Nutritional disorders are often encountered in hospital neurology practice, especially deficiencies of vitamins B1 and B12. Medical conditions can predispose to nutritional disorders. This chapter discusses the presentation, diagnosis, and management of B12 deficiency, B1 deficiency, protein-energy malnutrition, folate deficiency especially in the context of pregnancy, B6 deficiency, B6 toxicity, copper deficiency, and vitamin D deficiency. Wernicke encephalopathy and Korsakoff syndrome are also discussed.

Vitamins are organic compounds required by the body in small amounts to perform specific cellular functions. Nine vitamins (thiamine (vitamin B1), riboflavin (vitamin B2), pyridoxine (vitamin B6), cyanocobalamin (vitamin B12), niacin (nicotinic acid; vitamin B3), pantothenic acid (vitamin B5), biotin (vitamin B7; vitamin H), folic acid (folate; vitamin B9), and ascorbic acid (vitamin C)) are water soluble, while four (vitamins A, D, E, and K) are fat soluble. The importance of vitamins was first appreciated through recognition of their clinical deficiency state. However, this approach has led to the concept that the primary purpose of a vitamin is to prevent the associated clinical deficiency state and, consequently, unless patients exhibit signs of a specific clinical deficiency state, they are thought to be replete in the corresponding vitamin. This is a misunderstanding. In reality, most vitamins have many different functions which are incompletely understood, and impaired biochemical function and even functional problems affecting metabolic, immunological, or cognitive status can occur with marginal vitamin depletion long before overt clinical deficiency becomes evident. A high index of suspicion is thus essential in all patients who have malnutrition or malabsorption, to ensure that levels that might compromise health, resistance to disease, and recovery from injury or illness are not left untreated.

Where possible, pre-conception counselling is desirable for both prospective parents in order to address issues of lifestyle and nutrition so that pregnancy can proceed with the best possible start. If dieting is necessary this should be initiated as far in advance of the pregnancy as possible, because inadequate nutrition around the time of conception can influence the fetal growth trajectory and weight at birth. Regardless of BMI, women should also be encouraged to engage in regular aerobic exercise. Nutritional requirements in the pre-conception period include adequate amounts of oily fish to provide omega-3 fatty acids. All women planning a pregnancy should receive 400 #amp;#x00B5;g of folic acid daily, and vitamin B12 and vitamin D supplementation may be necessary, particular for vegetarians. Other vitamins and minerals should be considered, particularly iron and iodine, but most should be obtainable from a balanced diet. Harmful behaviours and environmental exposures should be avoided.

Vitamin B12 is required for the synthesis of fatty acids and myelin and so is crucial for normal neurological function and maintenance of the CNS. In conjunction with folate, it is involved in red blood cell formation and DNA synthesis, and in embryogenesis, it is important for proper neural tube formation and brain development. Maternal intake during pregnancy is important, as only newly absorbed vitamin B12, and not that stored in the maternal liver, is concentrated in the placenta. Despite the active transfer during pregnancy, the vitamin B12 content in the newborn is low, and the infant is dependent on breast milk for ongoing needs. Pregnant and lactating women should ensure that their diet contains sufficient (animal) sources of vitamin B12; those consuming vegan or strict vegetarian diets should either take vitamin B12 supplements or seek foods that have been fortified with vitamin B12.
