This study examines the interdependence of gender, sexuality and space in the early modern period, which saw the inception of architecture as a discipline and gave rise to the first custodial institutions for women, including convents for reformed prostitutes. Meanwhile, conduct manuals established prescriptive mandates for female use of space, concentrating especially on the liminal spaces of the home. This work traces literary prostitution in the Spanish Mediterranean through the sixteenth and seventeenth centuries, from the rise of courtesan culture in several key areas through the shift from tolerance of prostitution toward repression. Kuffner’s analysis pairs canonical and noncanonical works of fiction with didactic writing, architectural treatises, and legal mandates, tying the literary practice of prostitution to increasing control over female sexuality during the Counter Reformation. By tracing erotic negotiations in the female picaresque novel from its origins through later manifestations, she demonstrates that even as societal attitudes towards prostitution shifted dramatically, a countervailing tendency to view prostitution as an essential part of the social fabric undergirds many representations of literary prostitutes. Kuffner’s analysis reveals that the semblance of domestic enclosure figures as a primary erotic strategy in female picaresque fiction, allowing readers to assess the variety of strategies used by authors to comment on the relationship between unruly female sexuality and social order.

Expected utility is introduced. Risk aversion and its equivalence with concavity of the utility function (Jensen’s inequality) are explained. The concepts of relative risk aversion, absolute risk aversion, and risk tolerance are introduced. Certainty equivalents are defined. Expected utility is shown to imply second‐order risk aversion. Linear risk tolerance (hyperbolic absolute risk aversion), cautiousness parameters, constant relative risk aversion, and constant absolute risk aversion are described. Decreasing absolute risk aversion is shown to imply a preference for positive skewness. Preferences for kurtosis are discussed. Conditional expectations are introduced, and the law of iterated expectations is explained. Risk averse investors are shown to dislike mean‐independent noise.

This chapter discusses legal and conduct risk associated with the globalized financial market. It argues that the involvement of banks in both globalization and the financial crisis has increased the risk of banks being a target for criticism for the social ills and misfortunes associated with either of these phenomena. Such criticism often leads to lawsuits, political action, or adverse regulatory measures; and sometimes a combination of these things. The remainder of the chapter covers the globalization debate and context; how legal risk is affected by, and has sources in, current and enduring political and social objectives; the media’s influence on to the culture of fear and ‘zero risk tolerance’; the limits of global law; reactions to globalization; and the need for legal certainty.

Perioperative pain management continues to be a challenge for physicians. Postoperative pain can compromise patient progress and lead to poor outcomes or chronic pain. Opioid medications, the mainstay of treatment for perioperative pain, can cause opioid-induced hyperalgesia and opioid tolerance. Attempts should be made to modify factors that increase the risk for chronic postsurgical pain. Certain patient factors and anesthetic and surgical techniques have been implicated. Incorporating multimodal methods for perioperative pain management using nonconventional opioids, such as methadone, cyclooxygenase inhibitors, NMDA antagonists, and regional techniques can improve outcomes and prevent opioid-induced hyperalgesia, opioid tolerance, and chronic postsurgical pain in patients on long-term opioid therapy.

The portfolio choice model is introduced, and the first‐order condition is derived. Properties of the demand for a single risky asset are derived from second‐order risk aversion and decreasing absolute risk aversion. Optimal investments are independent of initial wealth for investors with constant absolute risk aversion. Optimal investments are affine functions of initial wealth for investors iwth linear risk tolerance. The optimal portfolio for an investor with constant absolute risk aversion is derived when asset returns are normally distributed. Investors with quadratic utility have mean‐variance preferences, and investors have mean‐variance preferences when returns are elliptically distributed.

Pareto optima and competitive equilibria are defined. Allocations are functions of market wealth (sharing rules) in Pareto optima, which means that all risks except market wealth are perfectly shared. Equilibria in complete markets are shown to be equivalent to Arrow‐Debreu equilibria and to be Pareto optimal. If investors all have linear risk tolerance with the same cautiousness parameter, then equilibria are Pareto optimal, equilibrium prices are independent of the initial wealth allocation (Gorman aggregation), and two‐fund separation holds (all investors hold the risk‐free asset and the market portfolio).
