De acordo com o Regulamento Geral dos Hospitais de El Salvador, o Hospital Infantil Benjamin Bloom atende crianças de até 12 anos de idade, enquanto o Hospital Nacional Rosales atende usuários a partir de 12 anos. Assim, quando um paciente menor chega aos 12 anos, ele deve ser transferido do hospital infantil para o hospital adulto. Nesse sentido, as especialidades que geram as transferências são: Nefrologia e Hemato Oncologia. É por isso que, diante do cenário acima estabelecido, surge a seguinte pergunta: Como melhorar e agilizar a comunicação e a colaboração entre as especialidades nefrologia e Hemato-Oncologia do Hospital Infantil Benjamin Bloom e do Hospital Nacional Rosales com base nas transferências de adolescentes com doenças crônicas? Dada a realidade descrita acima, propõe-se incorporar uma estratégia de telemedicina no fluxograma de transferência de pacientes do Hospital Infantil Benjamin Bloom e do Hospital Nacional de Rosales, uma vez que ela tem se mostrado uma ferramenta de comunicação eficiente e eficaz nos processos de saúde, especialmente nos contextos da pandemia COVID-19. Nesse sentido, os beneficiários diretos do projeto serão os membros da Equipe de Atenção Integral ao Adolescente do Hospital Nacional de Rosales e do Hospital Infantil Benjamin Bloom, que serão treinados no uso e administração de plataformas de comunicação síncronas com o objetivo de melhorar o processo de transferência de pacientes adolescentes com doenças crônicas.

Introducción. A partir del sufrimiento por dolor de los pacientes con cáncer, se elaboró el manual “Alivio del dolor en el cáncer” de la OMS que propuso 5 recomendaciones para la analgesia. Se han hecho modificaciones del planteamiento inicial para adaptarse al manejo del dolor agudo el cual va en sentido inverso: de más severo a más leve, pero los principios fundamentales se mantienen. Objetivo. Conocer la adherencia a los principios de la escala analgésica de la OMS en el manejo del dolor postquirúrgico en cirugía electiva en el Hospital Nacional Rosales en el periodo iniciando en abril 2018. Diseño y métodos. Se utilizó un diseño observacional, descriptivo, longitudinal, de concordancia de la indicación analgesia en pacientes de cirugía mayor electiva del Hospital Nacional Rosales desde el 1ero de agosto de 2018. Se compararon las indicaciones del centro estudiado contra lo ideal según los principios propuestos. Resultados. De 360 pacientes se encontró que la modalidad quirúrgica electiva más utilizada fue ingreso hospitalario 75.3%. 215 (59.7%) pacientes recibieron anestesia general. El índice de concordancia kappa para la adherencia a los principios analgésicos para el manejo del dolor post quirúrgico de la OMS en el Hospital Nacional Rosales fue pobre en los cuatro momentos medidos, con una tasa de adherencia del 0%, y no uso de escala analgésica para evaluar el dolor. Conclusión. En el Hospital Nacional Rosales no se están siguiendo los principios analgésicos de la OMS en el manejo del dolor post quirúrgico.

AbstractWe describe circumstances at the Hospital Rosales, located in San Salvador, El Salvador, and some salient observations from an infection control program begun in 1978. Findings include overuse of antibiotics, especially of penicillin and chloramphenicol; a predominance of gram-negative rod infections, especially Pseudomonas aeruginosa; a relative infrequency of Staphylococcus aureus infections; an apparent doubling of the mean duration of hospitalization for patients with nosocomial infections compared with other patients (22.1 days versus 11.0 days); documentation and partial correction of deficiencies in aseptic and antiseptic practices; an outbreak of Pseudomonas aeruginosa endophthalmitis traced to the hospital's factory for the manufacturing of intravenous fluids; and attitudinal problems such as the care of patients with rabies on open wards. Prevalence surveys conducted during 1981 and 1986 suggest a dramatic increase in the recent incidence of surgical wound infection (44% v 28%, P < 0.001). This latter observation suggests a direct relationship between infection rates and the hardships imposed by poverty and civil war.

Objetivo. Determinar el grado de similitud entre las bases de datos de infecciones asociadas a atención sanitaria diagnosticadas en hospitales de tercer nivel del Ministerio de Salud. Metodología. Observacional, descriptivo, comparando retrospectivamente las bases de datos digitales y formularios físicos de recolección de información de las infecciones asociadas a atención sanitaria de los hospitales de tercer nivel, durante 2015. La similitud entre las bases se especificó en porcentajes. Resultados. El Hospital Nacional de Niños Benjamín Bloom y el Hospital Nacional Rosales no elaboraron formularios para Notificación Individual de Infecciones Asociadas a Atención Sanitaria durante 2015. Lo anterior podría deberse a que los clínicos responsables del llenado del Formulario para Notificación Individual de Infecciones Asociadas a Atención Sanitaria, como los clínicos responsables de registrarla en los Formularios de Ingreso/Egreso, no ejecutan la obligatoriedad de reportar toda infección asociada a atención sanitaria detectada. El Hospital Nacional de la Mujer, a través del Formulario para Notificación Individual de Infecciones Asociadas a Atención Sanitaria, reportó en el Sistema Nacional de Vigilancia Epidemiológica cerca del 80% de las infecciones asociadas a atención sanitaria reportadas por su laboratorio durante 2015. Conclusiones. La similitud entre bases digitales de datos de infecciones asociadas a la atención sanitaria, así como entre estas y los formularios que las alimentan, no es satisfactoria en el Hospital Nacional de Niños Benjamín Bloom y Hospital Nacional Rosales. En el Hospital Nacional de la Mujer es satisfactoria, exceptuando el traslado de datos de los Formularios de Ingreso/Egreso, al Sistema de Morbimortalidad en Línea.Alerta Año 2018, Vol. 1 No. 1: 53-60

Introducción. El glaucoma de ángulo cerrado es una causa importante de ceguera que incrementa con la edad y que afecta principalmente a las mujeres. Este problema es la primera causa de discapacidad visual no reversible en El Salvador.Objetivo. Caracterizar clínica y morfológicamente el ángulo iridocorneal y cristalino en glaucoma por ángulo cerrado en pacientes atendidos entre mayo y julio de 2018 en el Hospital Nacional Rosales. Metodología. Estudio transversal descriptivo que incluyó a pacientes diagnosticados por primera vez con glaucoma en el Hospital Nacional Rosales, entre mayo y julio de 2018. En total, se incluyeron a 22 pacientes. Resultados. El 77.0% son mujeres entre 51 y 60 años. La agudeza visual se encontró reducida en más del 90.0%. La mayoría de personas presenta una presión intraocular de 30 mmHg. La profundidad de la cámara anterior en la mayoría de los pacientes se encuentra disminuida. La mayoría tienen ángulo cerrado con aumento del diámetro y del espesor del cristalino. Conclusión. El glaucoma de ángulo cerrado fue más frecuente en mujeres, de mayor rango de edad y con cambios característicos en la morfología de la cámara anterior del ojo e incremento de la presión intraocular.

Introducción. La diabetes mellitus y el evento cerebrovascular son enfermedades relacionadas al estilo de vida y asociadas entre sí, de interés en salud pública e investigación clínica. Objetivo. Se realiza una descripción de las características epidemiológicas, perfil clínico, complicaciones y mortalidad de pacientes ingresados en el Hospital Nacional Rosales desde el 1 de enero hasta el 31 de diciembre del 2017, con los diagnósticos de evento cerebrovascular isquémico y diabetes mellitus. Metodología. El presente estudio es de tipo descriptivo, observacional, retrospectivo. A partir de la revisión de 24 expedientes clínicos, obteniendo variables y aplicando análisis de frecuencias y asociación de variables. Resultados. Se identificó que la edad arriba de 50 años, los antecedentes de hipertensión arterial, las lipoproteínas de alta densidad debajo de valores de referencia, hipertensión y más de 10 años de evolución de diabetes mellitus fueron los factores asociados a ictus severo y con mayores complicaciones, incluyendo mortalidad. Conclusiones. La diabetes mellitus y otros factores de riesgo cardiovascular, se asocian a mayor morbimortalidad en pacientes con evento cerebrovascular isquémico.

El síndrome de Eisenmenger es la forma más severa de presentación de hipertensión arterial pulmonar secundaria a defectos cardíacos congénitos no reparados, aunque su prevalencia es baja, continúa siendo un reto para los sistemas de salud de los países en vías de desarrollo por su complejidad en el manejo. Presentación del caso. Paciente femenina sin antecedentes médicos conocidos quien consulta por disnea relacionada a los esfuerzos y policitemia. Intervención terapéutica. Se realiza ecocardiograma transesofágico que arroja la presencia de defecto interatrial tipo ostium secundum e hipertensión arterial pulmonar severa, con cortocircuito de derecha a izquierda, se inicia oxigenoterapia y terapia farmacológica. Evolución clínica. Paciente permaneció ingresada presentando notable mejora a la disnea, se le dio de alta con referencia a la clínica de cardiopatías congénitas del adulto en Hospital Nacional Rosales.

Se realizo un estudio de corte transversal para estudiar la presencia del síndrome de Burnout en el Hospital Escuela Oscar Danilo Rosales Arguello (HEODRA) de la ciudad de León, Nicaragua, en el año 2007. La población de estudio estuvo constituida por un total de 65 médicos y 100 del personal de enfermería.Se presentaron el doble de casos en el personal de enfermería que en los médicos. La dinámica de trabajo fue el factor mas percibido como desfavorable. La relación antigüedad laboral-casos tuvo comportamiento inverso, entre el personal de enfermería, a mas años labores mas casos, y lo contrario entre los médicos. La mayor parte de casos son jóvenes y se encuentran casados. Los casos predominantes en el personal de enfermería fueron del sexo femenino, en los médicos fue el masculino.Se recomienda buscar la equidad en la distribución de los horarios y trato interpersonal. Promover y mantener en forma permanente sistemas de evaluación del personal, y crear grupos terapéuticos de contención emocional dirigidos a todos el personal de trabajo, con el fin de promover mayor solidaridad, diálogo y respeto por las opiniones de los otros, mejorando los canales de comunicación entre los compañeros de trabajo.DOI: http://dx.doi.org/10.5377/universitas.v2i2.1653

Introducción: La Sepsis es una urgencia médica que si no se controla a tiempo puede evolucionar aShock Séptico, donde las alteraciones circulatorias y metabólicas aumentan el riesgo de muerte enquienes la padecen. Objetivo: Promover distintas iniciativas contra la Sepsis a nivel hospitalario,nacional y de la región, así impactar no solo a la comunidad médica sino también a la población engeneral. Metodología: El evento fue completamente gratuito, desarrollado el 13 de septiembre del 2021,con una duración de 120 minutos, fue realizado en el Hospital El Salvador en modalidad presencial ytransmitido en vivo. El evento científico duró aproximadamente 180 minutos. Resultados: Losexpositores fueron médicos que trabajan en distintas instituciones de El Salvador, y se contó con laintervención internacional. La transmisión en vivo fue a través de Facebook Live de Proyecto Sepsis ElSalvador, usando la plataforma StreamYard, logrando una interacción en tiempo real entre los panelistasy los asistentes presenciales y virtuales de El Salvador y otros países de Latinoamérica. La inscripciónse realizó a través de un formulario electrónico. El evento contó con el respaldo de representantes delHospital El Salvador, Hospital Nacional Rosales, Hospital San Rafael, Instituto Seguro Social y LaAcademia de Educación Superior Latinoamérica ACESLatinoamérica. Conclusiones: La unión dedistintos actores a nivel hospitalario, nacional en El Salvador e internacional para realizar formacióncontinua, es una estrategia efectiva en la lucha contra la Sepsis

La intubación submentoniana (ISM) es útil en procedimientos quirúrgicos en donde la intubación nasotraqueal está contraindicada y la intubación orotraqueal no es ideal, debido a la fijación intermaxilar. El cirujano maxilofacial Hernández Altemir (España) propuso una técnica de ISM que implicaba pasar el tubo endotraqueal a través de una incisión extraoral en la región submentoniana del suelo de la boca, para realizar reducción abierta de fracturas faciales. Este informe describe dos pacientes masculinos que se presentaron al Hospital Nacional Rosales, en el servicio de Cirugía Maxilofacial de El Salvador con historia sufrir accidente de tránsito. El primer caso, el examen radiológico evidenció fractura de sínfisis mandibular, fractura simple huesos propios nasales, y desviación del tabique nasal. El segundo caso es un paciente con fractura sínfisis y doble cóndilo mandibular. La ISM resulta una alternativa a la traqueostomía, ya que se realiza en menor tiempo, menor morbimortalidad, mínimos cuidados posoperatorios, y con una cicatriz estética.

La falta de estudios publicados en nuestro país en relación al tema, hace muy importante determinar la prevalencia de Candidiasis Oral, localizaciones más frecuentes y aparición de lesiones según tiempo de diagnóstico de VIH, en Pacientes atendidos en la Clínica de Infectología del Hospital Escuela Oscar Danilo Rosales Arguello de la ciudad de León. Posterior a aprobación por el comité de Ética Médica, UNAN León, se realizó estudio descriptivo de corte transversal, se recolectó información de fuente secundaria, con una muestra de 150 expedientes clínicos de pacientes, los datos de interés incluyeron características socio-demográficas del paciente, presencia, localización y clasificación de Candidiasis Oral. Se encontró prevalencia de candidiasis oral de 50.6%, siendo la presentación clínica más frecuente Candidiasis Pseudomembranosa, en 61.8%; la distribución de la enfermedad fue similar en ambos sexos; de los estudiados con candidiasis oral, el 61.8% presentaron al menos un episodio de enfermedad, siendo mayor en el grupo de 1-5 años de diagnóstico por VIH. La localización más frecuente de las lesiones fue en lengua, seguido de orofaringe. Se concluye que la prevalencia de candidiasis oral para este estudio superó 50%, lo que reafirma lo expresado en la bibliografía como una enfermedad muy frecuente en pacientes con VIH+. ABSTRACT Background Although, selective nonoperative management of penetrating pelvic injuries is a feasible, safe and effective alternative; lack of advanced imaging studies in developing countries to assess these patients, force us to perform more invasive procedures to treat them. Study design Cases analysis performing with patients with penetrating pelvic trauma underwent to laparotomy in General Hospital of Juarez City (Mexico), during January to December 2012. Results Among 44 patients with penetrating pelvic injuries and underwent laparotomy were included. There were 27 therapeutic laparotomies, 9 nontherapeutic laparotomies, 8 damage control laparotomies. Small bowel was the most affected organ, follow by both colon and rectum organ. Hypovolemic shock was the major cause of death and surgical site infections were the most common complications. Conclusion Despite, that nonoperative management is safe, penetrating wounds with transpelvic trajectory continue having an absolute indication for laparotomy in medical units in developing world. Introduction: Chagas disease is the most important parasitic disease in El Salvador and Latin America. The disease has two phases: acute and chronic, with an intermediate unspecified phase. The chronic phase develops in 30% of infected persons and its most common manifestation is cardiac disease. This study aimed to obtain reference data for the prevalence of T. cruzi seropositivity in a cohort of cardiac patients. Methodology: A cross-sectional study involving consecutive heart disease patients consulting at the National Hospital Rosales was conducted over a six-month period. Congenital heart disease patients were excluded. A survey, file review, and ELISA serological test were conducted for each subject. Results: Out of 455 subjects, 76 were seropositive for T. cruzi, giving a seropositivity prevalence of 16.7%. The average age of the patients was 58.09 years and the female:male ratio was 2.79:1. No significant difference was found between T. cruzi seropositive and seronegative patients in terms of age and gender. No association between T. cruzi serological status and either area of residence or seeing vectors in the house was found. However, living in the Salvadoran western region during childhood was significantly associated with seropositivity (p = 0.003). Other factors associated with T. cruzi seropositivity included: clinical diagnosis of cardiac Chagas disease; electrocardiographic (ECG) findings of a permanent pacemaker, all atrioventricular (AV) blocks and right bundle branch block; and grade III-IV radiological cardiomegaly. Conclusions: T. cruzi seropositivity prevalence was found to be 16.7% among heart disease patients in a public reference hospital.

La principal función fisiológica del sueño es mejorar el rendimiento físico y mental, esto ocurre si el sueño es de calidad y depende de diversos factores, El conocimiento de su calidad y factores que influyen en él y posibles trastornos, permitirán tener una base para buscar alternativas de prevención. El objetivo de esta investigación fue describir los factores que determinan un sueño de calidad en médicos residentes del Hospital Escuela Oscar Danilo Rosales. Fue descriptiva de corte trasversal realizada al 60% de médicos residentes de las especialidades. Se utilizó como instrumento de medición el índice de Calidad del Sueño de Pittsburg, proporciona una puntuación global de calidad y puntuaciones parciales en 7 componentes. Los factores incidentes en un sueño de calidad son laborales y el consumo de café, en todas las especialidades. Los principales trastornos del sueño que presentan los médicos residentes son: Insomnio de conciliación, insomnio de mantenimiento y sueño no reparador, dificultando la capacidad de concentración en la realización de las actividades asignadas. Se llegó a la conclusión que la calidad del sueño de los médicos residentes encuestados tiene un puntaje mayor a 5 puntos, considerado MALA CALIDAD DEL SUEÑO, la especialidad con mayor puntaje es Medicina interna.DOI: http://dx.doi.org/10.5377/universitas.v2i2.1648

Introduction. In El Salvador, encephalopathy is one of the clinical conditions with the greatest uncertainty when assessing the prognosis of a patient in the intensive care unit. The electroencephalogram offers a low cost, practical, non-invasive and highly available method through which an evaluation of the neurological status of the patient is achieved. Objective. Study was carried out with the aim of correlating electroencephalographic patterns and clinical evolution of patients with encephalopathy admitted to the medical and surgical intensive care unit during the months of September to December 2018, in National Hospital Rosales. Methodology. An observational, descriptive, longitudinal, prospective. Results. Patients with a diagnosis of encephalopathy were included and chronicled clinical and electroencephalographic variables,age, sex, comorbidities, cause of admission to the intensive care unit, medical or surgical, use of sedative or relaxing drugs, use of vasoactive amines, state of consciousness, complications during intrahospital stay, evolution and outcome: death, transfer or discharge. Conclusion: The most common pattern among patients was epileptiform, showing the highest degree of survival among them, when treated with antiepileptic drugs. The reactivity showed a prognostic value among the patients, 44.44% of 9 non-reactive patients died, and while the 6 reactive patients showed a 100% survival of patients with encephalopathy and EEG pattern with reactivity present during the period of time admitted.

El objetivo del presente estudio es determinar si la enfermedad de Chagas, es un factor de riesgo de insuficiencia cardiaca congestiva en pacientes ingresados al departamento de Medicina Interna del Hospital Escuela Dr. Oscar Danilo Rosales Argüello (HEODRA) de la Ciudad de León, la investigación se realizó entre Abril 2002 a Octubre 2003.Se condujo un estudio de base hospitalaria, caso-control no pareado en el HEODRA. Se estudiaron 151 casos con insuficiencia cardíaca congestiva (ICC), además cumplieron con un criterio mayor y al menos dos criterios menos según criterios de Framingham, para cada caso se seleccionó un control (151) sin insuficiencia cardíaca congestiva. Se calculó odds ratio, intervalo de confianza 95%. Análisis de regresión logística multivariada para controlar potenciales factores confusores.Los principales resultados fueron que, el 24.5% de insuficiencia cardiaca congestiva que ingresan al Servicio de Cardiología del HEODRA se encontró serología de Chagas positiva, en comparación con el 11.9% controles que presentaban serología de Chagas positiva, (OR: 2.39, IC95%: 1.29–4.44), siendo significativa. En el modelo multivariado, las variables que influyen en el desarrollo de ICC fueron: edad, sexo y serología de Chagas positiva.En conclusión, la enfermedad de Chagas esta asociada más de dos veces a insuficiencia cardiaca congestiva, así como otros factores asociados tales como: la edad >51 años, sexo femenino, siendo éstos estadísticamente significativos en el presente estudio.DOI: http://dx.doi.org/10.5377/universitas.v2i1.1642

Art, considered in its manifestations, whether through music, singing, painting, dancing, and all the possible expressions it can have, is undoubtedly a beautiful way to enrich the soul, the spirit, and human emotions. In this work, the different forms of understanding art, and the benefits that it can have on people, are exposed. It that art in its multiple manifestations can help control stress, anxiety, and depression. For this purpose, the investigators use bibliographic support to compare academic scenarios, artistic expressions that allow people to improve their emotional, physical, and psychological health. For this purpose, is implemented a bibliographic search to comparing academic scenarios, artistic manifestations that allow people to improve their emotional, physical, and psychological health. On the other hand, it was possible to verify that music performed as a half-hour therapy helps children with catastrophic illnesses, relieving pain. Rosacea is a common and chronic inflammatory skin disorder. The visiting trends and prescribing patterns involving Korean patients with rosacea have not been thoroughly examined. To examine the visiting trends of patients with rosacea, and to analyze the prescription patterns of Korean dermatologists managing such patients, a retrospective cross-sectional study on dermatology outpatients who visited the seven affiliated hospitals of The Catholic University of Korea between 2007 and 2018 was performed. A total of 56,651 visits were made by rosacea patients. The mean annual number of hospital visits made by rosacea patients over a 6-year period increased from 2456 in 2007–2012 to 6985 in 2013–2018. Hospital visits were most prevalent in female patients aged 40 to 59 years. There was no statistically significant difference in patient visitation between the seasons. As for prescriptions, systemic antibiotics were most commonly prescribed, followed by antihistamines, non-steroidal anti-inflammatory drugs, and retinoids. Among the topical agents, metronidazole was the most prescribed agent during 2007–2012, whereas calcineurin inhibitors were favored most during 2013–2018. Dermatology outpatient visits by individuals with rosacea have increased in Korea over time. The real-world prescription trend presented here may help dermatologists facilitate better treatment strategies and provide appropriate guidance to patients with rosacea.

Background:One third of patients with rheumatoid arthritis (RA) respond poorly to TNF inhibitors and related studies are inconsistent in predictive biomarkers. The identification of biomarkers that predict the treatment response prior to drug exposure is a current priority on the RA field. ACPA and RF are ubiquitously tested in RA patients, but other autoantibodies exist and may provide additional information on RA treatment response.Objectives:This study aimed to identify circulating autoantibodies for predicting response to infliximab (IFX) in RA.Methods:We profiled the autoantibody repertoire of baseline sera from 155 biologic naïve RA patients treated with IFX. The sera were provided by three independent clinical sources and distributed in one exploratory cohort (N=20) collected from Hospital Clínico Universitario of Santiago de Compostela (Spain), one replication cohort (N=61) collected from Hospital Universitario de A Coruña (Spain) and samples from the Swedish Farmacotherapy (SWEFOT) trial (Sweden) (N=74) for clinical validation. The presence of autoantibodies and their levels in serum were analysed in association with EULAR clinical response at 6 months follow-up: good response (GR, N=56), moderate (MR, N=55) and non-response (NR, N=44). A suspension bead array platform built on protein fragments within Human Protein Atlas and selected from an initial untargeted screening using an array containing 42000 antigens was employed to identify the IgG and IgA autoantibodies on the exploratory cohort. A replication and validation phases were carried out on the other two serum sample cohorts. Meta-analysis and Receiver Operating Curves were performed in order to assess the clinical relevance of the findings observed.Results:Meta-analysis revealed that the levels in serum of IgG autoantibodies against Centromere protein F (CENPF) are significantly increased in responders (good responders and moderate responders; N=111) to IFX compared to non-responders (N=44) (P=0.018). CENP-F is a proliferation-associated and cell cycle-dependent centromere autoantigen that might be involved in the increased or abnormal cell proliferation that occurs during RA process. Psychosocial impact of Rosacea in men has generally been a topic that in understudied in literature. We conducted a cross-sectional paper-based survey was conducted with male patients who have experienced rosacea for at least five years at the dermatology outpatient department at New Cross Hospital in Wolverhampton, England to determine the psychosocial impact of Rosacea on Men Quality of life and self-esteem. 52 % of the male population were noted to be “often” mentally pre-occupied by rosacea and frustrated due to rosacea. 85 % of the male population “never” used cosmetic camouflage. Mean function score is noted to be 6.81. This indicates that men are less in favour of coping mechanisms. Results from this study highlight that despite the emotional impact rosacea has on the male population, they do not frequently use coping mechanisms. This emphasises the psychosocial impact of rosacea on male population and the need for dermatologists to address the emotional impact on men as part of management as well as to raise awareness around this topic.

Introduction: Topical corticosteroids (TC) are useful for treatment of various dermatological conditions in all age groups. Due to its cosmetic application TCs misuse is intertwined with fairness creams in our colour conscious society where people are obsessed with fair colour. Aims and Objectives: To find the clinical presentations of steroid induced rosacea like-dermatitis on the face and to evaluate the purpose behind misusing TCs on the face. Materials and methods: Descriptive prospective questionnaire-type case series study was approved by Institutional Ethical Committee of BPKIHS which involved 205 patients diagnosed as Steroid Induced Rosacea who consulted Dermatology OPD between March to June 2017. Results: Among 205 patients enrolled, majority were from second to fourth decade (87.31%) of their life and maximum being females I.e. 91.2%. Around 97% of the study population were unaware of side effects of TCs. Most patients use Mometasone fuorate (58.04%) for melisma (88.29%). Friends acted as source of prescription (29.75%), followed by Pharmacist (24.88%) & Beauty centers (20%). Dermatologists prescribed TCs in only 6.34% cases. Sun exposure was identified as the triggering factor for steroid induced rosacea in 73.17% patients. Cheek was the commonest site involve in 92.19%, followed by forehead (65.36%), perioral area (46.83%), and chin (9.75%). Mixed type of lesions was the commonest lesion found in 52.20% patients, followed by diffuse facial erythema (36.09%), & telangiectasia (33.17%). Conclusion: TCs misuse in the younger people for the sake of fairer skin with little or no knowledge about the adverse effects of this medication should be stopped.

El idioma inglés es hablado por millones de personas en el mundo. A través de la historia, se ha convertido en el idioma de la ciencia debido a su papel en la difusión del conocimiento científico y la globalización1. Publicar artículos en inglés trae grandes ventajas relacionadas con la visibilidad e impacto, así como la posibilidad de aceptación en índices como Web of Science y Scopus2. Es por ello que se decidió invertir en talento humano que pueda traducir nuestra revista Alerta en un 100% a este idioma. Por tanto, esta nueva publicación, el volumen 5 número 2, es un sueño hecho realidad pues entregamos a la comunidad científica por primera vez una revista traducida en su totalidad al inglés, cada uno de los artículos disponible en PDF, XML, HTML y e-Pub en ambos idiomas. El contenido de este número incluye cuatro informes de caso. El primero, trata sobre el abordaje quirúrgico de un tumor pardo de maxilar en un paciente con síndrome de Sagliker; el segundo, relata el diagnóstico de cuatro neoplasias primarias sincrónicas en un adulto; el tercero, narra las manifestaciones clínicas de una fractura secundaria a un traumatismo leve y los estudios que confirmaron un sarcoma de Ewing. Los tres casos mencionados fueron atendidos en el servicio de radiología del Instituto Salvadoreño de Seguro Social. El cuarto y último informe sobre un caso de insensibilidad com pleta a los andrógenos abordado por las especialidades de endocrinología y urología del Hospital Nacional Rosales. Se incluye además, un artículo original que compara el riesgo de infección por SARS-CoV-2 posterior a la inmunización artificial e híbrida en el personal de salud, enviado por el Hospital Civil de Guadalajara, de México. Se publica también un artículo de odontología, titulado «Relación de la caries dental y el índice de masa corporal en población infantil». Además, se presenta una comunicación breve sobre el índice y la prevalencia de caries dental en niños de parvularia residentes de una zona urbanomarginal, presentado por la Universidad Evangélica del El Salvador y el Fondo Solidario para la Salud. Se han seleccionado en este número, tres revisiones narrativas. Una relata las estrategias no farmacológicas como adyuvantes para el manejo del dolor agudo asociado a punción en pediatría; la siguiente evidencia la importancia de conocer los factores que dificultan el diagnóstico del síndrome de taquicardia ortostática postural en adultos. La última trata sobre el impacto de los factores de estilo de vida en el sistema inmunológico y cómo modulan la susceptibilidad a agentes patógenos en adultos. Las tres fueron elaboradas por médicos en proceso de graduación de la Universidad Dr. José Matías Delgado. Nos complace que con cada número se incrementan las instituciones y los países que deciden publicar en Alerta. Esto, a la vez, se vuelve un compromiso para continuar el camino de la indexación y la construcción del prestigio de Alerta, que ya ha sido aceptada en bases de datos e índices como Latindex, LILACS, REDIB, AmeliCA, MIAR y recientemente en PERIÓDICA y BIBLAT de México. Es gracias a su indexación en BIBLAT que los artículos de Alerta ya cuentan con indicadores Altmetric3, Dimensions4 y PlumX5. Esto favorece la visibilidad y evidencia el número de veces que ha sido mencionado el trabajo de los investigadores en la web. Las altimétricas no sólo identifican el número de citas, también consideran otros aspectos como cuántos datos o bases de conocimiento se refieren a él, las veces que se ha comentado o discutido un texto, el número de visualizaciones y descargas de documentos y las menciones en medios, prensa, redes sociales, blogs y otras plataformas colaborativas; incluso, rastrea su uso en políticas gubernamentales3-5. Todas estas ventajas representan un beneficio para nuestros autores, al mostrar la cobertura y uso que tiene su publicación en toda la web y no solo las citaciones. Por ejemplo, Dimensions permite identificar el impacto de cada artículo gracias a cuatro métricas: citas totales, citas recientes (últimos 2 años), Field Citation Ratio (desempeño de un artículo comparado con otros del mismo tema) y Relative Citation Ratio, que muestra el desempeño de un artículo comparado con otros de la misma línea de investigación3. Esperamos que cada día, Alerta sea más reconocida y útil para toda la comunidad científica, academia y tomadores de decisiones clínicas, gestores y estudiantes en las ciencias de la salud, de todos los niveles.

Background. Rosacea is a significant problem, affecting 5.5% of the world population. Currently used treatment techniques such as transfer flaps and implants are insufficient to meet the needs of many patients, which suggests that alternative approaches are needed. Case Report. This report describes a case of rosacea complicated by giant rhinophyma treated with excision and closure by secondary intention and growth factor application. The patient was admitted to the Department of Dermatology at The First Affiliated Hospital of Wenzhou Medical University, Wenzhou, China, in July 2021, and underwent excision of the external nasal redundancy under general anesthesia. The postoperative wound was left open. The patient’s wounds healed completely 2 months after surgery, and there was no recurrence at 6-month follow-up. The wounds recovered well, with only slight scarring. Conclusion. The positive outcomes for this patient suggest that wound excision and closure by secondary intention and growth factor application may be beneficial for patients with rosacea complicated by giant rhinophyma.

Background. Granulomatous dermatoses, particularly on facial skin, pose a diagnostic challenge, as similar histologic patterns can be produced by different causes. Aim. To evaluate the correlation between clinical suspicion and histopathological findings in various facial granulomatous dermatoses. Materials and Methods. This retrospective, cross-sectional study included all patients with the histopathological diagnosis of facial granulomatous dermatoses from the years 2016 to 2021 in an academic hospital. Demographic, clinical, and histopathologic features were reviewed and analyzed. Results. In this study, 150 histopathological records with the diagnosis of facial granulomatous dermatoses from the years 2016 to 2021 were reviewed. The most common clinical diagnosis was rosacea 34 (23.6%), followed by sarcoidosis 27 (18.8%), leishmaniasis 15 (10.4%), and granulomatous rosacea 10 (6.9%). The frequency of clinical diagnosis of rosacea (70.6), sarcoidosis (66.7), foreign body G (62.5), TB (75), pseudolymphoma (75), acne agminata (66.7), and granulomatous rosacea (70) in female patients was higher than that in males (P value = 0.03). The effect of age on the type of both clinical and histopathological diagnosis was statistically significant (P value = 0.0001 and 0.004, respectively). Conclusion. Our study contributed significantly to the understanding of the clinicopathological aspects of facial granulomatous dermatoses and advocated for a multidisciplinary approach to the diagnosis and management of these complex skin conditions.

Background: Patients admitted in especial units such as coronary care unit (CCU) require intensive care for different aspects including skin problems. Here in this report we aimed to perform a survey on different skin lesions in patients admitted in CCU by the time of admission and when discharging. Methods: In this cross-sectional study, we had a survey on 200 patients admitted in CCU in AL-Zahra hospital, Isfahan. A dermatologist observed and examined each patients for any skin lesions by the time of admission and when discharging from CCU. Results: Here we showed that 15 percent of patients admitted in CCU have at least one skin lesion and also we indicated that seborrhea dermatitis and then rosacea have the most prevalence by the time of admission in CCU. Other skin lesions were acute skin failure. We also showed that prevalence of bed sore and pressure sores increased amazingly during admission. Conclusion: Rosacea, seborrhea dermatitis and drug allergic reactions after pressure sores are the most frequent skin lesions in CCU and there might be an association between rosacea and cardiovascular diseases.

Background. The use of topical steroids on the skin of the face should be carefully evaluated by the dermatologist; however, its misuse still occurs producing dermatological problem resembling rosacea. Objectives. To report the different clinical manifestations of steroid dermatitis resembling rosacea and to discover causes behind abusing topical steroids on the face. Methods. In this prospective observational study, 75 patients with steroid dermatitis resembling rosacea who had history of topical steroid use on their faces for at least 1–3 months were evaluated at the Department of Dermatology, Baghdad Teaching Hospital, between August 2010 and December 2012. Results. The majority of patients were young women who used a combinations of potent and very potent topical steroid for average period of 0.25–10 years. Facial redness and hotness, telangiectasia, and rebound phenomenon with papulopustular eruption were the main clinical presentations. The most common causes of using topical steroid on the face were pigmentary problems and acne through recommendations from nonmedical personnel. Conclusion. Topical steroid should not be used on the face unless it is under strict dermatological supervision.

The authors present case reports of two women, who were hospitalized in Department of Ophthalmology, University hospital Hradec Králové for ocular symptoms of rosacea. In the beginning there were relatively severe objective findings in the anterior segment of the eye in both cases – significant cornea graying with superficial and deep cornea vascularization in both eyes. On faces of both patients there were more or less typical sings of general illness. Especially in the first case there were severe papulopustules with crust all over the face. Subjectively Patients describe characteristic symptoms of dry eye. After dermatological consultation local and general anti-inflammatory treatment was set with consequent condition improving in both cases patients. In both cases we use two different general therapy formula according to literature (Azitromycin x Doxyhexal). In one case report, we found distinctive non-compliance during aftercare on our department with serious consequences. The purpose of our report is to point out ocular complications of rosacea and importance of oftalmologist´s and dermatologist´s cooperation during therapy of this illness with can lead to permanent damage of front segment of the eye and significant loss of visual acuity.

Use dermoscope to identify inflammatory dermatoses with facial erythema as the main feature and other atypical clinical manifestations. By reviewing the clinical data and dermoscopic images of outpatients in our hospital, four diseases of rosacea, seborrheic dermatitis, contact dermatitis and atopic dermatitis were screened out, and their dermoscopic characteristics were obtained: Rosacea is a diffuse distribution of polygonal blood vessels on a red or purple background; SD is an atypical vascularity on a yellow-red background with an oil-drop reddish halo around the hair follicle. Contact dermatitis is linear blood vessels and pleomorphic blood vessels in a light red background; AD is punctate blood vessels and spherical blood vessels under a light red background with a visible scaly distribution. Honeycomb pigmentation can be seen in seborrheic dermatitis and contact dermatitis. Combined with relevant clinical reports, the application of dermoscope in clinical diagnosis and treatment are discussed.

En 1685 llegó procedente de Palermo a la archidiócesis de Sevilla don Jaime de Palafox y Cardona, permaneciendo en ella como arzobispo de Sevilla hasta el año de su muerte en 1701. Durante su gobierno fueron diversas las devociones personales que calaronprofundamente en la epidermis de Andalucía, destacando entre ellas la del Santísimo Sacramento, la de Santa Teresa de Jesús y la de Santa Rosalía, además de la admiración y respeto que sintió por el sacerdocio. Todas ellas tuvieron una fuerte repercusión en la religiosidad de la Sevilla de finales del siglo XVII y se manifestaron en las diversas expresiones artísticas en la ciudad y en la promoción de edificios dedicados a dichas advocaciones. Este fue el caso de la Casa Hospicio de Venerables Sacerdotes de Sevilla, cuyos trabajos comenzaron bajo la tutela de su antecesor en la mitra hispalense, don Ambrosio Ignacio de Spínola y Guzmán, y siendo terminados bajo el mandato del prelado que cierra el siglo XVII.

Objective. To explore the clinical effect and safety of 5-aminolevulinic acid photodynamic therapy (ALA-PDT) combined with 1550 nm fractional laser therapy in the treatment of rosacea. Methods. 114 patients with type I and type II rosacea treated in our hospital from March 2018 to April 2020 were recruited. They were randomly assigned (1 : 1 : 1) to receive ALA-PDT (photodynamic group), 1550 nm fractional laser (laser group), or ALA-PDT and 1550 nm fractional laser therapy (combination group). Outcome measures included skin lesion scores, efficacy, and adverse reactions. Results. After treatment, patients in the three groups showed lower skin lesion scores than before treatment, and the combination group showed significantly lower results than the other groups ( P < 0.05 ). There was no significant difference in the total efficacy among the three groups ( P > 0.05 ), but the combination group outperformed the other groups in the comparison of the efficacy levels ( P < 0.05 ). Edema with lupus erythematosus, pain, and burning sensation was found in some cases but disappeared within 2 to 4 days after symptomatic treatment without interference with subsequent treatment. No patients had pigmentation, hypopigmentation, scars, or other serious adverse reactions. Conclusion. In the treatment of rosacea, ALA-PDT combined with a 1550 nm fractional laser can reduce redness and facilitate skin remodeling, with high efficacy and safety, so it is worthy of clinical promotion and application.

Objective: To determine the frequency of seborrheic dermatitis, bullous pemphigoid, rosacea, and melanomaamong the patients diagnosed with Parkinson Disease.Study Design: Cross sectional study.Place and Duration of Study: The study was carried out at Dermatology Department of Bakhtawar Amin Trustth th Teaching Hospital Multan from 13 May to 13 October 2020.Materials and Methods: A total of 80 patients of Parkinson disease were evaluated for skin disorders. Diseaseseverity was confirmed through Hoehn Yahr staging criteria (H-Y stage). Skin characteristics and diseasediagnosis was made through finding sebum production levels, hot/cold flush test, immunofluorescence andbiopsy evaluation for seborrheic dermatitis, rosacea, bullous pemphigoid, and melanoma, respectively. Thetest results were compared with control subject. Chi square test and student t-test were used to find thesignificance of results. P < 0.05 will be statistically significant.Results: Out of 80, 44 patients were positive for skin disorders. The frequency of seborrheic dermatitis was31.2% and was quite high as compared to control group that was 25% whereas rosacea, bullous pemphigoidand melanoma was found in 15%, 6.2% and 2.5% patients and in control group they were 11.2%, 2.5% and 1.0%respectively. To detect the significant difference between quantitative variables of test and control group, chisquare was used and student t-test was used to compare the qualitative variables. P < 0.05 will be statisticallysignificant.Conclusion: Skin disorders are highly prevalent among Parkinson's patients than control population.

Background/Objective: Skin diseases, especially those with visible manifestation, are considered to cause a major influence on global mental health. Therefore, we determined the prevalence and severity of anxiety, depression, and suicidal ideation in a large sample of patients with facial dermatoses, namely acne, rosacea, folliculitis, and perioral dermatitis. Methods: The mental health of patients with facial dermatoses and respective controls was assessed using the Hospital Anxiety and Depression Scale and questions concerning suicidal ideation. Results: The study included 543 patients with facial dermatoses and 497 healthy individuals. Anxiety was present in 37.6% of the patients (14.9% of controls), depression in 21.7% (6.8%), and suicidal thoughts in 9.8% (3.2%) (p < 0.001). Acne patients demonstrated the highest anxiety and depression subscale scores (mean ± standard deviation: 7.1 ± 0.25, 95% confidence interval (CI): 6.58–7.56; controls: 5 ± 0.23, 95% CI: 4.57–5.49). In rosacea 30% of the patients had depression symptoms (adjusted odds ratio (OR): 7.216, 95% CI: 4.122–12.632, p < 0.001), while in folliculitis patients 15.4% (OR: 3.138, 95% CI: 1.241–7.936, p = 0.016) had suicidal thoughts. Patients with anxiety symptoms and suicidal thoughts were on average younger than those without (28.3 ± 0.76 vs. 31.2 ± 0.66 years, p = 0.001 and 25.3 ± 0.98 vs. 30.5 ± 0.55 years, p = 0.007, respectively). Conclusion: Acne and rosacea are associated with anxiety, depression, and suicidal ideation in Lithuanian patients. Younger patients are more prone to report such symptoms than older ones.

Dupilumab is the first biological treatment approved for moderate-to-severe atopic dermatitis (AD). Efficacy and safety have been demonstrated in clinical trials, but real-life data is still limited. The objective of this study was to retrospectively evaluate Dupilumab treatment in AD patients in a real-life clinical setting. Effectiveness and safety outcomes were collected at baseline and after 2, 6, 10, 24, 39, and 52 weeks by using clinical scores for disease activity, as well as serological markers. Ninety-four patients from five dermatological hospitals were included. After 24 weeks of treatment, the median Investigator Global Assessment (IGA) and Eczema Area and Severity Index (EASI) showed a significant reduction compared to baseline (3.9 ± 0.7 vs. 1.4 ± 0.8 and 26.5 ± 12.5 vs. 6.4 ± 6.5). Interestingly, we observed rosacea-like folliculitis as an unexpected side effect in 6.4% of patients. Dupilumab proves to be an effective and well-tolerated treatment under real-life conditions. The occurrence of rosacea-like folliculitis warrants further mechanistic investigation.

Objective: To investigate the clinical efficacy of intense pulsed light combined with doxycycline in the treatment of rosacea. Methods: A sample of 60 patients with rosacea admitted to our hospital from January 2021 to December 2023 were selected and grouped into the observation group (n = 30) and the control group (n = 30) by using the randomized numerical table sampling method. The patients in the control group were treated with doxycycline, and the patients in the observation group were treated with intense pulsed light combined with doxycycline. The clinical effective rate and recurrence rate, clinical symptom score, skin barrier function indexes, and the incidence of adverse reactions in the two groups were compared. Results: The clinical effective rate of the observation group was higher than that of the control group, and the recurrence rate was lower than that of the control group (P < 0.05); the clinical symptom score of the observation group was lower than that of the control group after treatment (P < 0.05); the water content of the stratum corneum of the observation group was higher than that of the control group after treatment, and the amount of transepidermal water loss was lower than that of the control group (P < 0.05); the incidence of adverse reactions of the two groups did not have any significant difference in comparison (P > 0.05). Conclusion: The treatment effect of intense pulsed light combined with doxycycline in rosacea patients is remarkable. It can alleviate clinical symptoms and improve the skin barrier function, with a lower recurrence rate after treatment and higher therapeutic safety, which is suitable for popularization and use in healthcare institutions.

Introduction: Irrational use of topical corticosteroid (TC) is quite common in Ethiopia due to the unrestricted availability and use of topical steroids (TS) not only by general public but also by physicians and chemists. This practice is highly prevalent and sought after, owing to the quick relief of symptoms in different dermatological conditions and its nature of enhancing beauty for a certain period. Objective: This study aims to evaluate and analyze the prevalence of misuse of topical steroids, causes behind the misuse and consequences after the use of topical steroids among females in a dermatology outpatient department (OPD) at Ayder Referral Hospital, Ethiopia. Materials and Methods: A cross-sectional observational questionnaire based study was conducted from April to August 2015. Patients were questioned and assessed for misuse of topical steroids in terms of indication, dose, frequency, duration, effects after use and source of recommendation of topical steroids among female clients. Results: During the study period, 104 (27.1%) patients presented with inappropriate use of topical steroids among patients (n=384) who were attended the OPD of dermatology unit at Ayder Referral Hospital, the only hospital providing treatment, care and preventive measures for dermatological cases, with a response rate of 97%. A total of 72 patients were presented with adverse effects of topical steroids resulting from the misuse of these drugs for various purposes. Misuse of topical steroids was found to be most prevalent in the age group of 21-30 years (51.15%) followed by age group between 11-20 years (21.15%), who were severely affected due to its inappropriate use. Whereas, melasma (40.38%) and rosacea (35.6%) were the most common reasons of steroid abuse, followed by facial acne (26%), use as sunscreen 44 (42.3%), non-specific dermatosis 35 (33.7%) and skin lightening (10.58%). Patients using topical corticosteroids as over-the-counter (OTC) after recommendation made by friends and family (44.8%) was found to be a factor of influence on the misuse of topical steroids followed by pharmacist and drug store vendors as over the counter patterns of use (31.7%). The least were recommended by clinicians (10.57%) for the treatment of acne, melasma/hyper-pigmentation and to improve the appearance during weddings and other ceremonies. Dermovate and dermalar (70.18%) were the most commonly used topical steroid preparations, causing the patients to often develop fungal infections such as tinea incognito (26.4%), followed by facial acne (25.6%), telangiectasia and rosacea (41%) being the most common side effects experienced. Conclusion: Misuse of topical steroids was found to be very common owing to prescriptions by dermatologists, the major reason being recommendations by healthcare professionals, pharmacists, family and friends. Abuse of these steroids causes fungal, bacterial infections and rosacea which leads to many other secondary complications.

<p>This short biography traces the life and medical activities of Rosalie Sattler, née Feuerstein (1883–19??), who was employed as an official female physician at the Austro-Hungarian (AH) provincial public health department in Sarajevo from 1914–1919. Born in 1883 into a Jewish middle-class family in Chernivtsi (then Czernowitz), Ukraine, in Bukovina, the easternmost province in Austria, Feuerstein moved to Vienna in 1904 to study medicine. After earning her MD from Vienna University in 1909, she started her career as an assistant physician at the Kaiser Franz Josef Hospital in Vienna. In spring 1912, Feuerstein moved to Sarajevo to work as an intern at the local provincial hospital (Landeskrankenhaus). In the same year, she married AH district physician Moritz Sattler (1873–1927) in Vienna. In 1914, Sattler-Feuerstein successfully applied to be an AH official female physician in Bosnia. She was an employee of the provincial public health department in Sarajevo and never functioned as an official female physician in the sense of the relevant AH service ordinance. After the collapse of the monarchy, Sattler-Feuerstein continued to be employed as an official female physician of the Kingdom of Serbs, Croats, and Slovenes. She resigned from service in 1919 and established herself as a private general practitioner in Sarajevo with her husband, who had also resigned as an official physician and started to practice privately at that point. Widowed in 1927, she left Sarajevo for an unknown destination, likely in 1938–1939, and vanished from historical records.</p><p><strong> Conclusion</strong>. Rosalie Sattler-Feuerstein (1883–19??) came to Bosnia as the eighth AH official female physician and worked as an employee of the AH provincial public health department in Sarajevo from 1914–1919, after which she practiced as a private physician in Sarajevo for more than 25 years.</p>

Background: Topical steroids are the most commonly prescribed drugs in dermatology. The adverse effects of steroid misuse are noticeable 3 to 4 weeks after application. Steroid rosacea, hypertrichosis and acneiform eruptions are few of them. A new entity known as topical steroid dependent face, topical steroid dependent face (TSDF) has been recently coined to encompass symptoms such as erythema, burning sensation on attempted cessation of topical steroid application.Methods: A questionnaire-based analysis was done among patients attending dermatology outpatient department of government medical college hospital, Mysore between November 2018 to May 2019. Prior approval of the institutional ethics committee, and consent of patients were obtained. A total of 200 outpatients with facial dermatosis using topical steroids on face for a period greater than one month were taken up for study.Results: The results included population across different age groups, between 16 to 60 years. 56% belonged to the age group of 16 to 30 years. Most common steroid abused was mometasone cream 0.1% (50%), betamethasone valerate cream 0.1% (24.5%) followed by clobetasol ointment 0.05% (21.5%). The major adverse effect with steroid abuse, were acne 72% facial redness 67%. Hyperpigmentation 51%, hypertrichosis 32.5% and skin atrophy 21%.Conclusions: The present study highlights and creates awareness on the burden of facial topical steroid abuse and the poor attitude towards them.

BACKGROUND: Significant associations have been described between climatic factors and human health, which can occur in dermatologic diseases too. OBJECTIVE: To determine the influence of the climatic factors on the medical attentions of dermatologic diseases in a general hospital in Lima (Peru). PATIENTS AND METHODS: Observational study which was carried out in a national hospital between January 2004 and December 2007. The diagnoses were classified according to the ICD-10 system. The climate information was provided by the National Oceanographic and Atmospheric Administration from USA(NOAA). RESULTS: 3 294 patients were included in the study, the average age was 35.4 ± 20.7 years old, and 53.2% were females. The "El Niño" phenomenon was associated with an increase in the prevalence of actinic keratosis (p=0.002), viral warts (p=0.001) and rosacea (p=0.014). The "La Niña" phenomenon was associated with a reduction in viral warts (p=0.026). Spring was associated with an increase of dermatitis (p=0.003), and summer was associated with an increase of benign neoplasms (p=0.049). CONCLUSION: The climatic variations influenced the occurrence of certain dermatologic diseases. The present study may represent an orientation guide to specialists and general practitioners identifying the most common dermatologic diagnoses and thus enabling better preparation to treat these cases in determined seasons of the year.

El hipoadrenocortisismo es una enfermedad poco diagnosticada o subdiagnosticada, dentro de la medicina veterinaria de pequeñas especies en Ecuador. El objetivo del presente estudio es reportar el tratamiento y el diagnóstico de esta patología para lograr identificar en etapas tempranas y administrar una correcta terapéutica. La paciente llega a consulta, por un cuadro inespecífico, los tutores reportan que tiembla, esta inapetente y ha disminuido la actividad física hace 3 días. En otro servicio la recetaron omeprazol a 1mg/kg y meloxicam a 0.05mg/kg. El equipo médico del Hospital Argos identifica en la evaluación física mucosas húmedas y rosadas, TRC 4 segundos, FR: jadeo, dolor abdominal craneal severo. Estado de consciencia normal, pulso débil, SpO2: 93%, presión sistólica: 145mmHg. Se le sugieren exámenes generales en el Hospitales Veterinario Argos, hemograma, química sanguínea, coproparasitario, ecografía abdominal y lipasa pancreática. En donde resalta una neutrofilia moderada, hiponatremia y hipercalemia, con lipasa pancreática positiva (snap IDEXX). Se evalúa el cuadro junto con el servicio de Endocrinovet, y se determina un posible cuadro de hipoadrenocortisismo. Se indica una medición basal de cortisol, el cual resulta <1ug/dl, lo cual es altamente sospechoso de la enfermedad, ante la falta de disponibilidad de ACTH (gold standard de diagnóstico), para realizar una estimulación, para un diagnóstico definitivo, se decide instaurar tratamiento, por los exámenes y la clínica del paciente. En el presente se reportan las ventajes y desventajas del tratamiento instaurado en la paciente (fludrocortisona), en comparación con el tratamiento de elección pivalato de desoxicorticosterona, junto con la evolución del cuadro clínico de la paciente. En el presente trabajo se estudia la evolución de un tramo costero de la Ría de Vigo (NW de la península Ibérica) desde época romana hasta el siglo XVII, prestando particular atención a la Alta Edad Media. Desde el punto de vista arqueológico, la importancia del sector radica en la presencia de una salina de época romana, en uso desde los siglos II BC a III-IV AD, y su abandono para dar paso a la instauración de una iglesia y una necrópolis en la Alta Edad Media (a partir de los siglos IV-V AD). Se muestrearon, con alta resolución, tres secuencias edafo-sedimentarias y se analizaron diversas propiedades físico-químicas (pH, granulometría, contenido en C, N, S, P, Fe, As) y la composición mineralógica, con el fin de determinar la naturaleza de las distintas facies presentes, los procesos implicados y las causas asociadas. Los resultados indican modificaciones en la costa entre finales del periodo romano y comienzos de la Alta Edad Media, con unaevolución del medio desde una dinámica costera, con formaciones de marisma-lagoon costero, a una dinámica eólica, con dunas. Dicho cambio es coincidente con el cese de la explotación romana de sal y el inicio del periodo Frío Altomedieval. Estos resultados son similares a los encontrados en otros puntos de la Ría de Vigo. En los tres sectores comparados (Rosalía de Castro, Toralla y Hospital), a la par que se produce la progradación de las formaciones de marisma y duna, el uso antrópico del sector cambia. Aunque la pauta climática presenta un papel importante en la evolución de este sector, los cambios en la línea de costa que se detectan entre ambos periodos culturales pudo estar determinada también por causas socioecómicas. Evolution of a coastal area during the early Middle Ages in the NW of the Iberian Peninsula - This paper focuses on the evolution of a coastal stretch of the Ria de Vigo (NW Iberian Peninsula) during the early Middle Ages, which presents an important archaeological site with a roman salt mine. Salt exploitation in the Roman period ceased around the AD 3rd-5th centuries, and was followed by the establishment of a churcheand a cemetery during the Middle Ages. The properties (pH, grain size, C, N, S, P, Fe and As, and mineralogicalcomposition) of three pedo-sedimentary sequences were analysed in order to identify the different environmental changes that affected this sector. The results suggest signifcant changes occurred in the coast from the late Roman period (AD 3rth-5th centuries) into the early Middle Ages (AD 5th-6th centuries), with an evolution from a marine-continental dynamics, with formation of salt marshes, to a wind dynamics, with dune formations. This change is consistent with the abandonment of the Roman salt exploitation and the beginning of Cold Dark Ages. These results are similar to those found in other areas of the Ría de Vigo. In the three sectors compared (Rosalia de Castro, Toralla and Hospital) land use changed while the progradation of marsh and dune formations occurred. Although climate may have played a major role in the evolution of this sector, the changes in the coastline between the two cultural periods could be also related to socio-economic causes.

Objectives: Potassium iodide (KI) is a medication that has been used for decades in dermatology and it is mentioned as a treatment option in all major dermatology textbooks. Yet, there is little recent information on its efficacy. In our study, we wanted to retrospectively evaluate the therapy response to KI in our patients. Methods: The hospital information system was searched for patients treated with KI at the Department of Dermatology (University Hospital Zurich) in the last 20 years (January 1, 1998 to December 31, 2017). A total of 52 patients were found and, subsequently, 35 patients were included in our study. Results: KI was prescribed for the following skin conditions: erythema nodosum, disseminated granuloma anulare, necrobiosis lipoidica, nodular vasculitis, cutaneous sarcoidosis, and granulomatous perioral dermatitis/ rosacea. The median duration of KI intake was 5 ± 7.7 weeks (range 1–26). The global assessment of efficacy by the treating physician showed an improvement of disease in about a third of all patients. No response was seen in 14 patients and 9 even had a progression of disease. An adverse event was documented in 16 cases. Conclusions: Our findings show that an improvement was reached in only about a third of all cases. High response rates with only mild side effects (in 16 out of 35 patients) were observed.

BACKGROUND: Inevitably, the floors of mosques are contaminated with microorganisms, and the risk of pathogen transmission is probably high between the many visitors, but the issue has been infrequently studied. OBJECTIVES: Investigate microorganism variety and risk of contamination on commonly used carpets and rosaries (prayer beads). DESIGN: Cross-sectional SETTINGS: Mosques METHODS: This study was carried out in three different cities of Turkey in 2023, focusing on mosques located around hospitals. Forty mosques were included in the study and from each mosque 10 samples were collected from various parts of carpets and rosaries. The number of positive culture isolates were identified. MAIN OUTCOME MEASURES: Diversity and distribution of microorganisms isolated from mosque carpets and rosaries; methicillin-resistance rates in Staphylococci . SAMPLE SIZE: 400 samples RESULTS: Growth was observed in 368 (92%) of 400 samples examined. The microorganisms isolated in the highest number were methicillin-susceptible coagulase negative Staphylococci (MSCoNS) (59.8%), Microcooccus (41%) and diphtheroids (31.3%). The rates of total growth ( P =.001), including diphtheroids ( P =.018), methicillin-resistant coagulase negative Staphylococci (P=.001), Bacillus spp. ( P =.036) and Aspergillus spp. ( P =.002) rates were significantly higher in the rosary samples than carpet samples. At mosques in Tokat, a province center, 4 samples were positive for Acinetobacter baumannii , two samples were positive for Pseudomonas aeruginosa and one sample for methicillin-resistant Staphylococcus aureus (MRSA), and these were isolated from rosaries. 0.3% of Staphylococcus isolates were MRSA. CONCLUSION: As there is a high risk of contamination of carpets and prayer beads on the mosque floor with human flora, the use of appropriate hygiene practices is necessary. We also found some emerging bacteria in addition to the normal human flora. LIMITATIONS: Our study was conducted in three provinces. Further studies might cover a wider geography.

Antibiotic resistance is a major health concern worldwide as the list of bacterial pathogens that are insensitive to available antibiotics continues to grow in both hospitals and outpatient communities. The slow development of newer antibiotics adds to the formidable challenge that clinicians face with treatment of infections caused by antibiotic-resistant bacteria. This article discusses important caveats related to antibiotic use in dermatology. These include understanding that both topical and oral antibiotics contribute to the emergence and spread of resistant bacteria, that antibiotic monotherapy is to be avoided for treatment of acne vulgaris, that effective treatment of rosacea does not require the use of an antibiotic, that antibiotic therapy in the management of atopic dermatitis is best limited to treatment of an active clinical infection, and that routine post-operative use of a topical antibiotic is not suggested after most office-based dermatologic procedures. By following principles of antibiotic stewardship, dermatologists are major players in the battle against antibiotic resistance.

Background:Sarcoidosis is a multisystemic disease characterized by the formation of non-necrotizing granulomas. The most frequently affected organs are lungs, skin and eyes. Systemic corticosteroids are the most used drugs in the treatment of this disease. Conventional and biological immunosuppressants (IS) may also be used(1-3).Objectives:To evaluate the systemic treatment of sarcoidosis according to clinical domains.Methods:Study of all consecutive patients diagnosed with sarcoidosis between 1/1/1999 and 1/1/2019 in a tertiary university hospital. The diagnosis was established following the ATS/ERS/WASOG criteria(Eur Respir J. 1999;14(4):735-737):compatible clinical and radiological presentation, evidence of non-caseifying granulomas and exclusion of other granulomatous diseases.Results:We studied 377 patients (188 men/189 women), mean age at diagnosis of 46.0±14.8 years. After a mean follow-up of 13.0±9.3 years, 161 (42.7%) patients did not require treatment. The remaining 216 (57.3%) received oral glucocorticoids (206, 54.6%) with a maximum mean dose of 43.2±19.0 mg/day, conventional IS (60, 16.2%), biological therapy (28, 7.4%) and/or endovenous methylprednisolone (15, 4.0%). Biological therapy was indicated by pulmonary (9, 32.1%), ocular (9, 32.1%), neurological (3, 10.7%), muskuloeskeletal (3, 10.7%), cutaneous (2, 7.1%), nephrological involvement (1, 3.6%); and Heerfordt’s syndrome (1, 3.6%). Adalimumab and Infliximab were the biologics used more frequently (Table).TABLESystemic treatment of sarcoidosis according to clinical domains.Conclusion:Compared to other studies, the high percentage of patients who required systemic treatment is remarkable. La investigación acerca del covid-19 centra en la actualidad la actividad de la comunidad científica internacional. En el contexto de la pandemia, adquiere relevancia producir conocimiento sobre la incidencia del SARS-CoV-2 en el proceso gestacional, el embarazo, los efectos en la paciente obstétrica durante el parto, puerperio y el recién nacido. Es sabido que la mujer embarazada, debido a los cambios por los que atraviesa, es propensa a enfermedades respiratorias, y eso la hace particularmente sensible a la COVID-19. Se hace útil conformar un estado de la cuestión a partir de la revisión sistemática de la literatura acerca del tema, tomando como fuente principal los informes emanados por la Organización Mundial de la Salud, así como otras comunicaciones científicas, con el objetivo de describir algunas propuestas para la atención de pacientes embarazadas contagiadas o no. En la mayoría de las fuentes consultadas, se manifestó un acuerdo en la necesidad de estudiar y establecer un protocolo de atención a la mujer en estado de gestación y al feto, incluso al neonato.
