The assessment of men with fertility problems is described in this chapter. The main causes of male infertility are testicular insufficiency due to congenital and acquired causes, obstructions of the male genital tract, genetic and endocrine abnormalities, urogenital infections, and varicoceles. Lifestyle can also have a negative influence on semen quality: smoking, obesity, drugs, and anabolic steroids influence sperm parameters and may reduce natural conception. Some chronic diseases also have a negative influence on fertility. History taking and physical examination should focus on prevalent causes of male infertility. Many decisions on diagnosis and treatment of male infertility are based on a semen analysis. It is therefore essential that the investigation is performed according to the recommendations of the world health organization manual for semen analysis.

While spinal cord injury (SCI) does not affect female fertility, the condition most often results in infertility in males due to anejaculation and reduced semen quality. Anejaculation is caused by disruption of the autonomic nerve fibres, which are normally responsible for the ejaculation. The reason for the poor sperm quality has not been firmly established. If spinal cord injured men cannot ejaculate by sexual intercourse or masturbation, ejaculation can be induced by either penile vibratory stimulation or electroejaculation. Only if these methods fail should surgical sperm retrieval be considered. The method of insemination depends largely on the total motile sperm count and patient preference. With the right treatment, it is possible for most SCI men to have children.

This chapter argues against accounts of lying in terms of Gricean maxims. It first considers attempts to characterize lying in terms of Grice’s First Maxim of Quality, admonishing speakers to avoid saying what they believe to be false. Even though many lies are instances of covert violations of the First Maxim of Quality, the phenomenon of bald-faced lies demonstrates that some lies overtly violate the First Maxim of Quality. In light of this, one account takes lies to be violations of the First Maxim of Quality, covert or overt. This view is seen to wrongly count ironic utterances as lies. The chapter then goes on to discuss an alternative Gricean conception of lying in terms of the Supermaxim of Quality. Yet this view is seen to wrongly count false implicatures as lies. The chapter concludes that lying cannot satisfactorily be characterized in terms of Gricean maxims.

This chapter lays out the measure of peace developed by Erik Melander. As he sees it, peace concerns the conduct of politics with respect for physical integrity rights, using consensual decision making, on the basis of strong equality values. He draws on the work of Carl von Clausewitz for his insights, following Clausewitz’s trinity of dimensions of war but finding parallels in an evaluation of quality peace. To measure the concept, he employs the three dimensions to develop a map of quality peace in countries in the period from 1981 to 2008. Using data from a variety of sources, including CIRI data, his mapping leads to seven insights into the state of quality peace over the long term and around the world. The chapter concludes with discussion of further implications and considerations of the mappings’ limitations, encouraging further study.

This chapter tackles the problem of finding data-derived indicators to measure the quality of peace, versus a definition of peace simply as the absence of war. Conceptually, peace is seen as an equilibrium condition where resort to violence is minimal and where the highest quality of peace exists when the idea of armed violence approaches the unthinkable. The author draws upon the early work of Quincy Wright and Kenneth Boulding and progresses from there, establishing first their definitions of and conditions for peace. To put his theories to work, he introduces two proxy indicators: black market currency exchanges and bond market prices. Specifically, he examines and compares the premiums attached to the black market values of currencies in less stable economies and relates them to factors that promote destabilization of the equilibrium. Similarly, he compares the strip spreads on sovereign bonds as an indicator of government stability and instability.

This chapter suggests that, when considering the philosophical problem of other minds, we distinguish between "thick" and "thin" versions of it. While traditional approaches take the problem to be a thick one, more recent work can be seen as addressing only a thin variant. Dretske, while acknowledging the thick problem, proposes a perceptual model of our knowledge of other minds which addresses only the thin version. The chapter proposes that, in the place of the thick problem, we consider the quality of our interactions with others. Following Wittgenstein, it suggests that where individuals share a nature their interactions exhibit a quality that it calls "depth." Where that nature is not, or is only partially, shared, there one might expect to find the quality of the interaction between persons disturbed. The chapter suggests that this disturbance might explain the impaired quality of interaction between autistic and non-autistic individuals.

This chapter outlines some of the context and concerns. It goes on to reviews the range of psychosocial interventions available to improve quality of life and well-being of residents and those to treat mental health problems and behaviour seen as challenging. It provides a framework to consider the complex interaction between physical, mental and cognitive health which is required when selecting appropriate treatments. The most common interventions are reviewed and the evidence to support their use outlined, providing an indication of the circumstances in which they may be most helpful.

Some of the earliest clinical trials were conducted in infectious diseases. In the 1940s, the development of the first antibiotics for treating tuberculosis coincided with the recognition that rigorous clinical trials were required to determine optimum drug combinations and duration of treatment. The joint efforts of bacteriologists, clinicians, and statisticians promoted the development of clinical trials, acknowledging that clinically valid endpoints and careful statistical analysis are vital for trials to provide evidence of sufficient quality to guide clinical practice. This chapter covers key questions in this field addressed by good-quality trials. It covers clinical evidence important to practitioners both overseas and in the UK. It focuses on trials that have generated key data, while also covering trials which address clinical problems that are important worldwide and less commonly seen in the UK where critical evaluation of current trials might be difficult.

Qualitative consciousness is conscious experience marked by the presence of sensory qualities, like the experienced painfulness of having a piano dropped on your foot, or the consciousness of seeing the brilliant reds and oranges of a sunset. Over his career, philosopher David Rosenthal has defended an influential theoretical approach to explaining qualitative consciousness. This approach involves the development of two theories – the higher-order thought theory of mental state consciousness and the quality space theory of sensory quality. If the problem of explaining qualitative consciousness is divided into two more manageable pieces, the door opens to a satisfying explanation of what is seen by some to be an intractable explanatory puzzle. This interdisciplinary collection develops, criticizes, and expands upon themes inspired by Rosenthal's work. The result is an exciting collection of new essays by philosophers and scientists, which will be of interest to all those engaged in consciousness studies.

Aggression and violence are often overlooked in the aging population. Changes in personality, behavior, and mood are well known in psychiatric disorders, but they are also seen in neurologic conditions, often where there is alteration in brain chemistry, as in metabolic disorders; or alteration in brain structure, as in degenerative disorders, strokes, subdural hematomas, and tumors; or with alteration in brain function, as in epileptic and nonconvulsive status. This chapter provides an overview of specific behaviors, the underlying neurochemistry and pathophysiology behind specific disorders, and some treatment approaches. The consequences of unrecognized and untreated behavioral changes in the elderly can have dire consequences on quality of life—for example, leading to early institutionalization. Regular follow-up that includes questions about behavior would allow for early work-up and treatment to prevent out-of-control escalation. Family education, support, caregiver relief, and prevention of complications can go a long way in improving the quality of life for patients and their families.

Education in the Global South faces several key interrelated challenges, for which Open Educational Resources (OER) are seen to be part of the solution. The Research on Open Educational Resources for Development (ROER4D) project seeks to build on and contribute to the body of research on how OER can help to improve access, enhance quality and reduce the cost of education in the Global South. This volume examines aspects of educator and student adoption of OER and engagement in Open Educational Practices (OEP) in secondary and tertiary education as well as teacher professional development in 21 countries in South America, Sub-Saharan Africa and South and Southeast Asia. Many haematological issues can complicate end-of-life care, including cytopenias and venous thromboembolism (VTE). Anaemia is very common and can significantly impact quality of life; causes include haemorrhage, iron deficiency, nutritional deficiencies, and bone marrow infiltration. Neutropenia from bone marrow failure as a result of disease infiltration or from chemotherapy effects can result in life-threatening infections. Finally, VTE is commonly seen in cancer patients as well as those who require prolonged hospitalization. Symptoms can cause discomfort, mortality is increased, and treatment is associated with major bleeding. Understanding the therapeutic options and their adverse side effects is essential in the management of these complex problems. Despite the presence of effective therapies, it is also important to realize that events such as febrile neutropenia and pulmonary embolism are often seen at the end of life and intervention may not always impact prognosis. The risks of intervention should be weighed against expected benefits when developing appropriate palliative care plans.

The data is collected for 311 research institutes including autonomous institutes, government institutes, and research and advocacy groups. The data shows that the average size of faculty in 148 autonomous institutes was seven. It may not be too farfetched to say that such tiny faculty strength will have direct impact on the volume as well as quality of research output from these institutes. ‘Economics and allied subjects’ leads the research in autonomous institutes. In the case of government-run institutes, the research component is weak as their focus is mainly on training and capacity enhancement of government and another staff. Research and advocacy groups have focused primarily on advocacy.

Faecal incontinence, the uncontrolled loss of faecal material, is a surprisingly common condition with many epidemiological and aetiological similarities to that of urinary incontinence. The condition carries many challenges for the clinician, from obtaining a history from patients often too embarrassed to fully recount their symptoms, through to the difficulty in ascertaining whether a treatment has been a success or not. A wide range of pathological conditions cause the symptoms of faecal incontinence, yet in clinical practice the condition is generally seen in women following childbirth. Assessing the individual patient for a successful outcome includes ascertaining not only a reduction in incontinent episodes, but also patient satisfaction and an improved quality of life.

Zinc is an essential mineral involved in gene expression, cell growth and division, neurotransmission, and reproductive and immune functions. It is crucial for periods of growth, including pregnancy and lactation, infancy, childhood, and adolescence. Daily requirements in pregnancy increase by ~40, so adequate zinc stores should be ensured prior to pregnancy so that the high requirements of the developing fetus can be met. Inadequate zinc intake often accompanies general protein/calorie malnourishment but is also seen in individuals consuming poor quality diets. Diets low in animal protein and/or high in phytates (e.g. in whole grains) provide limited amounts of bioavailable zinc. Prenatal vitamins containing zinc are advisable with such diets. Zinc is required for male fertility, and supplementation may improve fertility in subfertile men.

As we have seen, numerous analysis and modeling tools that take into account the natural complexity of systems and dissemination and implementation processes are available, and the use of them is increasing over time. This chapter summarizes the characteristics, potential insights, and limitations of each modeling approach. It is important to note that modeling from a systems perspective, like all modeling approaches, requires assumptions about variables to include (or exclude), and hypothesized relationships dictate the quality of the model and the utility of the results. Screening for a disease must be clearly defined and differentiated from early detection. ‘Screening’ refers to the application of tests to the whole population or to defined segments such as males within certain defined high risk age groups. If applied in such a fashion ‘screening’ for prostate cancer may also be described epidemiologically as ‘secondary prevention’. While high-quality randomized studies show that screening reduces prostate cancer mortality by 21–44%, there is wide agreement that the introduction of population-based screening is at present premature because harms, mainly the high rate of overdiagnosis seen currently outweighs the benefits. This chapter attempts to put current knowledge into perspective with a set of recognized prerequirements for the application of screening, established by Wilson and Jungner in 1968.

Though often unrecognized, sleep disorders in MS are seen at higher frequency than the general population, and they may contribute to pain, fatigue and depression—symptoms commonly observed in MS patients. Since several immunological factors in serum have been implicated in the development of sleep disorders, and MS is proven to be characterized by immune abnormalities, the notion that MS and sleep disorders share a similar background seems reasonable. Investigation of sleep disorders in MS is important, especially considering that the treatment of sleep disturbance may contribute to a reduction in debilitating symptoms, such as fatigue. Thus, an increased clinical awareness and appropriate treatment of sleep disorders in the MS population may significantly improve the overall quality of life in these patients.

The older population is challenging to treat for numerous reasons, including comorbid conditions and increased susceptibility to adverse drug reactions, limiting medical therapy. They are at increased risk for loneliness and depression, which strongly impacts their cardiovascular outcomes, and they also have different values, usually prioritizing quality of life over mortality objectives. Finally, the elderly are underrepresented in cardiovascular clinical trials, thus limiting the applicability of guideline recommendations. This chapter emphasizes the importance of a comprehensive assessment of individual circumstances when assessing cardiovascular health in the elderly population. The chapter focuses on the role of nutrition, resiliency, and exercise for the prevention and treatment of cardiovascular disease. Nutrient deficiencies commonly seen with cardiovascular drugs are also discussed, as well as specific integrative strategies for optimizing dyslipidemia, atrial fibrillation, and heart failure in this population.

How does the English School work as part of Empirical International Relations (IR) theory? The English School depends heavily on historical accounts, and this article makes the case that history and theory should be seen as co-constitutive rather than as separate enterprises. Empirical IR theorists need to think about their own relationship to this question and clarify what “historical sensitivity” means to them. The English School offers both distinctive taxonomies for understanding the structure of international society, and an empirically constructed historical approach to identifying the primary institutions that define international society. If Empirical IR is open to historical-interpretive accounts, then its links to the English School are in part strong, because English School structural accounts would qualify; they are, in other ways, weak because the normative theory part of the English School would not qualify. Lying behind this judgement is a deeper issue: if Empirical IR theory confines itself to regularity-deterministic causal accounts, then there can be no links to English School work. Undertaking English School insights will help open up a wider view of Empirical IR theory.

Seven of the top 20 disorders affecting morbidity are mental illnesses: major depressive disorders, drug use disorders, anxiety disorders, alcohol use disorders, schizophrenia, bipolar disorder, and dysthymia. Behavioral health disorders are important determinants of work role disability and quality of life. Behavioral health disorders also have generally stronger “cross-domain” effects, exacerbating the diagnosis and treatment of many physical disorders and chronic medical conditions. In addition to the persistent stigma surrounding behavioral health disorders, there are issues of reimbursement, costs of care, availability of care based on provider and/or geographic availability, policy limitations, formulary restrictions, transportation, provider knowledge, patient knowledge, attitudinal/evaluative barriers, medication compliance, family support (or lack of it), inconvenience or inability to obtain an appointment, and refusal of treatment. This chapter provides an overview of the issues surrounding behavioral health care from a population health perspective, including pharmaceutical treatment and competencies for community-based pharmacists.

Atlas of Leaf Venation and Oil Gland Patterns in the Eucalypts is an aid to the identification of eucalypts in the field and a confirmation of the natural affinities between species and higher-level taxa on the basis of their comparative morphology. Its purpose is to standardise leaf venation and oil gland terminology and to demonstrate the taxonomic value of leaf venation and oil gland patterns within the eucalypts.
The work discusses the visible features of the adult leaves of eucalypts as seen with reflected and transmitted light. Because venation and oil glands become obscure in dried specimens this work relies entirely on the comprehensive sampling and observation of fresh leaves.
High quality, scaled, leaf venation images of vouchered specimens are used to compare all taxonomic groups in the eucalypts. All genera, sections, series and subseries are represented.

This chapter describes the history and organization of palliative care volunteering in The Netherlands, volunteer roles in different settings, legislative and political influences on palliative care volunteering. Today 200 member organizations are part of the umbrella organization VPTZ Nederland. Together, these member organizations run 87 almost-like-home-houses where medical staff are not employed by the hospice, but visit and care for the patient and family when needed, just as in the home situation, in 18 high-care hospices (with medical staff) and nine palliative care units (units in nursing homes that also have significant involvement of volunteers) and 135 volunteer home care teams. In 2015 10,893 volunteers were working in these organizations. In addition special attention is given to the concept of ‘being there’, which is seen as key to the identity, value and quality of hospice and palliative care volunteering.

Psoriasis is seen in many patients with psoriatic arthritis. It forms part of the CASPAR criteria for establishing the diagnosis of psoriatic arthritis (PsA). The psoriasis accompanying PsA may be mild or quite severe, with a very large impact on quality of life. The presence of cutaneous disease may alter the treatment of a patient’s PsA. Psoriasis affects up to 2–3% of the population of Europe, the United States and Canada. The majority of patients have chronic plaque psoriasis (80%). Other clinical phenotypes include guttate psoriasis, erythrodermic psoriasis, inverse psoriasis, sebopsoriasis and pustular subtypes including generalized pustular psoriasis. Nail involvement is a frequent occurrence and indicate an increased risk of developing psoriatic arthritis. In this chapter, the different psoriatic phenotypes are described as well as the histology and, briefly, the pathophysiology of psoriasis. Environmental factors that can impact psoriasis are reviewed.

This chapter examines the range of F. R. Leavis’s historical thinking, from the early work of his Ph.D. through to his post-1945 dealings with the nineteenth century. The chapter argues that, rather than being largely derived from Eliot’s work, Leavis’s thinking drew on older and more conventional strains of historical writing whose framework he never entirely shook off. It emphasizes the extent to which Leavis believed that a proper cultural history would yield an evaluative assessment of the quality of human living in various periods and hence an overall judgement about progress or decline. It shows that the two key periods in Leavis’s scheme were the seventeenth century, understood as the beginnings of the modern world, and the nineteenth century, seen as the triumph of a broadly mechanical and economistic world view that he tended to equate with ‘Utilitarianism’.

This chapter begins with the observation that a number of literary critics in this period express the hope that a new form of ‘cultural history’ would provide the basis for an evaluative assessment of the direction of social change. They look to those trained as critics, not to historians, for such an approach, one that tries to identify the ‘quality of living’ in evidence in various periods. The chapter shows how such work played an important part in a wider public’s understanding of history in the years between the 1940s and the 1970s, with the Pelican Guide to English Literature providing a notable illustration. The chapter suggests that the reception of Williams’s unconventional book, The Long Revolution, in 1961 can be seen to mark the end of the style of work considered in the previous chapters.

For the past ten years, the prevalence of suicide completions among service members has increased, with rates greatly exceeding those seen within the US civilian population. In response, the Department of Defense, Department of Veterans Affairs, and each branch of the military collaboratively implemented strategic approaches to improving existing suicide prevention programs and invested in the development of innovative programs. These approaches include consolidating suicide event reporting into a central database, reducing the stigma associated with help-seeking behaviors, enhancing resilience among service members, improving aspects impacting service members’ overall quality of life through an expansion of benefits and services, and improving the physical, psychological, social, and spiritual well-being of service members. This article explores the evolution of these suicide risk prevention efforts in consideration of the effectiveness of different strategies used by military branches and veteran programs.

The past decade or so of media promotion has seen a recurring trope of Fanboy to the Rescue—have no fear, it says, this revered franchise is being taken over by a writer, director, or producer who is a “fanboy.” In this trope—which, following Suzanne Scott, the book discusses in terms of the “fanboy auteur”—the fan credentials of writers, directors, and producers are presented as a guarantee of quality media-making. This is a strategy of marketing and branding; it is a claim, from the auteur himself or industry PR machines, that the presence of an auteur who is also a fan means the product is worth consuming. Such claims that fan credentials guarantee quality are often contested, with fans and critics alike rejecting various auteur figures as the true leader of their respective franchises. That split, between assertions of fan and auteur status and acceptance (or not) of that status, is key to unravelling the fan auteur. A Portrait of the Auteur as Fanboy examines the contemporary ascendance of the fan auteur through a series of case studies. We consider both thoroughly mainstream fan auteur figures like Zack Snyder and Joss Whedon and more offbeat ones like Kevin Smith. We examine those who explicitly identify as fans of the source material they engage, like Steven Moffat, E L James, and Patty Jenkins, and those who don’t but engage in fannish ways nonetheless, like Taika Waititi and J. K. Rowling.
