Inpatient treatment of children and adolescents forms an important part of modern psychiatric care. The place of residential treatment has changed: First, effective evidence-based community treatments are now more readily available; at the same time, the increasing recognition of youth mental illness has led to greater demands for admission. Admission of more suicidal adolescents with higher complexity and greater levels of violence and self-harm has resulted in a complex inpatient environment. Second, there is a drive for shorter admissions, partly driven by costs and resource limitations. The running of residential services is a complex activity, and understanding the group dynamics and careful integration of multiple therapeutic modalities by a multidisciplinary team is important to any successful outcome. There are specialized services, including eating disorder, forensic/secure, learning disability, and autism units. The outcome of inpatient care is favorable, although costly, and crucially requires managed integration with outreach and community psychiatric services.

This chapter considers the following categories of hearsay: statements in public documents, works of reference, evidence of age, evidence of reputation, and statements forming part of the res gestae, statements which are admissions made by an agent of a defendant. All of these categories were established at common law as exceptions to the rule against hearsay, and all of them have been preserved by statute. All of the cases considered, apart from evidence of age and res gestae statements, have been expressly preserved and given statutory force in both criminal proceedings and civil proceedings (by s 7(2) and (3) of the Civil Evidence Act 1995).

This chapter discusses several well-established principles whereby relevant evidence is excluded because of extrinsic considerations which outweigh the value that the evidence would have at trial. Three types of privilege are considered: (i) the privilege against self-incrimination (including statutory withdrawal of the privilege, compatibility with Article 6 of the European Convention on Human Rights, the compulsory production of pre-existing documents and materials, and substituted protection); (ii) legal professional privilege, which enables a client to protect the confidentiality of (a) communications between him and his lawyer made for the purpose of obtaining and giving legal advice (known as ‘legal advice privilege’) and (b) communications between him or his lawyer and third parties for the dominant purpose of preparation for pending or contemplated litigation (known as ‘litigation privilege’); and (iii) ‘without prejudice’ privilege, which enables settlement negotiations to be conducted without fear of proposed concessions being used in evidence at trial as admissions.

This chapter comments on Article 65 of the Rome Statute of the International Criminal Court. Article 65 deals with proceedings where the accused makes an admission of guilt pursuant to article 64, paragraph 8 (a). Upon the admission of guilt, the Trial Chamber has three options: it may convict the accused on the basis of the admission of guilt and any additional evidence presented; it may order the trial to continue under the ordinary procedure, disregarding the admission of guilt, with the further option of remitting the case to another Trial Chamber; or it may request the presentation of additional evidence, including witness testimony.

Patients with haematological malignancies require admission to the intensive care unit (ICU) due to the underlying disease, as a consequence of treatment with chemotherapy or after haematopoietic stem cell transplantation. With an increasing numbers of patients being diagnosed with these diseases and longer survival as treatments improve, the burden on ICU is anticipated to increase. There is compelling evidence that patients should not be denied admission to ICU based on the presence of a haematological malignancy. In this chapter the disease- and treatment-related reasons for ICU admission, outcome, and risk prediction scores for patients with haematological malignancies are discussed.

Anaesthetists have a central role in cardiopulmonary resuscitation (CPR). The incidence of treated out-of-hospital cardiopulmonary arrest is 40 per 100 000 population and is associated with a survival rate to hospital discharge of 8–10%. The incidence of in-hospital cardiac arrest (IHCA) is 1–5 per 1000 admissions and is associated with a survival rate to hospital discharge of 13–17%. The most effective strategy for reducing mortality from IHCA is to prevent it occurring by detecting and treating those at risk or to identify in advance those with no chance of survival and to make a decision not to attempt resuscitation. The European Resuscitation Council and the Resuscitation Council (UK) publish guidelines for CPR every 5 years and the evidence supporting these is described in the international consensus on CPR science. The advanced life support algorithm forms the core of the guidelines but the precise interventions depend on the circumstances of the cardiac arrest and the skills of the healthcare providers. High-quality CPR with minimal interruptions will optimize survival rates. Shockable rhythms are treated with defibrillation while minimizing the pause in chest compressions. Although adrenaline (epinephrine) is used in most cardiac arrests, no studies have shown that it improves long-term outcome. The post-cardiac arrest syndrome is common and requires multiple organ support in an intensive care unit. Therapy in this phase is aimed at improving neurological (e.g. targeted temperature management) and myocardial (e.g. percutaneous coronary intervention) outcomes. Based on standard outcome measurements (e.g. cerebral performance category), 75–80% of survivors will have a ‘good’ neurological outcome, but many of these will have subtle neurocognitive deficits.
