Abstract Background Due to severe immunosuppression, patients undergoing allogeneic hematopoietic stem cell transplantation (aSCT) are at increased risk of infection and especially bloodstream infections (BSI) remain a major cause of death. Knowledge of the specific epidemiology of pathogens and resistances is of utmost importance to optimize antimicrobial treatment strategies. Methods Based on the Cologne Cohort of Neutropenic Patients (CoCoNut) database, we conducted a retrospective analysis of blood cultures collected within 100 days following transplantation of patients undergoing aSCT between January 2009 and December 2018 at the University Hospital of Cologne, Germany. Contamination of coagulase-negative Staphylococci (CoNS) isolates (single positive isolate within 5 days) was considered within the analysis. Results In total, 843 aSCT patients were available for analysis (484/843 [57%] male). The median age was 53 (interquartile range [IQR] 43–62) years, predominant underlying diseases were acute myeloid leukemia (47%, 397/843), lymphoma (14%, 117/843), and acute lymphoblastic leukemia (11%, 89/843). Median inpatient stay was 39 (IQR 34–50) days, while 67/843 (8%) patients died. Antibacterial prophylaxis was administered in 289/843 (34%) and antifungal prophylaxis in 738/843 (88%) patients. BSI was diagnosed in 233/843 (28%) patients. In total, 5,489 pairs of blood cultures were taken (median 4 per patient, IQR 2–8), while a pathogen could only be detected in 922/5,489 (17%). Most frequent pathogens were CoNS (259/922, 28%), Enterococcus spp. (219/922, 24%), E. coli (132/922, 14%), Klebsiella spp. (44/922, 5%), P. aeruginosa (39/922, 4%), S. aureus (37/922, 4%), and Candida spp. (42/922, 5%). Polymicrobial infection was detected in 58/922 (6%) cases. Within Enterococci isolates, 24/219 (11%) were VRE. None of the Klebsiella, but 9/132 (7%) of E. coli isolates were ESBL positive. In 4/37 (11%) cases S. aureus isolates were MRSA. Conclusion Patients in the early phase after aSCT are at high risk of BSI with a predominantly gram-positive spectrum. Empirical antimicrobial treatment must consider pathogen epidemiology and resistance patterns while waiting for blood culture results. Disclosures All authors: No reported disclosures.

Abstract Background Osteoporosis is one of the leading causes of morbidity and mortality in older women and men. Men with osteoporosis have higher fracture-related morbidity and mortality rates compared to women, but screening rates in men remain much lower. Moreover, there is limited veteran-specific data despite the suggestion that veterans may be at higher risk for osteoporosis. (1) Objective: To assess current dual energy X-ray absorptiometry (DEXA) ordering practices within a veteran cohort. Design: A retrospective cohort study of male veterans aged71-75 years who were evaluated by a primary care provider (PCP) within the last 5 years (n = 13,174). Results DEXAs wereordered for ∼7% of the population (920/13,174), and 91.6% were completed (843/920). Over 60% of completed DEXAs were ordered due to the presence of risk factors (308/843, 36.5%), history of fractures (60/843, 7%), known osteopenia or osteoporosis (59/843, 7%), or osteopenia mentioned on another imaging modality (78/843, 9.3%), while only 33.5% (282/843) were ordered for screening purposes. Of those with DEXA results reported (838/843, 99.4%), 34.5% (289/838) were normal, 51% (427/838) were in the osteopenia range and 14.5% (122/838) were in the osteoporosis range. Regarding DEXAs obtained for screening purposes, 42% (118/282) were normal, 48% (136/282) were in the osteopenia range, and 10% (28/282) were in the osteoporosis range. Conclusions Despite the Endocrine Society and National Osteoporosis Foundation recommending screening men aged ≥70 years, less than 10% of male veterans eligible for osteoporosis screening based on age alone had a DEXA scan. Of those male veterans who received a DEXA for screening purposes, almost 60% were found to have an abnormal result. The lack of screening for osteoporosis in this population has the potential to worsen the disparity in bone health between men and women, and between veterans and non-veterans, making it difficult to determine the scope of a problem which has the potential to cause significant morbidity and mortality. <p>Two cultivars of faba bean (<em>Vicia faba</em> ‘Giza 843’ and ‘Nobaria 3’) that differ in aluminum (Al) tolerance were used to study cytogenetic and genomic alterations under the influence of Al Cl<sub>3</sub> (5, 15, and 25 mmol AlCl<sub>3</sub>) for different periods (6, 12 and 24 h). Under Al treatments, mitotic index in both cultivars decreased and total chromosomal abnormalities increased. The frequencies of micronuclei and chromosomal abnormalities (C-anaphase, metaphase-star chromosomes, breaks, sticky and disturbed chromosomes during metaphase or anaphase) in ‘Giza 843’ were lower than in ‘Nabaria 3’. Increase of the registered cytogenetic events under the influence of Al stress led to increase the detected polymorphism using RAPD and ISSR markers. Application of RAPD primers gave the same value of polymorphism in both faba bean cultivars under Al stress. Polymorphism average of nine ISSR primers of ’Giza 843’ (65.36 %) was lower than that of ‘Nobaria 3’ (71.59 %). Molecular markers, cytogenetic characteristics and seedling growth data indicate that Al tolerance of ‘Giza 843’ was higher than of ‘Nobaria 3’. This work shows that cytogenetic and ISSR techniques could be used efficiently to distinguish between the ability of two faba bean cultivars to tolerate toxic effects of Al.</p>

Abstract Background: The Breast Cancer Index (BCI) is a validated gene expression assay that provides a quantitative individualized risk of late recurrence and predicts the likelihood of benefit from extended endocrine therapy (EET) in HR+ early-stage breast cancer. The objective of this analysis was to assess the impact of BCI on clinical decision-making regarding EET in the BCI Registry Study. Methods: The BCI Registry Study is an ongoing, prospective, large-scale study to investigate the long-term clinical outcome, decision impact, and medication adherence in HR+ early-stage breast cancer patients receiving BCI testing as part of routine clinical care. Per protocol, physicians and patients completed pre- and post-BCI test questionnaires to assess the following: physician decision-making regarding EET; physician confidence with decision-making; patient preferences for EET; patient concerns about the cost, side effects, drug safety and benefit of EET; and patient satisfaction regarding treatment recommendation. Pre- and post-BCI responses were compared using McNemar’s test and Wilcoxon signed rank test. Results: Pre- and post-BCI test physician and patient questionnaires were completed for 843 and 823 patients, respectively. For the patients included in this analysis, mean age at enrollment was 65y and 88.4% of patients were postmenopausal. 74.7% of tumors were T1, 53.4% were G2, 75.3% N0, and 13.8% HER2-positive. Following BCI testing, physicians changed treatment recommendations for EET in 40.1% (338/843) of patients (p&lt; 0.0001). In cases in which physicians recommended EET prior to BCI testing, 44.7% (214/479) changed their recommendation not to extend endocrine therapy. Of these cases, 98.1% (210/214) were BCI Prognostic low risk and/or BCI (H/I) Predictive low likelihood of benefit from EET. In cases in which physicians did not recommend EET prior to BCI testing, 34.6% (124/358) changed their recommendation in favor of EET. In such cases, 87.1% (108/124) of cases were BCI Prognostic high risk and/or BCI (H/I) Predictive high likelihood of benefit from EET. Further, following BCI testing, 38.8% (327/843) of physicians felt more confident in their recommendation (p&lt; 0.0001). The percentage of physicians having high confidence levels (confident or strongly confident) increased from 58.1% (490/843) before BCI testing to 80.5% (679/843) after BCI testing. The percentage of physicians having low confidence levels (not at all confident, not confident, or ambivalent) decreased from 39.1% (330/843) before BCI testing to 18.7% (158/843) after BCI testing. In addition, 40.5% (341/843) of patients felt more comfortable with their EET decision (p&lt; 0.0001) following BCI testing. Notably, changes in patient preferences for EET correlated with BCI test results. In BCI (H/I)-Low patients, 46.9% (241/514) showed a decreased preference for EET (p&lt; 0.0001) while in BCI (H/I)-High patients, 28.2% (87/309) showed an increased preference for EET (p&lt; 0.0001). Compared with baseline, after BCI testing, significantly more patients were less concerned about cost (20.9%, p&lt; 0.0001), drug safety (25.4%, p=0.0014), and the benefits of EET (29.3%, p=0.0002). No significant change in concern regarding side-effects was observed (p=0.1486). Conclusions: This first analysis in a large patient cohort of the BCI Registry confirms previous findings on the significant clinical decision-making impact of BCI for extending adjuvant endocrine therapy. Incorporating BCI into clinical practice resulted in changes in physician recommendations for EET in over 40% of cases, while at the same time increasing physician confidence in their recommendations. Knowledge of the BCI result also improved patient satisfaction and reduced patient concerns regarding cost, drug safety and benefit of EET. Citation Format: Tara B. ObjectivesAlthough family planning in China has changed gradually since 1994, there are few studies about family planning and women’s reproductive rights. The main objective of this study was to examine awareness of sexual and reproductive healthcare rights (SRHCRs), and learn how factors related to family planning influence awareness of SRHCRs among married women of reproductive age in China.Methods and participantsInner Mongolia, Chongqing, Guangdong and Henan were selected for the study, and a total of 2504 married women of reproductive age were recruited. A self-administered anonymous questionnaire was used to collect information on participants’ awareness of SRHCRs.ResultsThere were a total of 10 843 (≤6×2504) responses, with a response rate of 72.17% (10 843/15 024) on average among participants regarding SRHCRs (a multiple response set). The highest response rate was for choice (Right 3) (90.64%, 95% CI 89.47%, 91.81%), followed by privacy (Right 5) (86.11%, 95% CI 84.72%, 87.50%) and information (Right 1) (84.47%, 95% CI 83.02%, 85.93%). Only 43.39% (95% CI 41.40%, 45.38%) of participants gave responses to safety (Right 4). Participants without children showed more interest in Right 1, in access (Right 2) and in Right 4. Those who utilised tests for fetal sex determination paid more attention to Rights 2 and 4. Women who accepted informed choice were more likely to be aware of all six rights except for Right 3 and dignity (Right 6). Those individuals who were satisfied or very satisfied with comprehensive sexual and reproductive health counselling services were more likely to show interest in all six rights.ConclusionsAwareness of SRHCRs among reproductive aged women in China is still inadequate. Family planning service providers might strengthen the service awareness of sex and reproductive health rights according to the different needs of women.

Background: Social media is like a magnanimous tree and us being leaves connected through twigs of imagination Today 4.33 billion people are connected to the internet, and almost all do by using their mobiles. In Pakistan, there were 71 million broadband subscribers and 69 million 3G/4G users in June 2019. Aims & Objectives: The current study aimed to find the relationship between social support, social media usage, and psychological well-being among undergraduates aged 18-25 years. Methods: A sample of eight hundred and forty-three male and female students was included in this research. A stratified random sampling technique was applied in the research. The sample was taken from departments of various institutions in Lahore. The duration of the study was 6 months. The study was a cross-sectional study and the data were collected using questionnaires based on Ryff's scale of psychological well-being, Media and Technology Usage and Attitudes Scale, and Inventory of Social Supportive Behavior (ISSB) scale. Results: The data were analyzed on SPSS 25 and the results indicated that social support received from close ties was positively correlated with psychological well-being [r(841) = .523, n = 843, p< .001]. At the same time, social support received from online sources had no significant impact on psychological well-being while social media usage was negatively correlated with psychological well-being [r(841) = -.215, n = 843, p< .001]. Using the regression analysis technique, the three variables in combination explained 34.6% of the variance in psychological well-being [R2 = 0.346, F(6, 836) = 75.37, p< 0.01]. Age and gender had no significant effect. Conclusion: These findings implicate that healthy interpersonal relationships between close ties and undergraduate students should be encouraged and controlled usage of social networking sites should be practiced.

Abstract. The mesoscale snow distribution over the Namco lake area of the Tibetan Plateau on October 2005 has been investigated in this paper. The control and sensitive experiments were conducted using Weather Research Model (WRF) with three nested grids that included a 1 km finest grid centered on the Namco station. Our simulation ran from 6 October through 10 October 2005, which was concurrent with long term meteorological observations. Evaluation against boundary layer meteorological tower measurements and flux observations showed that the model captured the observed 2 m temperature and 10 m winds reasonably well in the sensitive experiment. The results suggested that output snow depth maximum amounts from two simulated experiments were centered downwind shore of Namco lake. Modified surface parameters for example surface skin temperature on the lake help to increase simulated credibility.

The objective of this study was to gather data regarding vaccination and deaths related to COVID-19 of the elderly population living in Santos in the years 2020 and 2021 and, through the application of an adapted quality of life assessment scale and listening to the elderly over 60 years of age living in a Long-Term Care Institution located in the Ponta da Praia neighborhood, investigate an alleged relationship between the results collected and the pandemic period related to the Coronavirus experienced. Regarding vaccinations, according to data collected in July 2022, 1,148,207 doses were administered in total. As for deaths, between March and December 2020, 843 were recorded, and between January and December 2021, 979. The survey was carried out with an exploratory purpose using an evaluator scale, whose results were analyzed qualitatively. A total of 10 individuals of both genders were interviewed, 6 males and 4 females whose age range was established from the age of 60 years. The results revealed little variation between averages and specifically low impact related to COVID-19. Complaints about physical health, autonomy, and social relationships were raised.

Sawmills in the Canadian province of British Columbia (BC) will soon be confronted with a collective wood waste disposal problem (bark, sawdust and shavings) of about 3 Million bone dry tonne/y at an average wet basis moisture content of about 45%. About 40 existing sawmill beehive burners presently incinerate this waste. Emissions from these beehive burners exceed current provincial particulate limits. Markets for the waste — board plants, sawdust digesters, charcoal plants, etc. — are limited. The economics of 25 to 50 MW wood-fired, steam power plants is poor. 6¢/kW.h is needed to finance a plant; the major utility offers 2¢/kW.h. This paper describes a 3 MW to 5 MW, Nuovo Pignone PGT-5 co-generation plant fuelled by the 2000°F (1093°C) exhaust from a Heuristic EnvirOcycler, a two-stage, wood waste incinerator. The exhaust meets BC’s particulate limit of 0.052 grains/dscf (120 mg/Nm3). 37 to 65 Million Btu/h (39 to 68 GJ/h) of waste heat can be recovered from the system exhausts. In this application the PGT-5’s external combustion chamber is replaced by a “recuperator”, i.e., a high temperature, gas-to-air, heat exchanger. Two variations of the basic system are examined. One features a larger than necessary EnvirOcycler to generate additional steam in the waste heat boiler. The other variation discusses heating 1,550°F (843°C) air from the recuperator up to 1,796°F (980°C) with natural gas. The extra power generated can cost as little as 1.9¢/kW.h.
