Background Students in the United States spend a meaningful portion of their developmental lives in school. In recent years, researchers and educators have begun to focus explicitly on social and emotional learning (SEL) in the school setting. Initial evidence from meta-analyses suggests that curricula designed to promote SEL likely produce benefits in terms of social-emotional competence (SEC) and numerous related behavioral and affective outcomes. At the same time, there are often barriers to implementing such curricula as intended, and some researchers have questioned the strength of the evaluation data from SEL programs. As part of the effort to improve programming in SEL, this paper describes the protocol for a cluster randomized trial of the ACT OUT! Social Issue Theater program, a brief psychodramatic intervention to build SEC and reduce bullying behavior in students. Objective The objective of this trial is to examine if a short dose of interactive psychodrama can affect SEC metrics and bullying experiences in schoolchildren in either the short (2-week) or medium (6-month) term. Methods The ACT OUT! trial is a cluster randomized superiority trial with 2 parallel groups. The unit of measurement is the student, and the unit of randomization is the classroom. For each grade (fourth, seventh, and 10th), an even number of classrooms will be selected from each school—half will be assigned to the intervention arm and half will be assigned to the control arm. The intervention will consist of 3 moderated psychodramatic performances by trained actors, and the control condition will be the usual school day. Outcome data will be collected at baseline (preintervention), 2-week postintervention (short term), and 6-month postintervention (medium term). Outcomes will include social-emotional competency; self-reported bullying and experiences of being bullied; receptivity to the program; and school-level data on truancy, absenteeism, and referrals to school displinary action for bullying. A power analysis adjusted for clustering effect, design effect, and potential attrition yielded a need for approximately 1594 students, consisting of an estimated 80 classrooms split evenly into intervention and control arms. Results This study was funded in June 2019; approved by the Indiana University Institutional review board on September 17, 2019; began subject recruitment on November 5, 2019; and prospectively registered with ClinicalTrials.gov. Conclusions Many states have issued recommendations for the integration of SEL into schools. The proposed study uses a rigorous methodology to determine if the ACT OUT! psychodramatic intervention is a cost-effective means of bolstering SEC and reducing bullying incidence in schools. Due to increase in demand for energy as a result of human population explosion, industrialization and environmental hazards posed by fossil fuels, there is a need to source for alternative energy sources that are cheaper and environmental friendly. Three different lignocellulosic biomasses were studied for their suitability for bioethanol production. Fungi and yeasts were isolated using serial dilution and spread plate methods. Identification of both fungi and yeasts was done using their cultural and microscopy characteristics. Saccharification of the pre-treated biomass was done with both crude cellulase and mycelia inoculant. Bioethanol was produced using batch culture fermentation. Ethanol produced was detected using spectrometric method and quantified using High Performance Liquid Chromatography (HPLC). The effects of substrate concentration, pH and temperature on ethanol yield were optimized. Fifty fungal isolates were obtained from soil collected. Six yeasts, all Kluyveromyces species fermented three sugars to ethanol with isolate Kluyveromyces sp.Y2 having the shortest time. It was selected for fermentation. Aspergillus niger S48 had highest cellulase activity measured in a zone of hydrolysis of 26.0 mm. It had the highest glucanase activity, endoglucanase (0.462 U/mL) and exoglucanase (0.431 U/mL). The outcome of this study indicated that crude cellulase produced by Aspergillus niger S48 hydrolyzed the pre-treated rice chaff with 1.07 mg/mL of fermentable sugars higher than 0.87 mg/mL when the mycelia of the fungus was inoculated to pretreated rice chaff for hydrolysis. Ethanol was optimally produced at 12 % substrate concentration using rice chaff, at a temperature of 35 °C and pH 5.0.

Background: A child brings happiness, love and joy in the family and society and if a child born with Intellectual Developmental Disability (IDD), then parenting of IDD children requires a huge amount of liability, accountability, commitment and dedication. Children with intellectual disability are having debilitate physical and mental health conditions, lower longevity1. As parents are the primary caregivers, but lives of parents of children with IDD are much more complicated and stressful as compared to the lives of non-disabled childrens parents. Methodology: Quasi-experimental Time Series Research Design was used to assess the effectiveness of Parenting Skill Program on childs adaptive behavior among parents of children with Intellectual developmental disability in selected Special Schools of Delhi NCR.The tools used for data collection included a socio-demographic proforma and the Vineland Social Maturity Scale (VSMS). Structured interviews and pre-assessment tools, such as socio-demographic proforma and the Vineland Social Maturity Scale (VSMS), were administered to both experimental and control groups in the first month. Subsequently, the parenting skills program, consisting of 12 sessions, was implemented over a three-month period, with one session per week for parents in the experimental group. At the 6th, 9th, and 12th months, structured interviews using post-assessment tools were conducted with parents from both groups. Results: The findings of the study revealed thatthere was an improvement in the mean posttest score of childs adaptive behavior over the time. Specifically, at 6th month, it increased to (53.425±8.95), followed by (52.850±9.77) at 9th month and (55.288±8.87) at 12th month when compared to baseline score (46.362±8.09).This increase was found to be statistically significant within the group (F= 163.040, p 0.001).However, the control group, showed no significant change in the mean score of childs adaptive behavior and it remained same in 6th month, 9th month and 12th month as baseline, and no significant difference was observed within the group (F= 6.00, p 0.112). Hence, it could be interpreted that parenting skill program was effective in improving the childs adaptive behavior score of children with IDD in experimental group than in control group. Conclusion:It was concluded thatparenting skill program was effective in improving the childs adaptive behavior of children with IDD.

Abstract. Bartter's syndrome is characterized by chronic hypokalaemia, activation of the renin-angiotensin system and normal blood pressure. To investigate whether a generalized disturbance of sodium-potassium pump function might be of pathogenetic importance, lymphocytic sodium-potassium homeostasis was examined in 5 patients suffering from Bartter's syndrome. Two of the patients were treated with potassium chloride supplementation, the others were without medical treatment when studied. All were severely hypokalemic (serum potassium 2.8 ± 0.24 mmol/l, mean ± sem). Lymphocyte sodium and potassium concentration (14.4 ± 0.37 and 94.4 ± 7.7 mmol/l, respectively), ouabain sensitive 22Na-efflux rate constant (2.68 ± 0.25 h −1), and absolute ouabain sensitive efflux rate (38.16 ± 4.2 mmol · I−1 · h−1) did not differ from matched controls. Ouabain binding capacity was 126900 ± 235000 sites/cell in the patients vs 50400 ± 17900 in controls (p <0.05). In conclusion, patients with Bartter's syndrome may have an intrinsic abnormal pump function, characterized by an increased pump density and a low cation turn-over rate per pump unit.

Jaffna Municipal Council is the heart of Jaffna district with a land area of around 20.20 km2 and 27,820 families are living at present. JMC is maintaining the groundwater recharging ponds, wastewater disposal systems and drainages. At present storm water drainage is using as wastewater drainage and using as a wastewater collecting tank. Therefore, wastewater treatment is needed to provide a quality and affordable environment. A detailed literature review was carried out to understand the reasons for this problem and how to overcome them. A survey with a questionnaire and wastewater testing analysis was carried out. According to the analysis majority of the people are using open dug well, therefore, groundwater should be protected and 55.83% of the lands are 10-20 perches and some main parameters were above the standard level. Based on the findings of the feasibility study centralized conventional wastewater treatment plant with tertiary or advanced treatment is very much feasible. The common conventional process with advanced treatment was used and the treatment plant was designed to 17900 m3 /day to accommodate 109,535 people. The total cost of the project is 473 million. With the propose system waste water quality will be well below the required standard levels. A wastewater treatment plant is most important for JMC to reduce environmental pollution and reduce groundwater pollution and maintain the groundwater table.

Abstract The process of reforestation of forest stands is associated with the implementation of a number of measures aimed at the renewal of forest resources at certain financial costs. Reforestation activities in the Penza Region from 2008 to 2018 were carried out on an area of 19872.2 hectares, from 2019 to 2028 they are provided for on an area of 17900 hectares. In 2019 forest restoration was carried out on an area of 1392.4 hectares (10% higher than planned), including artificial reforestation by planting seedlings on an area of 561.3 hectares in the spring at the optimal time, as well as natural restoration (promoting natural renewal) of soil surface mineralization - 831 hectares (17% higher than planned). Total area of reforestation in 2020 - 1327.9 hectares, including planting of forest crops on an area of 427.4 hectares and the addition of plantings of different years on an area of 1198.1 hectares, agrotechnical and forestry care of forest crops - 4797.2 hectares. Within the framework of the federal project "Forest Conservation" of the national project "Ecology" for 2019-2020. artificial reforestation was carried out on an area of 270.1 hectares and agrotechnical care was carried out on an area of 3203.7 hectares. To increase the quantitative and qualitative characteristics of forests, contributing to an increase in their resistance to unfavorable biotic and abiotic environmental factors, it is necessary to increase the areas of artificial reforestation by introducing promising technologies for creating forest crops.

The paper reports a study of the chemical composition of 25400 grass silages produced in England and Wales, of which 1040 were from the period 1961–63, 17900 (15480 clamped and 2420 baled) were from 1982–88 and 6460 (5640 clamped and 820 baled) were from 1992–93. A sample of each silage had been analysed for dry matter (DM), pH, crude protein (CP) and volatile N and in most cases for ash.The baled silages were drier on average than the clamped silages, with a wider spread of DM contents. The clamped silages with the lowest pH on average were those with a DM content of c. 240 g/kg; clamped silages wetter than this were progressively higher in pH with reduction in DM content; clamped and baled silages drier than 240 g/kg were progressively higher in pH with increase in DM content, up to pH 5·9 in baled silages of 580 g DM/kg. At a given DM content pH was on average 0·6 units higher in baled than in clamped silages. The ash content of baled silages was similar to that of clamped silages. There was an increase in the mean CP content of clamped silages between 1961–63 and 1992–93 and a reduction in the proportion of CP which was volatile. The relationship between DM and volatile N as a proportion of total N was a curve rather than a straight line, with the steepest part of the curve at the lower end of the DM range. The wet silages harvested in 1961–63 were particularly high in volatile N. At a given DM content in the range 170–320 g/kg, volatile N was rather higher in baled than in clamped silages in both 1982–88 and 1992–93. Clamped silages with a DM content of at least 250 g/kg and baled silages with a DM content of at least 310 g/kg were typically well preserved in the sense of having <100 g volatile N/kg total N.

Background Schools increasingly prioritize social-emotional competence and bullying and cyberbullying prevention, so the development of novel, low-cost, and high-yield programs addressing these topics is important. Further, rigorous assessment of interventions prior to widespread dissemination is crucial. Objective This study assesses the effectiveness and implementation fidelity of the ACT Out! Social Issue Theater program, a 1-hour psychodramatic intervention by professional actors; it also measures students’ receptiveness to the intervention. Methods This study is a 2-arm cluster randomized control trial with 1:1 allocation that randomized either to the ACT Out! intervention or control (treatment as usual) at the classroom level (n=76 classrooms in 12 schools across 5 counties in Indiana, comprised of 1571 students at pretest in fourth, seventh, and tenth grades). The primary outcomes were self-reported social-emotional competence, bullying perpetration, and bullying victimization; the secondary outcomes were receptiveness to the intervention, implementation fidelity (independent observer observation), and prespecified subanalyses of social-emotional competence for seventh- and tenth-grade students. All outcomes were collected at baseline and 2-week posttest, with planned 3-months posttest data collection prevented due to the COVID-19 pandemic. Results Intervention fidelity was uniformly excellent (>96% adherence), and students were highly receptive to the program. However, trial results did not support the hypothesis that the intervention would increase participants’ social-emotional competence. The intervention’s impact on bullying was complicated to interpret and included some evidence of small interaction effects (reduced cyberbullying victimization and increased physical bullying perpetration). Additionally, pooled within-group reductions were also observed and discussed but were not appropriate for causal attribution. Conclusions This study found no superiority for a 1-hour ACT Out! intervention compared to treatment as usual for social-emotional competence or offline bullying, but some evidence of a small effect for cyberbullying. On the basis of these results and the within-group effects, as a next step, we encourage research into whether the ACT Out! intervention may engender a bystander effect not amenable to randomization by classroom. Therefore, we recommend a larger trial of the ACT Out! intervention that focuses specifically on cyberbullying, measures bystander behavior, is randomized by school, and is controlled for extant bullying prevention efforts at each school. The protease inhibitor Lopinavir (LPV) combined with low dose ritonavir (r) in the formulation of a soft gelatine capsule (SGC) has been widely used in antiretroviral therapy and has been shown to produce safe therapeutic plasma levels. In June 2006, the LPV/r combination was approved as a tablet (TBL) in Germany and since then has replaced the capsule form. The purpose of this study was to analyze and compare pharmacokinetics of Lopinavir both in the new meltrex tablet formulation and the LPV/r soft gelatine capsule during long-term treatment of HIV. Methods Included in this study were all patients followed at multiple study centers between January 2003 and August 2007 for whom LPV plasma levels were archived during therapeutic drug monitoring. A total of 4010 LPV plasma levels from 721 patients met these inclusion criteria. From these, those patients for whom complete data sets were missing were excluded from the study. Complete data sets were defined as those data that included information regarding cumulative daily dose, the patient's anti-retroviral drug regimen, and the time of measurement after drug intake. A total of 571 LPV plasma levels from 138 patients met the full criteria for inclusion in this retrospective study. Additionally, a small prospective study of LPV plasma levels in 6 patients with 24 data points was conducted in order to increase the reliability of this report. Results At 3 h after dosing, plasma levels of LPV were significantly higher in patients who had received the meltrex tablet (Mdn = 10030 ng/ml, min = 3323, max = 17280) compared to those who had received the soft gelatine capsule (Mdn = 7964 ng/ml, min = 120, max = 17900). This difference was statistically significant (p < 0.05). There were no statistically significant differences between the LPV plasma levels of the tablet and the capsule at 6, 9 and 12 h after dosing. Additionally, there were no differences between the trough levels of LPV in the two formulations. However, great variances of plasma levels were detected at time 12 h after dosing for both treatment groups (s(capsule) = 3213 ng/ml, s(tablet) = 2273 ng/ml). Conclusion The LPV/r tablet formulation is able to produce equivalent to significantly higher plasma levels of LPV when compared with the SGC formulation at time 3 h after drug intake. In addition, the known sex difference in the pharmacokinetic profile of the tablet formulation was verified in this study.

Background Digital therapeutics are evidence-based therapeutic interventions driven by high-quality software programs for the treatment, prevention, or management of a medical disorder or disease. Many studies in the western population have shown the effectiveness of mobile app–based digital therapeutics for improving glycemic control in patients with type 2 diabetes (T2D). However, few studies have assessed similar outcomes in the South Asian population. Objective This study aims to investigate the real-world effectiveness of the Wellthy CARE digital therapeutic for improving glycemic control among the South Asian population of Indian origin. Methods We analyzed deidentified data from 102 patients with T2D from India enrolled in a 16-week structured self-management program delivered using the Wellthy CARE mobile app. Patients recorded their meals, weight, physical activity, and blood sugar in the app, and they received lessons on self-care behaviors (healthy eating, being active, monitoring, medication adherence, problem solving, healthy coping, and reducing risks); feedback provided by an artificial intelligence–powered chatbot; and periodic interactions with certified diabetes educators via voice calls and chats. The primary outcome of the program was a change in glycated hemoglobin A1c (HbA1c). Secondary outcomes included the difference between preintervention and postintervention fasting blood glucose (FBG) and postprandial blood glucose (PPBG) levels; changes in BMI and weight at the completion of 16 weeks; and the association between program engagement and the changes in HbA1c, FBG, and PPBG levels. Results At the end of 16 weeks, the average change in HbA1c was –0.49% (n=102; 95% CI −0.73 to 0.25; P<.001). Of all the patients, 63.7% (65/102) had improved HbA1c levels, with a mean change of −1.16% (n=65; 95% CI −1.40 to −0.92; P<.001). The mean preintervention and postintervention FBG levels were 145 mg/dL (n=51; 95% CI 135-155) and 134 mg/dL (n=51; 95% CI 122-146; P=.02) and PPBG levels were 188 mg/dL (n=51; 95% CI 172-203) and 166 mg/dL (n=51; 95% CI 153-180; P=.03), respectively. The mean changes in BMI and weight were –0.47 kg/m2 (n=59; 95% CI −0.22 to −0.71; P<.001) and –1.32 kg (n=59; 95% CI −0.63 to −2.01; P<.001), respectively. There was a stepwise decrease in HbA1c, FBG, and PPBG levels as the program engagement increased. Patients in the highest tertile of program engagement had a significantly higher reduction in HbA1c (−0.84% vs −0.06%; P=.02), FBG (−21.4 mg/dL vs −0.18 mg/dL; P=.02), and PPBG levels (−22.03 mg/dL vs 2.35 mg/dL; P=.002) than those in the lowest tertile. Conclusions The use of the Wellthy CARE digital therapeutic for patients with T2D showed a significant reduction in the levels of HbA1c, FBG, and PPBG after 16 weeks. A higher level of participation showed improved glycemic control, suggesting the potential of the Wellthy CARE platform for better management of the disease.

Background General practices (GPs) in England have recently introduced a nationwide electronic personal health record (ePHR) system called Patient Online or GP online services, which allows patients to view parts of their medical records, book appointments, and request prescription refills. Although this system is free of charge, its adoption rates are low. To improve patients’ adoption and implementation success of the system, it is important to understand the factors affecting their use of the system. Objective The aim of this study is to explore patients’ perspectives of factors affecting their use of ePHRs in England. Methods A cross-sectional survey was carried out between August 21 and September 26, 2017. A questionnaire was used in this survey to collect mainly quantitative data through closed-ended questions in addition to qualitative data through an open-ended question. A convenience sample was recruited in 4 GPs in West Yorkshire, England. Given that the quantitative data were analyzed in a previous study, we analyzed the qualitative data using thematic analysis. Results Of the 800 eligible patients invited to participate in the survey, 624 (78.0%) returned a fully completed questionnaire. Of those returned questionnaires, the open-ended question was answered by 136/624 (21.8%) participants. A total of 2 meta-themes emerged from participants’ responses. The first meta-theme comprises 5 themes about why patients do not use Patient Online: concerns about using Patient Online, lack of awareness of Patient Online, challenges regarding internet and computers, perceived characteristics of nonusers, and preference for personal contact. The second meta-theme contains 1 theme about why patients use Patient Online: encouraging features of Patient Online. Conclusions The challenges and concerns that impede the use of Patient Online seem to be of greater importance than the facilitators that encourage its use. There are practical considerations that, if incorporated into the system, are likely to improve its adoption rate: Patient Online should be useful, easy to use, secure, and easy to access. Different channels should be used to increase the awareness of the system, and GPs should ease registration with the system and provide manuals, training sessions, and technical support. More research is needed to assess the effect of the new factors found in this study (eg, lack of trust, difficulty registering with Patient Online) and factors affecting the continuing use of the system.
