Background Several large health care systems in the United States have pioneered the integration of dentistry into their medical care delivery models. To date, no studies or case reports on combining medical information into the dental electronic health record within U.S. academic dental centers were found in the extant literature. Objective This report details how we developed and implemented customized primary care elements into axiUm, a popular dental practice management software primarily used in dental schools, to facilitate medical–dental clinical integration. This work was undertaken to provide the infrastructure for nurse practitioners and dentists to deliver a holistic, integrated, person-centered approach to care. Methods A multidisciplinary design team used the Centers for Disease Control and Prevention's Framework for Patient-centered Health Risk Assessments, an evidence-based framework to guide the adaptation of the existing axiUm dental record. The design featured individual data fields to source data, generate reports, and analyze information to improve clinical care and operations. Results To date, medical information on more than 260 dental patients over 600 clinic visits has been documented in the adapted electronic health record. Conclusion The customization capability of axiUm facilitated efficient and effective development and implementation processes. Training and user support were essential for effective implementation and led to further system refinements.

Resumo:

Recanalization of previously coiled aneurysms remains a major drawback of endovascular aneurysm therapy. We performed a prospective single arm trial to provide early initial data regarding the safety and angiographic durability of a new coil technology, the Axium MicroFX Polyglycolic/polylactic acid (PGLA) coil, which was designed to lower recanalization rates. Fifteen patients (16 aneurysms) were prospectively enrolled. Demographic and peri-procedural data were collected. Angiographic images of the initial coil embolization and three to six month follow-up angiographic images underwent blinded evaluation. Seven (47%) SAH and eight (53%) elective patients were enrolled. Blinded evaluation of the initial embolization demonstrated that 5/16 (31%) aneurysms achieved Raymond grade 1, 5/16 (31%) grade 2 and 6/16 (38%) grade 3. Three to six month angiography was obtained in 12/15 patients (80%); two patients expired (1 SAH, 1 elective) and one was lost to follow-up (SAH). All patients who underwent follow-up angiography had a mRS ≤1. Blinded evaluation of embolization demonstrated 7/13 aneurysms (54%) improved in Raymond grading, five (38%) were stable and one aneurysm (8%) worsened. One patient developed an asymptomatic peri-aneurysmal parent vessel stenosis. Axium MicroFX coils appear to be safe, though the small number of patients in this series obviates comparative analysis with other series. Further studies are needed with more patients to compare the angiographic durability of Axium MicroFX coils to other coils.

Ethiopia constitutes one of the most significant environmental, religious and cultural reserves on Earth. The study aims to assess the geotourism of the monuments. The study has been conducted using literature review, fieldwork, data collection and production of geotourism maps and interpretations. Geosites are important resources for geotourism development in Ethiopia along the Historic route of Ethiopian tourism. This route is the main tourism corridor of the country in which geotourism knowledge is very important. Among the major geotourism sites in the study area are, Axum Stelea and its Quary Site, archaeological sites, monasteries, Adwa Volcanic Mountain Chains, palaces, rock-hewn churches, and viewpoints. Therefore, the study has concluded that knowing and understanding the geotourism (which includes geosites and historical heritages) is very important for the development of tourism in Tigray, Ethiopia.

Background.In addition to beneficial traditional practices, there are around 140 harmful traditional practices affecting mothers and children in almost all ethnic groups of Ethiopia. Therefore this study might give a clue about their practice and associated factors. The objective of this study was to assess magnitude of harmful traditional practices among children less than 5 years of age in Axum Town, North Ethiopia.Methods.Community based cross-sectional study was conducted on 752 participants who were selected using multistage sampling. Simple random sampling method was used to select ketenas from all kebelles of Axum Town. After proportional allocation of sample size to eachketena, systematic random sampling method was used to get the study participants. Data was collected using interviewer administered questionnaire; it was entered and analyzed using SPSS version 16 and descriptive statistics was calculated.Results.Majority of the respondents (81.2%) were Orthodox, 78.2% of the mothers had no work, and majority of mothers had no formal education. Among the harmful traditional practices performed on children, uvula cutting alone was performed on 72.8% of children followed by milk teeth extraction and uvula cutting with eyebrow incision.Conclusion.The leading harmful traditional practice performed on children in this study was uvula cutting.

In endovascular coil embolization of intracranial aneurysms, very soft coils, often called “finishing coils,” are usually selected in the final stage of coil embolization. The authors developed a radiolucent coil made of thin nylon thread to evaluate the performance of coils under a situation simulating the course of embolization. The characteristics of various types of finishing coils were investigated using radiolucent coils. Experimental embolization was performed with a silicone aneurysm filled with radiolucent coils simulating the final stage of embolization. Three indices, i.e. area, perimeter, and circularity of the inserted coils, were investigated on the X-ray images after coil insertion. The coils used were as follows: Target Ultra Helical, MicroPlex Hypersoft, Axium Helix, ED Coil Extrasoft, and DeltaPlush. In the analysis of area and perimeter, there were significant differences in multiple comparisons. There was no significant difference in circularity, although it was generally ranked in order by coil brand. Target Ultra and MicroPlex Hypersoft had higher scores for area and perimeter and lower scores for circularity, in contrast to DeltaPlush, which had lower scores for area and perimeter and a higher score for circularity. Based on these results, the finishing coils were divided into three groups: Target Ultra Helical and MicroPlex Hypersoft; Axium Helix and ED Coil Extrasoft; DeltaPlush. They are better for use in early, midst, and end of finishing, respectively. The characteristics of various finishing coils were evaluated, and the results obtained reflected actual clinical experience and provide useful information to appropriately select finishing coils.

Introduction: Lantana camara makes available huge nitrogen rich moist biomass, which has potential to be utilized as a substrate for organic recycling. Its biomass has potential for utilization as organic manure, has antimicrobial, insecticidal and medicinal properties. Similarly, Lantana camara is one of the most widely occurring shrubs that have shown alarming growth in recent years, in our mandate areas especially in shire, Axum and Adwa. However, the complete eradication of this weed without further use of its biomass is very difficult and costly.

ABSTRACTINTRODUCTION: Unsafe abortion greatly contributes to maternal mortality and morbidity in the world particularly in developing countries. Poor knowledge and unfavorable attitude towards abortion among female youths is still significantly high in Ethiopia where abortion service is not fully legalized. So far, no studies related to abortion legalization have been conducted in Ethiopia, Axum town to identify the gap. Therefore, this study was aimed at assessing female youth level of knowledge and attitude towards legalization of abortion services in the town. METHODS: Community based cross-sectional study was conducted on 400 subjects in November 2015. Respondents were selected through systematic random sampling. Structured questionnaire was used to collect the data. Data was coded and entered using Epi-info version 7 and exported to SPSS Version 20 for analysis. Descriptive and logistic regression were computed. Statistical tests were considered significant at p<0.05and 95% confidence interval. RESULT: A Only 202(49.5%) and 190(47.5%) of respondents had good knowledge and favorable attitude respectively. Age, not knowing unsafe abortion complication, and lacking information on criteria of legal abortion service were significantly associated with knowledge. Lack of formal education and no access to information were also associated with attitude. CONCLUSION AND RECOMMENDATIONS: Almost half of the respondents have good knowledge and favorable attitude towards legalization of abortion service. Age, knowledge of unsafe abortion complication, and availability of clear source of information about abortion had strong association with knowledge and attitude. Introduction: Oral health mirrors systemic health; yet, few clinics worldwide provide dental care as part of primary medical care, nor are dental records commonly integrated with medical records. Objectives: To determine the degree to which misreporting of underlying health conditions poses problems for dental clinicians, we assessed misreporting of 2 common medical health conditions—hypertension and diabetes—at the time of dental examination and assessment. Methods: Using comparative chart analysis, we analyzed medical records of a diverse group of patients previously seen at the University of Texas Physician outpatient practice and then treated at the University of Texas Health Science Center at Houston School of Dentistry. Electronic health records of patients aged ≥18 y were extracted from 2 databases: Allscripts (University of Texas Physician) and axiUm (University of Texas Health Science Center at Houston). We identified 1,013 patients with the commonly occurring conditions of diabetes, hypertension, or both, with nonintegrated records contained in Allscripts and axiUm. We identified the percentage of those patients previously diagnosed with diabetes and/or hypertension by their physicians who failed to report these conditions to their dental clinicians. Results: Of those patients with diabetes, 15.1% misreported their diabetes condition to their dental clinicians, while 29.0% of patients with hypertension also misreported. There was no relationship between sex and misreporting of hypertension or diabetes, but age significantly affected reporting of hypertension, with misreporting decreasing with age. Conclusions: Because these conditions affect treatment planning in the dental clinic, misreporting of underlying medical conditions can have negative outcomes for dental patients. We conclude that policies that support the integration of medical and dental records would meaningfully increase the quality of health care delivered to patients, particularly those dental patients with underlying medical conditions. Knowledge Transfer Statement: Our study illustrates an urgent need for policy innovation within a currently fragmented health care delivery system. Dental clinicians rely on the accuracy of health information provided by patients, which we found was misreported in ~15% to 30% of dental patient records. An integrated health care system can close these misreporting gaps. Policies that support the integration of medical and dental records can improve the quality of health care delivered, particularly for dental patients with underlying medical conditions.

Abstract Recent studies in comparative rhetoric have brought much needed attention to traditions of rhetoric in non-Western cultures, including many in Africa. Yet the exclusive focus on contemporary African cultures limits understanding of the history of rhetoric in Africa. Although extensive data on African antiquity is lacking, we know that early Nubian and Ethiopian cultures were highly civilized, socially and politically. Literacy in the ancient cities of Napata, Meroe, and Axum, and in the medieval city of Timbuktu suggests that black Africa was not exclusively oral and not without recourse to a means of recording its uses of language. This essay adds a historical dimension to comparative studies of rhetoric in Africa, showing the depth and complexity of this little known aspect of African civilizations.

As a country, Ethiopia needs no introduction. Its three thousand years of history has been told and documented by many who lived in and traveled to Ethiopia The discovery of Lucy, the 3.2 million years old hominid, iconic fossil in the Afar region of Ethiopia in 1974, attests to the fact that Ethiopia is indeed one of the oldest nations in the world. The origin of the northern Ethiopian Empire, is chronicled in the legendary story of Cush, the son of Ham and the founder of the Axumite Kingdom, who gave the name Ethiopis to the area surrounding Axum and later to his son. Ethiopia is thus derived from it which in Greek means land of the burnt or black faces.

Resumo:

Este ensaio tem por objetivo pensar uma “Antiguidade Tardia Multipolar” a partir de postulados da História Global e dos Sistema-Mundo. Para tanto, selecionou-se um estudo de caso específico – a embaixada de 496 EC vinda provavelmente de Axum até Constantinopla acompanhada de duas girafas e um elefante – como caminho de investigação. A partir dele, argumenta-se que a diplomacia pode ser entendida um elemento de coesão estruturante para um Sistema-Mundo tardo-antigo, e que a linguagem diplomática nos permite pensar políticas de reconhecimento multipolar. Neste sentido, é preciso incluir a África (especialmente em suas porções ao sul do Saara) neste cenário amplo, e uma forma de fazê-lo é a partir da compreensão de que animais específicos, como girafas e elefantes, operam formas simbólicas de linguagem diplomática e, portanto, nos permitem vislumbrar protagonismos africanos em um âmbito mais global.

Background. Ectopic pregnancy is a neglected and challenging gynecologic problem in developing countries including Ethiopia. Objective. The present study is aimed at assessing the prevalence of ectopic pregnancy, its management outcomes, and factors associated with management outcomes in Tigray, North Ethiopia. Methods. We employed a four-year retrospective cross-sectional study from September 2015 to August 2019. We extracted data about all pregnant mothers who were admitted and managed for EPs in Axum, Tigray. Ectopic pregnancy and its outcomes (favorable and unfavorable) were the dependent variables, and age, residence, ethnicity, religion, parity, history of abortion, history of EP, pelvic infections, history of surgical procedures, and use contraceptives were the independent variables. We employed descriptive statistics and bivariate and multivariate logistic regression analyses using SPSS. Ethical clearance was obtained from Axum University, Tigray, Ethiopia. Results. The overall prevalence of ectopic pregnancy was 0.52% of total deliveries, which equates to 1 : 193 deliveries. Surgery for ectopic pregnancy accounts for 7.6% of all gynecological surgeries. Most participants were in the age group 26–30 years and lived in rural areas. Among the different EP implantation sites, most cases (92.4%) occurred in the fallopian tube, followed by 5.1% in the ovary and 2.5% in abdominal EPs. Surgical management (laparotomy) was undertaken for all the 79 women diagnosed with EPs, including laparotomy (100%), salpingo-oophorectomy (17.7%), salpingectomy (73.9%), oophorectomy (3.4%), cornual resection (2.5%), and removal of concepts tissue 2.5. The record reports that intraoperative procedure was correctly managed for 47 (59.5%) women but the condition of EP procedure was ruptured for about two-thirds (63.3%) of the women. Thirty (38%) patients had developed some complications after surgery including anemia ( hemoglobin < 10.5 ) ( n = 12 ), fever ( n = 10 ), wound infection ( n = 2 ), and pneumonia ( n = 2 ). Women who were from urban ( AOR = 11.2 , 95% CI: 2.65-47.2) and who had normal hemoglobin at presentation ( AOR = 9.94 , 95% CI: 2.03-48.7) were associated with favorable maternal outcomes. Conclusions. More than one-third of women with ectopic pregnancies had an unfavorable maternal outcome, which was higher among rural residents and anemic mothers. Women living in rural areas and anemia during pregnancy should seek special attention in the management of EPs. We also recommend improving the data management of hospitals in Ethiopia.

Сироватковий альбумін відіграє важливу роль у багатьох фізіологічних процесах. Останнім часом показано його безпосередню участь у реалізації протизапальних процесів. Багато нестероїдних протизапальних засобів, зокрема, виявляють захисний ефект проти теплової денатурації сироваткого альбуміну. У роботі наведено результати вивчення протизапальної активності in vitro пропоксазепаму та ряду похідних 1,4-бенз-діазепіну за ефектом протективного впливу на стабільність сироваткого альбуміну в умовах термічної денатурації. Протективний ефект сполук розраховували як ІС50, фізико-хімічні параметри визначали за допомогою програм ACD/Labs 12.01 та Chem Axom (Marvin Sketch 21.7). Встановлено, що за величинами ІС50 діазепам і пропоксазепам майже вдвічі перевищують референтний препарат ібупрофен. Сполуки з вільною гідроксигрупою в положенні 3 (оксазепам та 3-гідроксипропоксазепам) виявляли менший ефект, що може бути пов’язано з їх здатністю до зв’язування з бичачим сироватковим альбуміном. Поляризованість молекули похідних 1,4-бенздіазепіну більшою мірою впливає на їх зв’язування з бичачим сироватковим альбуміном та прояв їх захисної дії, ніж ліпофільність, що дає підставу припустити значний внесок індукційної взаємодії у цей процес.

Geoheritage conservation in Ethiopia: the case of the Simien Mountains Ethiopia constitutes one of the most significant environmental and cultural reserves on Earth. Ethiopia's natural and cultural tourist attractions are mostly associated with geological features: the active Ethiopian and Afar rifts as well as the Simien and Bale massifs are few examples. Ethiopia's cultural history, religious manifestations and civilization, like the rock-hewn churches of Lalibela and the stelae of Axum, are also imprinted in rock. Geomorphological and geological features, notably the isolation of the north-western highlands from the external world by the harsh Afar depression close to the sea, determined the route of Ethiopian history. Though tourism has been identified as a major sustainable development sector, systematic geoheritage evaluation and conservation strategies are lacking in the country. I this paper the Simien Mountains are presented as major geoheritages which should be prioritized for geoconservation in order to develop sustainable tourism (geotourism) in the area.

More than 250 beads and pendants have been registered from the Harbor Temple and its surroundings, the so-called “harbor temenos”, in the Red Sea port of Berenike. The Harbor Temple assemblage is dominated by South Asian glass beads dating from the 4th through early 6th centuries AD, but the bead finds from the presumed temenos show much greater variety in both type and date, the latter spanning the centuries from the 1st to the 5th century AD. Rather than being accidentally lost, the quantity and find context of the beads support the idea of a votive offering function. Stylistic similarities of some objects found in the Harbor Temple have led to their association with South Arabia or Axum (Rądkowska, Sidebotham, and Zych 2013); yet a much closer affiliation might also be considered. Similar ritual objects, as well as beads and pendants, have been recorded at contemporary temples and shrines in Nubia

Embora a Etiópia seja dotada de um cenário geológico adequado para abrigar variedades de pedras preciosas, ainda não foi bem investigada e explorada. No norte da Etiópia, estado regional de Tigrai, o mineral safira foi extraído por garimpeiros tradicionais e artesanais. Chilla, que fica a 35 km ao norte de Axum, é uma das atividades de mineração artesanal de safira praticada. Na Etiópia, particularmente na área de Chilla, não há nenhum estudo geológico e geoquímico sobre o mineral safirae suas rochas hospedeiras. Na área de Chilla, a maioria das safiras é encontrada em depósitos aluviais ou eluviais, tornando difícil estudar a relação entre as safiras e suas rochas hospedeiras. Aqui, apresentamos características geológicas preliminares do mineral safira dentro de basaltos alcalinos do terreno vulcânico terciário de Chilla. A investigação geológica e a análise geoquímica (XRF) indicam que o mineral safira está hospedado com basaltos alcalinos terciários e ocorreu no contato entre basalto alcalino e rochas graníticas intrusivas.

Vertebral artery dissection is a common cause of stroke in young adults without predisposing risk factors for cerebrovascular disease. We describe the case of a 28-year-old patient who presented with an ischemic stroke secondary to a stab wound to the neck that affected the vertebral artery. A physical examination revealed neurological deterioration (Glasgow 8/15), a sutured neck wound, no palpable hematoma, no thrills, and no active bleeding. A computed tomography angiography revealed a left vertebral artery arteriovenous fistula with a component of a pseudoaneurysm, for which a neurointerventional consultation was carried out. Due to neurological compromise, the airway was secured, and because the case involved a posterior fossa infarction with compression of the fourth ventricle and obstructive secondary hydrocephalus, an external ventricular shunt was inserted by neurosurgery. A fistula occlusion was performed with five Axium coils and a vial of Squid 12; the vertebral artery was catheterized, and a craniotomy was performed to manage hydrocephalus with a 12-mm H2O collecting system. The patient was discharged on the tenth day after admission with sequelae of left hemiparesis (predominantly brachial) and no other deficits. There was no hemorrhagic transformation on the control computed tomography scans and no further complications.

Background: Persistent fetal carotid-vertebrobasilar anastomoses are rare, with an incidence of <1%. The most common anomaly seen in this group is a persistent primitive trigeminal artery; others such as a persistent hypoglossal artery account for less than 15% of all persistent fetal anastomoses, making this finding exceedingly rare. Methods: We present the case of a 32-year-old-female with Poland syndrome (right-sided), who presented with thunderclap headache and reduced level of consciousness secondary to diffuse subarachnoid hemorrhage and hydrocephalus. CT and catheter angiography demonstrated an aneurysm of the V4 segment of the right vertebral artery arising from a persistent right hypoglossal artery, with an absent ipsilateral vertebral artery proximal to the anomaly. Results: Hydrocephalus was treated with an EVD, followed by a successful embolization of the V4 aneurysm with Axium coils. Subsequent MR studies demonstrated minimal recanalization of the aneurysm, and small foci of possible infarcts in the hippocampi. Four months later, the patient has some persistent short term memory difficulties but is otherwise neurologically intact. Conclusions: We present a rare finding of a persistent fetal hypoglossal artery with an associated vertebral aneurysm. The aneurysm was successfully treated endovascularly through coil embolization with minimal residual -neurological deficit. This vascular anomaly was ipsilateral to her Poland Syndrome defects.
