Health policy and planning play pivotal roles in the effective management of healthcare systems. These aspects encompass the formulation, implementation, and evaluation of strategies and regulations to optimize healthcare delivery. Robust health policies ensure equitable access, quality care, and cost-effectiveness, while planning entails resource allocation, infrastructure development, and workforce distribution. Successful health management systems hinge on evidence-based policies, stakeholder engagement, and adaptability to evolving health challenges. This abstract highlights the critical interplay between policy formulation and strategic planning, emphasizing their indispensable contributions to achieving efficient, accessible, and sustainable healthcare services.

ABSTRACT Manpower is the most crucial resource toward delivery of health planning. Health manpower refers to people who are trained to promote health, to prevent and to cure diseases, and to rehabilitate the sick. The aim of manpower planning is to make available the right kind of personnel in the right number with appropriate skills at the right place at the right time doing the right job. Various types of health resources are doctors, nurses, pharmacists, lab technicians, radiographer health assistants, health workers, auxiliary nurse midwife (ANM), accredited social health activists (ASHAs), anganwadi workers, trained dais, and so on. Currently, developing countries including India lag behind suggested norms of required health manpower. Presently, India produces 30,000 doctors, 18,000 specialists, 30,000 Ayurveda, Yoga and naturopathy, Unani, Siddha, and Homeopathy (AYUSH) graduates, 54,000 nurses, 15,000 ANMs, and 36,000 pharmacists annually. This production is not equal across the states, leading to unequal distribution of doctors. Such a skewed distribution results in large gaps in demand and availability. Various reasons for this are skewed production of health manpower, uneven human resource deployment and distribution, disconnected education and training, lack of job satisfaction, professional isolation, and lack of rural experience. The 12th Plan should aim to expand facilities for medical, nursing, and paramedical education; create new skilled health worker categories; enable AYUSH graduates to provide essential health care by upgrading their skills in modern medicine through bridge courses; establish a management system for human resource in health to actualize improved methods for recruitment, retention, and performance; put in place incentive-based structures; create career tracks for professional advancement based on competence; and, finally, build an independent and professional regulatory environment. This thesis develops quantitative methods which incorporate transportation modeling for tactical and operational home health logistics planning problems. We define home health nurse routing and scheduling (HHNRS) problems, which are dynamic periodic routing and scheduling problems with fixed appointment times, where a set of patients must be visited by a home health nurse according to a prescribed weekly frequency for a prescribed number of consecutive weeks during a planning horizon, and each patient visit must be assigned an appointment time belonging to an allowable menu of equally-spaced times. Patient requests are revealed incrementally, and appointment time selections must be made without knowledge of future requests. First, a static problem variant is studied to understand the impact of fixed appointment times on routing and scheduling decisions, independent of other complicating factors in the HHNRS problem. The costs of offering fixed appointment times are quantified, and purely distance-based heuristics are shown to have potential limitations for appointment time problems unless proposed arc cost transformations are used. Building on this result, a new rolling horizon capacity-based heuristic is developed for HHNRS problems. The heuristic considers interactions between travel times, service times, and the fixed appointment time menu when inserting appointments for currently revealed patient requests into partial nurse schedules. The heuristic is shown to outperform a distance-based heuristic on metrics which emphasize meeting as much patient demand as possible. The home health nurse districting (HHND) problem is a tactical planning problem which influences HHNRS problem solution quality. A set of geographic zones must be partitioned into districts to be served by home health nurses, such that workload is balanced across districts and nurse travel is minimized. A set partitioning model for HHND is formulated and a column generation heuristic is developed which integrates ideas from optimization and local search. Methods for estimating district travel and workload are developed and implemented within the heuristic, which outperforms local search on test instances.

Resumo:
Rationale: The health visiting service is not planned on the basis of the health needs of the local population. Although relevant information is available it has not been made accessible for use. This study aims to: 1. Obtain baseline data regarding aims, planning methods, health goals and information needs of health visitors in selected practices in Sheffield. 2. Provide information concerning the health visitors' potential caseload and monitor its impact on the production of community profiles and identification of health goals. 3. Evaluate whether health visitors perceive positive change in their planning abilities and whether information provided meets their information needs. 4. Evaluate the social and political effects on the organisation of the health visiting service relating to the information provision. Nature. Scope and Method: An action research approach is used. The sample included, health visitors (N = 31) and their managers (N = 8), who were interviewed with an audiotaped interview schedule, piloted (N = 11). The health visitor sampled attended information workshops resulting in their building community profiles, negotiating practice with managers and producing an innovative method of planning. Workshop discussions and interview results were relayed back to all participants. The health visitor sample completed an evaluation questionnaire. Organisational changes during the research period were recorded. Contribution to knowledge: Community profiles can be used to assist community diagnosis relating the planning of the health visiting service to the health needs of the population. Profiles can fill information gaps existing in the service. Organisational changes to aid profile effectiveness include implementing, an information policy and system, appraisals, clear general policy statements, management training, and addressing a series of changes sought by health visitors. Alternative sources of funding for the service are suggested, as is grassroots representation in the planning process. The study provides an insight into the information and planning needs of health visitors in their organisational setting.

Air pollution is a major global environmental concern that has significant impacts on public health. With increasing urbanization, cities are an appropriate unit for the analysis of this issue. Investigating the correlation between air quality, respiratory diseases, and urban parameters in five micro-locations in the case of Skopje the study finds that air quality is influenced by a variety of factors including urban parameters, land use, types of heating systems, and proximity to the burning of stubble and use of harmful fuels. To address the issue of air pollution in Skopje, the study recommends implementing measures such as promoting the use of open block typologies in urban planning, encouraging the use of sustainable heating systems, implementing stricter regulations on industrial activities, increasing the use of green spaces, and reducing the built footprint. Overall, addressing air pollution in Skopje requires a comprehensive approach that considers individual and structural factors. Keywords: air quality, health, urban morphology

Resumo:
Transportation systems can impact both physical and mental health, but the effects of transportation planning decisions on health, beyond safety and air pollution, are often overlooked or undervalued (Litman, 2013). Although not the only factor contributing to health disparities, transportation systems serve as a modifiable variable contributing to health behaviors and outcomes (Nieuwenhuijsen & Khreis, 2019). Following a similar approach to Singleton and Clifton (2017), a review of transportation planning documents from the ten most populous counties within the state of Montana was conducted to analyze the extent that health was considered. A directed and summative approach was used to identify and code health-related guidance statements, performance measures, and reference data in the domains of general health, safety, air quality, physical activity, accessibility, mental health, and equity. Although all transportation plans contained health-related guidance statements and reference data, only those transportation plans from counties containing a metropolitan planning organization (MPO) included health-related performance measures. The inclusion of health-related guidance statements and reference data is evidence that local Montana communities are interested in the impacts of transportation planning on health behaviors and outcomes. Without the federal requirements that MPOs must follow, rural governments are not utilizing health-related performance measures to fully understand performance of the local transportation system related to their community’s public health priorities. The existing performance management structure required of States and MPOs could serve as a guide for further integrating health-related performance measures into small urban and rural transportation planning and can assist local decision makers in analyzing progress toward community priorities (Singleton & Clifton, 2017).

This series of briefing sheets, developed by the Population Council, represents a starting point for the application of gender to the family planning (FP) and reproductive health (RH) arena. The information covers five key areas of RH: gender-based abuse, FP and gender issues among adolescents, sexuality and FP, men’s and women’s social and economic responsibilities for childrearing, and the complicated nexus between gender and FP/RH. In so doing, it provides a strong foundation for gender training. This concept was first developed to fulfill a need for gender training within the regional Operations Research/Technical Assistance Projects. To date, two such workshops have been funded by USAID. These activities not only reflect a clear commitment by USAID to the inclusion of gender in all aspects of programmatic planning and implementation but also to the empowerment of women in every phase of the development process. This tool will help guide program planners and policymakers toward making gender equity in FP and RH programs a reality for both women and men.
