Prosthetic joint infection (PJI) is a rare but refractory complication of arthroplasty. Accurate identification of pathogens is a key step for successful treatment of PJI, which remains a challenge for clinicians and laboratory workers. We designed a combined culture method with sonication of implants and incubation in a BD Bactec system to improve the effectiveness of pathogen diagnosis in PJI. The aims of this study were to investigate the diagnostic accuracy of sonicate fluid cultures in the BD Bactec system and to compare the results with those of synovial fluid cultures in the BD Bactec system. The prosthetic components removed were sonicated in Ringer's solution, and then sonicate fluid was incubated in Bactec bottles for 5 days. Synovial fluid was incubated in Bactec bottles for 5 days as a control. Synovial fluid cultures with Bactec bottles and sonicate fluid cultures with Bactec bottles showed sensitivities of 64% and 88%, respectively (P= 0.009), with specificities of 98% and 87% (P= 0.032), respectively. Sonicate fluid cultures with Bactec bottles were more sensitive than synovial fluid cultures with Bactec bottles regardless of whether antimicrobial agents were used within 14 days before surgery (81% versus 52%;P= 0.031) or not (93% versus 72%;P= 0.031). Sonication of explanted prostheses followed by incubation of the resulting sonicate fluid in Bactec bottles detected many more pathogens than did synovial fluid cultures with Bactec bottles. This method is also effective in cases with antibiotic treatment before surgery.

ABSTRACT Convulsions and encephalopathy are frequent complications of childhood shigellosis. We studied the role of nitric oxide (NO) inShigella-related seizures in an animal model. Pretreatment of mice with Shigella dysenteriae 60R sonicate elevated serum NO levels and enhanced the convulsive response to pentylenetetrazole (PTZ), as indicated by a higher mean convulsion score and a higher number of mice responding with seizures. Treatment of the mice with S-methylisothiourea sulfate (SMT), a potent inhibitor of inducible NO synthase (NOS), prevented the elevation of serum NO levels and concomitantly reduced the enhanced response to PTZ. The mean convulsion scores were 0.7, 0.7, 1.3, and 0.8 for mice treated with saline, saline and SMT, S. dysenteriae 60R sonicate, and S. dysenteriae60R sonicate with SMT, respectively (P = 0.001 for 60R sonicate versus saline and P = 0.013 for 60R sonicate versus 60R sonicate with SMT). The corresponding seizure rates were 40, 44, 75, and 47% for saline, saline with SMT, S. dysenteriae 60R sonicate, and S. dysenteriae 60R sonicate with SMT, respectively (P = 0.0004 for 60R sonicate versus saline and P = 0.005 for 60R sonicate versus 60R sonicate with SMT). In contrast, injection of N-nitro-l-arginine, a selective inhibitor of constitutive NOS, neither abolished the elevation of serum NO nor attenuated the enhancement of seizures. These findings indicate that NO, induced by S. dysenteriae 60R sonicate, is involved in enhancing the susceptibility to seizures caused by S. dysenteriae.

ABSTRACT A sandwich immunoassay for the rapid, low-level detection of Mycobacterium avium subsp. paratuberculosis has been developed. M. avium subsp. paratuberculosis is the causative agent of Johne's disease in cattle, and one of the major obstacles in controlling the spread of this disease is the inability to rapidly detect small amounts of bacteria or other diagnostic markers shed during the subclinical stage of infection. This paper details the development and performance of an assay for sonicated M. avium subsp. paratuberculosis lysate that is based on surface-enhanced Raman scattering (SERS). There are two key components of the assay: (i) an immobilized layer of monoclonal antibodies that target a surface protein on the microorganism; and (ii) extrinsic Raman labels (ERLs) that are designed to selectively bind to captured proteins and produce large SERS signals. By correlating the number of M. avium subsp. paratuberculosis bacilli present prior to sonication to the amount of total protein in the resulting sonicate, the detection limit determined for total protein can be translated to the microorganism concentration. These findings yield detection limits of 100 and 200 ng/ml (estimated to be 500 and 1,000 M. avium subsp. paratuberculosis bacilli/ml) for sonicate spiked in phosphate buffer and sonicate spiked in whole milk, respectively. Moreover, the time required to complete the assay, which includes sample preparation, antigen extraction, ERL incubation, and readout, is less than 24 h. The potential for incorporation of this novel assay into diagnostic laboratories is also briefly discussed.

Abstract The control system described here is used to monitor the performance of urinary reagent-strip tests for leukocytes. The presence of leukocyte esterase in the urine is used as a marker for leukocytes in urine. The control system is based on sonicated leukocytes, isolated from whole blood. The esterase activity of this sonicate is determined by spectrophotometry with the N-tosyl-L-alanine ester of 5-phenyl-3-hydroxypyrrole as substrate. The assay result is used to determine the amount of sonicate needed to prepare buffered esterase-containing solutions. Such control solutions mimic leukocytic urines and are stable for 6 h at room temperature. The variability of the control system was tested by preparing it five times in a day on five separate days. The overall CV for Leukostix Reagent Strips (Ames Division, Miles Laboratories, Inc.) when tested with these solutions and analyzed with a reflectance spectrophotometer was 8%; for visual readings it was 10%. The overall CV for Chemstrip LN Reagent Strips (Biodynamics, Indianapolis, IN) was 10%.

Abstract Mast cell-eosinophil interactions in allergy have not yet been completely defined. To determine whether mast cells influence eosinophil survival, human peripheral blood eosinophils were incubated with rat peritoneal mast cell sonicate. After 3 days, viable eosinophils in medium were 21.3% compared with 44% with mast cell sonicate. Like sonicate, supernatants of compound 48/80-activated mast cells enhanced eosinophil survival, demonstrating that the factor(s) involved is stored preformed and rapidly released. Increased eosinophil survival was due to an inhibition of apoptosis (morphologic analysis; annexin V/PI). Neutralizing Abs to granulocyte-macrophage CSF (GM-CSF), but not to IL-3 or IL-5, decreased by 61.7% the enhancing effect on eosinophil viability. Eosinophils are the source of GM-CSF since its release in the culture medium was inhibited by their incubation with the mast cell sonicate together with dexamethasone. In addition, eosinophils incubated with the sonicate expressed mRNA for GM-CSF. To partially characterize the mast cell-derived factor(s) increasing eosinophil survival, the sonicate was heated (56°C/30 min or 100°C/10 min) or preincubated with antihistamines or with anti-TNF-α-neutralizing Abs. Most of the activity was heat labile. TNF-α was found to be predominantly (70%) responsible, while histamine had no role. Mast cell sonicate also caused eosinophils to release eosinophil peroxidase and to display morphologic signs of activation. In conclusion, we have demonstrated that mast cells enhance eosinophil survival in part through their activation to produce and release the autocrine survival cytokine GM-CSF.

In this study the detection rates of bacterial infection of hip prostheses by culture and nonculture methods were compared for 120 patients with total hip revision surgery. By use of strict anaerobic bacteriological practice during the processing of samples and without enrichment, the incidence of infection by culture of material dislodged from retrieved prostheses after ultrasonication (sonicate) was 22%. Bacteria were observed by immunofluorescence microscopy in 63% of sonicate samples with a monoclonal antibody specific forPropionibacterium acnes and polyclonal antiserum specific for Staphylococcus spp. The bacteria were present either as single cells or in aggregates of up to 300 bacterial cells. These aggregates were not observed without sonication to dislodge the biofilm. Bacteria were observed in all of the culture-positive samples, and in some cases in which only one type of bacterium was identified by culture, both coccoid and coryneform bacteria were observed by immunofluorescence microscopy. Bacteria from skin-flake contamination were readily distinguishable from infecting bacteria by immunofluorescence microscopy. Examination of skin scrapings did not reveal large aggregates of bacteria but did reveal skin cells. These were not observed in the sonicates. Bacterial DNA was detected in 72% of sonicate samples by PCR amplification of a region of the bacterial 16S rRNA gene with universal primers. All of the culture-positive samples were also positive for bacterial DNA. Evidence of high-level infiltration either of neutrophils or of lymphocytes or macrophages into associated tissue was observed in 73% of patients. Our results indicate that the incidence of prosthetic joint infection is grossly underestimated by current culture detection methods. It is therefore imperative that current clinical practice with regard to the detection and subsequent treatment of prosthetic joint infection be reassessed in the light of these results.

Fifty-four subjects were assessed. The sensitivities of sonicate fluid culture were 77.8% (14 of 18) in subjects with an antimicrobial-loaded cemented prosthesis and 58.3% (21 of 36) in subjects with an antimicrobial-free prosthesis.

ABSTRACT We have previously demonstrated that culturing periprosthetic tissue in blood culture bottles (BCBs) improves sensitivity compared to conventional agar and broth culture methods for diagnosis of prosthetic joint infection (PJI). We have also shown that prosthesis sonication culture improves sensitivity compared to periprosthetic tissue culture using conventional agar and broth methods. The purpose of this study was to compare the diagnostic accuracy of tissue culture in BCBs (subsequently referred to as tissue culture) to prosthesis sonication culture (subsequently referred to as sonicate fluid culture). We studied 229 subjects who underwent arthroplasty revision or resection surgery between March 2016 and October 2017 at Mayo Clinic in Rochester, Minnesota. Using the Infectious Diseases Society of America (IDSA) PJI diagnostic criteria (omitting culture criteria) as the gold standard, the sensitivity of tissue culture was similar to that of the sonicate fluid culture (66.4% versus 73.1%, P = 0.07) but was significantly lower than that of the two tests combined (66.4% versus 76.9%, P < 0.001). Using Bayesian latent class modeling, which assumes no gold standard for PJI diagnosis, the sensitivity of tissue culture was slightly lower than that of sonicate fluid culture (86.3% versus 88.7%) and much lower than that of the two tests combined (86.3% versus 99.1%). In conclusion, tissue culture in BCBs reached sensitivity similar to that of prosthesis sonicate fluid culture for diagnosis of PJI, but the two tests combined had the highest sensitivity without compromising specificity. The combination of tissue culture in BCBs and sonicate fluid culture is recommended to achieve the highest level of microbiological diagnosis of PJI.

Denna rapport beskriver arbetet med ett projekt som haft målet att skapa rutiner för Philips Sonicares Facebooksida, för att öka engagemanget hos deras följare. Två fokusgrupper har utförts och med hjälp av dessa och tidigare forskning skapades uppdateringsrutiner för Facebooksidan, dessa följdes sedan i tre veckor. Efter tre veckor sammanställdes resultatet för att se vilka rutiner som fungerat och vilka som kunde uppdateras ytterligare. Resultatet visade att inlägg med underhållande innehåll fick mer engagemang från följarna, inlägg med information hade större räckvidd. Bilder ökade engagemanget, de tidpunkter som var bäst att publicera var mitt på dagen eller tidigt på kvällen.
This report describes the process of a project with the sole purpose to create routines that will increase the user engagement for Philips Sonicares Facebook page. Two focus groups were held, with the help of those and previous research, routines could be created for the Facebook page and those were used for three weeks. After this period, the results were compiled to see which routines had worked, and which needed to be updated. The results showed that posts with entertaining content got more engagement from the followers and posts with information got bigger reach. Also, pictures increased engagement, and the best times to publish posts were midday or early evening.

Il tomografo sonico è uno strumento di recente applicazione nell’analisi morfo-sintomatica delle alberature. Si tratta di uno strumento che sfrutta la propagazione delle onde sonore nel legno per determinarne la densità e le possibili alterazioni interne. Oltre all’applicazione su larga scala in un parco di Imola, per effettuare una valutazione approfondita di tutti gli esemplari, lo strumento è stato applicato per scopi diversi. In prima analisi è stato utilizzato per valutare stadi precoci di alterazione e l’evoluzione delle patologie interne nel tempo. Successivamente si voleva identificare il percorso di sostanze liquide iniettate con mezzi endoterapici nel tronco, attraverso l’applicazione di tomografia sonica sopra e sotto il punto di iniezione. In ultima analisi è stato effettuato un confronto tra tomografia sonica e risonanza magnetica nucleare per identificare patologie invisibili ai normali strumenti utilizzati nell’analisi della stabilità delle piante.
Sonic tomograph is a new instrument used in morpho-symptomatic analysis of trees. This tool use sonic waves propagation to establish wood density and probably internal decay. Sonic tomography was applicated on large scale in a urban park in Imola, to value all trees in depth way. Subsequently was used for different aims. First of all the instrument was applied to evaluate early stage of decay and development of decline in urban trees, by periodic tests in time. Afterwards was used to determine the travel of liquid substances injected in the trees by trunk injection methods. In the last analysis there was carried out a comparison between sonic tomograph and nuclear magnetic resonance to identify decay invisible to normal tools used in tree evaluation

Resumo: O objetivo deste estudo foi avaliar a efetividade de pontas sônicas diamantadas (Sonicparo ¿), na descontaminação de superfícies radiculares de dentes unirradiculares sob parâmetros clínicos (índice de placa, sangramento à sondagem, profundidade de sondagem, nível de inserção clínica relativo, e recessão gengival) e microbiológicos, comparados a instrumentação com curetas. Esta pesquisa trata-se de um teste clínico controlado radomizado com um desenho em boca dividida, envolvendo 12 pacientes com moderada a avançada periodontite crônica apresentando profundidade de sondagem> 4,4 mm. Os pacientes foram submetidos a tratamento inicial com sessões de remoção de placa supragengival e instruções de higiene bucal. Posteriormente, retalho cirúrgico foi realizado e a superfície dos dentes instrumentadas. Os parâmetros clínicos e microbiológicos foram avaliados no tempo inicial (O), e em 30, 90 e 180 dias após a cirurgia. Análise de variância e teste de Tukey foram realizados para todos os períodos (p<0.05) e os resultados mostraram que quando as pontas sônicas diamantadas foram comparadas a instrumentos manuais, melhoria dos parâmetros foi alcançada para ambos tratamentos, nenhuma diferença estatística foi mostrada em redução de profundidade de sondagem, índice de placa, sangramento à sondagem, ganho de nível de inserção, recessão gengival, tempo de instrumentação e reduções na contagem microbiana e unidades formadoras de colônias. Não existiram diferenças significantes entre os tratamentos ao longo do tempo para todos os parâmetros avaliados. Dentro dos limites deste estudo, pode ser concluído que o uso das pontas sônicas pode ser uma boa alternativa para a descontaminação radicular
Abstract: The aim of this study was to evaluate the effectiveness of diamond-coated sonic scaler insert (Sonicparo ¿), in the root surface debridement in single-rooted teeth on clinical (plaque index, bleeding on probing, probing depth, relative attachment levei clínical, and gingival recession) and microbiological parameters, compared to hand instruments. The investigation was a test-controlled randomized clínical trial with a splith-mouth design involving 12 patients with moderate to advanced chronic periodontites with pocket depth > 4,4 mm. The patients were submitted to initial treatment sessions of supragingival plaque, removal and were also instructed in oral hygiene techniques. After, flap surgery was carried out and the tooth surface instrumented. Clinical and microbiological parameters, were evaluated at baseline (O), in the 30, 90 and 180 days after surgery. Analysis of variance and Tukey test was done for all periods (p<0.05) and the results showed that when diamond-coated sonic scaler insert was compared to manual scalers, showed no statistical differences in probing depth reduction, plaque index, bleeding on probing, relative attachment level gain, gingival recession and reductions of the microbial counts and colony-forming units. There were no significant differences between regard with treatments for all parameters at each re-examination. Within the limits of this study, it can be concluded that the use the of diamond-coated sonic scaler insert can be a good alternative for root debridement

Resumo: O objetivo deste estudo foi comparar, usando réplicas de resina epóxica, a topografia de superfícies apicais de caninos humanos contento cavidades retrógradas preparadas com pontas ultra-sônicas antes e depois de seu preenchimento e acabamento com três diferentes técnicas. Também foi realizada uma avaliação quantitativa da capacidade de selamento marginal dessas obturações retrógradas. Oitenta e uma cavidades retrógradas preparadas em caninos humanos foram divididas em grupos de 27 cada. As cavidades foram preenchidas com Super-EBA, IRM, ou ProRoot-MTA e receberam um acabamento inicial com um brunidor de formato arredondado. Dezoito raízes de cada grupo receberam um acabamento final com brocas carbide de tungstênio 30 lâminas (9 raízes) ou com brocas de 28 mm Zekrya (9 raízes). Fotomicrografias de cada espécime obtidas com um microscópio eletrônico de varredura (MEV), antes e depois do preenchimento das cavidades retrógradas, foram comparadas através de um programa de análise de imagens. Após a análise da interface retrobturação/ dentina os espécimes foram preparados e imersos em uma solução de azul de metileno 2% por 12 horas. As raízes foram então trituradas e preparadas para análise em um espectrofotômetro de absorbância. Resultados obtidos mostraram que quando uma broca foi utilizada para um acabamento final nos materiais, uma área significativa (p < 0,05) de ranhuras marginais foi eliminada. A técnica de acabamento não afetou significativamente (p > 0,05) a incidência de fendas nos grupos retrobturados com MT A ou IRM. As obturações retrógradas feitas com Super EBA e IRM, e acabadas com um brunidor ou com brocas Zekrya apresentaram áreas calculadas das fendas significativamente (p < 0,05) maiores que as retrobturações feitas com MT A. Com relação à infiltração os resultados revelaram que o Pro Root MT A apresentou uma média de infiltração significativamente (p < 0,05) menor que retrobturações de Super-EBA e IRM. O fator técnica de acabamento não afetou significativamente (p > 0,05) a incidência de infiltração entre os materiais testados. O coeficiente de correção de Pearson indica que há uma associação positiva de 55,61 % entre as variáveis, ou seja, o acréscimo da quantidade de corante infiltrado está associado com o acréscimo da área de desadaptação das cavidades retrógradas
Abstract: The aim of this study was to compare, using epoxy resin replicas, the surface topography of root ends following ultrasonic preparation, and again after root-end fillings submitted to three different finishing techniques. A quantitatively evaluation of the sealing ability of Super-EBA, IRM and Pro Root MTA root-end fillings was also performed. Eighty-one ultrasonically root-end cavities prepared in human canines, were divided at random into three test groups of 27 each. The cavities were filled with Super-EBA, IRM, or ProRoot-MT A and finished by ball burnishing. Eighteen roots from each group received a final smoothing with either a 30-fluted tungsten carbide finishing bur, or a Zekrya carbide 28 mm bur. Scanning electron micrography (SEM) pictures of each sample were taken prior to and after root-end filling, and dentin/retrofilling interfaces inspected and compared with the aid of an image and analysis system. Samples were then prepared and immersed in 2% methylene blue dye solution for 12 hours. Roots were ground into a powder and prepared for analysis in an absorbency spectrophotometer. Results showed that when a finishing bur was used to finish the set materials, a significant (p < 0.05) area of marginal chipping was eliminated. The fishing technique did not significantly (p > 0.05) affect the incidence of gaps in groups retrofilled with MT A or IRM. Super EBA and IRM retrofillings finished with a ball burnisher or a Zekrya bur displayed a significantly (p < 0.05) larger calculated gap area than roots filled with MT A. In relation to microleakage the results revealed that Pro Root MT A displayed a significantly inferior mean dye microleakage (p < 0.05) than Super-EBA and IRM retrofillings. The finishing technique did not significantly (p > 0.05) affect the incidence of microleakage among the materials tested. The correlation coefficient of Pearson indicates that there is a positive, 55.61% association between the variables and this coefficient, that is, the increase of the quantity of infiltrated dye is associated with the increase of the root-end filling marginal disruption area

Resumo: O ultra-som na medicina tem passado por enorme evolução nas últimas décadas e ocupado posição de destaque cada vez maior como ferramenta para terapia e diagnóstico. Isso é devido principalmente ao fato de que os equipamentos de diagnóstico por ultra-som são de relativo baixo custo, o ultra-som é uma radiação não-ionizante e permite realização de exame por método não-invasivo e as imagens são geradas e visualizadas em tempo real. Na geração de imagens deste tipo, é comum a utilização de transdutores matriciais. Entretanto, o Brasil apresenta defasagem tecnológica com respeito à construção destes transdutores e à eletrônica envolvida em sua operação. O objetivo deste trabalho consistiu no desenvolvimento de circuitos eletrônicos com 12 canais de geração e de recepção de ondas ultra-sônicas para operação com transdutor matricial linear. O sistema é capaz de excitar transdutores piezoelétricos e receber ecos ultra-sônicos na faixa de 0,5 a 30 MHz e tem seus circuitos de recepção protegidos contra a alta tensão dos pulsos gerados para a excitação do transdutor. Os disparos dos elementos do transdutor e o tempo de corte dos sinais nos circuitos de recepção, para evitar receber sinais indesejáveis referentes ao período inicial de oscilação do transdutor, são controlados via circuito com microcontrolador PIC 16F877 que, juntamente com o programa de controle, foram desenvolvidos para conectar o sistema a um microcomputador. Os 12 canais foram caracterizados eletricamente e verificou-se seu funcionamento utilizando um transdutor piezoelétrico linear de 12 elementos com 1 MHz de freqüência central, especialmente desenvolvido para este trabalho. Os resultados mostraram que o sistema funciona adequadamente, gerando imagem de um phantom construído em nosso laboratório
Abstract: Ultrasound in medicine has gone through great evolution in the last few decades and has occupied important position as a tool for therapy and diagnosis. This is due to the ultrasound equipment be of relatively low-cost, ultrasound is a non-ionizing radiation, is a non-invasive imaging method, and the images are created and seen in real time. It is common the use of transducer arrays in order to generate this kind of image. There is a lack of know how in Brazil relative to the construction of these transducers and the involved electronics in their operation. The objective of this work was the development of a multi-purpose 12 channel pulser/receiver electronic circuitry to operate with linear transducer arrays. The system is able to fire ultrasound piezoelectric transducers and to receive ultrasound echo signals in the range 0.5-30 MHz. The system has reception circuits with protection against high voltage pulses. The firing of transducer elements and cutting time of the reception circuits, to avoid unwanted signals of natural initial transducer oscillations, can be controlled via PIC 16F877 hardware and software designed to connect the system to a microcomputer. The electrical characteristics of the 12 channel pulser/receiver and its use in firing a specially constructed 1 MHz 12 element PZT transducer array has been carried out and the images of a specially constructed phantom showed that it can be used in laboratory conditions

Resumo: O objetivo deste estudo foi avaliar longitudinalmente a efetividade de pontas sônicas diamantadas (RootplanerTM), na descontaminação de superfícies radiculares de dentes unirradiculares sob parâmetros clínicos (índice de placa, sangramento à sondagem, profundidade de sondagem, nível de inserção clínica relativo, e retração gengival) e microbiológicos (cultura microbiana e PCR), comparados a instrumentação com curetas. Esta pesquisa trata-se de um teste clínico controlado randomizado com um delineamento em parcela subdividida, envolvendo 12 pacientes com moderada a avançada periodontite crônica apresentando profundidade de sondagem ³ 5 mm. Os pacientes foram submetidos a tratamento inicial com sessões de remoção de placa e cálculo supragengival e instruções de higiene bucal. Posteriormente, retalho cirúrgico foi realizado e a superfície dos dentes instrumentada. Os parâmetros clínicos e microbiológicos foram avaliados no tempo inicial (T0), e em 30 dias, 6, 12 e 24 meses após a cirurgia. Os dados obtidos foram submetidos à análise estatística através de Análise de variância (ANOVA), teste de Tukey, realizados para todos os períodos (a=0.05), Friedman e Wilcoxon foram usados na avaliação não paramétrica das unidades formadoras de colônias. Os resultados mostraram que quando as pontas sônicas diamantadas foram comparadas a instrumentos manuais nenhuma diferença estatística foi encontrada em redução de profundidade de sondagem, índice de placa, sangramento a sondagem, ganho de inserção, recessão gengival. Bem como, nas reduções das contagens microbiológicas das UFCs e diminuição de A.a., P.g. e T.f.. Os resultados não mostraram diferença significativa entre os tratamentos, apenas entre os períodos de avaliação. Concluiu-se, dentro das limitações do estudo que as pontas sônicas diamantadas podem ser uma boa alternativa terapêutica para instrumentação radicular, mantendo a estabilidade da saúde dos tecidos periodontais em longo prazo
Abstract: The aim of this study was to evaluate the long term the effectiveness of diamond-coated sonic scaler insert (RootplanerTM), in the root surface debridement in single-rooted teeth on clinical (plaque index, bleeding on probing, probing depth, relative attachment level clinical, and gingival recession) and microbiological parameters (culture and PCR), compared to hand instruments. The investigation was a test-controlled randomized clinical trial with a splith-mouth design involving 12 patients with moderate to advanced chronic periodontites with pocket depth ³ 5 mm. The patients were submitted to initial treatment sessions of supragingival plaque and cauculus removal and were also instructed in oral hygiene techniques. After, flap surgery was carried out and the tooth surface instrumented. Clinical and microbiological parameters, were evaluated at baseline (0), in the 30 days, 6, 12 and 24 months after surgery. Analysis of variance and Tukey test was done goes all periods (a=0.05) and the results showed that when diamond-coated sonic scaler insert was compared to manual scalers, showed no statistical differences in probing depth reduction, plaque index, bleeding on probing, relative attachment level gain, gingival recession. As well as, reductions of the microbial counts and colony-forming units using Friedman and Wilcoxon test, and decrease of A.a, P.g. and T.f.. There were no significant differences between regard with treatments for all parameters at each re-examination. Within the limits of this study, it can be concluded that the use the of diamond-coated sonic scaler insert can be a good alternative for root debridement, maintaining the stability of the periodontal tissues in long term

Resumo: O objetivo deste estudo foi avaliar a resposta clínica dos tecidos periodontais à descontaminação das superfícies radiculares utilizando pontas ultra-sônicas diamantadas (CVDentUS®), em dentes unirradiculares, sob os parâmetros clínicos (índice de placa - IP, sangramento à sondagem - SS, profundidade de sondagem - PS, nível de inserção clínica relativo - NICR e recessão gengival - RG), comparados à instrumentação com curetas Gracey. Trata-se de um estudo clínico controlado e randomizado, com um desenho em boca dividida, envolvendo 17 pacientes com periodontite crônica de moderada a avançada, divididos em dois grupos distintos: grupo não-cirúrgico (sítios com 3mm < PS ? 5mm) e grupo cirúrgico (sítios com PS > 5mm). Os pacientes do grupo não-cirúrgico foram submetidos a tratamento de descontaminação radicular em campo fechado, com as pontas ultra-sônicas diamantadas CVDentUS® (lado teste) e com curetas Gracey 5/6 (lado controle). Os pacientes do grupo cirúrgico foram submetidos à retalho cirúrgico e à descontaminação da superfície radicular em campo aberto, seguindo o mesmo protocolo adotado para o grupo não-cirúrgico. Os parâmetros clínicos foram avaliados no tempo inicial, 30, 60 e 90 dias após os tratamentos. Os dados obtidos foram submetidos aos testes não-paramétricos, Mann Whitney, Wilcoxon e Friedman, com nível de significância de 5%. Os resultados não mostraram diferença significante entre os tratamentos e entre os grupos, exceto para o parâmetro clínico NICR que apresentou leve vantagem para o uso das curetas no grupo não-cirúrgico. Dentro dos limites deste estudo, podemos concluir que, clinicamente, as pontas ultra-sônicas diamantadas CVDentUS® foram tão eficientes quanto às curetas na terapia periodontal cirúrgica e não-cirúrgica

The investigation was a randomized and controlled clinical trial with a splith-mouth design involving 17 patients with moderate to advanced chronic periodontitis, divides into 2 groups: : non-surgical group (5mm ? pocket depth > 3mm) and surgical group (pocket depth > 5mm). The patients underwent initial treatment sessions of supragingival plaque removal and were also instructed on oral hygiene techniques. Non-surgical group patients were treated using the diamond-coated ultrasonic scaler insert CVDentUS® in test site and Gracey 5/6 curettes in control site. In surgical group, flap surgery was carried out and the tooth surface instrumented with the same protocols. Clinical parameters were evaluated at baseline (0), 30, 60, and 90 days after treatments. The data were analysed using non-parametric tests, Mann Whitney, Wilcoxon and Friedman. In the end of the study, the results showed no significant differences between the treatments and between groups, except to relative clinical attachment level that showed a slight advantage for the treatment with curettes in non-surgical group. Within the limits of this study, it can be concluded that the diamond-coated ultrasonic scaler insert (CVDentUS®) was such efficient as them curettes for surgical and nonsurgical periodontal therapy in patients with chronic periodontal disease

This work aims at analyzing and realizing a horn-type sonochemical reactor which can be operated in a very low ultrasonic power density but results in a large volume of cavitation zones. The sonoreactor contains three main components, namely a Langevin-type piezoelectric transducer (20 kHz), a metal horn, and a circular cylindrical sonicated cell filled with tap water. In order to diminish the generation of cavitation bubbles near the horn-tip, an enlarged cone-shaped horn is designed to reduce the ultrasonic intensity at the irradiating surface and to get better distribution of energy in the sonicated cell. It is demonstrated both numerically and experimentally that the cell geometry and the horn position have prominent effects on the pressure distribution of the ultrasound in the cell. With appropriate choices of these parameters, the whole reactor works at a resonant state. Several acoustic resonance modes observed in the simulation are realized experimentally and used for generating a large volume of cavitation field.
