This chapter focuses on aspects of anaesthesia for complex, major surgery such as hepatic resection and oesophagectomy. The theories and practice of enhanced recovery after surgery and perioperative optimisation with goal directed therapy are included here. The systemic impact of malignancy and its treatment modalities are also discussed. The practical and ethical aspects of organ transplantation are discussed. Anaesthesia for renal and hepatic transplant is described, as well as considerations for anaesthetising the transplant recipient for non-transplant surgery. Recognition of transurethral resection syndrome in urological surgery is potentially life-saving; causes, management and avoidance are discussed. The NICE criteria for performing bariatric surgery, types of surgery, and conduct of anaesthesia for this challenging patient group is also covered.

Training in oral and maxillofacial surgery is lengthy and demanding, requiring dual qualification in dentistry and medicine, followed by completion of a higher surgical training program. The ‘exit’ examination leading to the awarding of the specialty Fellowship of the Royal College of Surgeons in Oral & Maxillofacial Surgery, the FRCS (OMFS), represents the final academic step of training. For Section 1 of the examination, one day is dedicated to an examination of single best answers (SBAs). These are undertaken as two tests, each 2 hours 15 minutes in length. The questions encompass the entire daunting syllabus of Oral and Maxillofacial Surgery, including orthognathic surgery, craniomaxillofacial trauma, surgical oncology and dermatology, oral medicine, craniofacial surgery, dentoalveolar surgery, temporomandibular disorders, salivary gland disease, local flap and free tissue transfer, oral implantology, and cleft surgery. The aim of this book is to give candidates an understanding of the range of clinical subjects that would be encountered in an assessment appropriately blueprinted against the full breadth of the syllabus. This includes pertinent subjects that are generally not covered in other FRCS revision books such as ethics, consent, and negligence. As such it will be of interest to clinicians in related specialties, including dermatology, oral surgery, ENT, and plastic surgery.

Training in oral and maxillofacial surgery is lengthy and demanding, requiring dual qualification and basic training in dentistry and medicine, followed by completion of a higher surgical training programme. The ‘exit examination’ leading to the awarding of the specialty Fellowship of the Royal College of Surgeons in Oral and Maxillofacial Surgery (FRCS(OMFS)) represents the final academic step of training in the specialty. For part 2 of the FRCS(OMFS) examination, one day is dedicated to ‘vivas’, where the candidate is interviewed by two senior clinicians in their diagnosis and management of conditions in the specialty. As such it encompasses the entire remit of Oral and Maxillofacial Surgery, including orthognathic surgery, craniomaxillofacial trauma, surgical oncology and dermatology, oral medicine, craniofacial surgery, dentoalveolar surgery, temporomandibular joint disorders, salivary gland disease, local flap and free tissue transfer, oral implantology and cleft surgery. This daunting syllabus is covered in four viva sessions of thirty minutes, with six five-minute vivas in each session. The aim of this book is to give candidates a structure as to how to answer such scenarios as well as provide clinical subject matter encompassing the range of the specialty. This includes pertinent scenarios that are generally not covered in other FRCS revision books such as ethics, consent and negligence. As such it will be of interest to clinicians in related specialties, including dermatology, oral surgery, ENT, and plastic surgery.

The 3rd Edition of The Oxford Textbook of Geriatric Medicine brings together specialists from across the globe to provide every physician and health care provider involved in the care of older people with a comprehensive resource on the medical, social, and psychological issues they are likely to encounter in their practice and research. Beyond these issues, this comprehensive text provides insights into global population ageing, ageing-relevant policy developments, healthy ageing, lifecourse, multimorbidity, personalised and person-centred care.New material has been added throughout with a strong focus on integrating the impact of age-related physiological and cellular changes with the development of age-related diseases and conditions. Sections on sarcopenia, nutritional health, frailty and related geriatric syndromes have been expanded. Geriatric care principles from public health, primary and specialized care have also been updated and expanded. New models of care in general medicine and surgery and related sub-specialties, outpatient and emergency care, rehabilitation, oncology, palliative medicine and long-term care relevant to older adults are discussed in detail. In summary, the 3rd Edition of The Oxford Textbook of Geriatric Medicine 3e articulates important new global demographic trends and clinical practice patterns, the scientific basis of age-related diseases and conditions, and the ethical, legal, and socioeconomic concerns for healthcare policy and systems relevant to older adults around the globe.
