Perkebunan Nusantara IV adalah salah satu peruhsaan perkebunan yang dalam kegiatan usahanya banyak kasus tindak pidana pencurian dan penadahan kelapa sawit. Untuk itu tujuan penelitian ini dilakukan untuk mengetahui dan menganalisis tentang penerapan Pasal 107 Huruf D Undang-Undang Nomor 39 Tahun 2014 Tentang Perkebunan Untuk Meminimalisir Tindak Pidana Pencurian Yang Terjadi Di Kebun Milik PT. Perkebunan Nusantara IV. Hasil penelitian menunjukkan bahwa Penerapan Pasal 107 Huruf D Undang-Undang Nomor 39 Tahun 2014 Tentang Perkebunan di Kebun Milik PT. Perkebunan Nusantara IV yaitu di PTPN IV Unit Kebun Gunung Bayu dan PTPN IV Unit Kebun Mayang dapat meminimalisir terjadinya pencurian. Introduction: Patients needing surgical treatment for benign conditions are listed on first come first served basis in the department of ENT- Head and Neck Surgery (ENT-HNS), Tribhuvan University Teaching Hospital, Kathmandu, Nepal. The objective of this audit was to determine the average waiting period for elective surgery in various subspecialty units of the department and the patient turn up rate on the scheduled date. Methods: This was a retrospective audit based on the elective surgical waiting list register of year 2073 B.S. of four subspecialties in the department; namely unit I-Otology, Unit II- Head & Neck surgery, Unit III - Pediatric ENT and Unit IV- Rhinology Results: The overall average waiting period for elective surgery was 10.6 weeks. For GA cases it was 10.4 weeks for unit I and II, 13.2 and 8.3 weeks for unit III and IV respectively. For LA cases, waiting period in unit I and IV was 16.9 and 2.5 weeks respectively. The overall turn up rate on the scheduled contact date was 40.8% (46% for GA cases and 31% for LA cases). For unit I, it was 50% for GA cases and 26.6% for LA cases. For unit II and unit III GA cases, turn up rate was 38.4% and 43.8% respectively. In unit IV, it was 53.3% for GA cases and 66% for LA cases. Conclusion: The patient turn up rate on contact date in general decreased with longer waiting period for elective surgery.

This paper proposes a monitoring system retro-fittable for existing Intravenous (IV) infusion setup. Traditionally, doctors and nurses use their experience to estimate the time required by an IV bottle to empty which makes the IV therapy vulnerable to human error. The current study proposes an internet connected monitoring platform for IV drip chambers. The device enables doctors and nursing staff to monitor the drip parameters wirelessly while emphasizing on low costs and high degree of reliability. It has two main units, namely chamber unit and pole unit. Chamber unit houses two types of sensors, optical based for drop detection and capacitive based for level detection, both of which are placed on the chamber unit. The pole unit majorly consists of a microcontroller and a GSM-based (Global System Mobile Communication) communication module. In addition, the device was tested along with various parameters like accuracy, readout stability, change in fluid used, changes in ambient conditions, end chamber conditions, optical unit malfunctions. Finally, the monitored data was securely and reliably transmitted to commercial cloud service using HTTP API calls (Hyper Text Transfer Protocol) (Application Programming Interface). This data was stored and visualized for ease of readability for nurses and doctors.

Background: Di-2-ethylhexyl phthalate (DEHP), a phthalate compound found in medical devices, may cause toxic effects in premature infants. In this study, the objective is to quantify DEHP exposures from various intravenous and respiratory therapy devices, and to use these values to predict typical exposure for an infant in a neonatal unit. Methods: Common IV products used on infants are directed through various types of IV tubing (IVT) and analyzed for DEHP content. DEHP exposure for infants receiving respiratory therapy was determined indirectly through analysis of urine DEHP metabolites. By deriving these values for DEHP we calculated the daily exposure to DEHP from common IV fluids (IVF) and respiratory devices during hospitalization in a neonatal unit. Results: IVF labeled DEHP-positive showed very high concentrations of DEHP, but when passed through IVT, substantial amounts were adsorbed. DEHP was undetectable with all DEHP-negative IVF tests, except when passed through DEHP-positive IVT. The DEHP leached from most respiratory devices was relatively modest, except that detected from bubble CPAP. In 14 very low birthweight infants, the mean DEHP exposure was 182,369 mcg/kg over 81.2 days of the initial hospitalization. Ninety-eight percent of the exposure was from respiratory devices, with bubble CPAP accounting for 95% of the total DEHP exposure in these infants. Conclusions: The DEHP exposure in our neonatal unit can be reduced markedly by avoiding or modifying bubble CPAP equipment and avoiding IV tubing containing DEHP.

ABSTRAK PTPN IV Unit PKS Pabatu merupakan Pabrik minyak nabati yang gedungnya termasuk dalam kategori mudah terbakar. Kegiatan produksinya juga memiliki potensi kebakaran akibat penggunaan peralatan yang dapat menghasilkan suhu yang tinggi. Perlu adanya analisis penerapan sistem proteksi kebakaran aktif sebagai upaya pencegahan dan penanggulangan terhadap bahaya kebakaran. Tujuan umum dari penelitian ini adalah untuk mengetahui penerapan sistem proteksi kebakaran aktif dalam upaya pencegahan dan penanggulangan bahaya kebakaran di PTPN IV Unit PKS Pabatu. Metode penelitian yang digunakan adalah penelitian kualitatif dengan jenis studi kasus. Hasil penelitian yang didapatkan bahwa tingkat pemenuhan sistem proteksi aktif di area PTPN IV Unit PKS Pabatu sebesar 41,34% sesuai dengan peraturan perundangan yaitu alarm kebakaran 37,5% sesuai dengan Permenaker No.Per.02/MEN/1983, hidran 69,2% sesuai dengan Keputusan Menteri PU No.10/KPTS/2000, APAR 100% sesuai dengan Permenaker No.Per 04/Men/1980, tidak terdapat detektor dan sprinkler. Saran untuk pihak PTPN IV yaitu melakukan peninjauan ulang dan penambahan alarm kebakaran serta melengkapi sistem proteksi aktif seperti pengadaan sistem detektor dan sprinkler yang belum terdapat pada area pabrik PTPN IV Unit PKS Pabatu. BRI Unit Simpang IV Sipin focuses on the quality of service provided in the form of speed and convenience given to customers. One form of problem that becomes a phenomenon experienced by customers and needs to be the main concern for banks to always be improved and improved so that the bank's performance becomes more productive is the queue, queueing or waiting line that occurs while waiting for banking services. This research aims to know and analyze the characteristics of BRI Unit Simpang IV Sipin Jambi city and to know and analyze the effect of customer satisfaction on customer service BRI Unit Simpang IV Sipin Jambi City. In this study the data used is primary data. This primary data was obtained from live interviews using questionnaires on research objects with the time the study was conducted in January - March 2020. The result of this research is that the Quality of Customer Service service has an effect on the customer satisfaction of BRI Unit Simpang IV Sipin Jambi City. The amount of influence of the training is 19.8%. The service provided by BRI Customer Service Unit Simpang IV Sipin must still be maintained or worthy to be improved again, this can affect the satisfaction of existing customers.

Abstract Abstract 4197 Background: Graft-versus-host disease (GVHD) has emerged as a major cause of morbidity and mortality after double-unit cord blood transplantation (DCBT), but data concerning its manifestations, treatment response, and risk factors are limited. Moreover, the incidence and clinical characteristics of GVHD after day 100 in DCBT recipients have not been well described. Methods: We evaluated the incidence and nature of GVHD in 115 DCBT recipients (median 37 years, range 0.9–69) transplanted for hematologic malignancies with either myeloablative (n = 88) or non-myeloablative conditioning (n = 27). CB units were 4–6/6 human leukocyte antigen (HLA)-A,-B antigen, -DRB1 allele match to the recipient, and all patients received calcineurin-inhibitor/mycophenolate mofetil immunosuppression without anti-thymocyte globulin. Results: With a median follow-up of 33 months (range 8–73), the cumulative incidences of grade II-IV and III-IV acute GVHD (aGVHD) at day 180 were 53% (95% CI: 44–62) and 23% (95% CI: 15–31), respectively. Among patients with grade II-IV aGVHD, the median onset was 40 days (range 14–169), but earlier for those with grade III-IV (median 35 days). The gastrointestinal (GI) tract was the most commonly affected organ (80%, 14 upper gut, 9 lower gut, 26 both), followed by skin (39%), and liver (18%). Among patients with grade II-IV aGVHD, 29 (48%) were treated with systemic corticosteroids, 27 (44%) with budesonide alone, and 4 (7%) with topical corticosteroids. Budesonide was used as the sole treatment exclusively in adults for grade II disease predominantly affecting the gut. Treatment response by day 28 was 79% and 85% to systemic corticosteroids or budesonide, respectively. Of 89 engrafted patients disease-free at day 100, 54% subsequently had active GVHD with 79% of those affected having persistent/recurrent aGVHD or overlap syndrome. Classical chronic GVHD was quite uncommon affecting only 10 patients (21%) in the study. To take into account potential confounding variables associated with day 180 grade III-IV aGVHD incidence, a multivariate Cox regression analysis was performed (Table). Grade III-IV aGVHD incidence was lower if the engrafting unit-recipient human HLA-A,-B,-DRB1 allele match was > 4/6 (HR 0.385, p = 0.031) (Figure), whereas engrafting unit nucleated cell dose and unit-unit HLA-match were not significant. Conclusions: GVHD after DCBT was common in our study, predominantly affected the gut, and had a high rate of successful response to treatment. Late GVHD frequently had acute features. Our findings support the consideration of HLA-A,-B,-DRB1 allele donor-recipient (but not unit-unit) HLA-matching in unit selection. This would represent a practice change for most transplant centers. The effect of slope length on runoff and soil loss, and the loss and enrichment ratio of nitrogen from steep slopes planted to pineapple, were studied under natural field conditions over 3 sampling dates at Imbil, Gympie, Queensland. The experiments were conducted on 3 sites, with each site having 3 slope lengths of 9, 15 and 23 m, each replicated twice. The mean slope gradient was 32% with extreme variation from 24 to 37%. During sampling for soil loss measurement, subsamples were also collected, air-dried, weighed, and analysed for total nitrogen concentrations. Samples of the original soil (uneroded) were also taken and analysed and the concentration of total nitrogen in the original soil was used for enrichment ratio and nutrient nitrogen loss determination. The runoff per unit area was not significantly affected by slope length. Total soil loss per unit area in each erosion event increased with increasing slope length. The relationship between soil loss (Y ) and slope (L) is well fitted by the function: The measured sediment concentrations (in the case of site 3) for different slope lengths were higher than those for the estimated sediment concentration at the transport limit assuming either uniform overland flow, or furrow or rill geometry. The estimated sediment concentration at the transport limit provided evidence for the occurrence and importance of rilling which increased soil loss per unit area. Furthermore, sediment concentration estimated at the transport limit assuming rill or furrow geometry was higher than uniform overland flow and increased as slope length increased from 9 to 23 m. The enrichment ratio for total nitrogen decreased with increasing eroded sediment. The eroded sediment was found to contain a nitrogen concentration very similar to that of the original soil, resulting in the enrichment ratio, in some cases, being close to or slightly below unity. Total nitrogen loss per unit area showed a similar trend to total soil loss and it increased with increasing slope length. The relationship between total soil loss in the event (SL) and enrichment ratio (ER) for total nitrogen for all slope lengths is represented by the logarithmic function

Objective: We report a case of insulin desensitization in a patient with known allergy to multiple insulin preparations who presented with diabetic ketoacidosis (DKA). Methods: Clinical and laboratory data, and desensitization protocols are presented. Results: A 65-year-old woman with type 2 diabetes and a documented insulin allergy presented with severe DKA. She was managed initially with intravenous (IV) fluids, sodium bicarbonate, and hemodialysis. An intradermal skin test was positive for 0.01 units/mL of human regular insulin. A rapid desensitization protocol for IV human regular insulin was initiated after pretreatment with methylprednisolone, ranitidine, montelukast, and cetirizine. An initial dilution of 1 unit of insulin in 100,000 mL of 0.9% sodium chloride was started at 5 mL/hour IV. The dilution was increased at 60-minute intervals to 1 unit/10,000 mL, 1 unit/1,000 mL, 1 unit/100 mL, 1 unit/10 mL, then 1 unit/1 mL. The dose was then increased from 1 to 7 units/hour (0.1 units/kg body weight/hour). The anion gap closed after 24 hours, and overlapping desensitization was started for subcutaneous (SC) human regular insulin starting with 0.00001 units with a gradual increase to 7 units before meals and 6 units at bedtime over 5 days. There were no anaphylactic reactions to IV or SC insulin. She was discharged with human regular insulin SC 4 times daily, oral montelukast, cetirizine, diphenhydramine as needed, and an epinephrine pen. No allergic reactions were reported at follow-up visits. Conclusion: Rapid insulin desensitization is possible to allow treatment of DKA with human regular insulin IV in patients with known insulin allergy.

The pentanuclear title compound, [V5(C7H13O)10O5], has a metal–oxygen core that consists of five vanadyl(IV) centres bridged by the O atoms of cyclohexylmethanolate ligands. This particular ring topology is new to oxovanadium(IV) chemistry and resembles the structure proposed for [V5O15]5−on the basis of51V NMR studies in aqueous solution. The bulky cyclohexylmethanolate ligands adopt chair-like conformations and project outwards from the central cyclic core. The title compound crystallizes in a centrosymmetric triclinic unit cell, which contains four independent but chemically identical molecules in the asymmetric unit. The crystal structure is devoid of any significant intermolecular interactions.

Background: The prognostication of critically ill patients, in a systematic way, based on definite objective data is an integral part of the quality of care in Intensive Care Unit (ICU). Acute physiology and chronic health evaluation (APACHE) scoring systems provide an objective means of mortality prediction in Intensive Care Unit (ICU). The aims of this study were to compare the performance of APACHE II and APACHE IV in predicting mortality in our intensive care unit (ICU).Methods: A prospective observational study was conducted in a 13 bedded intensive care unit (ICU) of a tertiary level teaching hospital. All the patients above the age of 12 years, irrespective of diagnosis managed in ICU for >24hours were enrolled. APACHE II and APACHE IV scores were calculated based on the worst values in the first 24hours of admission. All enrolled patients were followed up, and outcome was recorded as survivors or non survivors. Observed mortality rates were compared with predicted mortality rates for both the APACHE II and APACHE IV. Receiver operator characteristic curves (ROC) were used to compare accuracy of the two scores.Results: APACHE II score of the patients ranged from 1 to 32 and APACHE IV score of the patients ranged from 25 to 142. There was good correlation between APACHE II and APACHE IV scores with the spearman’s rho value of 0.776 (P<0.01). Discrimination for APACHE II and APACHE IV models were good with area under ROC curve of 0.805 and 0.832 respectively. APACHE IV was more accurate than APACHE II in this regard. The cut-off point with best Youden index for APACHE II was 17 and for APACHE IV were 72 respectively for predicting mortality.Conclusions: Discrimination was better for APACHE IV than APACHE II model however Calibration was better for APACHE II than APACHE IV model in present study. There was good correlation between the two models observed in present study.

Lieberman and Phillips (2017; LP) introduced a multivariate stochastic unit root (STUR) model, which allows for random, time varying local departures from a unit root (UR) model, where nonlinear least squares (NLLS) may be used for estimation and inference on the STUR coefficient. In a structural version of this model where the driver variables of the STUR coefficient are endogenous, the NLLS estimate of the STUR parameter is inconsistent, as are the corresponding estimates of the associated covariance parameters. This paper develops a nonlinear instrumental variable (NLIV) as well as GMM estimators of the STUR parameter which conveniently addresses endogeneity. We derive the asymptotic distributions of the NLIV and GMM estimators and establish consistency under similar orthogonality and relevance conditions to those used in the linear model. An overidentification test and its asymptotic distribution are also developed. The results enable inference about structural STUR models and a mechanism for testing the local STUR model against a simple UR null, which complements usual UR tests. Simulations reveal that the asymptotic distributions of the NLIV and GMM estimators of the STUR parameter as well as the test for overidentifying restrictions perform well in small samples and that the distribution of the NLIV estimator is heavily leptokurtic with a limit theory which has Cauchy-like tails. Comparisons of STUR coefficient and standard UR coefficient tests show that the one-sided UR test performs poorly against the one-sided STUR coefficient test both as the sample size and departures from the null rise. The results are applied to study the relationships between stock returns and bond spread changes.

An out-patient parenteral antibiotic therapy program provided through a medical day care unit was evaluated in a tertiary care hospital. From July 11, 1988 to December 31, 1990, 122 patients were treated either on site at the unit or at home with self-administered intravenous antibiotics. In all, 142 courses of parenteral antibiotics (mostly cephalosporins and clindamycin) were given for a total of 124 infections, mostly bone and soft tissue infections (67 of 124, 54%). The duration of out-patient therapy ranged from two to 62 days with a mean duration of 9.4 days if treated at the unit, or 13.2 days in the home care model (1476 patient-days). Vein access was peripheral and catheters remained functional for an average of 4.9 days (range 0.5 to 22 days). Only two patients experienced adverse drug reactions that necessitated modification of treatment. One other case was readmitted to the hospital for surgical debridement. The average cost per patient-day was $66 compared with $375 for in-hospital therapy. This program proved to be safe, efficient, and cost-effective.

Desain penelitian yang digunakan pada penelitian ini adalah penelitian studi kasus dengan pendekatan kualitatif deskriptif. Teknik pengumpulan data dilakukan melalui wawancara, kuesioner, dan dokumentasi. Hasil dari penelitian ini menunjukan bahwa: (1) Penerapan sistem pengendalian intern penerimaan dan pengeluaran kas pada PTPN IV Unit Pks Pabatu dinilai telah sesuai dengan standar yang ada pada Peraturan Pemerintah 60 tahun 2008, (2) Terdapat kelemahan pada dua komponen sistem pengendalian internal berdasarkan Peraturan Pemerintah 60 tahun 2008 yang unsur-unsurnya belum sepenuhnya diterapkan oleh PTPN IV Unit PKS Pabatu yaitu komponen lingkungan pengendalian dan komponen kegiatan pengendalian.

1. Cross-correlation analysis was used to study the organization of inhibitory connections between type II or type III units and type IV principal cells in cat dorsal cochlear nucleus (DCN). Pairs of units were isolated using two microelectrodes so that information about the distance over which connections are made could be analyzed. Data were obtained from 51 pairs consisting of a type II and a type IV unit and from 22 pairs consisting of a type III and a type IV unit. The analyses in this paper concentrate on type II-type IV pairs. 2. Inhibitory troughs (ITs) are observed in the cross-correlograms of type II-type IV pairs (21/51 cases). An IT is a transient decrease in discharge probability in the postsynaptic (type IV) unit immediately after spikes in the presynaptic unit (type II). The average latency to the start of ITs is 0.73 ms, and the troughs are asymmetric with a faster leading phase. Small excitatory peaks accompany the ITs in type II units, but these are probably secondary effects associated with the IT. ITs are consistent with a monosynaptic, inhibitory connection between type II and type IV units. A variety of evidence suggests that type II responses are recorded from vertical cells, an interneuron in the deep layer of the DCN that may be glycinergic. 3. The cross-correlograms of type III-type IV pairs are more complex and variable than those of type II-type IV pairs--ITs are seen in 4/22 cases, and peaks of correlation that are symmetrically located around the origin (central mound or CM) are seen in 4/22 cases; two cases have both an IT and a CM. CMs result from shared sources of input. Whereas type II-type IV correlogram features change primarily in amplitude as stimulus conditions change, correlogram features in some type III-type IV pairs change qualitatively with stimulus conditions; correlograms are flat for some stimuli and show ITs or CMs or mixtures of the two for others. This variability suggests that the circuitry associated with type III-type IV pairs is more complex than a monosynaptic connection, and further analysis of type III-type IV pairs was not done. 4. In developing countries, intravenous (IV) catheter related infections (CRI) rate is generally high. Neonates are more susceptible to develop CRI. We examined the impact of a quality improvement project on IV CRI rates in the neonatal intensive care unit (NICU) of a district hospital in Rwanda. A pre- and post-intervention study was conducted from 2014 to 2016 to evaluate the IV CRI rate and nurses’ IV management technique. A written test was administered to evaluate their knowledge on the matter. The intervention had three components: First implementing an IV management policy. Secondly, training staff on the policy and finally, managers provided support and supervision during the change. We measured five indicators: (1) the IV CRI rate; (2) the percentage of nurses who tested 80% on IV management knowledge; (3) the percentage of IV devices changed following the World Health Organization (WHO) guideline; (4) IV management technique; and (5) the hospital length of stay (LOS). The IV CRI rate reduced from 32.1% to 14.5% (p < .001). The hospital LOS reduced from 15.31 to 7.43 days (p < .001). The compliance of changing IV following WHO guideline increased from 0% to 99% (p < .001); proper IV management technique use increased from 43% to 96% (p < .001); the mean rank of staff on IV management knowledge score significantly increased from 3.5 to 9.5 (p = .004). This project demonstrates that a quality improvement project can help address the IV CRI at very low cost in a resource-challenged setting.

A 35-year-old female patient with no previously documented allergies who was admitted for elective gynaecological surgery, developed rapid onset, severe anaphylaxis, with dyspnea and cardiovascular collapse, in the operating theatre after receiving routine IV cefazolin prior to induction of anesthesia. She failed to improve with two doses of intramuscular epinephrine followed by two boluses of intravenous epinephrine, but responded to an epinephrine infusion. She was assessed by Internal Medicine and discharged home the following day. This event demonstrates the speed, severity, and profound hypotension in an allergic reaction from intravenous medication, challenges in managing anaphylaxis, and importance of prompt administration of epinephrine via IM route, followed by IV if necessary, in the OR. The case highlighted the inability to ascertain the causative agent through typical allergy testing.

This paper describes the anatomical aspects of a functional brain model that develops A. R. Luria’s ideas. Five functional brain units are described on the basis of ontogenetic, anatomical, histological, functional, and clinical studies: preferential or primordial (unit I), limbic (unit II), cortical (unit III), basal ganglia (unit IV), and cerebellar (unit V). This review allows two large integrated and interrelated functional complexes to be distinguished: a primordial-limbic complex (units I and II) and a supralimbic one (units, III, IV and V). There is consensus that there exists a clear interplay among the cortex, the basal ganglia, and the cerebellum. Three main simplified parallel cortico-basal ganglia systems have been recognized: limbic, associative, and sensorimotor. Certain structures (e. g. neuromodulatory systems, hypothalamus, and paralimbic cortex) form functional links among units. Future studies are required to develop and improve the proposed model.

This paper focuses on the fourth movement of her popular trio Panta Rhei (1981), specifically on the return of a distinctive repeated-note unit at the beginning and throughout the refrains. Mamlok varies the entries of the repeated-note unit so that perceived accelerations and decelerations, as described by Christopher Hasty, occur. By completing projections early or late, Mamlok's work gives the impression of "time in flux" or Panta Rhei.

Land suitability evaluation was carried out to evaluate the suitability of soils of Okoko Item, Abia State, South-eastern Nigeria, for upland rice production. Free method of soil survey was adopted. Qualitative land suitability of the area for rain-fed rice production was done using FAO Land Suitability Classification System. The study area was delineated into four mapping units designated as OK I, OK II, OK III and OK IV. Mapping units OK I and OK II were generally flat, deep and well drained. Mapping unit OK III was generally flat, shallow to moderately deep and poorly to imperfectly drained while mapping unit OK IV was undulating with deep, well drained and gravelly soils. Soils were classified as Typic Rhodudults for OK I, Rhodic Paleudults for OK II, Mollic Epiaquents for OK III and Typic Plinthaqualfs for OK IV. Mapping units OK I and OK II were moderately suitable (S2), mapping unit OK III was permanently not suitable (N2) while mapping unit OK IV was marginally suitable (S3) for upland rice production. Majority of the area (61 %) is permanently not suitable (N2) for sustainable upland rice cultivation due to poor drainage. However, 26 % of the area is moderately suitable (S2) while 13% of the area is marginally suitable (S3) for sustainable upland rice cultivation due to dominance of rock outcrop and boulders.

One of the efforts to find out the satisfaction of employees of residential facilities is to use the C4.5 algorithm classification, where Dataset is obtained through the Questionnaire technique that will be given to employees at PTPN IV Marjandi Unit. As for the satisfaction of employees of residential facilities in terms of aspects (1) water facilities, (2) electricity facilities, (3) kitchen conditions, (4) bedroom conditions, (5) toilet conditions in dwellings inhabited by PTPN IV Unit plantation employees Marjandi. Data processing is assisted using Microsoft Excel software and to validate study data using a rapidminer tool. It is hoped that this research can provide solutions or become a reference for plantations to improve the quality of residential facilities provided to PTPN IV Marjandi Unit employees..

Intravenous (IV) magnesium is used as an adjunct therapy in management of status asthmaticus with a goal of reducing intubation rate. A recent review suggests that IV magnesium use in status asthmaticus reduces admission rates. This is contrary to the observation of practicing emergency room physicians. The goal of this study was to assess trends in IV magnesium use for status asthmaticus in US children’s hospitals over 8 years through a retrospective analysis of children younger than 18 years using the Pediatric Health Information System database. Outcomes were IV magnesium use, inpatient and intensive care unit admission rate, geometric mean length of stay, and 7-day all-cause readmission rate. IV magnesium use for asthma hospitalization more than doubled over 8 years (17% vs. 36%; P < .001). Yearly trends were not significantly associated with hospital or intensive care unit admission rate or 7-day all-cause readmissions, although length of stay was reduced (P < .001).

The paper presents the results of palaeomagnetic studies of Quaternary noncemented deposits from the section of Neporotove on the VIIIth terrace of the Dniester River valley with abnormal thickness (more than 20 m) of the channel alluvium near the village of Neporotove, located on the right bank of the Dniester River. Alluvial facies of river terraces’ deposits are valuable palaeogeographic archives but not quite complete. They are affected by denudation and often do not contain fossils. The possible way for their dating is provided by a palaeomagnetic method performed on suitable for sampling underlying and overlying beds. The alluvial sequence consists of four units. Unit I is composed of inclined gravel-pebble layers with the sand filler with a visible thickness of about 8 m. Unit II has bedded over the denudated surface of Unit I; it consists of light-yellow laminated aleurit loam, 5-30 cm thick underlaid by 0.5-1.5 bed of fine sand. We consider Unit II to be lacustrine deposits accumulated in quiet water. Unit III is represented with inclined or sub-horizontally layered gravel-pebble-boulder deposits with up to 10 m of visible thickness. It includes boulders and blocks of sedimentary rocks up to 1.0 in diameter, considered as drop-stones. In the roof of the gravel-pebble Unit III, there is the white carbonate layer, probably, the illuvial horizon of the palaeosol (mr1), which transited up into the brownish-red horizon A (Unit IV). Unit IV – dark-red sandy-gravel horizon, pedosediment, probably partly the reworked material washed into ice wages in the roof of Unit III. As a result of alternating field stepwise demagnetisation of natural remanence of sediments, we determined that loamy Unit II, which separates members of gravel stratum, and sandy Unit IV, which overlays packs of gravel alluvium, bear characteristic remanent magnetisation (RM) with normal geomagnetic polarity. Taking into consideration lithology and two-fold structure of terrace gravel alluvium as well as palaeomagnetic results, we assume the lacustrine deposits of Unit II was formed during Jaramillo palaeomagnetic subchron corresponding to the end of Shyrokyne Stage, and a pedosediment of Unit IV was formed during Martonosha Stage. The upper alluvial suite of a terrace (Unit III) was generally deposited during a cold stage,directly prior to the onset of lacustrine loam sedimentation, while the lower alluvial member (Unit I) dates from the preceding warm stage. Thus, we defined the geological age of the whole channel alluvium strata of the VIIIth Dniester terrace as Shyrokyne-Pryazovya Stage.

This study discusses determining the best security member in PTPN IV marjandi business unit Data sources used from the Marjandi Business Unit PTPN IV office. This study uses the Decision Support System (SPK) technique in the data processing process using Profile matching method. Profile matching method is a mechanism for decision making by assuming that there is an ideal target value that must be met by the subject under study, rather than the minimum level that must be met or passed. The purpose of this study is to help the management to make it easier to do the best security selection at the Marjandi PTPN IV business unit.

Kebakaran merupakan suatu bencana yang tidak diinginkan yang dapat menimbulkan kerugian dari berbagai pihak dan juga dapat banyak menelan korban jiwa. Bencana kebakaran harus dikelola dengan baik dan terencana dan harus adanya sistem tanggap darurat dalam melakukan penanggulangan dalam kebakaran. Pencegahan kebakaran dapat diwujudkan dengan membuat sistem tanggap darurat kebakaran, penyuluhan tentang pencegahan kebakaran, prosedur pencegahan kebakaran, dan pelatihan kebakaran. Penelitian ini bertujuan untuk mengetahui sejauh mana kesiapsiagaan petugas aparatur pemadam kebakaran. Manfaat peneltian memberikan masukan sebagai bahan evaluasi dan pertimbangan dalam upaya pencegahan kecelakaan dan gangguan kesehatan akibat kerja. Metode penelitian adalah deskriptif kualitatif dengan teknik pengumpulan data Observasi dan wawancara. Lokasi penelitian di BPBD UPT PBD Balikpapan Selatan. Hasil penelitian ini dapat diekathui bahwa Kesiapsiagaan pada petugas yang ada sudah baik terlihat dari adanya struktur organisasi yang ada dan pada sistem tanggap darurat yang ada terdapat tidak kesesuaian, dari hasil pengamatan yang dilakukan dari kesiapsiagaannya menurun dikarenakan adanya pandemi covid yang menyebabkan pelatihan yang seharusnya dilakukan secara berkala menjadi terhambat dalam beberapa bulan terakhir. Standar kualifikasi aparatur petugas pemadam yang ada di tempat ini juga memiliki tidak kesesuaian yang cukup signifikan rata-rata kesesuaian pada standar kualifikasi aparatur petugas kebakaran memiliki nilai sebesar 76%.

Introduction: Intensive care unit (ICU) prognostic models are predominantly used in more developed nations such as the United States, Europe and Australia. These are not that popular in Southeast Asian countries due to costs and technology considerations. The purpose of this study is to evaluate the suitability of the acute physiology and chronic health evaluation (APACHE) IV model in a single centre Malaysian ICU. Materials and Methods: A prospective study was conducted at the single centre ICU in Hospital Sultanah Aminah (HSA) Malaysia. External validation of APACHE IV involved a cohort of 916 patients who were admitted in 2009. Model performance was assessed through its calibration and discrimination abilities. A first-level customisation using logistic regression approach was also applied to improve model calibration. Results: APACHE IV exhibited good discrimination, with an area under receiver operating characteristic (ROC) curve of 0.78. However, the model’s overall fit was observed to be poor, as indicated by the Hosmer-Lemeshow goodness-of-fit test (Ĉ = 113, P <0.001). Predicted in-ICU mortality rate (28.1%) was significantly higher than the actual in-ICU mortality rate (18.8%). Model calibration was improved after applying first-level customisation (Ĉ = 6.39, P = 0.78) although discrimination was not affected. Conclusion: APACHE IV is not suitable for application in HSA ICU, without further customisation. The model’s lack of fit in the Malaysian study is attributed to differences in the baseline characteristics between HSA ICU and APACHE IV datasets. Other possible factors could be due to differences in clinical practice, quality and services of health care systems between Malaysia and the United States. Distilasi merupakan salah satu proses terpenting dalam pengolahan minyak bumi.<em> Propylene Recovery Unit (PRU) </em>merupakan unit yang berfungsi untuk memisahkan dan mengolah LPG campuran<em> (Propylene, Propane, Ethylene, Ethane, n-butane, methane, iso butane) </em>dari Unit LPG<em> Merox. </em>Produk utama yang dihasilkan dari unit<em> Propylene Recovery Unit </em>ini adalah produk <em>Propylene </em>dan<em> LPG. Feed </em>pada unit PRU berupa produk <em>LPG </em>campuran yang selanjutnya akan mengalami proses pemisahan pada kolom pemisahan <em>Propane Propylene. </em>Berdasarkan hasil evaluasi menggunakan simulasi dan analisa data diatas dapat disimpulkan bahwa jumlah <em>teoritical tray </em>berdasarkan umpan pada verifikasi data desain sudah sesuai dengan jumlah <em>teoritical tray</em> sebenarnya<em>. </em>Jumlah <em>teoritical tray </em>berdasarkan umpan pada bulan Mei dan Juni sudah sesuai dengan jumlah <em>teoritical</em> sebenarnya<em>. </em>Namun dengan nilai <em>Refuks Ratio Minimum </em>diatas batas maksimalnya<em>. </em>Pada optimasi <em>Refluk Rate </em>pada bulan Mei didapatkan hasil semakin tinggi nilai <em>Refluks Rate</em> maka jumlah <em>yield Propylene</em> nya akan naik sedangkan kemurnian nya akan menurun. Dan untuk optimasi Tekanan Kondenser didapatkan hasil bahwa semakin tinggi tekanan kondenser nya maka hasil <em>yield Propylene </em>juga akan semakin naik. Namun untuk kemurniannya, kenaikan tekanan kondenser tidak berpengaruh. Pada optimasi <em>Refluk Rate</em> pada bulan Juni didapatkan hasil semakin tinggi nilai <em>Refluks Rate</em> maka jumlah <em>yield Propylene </em>nya akan naik sedangkan untuk kemurnian nya akan menurun. The Seeb Formation (Middle Eocene) is an about 600 m thick transgressive carbonate succession deposited in the Batina and Muscat coastal region of Oman. The formation consists of five informal, but distinct units, and their stacking architecture suggests a deepening-upward, shallow marine depositional setting. Unit I is characterized by cross-bedded, sandy, bioclastic packstones to grainstones deposited in a high energy beach-to-intertidal environment. Unit II consists of indistinctly bedded, nodular, bioclastic (mainly larger foraminifera) packstones and wackestones deposited in a logoonal lagoonal environment. Unit III is defined by medium to thickly bedded, bioclastic packestones to grainstones and subordinate, laterally confined conglomerates. Prominent sedimentary structures in Unit III include hummocky and swaly cross-stratificiation, erosional surfaces, dewatering-induced deformations and laterally amalgamating beds. This unit represents sub-tidal sand shoals deposited in a storm-dominated shelf (between the fair-weather wave-base and storm-base). Unit IV is extensively burrowed, nodular, bioclastic wackestone to rudstone which is similar to Unit II in many aspects. Unit IV was deposited on the basinward side of the Unit III sand shoals below the reach of the storm-generated waves and currents. The uppermost Unit V is characterized by poorly-cemented bioclastic (large foraminiferal) rudstones with clay and silt-size quartz matrix. Bioclasts are generally intact with no apparent reworking. Deposition of Unit V is also envisaged as a low-energy, outershelf environment.
