Texto completo da fonteThe private long-term care insurance (LTCI) market is a puzzle, in France and in several developed countries (United-States, Spain, etc. . . ). The long-term care insurance market is small, yet long-term care is one of the largest uninsured financial risks facing the elderly. In addition public insurance is small in most countries. This research develops a new analysis of this long-term care insurance puzzle. It examines, on the basis of a new data set for France, whether the limited size of the insurance market is explained by market failures, or whether this sub optimal result is linked to institutional reasons ? The research leads to four results. In the first part, I argue that it is possible to push back the insurability frontier for long-term care at home. In the second part, I illustrate how individual preferences may lead some persons not to seek insure, and this for at least two reasons : an incomplete insurance contract ; current and expected health condition has a strong e§ect on wealth utility. On the basis of my data set, I argue that long-term care insurance can become a mass product particularly appreciated by middle classes. In the last part, I show, using standard tests, that there is no adverse selection on the private long-term care insurance market. This could be explained by offsetting effects. In view of these four results, I argue that the current limited size of LTCI market can be better explained by institutional reasons (social insurance criteria, tax disincentives) rather than by market failure. There are no economic obstacles to the development of this market. Nevertheless, a small share of the population will prefer not to insure

The first essay identifies a literature gap in ICT interventions for elderly recipients of professional home caregiving. It highlights the need for further research and recognizes the pivotal role of professional caregivers in these interventions. The second essay explores the distinctive perspectives of professional caregivers and reveals their substantial contributions to alleviating elderly loneliness through emotional companionship and cognitive support. In the third essay, mHealth solutions, including music and instant messaging, are introduced to address workplace loneliness among professional caregivers, offering practical insights for organizations to enhance caregiver well-being and retention. Collectively, these essays enrich our theoretical and practical comprehension of technology's potential to mitigate loneliness in elderly populations and professional caregiving contexts, thereby providing valuable insights for enhancing the quality of care and overall well-being

However, some do not have the choice: when chronic disorders begin, a fall happens or that the mind does not follow any more, it is a turning point in life (Lesourd, 2009) and rapidly comes the exclusion from society for an inclusion in geriatric institution. The main question of this study is to try to understand how elderly people with various dependencies can live: “this inhumane condition” (Malherbe, 2015) “in a total institution" (Goffman, 1968). With this yearlong study of 6 residents, whilst I managed an institution, I present the results of a clinical approach with them, based on narrative accounts and creation of identity booklets. It is mainly, Charles Altesse who will talk of his life, his daily experiences, about himself, of the struggle of living. Together we understand in a context of learning, how there is a lifelong work of construction or identity reconstruction which enables us to advance in a different way. One of the characteristics of this research is also to demonstrate the importance of the method, the “clinical-dialogical” (Lani-Bayle, 2010), in an emphatic listening and an exchange which allows finding resources via an attachment figure and to construct experimental knowledge helping to modify in a way to bear the unbearable end of life or to build knowledge by experience

All the health professions are thus increasingly faced with the management of elderly persons who are losing their independence and the demand for oral health care for elderly patients continues to increase.The originality of the work presented here is to understand the issue of the use of oral care of dependent elderly people in different places of residence. We considered both the physical and human environnement. Thus, the scientific interest of our research resides in the resolutely anthropobiological approach that places each dependent elderly person in their immediate environment.Our research is based on three studies. In the first work, throught a survey we consider the practitioners’ viewpoint and identify the difficulties they meet in providing care to ederly dependant patients. Then we analyzed the use of oral care and the oral health status of patients living at home or in institutions using data from a national survey.As part of a field survey of institutionalized populations, we used the data collected to compare the oral health experienced by seniors and the clinical reality of their oral status.Finally, we reported the elements obtained during collective interviews conducted with caregivers of institutions for the elderly

Texto completo da fonteOlder people living in nursing homes (NHs) suffer from numerous comorbidities and functional decline. Polymedication is frequent in this population. This increases the risk of potentially inappropriate drug prescribing (PIDP), which can lead to adverse drug events such as falls and hospitalization. Most French studies did not examine PPI with a global perspective, but focused on specific drug classes. Moreover, few studies have investigated, irrespective of the individual characteristics of residents, the structural and organizational characteristics of nursing homes on the quality of drug prescribing. Thesis work aimed to develop a new indicator of PIDP, which best reflects the global medication use of residents. Thesis works have been performed using the data of the IQUARE study (Impact d'une démarche Qualité sur l'évolution des pratiques et le déclin fonctionnel des REsidents), a quasi-experimental study (trial registration number: NCT01703689) investigating the impact of an intervention based on geriatric education with NH staff on quality indicators of care. First, we developed an indicator of PIDP, combining explicit and implicit criteria, identifying 71% of PIDP in NH residents at baseline. NH organizational (access to psychiatric advice and/or to hospitalization in a psychiatric unit) and structural (presence of a special care unit for dements) variables explained part of PIDP. In a second part, in order to validate the usefulness of this PIDP detection tool, we verified the long-term clinical impact of PIDP on adverse outcomes (death, number of hospitalizations...). We did not find any significant association between PIDP and death, nor between PIDP and number of hospitalizations. Finally, the general intervention implemented in the IQUARE study significantly reduced PIDP among NH residents at 18-month follow-up. Our research has provided important aspects that should be consider when constructing further new studies seeking to change prescribing patterns and to reduce the total number of drugs taken, but also to determine the final impact of these changes on clinical outcomes

It studies how these dependent or mentally-ill elders are cared for, focusing on innovative drug-free therapies (art therapy, esthetic therapy, psychomotricity).The field survey took place between 2008 and 2010 and used the method of exploratory filming, making the camera the main tool for the research. Three documentary films were made : Day Hospital (117 min), Short Stay (67 min) and Long Stay (120 min). While shedding some light on the way the institution works, these documentaries show the everyday life of a few patients and make them the main characters of the films. The written part analyses the strategy and the results of the field survey. It stresses the ethical issues raised by the presence of the filmmaker-observer in a highly sensitive environment and with very vulnerable subjects. The detailed analyses deal with the three main themes of the films : configuration of space and of material environment, physical techniques helping with loss of autonomy, interaction rituals between caregivers and patients
