This chapter describes the strategies for business continuity when a significant challenging event affects a hospital or other healthcare provider: the scenario is a norovirus outbreak affecting several wards and staff. The strategy includes business impact analysis and a disaster recovery plan. The importance of developing a generic response plan, rather than a response for each type of incident, is emphasized. The early involvement of the infection control team, isolating or cohorting patients, and liaison with the community are essential components of the response. The chapter describes how a ‘less serious event’ (a few reported cases) may rapidly escalate into a major incident. The business continuity plan should be implemented early, and should identify which services can be stopped, and which must continue. Finally, the importance of holding a multi-agency and multi-professional debrief meeting as soon as the incident is declared over is emphasized, with revision of the plan accordingly.

Fires can cause significant health concerns within local communities impacted by any associated smoke plume. This chapter discusses the potential public health concerns associated with fires, in particular fires at waste-processing installations. Using an example scenario, actions to be undertaken throughout the incident response, from initial acute phase to recovery, are considered, along with health concerns and fears, real or perceived, involvement of asbestos-contaminant material, multi-agency communication mechanisms, and potential issues associated with long-running fires. The multi-agency mechanisms for response are detailed, including the various coordinating groups (strategic, tactical, recovery coordinating groups (SCG, TCG, RCG, respectively), and expert cells (scientific and technical advisor cell, air quality cell (AQC)). Key points to note in the incident response include concerns raised by the local population, typical health effects associated with exposure to a smoke plume, and tools that support the response to the incident and the public health risk assessment.

This chapter covers two case studies and scenarios: a case of pulmonary tuberculosis (TB) in a college student; and a case of laboratory-confirmed Mycobacterium bovis in an adult with inflammatory bowel disease. The Mycobacterium bovis resulted in wider workplace and hospital contact tracing through convening an Incident Control Team. Background information on the epidemiology and clinical features of TB and the public health response to TB in educational, healthcare, and occupational settings are discussed. Case definitions, and a detailed risk assessment, with clear description of close contacts, priority groups, and the required public health actions, are described. ‘Top tips’ are given, to provide practical tips for the reader to think through the public health management of TB, and ‘tools of the trade’ list the laboratory and epidemiological components of the investigation.

Paediatric critical care retrieval provides some of the most challenging clinical scenarios for the retrieval physician. Children have a relatively low incidence of critical illness in comparison to adults and they constitute a minority of the population (around 20% or less in high-income countries). Approximately 50% of critically ill children are under 2 years of age, with a more even age-distribution from pre-school through to school-age and teenage years. Consequently, paediatric intensive care and paediatric intensive care retrieval are low volume, highly specialized areas of practice in healthcare systems that cater predominantly for adults. In comparison to neonatal retrieval, the case load in paediatric intensive care transport is small; however, there is a much wider spectrum of pathology. Thus, paediatric intensive care transport differs from both adult and neonatal retrieval. It requires appropriate specialist expertise and skills in the entire age-range and disease spectrum of paediatric intensive care as well as a well-designed paediatric retrieval system.

Scandali finanziari e crisi più e meno recenti pongono un fondamentale interrogativo: a che servono molteplici meccanismi di controllo innanzi ad un ineliminabile rischio di “abusi di mercato” e di frodi nei confronti dei risparmiatori? La risposta del legislatore è rappresentata da un consistente moto di riforme intese a conferire nuovi doveri, poteri e, soprattutto, credibilità ai controlli, interni ed esterni alle società, nonché all’autorità di vigilanza sul mercato. È dato supporre che una simile palingenesi della funzione di controllo non possa non incidere sulla responsabilità penale dei suoi titolari, in particolare, per l’omesso impedimento dei reati commessi dagli organi esecutivi di una società, in danno di un interesse collettivo di recente emersione: il “risparmio”. Questo volume ricostruisce tale problematica, rivisitando temi classici, quali il reato omissivo improprio e la compartecipazione omissiva, alla luce delle questioni sollevate dall’assurgere delle organizzazioni societarie ad apparati procedurali complessi, entro i quali lo schema gerarchico e i tradizionali equilibri di potere subiscono una significativa metamorfosi. Seizures are a common problem in intensive care units (ICU) and the advent of continuous electroencephalography is demonstrating that the incidence of seizures is still underestimated. Many patients considered encephalopathic from any cause are now found to be in non-convulsive status epilepticus. While the significance of non-convulsive seizures remains unclear, there is little disagreement that these seizures should be treated with antiseizure agents and prevention of any type of seizure is a reasonable therapeutic goal. Many antiseizure agents have been studied in ICU populations and extensive experience exists with drugs such as phenytoin, valproate, or pentobarbital. Since the previous edition of this textbook, several new antiseizure agents have been introduced. Levetiracetam, topiramate, and lacosamide have been established as reasonable pharmacologic options, in particular for treatment of status epilepticus. Patients with seizures in the ICU often present with challenging clinical scenarios, which influence the choice of antiseizure agents. For example, reduced liver or renal function, especially if needing continuous renal replacement therapy or intermittent haemodialysis, has an impact on drug level variability and susceptibility to seizure development. ICU patients will typically require a multitude of pharmacological agents for their specific clinical situation and drug–drug interactions must be considered. Additionally, many medications used in ICUs are associated with seizures, in particular, certain antibiotics. Overall, the development of new drugs and better monitoring methods will undoubtedly improve our ability to control seizures in ICU patients, but currently no treatment has been shown to be universally effective for challenges, such as refractory status epilepticus.

Leishmanioses are a large group of parasitic diseases range over the intertropical zones of America and Africa, and extend into temperate regions of South America, Southern Europe and Asia. The clinical aspect of the diseases is wide ranging from a simple, self-resolving cutaneous lesion to the potentially fatal visceral leishmaniosis, known as kala-azar. In numerous underdeveloped countries leishmanioses remain a major public health problem representing one of the most neglected diseases. Among 15 well-recognized Leishmania species known to infect humans, 13 have definite zoonotic nature, which include agents of visceral, cutaneous and mucocutaneous forms of the disease in both the Old and New Worlds. Mammal reservoir hosts belong to the marsupalia, edentata, carnivora, hyracoidea, and rodentia, maintaining sylvatic zoonotic foci in the deserts of Africa and Asia, the forests of South and Central America, as well as synanthropic foci in the Mediterranean basin and much of South America. Although the known vectors are all phlebotomine sandflies, these have a wide range of specific habits and habitats. The complexity of this group of infections has only recently been appreciated and is still being worked out. Currently, leishmanioses show a wider geographical distribution than previously known, with increased global incidence of human disease. Environmental, demographic and human behavioural factors contribute to the changing leishmaniosis landscape, which basically include increasing risk factors for zoonotic cutaneous leishmanioses, and new scenarios associated with the zoonotic entity of visceral leishmaniosis. In comparison with the anthroponotic entities of leishmaniosis, limited progresses were made for the control of the zoonotic ones, consisting mainly in new tools developed for the control of L. infantum in the canine reservoir.

Lo iuris utriusque doctor Paolo da Castro (1360/62-1441), nella sua lunga ed onorata carriera accademica, lesse soltanto il gius civile. Il vasto – e ancora non sufficientemente esplorato – campo dei suoi consilia mostra però una profonda conoscenza anche del diritto canonico, fatto di cui anche il pubblico dei richiedenti dovette esser consapevole, vista la fiducia con cui sollecitò al giurista responsi in materia non soltanto civilistica. Partendo, perciò, da alcuni consilia che affrontano tematiche di attinenza canonistica (specialmente quando la competenza dei canoni entrava in concorrenza col diritto civile, come nel caso emblematico dell’usura cui finirono per connettersi le svariate questioni implicanti i mai pacificati rapporti istituzionali con le comunità ebraiche residenti nelle città italiane), si sono confrontate le opinioni espresse dal giurista con le posizioni che egli tiene nei passi corrispondenti delle lecturae accademiche e con quelle del suo maestro Baldo degli Ubaldi e di altri autori vicini, per tempo o per altre affinità (e.g. Bartolo, Lorenzo Ridolfi, Oldrado, Antonio Roselli, Giovanni da Imola), tentando di ricostruire la linea di pensiero di Paolo da Castro nella materia affrontata e la sua incidenza sulla communis opinio posteriore. Una dipendenza di Paolo da Castro da alcune teorie economiche e morali risalenti a Pietro di Giovanni Olivi (che aveva insegnato per qualche anno alla scuola teologica di Santa Croce a Firenze) è stata ipotizzata a partire da alcune possibili coincidenze favorite dal lungo soggiorno fiorentino del giurista. Queste avrebbero influenzato il pensiero di Paolo indirizzandolo verso posizioni etiche severissime, anche nei confronti di quella Chiesa di cui il giurista fu sempre un devoto, ma non inerte, seguace. Il panorama che si è venuto delineando durante il percorso d’indagine, perciò, non solo – secondo l’A. – mostrerebbe scenarî forse inattesi nel quadro giurisprudenziale relativo alle tematiche trattate e al pensiero di Paolo da Castro, specialmente nel suo formarsi; ma, soprattutto, consentirebbe di giustificare (e dunque elidere), su basi squisitamente tecniche ma anche intellettuali, le discrasie presenti in Paolo, non meno che in molti altri giuristi dei secoli tra Trecento e Cinquecento, tra l’opinio espressa come consulente e le interpretazioni sostenute leggendo dalla cattedra universitaria.
