Cobb syndrome is an exceptional, non-inherited, genetic disorder characteristically constituted by vascular anomalies and neurological deficits. Spinal arteriovenous malformations appear in concurrence with cutaneous vascular lesions within the corresponding dermatome. Dermatome specific port wine stain upon the trunk, arteriovenous malformation, angioma, angiokeratoma, angiolipoma, cavernous haemangioma or lymphatic malformation is discerned in accompaniment with hyperreflexia, limb paresis, muscular cramps, sensory loss, bladder and bowel dysfunction, sudden paraplegia or subarachnoid haemorrhage. Spinal vascular lesions of Cobb syndrome can be adequately determined with magnetic resonance imaging (MRI), computerized tomography (CT) scan, plain radiography or angiography. Cobb syndrome can be appropriately managed with sclerotherapy, endovascular embolization, oral corticosteroids or surgical extermination of vascular lesions.

The main physiologic function of adipokines on the energy metabolism and the cardiovascular homeostasis, among others, are well known. In the last years, our laboratory is providing increasing evidence about the possible role of adipokines as mediators of vascular damage, by inducing different pro-inflammatory and pro-senescent mechanisms. The secretion of adipokines like visfatin or dipeptidylpeptidase-4 (DPP4) by adipose tissue, particularly by the visceral fat, is enhanced in obese and type 2 diabetic patients. Both adipokines are enzymes metabolically active that can induce the activation of specific receptors, namely type 4 “toll like receptors” (TLR4) for visfatin and “protease activated receptor 2” (PAR2) for DPP4. Stimulation of such receptors is triggering well known pro-inflammatory and pro-senescent mechanisms, like the nuclear transcription factor-κB (NF-κB) or the inflammasome NLRP3 (“nucleotide-binding, leucine-rich-repeat, pyrin domain containing 3”), a cell structure that transforms the immature forms of classic cytokines in their active derivates, interleukin-1β (IL-1β) or interleukin 18 (IL-18). It is worth to note that these classic cytokines are the final effectors for the harmful effects of the adipokines and its blockade can be a very relevant therapeutic approach. In our experiments, the specific antagonism of IL-1 receptors with anakinra prevents the inflammatory and senescent effects evoked by visfatin and DPP4. Moreover, this finding is in agreement with data from other researchers, as well as with the results of the CANTOS clinical trial, which demonstrate a very important cardioprotective cardiovascular effect mediated by the anti-inflammatory effect of the monoclonal antibody canakinumab. On the other hand, we have also provided experimental evidence about possible vasculopotective adipokines, such angiotensin-(1-7), which is able to induce, through activation of Mas receptors, the expression of klotho protein and the activation of antioxidant pathways, like the well known Nrf2-HO-1 (“nuclear factor erythroid-2 y hemoygenase-1”).

Arterial stiffness provides an indirect picture of vascular calcifications in terms of the effect they have on the systemic circulation and consequently on the heart. The present study sought to demonstrate the usefulness of outpatient investigation of vascular calcifications in dialysis patients using pulse wave velocity (PWV) determination. The aim of the study was to determine arterial resistance in patients with chronic dialysis kidney disease. The average study period was 48.5 ± 19.0 (SD) months. The study included 203 patients on dialysis with BCRT. The determination of arterial stiffness was measured at an interval of one hour after the dialysis session. The study highlighted the presence of an increased cardiovascular risk related to the presence of vascular calcifications without being able to establish a direct linear and causal link between their presence, location and the risk of cardiovascular events. The importance of highlighting calcifications and the consequences they generate means the possibility of influencing the main elements that can generate acute cardiovascular events. The results obtained and the correlations performed give an overview of the pathology studied, providing premises for therapeutic influence and risk assessment.

Vascular cognitive impairment is a term encompassing vascular dementia as well as milder forms of pre-dementia cognitive impairment related to vascular damage that do not meet the criteria for a diagnosis of dementia. Early diagnosis of cerebrovascular disease, including silent infarcts and small vessel disease is difficult, but important, as modification of cardiovascular risk factors is the mainstay of management to decrease further insults and deterioration, and maintain the patient’s independence within their family and social unit.

Abstract Cardiovascular disorders include various conditions characterized by morphological and functional defects of the heart and vascular system. Molecular biology techniques (in particular DNA sequencing) have recently offered new insights into the etiology of cardiovascular defects, revealing their association with germline as well as somatic mutations. Genetic tests are evaluated on the basis of their analytical and clinical validity, clinical utility, and ethical, legal and social implications. Next generation sequencing is so far the best approach for molecular diagnosis of congenital heart defects and vascular anomalies, the genetic and phenotypic heterogeneity of which makes them difficult to diagnose. Understanding the molecular causes of congenital heart defects and vascular anomalies has permitted clinical trials of drugs targeting affected genes and pathways. The articles in this Special Issue aim to provide guidance for those concerned with diagnosis and research in the field of cardiovascular defects. The approach to genetic testing is discussed.

Retina of rats OXYS 6 months of age by treatment of 4-methyl-2,6-diisobornylphenol (dibornol) was researched. Mainly, rats OXYS have of vascular disturbances such as reduction open functional vasculars of choroidea, destruction of retinal pigmental epithelium and retinal neurons. Dibornol was protected of retina increased area of open vascular and safety of retinal neurons. Besides dibornol was prevented thrombosis in retinal vasculars.

A male Polwarth lamb with a good body score was born with a pendulous, oval, red mass adhering to the skin in the left ventrolateral thoracic area. The nodule was removed when the lamb was five days old and sent for a gross and histopathological evaluation. Macroscopically, the mass was reddish, slightly oval and flat, well delineated, and contained a central ulcerated area. Microscopically, it showed a marked proliferation of well-differentiated endothelial cells with the formation of new blood vessels. An immunohistochemical analysis showed mild cytoplasmic immunostaining for the von Willebrand Factor in the endothelial cells. To the best of our knowledge, this is the first report of a cutaneous vascular hamartoma in a sheep.

Gastric antral vascular ectasia (GAVE) syndrome is an uncommon cause of chronic gastrointestinal bleeding as well as iron deficiency anaemia. We present two cases of GAVE, one with pernicious anaemia associated and the other is portal hypertension related. In both cases, progressive mucosal changes, which results in formation of GAVE, were documented. The changes were progression of multiple antral erythematous spots into linear configuration and finally to watermelon stomach.

A 47-year-old man presented with intussusception of the colon and was found to have a large tubulo-villous adenoma with high grade dysplasia, transforming into adenocarcinoma. The bowel adjacent to the polyp showed florid vascualr proliferation, mimicking a vascular neoplasm. However, there were no signs of atypia or malignancy in the vascular lesion. We illustrate an example of chronic obstruction inducing hypoxia, leading to neo-angiogenesis, histologically masquerading as a vascular neoplasm.

Introdução: A cardioembolia por fibrilhação auricular assume particular destaque etiológico nas doenças vasculares cerebrais devido à sua crescente incidência, elevado risco embólico e particularidades dos eventos clínicos causados. São objectivos deste trabalho analisar a frequência da fibrilação auricular numa população de doentes com acidente vascular cerebral isquémico observados num hospital nacional, estudar o impacto vital e funcional dos acidentes vasculares cerebrais causados por diferentes etiologias, e avaliar as opções antitrombóticas prévias e posteriores ao acidente vascular cerebral. Metodologia: Realizámos um estudo observacional retrospectivo incluindo todos os doentes internados num hospital central por acidente vascular cerebral isquémico em 2010 (pelo menos um ano de seguimento). A etiologia do acidente vascular cerebral foi definida pela classificação Trial of ORG 10172 in Acute Stroke (TOAST) modificada e o resultado funcional pela escala Rankin modificada. Realizámos análise descritiva das diferentes etiologias de acidente vascular cerebral e das prescrições antitrombóticas a doentes com fibrilhação auricular. Realizámos ainda um estudo de coorte para estudar o impacto clínico das opções antitrombóticas em prevenção secundária após acidente vascular cerebral cardioembólico. Resultados: Na nossa população (n = 631) encontramos frequência de cardioembolia (34,5%) superior à relatada na literatura. Os valores de mortalidade e morbilidade além das opções terapêuticas antitrombóticas em pré e pós-Doença Vascular Cerebral são semelhantes aos de outras séries, confirmando a gravidade dos acidentes vasculares cerebrais cardioembólicos e a subutilização dos antagonistas da vitamina K. A anticoagulação oral foi eficaz em prevenção secundária independentemente do estado funcional sequelar após acidente vascular cerebral. Conclusões: Apesar das recomendações terapêuticas inequívocas a anticoagulação oral continua a ser subutilizada em prevenção de Doença Vascular Cerebral. Confirmamos a eficácia clínica dos antagonistas da vitamina K em prevenção secundária, independentemente das limitações funcionais sequelares.

Anomalias vasculares constituem distúrbios de células endoteliais que podem afetar capilares, artérias, veias e/ou sistema linfático. São classificadas como “tumores vasculares” ou “malformações vasculares”. As malformações vasculares são subdivididas em malformações capilares, linfáticas, venosas, arteriovenosas e mistas de acordo com o tipo de vasos afetados. O objetivo do presente estudo é relatar um caso de um paciente com malformação vascular mista (veno-linfática) e descrever o planejamento e o tratamento dentário realizado. O paciente do sexo masculino, 7 anos, foi encaminhado para avaliação odontológica. A mãe relatou que o ele estava em tratamento médico no hospital por apresentar “hemangioma e malformação vascular linfática” em região bucal. Durante o exame clínico, foi observada malformação vascular veno-linfática no assoalho oral. A criança relatou dificuldade para realizar a higiene bucal devido à lesão, sendo encontradas várias lesões de cárie. O tratamento odontológico consistiu em restaurações dentárias, selantes e exodontia de dentes decíduos. O tratamento da malformação vascular mista foi realizado com solução esclerosante (OK-432). A atenção à saúde bucal de pacientes com malformações vasculares deve incluir orientações de higiene bucal, escovação supervisionada e indicação do uso de escovas macias para evitar lesões nas áreas já afetadas.

AbstractIntroduction:A vascular ring refers to encirclement of the trachea and oesophagus by an abnormal combination of derivatives of the aortic arch system. These malformations can cause variable degrees of compression of the oesophagus, trachea or both. Symptoms can range from no effect to severe stridor, dyspnoea and/or dysphagia.Method and results:This study presents a case series of six patients treated over a six-year period (2003–2009), illustrating the features of four different types of vascular ring; these types are discussed in detail. The clinical presentation, radiology, and microlaryngoscopy and bronchoscopy findings are also discussed.Conclusion:The management of children with vascular rings requires a high index of clinical suspicion to ensure prompt diagnosis. As many of these children present with airway symptoms, the paediatric otolaryngologist plays a key role in identifying and assessing their anatomical anomalies.
