Mercury and aluminum poisoning affects the entire human body, the cumulative toxic effects being noticed especially at the cognitive, neurological and metabolic level. Toxicity with heavy metals such as mercury, aluminum, arsenic, lead is one of the biggest threats to our health. The diagnosis of mercury, aluminum and arsenic poisoning is an important step in establishing food and chelation therapy, with an emphasis on the cause, not on the symptoms. We present the case of a 46-year-old patient with symptoms manifested for a period of 2 years, who was exposed to treatments structured according to manifestations, without improving the conditions presented, without performing specific investigations of heavy metal poisoning. The personalized diet and treatment plan, used in the case of this patient, for a period of 3 months were distinguished by a total solution of the manifestations initially presented, no side effects.

With the development of molecular techniques over time more than %60 of epilepsy has associated with mitochondrial (mt) dysfunction. Ketogenic diet (KD) has been used in the treatment of epilepsy since the 1920s. Aim. To evaluate the evidence behind KD in mt dysfunction in epilepsy. Methods. Databases PubMed, Google Scholar and MEDLINE were searched in an umbrella approach to 12 March 2021 in English. To identify relevant studies specific search strategies were devised for the following topics: (1) mitochondrial dysfunction (2) epilepsy (3) KD treatment. Results. From 1794 papers, 36 articles were included in analysis: 16 (%44.44) preclinical studies, 11 (%30.55) case reports, 9 (%25) clinical studies. In all the preclinic studies, KD regulated the number of mt profiles, transcripts of metabolic enzymes and encoding mt proteins, protected the mice against to seizures and had an anticonvulsant mechanism. Case reports and clinical trials have reported patients with good results in seizure control and mt functions, although not all of them give good results as well as preclinical. Conclusion. Healthcare institutions, researchers, neurologists, health promotion organizations, and dietitians should consider these results to improve KD programs and disease outcomes for mt dysfunction in epilepsy.

Recent evidence has demonstrated that Crohn’s disease may have its roots in dysbiosis of the microbiome and other environmental factors. One of the strongest risk factors linked to immune activation appears to be diet. Exclusion diets have been shown to ameliorate inflammation and induce remission in 70–80% of treatment-naïve children at disease onset, and to induce remission in patients that lose response or are refractory to currently recommended medical therapy. Recent studies have also linked dietary modulation of the microbiome with clinical remission, while reintroduction of the previous habitual diet led to reactivation of inflammation and reversion of the dysbiotic state. While dietary therapy has usually been used as a first line therapy as a bridge to immunomodulators, newer insights suggest that new treatment paradigms involving dietary therapy may allow different treatment strategies. This case-based narrative review will discuss the Crohn’s disease exclusion diet (CDED) as monotherapy, combination therapy with drugs, as a rescue therapy in refractory patients and for de-escalation from medical therapy.

Cealiac disease (CD) is an autoimmune disease, where autoantibodies (anti-transglutaminase (TG)2/anti- endomysial antibodies) attack mainly the intestinal tract. This disease is associated with several other autoimmune diseases. Ocular involvement in CD is often secondary to the intestinal malabsorption syndrome following the vitamin deficiency, so uveitis is rarely reported in the literature. We report the case of a 14-year-old girl, followed for coealiatic disease and type 1 diabetes, admitted for panuveitis of the left eye resistant to corticosteroid therapy, with a well response to a strict gluten-free diet. Uveitis secondary to coeliac disease is probably under-diagnosed, and some studies suggest the necessity to try a strict gluten-free diet in all idiopathic cortico-resistant uveitis.

Eosinophilic gastritis is a manifestation of gastrointestinal eosinophilic disease. The clinical symptoms are not specific, similar to other gastric disease complaints. The diagnostic aproach aside from clinical manifestations is also from endoscopic and histopathological features as the gold standard. Management includes drugs and diet. In some cases with strictures and perforations, endoscopic or even surgical intervention needed. Steroid-based therapy, especially topical preparations, is still the main choice. Other alternative therapies such as immunosuppression and targeted therapy showed good results in several case reports and small-scale studies. Further research to get a better management needed, considering the prevalence of this disease is elevated.

The correlation between health and diet has always been a subject of interest in the field of dermatology and medicine in general. However, studies in the literature are still scarce, and need further investigation in the field of inflammatory skin diseases. In this paper, we report a case of a patient with atopic dermatitis whose complete recovery occurred only after combining dupilumab therapy with a Mediterranean diet regimen.

Different diet-based approaches have been studied as potential adjuvants to standard cancer therapies in human clinical trials. However, these diets have been shown to have complications such as non-compliance and adverse side effects. This paper investigates four different types of diet-based approaches used in human clinical trials and compares their complications. The four diet-based approaches evaluated in this paper are ketogenic diet (KD), protein restriction, fasting and fasting mimicking diets (FMD), and combined interventions. Research shows that KDs have large reports of non-compliance from subjects, with subjects also experiencing significant weight loss, constipation, and fatigue. Protein restriction diets have greater levels of adherence from subjects but may lead to harmful hematological toxicities. Fasting and FMD showed greater adherence than subjects on KDs, and lower toxicities than subjects on protein restriction diets, but had a greater number of complaints of headaches, hunger, and dizziness. Finally, combined interventions have the fewest reports of side effects and non-compliance but suffer from a limited number of case studies. Given these results, diet-based interventions require further research to minimize side effects and non-compliance before becoming an accepted adjuvant to standard cancer therapy.

This article describes four new cases from our clinical practices of therapy-related acute leukemia after use of mitoxantrone in the treatment of patients with multiple sclerosis (MS) and reviews the mechanisms that predispose to this serious complication of treatment. The study consisted of a retrospective case series. Four previously unreported cases of secondary leukemia arising after mitoxantrone therapy in MS patients were identified, out of a combined cohort of 152 patients treated at our institutions. Three of the patients developed the acute promyelocytic leukemia subtype, after varying doses and latencies of mitoxantrone therapy. One of the study patients died. Characteristics of these patients and 10 previously reported cases are summarized. MS patients treated with mitoxantrone remain at risk for therapy-related leukemia, particularly acute promyelocytic leukemia. Strategies for minimization and prompt identification of this adverse event are needed.

Inflammatory bowel disease (IBD) is a chronic inflammatory condition affecting the gastrointestinal tract. The rising incidence of IBD has been associated with urbanization and shifts toward a Westernized diet. The intestinal microbiome has been a focus of disease pathogenesis and also therapeutic intervention. Dietary therapy for IBD has been well-studied with exclusive enteral nutrition, a formula-based diet with the exclusion of foods. In addition, interest in food-based exclusion diets has been increasing, with patients and families leading the charge. Challenges with dietary therapy for IBD include the lack of understanding of a detailed mechanistic pathway to explain the impact of diet on IBD pathogenesis and the difficult nature of designing and implementing dietary clinical trials. Epidemiological studies have demonstrated associations and intervention studies have demonstrated efficacy, but specific dietary targets remain as hypotheses at present. Current IBD therapy focuses on suppression of the immune system, yet the incomplete efficacy of present drugs suggests that other therapies must be developed and employed. Dietary interventions, with known ability to modulate the intestinal microbiome, are a unique opportunity to improve outcomes in IBD. Dietary intervention trials are challenging, and capturing both broad dietary patterns as well as exposure to individual food compounds is important. With increasing patient interest and preliminary research in dietary therapy indicating efficacy, it is imperative to further advance the science of utilizing diet in IBD, as well as to support patients by proactively addressing diet within their care plan.

Background and objectivesHyperphosphatemia is a persistent problem in individuals undergoing maintenance hemodialysis, which may contribute to vascular and bone complications. In some dialysis centers, dietitians work with patients to help them manage serum phosphate. Given the regularity of hyperphosphatemia in this population and constraints on kidney dietitian time, the authors aimed to evaluate the evidence for this practice.Design, setting, participants, & measurementsThere was a systematic review and meta-analysis of clinical trials. MEDLINE, Embase, CINAHL, Web of Science, Cochrane Central Register of Controlled Trials, and other databases were searched for controlled trials published from January 2000 until November 2019 in the English language. Included studies were required to examine the effect of phosphate-specific diet therapy provided by a dietitian on serum phosphate in individuals on hemodialysis. Risk of bias and certainty of evidence were assessed using the Grading of Recommendations, Assessment, Development, and Evaluations (GRADE) method.ResultsOf the 8054 titles/abstracts identified, 168 articles were reviewed, and 12 clinical trials (11 randomized, one nonrandomized) were included. Diet therapy reduced serum phosphate compared with controls in all studies, reaching statistical significance in eight studies, although overall certainty of evidence was low, primarily due to randomization issues and deviations from protocol. Monthly diet therapy (20–30 minutes) significantly lowered serum phosphate in patients with persistent hyperphosphatemia for 4–6 months, without compromising nutrition status (mean difference, −0.87 mg/dl; 95% confidence interval, −1.40 to −0.33 mg/dl), but appeared unlikely to maintain these effects if discontinued. Unfortunately, trials were too varied in design, setting, and approach to appropriately pool in meta-analysis, and were too limited in number to evaluate the timing, dose, and strategy of phosphate-specific diet therapy.ConclusionsThere is low-quality evidence that monthly diet therapy by a dietitian appears to be a safe and efficacious treatment for persistent hyperphosphatemia in patients on HD.

ENGLISH ABSTRACT: Food allergies in children are a growing problem and have various effects on these children, their households, the community and the economy. No medication or cure is currently available to consumers suffering from food-allergies. Medical teams are aware of the patients' need for support, but information is lacking and understanding of their basic needs and problems neglected. Two qualitative research methods, namely the focus group technique and case studies were used to investigate the emotional and physical environment of children allergic to wheat, soy, cow's milk and egg respectively. Five focus group meetings were conducted with the caregivers of food-allergic children. Two during the pilot study and three in the main study. A total of twenty three participants attended the three meetings of the main study. A meeting schedule was developed and the meetings carefully moderated. After the data had been obtained from the focus group meetings, there was a need for more in-depth information and understanding. Seven of the participants of the focus groups were identified, through developed criteria, to be further investigated as case studies. Five units of investigation were identified for the case studies. It was the (i) caregiver, (ii) allergic child, (iii) other children, (iv) "non-caregiver parent" and (v) household. Some of the matters that were investigated in both the focus groups and case studies were health consciousness, menu planning, recipe needs and use, reading of food labels, hidden allergens and the social and emotional effects of having and being a food-allergic child. A great need for better support and educational programs to assist these households were identified. Very few of the households were health conscious, while they all experienced problems with menu planning, identifying hidden allergens and suffered from social and or emotional problems. Very few participants made use of recipes or identified a need for allergen-free recipes. After having gained insight in the daily lives of these households, four - virtually identical, two-week menu cycles were developed to suit the life style requirements and restrictions of these children. The same recipes were used, but allergens omitted, replacements made and the recipes adapted as necessary. Product scouting was done for hidden allergens in selected popular convenience foods, especially those from the lowest level of the USDA Food Guide Pyramid. Hidden allergens appeared to be a problem when food is prepared and bought for the food allergic child. It should be taken into account that despite the information stated on the food label undeclared allergens might still be present in food. The two-week menu cycle for the child allergic to cow's milk was then quantitatively evaluated for nutritional adequacy by FoodFinder ™ 2 - a software program, for children aged between seven and ten years. All the macronutrients and micronutrients identified by the National Survey in 1999 as insufficient «67% of the RDA) among South African children aged between one and nine years, were evaluated as well as those mentioned in the WHO Dietary and Health Goals. All the nutrients were sufficient, except for calcium and Vitamin D. Supplementation is advised. The sensory acceptability of the menu-items in the two-week menu cycles were not evaluated. A vast number of issues pertaining to the food-allergic child require further research. A great need exists for the development of educational programs to support the households with food-allergic children. The identification of hidden allergens and the development of reliable sensory testing models for food-allergic children are all areas that should urgently be researched further.
AFRIKAANSE OPSOMMING: Voedsel-allergieë by kinders is 'n groeiende probleem met 'n verskeidenheid effekte op dié kinders, hulle huishoudings, die gemeenskap en die ekonomie. Tans is geen medikasie of kuur teen voedsel-allergieë beskikbaar nie. Mediese spanne is bewus van die behoefte aan sorg en leiding wat deur dié spesifieke groep verbruikers benodig word, maar inligting oor hulle behoeftes en probleme is ontoereikend. Twee kwalitatiewe navorsingsmetodes, naamlik die fokusgroeptegniek en gevallestudies, is gebruik om die emosionele en fisiese omgewing van kinders allergies vir onderskeidelik grane, soja, melk en eiers te ondersoek. Tydens 'n voorloperstudie is twee fokusgroepvergaderings gehou. Vir die hoofstudie is drie fokusgroepvergaderings met versorgers van kinders met 'n voedsel-allergie gehou met 'n totaal van drie en twintig deelnemers. 'n Vergaderingskedule is ontwikkel en die vergaderings is sorgvuldig gemodereer. Nadat die data van die fokusgroepe ingesamel is, was daar behoefte aan meer in diepte inligting en begrip. Aan die hand van ontwikkelde kriteria is sewe van die deelnemers aan die fokus groepe as gevallestudies geselekteer. Vyf eenhede van ondersoek is geïdentifiseer vir die gevallestudies. Dit is: (i) die allergiese kind, (ii) versorger, (iii) ander kinders, (iv) die "nie-versorgende" ouer en (v) die huishouding. Sommige van die aspekte wat in beide die fokusgroepe en gevallestudies ondersoek is, was gesondheidsbewustheid, spyskaartbeplanning, die gebruik en behoefte aan resepte, lees van voedseletikette, verskuilde allergene en die sosiale en emosionele effek van 'n voedsel-allergie op die kind sowel as die ouers. 'n Groot behoefte aan beter ondersteuning en opvoedkundige programme, om hierdie huishoudings te help, is geïdentifiseer. Baie min van die huishoudings was werkilk gesondheidsbewus, terwyl hulle almal probleme met spyskaartbeplanning, identifisering van verborge allergene en sosioemosionele probleme ondervind het. Weinig van die deelnemers het gebruik gemaak van resepte of het 'n behoefte aan allergeen-vrye resepte ondervind. Nadat insig in die daaglikse lewenstyl van hierdie huishouding verkry is, is vier amper-identiese twee-weekse siklusspyskaarte ontwikkel wat die lewenstyl en behoeftes van hierdie huishoudings aanspreek. Dieselfde resepte was gebruik vir al vier twee-weeksiklusspyskaarte, met die verskil dat verskillende allergene vervang is, na gelang van die allergie verskillende behoefte. Marknavorsing is gedoen op alledaagse produkte en is hoofsaaklik gefokus op die produkte wat die basis van die voedselpiramide uitmaak. Daar is bevind dat verskeulde allergene voorkom in produkte waar dit nie verwag word nie, en soms selfs nie op die voedseletikette verklaar word nie. Die twee-weeksiklusspyskaart van die melk-allergiese kind is kwantitatief ge-evalueer vir toereikendheid van die dieet vir kinders tussen die ouderdomme sewe tot tien jaar op die Foodfinder TM 2 - sagteware program. Die makro- en mikronutriente wat volgens 'n nasionale opname in 1999 as problematiese «67% van die RDA) vir kinders in die ouderdomsgroep een tot nege, uitgewys is, is geneem en die voedingstowwe is ook teen die Werêld Gesondheidsorganisasie (WHO) se dieetdoelwitte getoets. AI die nutriënte was voldoende volgens die analise, behalwe kalsium en Vitamen D. Supplementasie word aanbeveel vir dié twee nutriënte. Die sensoriese aanvaarbaarheid van die spyskaartitems in die siklusspyskaart is nie getoets nie. Verskeie aspekte wat vorendag gekom het, benodig verdere navorsing. 'n Groot behoefte aan die ontwikkeling van opvoedkundige programme, wat die huishoudings met voedsel-allergiese kinders ondersteun, bestaan. Die identifikasie van verskuilde allergene en die ontwikkeling van 'n beproefte sensoriese toetsmodel vir voedsel-allergiese kinders is potensiële gebiede wat dringend verder nagevors behoort te word.

Epidemiologic cohort studies often require a follow-up of and obtaining exposure and confounder information for several thousand subjects. This process can be time consuming and expensive. A sampling scheme which reduces costs is the case-cohort design. It involves obtaining exposure and confounder information for all cases and a random sample of the remainder of the cohort. In this thesis we consider the analysis of data from a case-cohort sample under the assumptions of a proportional hazards model. Specifically we review and describe techniques for point and interval estimation of parameters and tests of hypotheses first for a full cohort, then for a case-cohort sample. We apply the methods to a real example involving data from an occupational case-cohort study.

The relationship between dietary intake and the pathology of CFS has been an area of intense speculation without strong research support. There may be important links between diet and symptoms such that dietary interventions may be efficacious as adjunct therapy. This study was designed to assess any dietary abnormalities among Chronic Fatigue Syndrome patients. The purpose of this study is to make a controlled assessment of usual dietary intake so that dietary recommendations for CFS patients can be made. A Diet History Questionnaire, provided by the National Institute of Health, was used to analyze usual dietary intake among CFS patients. Women, ages I 8 and older, diagnosed by a physician with CFS, and were asked to complete the online survey. To complete the questionnaire, participants were provided with a user name and password and asked to answer a number of questions about their dietary habit. Twenty (n=20) women with CFS completed the questionnaire. The results were compiled and analyzed using Diet-Calc software and compared with nonnative data. Several nutrients were found to be deficient in more than 75% of the CFS patients.

The purpose of this study was to document the effects that a regular yoga therapy practice has on the quality of life for a paraplegic individual. Due to the unique nature of spinal cord injury (SCI), this was a case study with one participant. For 5 weeks the subject practiced a standardized yoga routine three times a week with a yoga teacher, receiving private instruction. Interview questions were asked before and after the intervention to document the participant's quality of life. Each week, the subject completed a modified SF-36 questionnaire as well as Cohen's Perceived Stress Survey. Perceived pain and perceived stress were the two primary variables monitored in the study. The results of this study demonstrated a slight reduction in perceived stress, a reduction in perceived pain, and an improvement in overall quality of life. Additionally, the subject increased her strength significantly from the beginning to the end of the study. Based on the findings in this study, it appears that a regular adapted yoga routine is beneficial for paraplegic individuals.

The clinical results of thromboembolism (TE) in Patients with prosthetic valVe endocarditis (PVE) and anticoagulant therapy were studied. 22 PVE patients (ll males and females each from 4 to 59 years old, average 32.7) were selected from 1939 patients who had undergone valve replacement at this hospital from 1964 and 1985. The complication frequency of TE and its clinical results, anticoagulant therapy and coagulation tests were investigated. Diagnostic criterion was determined in either of the following two: l) those patients who experienced valve replacement, with at least gradual pyrogenic symptons and inflammation factors such as a large increase in white blood cells, the progress of ESR and positive CRP, also with the same bacterium found more than twice in blood culture, also with the same bacterium found more than twice in blood culture, or 2) those patients who experienced valve replacement, with bacterial verruca found at re-valve replacement or at pathological anatomy. PVE onset took 2 days to 6.5 years (average 407 days) to appear after valve replacement. 8 out of the 22 PVE patients (36.3%) showed complications at TE onset, and 5 out of the 8 patients repeated. Embolism was found in 6 cases of brain, 4 cases of kidney, 2 cases of lung, 2 cases of limbs and 1 case of spleen, and 8 patients all died. On the other hand, 5 complications (22.7%) at bleeding were found in 3 cases of brain, 1 case of duodenum and 1 case of site of replaced aortic valve, and 4 patients died. Anticoagulant therapy was given to 21 out of the 22 PVE patients, and thrombotest (TT) values at TE onset were all less than 30%. Warfarin was administered as anticoagulant. 2 patients were administered with aspirin, but one was given with 250mg aspirin per day together with warfarin, and the other with 330mg aspirin per day alone. TT values at the onset of bleeding were from 10 to 56%. Anticoagulant therapy had been performed to the PVE patients since PVE onset did not yet appear, but complications coagulability by TT values, and all the patients died. In addition to this, complications at bleeding were found many and most of patients died even when the TT values were not so low. Therefore, we believe that the anticoagulant therapy that had been performed after PVE onset still needs further studies.

Malnutrition and neuro-disability are major public health problems in Africa. There is limited autism spectrum disorder (ASD) research in Kenya and families lack support aetiology is linked to witchcraft and sorcery. Research indicates an increase in ASD prevalence globally and in Africa. In Kenya, 2.2% (0.9 million people) live with some form of disability. Children diagnosed with autism spectrum disorder (ASD) suffer from neuro disabilities eliciting: altered sensory processing, restricted interests, and behavioural rigidity, allergies and aversions. Autism spectrum disorders have no cure, management is by use of interventions targeting autistic symptoms such as gastrointestinal disturbances, problem feeding behavior, linguistic development, non-verbal cognitive development, and motor development. The overall objective of this review was: to identify dietary and nutritional interventions available for the management of ASD symptoms in children and adolescents – Kenya. The study employed systematic review as the study design. The study included articles from Cochrane Library, PubMed, PMC, Google scholar, and Free Full databases. These were searched to identify studies published between September 2011 and September 2021. Eighteen articles were included: 12 randomized case-control trials, 3 open-label trials, one 2×2 factorial study, and 2 cross-over trials. The following dietary and nutritional interventions were evaluated: gluten and casein-free diet; ketogenic diets; probiotic supplements; specific carbohydrate diets; polyunsaturated fatty acids; vitamin and mineral supplantation (A, B6, B12, D, magnesium, folic acid); and alternative diets. Results from study indicate that vitamin and mineral supplementation was the most used intervention in management of ASD symptoms. This review established that there are limited or no studies on dietary and nutritional interventions in Kenya. Authors reported improvements in ASD symptoms in individuals receiving dietary and nutritional interventions such vitamin and mineral supplementation. The study findings will help policymakers and implementers to understand the consistency and precision and impact of these interventions. This will improve the safety and efficacy of interventions positively impacting the health and nutrition outcomes of children and adolescents with ASD. More research targeting ASD dietary and Nutritional Interventions is required in Kenya and other resource constrained settings. Purpose: for implementation of comprehensive assistance to pregnant women diagnosed with systolic pressure gradient, the study of a type of the psychological component of the gestational dominance was conducted in Maternity Hospital, City Clinical Hospital Nº 29 in Moscow (hereinafter referred to as PCGD). Materials and methods: The study compared PCGD of women with systolic pressure gradient and PCGD of women with other diagnoses, who received inpatient care in the pregnancy pathology department in Maternity Hospital, City Clinical Hospital Nº 29 in 2022. 680 pregnant women with a pregnancy term from 25 till 41 weeks participated in the study. There were 161 women diagnosed with systolic pressure gradient, 101 having diet therapy, and 60 having insulin therapy. The study was carried out with the help of a questionnaire by Dobryakov I.V. «Clinical and psychological definition of a type of the psychological component of the gestational dominance». 161 The analysis revealed that social factors and diagnose do not influence a type of PCGD. About 23% of women with different dianoses have the optimal type of PCGD. Twenty-two percent of women diagnosed with systolic pressure gradient having diet therapy and twenty-four percent of women diagnosed with systolic pressure gradient having insulin therapy have the optimal type of PCGD. The results of the study revealed that PCGD depends on a woman’s weight. The women having a normal body mass index choose the statements of the optimal type of PCGD more often (28,9%), it is more than the average sample. As weight increases, the number of optimal choices decreases. In the case of obesity of the 2nd degree, the optimal type of PCGD is found in 16.7% of the surveyed pregnant women, in the case of obesity of the 3rd degree in 8.7%. The number of euphoric choices increases with the decline of optimal choices: from 24% at normal weight to 29% at obesity of the 2 and 3 degrees. The level of significance of this result is more than 95%. Conclusions: The study's finding is that the type of PCGD does not depend on social factors, and diagnosis of systolic pressure gradient of the women having diet therapy or insulin therapy. PCGD is dependent on the weight of a woman. Women with normal weight have the optimal type of PCGD more than twice as often. Obese women more often choose statements of euphoric PCGD. Цель: для реализации комплексной помощи беременным женщинам с диагнозом ГСД, в роддоме ГКБ№ 29 г. Москвы было проведено исследование типа психологического компонента гестационной доминанты (далее ПКГД). Материалы и методы: Проведено сравнение ПКГД женщин с ГСД и ПКГД женщин с другими диагнозами, которые находились на стационарном лечении в отделении патологии беременных роддома ГКБ№29 в 2022 году. В исследовании приняло участие 680 беременных женщин со сроками беременности от 25 до 41 недели. Из них н 161 женщина с диагнозом ГСД: 101 на диетотерапии и 60 на инсулинотерапии. Исследование проводилось при помощи опросника И.В.Добрякова «Клинико-психологическое определение типа психологического компонента гестационной доминанты». Анализ показал, что на тип ПКГД не влияют социальные факторы, диагноз, с которым женщина находится в стационаре. Примерно 23% женщин с разными диагнозами имеют оптимальный тип ПКГД. У женщин с диагнозом ГСД на диетотерапии 22,5% и ГСД на инсулинотерапии 24% ПКГД оптимального типа Результаты исследования показали, что ПКГД зависит от веса женщины. Женщины, имеющие нормальный индекс массы тела, чаще выбирают утверждения оптимального стиля ПКГД (28,9%), что больше чем в средней выборке. По мере увеличения веса тела, количество оптимальных выборов уменьшается. Так, при ожирении 2 степени оптимальный тип ПКГД встречается у 16,7% опрошенных беременных женщин, при ожирении 3 степени – у 8.7% . На фоне снижения оптимальных выборов растет количество эйфорических выборов: с 24% при нормальном весе до 29 процентов при ожирении 2, 3 степени. Уровень значимости данного результата более 95% Выводы: Из этого исследования можно сделать вывод, что тип ПКГД не зависит от социальных факторов, наличия у женщины диагноза ГСД на диетотерапии или инсулине. ПКГД имеет зависимость от веса женщины. Женщины с нормальным весом более чем в 2 раза чаще имеют оптимальный тип ПКГД. Женщины, страдающие ожирением, чаще выбирают утверждения эйфорического ПКГД.

Introduction: Galactosemia is an autosomal recessive genetic condition, with alteration of galactose metabolism, leading to increased serum concentration of galactose (galactosemia). The first symptoms occur in the neonatal period, associated with the ingestion of galactose. Untreated patients usually have growth failure, liver and kidney dysfunction, tubulopathies, encephalopathy and susceptibility to infections. Case report: We describe a case of diagnostic investigation of a patient born at 38 weeks, after an uncomplicated gestation, with congenital cataracts, hepatomegaly, diabetes and Fanconi syndrome, as well as encephalopathy, hypotonia and cognitive deficit. She remained in the service for 15 days for diagnostic investigation, leading to hypothesis of galactosemia, confirmed later with genetic testing. Until then, the patient received unrestricted food, being instructed to change the diet, eliminating foods with galactose. After diagnosis, guidance and appropriate treatment were possible. Currently, patient is 20 years old, being monitored by neurology, ophthalmology, hepatology, occupational therapy and speech therapy teams. Conclusions: Brazil does not have neonatal screening for galactosmia, thus, the clinical recognition of its initial signs is important for early diagnosis and treatment, avoiding further complications and sequelae.

<div class="section abstract"><div class="htmlview paragraph">Light weight technologies are inevitable in the automotive industry to increase fuel efficiency and meet emission norms. An engine cylinder block is one of the major elements contributing approximately 3-4 % of the automobile weight. Aluminum cylinder block with cast-in liner is almost 40-55 % lighter than a conventional cast iron block [<span class="xref">1</span>] and hence the manufacturing processes and challenges associated with them are of high interest. A heterogeneous cast-in liner of gray cast iron in cast aluminum offers a low cost option, but the mechanical bond created between the liner and aluminum interface is prone to gap formation which affects the engine in terms of in-effective heat transfer, distortion and higher blow-by, and thereby high oil consumption &amp; higher emissions. This study aims at reducing this interface bonding gap by in-depth study of critical process parameters involved in manufacturing of cylinder blocks.</div><div class="htmlview paragraph">The study involved a single cylinder petrol engine block manufactured using High Pressure Die Casting (HPDC) process. Using detailed cause and effect analysis, various stages of HPDC like die design, including gating &amp; cooling system were studied &amp; improved using mold flow analysis. Die casting process parameters like die temperature, warm up shots, and biscuit thickness were optimized. Evaluation of bonding gap was carried out using non- destructive techniques like Immersion Ultrasonic Testing &amp; Computed Tomography Testing and a comparative analysis with experimental results of both methods are discussed. Considering initial investment, testing cost, testing time, sensitivity &amp; resolution, a feasible method may be selected for implementation.</div></div>

Objectives. Uncontrolled seizures in children 1 to 36 months old have serious short-term health risks and may be associated with substantial developmental, behavioral, and psychological impairments. We evaluated the effectiveness, comparative effectiveness, and harms of pharmacologic, dietary, surgical, neuromodulation, and gene therapy treatments for infantile epilepsies. Data sources. We searched Embase®, MEDLINE®, PubMed®, the Cochrane Library, and gray literature for studies published from January 1, 1999, to August 19, 2021. Review methods. Using standard Evidence-based Practice Center methods, we refined the scope and applied a priori inclusion criteria to the >10,000 articles identified. We ordered full text of any pediatric epilepsy articles to determine if they reported any data on those age 1 month to <36 months. We extracted key information from each included study, rated risk of bias, and rated the strength of evidence. We summarized the studies and outcomes narratively. Results. Forty-one studies (44 articles) met inclusion criteria. For pharmacotherapy, levetiracetam may cause seizure freedom in some patients (strength of evidence [SOE]: low), but data on other medications (topiramate, lamotrigine, phenytoin, vigabatrin, rufinamide, stiripentol) were insufficient to permit conclusions. Both ketogenic diet and the modified Atkins diet may reduce seizure frequency (SOE: low for both). In addition, the ketogenic diet may cause seizure freedom in some infants (SOE: low) and may be more likely than the modified Atkins diet to reduce seizure frequency (SOE: low). Both hemispherectomy/hemispherotomy and non-hemispheric surgical procedures may cause seizure freedom in some infants (SOE: low for both), but the precise proportion is too variable to estimate. For three medications (levetiracetam, topiramate, and lamotrigine), adverse effects may rarely be severe enough to warrant discontinuation (SOE: low). For topiramate, non-severe adverse effects include loss of appetite and upper respiratory tract infection (SOE: moderate). Harms of diets were sparsely reported. For surgical interventions, surgical mortality is rare for functional hemispherectomy/hemispherotomy and non-hemispheric procedures (SOE: low), but evidence was insufficient to permit quantitative estimates of mortality or morbidity risk. Hydrocephalus requiring shunt placement after multilobar, lobar, or focal resection is uncommon (SOE: low). No studies assessed neuromodulation or gene therapy. Conclusions. Levetiracetam, ketogenic diet, modified Atkins diet, and surgery all appear to be effective for some infants. However, the strength of the evidence is low for all of these modalities due to lack of control groups, low patient enrollment, and inconsistent reporting. Future studies should compare different pharmacologic treatments and compare pharmacotherapy with dietary therapy. Critical outcomes underrepresented in the literature include quality of life, sleep outcomes, and long-term development.

Review question / Objective: Among adult patients with leptomeningeal carcinomatosis from solid tumors (population) treated with chemotherapy, targeted therapy, or immunotherapy (intervention and comparator) what are the differences in overall survival (OS) and progression-free survival (PFS) and treatment response based on clinical trial outcomes? Eligibility criteria: Included articles reported 1) human subjects ≥ 18 years 2) diagnosis of leptomeningeal carcinomatosis from solid tumors confirmed by imaging or cerebrospinal fluid (CSF) cytology and clinical or neurological symptoms 3) clinical trials 4) with either PFS or MOS outcomes listed. Book chapters, case reports, review articles, observational studies, ed-itorials, and publications of leptomeningeal cancer from hematological tumors and studies consisting solely of pediatric patients were excluded from the analysis.

Review question / Objective: The effect of radiotherapy on post-treatment weight is incompletely defined. We determined the associations between radiotherapy and follow-up obesity in patients with craniopharyngioma (CP). Eligibility criteria: The inclusion criteria were as follows: (1) Participants: Patients who were clinically diagnosed with craniopharyngioma; (2) Intervention/control: For the control group, patients have not received radiotherapy, and the patients of the intervention group received radiotherapy alone, or surgery combined radiotherapy, or radiotherapy combined adjuvant therapy; (3) Outcome: The outcome was postoperative BMI or postoperative obesity. We excluded studies with (1) in vitro or animal experiments; (2) not written in English; (3) conference abstracts, reviews, correspondence, and comments; (4) case reports that included less than 10 patients.

Abstract: Multiple studies have found that psychiatric disorders, like mood disorders and substance use disorders, are highly comorbid among adults with either disorder. Integrated treatment refers to the treatment of two or more conditions and the use of multiple therapies such as the combination of psychotherapy and pharmacotherapy. Integrated therapy for comorbidity per numerous studies has consistently been superior to the treatment of individual disorders separately. The purpose of this QI project was to identify the effectiveness of Cognitive Behavioral Therapy (CBT) instead of current treatment as usual for treating Substance Use Disorder (SUD) or mental health diagnosis independently. It is a retrospective chart review. The review examines CBT's efficacy for engaging individuals with co-occurring mood and substance u se disorders in treatment by enhancing adherence and preventing disengagement and relapse. Methods: Forty adults aged 26-55 with a DSM-IV diagnosis of a mood disorder of Major Depressive Disorder and/or anxiety and concurrent substance use disorder (at least weekly use in the past month). Participants received 12 sessions of individual integrated CBT treatment delivered with case management over a 12-week period. Results: The intervention was associated with significant improvements in mood disorder, substance use, and coping skills at 4, 8, and 12 weeks post-treatment. Conclusions: These results provide some evidence for the effectiveness of the integrated CBT intervention in individuals with co-occurring disorders. Of note, all psychotherapies are efficacious; however, it would be more advantageous to develop a standardized CBT that identifies variables that facilitate treatment outcomes specifically to comorbid disorders of substance use and mood disorders. It is concluded that there is potentially more to be gained from further studies using randomized controlled designs to determine its efficacy.

Review question / Objective: The aim of this review is to identify and catalog studies that have reported the impact of indirect effects in interventions targeting malaria. Specific review questions are the following. 1. How many studies explicitly reported indirect effects? a. Have the number of publications increased in recent years? 2. What methodologies were employed by these studies to estimate the indirect effects? 3. Which terminologies were used to describe indirect effects? Background: Malaria is still a major health problem, particularly in sub-Saharan Africa, where 98% of global malaria mortality occurs. According to World Malaria Report 2022, the estimated case incidence was 229.4 per 1000 population in the African Region in 2021. Although the morbidity and mortality of malaria continued to decline from the 2000s to 2015 owing to many investments and interventions, such as long-lasting insecticide-treated net, rapid diagnostic tests, and artemisinin-based combination therapy, progress has been stalled since 2015. Moreover, especially after 2020, it has been reported that malaria incidence and mortality have increased in many African countries due to disruptions to services during the COVID-19 pandemic.

Review question / Objective: Is acupuncture a safe and effective therapy for constipation associated with Parkinson’s disease? Our aim is to assess the effectiveness and safety of acupuncture for constipation associated with PD and give guidance to future research direction. Condition being studied: Parkinson’s disease (PD) is a prevalent degenerative disease of nervous system characterized mainly by static tremor, bradykinesia, myotonia, postural gait disorders and other non-motor symptoms. According to variations on race, ethnicity, age and sex, the incidence of PD ranges from 8 to 20.5 per 100, 000 individuals annually. One global research shows that there were 6.1 million individuals suffer from PD in 2016 and will be 12 million patients around the world. According to several outcomes of case-control studies, the prevalence of constipation in PD varies from 28% to 61%. Constipation, as a common gastrointestinal disease which refers to the clinical presentation of reduced spontaneous complete bowel movement, dyschezia, feeling of incomplete defecation and outlet obstruction, is demonstrated to antedate the motor symptom and it's severity is related to the progression of PD. Acupuncture has been proved to act on the pathogenesis of constipation associated with PD. The proposed systematic review we're about to present is the first advanced evidence-based medical evidence in this area.
