The first histological and clinical description of the optic nerve drusens was given in the 19th century. Then authors considered drusens as congenital pathologies, and a study of 1999 showed that drusen is not the primary pathology, but the dysplasia of the optic nerve disk and its circulation predisposes to the formation of drusen. Central vision in patients with drusens of the optic nerve disc is reduced rarely, and their diagnosis is reduced to ascertaining of the fact of having drusen without taking into account the structural and functional changes in the visual system.Purpose of the work: to identify the pattern of changes in visual functions in children and adolescents with druses of the optic nerve disk, the development and addition of classification criteria.Patients and methods. 45 children with drusens were examined, a comprehensive analysis of the visual system included a standard ophthalmological examination, a comprehensive study of visual system with ultrasound scanning, OCT and OCTA, standard automated and pulsarperimetry.Based on the results of the cluster analysis, a graph of the distribution of patients into three groups (control group and two clinical groups) was constructed, and the structural and functional state of the visual system was analyzed. Drusens in children in the second group we marked as “peripheral”, and in the third — “central”. The carried out researches have shown, that the revealed disturbances are more expressed in the third clinical group. All this determines the need to complement the clinical classification of drusens and their separation depending on the location relative to the vessels of the optic nerve disk to the central and peripheral. The compression effect of drusen is accompanied with changes in the volume of the optic nerve disc, a violation of retinal hemodynamics, damage to the neuroglia and ganglion cells, which allows treating this pathology as a progressive neuroopticopathy, and the revealed dependence of changes on the localization of druses requires supplementing the existing classification.

PurposeTo evaluate the impact of drusen size and structure on retinal pigment epithelium (RPE) and photoreceptor layers in eyes with early to intermediate age-related macular degeneration (AMD) using polarisation-sensitive optical coherence tomography (OCT).DesignRetrospective investigation of an observational cross-sectional study.ParticipantsPatients with early to intermediate AMD.MethodsTwenty-five eyes of 25 patients with drusen were imaged with polarisation-sensitive OCT using macular volume scans. Each scan was manually graded for six distinct drusen characteristics and the integrity of both the overlying RPE and photoreceptor layer. The central scan of each single druse, as well as its diameter and location, were selected for statistical calculations.ResultsA total number of 5933 individual drusen including their adjacent RPE and photoreceptor layer were evaluated. 41.3% of all drusen demonstrated an intact overlying RPE; in 28.1% the RPE layer was irregular, but continuous. In 30.6%, the RPE layer signal was discontinuous above the area of drusen. The level of RPE alteration was significantly related to shape (p<0.001), internal reflectivity (p<0.001) and homogeneity (p<0.001) of the drusen and their diameter, with a higher probability for larger drusen to have a discontinuous RPE (OR 3.2, p<0.001). The number of drusen showing overlying foci or an altered photoreceptor layer was too small to be conclusive, but showed a trend towards an altered RPE if present.ConclusionsPolarisation-sensitive OCT reveals a correlation between specific drusen characteristics and the integrity of the overlying RPE layer. Drusen diameter and configuration were significantly associated with RPE loss.

Purpose: The problem of optic disc drusen (ODD) has been described in detail in several publications. However, less attention has been devoted to real haemodynamic parameters (HP) in ODD. It has been clinically demonstrated that the occurrence and progression of changes in the visual field in ODD are closely linked with the haemodynamics of the vascular supply of the eye – the optic nerve. ODD may visually overlap excavation of the disc of the optic nerve, on the basis of which it is more difficult to evaluate changes (scotomas) in the visual field in the case of glaucoma. Methods: Haemodynamic parameters were prospectively evaluated in 54 patients with compensated intraocular pressure and with optic disc drusens. Drusens in the head of the optic nerve were demonstrated by a fundus examination and B-scan ultrasonography (USG). The drusens were divided into 3 groups according to the size of the individual drusens or drusen complex. Group I: area size up to 1.9 mm. Group II: area size: 1.9-3.9 mm. Group III: area size > 4.0 mm. Flow (haemodynamic) parameters – maximum systolic velocity (MSV), minimum diastolic velocity (MDV), and resistivity index (RI) and pulsatility index (PI) were recorded in the central retinal artery (CRA), in the central retinal vein (CRV), in the temporal and nasal ciliares posteriores arteries breves (CPAb) and in the ophthalmic artery (OA). The values were divided into 1. Physiological: CRA: 8.7 ± 0.9 / 2.9 ± 0.6 cm/s, or RI: 0.70 ± 0.05, 2. Slightly impaired: CRA: 6.6 ± 0.8 / 2.0 ± 0.5 cm/s, or RI: 0.75 ± 0.04. 3. Significantly impaired: CRA: 5.2 ± 1.2 / 1.9 ± 0.7 cm/s, or RI: 0.79 ± 0.03. Results: No linear relationship was demonstrated between the size of the drusens and flow parameters. Slight impairment of HP in the CRA was present in 28.6% of drusens in group I, 48.3% in group II and 62.4% in group III. Significant impairment of HP in the CRA was present in 28.6% of drusens in group I, 48.3% in group II and 62.4% in group III. HP in the CPAb and OA were not of significant importance with regard to the presence and size of the drusens. The relationship between the individual variables was evaluated with the aid of a Pearson correlation coefficient: 0.213, group I P: 0.354, group II P: 0.073, group III P: 0.287. Conclusions: HP are more often impaired in “large” optic disc drusens (group III), rarely in group I ODDs – though this is not an absolute rule. It is not possible to predict haemodynamic parameters according to the size of the drusen formation in the optic nerve. It appears that impairment of the haemodynamic parameters is conditioned not only by the size of the ODD, but also by the locality (distance from lamina cribriformis) and also the intrapapillary relationship to the vascular system.

Background: To investigate associations and predictive factors between macular neovascularization (MNV) lesion variants and drusen types in patients with treatment-naïve neovascular age-related macular degeneration (AMD). Methods: Multimodal imaging was retrospectively reviewed for druse type (soft drusen, subretinal drusenoid deposits (SDDs) or mixed) and MNV type (MNV 1, MNV 2, MNV 1/2 or MNV 3). The Consensus on Neovascular AMD Nomenclature (CONAN) classification was used for characterizing MNV at baseline. Results: One eye of each eligible patient was included (n = 191). Patients with predominant SDDs had an increased adjusted odds ratio (aOR) for MNV 2 (23.4453, p = 0.0025) and any type of MNV 3 (8.7374, p < 0.0001). Patients with MNV 1/2 had an aOR for predominant SDDs (0.3284, p = 0.0084). Patients with MNV1 showed an aOR for SDDs (0.0357, p < 0.0001). Eyes with SDDs only without other drusen types showed an aOR for MNV 2 (9.2945, p < 0.0001). Conclusions: SDDs represent a common phenotypic characteristic in AMD eyes with treatment-naïve MNV. The aOR for eyes with predominant SDDs to develop MNV 2 and MNV 3 was much higher, possibly due to their location in the subretinal space. The predominant druse type may help to predict which type of MNV will develop during the course of AMD.

Optic disc drusens, which are congenital anomalies of optic nerve head, are depositions of mucopolysaccharides and pro­teinaceous material within the optic nerve head. They are usually bilateral and diagnosed incidentally during routine fundus examination where they may be mistaken for papilledema. We report a case of 63 year old female referred to the neuro-ophthalmology department with the clinical suspicion of disc edema. Simple noninvasive investigations like ultrasound B-scan, fundus autofluorescene along with the newer test like spectral domain optical coherence tomography helped us to diagnose it as a case of pseudopapilledema, as optic disc drusen. Thus, a high index of suspicion along with the ancillary tests is helpful in differentiating optic disc drusen from true papilledema. Purpose The purpose of this study was to examine the ocular and systemic risk profile of the fundus phenotype ≥ 20 small hard (macular) drusen (< 63 μm in diameter). Methods This single-center, cross-sectional study of 176 same-sex twin pairs aged 30 to 80 (median 60) years was a component of a framework study of the transition from not having age-related macular degeneration to having early AMD. Drusen categories assessed using fundus photography and optical coherence tomography included small hard drusen (diameter < 63 μm), intermediate soft drusen (63–125 μm), and large soft drusen (> 125 μm), of which the soft drusen are compatible with a diagnosis of AMD. Results Having ≥ 20 small hard drusen within or outside the macula was associated with increasing age, lower body mass index, shorter axial length, hyperopia, female sex, increasing high-density lipoprotein (HDL), high alcohol consumption, and with the presence of soft drusen. Conclusions Having ≥ 20 small hard drusen was associated with some AMD-related risk factors, but not with smoking, increasing body mass index, and higher blood pressure. Having ≥ 20 small hard drusen was also associated with soft drusen, in agreement with previous studies. These findings suggest that small hard drusen are not an early manifestation of AMD but the product of a distinct process of tissue alteration that promotes the development of AMD or some subtype thereof.

Drusen are the main aspect of detecting age-related macular degeneration (AMD). Ophthalmologists can evaluate the condition of AMD based on drusen in fundus images. However, in the early stage of AMD, the drusen areas are usually small and vague. This leads to challenges in the drusen segmentation task. Moreover, due to the high-resolution fundus images, it is hard to accurately predict the drusen areas with deep learning models. In this paper, we propose a multi-scale deep learning model for drusen segmentation. By exploiting both local and global information, we can improve the performance, especially in the early stages of AMD cases.

Drusen are common features in the ageing macula, caused by accumulation of extracellular materials beneath the retinal surface, visible in retinal fundus images as yellow spots. In the ophthalmologists’ opinion, the evaluation of the total drusen area, in a sequence of images taken during a treatment, will help to understand the disease progression and effectiveness. However, this evaluation is fastidious and difficult to reproduce when performed manually. A literature review on automated drusen detection showed that the works already published were limited to techniques of either adaptive or global thresholds which showed a tendency to produce a significant number of false positives. The purpose for this work was to propose an alternative method to automatically quantify drusen using advanced digital image processing techniques. This methodology is based on a detection and modelling algorithm to automatically quantify drusen. It includes an image pre-processing step to correct the uneven illumination by using smoothing splines fitting and to normalize the contrast. To quantify drusen a detection and modelling algorithm is adopted. The detection uses a new gradient based segmentation algorithm that isolates drusen and provides basic drusen characterization to the modelling stage. These are then fitted by Gaussian functions, to produce a model of the image, which is used to compute the affected areas. To validate the methodology, two software applications, one for semi-automated (MD3RI) and other for automated detection of drusen (AD3RI), were implemented. The first was developed for Ophthalmologists to manually analyse and mark drusen deposits, while the other implemented algorithms for automatic drusen quantification.Four studies to assess the methodology accuracy involving twelve specialists have taken place. These compared the automated method to the specialists and evaluated its repeatability. The studies were analysed regarding several indicators, which were based on the total affected area and on a pixel-to-pixel analysis. Due to the high variability among the graders involved in the first study, a new evaluation method, the Weighed Matching Analysis, was developed to improve the pixel-to-pixel analysis by using the statistical significance of the observations to differentiate positive and negative pixels. From the results of these studies it was concluded that the methodology proposed is capable to automatically measure drusen in an accurate and reproducible process. Also, the thesis proposes new image processing algorithms, for image pre-processing, image segmentation,image modelling and images comparison, which are also applicable to other image processing fields.

An investigation of the spectral reflectance of the fundus in normals and patients with common eye diseases was carried out using a multi wavelength scanning laser ophthalmoscope (SLO). With a knowledge of the spectral properties of the principal ocular pigments in the fundus, the appearance of the retina and choroid is predictable in normal subjects. The optic disc has a characteristic dark appearance when viewed with the SLO, and reasons for this may include a filtering effect of the confocal aperture to reduce the return of scattered light to the detector. In patients with macular exudates these features have a high reflectance in green-yellow light. An image processing method has been developed to quantify exudates and this was applied to SLO images and digitised colour slides of the same patients. Results show similar performance of processing in both image types with high accuracy (90% sensitivity for 95% specificity). A similar experiment was carried out on patients with macular drusen. These structures are of lower intensity than exudates and may have less well-defined borders. Again performance of the image processing methods showed broadly similar performance when comparing SLO images and digitised colour slides (60 to 68% sensitivity for 95% specificity). A study was carried out to assess the ability of the SLO to image the fundus in patients with cataract using a range of wavelengths. In conclusion the SLO offers the ability to image the fundus at selected wavelengths to enhance the desired features under investigation. As a fundus camera, used to generate digital images, it is unlikely to offer significant improvements over commercially available digital imaging CCD cameras. However, the SLO has properties which are likely to make it an ideal instrument for reflectometry, angiography and fundus topography. These applications are discussed in the final chapter.

Pour segmenter les images d'angiographie retinienne, notamment les images de drusen (taches jaunatres hyperfluorescentes), plusieurs methodes ont ete etudiees et proposees. Ces images presentent des difficultes d'analyse : du bruit, un contraste faible, parfois un nombre insuffisant de niveaux de gris et souvent une heterogeneite de certaines regions de l'image. Pour resoudre de tels problemes, nous proposons une methode de segmentation qui procede en deux etapes. La premiere etape concerne le rehaussement du contraste grace a un partitionnement optimal de l'image en trois classes : la premiere classe correspond aux drusen, qui apparaissent en clair, la deuxieme classe au fond de l'image et enfin la troisieme, nommee classe intermediaire, regroupant tous les pixels ambigus. Cette derniere classe est definie comme une region floue. La deuxieme etape de la methode de segmentation fait appel a la logique floue, qui est appliquee a la seule region floue. Elle consiste a attribuer a chaque niveau de gris un degre d'appartenance. Ensuite, l'application d'une methode iterative tenant compte du contexte spatial des pixels permet de decider de l'appartenance definitive d'un pixel a la classe drusen ou a la classe fond. Des etudes quantitatives sur des images de drusen de plusieurs patients ont permis d'effectuer deux types de comparaison : l'une avec une autre methode de segmentation, l'autre avec la detection manuelle faite par un praticien. La methode de segmentation proposee permet de quantifier de maniere objective les drusen au cours du temps ; les resultats obtenus confirment les donnees quantitatives connues en angiographie.

La segmentation de structures claires dans une image donnée est un problème très important dans la vision par ordinateur et particulièrement en imagerie biomédicale. En angiographie rétinienne la segmentation des drusen, taches jaunes un peu nuageuses situées au niveau de la rétine, constitue un enjeu très important dans le diagnostic et la prévention de complications graves. L'utilisation de méthodes classiques de segmentation de type seuillage, croissance de régions, extraction de contours ou contours actifs donne des résultats peu probants causés par l'imparfaite automatisation de ces méthodes. Nous présentons dans cette thèse une nouvelle approche de segmentation basée sur des transformations morphologiques nouvelles. Pour ceci nous définissons une classe particulière de composantes connexes de l'image : s'agit des maxima d'ordre p dont nous démontrons un certain nombre de propriétés. Ensuite à partir de ces composantes, nous construisons une fonction adaptative h(x), que nous utilisons comme fonction de contraste dans l'algorithme des h(x) maxima d'ordre r. Nous présentons des résultats expérimentaux d'extraction de drusen sur des images d'angiographie rétinienne. Nous avons aussi appliqué cette méthode de segmentation dans une séquence dynamique d'images pour les deux cas suivants : - suivi et évaluation de l'intensité moyenne d'un drusen dans une séquence d'image durant un examen à la fluorescéine. - comparaison des types et surfaces des drusen au cours du temps pour des examens différents.

Studies of electrooculogram testing of patients with age-related drusen and macular degeneration (aka senile macular degeneration, or SMD) have shown differing results. Of the two largest studies, one1 showed most patients with severe SMD to have low Arden ratios and most patients with drusen to have normal Arden ratios, and the other2 showed one-third of patients with drusen and SMD to have abnormal EOGs, with no correlation to severity of the clinical picture. These studies were performed using a non-ganzfeld type of stimulation. A recent study by us3 of twenty-one patients with drusen and SMD, using ganzfeld EOG testing, found that all patients had normal Arden ratios.

Age-related macular degeneration (AMD) is a spectrum of conditions affecting the eyes of the elderly, and is the leading cause of severe visual loss in people over 60 years of age.(1) Drusen, white-yellow excrescences below the retinal pigment epithelium, are associated with the development of the vision-threatening complications of age-related macular degeneration. However, drusen are seen ophthalmoscopically in over 30% of the elderly population. The vision-threatening aspects of AMD (which occur in approximately 1-3% of patients with drusen per year) include the development of a subretinal neovascular membrane and disciform scarring, retinal pigment epithelial detachment, and retinal pigment epithelial atrophy. At present, it is not possible to predict which people with drusen will go on to lose vision. Since prompt treatment of a subretinal neovascular membrane with laser photocoagulation can significantly reduce the frequency of severe visual loss,(2) there has been increased attention focused on finding ophthalmologic or visual function changes in drusen patients which may segregate a high-risk group.

Drusen are the hallmark of age-related macular degeneration, which is the major cause of severe vision loss in older adults in industrialized countries (1). However, the accurate quantification of drusen with standard clinical photography is possible in only a small portion of patients (2), since media changes in elderly subjects degrade image quality. Thus, for both research and patient management purposes, an alternative method of imaging the fundus with infrared light (3) and a scanning laser ophthalmoscope has been developed to visualize subretinal structures (4-5). This imaging technique minimizes both media changes and masking of foveal drusen by macular pigment, since there is a reduced absorption of infrared light by most ocular pigments.

Usred sve veće prijetnje koju nasilni ekstremizam (engl. violent extremism, VE) predstavlja širom svijeta države zapadnog Balkana suočavaju se sa znatnim izazovima u održavanju socijalne kohezije i stabilnosti. Kao i drugdje, narativi vjerske, krajnje desne i nacionalističke militantnosti snažno utječu na ranjivu mladu populaciju država zapadnog Balkana koja je zbog povijesti regije ispunjene etničkim, vjerskim i građanskim sukobima posebno podložna riziku od terorističke regrutacije u državama regije i inozemstvu. Pojedinci koji su otputovali kako bi se borili za nasilne ekstremističke organizacije u inozemstvu vraćaju se u svoje domovine uslijed teritorijalnih gubitaka koje su ekstremističke skupine doživjele u Siriji i Iraku. Istodobno etnonacionalistički ekstremizam nastavlja dobivati na snazi i širiti se regijom. I dok su neke od ovih tema detaljnije istražene u trenutačno aktualnoj literaturi, druge su i dalje slabo istražene. Za postojeće je teme istraživanja također potrebno više rada na terenu i čvršći teorijski temelji. Praznine u našem kolektivnom razumijevanju ukazuju na potrebu za dodatnim istraživanjem društvenih dinamika i dinamika nasilnog ekstremizma koje se razvijaju na zapadnom Balkanu. Bolje utemeljeno i detaljnije istraživanje može pomoći u informiranju i olakšati napore u sprječavanju / borbi protiv nasilnog ekstremizma (engl. Preventing/Countering Violent Extremism, P/CVE) u regiji. U 2019. godini mreža RESOLVE sazvala je lokalne i međunarodne stručnjake radi rasprave o prazninama u istraživanju i razvijanja preliminarnog popisa istraživačkih prioriteta za sprječavanje / borbu protiv nasilnog ekstremizma (P/CVE) na zapadnom Balkanu. Teme navedene u nastavku odražavaju njihovu zajedničku stručnost, dubinsko razumijevanje i posvećenost kontinuiranoj analizi trendova i dinamika nasilnog ekstremizma u regiji.
