Moncrieff & Smyth (1999, this issue) are certainly right that community treatment orders (CTOs) are high on the agenda and that psychiatrists need to think long and hard about them – about the political and ethical implications, not just their practical and therapeutic applications. Their concerns are important and reflect a wide constituency – identical views were expressed and considered during the consultation that preceded the College's document proposing a Community Supervision Order in 1993 (Royal College of Psychiatrists, 1993). Moncrieff & Smyth make no mention of that document, nor the limited, but recent, survey of psychiatrists' opinions that accompanied it (Burns et al, 1993). Do all College documents achieve obscurity quite so quickly?

ABSTRACTThe Nuclear Regulatory Commission (NRC) announced the availability of three new documents concerning radioactive materials in sewage sludge and ash from publicly owned treatment works (POTW). One of the documents is a report presenting the results of a volunteer survey of sewage sludge and ash samples provided by 313 POTWs. The second document is a dose modeling document, using multiple exposure pathway modeling focused on a series of generic scenarios, to track possible exposure of POTW workers and members of the general public to radioactivity from the sewage sludge or ash. The third document is a guidance report providing recommendations on the management of radioactivity in sewage sludge and ash for POTW owners and operators. This paper explains how radioactive materials enter POTWs, provides criteria for evaluating levels of radioactive material in sludge and ash, and gives a summary of the results of the survey and dose modeling efforts.

Work instructions laboratory equipment is one of the documents that must be available in the laboratory. This document provides information on the operation steps of the device correctly. A laboratory manager usually prints the document on A4 size paper and installs it on a part of the tool to facilitate access to the document. The treatment makes the work instruction document break down quickly and is not suitable to be applied to small sized tools. Therefore, it is necessary to manage the work instruction document for easy access, efficiency in the use of documents, and to improve the security of document storage. In this case, the QR Code can be used as a solution to the problem. QR Code is able to store documents and URL links in a small barcode image so that it can be use more efficient. Making a QR Code is preceded by storing documents at https://drive.google.com. The URL link at https://drive.google.com is entered into the QR-Code Studio 1.0 application and converted into a barcode. Barcodes are then printed and installed on each laboratory instrument. Scanning QR Code using a smartphone application in the form of a QR-Code Scanner. The results of the barcode scanned are the form of scripts for laboratory work instructions

Persons with mental illness (PWMI) often report negative perceptions of police treatment following receiving criminalising and heavy-handed police responses. To appropriately control officer discretion and to harness ethical, legal, and efficient police practice when encountering vulnerable and diverse individuals, police agencies across the world issue policy documents to their officers. These documents serve as a reflection regarding how police agencies aspire to manage PWMI in the community. Using a procedural justice framework, this research measures how a large police agency in Australia aspires to manage PWMI and whether the police policy document provides sufficient detail in advocating the appropriate and just police treatment of PWMI. A content analysis of the policy document revealed a lack of sufficient procedural guidelines in effectively controlling police officer discretion when encountering PWMI in the community. This article argues that without further consolidation to embed appropriate procedural guidelines into the policy document, the procedural policy gaps may have a negative effect on the experiences of PWMI when encountering the police.

Introduction and Aim: Plants form the primary source of medicinesboth in traditional and modern methods of treatment. People residing at remote and rural regions still depend on traditional herbal medicine for their primary health care needs. This study aimed to document ethnomedicinal plants used in the treatment of sprain in the Western Ghats region of Dakshina Kannada district, Karnataka state, India. Materials and Methods: An ethnobotanical study was carried out in Western Ghatsregion of Dakshina Kannada district, Karnataka during 2018-2021 to document the medicinal plants used by traditional practitioners to treat sprain. Data was collected using a semi structured questionnaire and open-ended interviews with herbal healers. Data was analyzed using simple statistical methods and few quantitative parameters. Results: A total of 40 plant species of belonging to 30 families have been documented. Some significant recorded plant species are Aloe vera, Litsea glutinosa, Cissus quadrangularis, Abrus pulchellus, Antidesma acidum, Holarrhena pubescens, Litsea wightiana and Ziziphus rugosa. Younger generations are ignoring the uses and cultural importance given to such ethno- medicinal practices. This knowledge is based on oral tradition often passed on to the next generation. Conclusion: Practitioners of this region do not have any written documents pertaining to their precious knowledge. Hence there is an urgent requirement to document and preserve the traditional knowledge inherited by tribes and ethnic communities before it is completely lost.

Reproductive tumors can impact conception, pregnancy, and birth in mammals. These impacts are well documented in humans, while data in other mammals are limited. An urgent need exists to understand the reproductive impact of these lesions in endangered species, because some endangered species have a documented high prevalence of reproductive tumors. This article documents that the prevalence of both benign and malignant neoplasia differs between African and Asian elephants, with Asian elephants more frequently diagnosed and negatively affected by both. The prevalence of these tumors across mammalian species is compared, and impact plus treatment options in human medicine are reviewed to inform decision making in elephants. Evidence suggests that reproductive tumors can negatively impact elephant conservation. Future studies that document reproductive outcomes, including the success of various treatment approaches in elephants with tumors will benefit conservation efforts.

An innovative form of radiotherapy, microbrachytherapy, is under development. This therapy targets solid, inoperable tumours by performing injections of liquid containing radioactive microspheres in suspension. Many injections are required to sufficiently cover the tumoural volume, and so to be able to deliver the position of these injections, a method of treatment planning has been developed and validated throughout this research. Throughout this work, three main questions are addressed: • How to perform the dosimetry for microbrachytherapy? • How to perform treatment planning for this modality? • What are the optimal injection properties to deliver the most efficient treatment? Microbrachytherapy dosimetry was performed by calculating the absorbed dose distribution for an injection. This distribution was then convolved at each injection position within the tumour to calculate the patient's absorbed dose distribution. Dosimetry of the tumour and the organs at risk was performed by extracting and analysing dose-volume histograms (DVHs). Once a method of dosimetry was put in place, optimisation algorithms were developed to generate patient-specific treatment plans. For this, three algorithms were tested and compared: Nelder-Mead Simplex, the Bees algorithm and the non-dominated sorting genetic algorithm II. It was found that, thanks to its MO optimisation, the non-dominated sorting algorithm II was the most flexible, and was used preferentially. Lastly, a comparison of injection parameters was performed. It was found that between 90Y, 166Ho, 131I and 177Lu, optimal injections consisted of microspheres of 90Y. Injection volumes of 5, 10 and 20 µL and initial activities of 5, 10 and 20 MBq were tested. It was found that 20 µL injections with 20 MBq were optimal because they minimise the number of injections required. This new technology combined with developments shown in this work demonstrate the feasibility - that was validated on animals - the ability to inject liquid containing radioactive microspheres in suspension to efficiently treat inoperable tumours whilst protecting surrounding healthy tissue. Such tumours, despite still having a poor prognosis, will surely have better support in the near future

New pharmacological interventions for oncology patients have resulted in longer lifespans after treatment completion and a large population of cancer survivors in communities. When patients complete treatment regimens for cancer, they leave the oncology practice with minimal knowledge of community resources that may assist them during this time of transition. These patients may not interact with care providers again until they return for a surveillance appointment in the future. Guided by Bandura's social cognitive theory, this project's aim was to develop a community-specific brochure that described the variety of services available to oncology patients and their families. The practice-focused question that steered this project asked if the developed patient education brochure on community specific resources was easy to understand and if it contained information pertinent to the oncology population. A comprehensive review of community-based resources was undertaken and placed into a brochure for the cancer survivor sand their families. Eleven participants, oncology nurse practitioners, and chemotherapy infusion nurses from the local outpatient community clinical practice were invited to be content experts. They each reviewed the brochure using the Patient Education Materials Assessment Tool for Printable Materials reporting 100% satisfaction with the content and applicability for the intended patient population. This project promotes social change by providing easy-to-understand transitional care informational for cancer survivors using a community-based brochure approach in a practice setting.

Several reports have suggested that reactive oxygen and nitrogen species are involved in SSc pathogenesis. SSc fibroblast from skin and internal organs overproduce ROS that trigger the proliferation of fibroblasts and the synthesis of type I collagen leading to the initiation and progresion of SSc. As in human SSc, skin fibroblasts from SSc mice constitutively produce large amounts of ROS. We have used this property to selectively induce apoptosis in the diseased fibroblast of SSc mice. Indeed, the organotelluride catalyst-(PHTE)2NQ and natural organosulfur compound – Dipropyltertrasulfide (DPTTS) are able of increasing ROS production by fibroblasts and inducing a lethal oxidative stress specificaly in SSc fibroblasts. This phenomenon has no impact on normal fibroblasts that present normal levels of ROS and a normal oxidant/antioxidant status. Many studies have also proved an importance of nitrogen species in the pathogenesis of SSc. In patients with SSc, the serum level of nitric oxide is significantly increased. Furthermore, NO can combine with other free radicals like superoxide anion (O•-2) to form the highly cytotoxic peroxynitrite (ONOO−) that contributes to inflammation, fibrosis and apoptosis of endothelial cells. Production of NO by endothelial cells or by fibroblasts can be stimulated by angiotensin II, the main peptide of the renin-angiotensin system (RAS). The level of angiotensin II is increased in SSc patients as well as in our HOCl mouse model and can promote proliferation of fibroblasts, fibrosis, and inflammation. These observations led us to test irbesartan, an angiotensin II type I receptor antagonist (AT1 RA) in the murine model of SSc. A new animal model based on chronic exposure to ROS and with many similarities to the human disease, allowed me to study new therapeutic approaches in SSc based on the cytotoxic action of pro-oxidative compounds - (PHTE)2NQ and DPTTS - and on the anti- nitrosative effect of irbesartan. These new therapeutic strategies open interesting perspectives in the treatment of SSc, where the therapeutic arsenal is currently still limited

Breast cancer is the most common cancer among women in the developed as well as the developing countries. There are a plethora of proposed solutions regarding possible medical interventions for breast cancer–one in particular is Breast Conserving Therapy (BCT). BCT comprises of complete surgical excision of the tumor (partialmastectomy), and post-operative radiotherapy for the remaining breast tissue. This is a feasible treatment for most women with breast cancer. The goal of BCT is toachieve local control of the cancer, as well as to preserve breast shape that appeases awoman’s cosmetic concerns. Although these goals are usually achieved, there are still occasional unexpected results, such as reexcision of the tumor due to a positive margin assessment, tumor local recurrence, unsatisfactory cosmetic results, and breastpain. Other than surgical experience and judgment, there are currently no toolswhich can predict the outcome of partial mastectomy on the contour and deformity of the treated breast. The objective of this dissertation is to propose computational framework, which contributes to BCT operations, this was achieve by exploring two areas. On the one hand we developed a multiscale model adapted for breast cancer tumor growth, ductal carcinoma in situ (DCIS). The model features included: nutrients growth limitation, wall degradation enzyme and HER2 chemical expression tumor phenotype. Our model successfully simulate some pattern of DCIS carcinoma.Among the interesting result we showed that the enzyme contributed to a greater tumor size and that when HER2 was over expressed, the growth limiting factor wasthe EGFR. On the other hand, we developed a virtual surgery box to simulate BCT surgery. The box will input MRI patient data and will output cosmetic and functional indicator to rate the impact of the surgery. It appears that stiffness of the tissue, resection radius as well as the lump quadrant location are the most sensitive parameters to the indicators. A healing model was also embedded to simulate the wound closure after resection, this model was stress dependent and illustrate anasymmetric wound closure progression. The tools developed in this research allows a new type of field convergence between the surgery and computation field. At the local level it will allow surgeons and patient to be able to communicate on the pertinence and necessity of performing alumpectomy surgery, enabling to anticipate the possible outcome of the operation. On the global aspect this type of tool gives birth to a new type of field: computational surgery, where computer scientist and surgeons work hand in hand to provide the best and the most reliable service to the patients

AbstractScar assessment has long relied on a variety of instruments, most of them questionnaires or scales that are susceptible to bias and limit the level of evidence in research. Throughout recent years, a variety of objective assessment tools have been developed. They allow for the exact quantification of different scar parameters such as texture, pliability, color, and size, thus facilitating detailed progress analysis throughout treatment. The advent of new technology, however, has its pitfalls, too. The number of different technologies available is hard to review, and oftentimes, clinical validation is lacking, making it hard for researchers to gauge whether a certain instrument is adequate for their individual projects. The use of each individual instruments has different requirements regarding patient preparation and calibration, and the measurements themselves can often be adjusted through a variety of parameters. This requires intensive familiarization with the respective instruments. Additionally, interpretation of the data can be complicated, and distinguishing statistically significant differences from clinically important differences remains difficult.Modern technology, however, allows researchers to measure treatment effects that otherwise would have eluded them through the use of subjective evaluation only.Through objective scar assessment technology and its detailed abilities to document changes throughout treatment, the evidence in scar-related research can be greatly improved, and current as well as new treatment options can finally be adequately gauged for their efficacy.

This document considers hostile conflicts between two swarms, called pursuers and evaders, based on a double integrator model. In order to convert the marginally stable dynamics into stable-capture behavior, we introduce a control strategy based on the relative positions and velocities of opposing swarm members. To evaluate its effectiveness, a Lyapunov-based stability analysis is performed. For simplicity, this document considers swarms with equal membership strengths and equal mass only. The modeling is based on a set of suggested interaction force profiles, which are functions of local vectors. The group pursuit is conceived in two phases: the approach phase during which the two swarms act like two individuals; and the assigned pursuit phase where each pursuer is assigned to an evader.

Preeclampsia is an illness characterized by the presence of high blood pressure which appears during the third trimester of the pregnancy. A step by step process is used to extract data from medical websites to obtain information about the patients who are pregnant women regarding their illnesses, symptoms and treatments. The existent DrOn ontology developed by the National Library of Medicine and the SYMP ontology offer information about these characteristics. The responses of the doctors are analyzed for matching the triggered information based on the features of interest. This is done to alleviate the task of the healthcare personnel who sift through the healthcare records when considering the proper treatment of a patient who also suffers from other diseases. The proposed online healthcare platform analyzes using Elasticsearch the acquired data from medical websites that is stored in the MongoDB NoSQL database. When a new patient writes about her health state while being pregnant, a new document is added to the MongoDB collection and the features used for matching are indexed into Elasticsearch. Regular expressions are used for extracting the relevant text. Natural language processing tasks like stemming and lemmatization improve the accuracy before reaching the matching step that is done based on encountered similarities. The important features that are clustered are detected by creating a document term matrix based on term frequency-inverse document frequency. While doing the crawling, the features regarding the illnesses, symptoms and illnesses are found using XPath to select the required information from the HTML belonging to the posts. The matching is done according to the features of the patient by using the weighted terms according to the matrix. After the matching is done, the patient is suggested a treatment which proved to be successful based on the previous analyzed cases. The medical professionals collaborate and can improve the existent knowledge base of the system through which the automatic treatment assignment is done. The shift to online healthcare platforms has improved the management of healthcare records, while the automatic treatment recommendation aims to enhance the monitoring of the patients, as well as the medical staff learns from the previous successfully solved cases.

The paper is focused on the implementation method for the General regulation plan of the railway systems in Belgrade, as a complex planning document that defines multiple railway systems and other objects which overlap with existing and planned structures in the urban area, which affect the complex network of existing planning documents, and which also overlap between themselves. The phased adoption also contributes to the complexity of the Plan. Such a planning document required the development of a new methodology and the solutions for complex problems, from the relationship between the phases and the general part of the Plan, through the relationship of the Plan and valid planning documentation, to the treatment of a construction plot where different systems or land uses overlap on several levels. The aim of the paper is to present the challenges faced in the process of defining the methods for the implementation of this Plan, the methodology used to overcome the problems, and the analysis of the possible improvement of the defined methodology

The need to provide standardized guidance for the use of air cleaning systems in nuclear facilities was recognized in the 1960’s when plans for nuclear facilities were at their peak. The American National Standards Institute (ANSI) asked a group of experts to generate documents for boiling-water-reactor standby gas treatment systems for accident mitigation. These experts immediately recognized that their scope needed expansion to include all air cleaning safety systems at all types of nuclear facilities. Their efforts resulted in the issuance of documents that provided guidance for the components in air cleaning systems (ANSI N509), and guidance for the testing of these systems (ANSI N510). Subsequently, it was recognized that this guidance needed to be formalized and implemented in a code format, with requirements instead of recommendations. Various other organizations were also providing guidance in different forms. The US government (the Nuclear Regulatory Commission) issued regulatory guides, and the American Society for Testing of Materials (ASTM) issued many documents providing specifications for activated carbon used in air cleaning systems for radioiodine removal. Acknowledging the need to consolidate all of these documents in a single source, the American Society for Mechanical Engineers (ASME) requested the air cleaning experts to work on publishing a code section under ASME auspices. This code section, designated AG-1, was assigned to a newly formed committee, the ASME Committee on Nuclear Air and Gas Treatment (CONAGT). This Committee started work in the mid 1970’s, and has issued various sections of the code since then, and the document now totals 600 pages. This code section covers the design, construction, installation, operation and testing of air cleaning systems in nuclear facilities. Work continues on updates to these sections of the AG-1 Code, as well as new sections specifically addressing gas processing systems. ASME code section AG-1 is the main international document for nuclear air cleaning systems for safe operation of nuclear power facilities, to ensure the safety of workers, and to protect public health and safety and the environment. The Code has four divisions, and a membership of over 100 of the premier air cleaning experts from 11 different countries.

Resumo:
Lavender oil obtained from the flowers of English lavender (Lavandula angustifolia Mill., Lamiaceae) is widely used in the production of fragrances, cosmetics, food, beverage products, and pharmaceuticals. The essential oil (EO) is used orally, by inhalation as a sleep aid, and can be applied topically for skin treatment. Adulteration of lavender oil is considered to be widespread due to high product demand and lower prices for chemically similar EOs and compounded fragrances. Some of the known adulterants are EOs from spike lavender (Lavandula latifolia Medik.) or lavandin (Lavandula × intermedia Emeric ex Loisel, syn. Lavandula angustifolia Mill. × Lavandula latifolia Medik.), vegetable oils, glycols, and naturally occurring volatile compounds from lower-cost sources (linalool, linalyl acetate, lavandulyl acetate). This Laboratory Guidance Document (LGD) presents a review of various analytical methods used to detect adulteration of EO of English lavender with lavandin, spike lavender, linalool and linalyl acetate-rich EOs, terpenes, and other chemicals. This document can be used in conjunction with the “Adulteration of English Lavender (Lavandula angustifolia) EO Botanical Adulterants Prevention Bulletin” published by the ABC-AHP-NCNPR Botanical Adulterants Prevention Program in 2020.
