revista científica. Un equipo, liderado el Dr. Raul Sánchez, flamante Editor Jefe de la revista, empezó a trabajar con el objetivo de renovar, modernizar y adaptar la revista a las necesidades del siglo XXI. Puede que les sorprenda pero AUDITIO es una revista pionera en audiología pues fue diseñada, por la Fundación Canaria Dr. Barajas en 2001, como una revista electrónica de acceso libre. Les aseguro que en 2001 este formato era toda una novedad y este hecho ha marcado su existencia. La distribución de AUDITIO a través de internet entre la comunidad hispanohablante la convirtió en la revista de Audiología de mayor difusión en lengua española y por tanto, una de las revistas más leídas. AUDITIO siempre ha mantenido su orientación académica y ha cumplido con éxito su objetivo: la divulgación gratuita de la Audiología. Entonces, ¿por qué tenía que ser renovada?. Era una demanda de nuestros asociados y una espina clavada en muchos de nosotros. Tanto que se convirtió en un objetivo de mi programa de actuación para la legislatura de Presidencia en las elecciones del 2020. El motivo de esa demanda era claro: su falta de periodicidad. En el último lustro AUDITIO ha sufrido altibajos y la falta de regularidad, por una menor recepción de publicaciones. No ha sido por falta de trabajo de su Editora Jefe, la Dra. Bartolomé, que supo rodearse de un gran equipo de revisores, supo incentivar a los universitarios y manejó con maestría sus contactos con prestigiosos investigadores para mantener la mayor regularidad posible. Todos debemos agradecerle su esfuerzo, dedicación y resultados pero AUDITIO enfermaba lentamente y no supimos detectarlo ni fuimos capaces de encontrar soluciones. Y ante la enfermedad, renovar o morir. Era la cruel realidad que AEDA tenía que afrontar. Y tras profundizar en los motivos, descubrimos que AUDITIO aun tenía posibilidades y decidimos apostar por ella y marcar un punto de inflexión. AUDITIO y la audiologia en español lo merece. Porque la audiología es una de las disciplinas de la ciencia con mayor producción científica y en ella debemos fundamentar su desarrollo futuro. AUDITIO debe mantener su papel único, como medio de expresión dentro de la audiología en lengua española. Un elemento clave en la promoción de la práctica clínica basada en la evidencia científica y el altavoz del conocimiento en audiología en lengua española donde publicar nuevas investigaciones, proyectos, guías de la práctica clínica, resúmenes de congresos etc. Un medio de comunicación ágil, joven y dinámico, ajustado al mayor rigor científico. AUDITIO merece la pena y debe tener su espacio en nuestra disciplina y nuestra lengua (la 2ª lengua materna más hablada en el mundo). Hoy, Día Mundial de la Audición, empieza una nueva era para AUDITIO. Una revista cuyas publicaciones serán fácilmente localizables por los lectores, una revista moderna, acorde a las necesidades de autores y lectores. Para ello, aspiramos fuerte y adoptamos el sistema Open Journal System (para la gestión de contenido, postulaciones, revisiones y producción), registramos todo el contenido con Digital Object Identifiers (DOI), adecuamos la revista a los estándares de calidad de FECYT y ajustamos los contenidos para una mejor indexación de Google Scholar, RedIB, PubMed, WoS, Scielo, etc. Un nuevo tiempo, un salto de calidad, un objetivo al alcance de esta Junta Directiva, un esfuerzo de los asociados de AEDA y un brillante trabajo, catalizado por su vocal y Director Jefe de la revista, Dr. Raul Sánchez-López. ¡Enhorabuena a todos!

Abstract Adenosine accumulation within the tumor microenvironment can severely limit antitumor immunity by promoting the expansion of immunosuppressive cell types and impairing immune cell function, including T cells, natural killer cells and dendritic cells. The generation of adenosine from adenosine monophosphate (AMP) requires the activity of a cell surface ecto-5’-nucleotidase, CD73, expressed on the surface of various cell types including immune cells. CD73 overexpression is observed in many tumor types and correlates with unfavorable clinical outcomes. Given the essential role of CD73 in generating adenosine, inhibition of CD73 is a promising therapeutic strategy. Recent clinical proof of concept data demonstrated a significant improvement in progression free survival for non-small cell lung cancer patients upon targeting CD73 with the oleclumab anti-CD73 antibody in combination with anti-PDL1, relative to anti-PDL1 checkpoint inhibitor treatment alone.We developed ORIC-533, a potent, orally bioavailable, AMP-competitive, small molecule inhibitor of CD73, that is highly selective for CD73 and exhibits picomolar potency in biochemical assays, completely blocking adenosine production from AMP. In ex vivo assays, ORIC-533 displayed greater potency in restoring immunosuppressed CD8+ T cell proliferation and activation compared to oleclumab, as well as greater potency versus small molecule inhibitors of CD73 and the adenosine receptor. In multiple myeloma (MM) patients, elevated adenosine levels in the bone marrow (BM) niche correlate with progression of multiple myeloma through a CD73-mediated pathway. Moreover, we recently reported a functional role of CD73 signaling in BM of MM patients, indicating that CD73 inhibition may represent a unique vulnerability and treatment strategy for MM (Ray et al., abstract #2675, ASH 2021). We tested the impact of CD73 inhibition in bone marrow aspirates from patients with relapsed or refractory MM, using an autologous ex vivo assay system comprised of the multiple myeloma BM microenvironment. CD73 inhibition stimulated the activation of plasmacytoid dendritic cells and T cell activation in the context of the MM BM milieu. Moreover, ORIC CD73 inhibitor as a single agent overcame immune suppression and triggered significant lysis and cell death of multiple myeloma cells by autologous T-cells in the bone marrow microenvironment. Taken together, these results demonstrate that the ORIC CD73 inhibitor potently inhibits the adenosine pathway, which restores anti-tumor immunity and therefore holds potential for patients with MM. ISRCTN: 12451433 Background: Myelofibrosis (MF), a chronic myeloproliferative neoplasm (MPN), often results in progressive splenomegaly and cytopenia (1). Acute myeloid leukaemia develops in 10-20% of patients, and median survival with intermediate-2 disease is four years (2). Stem cell transplant can be curative, but is only suitable for younger, comorbidity-free patients (median MF diagnosis is 73.6 years) (3). Janus Kinase 2 (JAK2) mutation is common in MPN, and the JAK1/2 inhibitor ruxolitinib was approved in 2011 (4). However, patients still experience high disease burden, with a clear need for further treatments. Research has now focused on bromodomain and extra terminal (BET) inhibitors (5), in particular the role these have in disease associated inflammation (6). Preclinical data suggests BETi disrupts this inflammation leading to anti-fibrotic activity within MPN models. BETi with ruxolitinib eliminates fibrosis in MF mouse models thus overcoming a limitation of ruxolitinib monotherapy (7). OPN-2853 (previously PLX2853) is potent against isolated bromodomains from multiple BET proteins with rapid drug absorption, high Cmax, and short half-life differentiating it from other BETi. Preclinical efficacy has been demonstrated across a range of hematologic malignancies. PROMise is evaluating OPN-2853 in combination with ruxolitinib to identify the recommended phase 2 dose (RP2D). Methods: Primary Objectives: To assess the RP2D of OPN-2853 in combination with ruxolitinib that is safe and tolerableTo assess the efficacy of the combination of OPN-2853 and ruxolitinib for reduction in spleen size PROMise is a single country (UK) multicentre study, recruiting up to 60 patients with high or intermediate-2 risk MF who are not receiving an adequate response on ruxolitinib alone. Patients must have been treated with ruxolitinib for at least 24 weeks, on a stable dose for at least 4 weeks, and have ongoing splenomegaly. PROMise is split into three ruxolitinib dose categories based on platelet count which are assessed independently, Low Dose (5mg-20mg daily), Mid Dose (25mg-45mg daily) and High Dose (≥50mg daily). These are evaluated in combination with three OPN-2853 dose levels, Level 1 (20mg OD Decreased Dose), Level 2 (40mg OD Starting Dose) and Level 3 (80mg OD Increased Dose). Cohorts of patients (n=2-4) receive up to eight 21-day cycles of OPN-2853 and ruxolitinib. Patients are closely monitored for Dose Limiting Toxicities in cycle 1. The Trial Safety Committee (TSC) meets following cycle 1 to establish the dose recommendations of OPN-2853 for each dose category using the continual reassessment method (CRM). The CRM is utilised independently for each dose category, as such it is possible for the low, mid and high doses to be allocated to different OPN-2853 doses. Following cycle 8, patients may continue to receive OPN-2853 if experiencing clinical benefit and are followed up annually. Peripheral blood, bone marrow aspirate and bone marrow trephine samples are taken from all patients to measure the proportion who achieve a molecular response, and to determine the on target transcriptional response in myeloid cells. Additionally, pharmacokinetic samples are taken at specified timepoints pre and post dose to establish PK properties of OPN-2853 in combination with ruxolitinib. The higher education system in India is creating newer structures of hierarchy and differentiation to become distinct. This is largely due to the rising demand for professional education, the returns of which are perceived to be higher than a general degree. Middle-class families have always operated within distinctive structural frameworks to contest for social cachet through education. In India, one such arena is the Indian Institute of Technology (IIT). To determine who is meritorious enough to secure an IIT seat, the Joint Entrance Exam (JEE) has become the arbiter where merit is the relative rank of contestants and shadow education a key training site to ace the JEE. This article is based on a sociological analysis of family credentialing strategies around shadow education that prepares aspirants for the IIT-JEE. Drawing on an analysis of the middle class in Delhi, I discuss how these strategies are feeding fodder to the aspirants’ dream of pursuing an IIT degree and exacerbating educational stratification. I also show how the private coaching market in India reinforces this divide by way of a segregation policy which incentivises a few to secure higher ranks in the JEE.

As per the Merriam Webster dictionary, the definition of the distance learning is - “It is a method of study where teachers and students do not meet in a classroom but use the Internet, e- mail, video conference, audio conference mediums of the study. The assessments and doubt clearing also happen online and still there are some planned face to face interaction programs with the students.” In the late 1900s, correspondence courses started coming into the picture. These courses were mainly introduced for the working professionals and for the people who wish to go for competitive examinations as travelling to the university is difficult if it is far off. This is to enhance the access and reach for the learners. Many of the tutorial companies and coaching centers also use distance learning model to cater to the needs of IIT JEE/Civil services aspirants in India. However, distance-learning courses have much better acceptance in the western countries. In Europe and America, they have wider acceptance and several studies done have proven the need of the distance courses and the perception of the people about the distance courses is really good.

AbstractIntroductionDue to the COVID‐19 pandemic, the Undergraduate Medical Doctor (MD) Programme at McMaster University (Hamilton, Canada) was unable to run in‐person medical school interviews in March 2020, prompting an alternate solution that maximised admission opportunities for Indigenous applicants, prioritised admission for those rated most highly in the interview determination process, and allocated subsequent offers via lottery.MethodsA short survey was administered to applicants who had been offered an admissions interview and were subsequently impacted by the admissions adaptations. The survey elicited perceptions of the adaptation through Likert scale ratings and free‐text responses. Survey data were analysed via a sequential (quantitative to qualitative) mixed‐methods design.Results196 of 552 potential participants completed the survey. Across quantitative and qualitative analyses, respondents reported that the adaptation had a negative impact on their professional development and personal life. Ratings of negative perception were greater for those who did not receive an offer than for those who accepted or declined an offer. Free text responses emphasised considerable criticism for the lottery portion of the adaptation and displeasure that efforts made in constructing applications were less relevant than anticipated.DiscussionThe negative responses to this unexpected change highlight the profound upstream impact admission policies have on the preapplication behaviours of aspiring medical students. The outcomes support a refined understanding of the value candidates place on the interview in appraising their own suitability for a career as a physician.

Severe COVID-19 patients develop acute respiratory distress syndrome that may progress to cytokine storm syndrome, organ dysfunction, and death. Considering that neutrophil extracellular traps (NETs) have been described as important mediators of tissue damage in inflammatory diseases, we investigated whether NETs would be involved in COVID-19 pathophysiology. A cohort of 32 hospitalized patients with a confirmed diagnosis of COVID-19 and healthy controls were enrolled. The concentration of NETs was augmented in plasma, tracheal aspirate, and lung autopsies tissues from COVID-19 patients, and their neutrophils released higher levels of NETs. Notably, we found that viable SARS-CoV-2 can directly induce the release of NETs by healthy neutrophils. Mechanistically, NETs triggered by SARS-CoV-2 depend on angiotensin-converting enzyme 2, serine protease, virus replication, and PAD-4. Finally, NETs released by SARS-CoV-2–activated neutrophils promote lung epithelial cell death in vitro. These results unravel a possible detrimental role of NETs in the pathophysiology of COVID-19. Therefore, the inhibition of NETs represents a potential therapeutic target for COVID-19.
