This article discusses the modulation design of plasmonics for diagnosis and drug screening applications. It begins with an overview of the advances made in terms of theoretical insights, focusing on the origins of surface plasmon wave and manipulation, admittance loci design method, and surface plasmon grating coupled emission. It then considers how prism coupler, Ge-doped silica waveguide, nanograting and active plasmonics can trigger the excitation of surface plasmon resonance (SPR). It also examines the metallic effect of long-range surface plasmon resonance and conducting metal oxide as adhesive layer before describing three SPR waveguide biosensors that were developed for the realization of a hand-held SPR system. In particular, it presents a lateral-flow microfluidic channel based on a nitrocellulose membrane and integrated with a SPR waveguide biosensor to achieve dynamic detection. Finally, the article evaluates the biomolecular layer effect, with emphasis on kinetics analysis of antibody binding.

Acute kidney injury (AKI) remains a challenging clinical problem for clinicians caring for critically-ill patients due largely to the paucity of specific therapeutic interventions aimed at mitigating poor outcome. Those patients most at risk for the development of AKI can often be identified by an assessment of demographic, clinical, diagnostic, and procedure-related factors couple with early and intensive bedside monitoring. Importantly, critically-ill patients are often exposed to multiple discrete risks that can accumulate during their course that can negatively impact not only the duration and severity of AKI, but also probability of recovery, and long-term functional decline and risk of development of chronic kidney disease. All critically-ill patients at risk of or with milder forms of AKI should have support individualized. A clear understanding of the scope, complexity, and general principals of prevention and management of AKI are indispensable in the care of these patients and will discussed in this chapter.

Sodium is the main ion of the extracellular compartments, and it is through control of sodium reabsorption that the kidneys maintain volume homoeostasis and systemic blood pressure. The amount of sodium that is first filtered by the glomerulus and then reabsorbed in the tubule is quite staggering: assuming a glomerular filtration rate of 100 mL/min and a serum sodium concentration of 140 mmol/L, an average-sized person filters about 20,000 mmol of sodium per day, equivalent to the amount in 1.2 kg of cooking salt. In the steady state, the amount of sodium excreted is equal to the amount ingested. An average Western diet contains about 8–10 g of salt per day; a low-salt diet may be around 2 g per day. Under physiological conditions, the tubules reabsorb about 99% of filtered sodium. This enormous task is accomplished by a combination of distinct and sequentially oriented sodium or sodium-coupled transport systems along the nephron and the concerted and parallel action of some of these systems within the kidney. These are described, along with the consequences of disorders of the processes. A diagnostic approach to salt-losing states such as Fanconi, Bartter Gitelman and other syndromes, and hypoaldosteronism, is described.

Musculoskeletal ultrasound (MSUS) is rapidly becoming a standard part of many rheumatologists' daily clinical practice. MSUS is safe, increasingly widely available, relatively low cost, non-invasive, and hence very acceptable to the patient. Current problems with availability of training, mentoring, and accreditation procedures need to be overcome for MSUS to reach its full potential for rheumatologists. MSUS is capable of improving clinical diagnosis and the accuracy of intervention. MSUS is more sensitive than clinical examination in the detection of synovitis and effusion and is capable of rapid targeted assessment of widely spaced joints coupled with clinical correlation. MSUS has advantages over other imaging modalities; the ability to display dynamic real-time movement makes it the imaging modality of choice for tendon problems. It is significantly more sensitive than plain radiology in the demonstration of early erosive changes, and although its sensitivity is less than that of MRI for the detection of erosions it is far more practical, timely, and available. The combination of sensitivity in detection of synovitis, tenosynovitis, and erosions makes it an ideal imaging modality in the context of an early arthritis clinic. Power Doppler has been shown to be an effective way of evaluating synovitis and hence is of value in early diagnosis and monitoring of inflammatory arthritides. The accuracy of placement of local injection therapies is enhanced by MSUS, and it significantly increases the diagnostic success rate of aspiration of joints and bursas. The flexibility of ultrasound as a tool for rheumatologists is shown by its application in the assessment of vasculitides, peripheral nerve pathology, salivary glands, and skin lesions.

The book is about how journalists know what they know, who gets to decide what good journalism is, and how we know when it’s done right. Until a couple decades ago, these questions were rarely asked by journalists. When journalists were questioned by malcontented publics and critics about how they were doing journalism, these questions were easily ignored. Now, if you’re on social media, you’re likely to see multiple critiques of journalism on a daily basis. It seems not only convenient but pragmatic to give most of the credit to digital technologies and/or market failure for how relationships between journalists and diverse audiences have changed. This book rests on a different assumption, however. We contend that technologies offer a diagnostic to understand much deeper, persistent, and structural problems confronting journalism. Counter to much of the recent journalism scholarship, we argue that you can’t talk about the role journalists and journalism organizations could, should, and have played in society without talking about gender, race, other intersectional concerns—and settler-colonialism. Drawing on mixed methods and ethnography as well as interdisciplinary scholarship, this book examines the reckoning under way between journalists, their methods and their audiences in sites as diverse as social media, legacy newsrooms, journalism startups, novel forms of journalism memoir, and among indigenous journalists. The book explores journalism’s long-standing harms alongside repair, reform, and transformation. It suggests that a turn to strong objectivity and systems journalism provides a path forward.
