This book is an effort to try to capture the broad outlines of the significant economic, market, social, and demographic factors that will shape the future of China and the role that aging will play in the whole mix of influences. Aging in developed societies and economies has been widely studied. In such nations as Japan, Germany, Italy, and Spain, for example, we know that as populations age, their societies decline, leaving fewer younger workers to support the growing number of people who will become dependent on costly health care systems, or whose basic needs, such as food and shelter, will need to be subsidized. But less work has been done in assessing the potential impact of aging in developing countries, where the majority of people may be working poor, not middle class—as in the case of China. As China restructures its economy, the old benefits packages previously available to urban workers (and not rural workers) are being replaced by a patchwork of benefits across a wide range of enterprises.

Paid home care sits at the nexus of two of the United States’ biggest social challenges: rising inequality and an aging population. Policy and advocacy initiatives typically treat poverty and care of the aged as distinct forms of vulnerability. They are seen as having separate social causes that require different solutions. Using rich ethnographic narrative based on fieldwork in Chicago, this book examines the diverse relationships generated by care and their connections to longer national histories, policies, and institutional contexts. The vulnerabilities of older adults and care workers are commingled: low wages and poor working conditions render workers’ lives precarious. In turn, high turnover rates and endemic worker shortages translate into wait lists and lower quality care for older adults. In home care, the fate of older adults and the working poor are bound together, entangled by the broader indifference of a society that devalues both aging and care.

The number of individuals with intellectual disability and/or autism spectrum disorder who are 65 or older is growing because of increased longevity and a proportionally larger number of individuals being diagnosed across the lifespan. Many of these individuals live with caregivers who are aging and thus eventually need out-of-home placement. Individuals with Down syndrome may also need out-of-home placement as they have an exceedingly high risk of neurocognitive disorder with age. This chapter provides the epidemiology of older adults with intellectual disability and/or autism spectrum disorder and discusses the changes in criteria for these in DSM-5. The chapter also discusses the challenges these individuals and the forensic psychiatrist may face in the judicial system. Individuals may not understand their rights and may be prone to confess or be coerced. They may also lack of full comprehension, making it difficult for them to assist in their defense and describe details of offense. The overlap of geriatric psychiatry and intellectual disability and/or autism spectrum disorder and the law is an area ripe for further research.

A critical variable determining the future relationship between China and the United States is the change in the relative balance of power, which depends on the sustainability of China’s economic growth. While China has produced three decades of double-digit growth, this paper argues that China’s rise has peaked. The factors that have contributed to China’s rapid growth, such as efficiency gains produced by market-oriented reforms, practically unlimited access to global markets, and the demographic dividend, are either disappearing or dissipating. Simultaneously, obstacles to future growth, such as systemic corruption, environmental degradation, and demographic ageing, are becoming more salient. Economic slowdown will threaten the survival strategy of the ruling Chinese Communist Party (CCP). Since the 1989 Tiananmen crackdown, the CCP has followed a sophisticated strategy to maintain power. An economic slowdown will likely cause an unravelling of this strategy, which depends on revenues generated by growth for its sustainability.

This book is a discussion of philosophical, legal, and medical issues related to aging, dying, and death. It considers different views about whether and why death is bad for the person who dies, and whether these views bear on why it would be bad if there were no more persons at all. The book looks at how the general public is being asked to think about end-of-life issues by examining some questionnaires and conversation guides that have been developed. It also considers views about the process of dying and whether it might make sense to not resist death, or even to bring about the end of one’s life, given certain views about meaning in life and what things it is worth living on to get and do. Some hold that it is not only serious illness but ordinary aging that may give rise to some of these questions and the book considers various ways in which aging and the distribution of goods and bads in a life could occur. Physician-assisted suicide would be one way to end one’s life and the book examines arguments about its moral permissibility and whether or not it should be legalized as a matter of public policy. This discussion draws on capital punishment debates concerning state action and also on methods of balancing costs and benefits. The book examines the views of such prominent philosophers, doctors, and legal theorists as Shelly Kagan, Susan Wolf, Atul Gawande, Ezekiel Emanuel, Cass Sunstein, Neil Gorsuch, and others.

Latin America and the Caribbean, unlike other regions, is still quite young demographically: people over age 60 make up around 11% of the total population. However, the region is expected to experience the fastest rate of population aging in the world over the coming decades. This projected growth of the elderly population raises challenges related to pensions, health, and long-term care. At the same time, it opens up numerous business opportunities in different sectorshousing, tourism, care, and transportation, for examplethat could generate millions of new jobs. These opportunities are termed the “silver economy,” which has the potential to be one of the drivers of post-pandemic economic recovery. Importantly, women play key roles in many areas of this market, as noted in the first report published by the IDB on this subject (Okumura et al., 2020). This report maps the actors whose products or services are intended for older people and examines silver economy trends in the region by sector: health, long-term care, finance, housing, transportation, job market, education, entertainment, and digitization. The mapping identified 245 actors whose products or services are intended for older people, and it yielded three main findings. The first is that the majority of the actors (40%) operate in the health and care sectors. The prevalence of these sectors could be due to the fact that they are made up of many small players, and it could also suggest a still limited role of older people in active consumption, investment, and the job market in the region. The second finding is that 90% of the silver economy actors identified by the study operate exclusively in their countries of origin, and that Mexico has the most actors (47), followed by the Southern Cone countriesBrazil, Chile, and Argentinawhich have the regions highest rates of population aging. The third finding is that private investment dominates the silver economy ecosystem, as nearly 3 out of every 4 actors offering services to the elderly population are for-profit enterprises. The sectors and markets of the silver economy differ in size and degree of maturity. For example, the long-term care sector, which includes residential care settings, is the oldest and has the largest number of actors, while sectors like digital, home automation, and cohousing are still emerging. Across all sectors, however, there are innovative initiatives that hold great potential for growth. This report examines the main development trends of the silver economy in the region and presents examples of initiatives that are already underway. The health sector has a wealth of initiatives designed to make managing chronic diseases easier and to prevent and reduce the impact of functional limitations through practices that encourage active aging. In the area of long term careone of the most powerful drivers of job creationinitiatives to train human resources and offer home care services are flourishing. The financial sector is beginning to meet a wide range of demands from older people by offering unique services such as remittances or property management, in addition to more traditional pensions, savings, and investment services. The housing sector is adapting rapidly to the changes resulting from population aging. This shift can be seen, for example, in developments in the area of cohousing or collaborative housing, and in the rise of smart homes, which are emerging as potential solutions. In the area of transportation, specific solutions are being developed to meet the unique mobility needs of older people, whose economic and social participation is on the rise. The job market offers older people opportunities to continue contributing to society, either by sharing their experience or by earning income. The education sector is developing solutions that promote active aging and the ongoing participation of older people in the regions economic and social life. Entertainment services for older people are expanding, with the emergence of multiple online services. Lastly, digitization is a cross-cutting and fundamental challenge for the silver economy, and various initiatives in the region that directly address this issue were identified. Additionally, in several sectors we identified actors with a clear focus on gender, and these primarily provide support to women. Of a total of 245 actors identified by the mapping exercise, we take a closer look at 11 different stories of the development of the silver economy in the region. The featured organizations are RAFAM Internacional (Argentina), TeleDx (Chile), Bonanza Asistencia (Costa Rica), NudaProp (Uruguay), Contraticos (Costa Rica), Maturi (Brazil), Someone Somewhere (Mexico), CONAPE (Dominican Republic), Fundación Saldarriaga Concha (Colombia), Plan Ibirapitá (Uruguay), and Canitas (Mexico). These organizations were chosen based on criteria such as how innovative their business models are, the current size and growth potential of their initiatives, and their impact on society. This study is a first step towards mapping the silver economy in Latin America and the Caribbean, and the hope is to broaden the scope of this mapping exercise through future research and through the creation of a community of actors to promote the regional integration of initiatives in this field.

This chapter introduces the concept of the “archaeomodern” and its connection to the aging of the quintessential modern medium of film. It sketches the historical and cultural background of the archaeomodern turn in the late twentieth century, including the development of an obsession with the past in the heritage industry and the rise of postmodernism. It then discusses two phenomena from the 1980s and 1990s—a mannerist or baroque revival, and the development of media archaeology—that complicate the habitual association between tradition and the past or modernity and the future. The introduction suggests that archaeomodern cinema was characterized by the return to failed or abandoned modern experiments and other relics from the modern past.

Notwithstanding the terrible price the world has paid in the Coronavirus pandemic, the fact remains that longevity at older ages is likely to continue to rise in the medium and longer term. This volume explores how the private and public sectors can collaborate via public-private partnerships (PPPs) to develop new mechanisms to reduce older people’s risk of outliving their assets in later life. As we show in this volume, PPPs typically involve shared government financing alongside private-sector partner expertise, management responsibility, and accountability. In addition to offering empirical evidence on examples where this is working well, our contributors provide case studies, discuss survey results, and examine a variety of different financial and insurance products to better meet the needs of the aging population. The volume will be informative to researchers, plan sponsors, students, and policymakers seeking to enhance retirement plan offerings.

Home-based primary care is on the rise for several reasons: an aging population for which office-based care is less convenient, favorable changes in Medicare fee-for-service reimbursement, and a greater recognition that the care of sick homebound adults may be better done in their home. Home care practices can care for homebound patients with multiple comorbidities more efficiently, with lower costs and equivalent or superior outcomes. Home care is invaluable for individuals who may not be able to travel easily to a clinician’s office. Integrative Medicine may provide helpful treatment modalities for individuals being cared for at home—in particular, complementary treatments for common conditions such as chronic pain, falls, depression, anxiety, and insomnia. The home has also become an important educational setting in which learners can readily acquire the knowledge, skills, and attitudes that are essential in providing optimal care for complex, frail patients.

Cognitive impairment can thus infiltrate all aspects of healthcare, making it necessary for clinicians and clinical researchers to have an integrated knowledge of the spectrum of adult cognitive disorders. The Oxford Handbook of Adult Cognitive Disorders is meant to serve as an up-to-date, scholarly, and comprehensive volume covering most diseases, conditions, and injuries resulting in impairments in cognitive function in adults. Topics covered include normal cognitive and brain aging, the impact of medical disorders (e.g., cardiovascular, liver, pulmonary) and psychiatric illnesses (e.g., depression and bipolar disorder) on cognitive function, adult neurodevelopmental disorders (e.g., Down Syndrome, Attention Deficit/Hyperactivity Disorder), as well as the various neurological conditions (e.g., Alzheimer’s disease, chronic traumatic encephalopathy, concussion). While trust and trustworthiness provide a fundamental foundation for human relationships, little is known about how trusting and trustworthy behavior in social dilemmas is related to age and aging. A few papers use data from surveys such as the World Values Survey to address a potential connection between trust and age. This chapter mainly focuses on trusting and trustworthy behavior elicited with the use of the seminal trust game and with games implementing a similar incentivized interaction structure. The results suggest that trust and trustworthiness increase with young age until adolescence. Trustworthiness reaches the level of adults at an earlier age (at around 15-16 years of age) than trusting behavior (around adulthood). Survey results differ from incentivized experiments when it comes to a potential development of trust in adulthood. The former indicate a steady rise in trust levels at a small rate when becoming older, whereas the latter show a decline, starting at an age of about 60 years.
