Resumo:

First list of bee species of the Centre-Val de Loire region (France) declined by departments over the period 1990–2021. (Hymenoptera: Apoidea: Anthophila). - This article presents the taxonomic list of bees in the Centre-Val de Loire region (France) by department over the period 1990–2021. This study reveals the presence of 287 bee species in the region, with disparities between departments: between 50 and 60 species are recorded in Indre and Eure-et-Loir, compared with almost 200 species in Loiret (182) and Loir-et-Cher (195). These results are meant as an encouragement to develop our knowledge of the bee fauna in our region.

Resumo:

Confirmation of the presence of Strophingia proxima Hodkinson, 1981, in metropolitan France (Hemiptera, Liviidae). The presence of the psyllid Strophingia proxima Hodkinson, 1981, is confirmed in Metropolitan France by recent collections in the Pays de la Loire and Centre-Val de Loire regions. We also report older collections from the Nouvelle-Aquitaine region. All recent specimens, as well as older ones from the Vienne department, were collected from Erica scoparia L. A diagnosis of the species is provided, and its distribution, biology and ecology are discussed.

Resumo:

Depuis le début des années 1990, l’archéologie recense en Île-de-France des vestiges des guerres contemporaines. Les premières synthèses, produites dans les années 2010, ont concerné le camp retranché de Paris, système de défense de la capitale lors de la Première Guerre mondiale. Des prospections pédestres et LiDAR en forêt ainsi que des opérations d’archéologie préventive ont en effet permis de documenter des centaines d’ouvrages de fortification de campagne. En 2018, une première approche diachronique et multiscalaire des conflits contemporains a été engagée. L’évolution des modes de défense et d’attaque de Paris de 1870 à 1940 a pu être étudiée, de l’échelle de la structure archéologique à celle de leur insertion dans un système régional. Depuis 2020, un programme collectif de recherche fédère des archéologues rattachés à divers services intervenants en régions Île-de-France et Centre-Val de Loire. Ce programme vise notamment à inventorier de manière normalisée les vestiges des conflits, à développer l’intégration de sources historiques dans un SIG ou encore à produire une recension des archives utilisables dans le cadre d’une confrontation avec les données archéologiques. L’inventaire des vestiges martiaux a permis de dégager plusieurs thématiques abordables par l’archéologie pour les régions concernées.

The identification criteria are discussed and illustrated. L'analyse des données du recensement agricole de 2020 révèle une reprise de l'augmentation des surfaces irriguées et équipées en France métropolitaine, après une période de stagnation observée en 2010. En 2020, la surface agricole irriguée atteint 1,8 million d'hectares, représentant 6,8 % de la surface agricole utile (SAU) nationale. Cette dynamique est marquée par une forte hétérogénéité spatiale, influencée par des facteurs historiques, topographiques et pédoclimatiques. Les régions de la Nouvelle-Aquitaine, du Centre-Val de Loire et de l'Occitanie concentrent les plus grandes surfaces irriguées. L'étude met en évidence une diversification des cultures irriguées, avec une diminution relative de l'irrigation du maïs et une augmentation notable de celle de la vigne et des cultures industrielles. Ces résultats soulignent l'importance de l'irrigation comme réponse aux défis climatiques, tout en posant la question de la durabilité des pratiques actuelles.

Depuis les années 2010, la réduction du gaspillage alimentaire relève, en France, de l’articulation de diverses politiques publiques, entre alimentation, agriculture et économie circulaire. L’article analyse les modalités de traduction située des politiques nationales liées à la prévention du gaspillage alimentaire dans la restauration scolaire de huit communes du Centre-Val de Loire, région qui se veut exemplaire sur le sujet. La simultanéité d’actions interreliées sur la chaîne de restauration, en lien avec des approvisionnements qualitatifs, pointe des engagements municipaux réels, mais inégaux, pour une consommation responsable prévenant les gaspillages. En fonction de lectures locales du gaspillage, et de l’intérêt pour les questions alimentaires, une plus ou moins grande diversité de parties prenantes partageant ses expériences, à différents échelons, est impliquée. Or, prévenir le gaspillage alimentaire se révèle couteux en temps et en moyens, et ne peut prendre sens que dans le changement des pratiques d’alimentation, dans le cadre des transitions agro-alimentaires.

Termites are social insects that can also be major pests. A well-known problem species is the subterranean termite, Reticulitermes flavipes. It is invasive in France and is thought to have arrived from Louisiana during the 18th century. While the putative source of French populations has been identified, little is known about how the termite spread following its establishment. Here, we examined expansion patterns at different spatial scales in urban areas to clarify how R. flavipes spread in France. Based on our analyses of phylogeography and population genetics, results suggest a scenario of successive introductions into the Charente-Maritime region, on the Atlantic Coast. Two major expansion fronts formed: one that spread toward the northeast and the other toward the southeast. At the regional scale, different spatial and genetic distribution patterns were observed: there was heterogeneity in Île-de-France and aggregation in Centre-Val de Loire. At the local scale, we found that our three focal urban sites each formed a single large colony that contained several secondary reproductives. Our findings represent a second step in efforts to reconstruct termite’s invasion dynamics. They also highlight the role that may have been played by the French railway network in transporting termites over long distances.

The literature in economic geography is increasingly interested in the role of human agency in local and regional development. The understanding of the role of agents of change on path development is especially critical for small- and medium-sized towns (SMSTs), which tend to be less diversified and to have fewer localised capabilities. The town of Vierzon in the Centre-Val-de-Loire region in France is a medium-sized old industrial town in a structural crisis. It is suffering from deindustrialisation, population decline, unemployment and poverty. This article takes a micro-perspective with a case study approach to explore the creation and growth of Ledger – a frontier blockchain start-up that designs, produces and commercialises hardware wallets for cryptocurrencies – against all odds in Vierzon, France. It underlines the role of a Window of Locational Opportunity, agency, chance, the broader institutional context in the location of Ledger in an unlikely place and subsequent unrelated diversification and path creation. It contributes to the debate on the interplay between agency and structure by linking agents of change in SMSTs to the broader institutional context and to the understanding of the emergence of place-based leadership.

ObjectiveTo evaluate the SARS-CoV-2 seroprevalence among local authority workers, depending on their position and potential interaction with the public.MethodsA cohort of volunteer participants was recruited among local authority workers of the Centre Val de Loire region in France, to be tested using a rapid serological test (COVID-PRESTO). The collected data were analysed by comparing different parameters including, gender, age, position held, and contact or not with the public. The study was carried out from August to December 2020 and included 3228 participants (n=3228), from 18 to 65 years old.ResultsThe seroprevalence of SARS-CoV-2 among local authority workers was estimated at 3.04%. No significant difference could be observed according to the position held by the workers and whether they were or not in contact with the public. Nevertheless, a significant difference was observed between the different investigating centres, in correlation with the geographical location.ConclusionContact with members of the public was not a critical parameter for SARS-CoV-2 seroprevalence as long as protective measures are applied. Among the population included in the study, childcare workers were more at risk of getting infected by the virus.Trial registration numberNCT04387968

AbstractThe European pond turtle, Emys orbicularis, is a wide ranging species, distributed from Northwest Africa over a large part of Europe and Asia Minor to the Caspian and Aral Seas. For 106 pond turtles from France mtDNA sequence variation has been assessed, using a 1031 bp portion of the mitochondrial cytochrome b gene. Three of nine haploclades currently known from the entire species' range were found in France. One clade (II) is represented with four very similar haplotypes, differing by one mutation, and the two other clades (V, VI) are represented with one haplotype each. A syntopic occurrence of clades II and V is reported for the first time for the Camargue. Besides, clade II occurs in the French regions Aquitaine, Centre-Val de Loire, and Rhône-Alpes. Outside of France, it is found mainly in the catchment areas of the Danube and Oder rivers and in the Balkans. Haploclade V, which is also known from the Apennine peninsula, Sardinia, and the northern Mediterranean coast of Spain, is restricted in France to Corsica and the Provence-Alpes-Côte d'Azur region. A single individual bearing a haplotype of an Iberian and North African clade (VI) was found in Aquitaine near Pau. This could indicate gene flow between the Iberian peninsula and West France, if the specimen is native. The distribution of the distinct haploclades in France probably reflects Holocene range expansions, especially of haploclade II turtles. In the postglacial, haploclade II terrapins arrived from the east and spread over the Rhône corridor to the Mediterranean coast. In the southern Rhône area they met and hybridized with haploclade V turtles. Further research is needed to clarify whether this hybridization is a locally restricted phenomenon.

Dans le contexte du changement climatique, les plantations forestières sont parfois présentées comme une solution d’atténuation et d’adaptation, certes questionnable. Pour que cette option soit réellement une solution qui autorise une plus grande résilience territoriale face au changement climatique, il importe que la présence de ces plantations sur le territoire soit envisagée de manière concertée. Cette démarche est précisément en train de se mettre en place en France : après des années de controverses, les acteurs de la filière populicole et ceux impliqués dans la protection de l’environnement ou des paysages semblent désormais disposés au dialogue, notamment dans le Val de Loire, à l’initiative du CRPF (Centre régional de la propriété forestière). Dans cet article, nous analysons cet exemple, après avoir rappelé les controverses qui entourent les peupleraies et précisé la méthodologie : nous nous fondons surtout sur des observations et des entretiens. Même si elle bénéficie d’un contexte favorable (le recul des peupleraies), cette démarche apparaît comme un exemple de stratégie pour une plus grande résilience territoriale, dans la mesure où les acteurs territoriaux sont parvenus à s’accorder sur des préconisations visant à améliorer ces plantations, au moins sur le plan environnemental. Pour autant, des améliorations sont nécessaires, afin de prendre davantage en compte le volet social.

ObjectivesTo explore the patient experience of a spirometry test used to confirm chronic obstructive pulmonary disease (COPD) diagnosis in patients with suspected smoking-related COPD.DesignThis is a qualitative study, performed with open interviews in adults following a routine spirometry test to confirm COPD diagnosis. Data were analysed with a phenomenological-inspired micro-phenomenology approach.ParticipantsEligible patients were recruited through their general practitioner, 10 were interviewed.SettingPrimary care in Centre-Val-De-Loire area, France, in 2018.ResultsParticipants reported the spirometry test experience as being unfamiliar but gave meaning to the symptoms they experience. Participants expressed a desire to perform the test well and a willingness to confront their state of health. After the spirometry had been completed and the results announced, participants moved through stages of grief from their pre-spirometry self and symptom perception to a state of acceptance. Overall, participants expressed a narrative of an evolving cognitive and corporeal awareness during this spirometry experience. The verbatim quotes describe a cognitive rupture with their chronic illness usually considered as a ‘way of life’.ConclusionsA spirometry test goes beyond a diagnostic value, providing patients with an opportunity to gain awareness of their own state of health, reframe their breathlessness-related limitations and thus begin to accept the disease. These awareness gains may be considered as small steps towards health behavioural change. Spirometry may have educative potential and support lifestyle changes.

Low temperature discharges with their unique non-equilibrium properties are often used in industrial processes for the deposition of ultra-thin (plasma) polymer films. Applications for this type of thin films range from protective coatings to their use in membranes, biosensors and batteries. However, the properties of these thin films are highly dependent on the choice of discharge parameters, both in terms of their surface topography and their chemical composition. This paper deals with the great potential of pulsed discharges for the controlled deposition of plasma polymer films. The influence of pulse frequency and duty cycle on the deposition process and the properties of the resulting thin films is discussed from a theoretical point of view and is illustrated by experimental results. The latter concern examples of experiments carried out with discharges operated with acetylene or aniline, as well as functional coatings on top of MoS2, 2D graphene or CNTs. (1) Background: While inequalities in the prevalence of cancer, access to care, and survival have been well documented, less research has focused on inequalities in the uptake of supportive oncology care. Given its contribution to improving the quality of life of people affected by cancer, access to such care is a major public health issue. The present study focuses on the access and uptake of those supportive oncology care services. (2) Methods: This study is based on qualitative research methodology, using a thematic analysis tree on NVivo© analysis software. First, an exploratory survey was conducted with users of oncology services, and professionals from these services and supportive oncology care. Then, individual interviews were conducted in June 2022 among people who are currently being treated or have been treated for cancer. (3) Results: The experiences of the 33 respondents revealed that significant variations in the uptake of supportive oncology care are underpinned by identifiable disparities in their healthcare pathways: in their assimilation of information, difficulties in accessing oncology care, personal reluctance and motivations, perceived needs and benefits, and use of other medicines. (4) Conclusion: This study aims to gain some insight into disparities in the uptake of supportive care in the Centre-Val de Loire region (France). Thus, it provides a better understanding of the complex ways in which these inequalities in supportive oncology care uptake are constructed.

Objectives To estimate the prevalence of malnutrition in elderly cancer patients and its association with frailty and primary cancer treatment recommendations in patients with the two most prevalent cancers (colorectal cancer, CRC and breast cancer, BC) in the Centre-Val de Loire region of France. Methods The entire cohort of 704 patients aged 75 years or older presenting with cancer who underwent comprehensive geriatric assessment (CGA) between 2014 and 2017 was included. Nutritional status, frailty (defined by the Balducci classification system based on CGA parameters and comorbidity), and pathological criteria were analyzed in terms of the cancer treatment recommendations suggested by geriatricians both in the whole cohort and in those with CRC and BC. Results In the whole group of 704 patients (84.3 +/- 4.8 years), the prevalence of malnutrition was 62.9%, and was higher in CRC than in BC patients (71.1% vs 55.4%, P < 0.01). In a multivariate analysis, malnutrition and frailty (as determined by the Balducci classification system) were independently related in CRC patients (OR: 7.28, 95%CI, 1.58~34.03; P = 0.012) and were unrelated to metastasis [odds ratio (OR): 1.34, 95%CI, 0.56~3.18; P = 0.5]. By contrast, malnutrition in BC patients was related exclusively to the extent of metastasis (OR: 3.52, 95%CI, 1.50~8.24; P = 0.002). It was also demonstrated that geriatricians had a greater tendency to suggest only palliative care to CRC patients presenting with malnutrition (15.4% vs 2.7%, P = 0.006) than to BC patients (9.8% vs 5.4%, NS). Conclusion Malnutrition in elderly cancer patients is prevalent, especially in those with colorectal cancer, where malnutrition is frailty-related and may strongly impact on cancer treatment strategies.

Abstract The Reeves’ muntjac Muntiacus reevesi is an ungulate native to China and Taiwan that has been released into the wild in several continental European countries and the United Kingdom. We present recent records of the species in mainland France. We also investigated the dispersal rate and density of the established population in the Centre-Val de Loire region. The number of observations of M. reevesi was low in the period 2008–2016, increased sharply until 2019 and then decreased until 2023. From a comparison between the distribution in 2008–2012 and 2013–2017, the dispersal rate was estimated at an average of 3.2 km/year, and 7.6 km/year between 2013–2017 and 2018–2023. Based on a camera trap protocol, the population density was assumed to be very low in forested areas but estimated with a lack of robustness due to a single observation. Further efforts must be made to better assess the risk of an increase in the wild population of Reeves’ muntjac in the Centre-Val de Loire region. To prevent potential impacts, rapid control measures can be put in place in the large area (10,800 km2) where muntjacs are now rapidly spreading.

In the wake of the discovery of numerous large blade workshops at Le Grand-Pressigny site (Indre-et-Loire, France), which initially aroused great interest, the mid-Loire Valley region became central to studies of flint diffusion. Despite the quality of the initial work, the widely shared view now is that the capacity for this concept to continue to provide useful archaeological modelling has diminished. Establishing real and actual correlations between archaeological objects and geological samples remains difficult, making it almost impossible to determine the source of certain materials represented in an archaeological series. In response to this problem, the French collective research project "Réseau de lithothèque en région Centre-Val de Loire" assembled about 30 amateur and professional researchers from various European institutions to work on three strategic missions: Mission 1: establish a list of current lithotheques (“rock-libraries” or comparative collections), verify their contents and complete them if necessary; Mission 2: link the major types of depositional environments with the properties acquired by flint through its life; Mission 3: spread knowledge and information both within the project and to all interested persons. The project is closely associated with similar French programs already in place in Auvergne-Rhône-Alpes and Aquitaine regions. The ultimate objective is to build a national and international community around a common project, bringing participants together regularly to share knowledge and technical expertise.

Les personnes accueillies dans une communauté d’Emmaüs représentent une population très hétérogène au sein de laquelle il peut y avoir des auteurs d’infractions à caractère sexuel (AICS). Ces communautés offrent théoriquement à toutes personnes la possibilité de les rejoindre pourvu qu’en échange elles puissent travailler pour faire vivre la collectivité. La compétence n’est pas nécessairement requise, car le travail est adapté aux possibilités de chacun. Cela peut permettre aussi bien à une personne souffrant de troubles psychiques chroniques comme la schizophrénie d’y trouver sa place (si sa maladie est suffisamment stabilisée pour supporter un minimum de contrainte liées à la vie collective), qu’à un AICS en situation post-pénale et en situation d’exclusion sociale. Les AICS ont besoin d’avoir des lieux de vie qui peuvent les accueillir qui ne sont pas si nombreux, surtout en cas de violences intrafamiliales. Ils vont cumuler des difficultés multiples : faibles ressources financières, faible niveau d’étude et de formation, auxquelles seront associées des difficultés de réinsertion sociale. Celles-ci peuvent représenter autant de facteurs de risque de récidive de l’acte délictueux ou criminel pour lequel ils ont été condamnés ou pour tout autre forme de violence. Mais si l’accueil dans une communauté de vie représente une forme de « protection » par la dimension contenante qu’elle offre, les interactions sociales qu’elle permet, il n’est pas une garantie contre la reprise d’une carrière délinquante ou criminelle. Les communautés d’Emmaüs permettent l’intégration dans une communauté de vie et un retour au sein de la société pour que les personnes accueillies puissent un jour mener à bien ou non leur projet de réinsertion sociale.

Territorial inequalities in access to care and the lack of health practitioners represent one of the important challenges health systems are facing worldwide. Territorial management seems to be the discipline to address these concerns in a holistic and interdisciplinary way, specifically via the concept of lived territory. Territorial management and health geography share the same vision on the definition of the lived territory, namely a territory which is a social construction, dynamic and shaped by its users. However, territorial management lacks tools to define the lived territory, whereas the ‘relative flows’ method in health geography identifies users’ real healthcare consumption on the territory, offering an operational tool for stakeholders, including healthcare professionals and local decision makers. Focusing on the intersection of management and health geography, this study is looking to address the question: to what extent would the inter- and transdisciplinary approach enable an effective response to the difficulties of access to care in the territory? This research is based on a case study of the French region Centre-Val de Loire. The findings of the study emphasise an added value of the inter- and transdisciplinary approach in operationalisation of territorial management discipline. The lived territory concept appears a most appropriate grid in the evaluation of inequalities in access to care and thus an effective tool to mobilise the involvement of healthcare stakeholders in a new territorial organisation centred on user needs in care.

La Cistude d’Europe Emys orbicularis (Linnaeus, 1758) est une tortue d’eau douce menacée sur toute son aire de répartition. Le site Natura 2000 « Grande Brenne » qui fait l’objet de l’étude se situe au centre de la France dans le département de l’Indre (Région Centre-Val de Loire) au sein du Parc naturel régional de la Brenne, territoire propice à l’espèce qui y vit en métapopulations occupant des chaines d’étangs. L’objectif de l’étude est de quantifier en quoi les habitats de landes, forêt, prairies et les cultures créent des contextes paysagers favorisant, ou non, l’occupation des étangs ou/et certaines dynamiques de transition. L’étude d’échelle macro-écologique choisie se base sur les modèles dynamiques de MacKenzie pour rechercher l’existence d’une relation entre les grands types d’habitats terrestres présents sur le site et la dynamique d’occupation de l’espèce. Un échantillon représentatif de 61 étangs suivis durant sept années est prospecté cinq fois par saison afin d’obtenir un ensemble d’histoires de détection. Le paysage à proximité des sites est caractérisé par quatre variables. Les résultats montrent que la présence de landes et de forêts dans le paysage avoisinant les étangs semble favoriser l’occupation de la Cistude. Un scénario de colonisation entre 2011 et 2017 de la périphérie sud du site est suggéré. L’étude permet d’envisager des interactions entre l’occupation de l’espèce et les habitats terrestres présents sur le site. Elle montre aussi qu’il est essentiel de considérer non seulement l’écosystème étang dans la gestion de la Cistude d’Europe, mais également l’ensemble des habitats nécessaires au déroulement du cycle de l’espèce.

ObjectiveThe study aimed at: i) analyses the regional characteristics and riskfactors of severe influenza, taking into account dominant circulatingvirus(es) ii) estimate the regional completeness of the surveillancesystem.IntroductionEvery year, circulating influenza viruses generate a significantnumber of deaths. During the 2009 pandemic influenza A(H1N1),a national non mandatory surveillance system of severe influenzacases admitted to intensive care units(ICU) was set up in France.This surveillance is regionally driven by the regional offices (CIRE)of Santé publique France, the French Public Health Agency. Thisreport provides epidemiologic analysis of the recorded data sincethe implementation of surveillance in the Centre-Val de Loire regionover seasons 2009-10 to 2015-16 in regard of influenza epidemicsdynamics.MethodsSurveillance was carried out each year from October to April.Descriptive and analytic analyses were conducted to comparepopulation characteristics, pre-existing risk factors and the clinicaldata according to influenza season and dominant circulatinginfluenza virus(es). Logistic regressions were performed to identifyfactors associated with an increased risk of acute respiratory distresssyndrome (ARDS) or death. Two capture-recapture analyses wereperformed to establish the completeness of the surveillance systemin the region. The first one was realized on all cases, using two datasources (hospital records/surveillance data) and the second one, onlyon deaths, using three data sources(additional source: medical deathcertificates).ResultsFrom 2009-10 to 2015-16, the outbreak of influenza epidemicswas started more and more late. The number of severe influenzacases reported in the Loire Valley varied from 19 in 2010-11 to 75 in2014-15. Overall, the most affected population was adults, from 41%in 2011-12 to 83% in 2009-10. However seniors (more than 65 yearsold) represented an important part of patients during three epidemics:50% in 2011-12 and around 45% during the two last seasons;during these epidemics, men, (60%-68%), were more affected thanwomen. Patients’ pre-existing risk factors were mainly: being olderthan 65 years old and suffering of cardiac or pulmonary diseases.The comparison by dominant viruses over the seasons revealed thatwhen A(H1N1) virus prevailed, severe influenza occurred mainlyin adults patients with any type of pre-existing risk factors whereaswhen A(H3N2) virus prevailed, seniors with pre-existing pulmonarydisease were the most affected. More than a third of patientsdeclared an ARDS. The overall observed lethality was close to 16%.ARDS occurred more frequently in patients who were middle-aged(45-64 years), immunocompromised or infected with A(H1N1).Pre-existing pulmonary disease was a protective factor. Risk factorsassociated with death were being older than 65 years, male and havingdeclared an ARDS. The completeness of this surveillance system wasestimated by capture-recapture methods at 59% for severe influenzacases and 40% for death cases.ConclusionsThe epidemiology of severe influenza and epidemics dynamics inthe Centre-Val de Loire follow the national trends. Every season ischaracterized by the same dominant virus at national and regionallevels in intensive care units. Influenza epidemics 2009-10 and2014-15 were particularly long and severe, the first dominatedby the A(H1N1)pdm09 virus and the second by the A(H3N2).Our study has demonstrated that the populations at risk of severeinfluenza differ according to the circulating virus(es). Accordingto the obtained estimations, the completeness of the surveillancesystem, based on voluntary report by physicians, can be consideredas satisfactory. Regarding influenza deaths relatively low percentageof completeness may be explained by the fact that two sources arehospital based whereas the third one, medical death certificates,includes all influenzadeaths with no information on the death place.Many patients were not vaccinated or their status was unknown. Mostcases admitted to ICU presented pre-existing risk factors includedin eligibility criteria in influenza vaccination policies. This studyoutlines the importance of vaccination as the first prevention measure.

Contexte : Les structures douleur chronique (SDC) assurent la prise en charge des patients souffrant de douleurs chroniques les plus complexes et les plus réfractaires en France. En 2018, on recensait 245 SDC labellisées par les agences régionales de santé (ARS). Malgré cette couverture territoriale importante, les SDC sont aujourd’hui menacées. Objectifs : Obtenir une photographie de l’offre de soins des SDC, en précisant à la fois les caractéristiques des SDC et les données démographiques des professionnels y exerçant. Méthodologie : L’enquête était réalisée sous l’égide de la Société française d’étude et de traitement de la douleur (SFETD) auprès de l’ensemble des SDC labellisées par les ARS. Les responsables des SDC étaient invités à renseigner, sur un questionnaire en ligne, les caractéristiques de la SDC et les données démographiques du personnel médical et non médical. Résultats : Deux cent vingt-trois (91 %) des 245 SDC ont répondu. Si le maillage territorial des SDC est respecté, avec en médiane 0,59 médecin douleur pour 100 000 habitants en France, il existe une grande hétérogénéité dans la répartition des équivalents temps plein (ETP) d’une région à l’autre, avec une variation d’un facteur 2,8 pour les médecins et d’un facteur 5 pour les psychologues et infirmiers. Les régions les moins dotées sont la région Centre-Val de Loire et la région Grand Est. Cinquante-quatre pour cent (54 %) des SDC proposent des consultations à la fois pour la douleur cancéreuse et pour la douleur chronique non cancéreuse. Seulement 6 % des SDC sont dédiés à la douleur de l’enfant. Le délai d’attente médian est de trois mois, avec des extrêmes pouvant atteindre un an. Au total, 835 médecins, représentant 388 ETP, travaillent dans les SDC. Seulement 58 % des médecins sont titulaires ; 18 % exercent à temps plein et 43 % exercent à moins de 0,2 ETP. Le départ à la retraite de 177 médecins est attendu dans les cinq ans, représentant 91 ETP, soit 24 % du total des ETP médicaux. Conclusion : La pérennisation des SDC en France est menacée dans les cinq ans à venir. Il y a urgence à agir. Des mesures concrètes sont proposées par la SFETD pour anticiper la catastrophe annoncée.

ObjectiveTo analyse population coverage of syndromic surveillance(SS)based on emergency care data by studying i)the attractiveness ofrespectively SOS Médecins (Emergency care general practitioners)and Hospital emergency departments in the Centre-Val de Loireregion and ii) the contribution of ecological deprivation factors inemergency access to healthcare.IntroductionSOS Médecins France (SOS Med) is the first private and permanentnetwork of general practitioners providing emergency care in France.Besides Hospital emergency departments (HED), SOS Med istherefore a major source of data for detecting and measuring near-real-time health phenomena. The emergency services provided by theSOS Med have been subject to important changes in the recent years.Their services are enriched by a medical consultation center togetherwith extended working hours. Besides, the south of the region ismarkedly affected by a declining number of medical practitionersThis study was conducted to analyze the regional population coverageof emergency healthcare data provided by HED and SOS Med tothe French syndromic surveillance system (SurSaUD®) takinginto account distance, health care offer, demographic factors andecological deprivation factors.MethodsAn analysis of the activities and geographic attraction was carriedout based on the data respectively provided by the three regional SOSMed and three HED (Bourges, Orléans and Tours). Quasi-Poissonregression modelling was used to identify the factors influencing theattractiveness of each organization. Next, the findings were refinedthrough spatial analysis of the attractiveness of HED and SOS Medand analysis of the contribution of deprivation based on socio-economical and healthcare facilities ecological indexes.ResultsIn terms of age group, children under 2 years required the largestservice consultations as well as seniors over 75 who sought moreemergency visits at home. The SOS Med were almost always active inurban areas and at least once in two due to continuity of care. So theyare an efficient source of general medical care given present workhours. Distance as an influential factor may explain the differencesin attraction to the support type. The extent of the attraction appearsin 36% SOS Med Bourges and 14% for SOS Med Orleans. Addthe extent of attraction for SOS, remote consultation for SOS Medassociations are a good use of care in general practice in present workhours scheme.In terms of monitoring of epidemics, we note that the SOSMédecins associations are most active in winter, particularly duringthe seasonal epidemics of influenza. This can be explained by the factof patient referrals during calls. The most serious cases are redirectedto the ED and cases of general medicine to the SOS Médecins.It is also important to note that the attraction of ED ofCHR Orléanscovers more or less important a large part of the regional territory,which is not visible to the ED ofCH Bourges. It should neverthelessbe noted that theCHR Orleansa larger bed capacity than theCH Bourges.ConclusionsThis research has analysed the changes taking place in the SOSmédecins associations in the Centre-Val de Loire region. Findingsshows that these associations help ensure access to general medicalcare in a context of strongly reduced medical demography althoughwith an uneven, primarily urban, geographical coverage. Withbetter knowledge of the geographic span and sources and types ofemergency care provision, further research can be undertaken tofurther refine and interpret the data.

AbstractAgricultural areas represent one of the major ecosystems of the world. Intensification of agricultural practices produced openfields characterized by low biological diversity. Nevertheless, the distance up to which intensive agricultural fields alter surrounding natural systems is rarely quantified. We determined the spatial scale at which agricultural landscapes alter the diversity of Odonates, a key taxon in wetland ponds, and we tested to what extent citizen science data can be used reliably for this purpose. We compiled 7731 observations made in a portion of the region Centre‐Val‐de‐Loire (France) over 10 years by naturalists on 729 water bodies to analyze the effect of agricultural landscapes (mainly wheat, rapeseed, sunflower) on the species richness of both damselflies and dragonflies in lentic systems. Sixty species were reported over the 10‐year period. For dragonflies, intensive agricultural landscapes best explained their richness at the scales of 800 and 1600 m for overall and autochthonous species, respectively, when using the full dataset. The spatial scale was smaller for damselflies, at 200 m for both overall and autochthonous species. These distances were not severely impacted when constraining the data to consider several biases. Multimodel averaging showed that the proportion of intensive agriculture decreased species richness, despite the potential biases inherent to an imperfect database acquired by citizens. This imperfect citizen dataset allows to infer the lowest effect size of agriculture on species richness. Quantitatively, this effect was more important for autochthonous species. Interestingly, both relatively rare taxa and common or generalist species can be under threat in intensive agricultural landscapes, calling for more ecotoxicological studies. The influence of agricultural practices from a distance implies that conservation and management plans of wetland ponds should consider the landscape ecological characteristics and not only the pond features. Conservation efforts focusing too locally on a site may be undermined because intensive agriculture from a distance limits the potential for the site to recover highly diverse communities. These distant effects should be integrated by policy‐makers when deciding which wetland pond should benefit from a conservation plan or which conservation action may be planned, implementing, for instance, buffer zones and/or ecological corridors composed of natural vegetation.

As part of a cereals virome project high throughput sequencing (HTS)-based viral indexing was performed on plants with symptoms of barley yellow dwarf disease collected in June (2017-2020) in the main French cereals production areas. Total RNAs from 32 individual plants were purified (RNeasy Plant Mini Kit, Qiagen, Courtaboeuf, France) and Illumina sequenced (2x150 nt) following ribodepletion (Genewiz-Azenta, Leipzig, Germany). Following quality trimming, reads for each sample were de novo assembled (CLC Genomics Workbench 21, Qiagen) [1] and contigs annotated by BlastX analysis. In four winter barley samples collected in 2018 (18-58, 18-325 and 18-326) and 2019 (19-30A), besides contigs representing diverse viruses such as barley yellow dwarf viruses-PAV and PAS, Hordeum vulgare endornavirus, cereal yellow dwarf virus-RPV (18-326), wheat dwarf virus (18-325 and 18-326) and a novel Polerovirus (18-58 and 18-326), large contigs with high identity to barley virus G (BVG) were identified. BVG, a tentative Polerovirus, was initially reported in barley in South Korea in 2016 [2] and has so far been identified in a few other hosts including wheat, oat, maize, proso and foxtail millets as well as switchgrass. It has been reported from the USA and Australia [3] and, in Europe, from the Netherlands, Germany, Hungary and Greece [4]. Large BVG scaffolds representing near complete genomes could be reconstructed for each sample, integrating a total of 128.339, 7.188, 8.078 and 20.073 reads, for samples 19-30A, 18-58, 18325 and 18-326 respectively. Given that between 17.2 and 20.5 million reads had been obtained per sample, these values translate into between 0.04% (18-58 and 18-325) and 0.6% (19-30A) of total reads, and to average coverages of between 158x (18-58) and 2866x (19-30A) for the genomic scaffolds. The four assembled sequences (5584-5610 nt) have been deposited in GenBank (ON419453-ON419456). They are nearly identical (98.4 to 99.5% nt identity) and share between 97.7% and 98.5% nt identity with a barley reference isolate from the South Korea (NC_029906). To confirm the presence of BVG, a primer pair was designed based on available BVG sequences. Primers BVG-F(5’-CTAGCCCAACGAGTTGCGGG-3’) and BVG-R(5’-GGTACAGAAGCTCTACGGTTC-3’) amplifying a 394 nt were used in a two-step RT-PCR on new RNA extracts obtained from the 18-325 and 18-326 infected plants. The amplicons were directly sequenced and showed respectively 99.2% (ON419457, 18-325) and 100% (18-326) nt identity with the corresponding de novo scaffolds. The four analyzed samples have been collected respectively in 2018 (18-58, 18-325, 18-326) and 2019 (19-30A) in three different regions of France (Auvergne-Rhône-Alpes, Occitanie and Centre-Val de Loire), indicating a wide distribution and a persistence over time of BVG in France. To our knowledge, this represents the first report of a natural infection of BVG in cultivated winter barley in France. Presence of BVG may have been overlooked in a range of situation, as indicated by its retrospective discovery in a 34 years old Australian sample [3], possibly explaining its broad distribution in France. While the mixed infection status of the analyzed plants precludes any conclusion on its pathogenicity in French cereals, BVG has been reported to be associated with a range of symptoms in various hosts so that further studies to evaluate its prevalence and impact in France and to begin to understand its epidemiology are clearly warranted by the present results.
