Jacob Israël de Haan was romanschrijver, dichter en jurist. In Nederland was hij bekend als schrijver van Pijpelijntjes (1904), een roman waarin openlijk homoseksualiteit werd beschreven. Hij bekeerde zich tot het zionisme en emigreerde in 1919 naar Palestina als correspondent voor het Algemeen Handelsblad. Zijn feuilletons schetsen een levendig beeld van de politieke situatie en het leven van alledag in het nieuwe Joods Nationaal Tehuis, dat zich dankzij de Balfour Declaratie in Palestina ontwikkelde. In Jeruzalem sloot hij zich echter al snel aan bij de anti- zionistische ultraorthodoxe gemeenschap van rabbijn Chaim Sonnenfeld en bij de Agoedat Israël, de internationale organisatie van orthodoxe Joden. Hij werd hun juridisch adviseur en politiek woordvoerder in hun conflict met de zionistische organisatie. De Haan leverde onder andere kritiek op deze organisatie omdat zij bij de verwezenlijking van het Joods Nationaal Tehuis te weinig rekening hield met de Palestijnse Arabieren, die negentig procent van de bevolking uitmaakten. In juni 1924 werd hij door een zionistische commando vermoord.

This book provides newcomers and more experienced researchers with the very basics of magnetoencephalography (MEG) and electroencephalography (EEG)—two noninvasive methods that can inform about the neurodynamics of the human brain on a millisecond scale. These two closely related methods are addressed side by side, starting from their physical and physiological bases and then advancing to methods of data acquisition, analysis, visualization, and interpretation. Special attention is paid to careful experimentation, guiding the readers to differentiate brain signals from various biological and non-biological artifacts and to ascertain that the collected data are reliable. The strengths and weaknesses of MEG and EEG are presented relative to each other and to other available brain-imaging methods. Necessary instrumentation and laboratory set-ups, as well as potential pitfalls in data collection and analysis are discussed. Spontaneous brain rhythms and evoked responses to sensory and multisensory stimulation are covered and examined both in healthy individuals and in various brain disorders, such as epilepsy. MEG/EEG signals related to motor, cognitive, and social events are discussed as well. The integration of MEG and EEG information with other methods to assess human brain function is discussed with respect to the current state-of-the art in the field. The book ends with a look to future developments in equipment design, and experimentation, emphasizing the role of accurate temporal information for human brain function.

This chapter provides an elementary introduction to the theory and practical application of electroencephalogram (EEG) recording for the purpose of studying neurocognitive processes. It is aimed at readers who have had little or no experience in EEG data collection, and would like to gain a better understanding of scientific papers employing this methodology or start their own EEG experiment. We begin with a definition of EEG, and a summary of the strengths and limitations of EEG-based techniques. Following this is a description of the basic theory concerning the cellular mechanisms underlying EEG, as well as two types of data generated by EEG recording. We then present a brief summary of the equipment necessary for EEG data acquisition and important considerations for presentation software. Finally, we provide an overview of the protocol for data acquisition and processing, as well as methods for quantifying both EEG and event-related potentials data.

Electroencephalography (EEG), a dynamic real-time recording of electrical neocortical brain activity, began in the 1600s with the discovery of electrical phenomena and the concept of an “action current.” The galvanometer was introduced in the 1800s and the first bioelectrical observations of human brain signals were made in the 1900s. Certain EEG patterns were associated with brain disorders, increasing the clinical and scientific use of EEG. In the 1980s, technical advances allowed EEGs to be digitized and linked with videotape recording. In the 1990s, digital data storage increased and computer networking enabled remote real-time EEG reading, which made possible continuous EEG (cEEG) monitoring. Manual cEEG analysis became increasingly labor-intensive, calling for methods to assist this process. In the 2000s, complex algorithms enabling quantitative EEG analyses were introduced, with a new focus on shared activity between rhythms, including phase and magnitude synchrony. The automation of spectral analysis enabled studies of spectral content.

Infraslow electroencephalographic (EEG) activity refers to frequencies below the conventional clinical EEG range that starts at about 0.5 Hz. Evidence suggests that salient EEG signals in the infraslow range are essential parts of many physiological and pathological conditions. In addition, brain is known to exhibit multitude of infraslow processes, which may be observed directly as fluctuations in the EEG signal amplitude, as infraslow fluctuations or intermittency in other neurophysiological signals, or as fluctuations in behavioural performance. Both physiological and pathological EEG activity may range from 0.01 Hz to several hundred Hz. In the clinical context, infraslow activity is commonly observed in the neonatal EEG, during and prior to epileptic seizures, and during sleep and arousals. Laboratory studies have demonstrated the presence of spontaneous infraslow EEG fluctuations or very slow event-related potentials in awake and sleeping subjects. Infraslow activity may not only arise in cortical and subcortical networks but is also likely to involve non-neuronal generators such as glial networks. The full, physiologically relevant range of brain mechanisms can be readily recorded with wide dynamic range direct-current (DC)-coupled amplifiers or full-band EEG (FbEEG). Due to the different underlying mechanisms, a single FbEEG recording can even be perceived as a multimodal recording where distinct brain modalities can be studied simultaneously by performing data analysis for different frequency ranges. FbEEG is likely to become the standard approach for a wide range of applications in both basic science and in the clinic.

The field of continuous electroencephalographic monitoring (cEEG) in the intensive care unit has dramatically expanded over the past two decades. Expansion of cEEG programs led to recognition of the frequent occurrence of electrographic seizures, and complex rhythmic and periodic patterns in various critically ill populations. The majority of electrographic seizures are of nonconvulsive nature, hence the need for cEEG for their identification. Guidelines on when and how to perform cEEG and standardized nomenclature for description of rhythmic and periodic patterns are now available. Quantitative EEG analysis methods depict EEG data in a compressed (hours on one screen) colorful graphical representation, facilitating early identification of key events, recognition of slow, long-term trends, and timely therapeutic intervention. Integration of EEG with other invasive and noninvasive modalities of monitoring brain function provides critical information about the development of secondary neuronal injury, providing a valuable window of opportunity for intervention before irreversible damage ensues.

Electroencephalography monitoring provides a method for monitoring brain function, which can complement other forms of monitoring, such as monitoring of intracranial pressure and derived parameters, such as cerebral perfusion pressure. Continuous electroencephalogram (EEG) monitoring can be helpful in seizure detection after brain injury and coma. Seizures can be detected by visual inspection of the raw EEG and/or processed EEG data. Treatment of status epilepticus can be improved by rapid identification and abolition of seizures using continuous EEG. Quantitative EEG can also be used to detect brain ischaemia and seizures, to monitor sedation and aid prognosis.

De combinatie van GPS-metingen met geoïdehoogteverschillen levert orthometrische hoogteverschillen op die kunnen worden gebruikt als controle op, of vervanging van waterpasmetingen. Omdat voor deze orthometrische hoogten (NAP-hoogten) voor veel toepassingen een cm-precisie gewenst is, dient ook de geoïde op dit precisie-niveau bekend te zijn. Om de geoïde zo precies te bepalen is een drietal zaken van belang: om te beginnen de dichtheid en kwaliteit van de beschikbare zwaartekrachtdata, daarnaast de kwaliteit van de theoretische oplossing van het randvoorwaardeprobleem voor de bepaling van de geoïde, en bovendien is, voor de praktische geoïdeberekening, de kwaliteit van de rekenmethoden, uit eerder genoemde theoretische oplossing, van belang. De beschrijving van deze probleemgebieden komt in dit proefschrift aan de orde, met als doel de daadwerkelijke geoïdeberekening voor Nederland. Allereerst richten we ons op de techniek van geoïdebepaling met behulp van de Stokes oplossing voor het randvoorwaardeprobleem. Hier worden de numerieke integratie van Stokes formule en de kleinste-kwadraten collocatiemethode beschreven en met elkaar vergeleken. Er wordt getoond dat beide methoden voor praktische berekeningen niet dezelfde resultaten opleveren. Voor de bepaling van empirische covariantiefuncties, die nodig zijn voor collocatie en foutberekeningen, wordt een nieuwe techniek voorgesteld. De praktische geoïdeberekening zal worden uitgevoerd door een combinatie van globale zwaartekrachtinformatie uit een geopotentiaalmodel en regionale zwaartekrachtmetingen en gemiddelde zwaartekrachtwaarden. De verschillende mogelijkheden van combineren via kernfunctiemodificaties worden daartoe op een originele manier geïntroduceerd. Tevens wordt een formele foutvoortplanting van zwaartekrachtdata naar geoïdehoogte(verschillen) behandeld. Vervolgens worden de data behandeld die beschikbaar zijn voor de geoïdebepaling voor Nederland. Aan het nieuw gemeten zwaartekrachtnet voor Nederland wordt uitgebreid aandacht geschonken, waarna een analyse en vergelijking volgen van de beschikbare (oude en nieuwe) zwaartekrachtdatasets. Daarna wordt beschreven hoe uit de gemeten puntzwaartekrachtwaarden optimaal de gemiddelde blokwaarden kunnen worden bepaald, en hoe de foutberekening daarbij verloopt. Ten slotte wordt de overige informatie beschreven die van belang is voor de geoïdeberekening: het geopotentiaalmodel, geoïdehoogten uit de combinatie van GPS- en waterpasmetingen, en schietloodafwijkingen. Deze laatste twee leveren onafhankelijke geoïde-informatie. Op basis van de beschikbare zwaartekrachtdata worden diverse geoetests gedaan. Hierbij wordt ingegaan op de invloed van predictie-parameters van gemiddelde zwaartekrachtblokwaarden, de verschillende kernfunctiemodificaties en de evaluatietechnieken. Tevens wordt een vergelijking met onafhankelijke geoïde-informatie uitgevoerd. Alvorens de geoïdeberekening te kunnen uitvoeren wordt de kwaliteit van de Stokes oplossing voor geoïdebepaling nader bestudeerd, waarbij we ons richten op de Nederlandse situatie. We kijken naar een betere oplossing voor het randvoorwaardeprobleem door Molodenskii, en naar de invloed van atmosfeeraantrekking en ellipsoïdische correcties. Hieruit blijkt dat het voor Nederland mogelijk moet zijn om cm-precisie te bereiken voor geoïdehoogteverschillen. Het proefschrift wordt afgesloten met de beschrijving van de daadwerkelijke geoïdeberekening voor Nederland. De procedure die is gevolgd is een combinatie van de methoden van Meissl en Wong&Gore met L = 32. Er wordt gebruik gemaakt van het OSU91A geopotentiaalmodel en de nieuwe Nederlandse data en overige Europese data in een gebied van 5o rondom Nederland, waarmee 3'x5' gemiddelde waarden zijn bepaald. Tevens is de formele foutvoortplanting uitgevoerd. Vervolgens wordt een vergelijking met GPS- en waterpasmetingen, en met schietloodafwijkingen gemaakt. Hiermee wordt een correctievlak bepaald om tot de best mogelijke geoïde voor Nederland te komen. Tenslotte wordt de berekende WGS84-geoïde ook getransformeerd naar het lokale Nederlandse Bessel-referentiesysteem. Het blijkt dat de geoïde binnen Nederland met een precisie van 1 tot enkele cm is bepaald.

This chapter examines the technical issues in modelling health state valuation data. Most measures of health define too many states to directly value all of them (e.g. SF-6D defines 18,000 health states). The solution has been to value a subset and by using modelling to predict the values of all states. This chapter reviews two approaches to modelling: one using multiattribute utility theory to determine health values given an assumed functional form; and the other is using statistical modelling of SF-6D preference data that are skewed, bimodal, and clustered by respondents. This chapter examines the selection of health states for valuation, data preparation, model specification, and techniques for modelling the data starting with ordinary least squares (OLS) and moving on to more complex techniques including Bayesian non-parametric and semi-parametric approaches, and a hybrid approach that combines cardinal preference data with the results of paired data from a discrete choice experiment.

This chapter gives an overview of approaches used to understand the generation of electroencephalographic (EEG) signals using computational models. The basic concept is that appropriate modeling of neuronal networks, based on relevant anatomical and physiological data, allows researchers to test hypotheses about the nature of EEG signals. Here these models are considered at different levels of complexity. The first level is based on single cell biophysical properties anchored in classic Hodgkin-Huxley theory. The second level emphasizes on detailed neuronal networks and their role in generating different kinds of EEG oscillations. At the third level are models derived from the Wilson-Cowan approach, which constitutes the backbone of neural mass models. Another part of the chapter is dedicated to models of epileptiform activities. Finally, the themes of nonlinear dynamic systems and topological models in EEG generation are discussed.

Quantitative electroencephalogram (qEEG) is the transformation of the EEG by spectral analysis in which the amount of electrical activity at a particular frequency is determined and compared against a normative data base. EEG findings are specific for different psychiatric problems and help reveal brain abnormalities associated with psychological symptoms. Repetitive transcranial magnetic stimulation (rTMS) is a system of delivering multiple pulses within a short time period that induce changes that outlast the stimulation period. Operant conditioning involves providing a reward to increase the probability of a certain behavior. Neurofeedback involves recording, analyzing, and presenting results of qEEG analyses in near real-time to patients in order to promote changes in brain electrical activity. This chapter reviews how neuromodulation works both clinically and from a neurophysiological perspective. This chapter also reviews current clinical data on the use of neuromodulation approaches for improving mental health and well-being.

This chapter provides a broad definition of surveillance in relation to health protection, including infections, environmental hazards, and health delivery. Surveillance systems include active, passive, and sentinel surveillance, and guidelines are provided for assessing the quality of surveillance data. Surveillance tools include the statutory notification of diseases, laboratory reporting, and prevalence surveys. In addition, syndromic surveillance, the (near) real-time collection of data has become increasingly used, facilitated by increasing use of digital data collection within health care settings and the availability of other digital data sources (e.g. social media). Other surveillance types include event-based surveillance (e.g. during major sporting events), and environmental surveillance, including food related disease, air pollution and chemical hazards. The chapter finally brings these together in integrated surveillance and the use of such surveillance in health planning and assessment.

In axial spondyloarthritis (axSpA) there is little evidence to support use of classical synthetic disease-modifying antirheumatic drugs (DMARDs), with the majority of studies performed in advanced ankylosing spondylitis. Sulfasalazine is the best investigated DMARD in axSpA. Its positive clinical effect, if any, seems to be more prominent in the presence of peripheral arthritis, although a certain proportion of patients with axial disease might benefit from sulfasalazine therapy. Available data indicate that there is no evidence that methotrexate might be effective in axial disease, and only marginal evidence exists in support of methotrexate use in case of peripheral involvement. No true disease-modifying properties (e.g. retardation of structural damage progression in the spine) have been demonstrated for DMARDs in axSpA to date. Efficacy of a combination therapy (e.g. methotrexate plus sulfasalazine) as well as benefits of methotrexate (or other DMARDs) in addition to tumour necrosis factor α inhibitors in axSpA remain uncertain.

Epilepsy syndromes of childhood are a heterogeneous group of disorders that occur at specific neurodevelopmental stages, with a variable prognosis ranging from benign to catastrophic. In clinical practice they are categorized based on seizure type, age of onset, clinical presentation, electroencephalographic (EEG) findings, as well as response to treatment. In addition, recent advancements in neuroimaging and genetic testing have become important diagnostic tools revealing underlying defects for some of these syndromes. This knowledge has consequences for clinical practice, as it opens new perspectives for early diagnosis, prognosis and treatment. Here, we provide an up-to-date overview of the most common pediatric epilepsy syndromes, their clinical findings, associated EEG findings, and clinical management.

De wetenschappelijke beoefening van de neerlandistiek heeft ook buiten het Nederlandse taalgebied een lange geschiedenis. In sommige landen waren er al in de eerste helft van de negentiende eeuw taal- en letterkundigen die zich met de Nederlandse taal en de Nederlandstalige literatuur bezighielden. De geschiedenis van de internationale neerlandistiek moet tot op heden nog geschreven worden. Deze bundel met artikelen, die teruggaat op een congres dat in 2008 aan de Freie Universität Berlin gehouden werd, wil daartoe een eerste aanzet geven. De artikelen, die in het Nederlands geschreven zijn en van een Duitse en een Engelse samenvatting vergezeld gaan, geven een goede indruk van een vakgebied dat in een groot aantal Europese landen niet alleen op een lange traditie kan bogen, maar vandaag de dag bovendien ook rijkgeschakeerd en springlevend is.

Clinical Electromyography in Clinical Practice provides case-based learning of clinical Electromyography (EMG) with a main mission of reducing the gap between theory and practice in the field of electrodiagnostic medicine. The book format includes four introductory chapters that acquaint the discipline and scope of the EMG Examination to the beginners. This include chapters on nerve conduction studies, needle EMG, and specialized testing including late responses, repetitive nerve stimulation and single fiber EMG. Discussion on the electrodiagnostic and clinical EMG findings in the numerous neuromuscular disorders including anterior horn cell disorders, peripheral neuropathies, neuromuscular junction disorders and myopathies. The relevant technical aspects and normative data are discussed along with the personal characteristics needed. Sections cover the investigative strategies that need to be employed, the hereditary and acquired conditions that are common reasons for referral for electromyography (EMG), following an anatomical template, and conditions in which EMG has a particular role. The use of stimulation single fibre EMG in the diagnosis of neuromuscular junction disorders in discussed in particular detail. This chapters highlights the changing role of EMG but emphasises that it still has an important role in the management of pediatric neuromuscular disorders.

Psychische en neurologische stoornissen en aandoeningen veroorzaakt door middelengebruik (MNS-aandoeningen) komen veel voor en veroorzaken over de hele wereld een grote ziektelast en invaliditeit. Om de kloof tussen de beschikbare middelen en de grote vraag naar hulpverlening te overbruggen, heeft de Wereldgezondheidsorganisatie het Actieprogramma voor de Geestelijke Gezondheid (mhGAP) gelanceerd. Het doel van mhGAP is om zorg en diensten op te schalen met behulp van evidence-based interventies voor de preventie en behandeling van prioritaire MNS-aandoeningen. De mhGAP-interventiegids versie 1.0 voor MNS-aandoeningen voor niet-gespecialiseerde zorginstellingen werd in 2010 ontwikkeld als een eenvoudig technisch hulpmiddel om de geïntegreerde behandeling van prioritaire MNS-aandoeningen mogelijk te maken met behulp van protocollen voor klinische besluitvorming. De mhGAP-IG 1.0-versie wordt gebruikt in meer dan 90 landen en is een groot succes. Het is een genoegen om de mhGAP versie 2.0 te presenteren, met nieuwe informatie over begeleiding gebaseerd op bewijzen, verbeterde bruikbaarheid en nieuwe onderdelen die door zowel zorgverleners als programmamanagers kunnen worden gebruikt voor een nog groter bereik. We hopen dat deze gids een richtlijn zal blijven bieden voor het leveren van zorg en diensten aan mensen met MNS-stoornissen over de hele wereld en dat dit ons een stap dichterbij ons doel van universele gezondheidsdekking brengt.

Until recently, borderline personality disorder (BPD) has been the stepchild of psychiatric disorders. Many researchers even questioned its existence. This chapter reviews several major themes in the neuroimaging of ASDs to date (see summary of representative themes in Table 3.1), including substantial and essential contributions from the modular framework. The chapter begins, however, with a discussion of several challenges related to the diversity of ASDs in terms of factors such as age, level of functioning, and symptom presentation. Progress in the ability to identify more homogenous subgroups, based on targeted phenotypic measures, opens the door to link neuroimaging with genetics findings and also with treatment outcome data. This should lead to better understanding of both the causes of ASDs and the best approaches to intervention. The chapter is divided according to two broad, related themes related to social information processing and cognitive factors in ASDs. Within these themes, the chapter considers evidence from both structural and functional imaging studies as well as relatively newer approaches to connectivity, including diffusion tensor imaging. The primary focus of this chapter is on research utilizing functional magnetic resonance imaging (fMRI) and electroencephalography (EEG). Although several early neuroimaging studies utilized positron emission tomography scanning, these studies are rare now and are not addressed in depth. New techniques such as near-infrared spectroscopy suggest tremendous promise for noninvasive imaging of expanded age groups and severity levels of ASDs; however, these studies are also few in number and are touched on only briefly.

Maximal intensity is any activity where the exercise-supplying metabolism demonstrates a higher anaerobic ATP yield than the oxidative phosphorylation metabolism. Ethical considerations prevent muscle biopsy techniques in young people, resulting in indirect inferences about anaerobic metabolism during exercise being applied to mostly mechanically derived measurements. These measurements are largely based on cycle ergometry tests like the Wingate test. Compared to aerobic data, maximal-intensity data sets are infrequently published, female data across all age ranges are lacking, and application is limited by a focus on sports performance rather than health. However, regardless of how these data are analysed, children and adolescent performance is inferior to adults. Most studies attempt to explain this from a quantitative muscle (and age) perspective, while explanations of qualitative factors, e.g. hormonal and neuromuscular, have proved elusive. Future studies should focus on the mechanisms underpinning maximal-intensity exercise as an important component of everyday physical activity.

In deze bundel bieden dertien oud-promovendi van Harry Kunneman een handzaam overzicht van de belangrijkste bevindingen uit hun dissertatieonderzoek. Bovendien delen zij hun voortschrijdende inzicht rond de - even actuele als weerbarstige - maatschappelijke vragen waar zij in hun professionele praktijken mee geconfronteerd worden. Daarmee biedt deze bundel ook belangrijke richtingwijzers voor werk dat deugt en deugd doet in een breed scala van maatschappelijke domeinen, zoals de politie, het sociale domein, het onderwijs, de zorg, het recht, de ontwikkelingssamenwerking, duurzaamheidswerk en ethische vormen van technologische innovatie. Deze bundel komt voort uit de ‘Werkplaats Normatieve Professionalisering’ waarin wij als oud-promovendi, samen met Harry Kunneman, in gesprek zijn gegaan over de actuele betekenis van onze promotieonderzoeken en als gezellen elkaars hoofdstukken van opbouwend commentaar hebben voorzien. Daarmee vormde ‘waardenwerk’ niet alleen een belangrijk thema in onze gesprekken. We deden ook waardenwerk: we vormden een gezelschap waarin we samen nadachten over vragen die soms heel dichtbij kwamen, elkaar kritisch bevroegen, inspireerden en ondersteunden. We leven in een tijdperk van meningen. Op sociale media, in talkshows op tv en in talloze vlogs en blogs buitelen de meningen over elkaar heen. Iedereen lijkt overal iets van te vinden, en ook te móeten vinden. We staan met ons allen bloot aan een stortvloed van besmettelijke opinitis, die we overigens ook met ons allen veroorzaken. Veel meningen zijn aangepraat en er wordt veel nagepraat, beïnvloed door de bubbel waartoe men behoort of door publieke figuren en influencers die hun mening zó overtuigend presenteren dat grote groepen hun mening voor waar aannemen. De filosofie maakt al sinds de oudheid onderscheid tussen ‘meningen’, die schijnkennis vertegenwoordigen, en ‘waarheid’ op basis van kennis die voortkomt uit denken. Zelfstandig oordelen betekent in de eerste plaats onafhankelijk oordelen; onafhankelijk van meningen, drogbeelden en sentimenten. De bijdragen in deze bundel verkennen hoe onafhankelijk oordelen tot stand komt. Hoe kunnen we zélf oordelen over waarheid en geloofwaardigheid? Welke rol spelen anderen daarbij? Nabije anderen, maar ook bubbel-overstijgende inspirerende denkers, historische of fictionele figuren, die uitnodigen tot nieuwe interpretaties en ideeën. En hoe krijgt een oordeel gezag? Is dat door imiterende herhaling, door het volume van de spreker, door slaafse volgzaamheid? Of door eigen, zelfstandige interpretatie? In Opinitis gaan filosofen, theologen, cultuurhistorici en publicisten in op bovenstaande vragen. Zij onderzoeken vanuit verschillende invalshoeken hoe onze oordelen tot stand komen, hoe we ons verhouden tot de macht van de media en de invloed van de meerderheid, en hoe we aan onze eigen bubbel kunnen ontsnappen.

‘Wat werkt’ voor de jeugdzorg, de maatschappelijke opvang, ggz, gezinshuizen, de forensische psychiatrie, gehandicaptenzorg, het speciaal (passend) onderwijs en gemeenten. Nederland is een welvarend land en onze kinderen behoren zelfs tot de meest gelukkige ter wereld. Toch geldt dat niet voor iedereen. Sommige kinderen en volwassenen hebben een biologische, psychologische en sociale kwetsbaarheid en een mindere weerbaarheid. Leefklimaat! geeft concreet werkende handvatten voor het verbeteren van de zorg aan deze groep. Het zijn geen magische toverstokjes, maar we weten inmiddels wel wat echt werkt. Deze kinderen en volwassenen hebben negatieve jeugdervaringen meegemaakt waarvan de effecten stapelen en soms leiden tot ervaringen die de grond onder hun voeten doet wegvallen, de zogenaamde ‘contrastervaringen’. Het gevolg is veel pijn, een verstoorde ontwikkeling en probleemgedrag, crimineel gedrag of ‘verward gedrag’. Hoe buig je dit om naar verbetering en herstel en creëer je een positief leef-, leer – en/of werkklimaat? De afwijzing van de maatschappij leidt tot verdere frustratie van psychologische basisbehoeften zoals verbondenheid, competentie of zingeving en autonomie. Het boek laat zien hoe je aan de hand van deze zgn. zelfdeterminatietheorie weer aan deze basisbehoeften kunt voldoen. Verder gaat het boek in op tal van belangrijke onderwerpen als: separeren, suicide, DIS, eetproblemen, LHBT, LVB, crisisplaatsingen, teamcoaching etc. Daarnaast is het zorglandschap de afgelopen jaren ingrijpend gewijzigd. Dit heeft ertoe geleid dat in toenemende mate kinderen en volwassenen met zeer ernstige problematiek in instellingen, scholen en gezinshuizen terecht komen, waar veel professionals nog weinig ervaring mee hebben. Leefklimaat! biedt alle (aanstaande) professionals die deze zorg willen verbeteren belangrijke kennis en tools. Vanuit de Universiteit Utrecht werd een grote groep jonge veelplegers jarenlang gevolgd. Zie Weijers (2019) 'Veelplegers aanpakken' en Weijers (2022) 'Kansen bieden, kansen pakken'. Centraal in deze longitudinale studie stonden de vragen wat jonge veelplegers doet stoppen en wat de beste aanpak is van deze jongeren. Twee belangrijke bevindingen zijn dat er verschillende fasen in het proces van stoppen met een criminele levenswijze kunnen worden onderscheiden en dat elke fase een eigen geheel van interventies op het gebied van straf en zorg vraagt. Naar aanleiding van de presentatie van de uitkomsten van deze studie ontstond vanuit het veld de vraag naar een methodiek om deze verschillende fasen in het proces van desistance from crime in kaart te kunnen brengen. Daartoe is speciaal voor dit doel een Vragenlijst Veelplegers samengesteld. Allereerst wordt een korte samenvatting gegeven van de vier fasen zoals die in genoemde studie zijn beschreven. Vervolgens wordt een beknopte handleiding voor het gebruik van de vragenlijst gegeven, gevolgd door de Vragenlijst Veelplegers zelf.

Autism spectrum disorder (ASD) is defined by impairments in social communication, along with the presence of restricted and repetitive behaviors. Approximately half of affected children also suffer from intellectual disability (ID), which is defined by cognitive and adaptive functioning at least two standard deviations below the mean (e.g., standard scores <70). It is now well established that genetic factors are major contributors to both ASD and ID. The following chapter will outline emerging themes and integrate data from human and animal studies using genetic, imaging, and histopathological methods. Emerging findings from studies of the immune system and the microbiome will also be briefly reviewed.

Substance addiction is a chronic relapsing disorder. It is the manifestation of the long-term pharmacological actions of substances on the receptor mechanisms of the brain’s neural circuitry. There are different neurotransmitter systems (e.g. dopamine, GABA, opioids) and even appetite hormones, acting within this neural circuitry in addiction, but their collective roles in the disorder remain equivocal. Importantly, disturbances to these systems may also pre-date addiction, which leads people to initiate substance abuse (e.g. stress, reward sensitivity). People may also be at an increased risk of initiating substance abuse due to age (e.g. adolescence), where there is an imbalance in the maturation of neural circuits. The pharmacokinetics of addictive drugs also plays a significant role in their abuse potential. Addiction is a recurrent disorder that is difficult to cure. Treatments that possess efficacy in aiding people to remain abstinent and stabilize their lives are the best courses of treatment for addiction disorders.

Multifactorial mechanisms, including varying degrees of polygenic risk, contribute to most child onset psychiatric disorders. Methods to better understand the biological impact of inherited low-risk variation are emerging, and these studies may be useful to develop novel treatments for childhood onset psychiatric disorders. In some neurodevelopmental disorders, specifically autism spectrum disorder (ASD) and intellectual disability (ID), recurrent spontaneously mutated genes have been identified. This leads to the current focus on individual, high-risk targets (e.g., SHANK3, FMR1, MECP2, CHD8) for development of novel treatments. This chapter summarizes and begins to compare neurobiological data from several distinct single gene disorders as a means to guide further therapeutic development based on overlapping pathways of interest.

Some treatments cause sterility, but in quite a number, fertility is unscathed, or at any rate, subsequently recovers. The chapter also references industrial, environmental, and recreational factors. A notable and substantially reassuring conclusion from these data is the apparent dearth of instances of an extrinsic factor having caused a chromosome abnormality in the sperm or egg of an exposed person.

Paediatric Electromyography is a single-author textbook which covers the full range of applications of the techniques of nerve conduction and electromyography (EMG) in children from the neonatal period to the late teenage years. It comprises five sections. Section 1 in its first chapter, gives a detailed introduction to the different skills that are needed to effect successful interventions in paediatric EMG. The emphasis here is that paediatric EMG is not simply adult EMG applied to younger subjects. Its second chapter is an introduction to the basic physiology which is common to any practice of nerve and muscle study. The next three sections (2–4), each comprised of three chapters, are structured anatomically covering in order, nerves, muscles, and neuromuscular junctions. All follow a similar pattern with the first chapter of the section dedicated to the underlying physiology needed for interpretation of the techniques used in the investigation of that particular part of the nervous system. The second chapter gives the pathophysiological associations and the final chapter covers any aspect not covered in the previous two chapters. In section 5 the techniques needed to deal with the more unusual clinical requests, such as investigation of facial palsy, swallowing abnormalities, brachial plexus injuries, and diaphragmatic problems are brought together in a final chapter. The book is concluded with three appendices. Appendix 1 describes protocols devised to cover the differing clinical request sent to any laboratory. Appendix 2 gives a comprehensive database of normative data, often derived from e-norm methodology, and intending to cover every measure recorded. Appendix 3 is an illustrated description of electrode placements for all the common nerve studies.

This chapter summarizes data on sleep–wake disturbances in humans at high altitude and in space. High altitude exposure is associated with periodic breathing and a trend toward reduced slow-wave sleep and sleep efficiency in healthy individuals. Some subjects are affected by altitude-related illness (eg, acute and chronic mountain sickness, high-altitude cerebral and pulmonary edema). Several drugs are available to prevent and treat these conditions. Data about the effects of microgravity on sleep are limited and do not allow the drawing of firm conclusions. Microgravity and physical and psychological factors are responsible for sleep–wake disturbances during space travel. Space missions are associated with sleep restriction and disruption and circadian rhythm disturbances encouraging use of sleep medication. An unexplained and unexpected finding is the improvement in upper airway obstructive breathing events and snoring during space flight.

The clinical presentation of myocarditis is multifaceted and electrocardiogram (ECG) changes as well as biomarkers tend to be non-specific. Therefore, the diagnosis of myocarditis can be challenging and should be based on an integrated approach including patient history, physical examination, non-invasive tests such as ECG and serum biomarkers, and non-invasive cardiac imaging. As myocarditis may lead to global ventricular dysfunction, regional wall motion abnormalities, and/or diastolic dysfunction, echocardiography should be routinely performed. However, hallmarks of acute myocarditis comprise structural changes such as cardiomyocyte swelling, an increase in extracellular space and water content, accumulation of inflammatory cells, potential necrosis or apoptosis of cardiomyocytes, and myocardial remodelling with fibrotic tissue replacement that can be depicted by cardiovascular magnetic resonance. Nuclear techniques are still not routinely recommended for the work-up of myocarditis—with the possible exception of suspected sarcoidosis—due to limited data, limited diagnostic specificity, limited availability, and risk from radiation exposure. This chapter focuses on those non-invasive cardiac imaging techniques that are used in daily clinical practice for work-up of suspected myocarditis. However, as research continues and novel imaging techniques become available, it is hoped that even more accurate and timely diagnosis of myocarditis will be possible in the near future.

Dit is géén Poetinboek. De spontane ontwikkeling van peuters en kleuters staat onder grote druk. Niet alleen pedagogen en leerkrachten zijn bezorgd over de huidige ontwikkelingen binnen het onderwijs aan jonge kinderen, waar leerlingvolgsystemen, test- en compensatieprogrammas de overhand krijgen. Ook medisch specialisten maken zich zorgen. Meer dan vroeger zien zij hun poliklinieken volstromen met gezonde kinderen, bij wie desalniettemin een achterstand in de motorische, sociaal-emotionele of taalontwikkeling wordt verondersteld. Hieruit wordt pijnlijk duidelijk dat de variaties die bij een normaal ontwikkelingsproces horen niet altijd meer erkend worden. In deze bundel met discussiebijdragen van deskundigen uit het onderwijs, de pedagogiek, ontwikkelingspsychologie en medische wetenschappen, wordt aandacht gevraagd voor de eigen positie en identiteit van peuters en kleuters in het onderwijs. Het requiem (of de dodenmis) is een van de oudste en langst overlevende genres uit de westerse muziekgeschiedenis. Honderden jaren heeft het enkel bestaan in zijn oorspronkelijke gregoriaanse gedaante, maar vanaf ca. 1450 hebben heel wat grote componisten zich laten inspireren tot het schrijven van magistrale nieuwe dodenmissen. Dies Irae laat de lezer op toegankelijke wijze kennismaken met deze meesterwerken zonder dat muzikale voorkennis vereist is. Na een inleiding over de betekenis en structuur van de tekst en muziek van de aloude Latijnse dodenmis, volgt een kroniek in drie delen die de lezer door de geschiedenis van het requiem gidst. In aparte hoofdstukken komen alle grote en bekende requiems aan bod (onder meer van Mozart, Brahms, Verdi, Fauré, Britten en Webber), evenals enkele minder populaire maar minstens even aangrijpende werken (zoals de requiems van Ockeghem, Schütz, Weill, Ligeti, Stravinsky en Zimmermann). Elk requiem wordt volgens een vast stramien besproken, zodat de lezer vrijelijk kan grasduinen door verschillende perioden en composities: eerst wordt ieder werk muziekhistorisch gesitueerd, daarna volgt een korte bespreking van elk deel afzonderlijk. Door de besprekingen te koppelen aan één referentieopname per werk, kan de lezer de uitleg meteen ook aan de luisterpraktijk toetsen. Onder de leiding van Pieter Bergé, en met medewerking van Jan Christiaens werden alle bijdragen aan Dies Irae. The economic burden of rheumatic diseases for society, various payers, and last but not least the individual patient has been increasingly recognized. In addition to the well-known impact of back pain and osteoarthritis, the upcoming new and expensive therapies have made this issue especially intriguing in the treatment of rheumatoid arthritis (RA). A mean international estimate of the total annual costs of RA, mainly consisting of direct resource consumption and indirect costs due to productivity losses relating to paid work, comes to about €5600. Other inflammatory rheumatologic diseases (ankylosing spondylitis, psoriatic arthritis, lupus erythematosus) generate similar costs. The implementation of expensive biological drugs in rheumatic care has also led to the pressing need to determine the relation of their costs and clinical outcome (e.g. quality-adjusted life-years, QALYs) in order to compare different treatment strategies in defined patient groups. In RA the health-economic evidence for the cost-effectiveness of biologicals is already quite substantial in terms of treatment of early and advanced RA, as last option treatment of patients refractory to TNF inhibitors. Their cost-effectiveness as first line treatment is less clear. All biologicals have proved their cost-effectiveness in various settings depending on patient selection. It has been clearly demonstrated that adherence to the current guidelines, including monitoring of their effectiveness. leads to cost-effective scenarios. In TNF-refractory RA, abatacept and rituximab have proved to be economically favourable strategies. Economic data on other inflammatory rheumatic entities is relatively sparse. Incomplete long-term and observational data are still the most prominent gaps in health-economic evidence relating to rheumatic disorders.

In the work-up of suspected or known coronary artery disease (CAD), cardiovascular magnetic resonance (CMR) is an established technique and it is recommended by most recent guidelines. Stress dobutamine and stress perfusion CMR yield sensitivities and specificities to detect anatomically defined CAD (>50% coronary stenoses) ranging from 83% to 91% and from 83% to 86%, respectively, with areas under the receiver operating characteristic curve (AUCs) of 0.80–0.93. Multicentre trials report AUCs of 0.75–0.91 to detect CAD and showed superiority over scintigraphic techniques. Increasing evidence in thousands of patients demonstrates the highly predictive value of CMR. Exclusion of ischaemia by CMR goes along with excellent event-free survival rates of 0.5–0.9%/year. Cost analyses in large data sets (e.g. in the European CMR registry), suggest considerable cost savings for CMR over first-line invasive strategies in suspected CAD. Tissue characterization by CMR to detect scar, necrosis, oedema, microvascular obstruction, or haemorrhage is of particular importance in the setting of acute coronary syndromes and this application is emerging as the number of centres offering CMR increases.

This chapter summarizes scoring guidelines for sleep stages, breathing (airflow and respiratory effort), and arousals based on the principles enumerated by the American Academy of Sleep Medicine (AASM). Current established clinical standards are largely based on those published by the AASM and rules implemented by Centers for Medicare and Medicaid services in the USA. These rules include scoring of rapid eye movement (REM) and non-REM (NREM) sleep stages, central nervous system (CNS) arousals, and breathing events. Other clinically relevant polysomnographic events exist (eg, limb movements, bruxism, and electrocardiographic events) but such events are beyond the scope of this chapter. The material presented in this chapter can serve as a framework to provide general information to patients and clinicians about methods of performing the tests and gathering summary data.

Since the start of the international project in 1997, over 1500 patients have received a haematopoietic stem cell transplant (HSCT), mostly autologous, as treatment for a severe autoimmune disease, with overall 85% 5-year survival and 43% progression-free survival. Around 30% of patients in all disease subgroups had a complete response, often durable despite full immune reconstitution. In many cases, e.g. systemic sclerosis, morphological improvement such as reduction of skin collagen and normalization of microvasculature was documented, beyond any predicted known effects of intense immunosuppression alone. It is hoped that the results of the three running large prospective randomized controlled trials will allow modification of the protocols to reduce the high transplant-related mortality which relates to regimen intensity, age of patient, and comorbidity. Multipotent mesenchymal stromal cells (MSC) have been recently tested in various autoimmune diseases, exploiting their immune modulating properties and apparent low acute toxicity. MSC display immune privilege in that the patient requires no immunosuppression prior to allogeneic MSC infusion. Despite encouraging small phase I/II studies, no positive data from randomized prospective studies are as yet available in the peer-reviewed literature.

Attempted and completed suicide among older adults are global public health challenges of escalating significance. This chapter presents epidemiological data on such behaviours, and addresses the risk factors in the domains of mental health, personality, physical health and functioning, social factors (e.g. life events and social support) and neurobiological mechanisms, as derived from retrospective (mainly psychological autopsy [PA] studies) and prospective case-control studies. Suicide prevention initiatives in the elderly have taken a great stride forward in the past decade based on better understanding of the risk and protective factors. Given the complex, multi-determined nature of suicidal behaviours, further improvements will require sustained collaborations across clinicians, researchers, health administrators and politicians in different nations. Furthermore, older males are generally at higher risk of suicide than females; they tend to use more lethal means in their suicide acts, are more susceptible to the effects of bereavement and widowhood, and respond less favourably to comprehensive suicide prevention programs. Further research on the gender differences of suicidal behaviours is urgently needed, to understand the different underlying psychopathological mechanisms, and to adequately address the healthcare needs of older men, the largest group of completed suicides across the world.
