The purpose of the study: analysis of the risk assessment of cardiovascular events using the SCORE/SCORE2 scales in patients at the outpatient stage. Design: comparative study. Materials and methods. A retrospective analysis of 140 electronic medical records of outpatients was conducted. The criteria for inclusion in the study were: the age of respondents over 40 years old, the absence of concomitant cardiovascular diseases, type 2 diabetes. The following indicators were analyzed in patients: the level of total cholesterol, blood pressure, on the basis of which the risk of cardiovascular events was calculated using the SCORE and SCORE2 scales, for older people — SCORE2-OP. A comparative assessment of the results was carried out. Results. Statistically significant differences were identified between the values obtained when assessing cardiovascular events using SCORE (Me men = 9.5 points, Me women = 3 points, p < 0.001) and SCORE2 (Me men = 22 points, Me women = 18 points, p = 0.049) in the age group from 50 to 70 years. It was found that most patients have a moderate risk of developing cardiovascular complications according to the SCORE scale, and in accordance with the updated SCORE2 algorithm - a very high risk. Conclusion. A comparison of the 10-year risk of cardiovascular complications calculated using SCORE and the new SCORE2 algorithm revealed significant differences between them. Patients are at very high risk of complications (according to the SCORE2 questionnaire), which absolutely increases the risk of complications. Significantly higher values for SCORE2, SCORE2-OP can be associated with indicators such as blood pressure levels and lipid profile indicators. In the Russian Federation, disease prevention in primary care is a priority area of medical care for the adult population. An automated assessment of the risk of death from cardiovascular disease can be an auxiliary preventive tool if integrated intoa medical information system. P u r p o s e o f t h e s t u d y – to substantiate the automation of the process of assessing the risk of death from cardiovascular diseases over the next 10 years using the SCORE scale at the level of a city hospital. M a t e r i a l s a n d m e t h o d s . The study was carried outat the State Budgetary Medical Institution of Arkhangelsk region “Arkhangelsk City Clinical Hospital № 7”. The completeness and quality of electronic patient records were assessed. The SCORE calculator was integrated into the examination protocol for primary care physicians. The effects of the innovation on the completeness and quality of electronic patient records were assessed. R e s u l t s a n d d i s c u s s i o n . Among the served population aged 40 to 79 years, any information on risk factors for cardiovascular diseases was reflected in 28,7% of electronic patient records. Data on all variables used to assess the risk on the SCORE scale (gender, age, systolic blood pressure, smoking, cholesterol level) were present for 20,8% of the patients. The introduction of the SCORE automated calculator into the examination protocol of a general practitioner and a cardiologist made it possibleto increase the completeness of data on cardiovascular disease risk factors used for the risk assessment on the SCORE scale to 97,4%. C o n c l u s i o n s . The introduction of the SCORE automated calculator has significantly increased the completeness of data on the main risk factors for cardiovascular diseases in patients’ electronic patient records. The innovation makes it possible to reduce labor costs forestimatingthe risks, allowsmore effective identification of persons subject to dispensary observation.

Background The authors performed a prospective study to determine whether breath test analysis using an electronic nose correlates with a clinical pneumonia score. Methods Exhaled gas was sampled from the expiratory limb of the ventilator in mechanically ventilated surgical intensive care patients and assayed with the electronic nose. Components of a clinical pneumonia score were recorded concurrently. Results The score predicted by the electronic nose showed good correlation with the actual pneumonia score (r = 0.81). Bland Altman analysis showed a mean bias of 0.0 (limits +/- 2.6). Conclusions The electronic nose is a new biosensor technology that correlates with a clinical pneumonia score.

Summary Objectives: Evidence-based clinical scores are used frequently in clinical practice, but data collection and data entry can be time consuming and hinder their use. We investigated the programmability of 168 common clinical calculators for automation within electronic health records. Methods: We manually reviewed and categorized variables from 168 clinical calculators as being extractable from structured data, unstructured data, or both. Advanced data retrieval methods from unstructured data sources were tabulated for diagnoses, non-laboratory test results, clinical history, and examination findings. Results: We identified 534 unique variables, of which 203/534 (37.8%) were extractable from structured data and 269/534 (50.4.7%) were potentially extractable using advanced techniques. Nearly half (265/534, 49.6%) of all variables were not retrievable. Only 26/168 (15.5%) of scores were completely programmable using only structured data and 43/168 (25.6%) could potentially be programmable using widely available advanced information retrieval techniques. Scores relying on clinical examination findings or clinical judgments were most often not completely programmable. Conclusion: Complete automation is not possible for most clinical scores because of the high prevalence of clinical examination findings or clinical judgments – partial automation is the most that can be achieved. This thesis is concerned with computational methods for alignment and score-informed transcription of piano music. Firstly, several methods are proposed to improve the alignment robustness and accuracywhen various versions of one piece of music showcomplex differences with respect to acoustic conditions or musical interpretation. Secondly, score to performance alignment is applied to enable score-informed transcription. Although music alignment methods have considerably improved in accuracy in recent years, the task remains challenging. The research in this thesis aims to improve the robustness for some cases where there are substantial differences between versions and state-of-the-art methods may fail in identifying a correct alignment. This thesis first exploits the availability of multiple versions of the piece to be aligned. By processing these jointly, the alignment process can be stabilised by exploiting additional examples of how a section might be interpreted or which acoustic conditions may arise. Two methods are proposed, progressive alignment and profile HMM, both adapted from the multiple biological sequence alignment task. Experiments demonstrate that these methods can indeed improve the alignment accuracy and robustness over comparable pairwise methods. Secondly, this thesis presents a score to performance alignment method that can improve the robustness in cases where some musical voices, such as the melody, are played asynchronously to others - a stylistic device used in musical expression. The asynchronies between the melody and the accompaniment are handled by treating the voices as separate timelines in a multi-dimensional variant of dynamic time warping (DTW). The method measurably improves the alignment accuracy for pieces with asynchronous voices and preserves the accuracy otherwise. Once an accurate alignment between a score and an audio recording is available, the score information can be exploited as prior knowledge in automatic music transcription (AMT), for scenarios where score is available, such as music tutoring. Score-informed dictionary learning is used to learn the spectral pattern of each pitch that describes the energy distribution of the associated notes in the recording. More precisely, the dictionary learning process in non-negative matrix factorization (NMF) is constrained using the aligned score. This way, by adapting the dictionary to a given recording, the proposed method improves the accuracy over the state-of-the-art.

In this analysis, we use National Ambulatory Medical Care Survey data to investigate whether the adoption of the ambulatory electronic health records (EHR) system is associated with changes in patient-specific health education prescription rates, patient-physician interaction time, and returned appointment rates. We estimate the treatment effect of EHR adoption with multinomial propensity score weighting adjusted regressions. We find evidence to suggest that full EHR adoption positively affects patient-specific health education prescription rates. We find no robust evidence to show a significant effect of the EHR system on time utilization or returned appointment rates. We discuss possible reasons for our findings. We recommend linking patient education with quality improvement efforts and improving the usability of EHR systems.

In Chapters 1 and 2 there are definitions and a review of electroacoustic music, and then visualization generally and as applied to music. Chapter 3 is a review of specific and relevant literature as regards to the visualization of electroacoustic music. Chapter 4 introduces the concepts of imagining as opposed to discovering new sound, and what is important to this research about these terms; in addition what is meant and indicated by them. Chapter 5 deals with the responses that composers currently working have made to the enquiry concerning visualization. In this chapter these responses are dealt with as case studies. In a similar way, Chapter 6 looks at some examples of historical work in electroacoustic music, again as case studies. In Chapter 7 a taxonomical structure for the use of visualization in electroacoustic composition is established and derived from the case study results. Chapter 8 looks at relevant examples of software and how they offer visualization case studies. Chapter 9 looks at the place of the archive in various stages of the compositional process. Chapter 10 investigates the problems of visualizing musical timbre as possible evidence for future strategies. Chapter 11 offers some conclusions and implications as to the main research questions, as well as more specific outlines of potential strategies for the visualization of electroacoustic music.

In this thesis, the impact of packet losses on the quality of received videos sent across a network that exhibit normal network perturbations such as jitters, delays, packet drops etc has been examined. Dynamic behavior of a normal network has been simulated using Linux and the Network Emulator (NetEm). Peoples’ perceptions on the quality of the received video were used in rating the qualities of several videos with differing speeds. In accordance with ITU’s guideline of using Mean Opinion Scores (MOS), the effects of packet drops were analyzed. Excel and Matlab were used as tools in analyzing the peoples’ opinions which indicates the impacts that different loss rates has on the transmitted videos. Statistical methods used for evaluation of data are mean and variance. We conclude that people have convergence of opinions when losses become extremely high on videos with highly variable scene changes

Before market authorization of new medicines, their efficacy and safety are evaluated using randomized controlled trials. While there is no doubt about the scientific value of randomized trials, they are usually conducted in selected populations with questionable generalizability to routine care.  In the digital data revolution era, with healthcare data growing at an unprecedented rate, drug monitoring in routine care is still highly under-utilized. Although many countries have access to data on prescription drugs at the individual level in ambulatory care, such data are often missing for hospitals. This is a growing problem considering the clear trend towards more new and expensive drugs administered in the hospital setting. The aim of this thesis was therefore to develop methods for extracting data on drug use from a hospital-based electronic health record system and further to build and evaluate models for real-time monitoring of effectiveness and safety of new drugs in routine care using data from electronic health records and regional and national health care registers. Using the developed techniques, we were able to demonstrate drug use and health service utilization for inflammatory bowel disease and to evaluate the comparative effectiveness and safety of antiarrhythmic drugs. With a rapidly evolving drug development, it is important to optimize the evaluation of effectiveness, safety and health economic value of new medicines in routine care. We believe that the models described in this thesis could contribute to fulfil this need.

I det här masterarbetet har jag musik för tre olika musikaliska sammanhang; elektronisk pop i samarbete med kurskollega, kortfilmsmusik i samarbete med filmregissör och musik till statistik i samarbete med SCB. De tre delprojekten rör sig inom olika musikaliska uttryck och har olika instrumentering. En frågeställning som genomsyrar tankegångarna i textens reflektioner är vilka likheter i fråga om uttryck och processer de tre olika verken har även om de har olika estetiska ramar och förutsättningar. Arbetet har genomförts i syfte att undersöka hur de olika inriktningarna påverkar och influerar mig som musikskapare. Går det att se hur flödet av influenser ser ut? Vidare tar jag upp olika föreställningar om att som enskild musikskapare arbeta inom olika inriktningar och medieuttryck. Metoden har utgjorts av kompositionsarbete, improvisation, musikproduktion och studioinspelningar. Resultatet av arbetet är tre olika musikverk varav två; kortfilmsmusik och musik till statistik, är en sorts beställningsverk med målet att spelas i den offentliga kulturen.
In this master dissertation, I have created music for three different musical contexts; electronic pop in collaboration with a program colleague from KMH, short film music in collaboration with film director and finally music to statistics in collaboration with Statistics Sweden (SCB. The three sub-projects move within different musical expressions and have different instrumentation. An issue that is central in the reflections of the text is what similarities in terms of expression and processes the three different works have, although they have different aesthetic frameworks. The work aims to find out how the different directions affect and influence me as a music creator. Can you see what the flow of influences looks like? Furthermore, I discuss different conceptions of working as an individual music creator in different directions and media expressions. The method has consisted of composition work, improvisation, music production and studio recordings. The result of the work is three different pieces of music, two of which; short-film music and music for statistics, is a kind of commissioned work with the goal of being played in the public culture.

Transfigurations is a symphonic work in one movement for orchestra and live computer processing utilizing the graphical audio programming language Pure Data. The score and patch for this piece are accompanied by an essay describing the audio processing techniques and the compositional processes employed in this work. Programming methods discussed include strategies for data capture, patch structure, user interface, and processor management. All audio processing in the work is realized in realtime. These sounds are derived directly from the orchestra in performance, except for the last. The processes involved in Transfigurations include pitch and amplitude tracking, pitch-shifting, filtering, frequency and amplitude modulation, granular synthesis, delay, and convolution. The final sounds from the computer employ stochastic processes for synthesis which are derived from the germinal materials of the piece. The essay also discusses the aesthetic philosophy and formal structure of the work, principle themes and motives, and formative pitch materials, as well as the compositional processes in each section. The final discourse of the essay considers microphone and loudspeaker setups, patch preparation and leveling, and strategies for rehearsal and performance.

AbstractPre-election logic and accuracy (L$$ { \& }$$ & A) testing is a process in which election officials validate the behavior of voting equipment by casting a known set of test ballots and confirming the expected results. Ideally, such testing can serve to detect certain forms of human error or fraud and help bolster voter confidence. We present the first detailed analysis of L$$ { \& }$$ & A testing practices across the United States. We find that while all states require L$$ { \& }$$ & A testing before every election, their implementations vary dramatically in scope, transparency, and rigorousness. We summarize each state’s requirements and score them according to uniform criteria. We also highlight best practices and flag opportunities for improvement, in hopes of encouraging broader adoption of more effective L$$ { \& }$$ & A processes.

Review question / Objective: Patients who meet the clinical diagnostic criteria of Gilles de la Tourette syndrome (DSM-IV/DSM-V) are included, regardless of race, sex, age, etc. Deep brain electrical stimulation for the treatment of Tourette's syndrome, with no limit on the specific procedure and duration of stimulation. Randomized controlled trials were selected, the language was limited to Chinese and English, and there were no restrictions on race, age, sex and so on. The main outcome indicators were the Yale Global tic severity scale score. Other outcome indicators included the Modified Rush Video Rating Scale score, Beck's Depression Inventory score, State-Trait Anxiety Inventory score, Gilles de la Tourette Syndrome-Quality of Life Scale score and Yale Brown Obsessive Compulsive Scale score. The 2016 Quality Payment Program (QPP) is a Medicare reimbursement reform designed to incentivize value-based care over volume-based care. A core tenet of the QPP is integrated utilization of certified electronic health record technology (CEHRT). Adopting and implementing CEHRT is a resource-intensive process, requiring both financial capital and human capital (in the form of knowledge and time). Adoption can be especially challenging for small or rural practices that may not have access to such capital. In this issue brief, we discuss the role of CEHRT in the QPP and offer policy recommendations to help small and rural practices improve their health information technology (IT) capabilities with regards to participation in value-based care. The QPP requires practices to have health IT capabilities, both as a requirement for a complete performance score and to facilitate reporting. Practices that are unable to implement CEHRT will have difficulty complying with the new reimbursement system, and will likely incur financial losses. We recommend monetary support and staff training to small and rural practices for the adoption of CEHRT, and we recommend assistance to help practices comply with the requirements of the QPP and coordinate with other small and rural practices for reporting purposes.

Review question / Objective: This review aims to (1) determine how pain is assessed or what types of questionnaires are used, (2) examine whether there are differences in pain characteristics between men and women with FMS, and (3) describe how pain is conceptualized or manifested in patients at a qualitative level. In this study, the scoping review method of articles published in the last 5 years (2016-2022) was used. Information sources: The bibliographic search was carried out during the months of February and July 2022. The electronic databases used for the search were PubMed, SCOPUS, CINAHL, Web of Science and Google Scholar. In each of these, an exhaustive search was performed using a combination of Boolean logic and truncations for the following keywords: "pain", "fibromyalgia", "men", "women", "conceptualiza-tion", "manifestation", "score" and “assessment".

Background. Long COVID is characterized by persistent, new, or relapsing symptoms following severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection. A standardized and reliable definition is needed to accurately identify patients with long COVID, and a number of models of care have been developed or proposed to provide the services needed to manage this complex condition. Purpose. The purpose of this Technical Brief is to summarize definitions of long COVID and describe what is known about long COVID models of care, including models currently in use, promising approaches, advantages and disadvantages of models in different populations and settings, barriers and facilitators to implementation, access and equity issues, and needed research. Methods. We performed searches in electronic databases from 2021 to November 2023, reviewed reference lists, searched grey literature sources, and interviewed Key Informants. We described key definitions of long COVID, identified components characterizing different long COVID models of care, developed a framework to categorize models based on these components, described representative practice- and systems-based models of care, and identified future research needs. Findings. We identified five definitions for long COVID based on clinical criteria and one proposed definition based on a summary symptom score. Clinical definitions varied with regard to requirement for documenting acute SARS-CoV-2 infection, timing of onset, and duration of symptoms. One newly proposed definition developed using data from people with symptoms for greater than 6 months is based on exceeding a threshold on a composite symptom score and requires further validation. Based on 49 long COVID models of care described in the literature review or by Key Informants, we identified five key principles of long COVID care: (1) core “lead” team; (2) broad multidisciplinary expertise; (3) broad range of diagnostic and therapeutic services; (4) patient-centered, individualized, and equitable care; and (5) capacity to meet demand. Models of care varied with regard to how they addressed these principles. We developed a framework for describing and categorizing long COVID models of care based on seven key components that varied across models: (1) home department or clinical setting; (2) clinical lead; (3) co-location of other specialties; (4) role of primary care; (5) population managed; (6) use of teleservices; and (7) whether the model was practice- or system-based. Using this framework, we described 10 representative practice-based and 3 systems-based long COVID models of care. There was overlap between model components as well as variability within the same model. Across models, implementation strategies addressed multispecialty collaboration, use of systematic intake and assessment methods, care coordination, and education and training of clinic staff. viii Research is needed to understand appropriate methods for measuring quality of care, approaches for providing access to underserved populations, strategies to ensure sustainability, provision of long COVID care in areas lacking multidisciplinary expertise, optimal education and training and care coordination methods, outcomes of long COVID models of care, and strategies for integrating long COVID management into primary care. Decisions about long COVID models of care may best be tailored to address the unique milieu of each implementation setting, leveraging the resources and expertise available. Conclusions. Definitions of long COVID vary and efforts are ongoing to develop a more standardized and reliable definition. A framework based on key model components may be useful to describe and categorize different long COVID models of care. Research is needed to clarify optimal long COVID models of care in different settings and to understand effective strategies for overcoming implementation barriers, including integration of long COVID management in primary care. The models of care presented in this Technical Brief may help inform the individualized implementation of long COVID models of care in different settings.

Review question / Objective: Research Question: How is health information management carried out and how have electronic health records, interoperability and information exchange, and public information sharing developed? The scope review aims to (1) provide an overview, bibliometric, on Health communication and sustainability, (2) pointing out the thematic trends, research topics, theories, methods presented in the studies, countries and authors, (3) contribute to the development of emerging issues and opportunities in global social public relations in order to identify gaps in the literature and guide future research directions. Condition being studied: A scoping review of how academia identifies relevant topics in Health communication and public relations contributes to better decision making in ways that are relevant at the national, organizational, and workforce levels. It also provides new insights for policy makers and a better sustainable global understanding of the topic.

«The electronic magazine with rating system of an estimation of individual and collective work of students» (EM) is developed in document Microsoft Excel with use of macros. EM allows to automate all the calculated operations connected with estimation of amount scored by students in each form of the current control. EM provides automatic calculation of rating of the student with reflection of a maximum quantity of the points received in given educational group. The rating equal to “1” is assigned to the student who has got a maximum quantity of points for the certain date. For the other students the share of their points in this maximum size is indicated. The choice of an estimation is made in an alphabetic format according to requirements of the European translation system of test units for the international recognition of results of educational outcomes (ECTS - European Credit Transfer System), by use of a corresponding scale of an estimation. The list of students is placed on the first page of magazine and automatically displayed on all subsequent pages. For each page of magazine the optimal size of document printing is set with automatic enter of current date and time. Owing to accounting rate of complexity of task EM is the universal technical tool which can be used for any subject matter.

The article presents the implementation of future agricultural engineers’ training technology in the informational and educational environment. To train future agricultural engineers, it is advisable to form tutorials for the study of each discipline in the conditions of informational and educational environment. Such tutorials are an assistance in mastering both theoretical material and course navigation, where interactive electronic learning tools are presented to perform tasks in the informational and educational environment. Higher education applicants perform such tasks directly in the classroom with the help of gadgets or personal computers. The final grade is formed from the scores obtained in the classroom and the rating of higher education applicants while studying in the informational and educational environment. The outlined approach is able to help in the quality of learning content. The use of interactive audiovisual online tools allows to get acquainted with the theoretical, practical and experimental provisions clearly, it is important for the training of future agricultural engineers. At the end of the experiment, it can be argued that the developed technology increases the level of motivation and self-incentive to work in the informational and educational environment. The application of the presented technology provides an opportunity to combine the educational process in the classroom with learning in the informational and educational environment, forms analytical abilities and competencies in professional activity. The reliability of the obtained results was checked using the λ Kolmogorov-Smirnov criterion. It is determined that when using this technology in the educational process, the indicators in the experimental group increased, which displays the effectiveness of training bachelors in agricultural engineering in the conditions of informational and educational environment.

To identify meta-analyses for inclusion, we considered peer-reviewed literature published between 2000 and 2020; we searched five education-focused electronic databases, scanned the table of contents of six special education journals, reviewed the curriculum vitae of researchers who frequently publish meta-analyses in mathematics and special education, and scanned the reference lists of meta-analyses that met inclusion criteria. To be included in this systematic review, meta-analyses must have reported on the effectiveness of mathematics-focused interventions, provided a summary effect for a mathematics outcome variable, and included school-aged participants with or at risk of having a disability. We identified 22 meta-analyses for inclusion. We coded each meta-analysis for 53 quality indicators (QIs) across eight categories based on recommendations from Talbott et al. (2018). Overall, the meta-analyses met 61% of QIs and results indicated that meta-analyses most frequently met QIs related to providing a clear purpose (95%) and data analysis plan (77%), whereas meta-analyses typically met fewer QIs related to describing participants (39%) and explaining the abstract screening process (48%). We discuss the variation in QI scores within and across the quality categories and provide recommendations for future researchers so that reporting in meta-analyses may be enhanced. Limitations of the current study are that grey literature was not considered for inclusion and that only meta-analyses were included; this limits the generalizability of the results to other research syntheses (e.g., narrative reviews, systematic reviews) and publication types (e.g., dissertations).
