The starting point of this brief discussion is the emendation in line 782 of Aeschylus' Septem proposed by M.L. West in his 1990 Teubner edition. In the fifth strophe of the second stasimon, the chorus recollects the misfortunes that struck Oedipus when he finally discovered the truth about his marriage. In a recent paper, M. Finkelberg has endorsed part of M.L. West's emendation of the fifth strophe of the second stasimon in Aeschylus’ Seven against Thebes (= Sept.). In her opinion, accepting West's emendation also allows adopting earlier emendations proposed by Schütz and Prien, leading to a better understanding of the passage. It is recalled that this is where the chorus relates the disasters that ensued from Oedipus’ discovery of the truth about his marriage. In the following short discussion, I intend to revisit and defend once again the reading, according to which the two acts mentioned in connection with Oedipus’ discovery are gouging out his eyes and casting a curse on his sons, and not murdering his father and bedding his mother.

The study reported here was designed to determine whether a phytoestrogen-containing soy extract (SSE) could negate/overwhelm the inhibitory effects of ICI 182 780 on the growth of estrogen-sustained human breast cancer xenografts (MCF-7), in ovariectomized athymic mice. As expected, estradiol-supplemented tumors did not grow over the study period in ICI 182 780-treated females; concomitant administration of 50 mg/kg per day SSE slightly potentiated the inhibitory activity of the drug, while at 100 mg/kg per day, SSE partially negated ICI 182 780 activity. In keeping with these in vivo outcomes, we observed that the level of cyclin D1 (and progesterone receptor) in MCF-7 xenografts was considerably reduced by ICI 182 780, an effect enhanced by concomitant treatment with 50 SSE, but reduced by the higher dosage (i.e. 100 mg/kg per day). Thrombospondin-1 (TSP-1) and kallikrein 6 (KLK6) levels were also reduced following ICI 182 780, although to a lesser degree; again, combined anti-estrogen and SSE produced a dose-dependent regulation in TSP-1 and KLK6 tumor level, with a further reduction in the mRNA gene expression at 50 SSE (compared with ICI 182 780) and a partial reversion of the drug-induced down-regulation at 100 mg/kg per day. No modulation was detected in the serum concentration of IGF-1 (a potent mitogen for estrogen receptor-positive breast cancer cell lines) either upon treatment with ICI 182 780 or concomitant administration of the anti-estrogen with SSE. In conclusion, results from this study raise concerns about the consumption of isoflavone supplements in conjunction with ICI 182 780 therapy, in postmenopausal women with estrogen-dependent breast cancer.

A cicatrização é um processo fisiológico que ocorre diante de injúriascausadas em tecidos vivos. Muitas pesquisas estão sendo realizadasvisando à estimulação do reparo tecidual. O objetivo deste artigo foiavaliar histologicamente a resposta de tecidos epitelial, conjuntivo eósseo submetidos à laserterapia de baixa intensidade (LBI) em ummodelo experimental de reprodução alveolar. Como metodologia,foram realizadas aplicações de LBI em quatro grupos de cinco ratos,nos seguintes parâmetros: 10 mW potência única e 660 nm – 7,5 J/cm2;660 nm – 15 J/cm2 ; 780 nm – 7,5 J/cm2; e 780 nm – 15 J/cm2. Ostempos experimentais foram 3, 7, 14, 21 e 28 dias. Como resultado,os tecidos epitelial e conjuntivo mostraram renovação celular atípicae acelerada durante o período de irradiação; o tecido ósseo teve suaneoformação acelerada, porém de igual padrão ao grupo controle.Concluiu-se que os tecidos epitelial e conjuntivo reagiram àestimulação de LBI com renovação celular constante. No tecido ósseo houve uma aceleração da neoformação e reparação óssea, dentro dospadrões de normalidade.

Objective This study aimed to investigate the awareness of the epidemic among college students and their mental health as well as to explore the association between their awareness of the epidemic mental health and the daily mobile phone screen use time, in order to provide guidance for the publicity of school epidemic prevention and control knowledge and the psychological counseling of students. Methods A cross-sectional design was employed among 780 college students, The Pandemic Fatigue Questionnaire, epidemic prevention and control knowledge and the mental health Scale were used to collect data through an online survey. Results 1. Awareness rate of the transmission routes and protective measures of COVID-19 among college students is higher when the daily mobile screen use time is 3–7 hours. 2. 21.79% of the 780 college students felt stressed; 24.87% felt anxious; 19.23% showed depression. 3. The scores of each subscale in the daily mobile phone screen use time of 3–7 hours and more than 7 hours were higher, and the scores of each subscale in the group of more than 7 hours were the highest. Further correlation analysis found that the time spent on mobile phone screens was positively correlated with stress, anxiety, and depression scores (r = 0.155, 0.180, 0.182, P<0.01). Conclusion During the COVID-19 isolation and control period, college students with different mobile screen usage time have different understandings of the epidemic. Long-term mobile screen use is related to the occurrence of psychological problems such as stress, anxiety, and depression. Therefore, education departments and schools should pay attention to college students’ mobile phone use time to reduce the occurrence of bad psychological state of students.

Additively-manufactured Ti-6Al-4V (Ti64) exhibits high strength but in some cases inferior elongation to those of conventionally manufactured materials. Post-processing of additively manufactured Ti64 components is investigated to modify the mechanical properties for specific applications while still utilizing the benefits of the additive manufacturing process. The mechanical properties and fatigue resistance of Ti64 samples made by electron beam melting were tested in the as-built state. Several heat treatments (up to 1000 °C) were performed to study their effect on the microstructure and mechanical properties. Phase content during heating was tested with high reliability by neutron diffraction at Los Alamos National Laboratory. Two different hot isostatic pressings (HIP) cycles were tested, one at low temperature (780 °C), the other is at the standard temperature (920 °C). The results show that lowering the HIP holding temperature retains the fine microstructure (~1% β phase) and the 0.2% proof stress of the as-built samples (1038 MPa), but gives rise to higher elongation (~14%) and better fatigue life. The material subjected to a higher HIP temperature had a coarser microstructure, more residual β phase (~2% difference), displayed slightly lower Vickers hardness (~15 HV10N), 0.2% proof stress (~60 MPa) and ultimate stresses (~40 MPa) than the material HIP’ed at 780 °C, but had superior elongation (~6%) and fatigue resistance. Heat treatment at 1000 °C entirely altered the microstructure (~7% β phase), yield elongation of 13.7% but decrease the 0.2% proof-stress to 927 MPa. The results of the HIP at 780 °C imply it would be beneficial to lower the standard ASTM HIP temperature for Ti6Al4V additively manufactured by electron beam melting.

A esquistossomose é uma doença tropical negligenciada, com alta morbidade e mortalidade, que afeta atualmente mais de 200 milhões de pessoas em todo o mundo, tendo o praziquantel (PZQ) como única opção de tratamento. Estudos descrevem várias cepas do parasito que são resistentes ao PZQ, fazendo-se necessário, estudos de novos medicamentos que possam ser utilizados no tratamento da esquistossomose. Um dos princípios do planejamento racional de fármacos consiste na escolha de alvos onde a droga produza efeito tóxico seletivo ao patógeno, e para isso é necessário definir o mecanismo de ação do fármaco proposto. A 7-epiclusianona (7-epi) é uma benzofenona que já teve atividade esquistossomicida descrita, tendo como critérios de eficácia a motilidade, danos no tegumento, acasalamento, postura de ovos, movimentação do sistema digestório e excretor. Neste sentido, o presente estudo analisou o impacto da 7-epi nos mecanismos de proteção dos vermes adultos de S. mansoni com o auxílio de ferramentas bioquímicas e biomoleculares. Uma investigação do grau de estresse oxidativo pela titulação de malondialdeído mostrou que a 7-epi na concentração de 12,5 μg/mL possui uma taxa de peroxidação lipídica no parasito reduzida quando comparada ao controle Kolliphor®, mas aumentada na concentração de 50 μg/mL (p < 0,001). De modo similar, quando se investigou a atividade no contexto ex vivo da superóxido dismutase (SOD), uma importante enzima detoxificadora do parasito, foi possível visualizar que a 7-epi interfere na sua atividade de forma significativa em relação aos controles RPMI 1640 e Kolliphor® à partir da concentração de 12,5 μg/mL (p < 0,001). Essa dosagem corresponde a 30% de inibição da atividade enzimática total em relação ao Kolliphor® e está muito próxima da dosagem de 7-epi que matou 90% dos parasitos (ED90=13,08 μg/mL) e na qual os danos no tegumento se apresentaram intensos. Complementarmente, na análise pelo docking molecular pode-se observar que a inibição da enzima não foi realizada diretamente no sítio, e que as melhores energias de interação com a 7-epi foram em uma cavidade próxima ao sítio e na junção dos monômeros. Finalmente, foram feitas análises quantitativas por qPCR do RNAm da SOD do parasito, cujos resultados não demonstraram mudança expressiva na transcrição da enzima após ação da 7-epi. Embora haja um impacto sobre a atividade da SOD do S. mansoni, este não parece ser o mecanismo principal de ação da 7-epi. Desta forma, é imprescindível a avaliação inibitória deste composto em outras enzimas do parasito, bem como a análise de outros mecanismos não enzimáticos que elucidem a ação da 7-epi em vermes adultos de S. mansoni.Schistosomiasis is a neglected tropical disease with high morbidity and mortality, which currently affects more than 200 million people worldwide, counting only with praziquantel (PZQ) as a treatment option. Studies describe various parasite strains that are resistant to PZQ, making it necessary, studies of new drugs that can be used to treat schistosomiasis. One of the principles of drugs rational planning is to choose targets where the drug produces selective toxic effects on the pathogen, and for this is necessary to define the action mechanism of the proposed pharmaco. 7-epiclusianone (7-epi) already has schistosomicidal activity described, using as criteria of effectiveness: the motility, integument damage, mating, egg-laying and motility of the digestive and excretory system. In this direction, the present study analyzed the impact of the 7-epi against the protective mechanisms of adult worms of S. mansoni with the support of biochemical and biomolecular tools. An investigation of the degree of oxidative stress by malondialdehyde titre showed that the 7-epi at a concentration of 12,5 μg/mL has lower levels of lipid peroxidation rate comparing to the Kolliphor® control, but increased in concentration of 50 μg/ml (p <0,001). Similarly, when it investigated the activity of superoxide dismutase (SOD), an important parasite detox enzyme, in ex vivo context it was possible to see the 7-epi interfere in its activity significantly, compared to controls RPMI-1640 and Kolliphor® at a concentration of 12,5 μg /mL (p <0,001). This value corresponds to 30% of the entire enzyme activity regarding Kolliphor® and is close to the effective dose that killed 90% of the parasites (ED90=13,08 μg/mL) in which integument damages were intense. In addition, the analysis by molecular docking can observe that the inhibition of the enzyme was not performed directly on site, and that the best interaction energies with 7-epi were in a cavity near the site and in the dimer’s junction. Finally, quantitative analyzes were made by qPCR of mRNA from SOD of the parasite, that results showed no significant change in enzyme transcription after the 7-epi action. Although there is an impact on the activity of SOD of S. mansoni, this does not seem to be 7-epi main mechanism of action. Thus, it is essential the inhibitory evaluation of this compound in other parasite enzymes as well as the analysis of other non-enzymatic mechanisms that elucidate the action of 7-epi in adult worms of S. mansoni.

Die Geräte, durch die Musik im Zeitalter der technischen (Re-)Produktion gehört wird, haben sich immer wieder gewandelt. Solche Geräte müssen überhört werden. Nur so kann Musik gehört werden. Trotzdem – so die These der Arbeit – organisieren diese Geräte das Hören und sind Agenturen einer Bewirtschaftung und Technisierung der Wahrnehmung. In der Arbeit wird anhand von zwei Fallstudien aus der Geschichte der Rock- und Popmusik gezeigt, wie solche Geräte sowohl den Hörer als auch die gehörte Musik formieren. Durch Hörtechnologien bilden sich neue Hörpraktiken heraus und die Körperlichkeit des Hörers wird neu bestimmt. Die Anpassung von Klanggeschehen an spezifische Hörtechnologien wird im Mastering – dem letzten Schritt der technischen Musikproduktion – untersuchbar. Die Geräte, durch die Musik gehört wird, sind also weder schlichte Wiedergabetechnologien noch bloße elektrotechnische Artefakte. Vielmehr sind sie Gefüge aus Klanglichkeit, Körperlichkeit und Technologie. Diese werden in der Arbeit als Hörgeräte auf den Begriff gebracht. Die Hörgeräte der Rock- und Popmusik zielen – wie ihre medizintechnischen Verwandten – auf die Materialität der Wahrnehmung. Im Gegensatz zu diesen funktionieren sie jedoch nicht als Prothesen, die an einer gattungsweit postulierten Norm ausgerichtet sind. Statt Normen bergen sie Exzesse – an Serialität und Wiederholung – sowie Eskalationen – von Lautstärke und von hohen und tiefen Frequenzen. Die Arbeit ist in drei Kapiteln gegliedert. Im ersten Kapitel wird die These der Arbeit in Bezug auf theoretische Diskurse der Musik-, Kultur- und Medienwissenschaft verortet und eine begriffliche Systematik entwickelt. Kapitel zwei und drei sind Fallstudien gewidmet. In der ersten wird das Jukeboxhören der Rock’n’Roll-Kultur der 1950er Jahre untersucht, in der zweiten das Soundsystemhören der Disco- und Clubkultur der 1970er bis 1990er Jahre. Die im ersten Kapitel entwickelte begriffliche Systematik macht die Fallstudien vergleichbar.The devices by which music is listened to in the age of technological (re-)production have changed over and over again. These devices must be imperceptible to the ear. Only then can music be heard. Nonetheless – this is the claim of the thesis – these devices organize hearing and are agents of a cultivation and technization of perception. Based on two case studies from the history of rock and pop music, this thesis reveals how such devices constitute not only the listener but also the music which is listened to. Through listening technologies new listening practices emerge and the corporality of the listener is newly defined. The adaptation of sound to specific listening technologies can be analysed during the mastering process, the last step in technological music production. The devices by which music is listened to are thus neither simple technologies of reproduction nor mere electrotechnical artefacts. Rather, they are assemblages of sound, corporality, and technology. In this thesis these assemblages are called “Hörgeräte” (listening devices). The listening devices of rock and pop music target – like medical-technical “Hörgeräte” (hearing aids) – the materiality of perception. Contrary to medical technologies, however, listening devices do not function as prostheses, which are calibrated according to medical industry standards. Instead, they contain excesses – of seriality and repetition – and escalations – of amplitude and high and low frequencies. The thesis is arranged in three chapters. In the first chapter I situate the main argument of the thesis within discourses of musicology, media and cultural studies, and develop my own terminology. Chapters two and three deal with case studies. In chapter two I investigate jukebox listening in 1950s rock’n’roll culture, whilst in chapter three I explore sound system listening in disco and club culture from the 1970s to the 1990s. The terminology developed in chapter one enables a comparison of the case studies.

This study deals with a characteristic of the tenor aria “Des Vaters Stimme ließ sich hören” (BWV 7/4). Why does Bach use two imitating violins? So far unconsidered exegetical and hymnological sources can give a plausible answer to that question. Based on a detailed observation of these sources, the study aims to encourage us to think again about the various hermeneutical approaches to Bach’s sacred vocal music and how Bach can be located in the field of tension between individuality and ‘Zeitgeist’.

Negli ultimi anni numerose riflessioni hanno insistito sull’importanza dei parchi agricoli come strumenti in grado di contribuire alla ricreazione di rapporti di reciprocità tra urbano e rurale, giocati sulla valorizzazione della loro prossimità. I parchi possono inoltre costituire l’occasione per sperimentare nuove forme di progettualità alla scala territoriale, orientate verso la costruzione di scenari unitari e coerenti sul piano morfologico, funzionale, ambientale. Il volume offre una riflessione su questi temi restituendo gli esiti di un’esperienza di progettazione che ha coinvolto la scuola territorialista di Empoli (CdL in Pianificazione della Città e del Territorio) e quella dell’ingegneria territoriale di Clermont Ferrand (AgroParisTech ENGREF). Gli scenari che ne sono derivati operano una sintesi tra questi due approcci, integrando l’interpretazione patrimoniale con i processi di diagnostic territorial e con la costruzione di territorialità condivise.

Content-Based Image Retrieval (CBIR) technology has been proposed to benefit not only the management of increasingly large medical image collections, but also to aid clinical care, biomedical research, and education. Based on a literature review, we conclude that there is widespread enthusiasm for CBIR in the engineering research community, but the application of this technology to solve practical medical problems is a goal yet to be realized. Furthermore, we highlight “gaps” between desired CBIR system functionality and what has been achieved to date, present a comparative analysis of four state-of-the-art CBIR implementations using the gap approach for illustration, and suggest that high-priority gaps to be overcome lie in CBIR interfaces and functionality that better serve the clinical and biomedical research communities.

Evaluating the success of computer-aided decision support systems depends upon a reliable reference standard, a ground truth. The ideal gold standard is expected to result from the marking, labeling, and rating by domain experts of the image of interest. However experts often disagree, and this lack of agreement challenges the development and evaluation of image-based feature prediction of expert-defined “truth.” The following discussion addresses the success and limitation of developing computer-aided models to characterize suspicious pulmonary nodules based upon ratings provided by multiple expert radiologists. These prediction models attempt to bridge the semantic gap between images and medically-meaningful, descriptive opinions about visual characteristics of nodules. The resultant computer-aided diagnostic characterizations (CADc) are directly usable for indexing and retrieving in content-based medical image retrieval and supporting computer-aided diagnosis. The predictive performance of CADc models are directly related to the extent of agreement between radiologists; the models better predict radiologists’ opinions when radiologists agree more with each other about the characteristics of nodules.

In this chapter, we will discuss the rise of multi-modal imaging in clinical practice. We will summarize some of our pioneering CBIR achievements working with these data, exemplified by a specific application domain of PET-CT. We will also discuss the future challenges in this significantly important emerging area.

This chapter discusses key aspects concerning the performance of Content-based Image Retrieval (CBIR) systems. The so-called performance gap plays an important role regarding the acceptability of CBIR systems by the users. It provides a timely answer to the actual demand for computational support from CBIR systems that provide similarity queries processing. Focusing on the performance gap, this chapter explains and discusses the main problems currently under investigation: the use of many features to represent images, the lack of appropriate indexing structures to retrieve images and features, deficient query plans employed to execute similarity queries, and the poor quality of results obtained by the CBIR system. We discuss how to overcome these problems, introducing techniques such as how to employ feature selection techniques to beat the “dimensionality curse” and how to use proper access methods to support fast and effective indexing and retrieval of images, stressing the importance of using query optimization approaches.

Medical image content-based retrieval entails several possible scenarios. One scenario relates to retrieving based on image landmarks. In this scenario, quantitative image primitives are extracted from the image content, in an extensive pre-processing phase, following which these quantities serve as metadata in the archive, for any future search. A second scenario is one in which image-to-image matching is desired. In this scenario, the query input is an image or part of an image and the search is conducted by a comparison on the image level. In this paper we review both retrieval scenarios via example systems developed in recent years in our lab. An example for image landmark retrieval for cervix cancer research is described based on a joint collaboration with National Cancer Institute (NCI) and the National Library of Medicine (NLM) at NIH. The goal of the system is to facilitate training and research via a large archive of uterine cervix images

Digital management of medical images is becoming increasingly important as the number of images being created in medical settings everyday is growing rapidly. Content-based image retrieval or techniques based on the query-by-example paradigm have been studied extensively in computer vision. However, the global, low level visual features automatically extracted by these algorithms do not always correspond to high level concepts that a user has in his mind for searching. The role of image retrieval in diagnostic medicine can be quite complex, making it difficult for the user to express his/her information needs appropriately. Image retrieval in medicine needs to evolve from purely visual retrieval to a more holistic, case-based approach that incorporates various multimedia data sources. These include multiple images, free text, structured data, as well as external knowledge sources and ontologies.

Despite the numerous purported benefits of Electronic Medical Records (EMR), medical practices have been extremely reluctant to embrace the technology. One of the barriers believed to be responsible for the slow adoption of EMR technology is resistance by many physicians who are not convinced of the usefulness of EMR systems. This study used a mail survey of physicians associated with a multi-specialty clinic to examine potential characteristics of physicians that might help identify those individuals that are most likely to pose a threat to the successful EMR implementation. Age and gender of the physicians was generally not associated with anticipated use. However, an analysis of variance indicated self-rated computer knowledge and area of medical specialty were highly related to expected use of EMR functions. Results indicating that anticipated use of various EMR functions depend on medical specialty denotes one of the many difficulties of developing EMR systems for multi-specialty clinics.

Clinical Decision Support Systems (CDSS) are typically constructed from expert knowledge and are often reliant on inputs that are difficult to obtain and on tacit knowledge that only experienced clinicians possess. Research described in this article uses empirical results from a clinical trial of a CDSS with a decision model based on expert knowledge to show that there are differences in how clinician groups of the same specialty, but different level of expertise, elicit necessary CDSS input variables and use said variables in their clinical decisions. This article reports that novice clinicians have difficulty eliciting CDSS input variables that require physical examination, yet they still use these incorrectly elicited variables in making their clinical decisions. Implications for the design of CDSS are discussed.

Urgent requests and critical messages in healthcare applications must be delivered and handled timely instead of in an ad-hoc manner for most current systems. Therefore, we extend a sophisticated alert management system (AMS) to handle process and data integration in healthcare chain workflow management under urgency constraints. Alerts are associated with healthcare tasks to capture the parameters for their routing and urgency requirements in order to match them with the specialties of healthcare personnel or the functionalities of Web Services providers. Monitoring is essential to ensure the timeliness and availability of services as well as to ensure the identification of exceptions. We outline our implementation framework with Web Services for the communications among healthcare service providers together with mobile devices for medical professionals. We demonstrate the applicability of our approach with a prototype medical house-call system (MHCS) and evaluate our approach with medical professionals and various stakeholders.

This chapter seeks for deeper understanding of the user experience in mobile healthcare settings. It studies physicians’ mobile user experiences with evidence-based medical guidelines and drug information databases with the concept of flow as the research vehicle. The data was collected among all of the 352 users of a mobile medical application. The response rate was 66.5% (n=234). The results demonstrate that it is the orientation and navigation within the system, rather than usefulness and ease of use, in par with perceived challenges, focused attention and learning that lead to positive user experience. This supports the fact that finding relevant pieces of information is essential in the system utilization. The results also provide support for the claim that mobile applications are not only beneficial for patient safety, but they may also improve the computer and professional skills of the physicians. The frequent use of the system was noted to improve physicians’ computer skills, the feeling of being in control of the system, and their perception of the system’s ease of use. Moreover, our findings suggest that learning may play a greater role for knowledge work than often suggested.

In a prospective studie 129 patients with hip fracture surgery under LDH-prophylaxis (3x5000 U Na-heparinat) were examined in order to find an answer tothe question, if there is a correlation between inhibitor activity, parameters of fibrinolysis, plasma heparin activity and the incidence of DVT.100 patients with ascending phlebography on day 7 to9 post op. were taken into final consideration (blood collection on admission and on day 1, 2, 4, 7 after surgery):1)Incidence of DVT : 17 %.2)Inhibitors: No difference between patients with and without DVT with respect to AT III activity and prot. C concentration ( prot. C activity in progress).3)Fibrinolysis: Elevated levels of DD-fragment (x = 1 780 mg/ml), t-PA Inhibitor (x = 31 AU) and fibrinogen (417 rng/dl) before operation due to preceding trauma. No significant difference between t-PA, t-PA inhibitor and antiplasmin with respect to DVT. Whileplasminogen concent was significantly increased in patients with DVT on day 4 and 7, DD-fragments had lower values on day 7 (x = 1395 / x = 2140 ng/ml).4)Heparin effect: Plasma heparin activity was assesed by an amidolytic anti ll^assay. Although plasmaticheparin action only represents one aspect of thromboprophylactic heparin-activity, there is an obvious difference between patients with and without DVT withrespect to plasmatic heparin activity( p <0.005).

We have discovered a new cooling process that uses an applied magnetic field to mix differentially light shifted atomic ground state sublevels. In related experiments cooling is achieved by mixing caused by polarization gradients. Because the energy loss mechanism stems from the light shift and not the momentum transfer of a scattering event, the accessible temperature is much lower than the limit of Doppler cooling. We used this method for optical collimation of an 85Rb atomic beam to a transverse rms speed of 3 cm/s, well below the v = ( 7 ℏ γ / 20 M ) ≅ 10 cm / s of the Doppler limit for two-level atoms in this 1-D optical molasses. This low velocity, limited only by experimental geometry, is achieved only when a magnetic field of ~20 µT is applied perpendicular to the axis of the circularly polarized diode laser beam. We use a thermal beam of natural Rb produced by an oven at T ~ 150°C with an ~0.33-mm diam aperture and a defining aperture of ~0.33-mm diam about 24 cm away. The atoms are optically collimated by laser beams transverse to their motion that excite the transition at λ = 780 nm. The atomic beam profile is measured with a vertically oriented scanning hot tungsten wire, 25 µm in diameter, 1.3 m away.

Abstract XYZ Field in Central Sumatra is the largest waterflood operation in Southeast Asia. Water Injection Pump (WIP) is the critical equipment to enable this waterflood operation to maximize the oil production process. Based on WIP performance evaluation in Jan 2017 – Jun 2018, it was found that the average WIP reliability was 92.5% and average Loss Production Opportunity (LPO) was 268 BOPD. The contributing factors for this poor WIP performance were not only from the internal WIP unit but also from its external system. Integrated Water System Evaluation was introduced to improve WIP performance to its desired level and to minimize LPO impact. From the internal system, the evaluation was started by analyzing the failure mode for each 78 WIP's, finding root causes, and developing mitigation plans to prevent failure from re-occurring in the future. It was found that there were 8 WIPs in low-efficiency condition and 7 WIPs operating outside the Best Efficiency Point (BEP). The evaluation was continued to the external system which consists of water injection line network and water injection well (WIW). It was found that the WIP discharge pressure was increasing from 700 PSIG up to 780 PSIG in all XYZ field areas. The main root causes were unbalanced water distribution and several WIW's being off due to various reasons. Based on comprehensive analysis of the XYZ Field water system, it was recommended to perform the following actions to improve the WIP Performance: 1) Perform a proactive overhaul program for 8 WIPs with low-efficiency issues, 2) Adjust the Control Valve opening to ensure WIP operates within BEP, 3) Develop recommended discharge pressure setting range for each WIPs, 4) Re-POI (Put on Injection) inactive WIWs, 5) Perform fluid dispatching to balance water distribution in XYZ Field areas, such as transfer 100,000 Barrels Fluid Per Day (BFPD) from Area 4 to Area 6, transfer 55,000 BFPD from Area 5 to Area 4, and isolate interconnection between Area 1 and Area 3. After implementation, there are some improvements as follows: 1) The range of WIP within BEP was increased from 37% to 62%, 2) WIP Reliability was increased from 89% (Jan 2017) to 96% (Jul 2018), 3) LPO due to WIP failure was decreased from 268 BOPD to 42 BOPD, 4) Average WIP discharge pressure was decreased from 780 PSIG to 700 PSIG. To sustain these improvements, the WIP performance monitoring flow process and tools have been developed. In recent years, when Company established aggressive programs to increase oil production, we experienced a similar condition that impacted WIP performance. But by having the sustained flow process and tools, we can manage the situation well and prevent major LPO and reliability incidents.

An attractive candidate for performing atom guidance is the evanescent field at the border of a dielectric light guide since providing a short range (repulsive) potential. In contrast to passive hollow capillaries, we have realised a lasing one made up from highly doped phosphate glass. Compared to its passive counterpart, the laser capillary has the big advantage that the in-coupled light being spectrally removed from the atomic transition of the atoms to be manipulated is used for pumping the laser. The capillary had a diameter of 70 μm possessing a protection coating. The bore diameter was 11 pm. The doping concentration amounted to 2·1020 cm-3 Nd3+. The capillary length was 1.6 cm (7 cm), butt coupled mirrors form the resonator. The capillary could be illuminated side-on or end-on with pump radiation for the active ions delivered by a diode laser at λ=805 nm. The output mirror had a transmission of < 1% to realise a high intracavity power being desirable to create a strong optical potential necessary in evanescent waveguiding of atoms. The near field distribution is shown in Fig.1. To proof the reaction of the evanescent field on an absorptive disturbance we have filled the capillary with an ir-dye (concentration ~0.07 g/1) finding a strongly modulated (mode locked) output when pulsing the diode laser, Fig.2. (The transverse damping distance in the dye solution is only ~0.3 μm). For an empty capillary the emission gets stationary after some typical relaxation oscillations. To estimate the optical potential we have made a numerical analysis of the laser field distribution in the hollow waveguide. Fig.3 shows the mode field at λ=780 nm in the capillary. We have calculated the optical potential U(r) to guide 85Rb atoms with an intra fiber power of 2 W. The frequency detuning from the atomic resonance is 10 GHz, the saturation intensity of the atomic transition is 1.8 mW/cm The optical barrier as a function of distance from the inner surface in terms of temperature is shown in Fig.1c (inset). The realisation of active atomic waveguides may stimulate interesting studies in atom optics, near field optics and cavity QED.

ЦЕЛЬ: оценить состояние углеводного обмена у долгожителей Москвы и близлежащих регионов.
МАТЕРИАЛЫ И МЕТОДЫ: в это когортное исследование включались лица в возрасте 90 лет и старше, давшие письменное согласие на участие. В домашних условиях проводилась комплексная гериатрическая оценка, включавшая сбор анамнеза, применение стандартных гериатрических шкал и опросников, выполнение антропометрических измерений. Всем участникам оценивались следующие лабораторные показатели: глюкоза, инсулин, индекс инсулинорезистентности HOMA-IR, гликированный гемоглобин (HbA1c). Интерпретация состояния углеводного обмена проводилась согласно актуальным на 2022 г. клиническим рекомендациям. Статистический анализ данных проводился с помощью языка программирования R версии 4.1.3. Количественные переменные представлены в виде Me (Q1-Q3). Для сравнения непараметрических значений применялся критерий Краскела-Уоллиса, номинальных переменных – χ2 Пирсона или точный критерий Фишера. Исследование было одобрено локальным этическим комитетом.
РЕЗУЛЬТАТЫ: за период с 2019 по 2022 г. в исследование было включено 3 811 человек (75,4% женщины) в возрасте 92 (91–94) года. Средний индекс массы тела составил 25,4 (23-28,2) кг/м2, доля участников с ожирением составила 15,2%, при этом у 72,9% пациентов были значения окружности талии соответствовали абдоминальному ожирению. Средние значения глюкозы составили 5 (4,5 – 5,7) ммоль/л, HbA1c 5,6 (5,4 – 5,9)%, HOMA-IR – 1,5 (0,9 – 2,6).
В группу СД были отнесены 20,5% (780); предиабета – 16,1% (613). Все случаи СД были интерпретированы как 2 тип. В группе с СД большая часть пациентов 71,7% имела диагноз на основании данных анамнеза; у 28,3% пациентов были впервые выявлены значения параметров углеводного обмена в диабетическом диапазоне. С клинической точки зрения, значения показателей углеводного обмена в группе пациентов с СД были близки к значениям в группе предиабета: глюкоза - 6,3 (5,1 - 7,9) и 5,9 (5 - 6,4) ммоль/л, HbA1c 6,2 (5,6 - 7) и 6,1 (6 - 6,2)%, HOMA-IR 2,3 (1,3 - 6,1) и 2,2 (1,4 - 4,5), соответственно. При этом лишь 4,8% (38) участников с СД получали сахароснижающую терапию (ССТ): 3,8% (30) монтерапию инсулином, 0,6% (5) – терапию пероральными сахароснижающими препаратами, 0,4% (3) – комбинированную терапию. Несмотря на то, что в группе с впервые выявленным СД значения показателей углеводного обмена были статистически значимо выше, чем в группе с ранее выявленным СД (глюкоза 7,4 и 5,9 ммоль/л; HbA1c 6,8 и 6,1%, соответственно), они всё равно были ниже предполагаемых целевых значений для этой возрастной категории. Дополнительно пациенты были разделены на возрастные подгруппы: 90-94 лет (3091), 95-99 лет (674), >100 лет (46); достоверных различий в значениях показателей глюкозы и HbA1c, частоте СД и предиабета между группами получено не было.
ВЫВОДЫ: доля пациентов с СД в исследованной когорте долгожителей была практически сопоставима с данными о распространённости СД в группе пожилых людей, при этом показатели углеводного обмена указывали на компенсацию углеводного обмена даже при низкой доле пациентов, получающих ССТ.

Introdução: A gravidez ectópica, em muitos países, é a principal causa de mortalidade materna no primeiro trimestre de gestação. Sua incidência varia de 1 a 2% de todas as gestações e pode ocorrer em diferentes locais, como as tubas uterinas, a região cornual, os ovários, a cicatriz de cesariana ou a cavidade abdominal. Neste relato de caso, apresentamos uma ocorrência rara de gravidez ectópica de origem ovariana, mimetizando uma mola hidatiforme parcial (MHP). Relato de caso: J.C.S., uma paciente de 24 anos, gravidez III, parto II, foi encaminhada em março de 2022 para um centro de referência em doença trofoblástica gestacional (DTG) em um hospital quaternário. Ela vinha de São Pedro da Aldeia com suspeita de MHP. A paciente trouxe um exame de ultrassonografia externa que identificou uma gestação tópica com feto único, vivo, apresentando adramnia (ausência de líquido amniótico), peso fetal estimado de 780 g e uma massa com septos finos no útero, com um padrão semelhante a flocos de neve, sugerindo mola hidatiforme. A paciente relatava dor abdominal e durante o exame físico foi observado que o fundo do útero estava na altura da cicatriz umbilical, com uma massa endurecida que se estendia até o hipocôndrio esquerdo. Partes fetais eram palpáveis e os batimentos cardíacos fetais eram audíveis ao sonar. A paciente foi submetida a um novo exame de ultrassonografia no hospital quaternário, que identificou uma gestação abdominal (GA) com feto único, vivo, peso fetal estimado de 665 g e biometria correspondente a 25/26 semanas. A placenta foi identificada em meio às alças intestinais e havia oligodramnia (diminuição do líquido amniótico). Uma ressonância magnética confirmou o diagnóstico de GA, com a placenta aparentemente implantada no mesentério em íntimo contato com o fundo do útero. Após discussão entre a equipe médica e a paciente, foi realizada corticoterapia para a maturação pulmonar fetal. Após 48 horas, a paciente foi submetida a uma laparotomia exploratória, durante a qual o feto vivo, em adramnia, foi extraído e precisou ser reanimado ainda na sala de parto. O procedimento foi complicado devido à placenta implantada no fundo do útero e no ovário direito, além de múltiplas aderências no omento, peritônio, cólon direito e tubas uterinas. Foi realizada a lise das aderências, ligadura bilateral das tubas uterinas e apendicectomia tática. O exame histopatológico da placenta revelou que a gestação era de origem ovariana. A paciente teve uma boa evolução pós-operatória e recebeu alta 48 horas após o procedimento. Infelizmente, o recém-nascido faleceu aos 7 dias de vida devido a complicações de prematuridade extrema. Comentários: O diagnóstico ultrassonográfico de MHP pode ser difícil, especialmente fora dos centros de referência. Especialmente nos casos em que o feto é vivo e viável, é necessário ter cautela em relação às condutas ativas, e uma equipe médica experiente deve revisar o caso. No caso apresentado, a paciente tinha uma condição ainda mais rara do que a MHP, sendo a gravidez abdominal responsável por apenas 1% de todos os casos de gravidez ectópica. A abordagem desses casos deve ser individualizada, com a tomada de decisão compartilhada com a paciente.

Com a crescente disseminação do Building Information Modelling (BIM) no Brasil, a academia passou a discutir e aprimorar suas grades visando a inserção da metodologia nos cursos de Arquitetura e Engenharia. A graduação em Engenharia Civil da Universidade Federal do Rio de Janeiro (UFRJ), campus Macaé, capacita profissionais para atuar nos mais diversos setores desta área de conhecimento, apresentando um caráter generalista desde sua concepção. Entretanto, o curso conta somente com uma disciplina eletiva com enfoque no paradigma BIM, intitulada “Introdução ao BIM”. Portanto, o objetivo desta pesquisa é apresentar uma avaliação da grade curricular do curso e o potencial alinhamento com a metodologia BIM. O trabalho é fruto da pesquisa de conclusão de curso para obtenção do título de Engenheiro Civil. A análise geral do Projeto Pedagógico do Curso (PPC) foi realizada mediante a aplicação do método desenvolvido pela professora Checcucci (2014). Esta divide-se entre as matérias do Núcleo Básico (em que as disciplinas são lecionadas em conjunto com a Engenharia de Produção e Engenharia Mecânica), Núcleo Profissionalizante e Núcleo Específico. Este último apresenta uma segmentação em cinco eixos temáticos, são eles: Sistemas Estruturais; Sistemas Hidrológicos e Saneamento; Sistemas Geomecânicos; Sistemas de Construção Civil; e Sistemas de Transportes e Logística. De modo a facilitar a leitura da grade e torná-la mais harmônica, a mesma foi reformulada e representada no programa Microsoft Excel. Posteriormente, foi realizado um reagrupamento das disciplinas por cores, utilizando os quadros referentes às cargas horárias e código de 8 cores que separa o núcleo comum e os eixos temáticos que compõem o núcleo profissionalizante. O próximo passo foi a identificação quantitativa e qualitativa das disciplinas que efetivamente apresentam interação com o BIM. Inicialmente, foram removidos todos os componentes curriculares que não possuem qualquer interface com o BIM, sendo mantidos somente aqueles que apresentam potencial para trabalhar algum de seus conceitos, a depender do enfoque conferido pelo professor, ou que possui interface clara com o BIM. Por fim, com as análises efetuadas e as disciplinas classificadas, foi possível gerar um quantitativo de todas as componentes curriculares que possuem conexões com o BIM. Observou-se que de 27 matérias do Núcleo Básico, 13 têm possíveis interações com o BIM, e dessas 13 disciplinas, 4 apresentam interações claras. Já no eixo temático de Sistemas Estruturais todas as 10 disciplinas possuem vínculos com a metodologia, e somente 4 possuem conexões claras. Em Sistemas Hidrológicos e Saneamento, de 5 disciplinas somente 1 exibe possíveis relações com o BIM. O eixo temático de Sistemas Geomecânicos, conta com 5 disciplinas, dentre as quais 2 expõem possíveis elos com o BIM. Das 8 disciplinas de Sistemas de Construção Civil, 7 manifestam possíveis associações e 6 demonstram ligações claras com o BIM. No eixo temático de Sistemas de Transportes e Logística, nenhuma das 3 componentes curriculares apresentam relação com a metodologia. Por fim, a disciplina do TCC não possui diretrizes e ementas específicas, permitindo que todos os aspectos relativos ao BIM abordados, cabendo ao discente e ao seu orientador definirem qual linha de pesquisa pretendem adotar. Podemos concluir, portanto, que é viável inserir a metodologia BIM já no primeiro período do núcleo de conteúdos básicos, trabalhando conceitos que envolvem colaboração, programação e informática que intrínsecos à utilização dos softwares, além da introdução à modelagem BIM. As 4 disciplinas, a saber: “Introdução à Engenharia”, “Sistemas Projetivos”, “Engenharia e Sustentabilidade” e “Desenho Computacional”, revelam que é possível debater assuntos que permeiam o BIM de forma clara já nos períodos iniciais do curso. É importante ressaltar a disciplina “Introdução à Engenharia”, como a única da grade em que é possível transitar por todos os conceitos que regem o paradigma BIM. Entretanto, o fato de ser uma disciplina com baixa carga horária e presente no 1º período de integração curricular, pode dificultar a absorção por parte dos alunos sobre todas as vantagens que o BIM pode proporcionar. Além disso, se trata de uma disciplina que é lecionada em conjunto com todos os alunos que adentram o curso de Engenharia da UFRJ Macaé (Civil, Mecânica e Produção). Conclui-se, também, que mais da metade das disciplinas que compõem o núcleo profissionalizante podem trabalhar os aspectos do BIM, permeando sobre diversos conceitos, etapas e projetos. Vale destacar que a única disciplina na qual foi encontrada relação com a etapa de demolição e requalificação da edificação, foi “Engenharia e Sustentabilidade”, mostrando que esse é um aspecto pouco abordado na grade curricular. A etapa do uso (operação e manutenção) pode ser abordada na disciplina “Edificações II”, ao introduzir o tema de patologia da construção, e também na disciplina de “Engenharia Portuária e Costeira”, sob o enfoque da operação de portos. Todas as disciplinas do Eixo de Sistemas Estruturais possuem ao menos uma interface com o BIM, nas quais é possível trabalhar todos os seus conceitos, podendo passar pelas etapas de projetação, planejamento e construção. Vale ressaltar que este é o eixo do ciclo de profissionalização com a maior carga horária da grade. O Eixo dos Sistemas de Construção Civil é o que possui mais disciplinas e maior carga horária com interface clara com o paradigma BIM. Todos os conceitos estão contemplados nessas disciplinas, salientando que o BIM pode oferecer uma contribuição importante ao desenvolvimento de profissionais com ampla visão sobre os processos que envolvem sua atuação. A disciplina “Desenho Técnico Aplicado à Engenharia Civil” deve ser a responsável por discutir e apresentar as funcionalidades de um software BIM específico, proporcionando ao aluno o desenvolvimento dos diversos tipos de projetos. Sendo assim, é possível trabalhar todas as disciplinas de projeto BIM nas disciplinas já existentes na grade. Por fim, em termos quantitativos, a carga horária de disciplinas com interface clara com o BIM (780 horas) representam 23% da carga horária total de disciplinas obrigatórias (3390 horas), e 30,6% (630 horas) dos Ciclos Profissionalizante e Específico (2055 horas). O BIM exige a atualização dos profissionais da construção civil, por meio de ferramentas que auxiliem no processo de modelagem, análise, simulação e cooperação em um trabalho multidisciplinar. Diante dessa necessidade, o Governo Federal criou a Estratégia Nacional de Disseminação do BIM em 2017 com o intuito de estabelecer as métricas necessárias para atingir os objetivos. Essa iniciativa incluiu as universidades, consideradas como pontos de partida para a difusão de tal conhecimento. A rede de células BIM toma norteamento nas diretrizes do governo com o intuito de disseminar o conceito entre os cursos de Arquitetura, Engenharia Civil e afins. O presente trabalho faz parte da rede de células BIM e tem como objetivo a realização do diagnóstico da matriz curricular do curso de graduação em Engenharia Civil da UFRJ campus Macaé. Adotou-se o método de Checcucci (2014), que consiste na análise das ementas das disciplinas, mapeamento e categorização dos componentes curriculares, a fim de evidenciar as interfaces com o BIM. Os resultados são expostos em caixas individuais em cores e tons para cada núcleo de conteúdo e o item da categoria. Para tornar a análise efetiva e organizada, utilizam-se oito categorias BIM que podem ser abordadas ou não na disciplina (figura 1). A partir de cada ementa, é possível identificar a relação com o BIM, observando se há alguma interface, se depende do foco dado pelo professor ou se a interface é clara. Após a análise, a grade curricular é remodelada com gradações de cores, sendo possível fazer algumas constatações e observações acerca de lacunas identificadas e processos de ensino que podem ser alterados ou melhorados.
A graduação em Engenharia Civil da UFRJ Macaé possui 10 períodos divididos em: Ciclo Básico, composto por disciplinas fundamentais na formação inicial do Engenheiro, com oito eixos temáticos que totalizam 1470 horas; e Ciclo Profissional, composto por disciplinas e atividades complementares, totalizando 2055 horas (UFRJ-Macaé, 2017) (figura 2). A grade foi inicialmente reformulada e representada no programa Microsoft Excel utilizando a matriz disponibilizada pelo método. Foi adotado o quadro da figura 1 para cada componente curricular (disciplinas e atividades), o qual é subdividido em oito categorias nomeadas com as letras A até H. Em seguida, para cada quadro adotaram-se as cores representadas na figura 3 de acordo com o núcleo ao qual pertence. Essas cores têm três variações de tons de acordo com a interface com os conceitos do BIM. Nesta etapa da aplicação do método, foi adotada apenas a categoria A, que considera a interface do BIM com o componente curricular, este analisado de forma geral: sem interface, possível interface ou clara interface, de acordo com o conteúdo de cada ementa. Dessa forma, o quadro do componente foi pintado por completo na respectiva cor, seguindo a paleta da figura 3. O resultado foi a matriz da figura 4. Após a análise da grade curricular, foram realizadas manipulações com a intenção de identificar as principais disciplinas que têm interação com o BIM. Inicialmente, foram retirados todos os componentes curriculares que não possuem interface com o paradigma BIM, permanecendo somente aqueles que têm possível interface ou interface clara (figura 6). Para obter uma representação mais limpa, os números e as linhas que delimitam as caixas foram ocultadas, permanecendo apenas com a gradação de cores. A matriz da figura 5 mostra o resultado desse detalhamento analítico, evidenciando a filtragem mais apurada da grade curricular. Em seguida, foram mantidas somente as disciplinas nas quais se encontrou uma interface clara com o paradigma BIM, resultando na matriz da figura 7, que aprofunda ainda mais o detalhamento analítico. Com as análises efetuadas e as grades pintadas, foi possível gerar o Quadro da figura 8, que mostra quantitativamente em colunas, as disciplinas em que o paradigma pode ser trabalhado: a primeira mostra a matriz original do curso; a segunda, quais as disciplinas que têm possível interface com o BIM; e a terceira, que mostra somente as disciplinas que possuem interface clara com o paradigma BIM.
Constatou-se que é possível trabalhar todas as etapas do BIM nas disciplinas já existentes na grade, começando com a inserção de forma clara em três disciplinas do primeiro período do Ciclo Básico. Mais da metade das disciplinas que compõem o Núcleo de Conteúdos Profissionalizantes podem permear os conceitos, etapas e projetos BIM. Todas as disciplinas do Eixo Sistemas Estruturais possuem ao menos uma interface com o BIM com potencial para todos os conceitos e etapas. Assim como o Eixo Sistemas de Construção Civil possui a maior quantidade de disciplinas e carga horária com interface clara com BIM. Os processos iniciais de modelagem devem ser introduzidos na disciplina “Desenho Técnico Aplicado à Engenharia Civil”, pois é a cadeira que ambienta os alunos na área de expressão gráfica e projetos técnicos. 23% (780 horas) da CH total de disciplinas obrigatórias (3390 horas) têm interface clara com BIM e 30,6% (630 horas) dos Ciclos Profissionalizante e Específico (2055 horas). A fim de gerar melhores resultados para definir a forma que o BIM deve ser abordado na grade curricular de Engenharia Civil da UFRJ Macaé, devem ser realizados estudos direcionados aos professores de cada Eixo Temático, entendendo suas demandas, dificuldades e possíveis oportunidades ao adotar o BIM. Trabalhos acadêmicos com enfoque no BIM também podem ser realizados por alunos, a fim de salientar a importância de sua adoção e fomentar incentivos públicos e privados em novos laboratórios de informática e infraestrutura. É importante comentar que um currículo balizado somente em ferramentas BIM tende a se tornar ultrapassado devido ao rápido avanço tecnológico. Porém, seus conceitos e, principalmente, a visão sistêmica de todos os processos e relações que podem acontecer dentro de uma construção, propiciam a formação de profissionais capazes de formular soluções de maneira mais rápida e eficaz. Desta forma, é possível concluir que a adoção do BIM, mesmo que de forma superficial, é fundamental na formação do Engenheiro Civil formado na UFRJ Macaé.

Review question / Objective: The aim of this systematic review is to synthesize the scientific evidence that evaluates fatigue and fracture resistance, survival, and stress distribution, of composite resin CAD/CAM and ceramic CAD/CAM occlusal veneers in posterior teeth with severe occlusal wear. Condition being studied: Currently there is an increase in cases of dental wear, due to several factors such as: excessive consumption of carbonated drinks, a diet high in acids, gastric diseases, anorexia, bulimia, dental grinding, use of highly abrasive toothpastes, or a combination of these(9) (10) (11) (12); which affect the patient in several aspects: loss of vertical dimension, sensitivity due to the exposure of dentin, esthetics, affectation of the neuromuscular system(11) (13) (14). With the advent of minimally invasive dentistry, occlusal veneers have been found to be a valid option to rehabilitate this type of cases and thus avoid greater wear of the dental structure with full coverage restorations. Sometimes when performing a tabletop it is not necessary to perform any preparation, thus preserving the maximum amount of dental tissue(3) (6) (15). Due to the masticatory load either in patients without parafunction where the maximum masticatory force is approximately 424 N for women and 630 N for men or in those who present parafunction where the maximum bite force can vary from 780 to 1120N(7), it is necessary that the occlusal veneers support that load which makes indispensable a compilation of studies investigating both fatigue and fracture resistance and the survival rate of occlusal veneers in different materials and thicknesses.
