Texto completo da fonteUn premier chapitre étudie l'impact de la régulation de la démographie médicale sur les carrières des médecins généralistes. Nous montrons que les inégalités intergénérationnelles sont considérables et que la régulation démographique influence fortement les honoraires des médecins. Les médecins installés dans les années 1980 subissent les impacts conjoints du baby-boom et d'un numerus clausus élevé : il perçoivent les honoraires les plus faibles. En revanche, la diminution progressive du numerus clausus a amélioré la situation financière des cohortes installées à partir du milieu des années 1990.
Un deuxième chapitre s'intéresse à un phénomène remarquable dans la population des médecins généralistes : l'existence d'une importante minorité de médecins à faibles revenus. On observe que 5 à 7% des médecins généralistes ont des revenus mensuels inférieurs à 1,5 SMIC net. Nous montrons que ces faibles revenus résultent de la plus grande préférence de ces médecins pour le loisir. Cette très faible activité n'est pas le signe d'une dégradation du statut de médecin. Elle reflète plutôt un avantage de la profession de médecin libéral : les médecins peuvent choisir de travailler peu
Un troisième chapitre évalue le positionnement des médecins généralistes en comparant leurs revenus à ceux de salariés situés dans le haut de la hiérarchie salariale : les cadres supérieurs. La comparaison de la valeur des carrières des médecins et des cadres permet de mesurer l'avantage relatif à être médecin. L'analyse montre qu'il existe un très net avantage financier à être médecin généraliste et que cet avantage s'est accru au cours du temps. L'existence d'une rente pour les médecins résulte du concours à l'entrée des études de médecine. Cette rente a évolué positivement avec les restrictions sur le numerus clausus.

The research undertook to explore the role of professional experiences, dailylife, exchanges between peers, and all other forms of so-called « informal » learning inenhancing the professional skills of medical students and physicians. The author laboredunder the following hypothesis: all learning involves both formal and informal aspects,and the informal aspects can be found in cooperative, experiential and reflexive typepedagogical constructs. Finally, « informal » and « non-formal » learning opportunitiescertainly play a role in the acquisition of knowledge and skills among health care professionals.Three teaching mechanisms frequently used during the 3rd cycle of initial medical trainingas well as in medical continuing education programs were made available to medicalstudents and physicians working in ambulatory setting. These tools, which encourageparticipants to marshal their own experiences, to reflect thoughtfully and to cooperatewith each other, can improve the skills of medical students and physicians.Each of the three teaching mechanisms studied herein, factoring in individual backgroundsand proficiencies, contain both formal and informal knowledge and skills which,as a whole, contributed to the transformation of the participants. However, it is importantto note that the distinction between formal and informal learning is primarily theoretical,and may simply depend on context or location. Therefore, rather than opposethem, it may be more appropriate to consider that these types of learning lie on a continuum

Texto completo da fonteThis thesis addresses several issues: on the one hand, we focus on policies for controlling drug expenditures: on the other hand, we analyze the price competition on the OTC drugs market, in a context of drugs delisting policies.In recent years, some plans to affect change and improve prescribing practices are developed, with the emergence of physician profiling methods. The first chapter wonders about such a profiling in the current information system. Results show that outliers can be correctly identified; but to broaden profiling, we have to use some disease-specific indicators, or to improve information systems with diagnostic codes to define more general indicators.In addition to patient health, various sociological and economic factors may influence physician practices, including the competitive environment. The second chapter focuses on the unequal spatial distribution of general practitioners on drugs prescribing variability. Results are sensitive to the specification used for the medical density variable and suggest there is no effect of the density variation on the average prescribing cost per patient and per physician. However, this result reflects the sum of two opposite effects that cancel.In a context where the sale of OTC drugs outside pharmacies is controversial, the third chapter show there is no price competition between pharmacies on this market segment

Texto completo da fonteThe teaching of French as language of schooling - to pupils whose first language is not French - has been the subject of study starting from the beginning of the 1990’s. Since then, the education of immigrant school children in France has been researched progressively and governmental guidance has been introduced to specifically target their development. This guidance emphasizes that full inclusion of immigrant children into the educational system relies on successful application of French in disciplines where language skills are often considered to be of secondary importance, or a “tool”. However, didactic and pedagogical methods have not emerged to reliably deliver this goal.The integration of approaches for teaching of French for specific purposes and French as language of schooling would bring perspectives to defining the priority needs of these pupils, starting from a comprehensive analysis of the school situations that they face from the beginning of their school education in France. The aim of this thesis is to transpose an approach for French for specific purposes in a professional engineering environment into a school context, thereby contributing to the definition of the learning needs and to enable the development of appropriate teaching of French as language of schooling. More broadly, it aims to provide methodologies and didactic solutions to better address specials needs.A methodological framework is established, articulated around the main stages of French for specific purposes: analyzing the needs, collating and analyzing data on spoken and written abilities at school, and finally, defining a didactic approach based on the identified needs.The approach has been piloted and the results of three such trials are presented for different disciplines taught at the first year of senior school: history-geography, biology and mathematics. This study required the development of multimodal corpus, including filmed class sessions and written data from textbooks, student notebooks and teachers notes on the whiteboard. The identified needs are linguistic, but also cultural, cognitive and methodological.The analysis conducted in this work reveals that the use of semiotic objects is common in all of the disciplines investigated. It was also observed the increasing influence of praxeological perspective at school due to the introduction of digital tools in the classroom

Martin du Gard, Duhamel and Camus put forward three types of heroes who outline the evolution of a humanist faith. A prototype interesting in his profession and because of his vision of the world, A. Thibault arises from his author's fascination for the actors and the subject of medicine. He first seems to be reduced to his practise. But this typical doctor is not so far from "literature" as he claims to be: hardships make him more humane through self-discovery. To this "positive" doctor, l. Pasquier brings the new light of criticism. Duhamel can develop the reflection since he has closely explored life sciences and therapeutics. The character he creates embodies the successive disenchantments and experiences the prestige of his art: impostures are unmasked but absurdity remains badly recognized so that humanism is only approached through a negative way. Camus choses the doctor with no biographical motivation. Here, the symbolic value of the character prevails. Then, radicalization is essential. Rieux views his job as a way to resist the lethal condition. Submerged in misery he shows through example and the writing of his chronicle that man deserves trust: he is a witness for man against god

Texto completo da fonteTeaching of religious facts in schools is regularly debated in French society as well as within the teaching establishment itself. The History syllabuses are much concerned with this topic and, for several years now, there has been wide-ranging thinking about the issue. School textbooks illustrate this, taking various degrees of liberty, according to the dates of publication. Syllabuses and textbooks have been chosen here to study the question of religious facts and their evolution. It was decided to study how publications for sixth and seventh grades approach the three monotheistic religions and their origins. In order to underline continuities or discontinuities, it has been decided to study a long period, observing the school syllabuses from 1957 until now in French system “college unique”, thus covering the whole of the Fifth Republic. The objective is to analyse both the evolutions of the syllabuses and textbooks whose contents, by reason of successive ministerial instructions, have been subject to considerable change. Such change is linked to the growing importance given to documents, both textual and iconographic, which support the lesson and improve skills through exercises. The nature of the exercises asked of pupils also varies according to the changes in the syllabuses. The contents-either texts or documents-are analyzed. The aim of this research is to ascertain whether the form of the textbooks impacts the core content, notably their doctrinal content as far as the presentation of the three monotheistic religions is concerned
