Abstract Purpose: The NCCN Harmonized Guidelines for sub-Saharan Africa (SSA) provide evidence-based recommendations for standardized breast cancer (BC) care that are resource-appropriate, regionally developed and endorsed. This study aims to evaluate the proportion of BC patients receiving guideline concordant care in Southwest Nigeria, and their survival outcomes. Methods: A retrospective analysis was performed of consecutive patients diagnosed with non-metastatic BC at Obafemi Awolowo University Teaching Hospital, a public referral hospital in Ife, Nigeria. The NCCN Harmonized Guidelines for SSA - Breast Cancer, version 4.2021 were used to define concordant care, following the minimum practice standards for lower resource environments. Concordance was estimated with exact 95% confidence intervals. Overall survival (OS) was estimated with Kaplan Meier methods. Results: Between January 2016-December 2021, 517 patients were diagnosed with non-metastatic BC (stage I/II: n=101, 20%; stage III: n=416, 80%). Median age was 49y. Diagnostic work-up was guideline-concordant in 6.4% (n=33/517; 95%CI 4.4-8.8%). 98% (n=508/517) received a pre-treatment tissue diagnosis, 86% (n=373/435) completed an appropriate metastatic work-up, but only 18% (n=91/516) underwent breast imaging and 28% (n=142/517) had hormone receptor testing. Locoregional treatment concordance was 3.7% (n=19/517; 95% CI 2.2-5.7%) overall. 43.1% (n=223/517) of patients underwent surgery and 4.3% (n=22/517) received adjuvant radiation where indicated. 24% (n=122/517) received adjuvant chemotherapy. Guideline concordance was highest for neoadjuvant systemic treatment (55%, n=181/329; 95%CI 49-60%), followed by hormonal therapy where hormone receptor status was known (47%, n=24/51; 33-62%). Median follow-up was 17 months (IQR: 3-40). 3-year OS was 70% (59-84%) for stage I/II patients and 46% (40-53%) for stage III. Improved OS was seen across all major treatment categories for stage III patients who received guideline-concordant care compared to those who did not. Conclusion: Concordance with resource-adapted, regional practice guidelines for BC was low. Under-utilization of surgery, limited access to radiation and low rates of hormone receptor testing/therapy were key drivers of this finding. The association of concordant care with improved OS supports the utility of the guidelines & identifies areas where investments to strengthen care through guideline adherence could lead to better survival outcomes. This time, the poem of (Tcheonzamun 513th-528th) was studied for the work of translation. And the title of this work is ‘I am lucky, because I have three sisters and a brother at home! (Hyeongze Maneun Zibe Saneun Zaemi!)’ The present researcher translated three times on the different method. <Number in Tcheonzamun Chinese characters (Pronunciation of Korean language in English alphabet)> 513-516 世 (Se) 祿(Log) 侈(Tchi) 富(Bu). The husband, do you think that a lot of money (富) is necessary (侈) for the good behavior or the favorable act (祿) to your wife (世)? 517-520 車(Tcha) 駕(Ga) 肥(Bi) 輕(Gyeong). No, it is not so! A leghíresebb ókori gyógyító Aszklépiosz volt, akinek származásáról megoszlanak a vélemények, valamint arról is, hogy isten volt-e, vagy ember. Kultusza a Kr. e. 6. században alakult ki és terjedt el több területen és egészen az antikvitás végéig virágzott. Elsőként a Kr. e. 8. században Homérosz Iliászában (II, 731-32; IV, 193-194, XI, 517-518) olvashatunk róla, mint halandó hősről, aki Makháón és Podaleiriosz apja. (Edelstein – Edelstein, 1945: I,1-2.) A homéroszi Aszklépiosz himnuszból megtudhatjuk, hogy Trikka (ókori Thesszália) szülötte volt. A thesszáliaiak legalábbis így gondolták, a messzéniaiak szerint azonban inkább a messzéniai Trikkában, míg az árkádiaiak szerint Thelpuszában született. (Graves, 1970: I. 276.)

The connection between emotions and ethical views or ethical inquiry has been considered intimate by a number of philosophers. Based on Peirce’s discussion on the emotional interpretants in MS 318, I will suggest that such interpretants could be exploited in ethical inquiry. I will first argue, drawing on T. L. Short’s interpretation of Peirce, that there are final emotional interpretants, and such emotional interpretants actually formed (or dynamical) can be more or less appropriate concerning the sign’s (dynamical) objects. I will then explore the prospect that emotional interpretants could be harnessed for the particular cognitive purpose of ethical inquiry, concluding that normative judgments based on feelings could serve as its observational part.Includes: Comment. Abstract This article aims to highlight the importance trough space and time of the canonical legislation promoted by saint Caesarius, bishop of Arles, in Provence. It starts at the council of Agde, in 506, not convened by Caesarius but presided by him. The influence of that council should not be limited, as it is usual, to the five councils he directly gathered: Arles IV (524), Carpentras (527), Orange II (529), Vaison II (529) and Marseille (533). The first national council in the franc kingdom, Orléans I (511), as well as the first national council in the burgundian kingdom, Epaone (517), are largely tributaries of Agde. Moreover, the author comments more than 300 norms taken by something like 54 other councils, till the council of Trosly (909), which shows a clear filiation from Agde.

I conducted three interlocking studies to explore the concept of court culture and to develop a court culture scale. In study 1, I conducted in-depth interviews of legal professionals and clients (n=51) from the Indian states of Uttar Pradesh (UP) and Uttarakhand (UK) to identify the indicators of court culture. In study 2, I generated the items and refined them. In study 3, I surveyed legal professionals to assess measurement dimensionality and reliability (n=517). The scale indicated seven independent court culture dimensions: outcome orientation, manipulation, discipline, individualism & collectivism, work orientation, pride, and professionalism.

Salmonella entericasubsp.entericaserovar Stanley (S. Stanley) is a pathogen that contaminates food, and is related toSalmonellaoutbreaks in a variety of hosts such as humans and farm animals through products like dairy items and vegetables. Despite the fact that several vaccines ofSalmonellastrains had been constructed, none of them were developed according to serovar Stanley up to this day. This study presents results of genome sequencing and analysis on ourS. Stanley UPM 517 strain taken from fecal swabs of 21-day-old healthy commercial chickens in Perak, Malaysia and usedSalmonella entericasubsp.entericaserovar Typhimurium LT2 (S. Typhimurium LT2) as a reference to be compared with. First, sequencing and assembling of theSalmonellaStanley UPM 517 genome into a contiguous form were done. The work was then continued with scaffolding and gap filling. Annotation and alignment of the draft genome was performed withS. Typhimurium LT2. The other elements of virulence estimated in this study includedSalmonellapathogenicity islands, resistance genes, prophages, virulence factors, plasmid regions, restriction-modification sites and the CRISPR-Cas system. TheS. Stanley UPM 517 draft genome had a length of 4,736,817 bp with 4,730 coding sequence and 58 RNAs. It was discovered via genomic analysis on this strain that there were antimicrobial resistance properties toward a wide variety of antibiotics. Tcf and ste, the two fimbrial virulence clusters related with human and broiler intestinal colonizations which were not found inS. Typhimurium LT2, were atypically discovered in theS. Stanley UPM 517 genome. These clusters are involved in the intestinal colonization of human and broilers, respectively. There were sevenSalmonellapathogenicity islands (SPIs) within the draft genome, which contained the virulence factors associated withSalmonellainfection (except SPI-14). Five intact prophage regions, mostly comprising of the protein encoding Gifsy-1, Fels-1, RE-2010 and SEN34 prophages, were also encoded in the draft genome. Also identified were Type I–III restriction-modification sites and the CRISPR-Cas system of the Type I–E subtype. As this strain exhibited resistance toward numerous antibiotics, we distinguished several genes that had the potential for removal in the construction of a possible vaccine candidate to restrain and lessen the pervasiveness of salmonellosis and to function as an alternative to antibiotics.

Charles Kingsley's children's novelThe Water-Babies (WB), written in the spring and summer of 1862, is a politically anxious text. In this essay I argue that although The Water-Babies’ overall structure appears to be chaotic and arbitrary – J. M. I. Klaver, for example, deems the work a “Victorian fantasy crowded with Kingsley's hobby-horses” where he “pour[s] out whatever he had on his mind” (517) – in fact Kingsley's disquietude concerning the Irish Famine, U.S. slavery, and the condition of the British working classes provides a logical framework for the text.

Introduction: The aim of the study was to determine if the common bile duct (CBD) diameter increases physiologically with age and investigate whether gallstones lead to changes in CBD diameter. Method: Our study is composed of 721 patients in 2 groups: 517 asymptomatic patients and 204 gallstones patients who underwent MRCP. Their CBD diameter was measured at its widest visible portion on MRCP. Simple linear regression of the average of these measurements was used to investigate the relationship between age and CBD diameter in both populations. Subjects were further divided into 8 subgroups age-wise, respectively, and their means compared decade-wise using ANOVA. Furthermore, each subgroup mean from both populations was compared with each other using a t test. Results: Among the 517 subjects, the mean CBD diameter was 5.3663 mm ± 1.43546 and correlated with age (p < 0.01), dilating at 0.07 mm/year. The mean diameter of our oldest group was 7.9 mm, resulting in a plausible upper limit of 8 mm for the asymptomatic population. The mean CBD diameter of the cholelithiasis population was 5.6738 mm ± 1.40986 and also correlated with age (p < 0.01). The mean CBD diameter of the age groups from the cholelithiasis population was larger than that from the asymptomatic population, but none were statistically significant. Conclusion: Although the CBD increases physiologically with age, there is no significant difference in CBD size between the general population and the cholelithiasis patients at any particular age. Gallstones do not independently dilate the CBD.

Resumo:

Following in the footsteps of Ian Macpherson, I offer an interpretation of an anonymous invención found in the Cancionero general (11CG-517) and the British Library Cancionero (LB1-251). I maintain that the image displayed by the jouster was inspired by the iconography of the virgin-martyr Saint Margaret of Antioch, and I propose a theory about the identity of the lady addressed and, with less certainty, about the identity of the jouster and the occasion when the invención was displayed, using Pinar’s Juego trobado as a tool of research and invoking a passage on the sinfulness of the fashion among ladies and damsels for wearing hooped petticoats in a treatise by Hernando de Talavera.

Od uvođenja rashladnih sredstava HCFC baziranih na R22 traže se rešenja koja će biti trajnijeg karaktera. Prva zamena za HCFC – R22 bazirana na HFC R404A u Evropi je prevaziđena uvođenjem propisa EN 517/2014 (sa važnošću od 1.1.2015) kao naslednika prethodne direktive EN 862/2006. Servisna zabrana rada sa R404A i R507A od 2020. godine u Evropi će otežati serviserima potragu za budućim zamenama i obeležavanjima za šta se treba spremati već danas. Alternative koje su danas predložene za R404A (GWP 3922) jesu: R407A (2107), R407C (1774), R407F(1825), R410A (2088), R442A (1888), R449A (1397), HFO – R452A (2140) i radi poređenja R32 (675). U radu će biti odabrane najperspektivnije alternative i navedene uporedne termodinamičke karakteristike na koje inženjeri koji projektuju sisteme i serviseri u eksploataciji treba da obrate pažnju.

Gel filtration chromatography technique was used (Sepharose CL - 68) of CA 125 from malignant breast tumor homogenate and the isolation of (CA 125 I 1251 - anti CA 125 antibody), complex was prepared from crude CA 125 , isolated CA 125 and standard human CA 125. The results revealed the presence of two forms of CA 125 (Agl & AgII) and found to be 517 Kda and 38 Kda respectively; The isolation of CA 125 shov,:ed a purification fold of 17.5 Agl and 15 for AgII.

This study focused on whether people with different individualistic and collectivistic (I-C) tendencies speak different languages, belong to different religious groups and are of different gender. These three independent variables were examined using data from 517 college students in Lebanon: a multi-lingual society where both worldviews (I and C) co-exist. The Twenty Statement Test, Triandis' Attitude items, and ten of Schwartz's Value items were used to empirically test the I-C orientations and the above variables. Results indicated that language plays a primary role in individuals' orientations; respondents who used the Arabic language were consistently more collectivist than those who used either English or French. The discussion focused on the role of second language, specifically, English or French, which when learned by Arabic native speakers, enhanced the accessibility of private self-cognitions. Religion seemed to impact individuals' orientations in certain domains, but not consistently. Gender did not appear to be of significance in I-C orientation in this study.

Crigler-Najjar syndrome (CNS) was first described in 1952 in Maryland, USA as congenital familial non-hemolytic jaundice with kernicterus by Crigler JF and Najjar VA.1 CNS is a rare genetic disorder characterized by abnormalities in bilirubin metabolism and evident by persistent increase of unconjugated bilirubin. During the first days of life, the syndrome clinically manifests as intense unconjugated hyperbilirubinemia without evidence of hemolysis. It consists of two types, type I and type II. Crigler-Najjar Syndrome is mostly autosomal recessive disorder, but variation may occur in the inheritance of CNS II.2 The key pathogenesis is defect in bilirubin conjugation due to complete or partial deficiency of uridine 5'-diphosphate-glucuronosyl transferase (UGT). This enzyme is required for the conjugation and further excretion of bilirubin from the body. In type I CNS the enzyme activity is completely absent and in type II there is partial absence of the enzyme. Therefore, Type I is more severe form and usually fatal with kernicterus at the age of 1-2 years.3-5 TypeII is less severe and has better prognosis. Patients with CN type II suffer from less jaundice, less neurological impairment, and show a fair response to phenobarbitone therapy (serum bilirubin levels decrease by at least 25%).6 Both males and females are equally affected by CNS. I provvedimenti legislativi (L. 502/92 e L. 517/93) che hanno prodotto la cosiddetta aziendalizzazione della Sanità hanno anche modificato il suo sistema di finanziamento: da un finanziamento dei fattori produttivi, a pie' di lista, a un finanziamento a prestazione con un sistema tariffario.La Regione Lombardia, che ha particolarmente enfatizzato nel sistema sanitario riformato la separazione tra soggetto erogatore dei servizi (l'Azienda Ospedaliera) e il soggetto acquirente (l'ASL), per poter finanziare il servizio psichiatrico, erogato dall'Azienda Ospedaliera, ha esteso il sistema delle tariffe anche alla psichiatria, prevedendo attraverso atti regionali una puntuale valorizzazione di tutte le attività psichiatriche sia territoriali sia residenziali e semiresidenziali.È risultato subito evidente che questo sistema presenta delle criticità, in parte contingenti: in particolare i DRG non descrivono in maniera adeguata la complessità del caso, mentre le tariffe delle prestazioni ambulatoriali non coprono adeguatamente i costi.A fronte di queste criticità alia psichiatria si aprono due strade possibili: sottoporre a revisione DRG e tariffe oppure, in alternativa, remunerare i servizi sulla base dei bisogni di cura dei pazienti e della complessità dei trattamenti richiesti.

ABSTRACT A total of 395 Haemophilus influenzae strains from 226 Japanese institutions participating in the Nationwide Surveillance Study Group for Bacterial Meningitis were received from 1999 to 2002. All strains were analyzed by PCR to identify the resistance genes, and their susceptibilities to β-lactam agents were determined. Of these strains, 29.1% were β-lactamase nonproducing and ampicillin (AMP) susceptible (BLNAS) and lacked all resistance genes; 15.4% were β-lactamase producing and AMP resistant and had the bla TEM-1 gene; 30.6% were β-lactamase nonproducing and AMP resistant (low-BLNAR) and had a Lys-526 or His-517 amino acid substitution in ftsI encoding PBP 3; 13.9% were β-lactamase nonproducing and AMP resistant (BLNAR) and had an additional substitution of Thr-385 in ftsI; 9.1% were amoxicillin-clavulanic acid resistant (BLPACR I) and had the bla TEM-1 gene and a Lys-526 or His-517 amino acid substitution in ftsI; and 1.8% showed resistance similar to that of the BLPACR I group (BLPACR II) but had bla TEM-1 gene and ftsI substitutions, as was the case for the BLNAR strains. All but three strains were serotype b. The prevalence of BLNAR strains has increased rapidly: 0% in 1999, 5.8% in 2000, 14.1% in 2001, and 21.3% in 2002. The MICs at which 90% of BLNAR isolates were inhibited were as follows: AMP, 16 μg/ml; cefotaxime, 1 μg/ml; ceftriaxone, 0.25 μg/ml; and meropenem, 0.5 μg/ml. All of these values were higher than those for the BLNAS counterpart strains. The relatively wide distributions of the β-lactam MICs for BLNAR strains presumably reflect variations in ftsI gene mutations. Pulsed-field gel electrophoresis suggested the rapid spread of specific H. influenzae type b strains throughout Japan. Expedited vaccination, rapid identification, and judicious antibiotic use could slow their spread.

Cruise tourism is the fastest-growing branch of the tourism sector, and many have turned to it as a development strategy despite little systematic evidence of its equilibrium effects. I match 10.6 million automatic identification system (AIS) locations from 517 cruise ships arriving in 265 port destinations to 355,463 Demographic and Health Survey (DHS) women’s surveys in 23 countries to estimate cruise tourism’s relationship with women’s labor market participation and educational attainment. Using fixed effects to identify changes in tourism over time, I estimate that doubling cruise ship arrivals is associated with a 4.9-percentage point increase in labor participation and one-quarter more years of education. These results would be consistent with port cities offering more job opportunities for older women and increased opportunity and available income for education, possibly in anticipation of improved employment prospects. A simple, rapid, sensitive and suitable method for analysis of A methyldopa(I), levodopa (II) andcarbidopa (III) either in pure from or in pharmaceutical formulations is described. The proposed method is based on an oxidative coupling reaction with 3-methyl-2-benzothiazolinon hydra zone hydrochloride (MBTH) in the presence of an oxidant (Ce+4 or Fe+3) to give colored species. The resulting colors are well developed within 5 minutes at 20 ± 5°C andstable for at least 2 hours. The λmax were 517, 477 and 474 nm for I, II, both III and IV respectively. A linear correlation was found between the absorbance and the concentration. Beer's law was obeyed for concentration ranges from 1-7 mg/ml, 1- 11 mg/ml, 1-8 mg/ml and 2-24 mg/ml for I, II, III and IV, respectively. The molar absorptiveness and was calculated. The results of analysis of pure drugs by the proposed method are in good agreement with the official BP and USP methods. The results obtained showed recoveries of 99.80 ± 0.27, 100.40 ± 1.26, 99.89 ±0.21 and 99.96 ± 0.36 for I, II, III and IV, in order. The proposed method was successfully applied to determine the studied drugs in their pharmaceutical dosage forms.

Purpose: To analyse the strength of materials by means of optimization, find the best value of the strength test of mutually influential materials with a variation of roll-hoop height. Design/methodology/approach: The research began with the design of a threedimensional model by varying the height of the roll-hoop on chassis types: A, B, C, D, E, F, G, H ,and I. The height of the main roll hoop at each chassis is: 502, 504, 506 508, 510, 512, 514, 516 and 518 mm. Then by using the student version of Autodesk Inventor, a simulation is made to test: Deflection, Normal stress, Shear stress (T-x / T-y) and Torsional stress. The results of this test are used to analyse the types of chassis that have been designed so that the best chassis design is obtained. Findings: The results obtained in this study are the value of Normal stress decreases with increasing roll-hoop height, and applies inversely to the torsional stress value. Deflection values tend to be stable with increasing roll-hoop height, while Shear stress T-x and T-y values tend to fluctuate. Research limitations/implications: The chassis material uses carbon steel which has mechanical property values in accordance with 2015 FSAE Standard regulations. Practical implications: The optimization results of the design of the roll hoop height on the chassis show that the chassis type B with the main roll hoop height of 504 mm is the best with the lowest deflection value and the difference in tension according to the FSAE rules. Originality/value: The research that has been done only tests the strength of the ingredients separately. In this study trying to analyse the strength of the material by way of optimization to find the best value from the strength test of material that influence each other with a variation of roll-hoop height.

517 Background: The addition of immunotherapy (IO) to chemotherapy in triple negative breast cancer (TNBC) has improved survival outcomes and recent studies have also shown improvements in pCR rates in early high-risk HR+/HER2- breast cancer. However, these improvements are not without toxicities and thus there is a critical need for biomarkers to select patients that are most likely to benefit from immunotherapy. MHC-I and tumor-specific MHC-II (tsMHC-II) expression are candidate biomarkers for predicting PD-(L)1 checkpoint inhibition benefit and have previously shown potential for predicting treatment response in multiple tumor types, including breast cancer. However, the existing data were generated from clinical trials with limited racial/ethnic diversity. Here we report associations between MHC-I and MHC-II expression and clinicopathological features in a large, racially and ethnically diverse breast cancer cohort. Methods: We performed multiplexed immunofluorescent tissue-based analysis of participants in the Carolina Breast Cancer Study (CBCS). Intrinsic subtype and P53 mutant-like status were identified using RNA-based multigene assays. To evaluate MHC-I we ranked all participants based on tumoral MHC-I intensity, with the top 33% categorized as "MHC-IHigh"; MHC-II+ expression on tumor cells was defined as ≥5% of tumor cells. Association between MHC-I/II and clinicopathological features by race was tested using Chi-square test or fisher-exact test and relative frequency difference (RFD) was calculated. Results: This study included 1630 participants (48% Black and 52% non-Black) with Stage I-III breast cancer. Black participants had a higher incidence of TNBC (25% vs. 12.5%, p=<0.001) and Basal-like (29% vs. 14%, p=<0.001) tumors. Basal-like and TP53 mutant tumors were more frequently observed among Black participants with HR+/HER2- tumors (11% vs. 5.5%, p=0.006 for Basal-like; 26% vs. 17%, p=0.003 for p53 mutant-like). While there was a similar distribution of tsMHC-II+ tumors between Black and non-Black participants, the relative frequency of tsMHC-II+ tumors was significantly more common in Black participants with HR+/HER2- breast cancer compared to non-Black participants (7.9% vs. 5%, p=0.04). Moreover, Black participants also were significantly more likely to have MHC-Ihigh tumors (38.9% vs. 28.4%, p=0.01), which, after IHC subtype stratification, was only significant among participants with HR+/HER2- breast cancer (28.2% vs. 22.3%, p=0.02). Conclusions: In this diverse breast cancer cohort, MHC-I and MHC-II expression, candidate predictive biomarkers of IO benefit, were more prevalent in tumors from Black women with HR+/HER2- breast cancer. These results underscore the importance of diverse and equitable clinical trial enrollment in future IO trials, especially in HR+/HER2- breast cancer.

Tumor recurrence poses a significant challenge to the clinical management of stage I lung adenocarcinoma after curative surgical resection. Matrix metalloproteinases (MMPs) increase expression and correlate with recurrence and metastasis in surgically resected non-small cell lung cancer. However, the impact of MMPs on survival outcome varies, and their roles in patients with stage I lung adenocarcinoma remain unclear. In two discovery cohorts, we first analyzed 226 stage I–II lung adenocarcinoma cases in the GSE31210 cohort using a clustering-based method and identified a 150-gene MMP cluster with increased expression in tumors associated with worse survival outcomes. A similar analysis was performed on 517 lung adenocarcinoma cases in the Cancer Genome Atlas cohort. A 185-gene MMP cluster was identified, which also showed increased expression in tumors and correlated with poor survival outcomes. We further streamlined from the discovery cohorts a 36-gene MMP signature significantly associated with recurrence and worse overall survival in patients with stage I lung adenocarcinoma after surgical resection. After adjusting for covariates, the high MMP-gene signature expression remained an independent risk factor. In addition, the MMP-gene signature showed enrichment in epidermal growth factor receptor wild-type lung tumors, especially for those with Kirsten rat sarcoma virus mutations. Using an independent validation cohort, we further validated the MMP-gene signature in 70 stage I lung adenocarcinoma cases. In conclusion, MMP-gene signature is a potential predictive and prognostic biomarker to stratify patients with stage I lung adenocarcinoma into subgroups based on their risk of recurrence for aiding physicians in deciding the personalized adjuvant therapeutics.

Three simple, accurate and sensitive spectrophotometric methods (A, B and C) for the determination of cefuroxime and ceftazidime in bulk samples and in dosage forms are described. They are based on the reaction with nitrous acid forming a nitroso derivatives which can be measured at λmax 350 and 355 nm for cefuroxime (I) and ceftazidime (II), respectively (method A) or by oxidation of drug I or II with an excess of freshly prepared hypobromite and the residual hypobromite was treated with sodium fluorescein at the optimum experimental conditions and measured at λmax at 517 nm (method B). Method C is based on the formation of tris (0-phenanthroline) iron(II) complex (ferroin) upon the oxidation of the studied drug I or II with an iron (III)-o-phenanthroline mixture in acetate buffer solution of pH 3.6 and measuring at λmax 509 nm. Regression analysis of Beer-Lambert plots showed good correlation in the concentration ranges 0.2 – 6.0, 0.2 – 3.2 and 0.1 – 5.6 μg ml−1 for methods A, B and C, respectively. The apparent molar absorptivity, Sandell sensitivity, detection and quantitation limits were calculated. For more accurate results, Ringbom optimum concentration range was 0.2 – 5.6 μg ml−1. The validity of the proposed methods was tested by analysing dosage forms containing the studied drugs I and II. The relative standard deviations were ≤ 1.25% with recoveries 98.6 – 101.4% .

ABSTRACT The affinity of [3H]benzylpenicillin for penicillin-binding protein (PBP) 3A was reduced in 25 clinical isolates of β-lactamase-negative ampicillin (AMP)-resistant (BLNAR)Haemophilus influenzae for which the AMP MIC was ≥1.0 μg/ml. The affinities of PBP 3B and PBP 4 were also reduced in some strains. The sequences of the ftsI gene encoding the transpeptidase domain of PBP 3A and/or PBP 3B and of thedacB gene encoding PBP 4 were determined for these strains and compared to those of AMP-susceptible Rd strains. The BLNAR strains were classified into three groups on the basis of deduced amino acid substitutions in the ftsI gene, which is thought to be involved in septal peptidoglycan synthesis. His-517, near the conserved Lys-Thr-Gly (KTG) motif, was substituted for Arg-517 in group I strains (n = 9), and Lys-526 was substituted for Asn-526 in group II strains (n = 12). In group III strains (n = 4), three residues (Met-377, Ser-385, and Leu-389), positioned near the conserved Ser-Ser-Asn (SSN) motif, were replaced with Ile, Thr, and Phe, respectively, in addition to the replacement with Lys-526. The MICs of cephem antibiotics with relatively high affinities for PBP 3A and PBP 3B were higher than those of AMP and meropenem for group III strains. The MICs of β-lactams forH. influenzae transformants into which the ftsIgene from BLNAR strains was introduced were as high as those for the donors, and PBP 3A and PBP 3B showed decreased affinities for β-lactams. There was no clear relationship between 7-bp deletions in the dacB gene and AMP susceptibility. Even though mutations in another gene(s) may be involved in β-lactam resistance, these data indicate that mutations in the ftsI gene are the most important for development of resistance to β-lactams in BLNAR strains.

ObjectivesTo describe the safety of vaccines against SARS-CoV-2 in people with inflammatory/autoimmune rheumatic and musculoskeletal disease (I-RMD).MethodsPhysician-reported registry of I-RMD and non-inflammatory RMD (NI-RMDs) patients vaccinated against SARS-CoV-2. From 5 February 2021 to 27 July 2021, we collected data on demographics, vaccination, RMD diagnosis, disease activity, immunomodulatory/immunosuppressive treatments, flares, adverse events (AEs) and SARS-CoV-2 breakthrough infections. Data were analysed descriptively.ResultsThe study included 5121 participants from 30 countries, 90% with I-RMDs (n=4604, 68% female, mean age 60.5 years) and 10% with NI-RMDs (n=517, 77% female, mean age 71.4). Inflammatory joint diseases (58%), connective tissue diseases (18%) and vasculitis (12%) were the most frequent diagnostic groups; 54% received conventional synthetic disease-modifying antirheumatic drugs (DMARDs), 42% biological DMARDs and 35% immunosuppressants. Most patients received the Pfizer/BioNTech vaccine (70%), 17% AstraZeneca/Oxford and 8% Moderna. In fully vaccinated cases, breakthrough infections were reported in 0.7% of I-RMD patients and 1.1% of NI-RMD patients. I-RMD flares were reported in 4.4% of cases (0.6% severe), 1.5% resulting in medication changes. AEs were reported in 37% of cases (37% I-RMD, 40% NI-RMD), serious AEs in 0.5% (0.4% I-RMD, 1.9% NI-RMD).ConclusionThe safety profiles of SARS-CoV-2 vaccines in patients with I-RMD was reassuring and comparable with patients with NI-RMDs. The majority of patients tolerated their vaccination well with rare reports of I-RMD flare and very rare reports of serious AEs. These findings should provide reassurance to rheumatologists and vaccine recipients and promote confidence in SARS-CoV-2 vaccine safety in I-RMD patients.

The Noctiluca scintillans (Macartney) Kofoid & Swezy a dinoflagellate of marine environment can form bloom and generates substances that are noxious to aquatic organisms. An intense bloom of the N. scintillans in Gadani, Baluchistan coast along Northern Arabian Sea is reported in the present study. Sea water was collected using Niskin water sampler from three locations for water quality and nutrient analysis. Maximum number (6780 cells/l) of phytoplankton cells were observed in Station-I than that of Station-II (4180 cells/l) and in Station III (3580 cells/l). Maximum number (2740 cells/l) of Noctiluca was observed in station I than station II (1020 cells/l) and minimum (620 cells/l) in Station III. Among toxic species of dinoflagellates, Alexandrium tamaresence, Ceratium fusus, Dinophysis caudata, Gyrodinium spirale, Prorocentrum micans, Prorocentrum arcuratum and Scrippsiella trochoidea were observed. The Noctiluca scintillans can transfer toxins to higher trophic levels by grazing these toxic dinoflagellates. The increase in intensity of algal blooms occurring in marine waters cause detrimental effects including bio toxins, physical damage and the anoxia which caused massive fish mortality, affect fishery resources, biodiversity and vulnerable to the aquatic ecosystem. Decline types of passenger turnover of 517 airports worldwide in 2020 compared to 2019, caused by COVID-19, are analyzed. This decay was uneven in space, as well as in individual countries, areas, poles, centers and cores. The strongest decline in 2020 occurred at airports of Europe (especially in the UK, Spain, Germany, and Italy) and Asia (particularly in India and Malaysia), medium – in the countries of America (the USA, Mexico, Brazil) and Norway, moderate – in Russia, weak – in China. 48% of all airports covered by the study showed a strong decline in passenger turnover (from 65 to 85%), 32% – medium (45 to 65%), 10% – moderate (30 to 45%). A catastrophic decline (over 85%) was recorded at 14 airports (2.7%), weak (15-30%) – in 26 (5%), insignificant (3-15%) – in 9 (1.7%). Stagnation (from -3% to + 3%) was noted only in 1 airport, and growth (more than + 3%) – in 3. The largest international airports with a high proportion of international transit passengers and airports with a narrow resort and tourist specialization were most affected by the covid crisis; less – airports with a dominance of domestic traffic. The pandemic has deformed the persistent territorial structure of world air traffic, which has formed over the past 60 years, and also violated the already established hierarchy of the largest and large airports not only around the world, but also within individual mega-regions and countries with big territories and populations. Zróżnicowanie geograficzne spadku ruchu pasażerskiego na lotniskach świata w 2020 r. spowodowane pandemią COVID-19 W artykule poddano analizie rodzaje spadków ruchu pasażerów na lotniskach spowodowanych skutkami pandemii COVID-19. Badanie przeprowadzono dla 517 portów lotniczych świata porównując zmiany jakie zaszły 2020 r. w stosunku do poprzedniego 2019 r. Obserwowane było niezwykle nierównomierne rozmieszczenie tego zjawiska w przestrzeni, a także w skali poszczególnych krajów, obszarów, biegunów, centrów i rdzeni. Największy spadek liczby obsługiwanych pasażerów wystąpił w 2020 r. na lotniskach Europy (szczególnie w Wielkiej Brytanii, Hiszpanii, Niemczech i Włoszech) oraz Azji (szczególnie 19 w Indiach i Malezji). Średnie nasilenie tego procesu wystąpiło w krajach obu Ameryk (USA, Meksyk, Brazylia) oraz w Norwegii. Natomiast w Rosji spadek liczby obsługiwanych pasażerów miał charakter umiarkowany, a w przypadku Chin wręcz słaby. 48% wszystkich portów lotniczych objętych badaniem było dotkniętych silnymi spadkami (od 65 do 85%), 32% – średnimi (od 45 do 65%), 10% – umiarkowanymi (30 do 45%). Katastrofalne zmniejszenie się ruchu pasażerów (ponad 85%) odnotowano na 14 lotniskach (2,7%), słabe (15-30%) – w 26 (5%), nieznaczny (3-15%) – w 9 (1,7%). Brak zmiany (od -3% do +3%) miał miejsce tylko na jednym lotnisku, a wzrost (ponad +3%) – w trzech innych portach lotniczych. Największe międzynarodowe porty lotnicze z wysokim odsetkiem pasażerów w tranzycie międzynarodowym oraz porty lotnicze o wąskiej specjalizacji uzdrowiskowej i turystycznej najbardziej ucierpiały w wyniku kryzysu pandemicznego – mniej natomiast lotniska z przewagą ruchu krajowego. Pandemia zdeformowała stabilną strukturę terytorialną światowego ruchu lotniczego, która ukształtowała się w ciągu ostatnich 60 lat, a także naruszyła ustanowioną już hierarchię największych i większych lotnisk nie tylko na całym świecie, ale także w obrębie poszczególnych megaregionów i krajów z dużymi terytoriami i populacjami.

U povijesnim izvorima postoji anegdota o satu orguljaške improvizacije koji je 1880- tih godina mladi Claude Debussy (1862. – 1918.) pohađao kod tada već renomiranoga skladatelja i orguljaša Césara Francka (1822. – 1890.). Prema svjedočenjima, Franck je mladomu i samouvjerenomu studentu uporno ponavljao (cit.): „Moduliraj! Moduliraj! Moduliraj!ˮ, što je Debussy uporno odbijao jer se (parafr.) „osjećao najbolje upravo u polaznome tonalitetuˮ. Naravno, među pripadnicima dviju različitih generacija došlo je do izvjesnoga nesporazuma, no postavlja se pitanje je li on bio stilsko-estetske ili samo terminološke prirode? 526 Rasprave 44/2 (2018.) str. 517–526 U današnje vrijeme pojam modulacija nedvosmisleno se izjednačava s promjenom tonaliteta. Međutim, u francuskoj teorijskoj literaturi 19. stoljeća (kao i u tadašnjoj europskoj literaturi općenito) taj je termin prošao značajan razvojni put, pri čemu se (u skladu s ranijim diskusijama iz vremena prosvjetiteljstva) najprije najčešće sagledavao znatno šire, kao način tonalnoga ostvarivanja (npr. u knjigama Fétisa i de Momignyja ), prije nego što je počeo dobivati suvremene paradigmatske pretpostavke (npr. u „Udžbeniku kompozicijeˮ Antona Reicha). Međutim, čak i kad je modulacija postala sinonim za promjenu tonalnoga centra, pojedini teoretičari mlađe generacije nastavili su insistirati na tome da ona može imati i šire značenje („Traktat o harmonijiˮ Napoleona Henrija Rebera). U ovom je radu predočeno na koji se način odvijao paradigmatski razvoj pojma modulacija te kako različite suvremene kategorizacije promjene tonaliteta zapravo vuku korijene iz ranijih značenja toga pojma. Stoga se, između ostaloga, pokušalo zaključiti i je li nesporazum s početka teksta odražavao estetski ili puki teorijsko-terminološki procijep u razumijevanju između kompozitora školovanoga u prvoj polovici 19. stoljeća nasuprot njegovu kolegi nekoliko stoljeća poslije.

La capacidad en tecnologías de información (TI) se reconoce como un conjunto de recursos en TI y su sinergia, que podrán potenciar a las organizaciones en el desarrollo de sus capacidades organizacionales y en la consecución de ventajas competitivas sostenibles. Múltiples definiciones de este constructo han sido propuestas; sin embargo, las condiciones particulares de las micros, pequeñas y medianas empresas (mipymes) hacen necesario proponer y validar un modelo que permita evaluar la capacidad en TI en este tipo de contextos. A partir de un análisis factorial exploratorio y confirmatorio, sobre una muestra de 517 mipymes, se valida un modelo de capacidad en TI. El modelo resultante describe la capacidad en TI en cuatro categorías principales: capacidad de cohesión organizacional del área de TI (categoría I), capacidad estratégica en TI (categoría II), infraestructura de TI (categoría III) y capacidad de relacionamiento externo (categoría IV), siendo las categorías I y II las que tienen una influencia más significativa sobre la capacidad de TI. La categoría I está representada por el conocimiento organizacional del personal de TI y su capacidad de relacionamiento al interior de la organización. La categoría II está descrita por el conocimiento estratégico de las TI y su capacidad de planeación estratégica en TI. Este modelo es una base para la medición de la capacidad en ti de las mipymes, así como para la generación de políticas públicas.

The effect of a 6-h intra-arterial infusion of recombinant human (rh) insulin-like growth factor I (IGF-I) on forearm muscle metabolism was studied in 19 postabsorptive subjects. Forearm glucose, lactate, and phenylalanine (Phe) balances, as well as estimates of protein degradation (Phe Ra) and synthesis (Phe Rd) were measured before and at 3 and 6 h into an infusion of rhIGF-I at a dose of 1.8 (n = 6), 6.0 (n = 8), or 10.0 (n = 5) micrograms.kg-1.h-1. In response to intra-arterial IGF-I, deep venous IGF-I rose by 55, 141, and 315%, respectively (all P < 0.01), and forearm blood flow accelerated by 75 (1.8 microgram), 213 (6.0 micrograms), and 159% (10.0 micrograms; all P < 0.02). No change in forearm glucose uptake was observed at the lowest dose, whereas four- to sixfold increases were observed at both the 6 and 10 micrograms.kg-1.h-1 doses (both P < 0.02). Forearm Phe balance shifted positively at all three doses by 27 +/- 6, 48 +/- 7, and 51 +/- 9 nmol.min-1 x 100 ml-1, respectively (all P < 0.01). At all three doses, Phe Rd increased comparably by 49-74% (all P < 0.05). At the 6.0 and 10.0 but not the 1.8 microgram.kg-1.h-1 dose, Phe Ra decreased by approximately 45% (P < 0.02). Forearm muscle metabolism was also studied in the contralateral non-IGF-infused arm at these three doses. Despite increases in deep venous IGF-I up to 517 ng/ml due to recirculating IGF-I (10.0 micrograms.kg-1.h-1 dose), contralateral forearm muscle glucose, lactate, or Phe handling did not change. In conclusion, intra-arterial IGF-I exhibits growth hormone-like effects at all doses tested, whereas the insulin-like effects are observed at higher doses; these effects appear dependent on the route of administration.

The article presents new data on ages (U-Pb zircon dating, SIMS SHRIMP-II) and chemical compositions of rocks from gabbro-granitic and granite-leucogranitic magmatic associations. These rocks preceded the formation of Li-enriched spodumene pegmatites of the Tserigiyngol-Burchin ore cluster (Russian: ЦБРУ), one of the main clusters in the South Sangilen pegmatite belt (SSB) located in the Tuva-Mongolian massif being a part of the Central Asian Fold Belt. We investigated the rocks from the Upper Tserigiyngol, Uchuglyk and Temenchulu plutons, and pegmatites from two neighbouring fields. We distinguish three impulses of granitic magmatism (517±7, 508±7, and 488±6 Ma), which are attributed to different stages of the Early Paleozoic collision orogeny (520-480 Ma). The period when the Li-enriched pegmatites were formed (494±7 Ma) is close to the magmatism impulse at 488±6 Ma. Differences are discovered in compositional and isotopic (Sm-Nd) features of granites dominating at the following stages of collisional orogeny: (1) early collision (517±7 Ma) – I-type granites, eNd(T)=0–1.5, TNd (DM-2st)=1.2–1.1 b.y., and (2) late collision (488±6 Ma) – A-2-type granites, eNd(T)=–3.0…–1.6, TNd (DM-2st)=1.5–1.4 b.y., which are due to different sources. Our study shows that facies transitions are absent between the late-collision granites (488±6 Ma) and the spodumene pegmatites from the Tserigiyngol-Burchin ore cluster (494±7 Ma), although these rocks are close in age. In terms of geochemical features, the spodumene pegmatites from the cluster are strongly different from both the late-collision granites and spodumene pegmatites from other SSB fields, including the large Tastyg lithium deposit. We have analysed the role of interactions between the crustal and mantle materials in the formation of granitoid sources in the Tserigiyngol-Burchin ore cluster, and described their evolution in time and the influence on the pegmatite rare-element specialization.
