A questionnaire survey of a stratified random sample (n=180) of registered nurses employed at two hospitals in the Australian Capital Territory was conducted to identify factors contributing to their job satisfaction and measure levels of satisfaction of registered nurses Grade 1, Grade 2 and Grade 3. The response rate was 75%. The survey found that registered nurses were satisfied with the scheduling, opportunities to utilize skills, working conditions, working relationships, leadership, decision making on patient care and intrinsic components of Job satisfaction and dissatisfied with the salary component. Registered nurses Grade 2 were more satisfied than the Grade 3 group and registered nurses Grade 1 were least satisfied (P=< .05).

Spatial ability is the skill associated with mental relations among objects, the process of maintaining the physical aspects of an object after mentally rotating it in space. Many studies report a strong association of spatial ability with success in various areas of health care, especially surgery, radiology and dentistry. To date, similar investigations in professional nursing could not be located. Registered nurses, employed in an acute care multi-hospital setting, were surveyed using the Shipley-2Block Pattern Test, the Group Embedded Figures Test, and a newly created test of general nursing knowledge. The sample size of 123 nurses was composed of 31 male nurses and 92 female nurses. Data was collected between May and August of 2013 and analyzed using R, version 2.15.2. The present study did not demonstrate a statistically significant effect for gender differences on two measures of spatial ability. However, Cohen’s d effect sizes for mean gender differences in the present study are consistent with prior studies. This may suggest the nursing profession is comparable with other professions where males perform higher than females on spatial ability. The present study should be considered an initial step toward evaluating the relevance of spatial ability in the performance of nursing care.

The purpose of the study was to 'examine the idea that the processes of normal grieving can be impaired or complicated by virtue of an individual being a nurse or midwife'. A mixed methodology was utilised to allow data to be collected at both exploratory and descriptive levels, and to provide reinforcement via the triangulation of data relating to key concepts, derived from a variety of methods. At the exploratory level, the study examined the incidence of factors said to predispose towards complicated grief (Murray-Parkes 1972), within a population of nurses and midwives. This alongside consideration of factors within the socialising environment of hospital nurse/midwifery, which might engender or reinforce certain personality traits. The study also investigated the respondents' views on their role within their family. Interviews with bereaved nurses and midwives, then explored the implications of these traits and norms of behaviour, for those who had actually experienced the death of a loved one. This provided data at the level of description, as relationships between identified concepts were examined.In the event, the data suggested that certain predisposing factors to complicated grief: a 'coper' self-concept; a strong sense of personal independence from others; and a perceived lack of functional social support, both at home and at work, were prevalent within the group studied. They also indicated a range of difficulties which may arise when nurses and midwives become consumers of the services they usually provide, and the related problem(s) of being the 'family nurse'. Interviews with bereaved nurses and midwives, then identified how one or more of these issues can impact upon the experience of family hospitalisation, and ensuing bereavement, to the detriment of individual nurses and midwives - particularly by predisposing them to delayed onset of their grief, perhaps for months or even years.

The changing context of mental health care provision in the United Kingdom in the last three decades has seen significant change in acute inpatient services. Service users have expressed dissatisfaction about current service provision and care. Nurses continue to represent the largest professional group providing care in these services. Comparatively little is known from nurses' perspectives about the present nature of the nurse-patient relationship in acute mental health inpatient settings. The purpose of this research study was to explore with a sample of 14 registered mental nurses, their experiences about the nature of the nurse-patient relationship. An interpretive, phenomenological approach was adopted. The principal research question asked 'What are registered mental nurses' experiences of the nature of the nurse-patient relationship in working age acute inpatient mental health care settings?' Following ethical approval, unstructured, individual interviews were used as the primary method of data collection. Transcribed interview texts were analysed hermeneutically, supplemented with the researcher's field notes and reflective journal. Key findings included the impact of poor ward environments, increased patients' acuity of illness, and multiple, competing demands placed on nursing time. Although nurses valued human interpersonal relationships with patients, they struggled to achieve their aspirations for the nurse-patient relationship, owing to a range of factors including organisational constraints, inexperience, and limited post-registration professional development. Nurses could experience stress and emotional fatigue, and their needs for support were not always met. These factors may challenge possibilities for the development of therapeutic nurse-patient relationships. Whilst nurses appear to acknowledge the benefits of therapeutic nurse-patient relationships, they may be insufficiently prepared to achieve their ideals, given the challenges of working within an area of practice which provides for short term admissions focussed on acute risk management and containment.

Employee voluntary turnover is a management problem that impacts the strategic sustainability goals of health care organizations. Thirty-three percent of newly qualified graduate registered nurses (RNs) in the United States leave their profession within the first year of employment. As a result, health care organizations pay over $85,000 per nurse and up to $6.4 million annually to replace RNs in hospitals with more than 600 beds. The purpose of this single case study was to explore strategies nurse managers used to reduce voluntary turnover of RNs. The study population comprised 5 nurse managers from a hospital in Texas, United States. The conceptual framework included Herzberg's 2-factor theory and Maslow's hierarchy of needs. The data collection process included semistructured, face-to-face interviews and review of organizational documents. The data analysis process included thematic and matrix coding queries and keyword clustering. Yin's 5-step process of data analysis was used to identify 3 themes: job satisfaction, leadership support, and training and development. The implications of this study for social change include improvements in the quality of patient care and increased employment opportunities in health care organizations. An increase in economic growth may result in the stabilization of health care organizations' competitive compensation and opportunities to reinvest in the communities' educational programs, health, and wellness.

The purpose of this study is to describe newly graduated nurses’ self-reported perception of competence at the commencement of their first job as a registered nurse. Participants were new graduate nurses starting their first job in nursing in the acute care setting. Data was collected using the Nurse Professional Competence scale (Nilsson et al., 2014) between November 2016 and June 2017 in one hospital network. The tool includes eight areas of competence which are divided into two themes. Theme one is patient related nursing and theme two is nursing care organization and development. The survey was administered in a paper and pencil format at the start of the nursing orientation program. Demographic data collected included gender, age, months since licensed, highest nursing degree and prior work experience in health care. Findings revealed that nurses perceived themselves as most competent in the areas of value-based nursing care, teaching/learning support, and legislation in nursing and safety planning and least competent in the area of education and supervision of staff and students. Theme one competencies’ scores were higher than theme two competencies. There was a statistically significant finding among nurses who graduated with an accelerated BSN degree as compared to those with an ADN degree in the competence areas of medical technical care (p=0.040) and teaching learning support (p=.007). In addition, those nurses who had prior pre-licensure experience in healthcare related clerical roles had a statistically significant (p=.040) difference in the leadership and development in nursing competence area as compared to those with experience in other health care related jobs. The results of the study provide guidance to academia and practice in identifying new graduates’ areas of strengths and weaknesses to ensure safe entry-level nursing practice.

Over 4 million avoidable hospital admissions result from medication errors (IMS Insitute for Healthcare Informatics, 2013). Human error accounts for 80% of all medical errors (Palmieri, DeLucia, Peterson, Ott, & Green, 2008). Medication administration is a complex process. It is important to understand the cognitive load (CL) of Registered Nurses (RNs) working in an electronic health record environment to identify the risk factors of medication errors. The purpose of this study is to investigate the factors that influence the CL of RNs during medication administration who are working in an electronic health record environment. Simulated medication administration scenarios with varying degrees of multi-tasking were completed with 30 participants. When RNs multi-task during medication administration their CL increases. Furthermore, RNs who have poor sleep quality cannot process high-level tasks as well as those RNs who report a good sleep quality. Future work can limit EEG lead placement to the frontal channels of the EEG. Furthermore, replication of this study with a larger sample and a broader range of competing tasks is indicated.

This study examines the perceived causes of occupational stress amongst female Registered Nurses working in the clinical setting at both Calvary Public and Calvary Private Hospitals ACT Incorporated. A modified Gray-Toft, Anderson Nursing Stress Scale (1981) was used as the instrument for the research. The questionnaire was modified by the addition of six questions. The questionnaire was distributed to seventy female Registered Nurses. Twenty questionnaires were distributed to staff working within the Private Hospital and fifty to staff in the Public Hospital. Relevant data dating back to 1956 was examined regarding stress and burnout amongst nurses and allied health professionals. The results of this study, not surprisingly, supported the finding of Gray-Toft, Anderson. That is that nursing remains an inherently stressful occupation. Respondents working in both public and private hospitals indicated that Lack of Funding and Death and Dying were considered to be major sources of perceived stress. Registered Nurses working in the Public Hospital perceive higher levels of stress than do those Registered Nurses working within the Private Hospital with respect to relative staffing levels, workload and funding. Comparisons by age group indicated a generally higher reported level for the youngest age group. Comparison of full time versus part time staff indicated a significantly higher level of perceived stress from full time staff. Analysis of reported stress by ward type indicated that staff working in the "medical" groupings experienced higher perceived levels of stress than those working in the "surgical" groupings.

Health personnel, especially nurses, are often victims of workplace violence. Unfortunately, little is known about the nurses' experience of violence. A research study was initiated to further explore the nurses' accounts of workplace violence so as to make dimensions of the nurses' experience visible and more fully understood. Interpretive description was the research methodology adopted for this study. Using theoretical sampling, ten Registered Nurses from the lower mainland and Vancouver Island, British Columbia participated in semi structured, audiotaped interviews. In this research, the nurses' experience of workplace violence emerged as a highly complex entity, deeply embedded in relationships and context. How nurses perceive the contextual factors of the organization, their immediate work environment and their individual attributes were found to play a significant role in how they respond to the phenomenon. The findings of this study suggest that organizational culture is an important determinant in managing workplace violence and that policy and administrative personnel play a pivotal role in influencing the problem. Nursing culture also influences the nurses' expectations, assumptions and actions towards violence. Participants voiced that role conflict often challenged their ability to enact acquired professional ideals and that that they routinely undertake roles in dealing with violence that are not appropriate to their level of knowledge or skill. Within the nurses' immediate work environment, bullying as well as physical and verbal abuse was commonplace. Overcrowding, long waits for service, poor environmental design and inadequate staff to patient ratios were seen as factors that increased nurses' risk. Individual factors were associated with emotional and psychological harms that nurses endured. Workplace violence affected self-concept, self-esteem, self-efficacy and the nurses' sense of control. Moral distress, self-blame, feelings of failure, loss of motivation and leaving the nursing profession were significant findings. The results of this study demonstrate a need to re-think how we can address workplace violence in nursing. Research and intervention is needed to further explore organizational policy and governing structures, the culture and climate of practice environments, and the fundamental role nursing education programs have in preparing nurses to manage workplace violence.

The purpose of this multiple case study was to explore strategies that healthcare facility leaders in Central Minnesota use to recruit and retain qualified nurses. Participants were 6 healthcare facility leaders including 2 nursing directors, 2 human resource personnel, 1 nurse supervisor/administrator, and 1 nurse recruiter who had the knowledge and experience in recruitment and retention of Registered Nurses (RNs) in healthcare facilities in Central Minnesota. The Herzberg 2-factor theory was the conceptual framework. Semistructured interviews were used to collect data. Data were analyzed using Morse's 4 steps data analysis process. The major themes were recruitment strategies and retention incentives. The recruitment strategies were the various hiring methods participants used to attract and gain RNs, and the retention incentives were the benefits that motivated and retained RNs. Participants relied on both recruitment strategies and retention incentives to manage the shortages of RNs. The results provide healthcare facility leaders with additional information about how to successfully recruit and retain qualified RNs, which may lead to a larger and potentially satisfied nursing population. Social implications include strengthening the nursing workforce, increasing productivity for healthcare facilities, providing better customer service and increased patient satisfaction, and contributing to more satisfied RNs and families, as well as helping the local communities and the economy.

Nurse shortages and nurse turnover are major issues in the health care industry. As 4 generations of nurses are working side by side for the first time in history in the health care industry, nurse leaders need to understand the generational differences in order to bridge the gap on retaining the nurses in the workforce. The primary focus of this applied dissertation study was to explore and obtain the lived experiences of leading the nursing intergenerational cohorts, as well as the strategies that nurse leaders or nurse managers can utilize to meritoriously attract, retain, and motivate the generational nursing workforce. The Leadership Questionnaire, designed in 2008 by Dr. Nelson, was utilized to interview 5 nurse administrators of the phenomenon to comprehend how the health care nurse administrators can utilize productive techniques of leading the nursing generational cohorts. The target population was members of a professional long-term care association. Once the nurse administrators agreed to participate on the study and signed the consent form, the researcher scheduled an initial 45-minute interview of three 15- to 30-minute interviews over a 3-month period. The data collected as a result of this study revealed findings: (a) the intergenerational educational gap in the nursing workforce, (b) the needs of the intergenerational nurses, (c) the critical aspect of continuing of professional education training development for the nurses, and (d) the critical leadership values on leading the intergenerational nursing cohorts. This applied research study dissertation intended to assist nurse leaders to reframe perceptions regarding the nurses’ intergenerational group (e.g., Baby Boomers, Generation X, and Generation Y) differences and to view these differences in attitudes and behaviors as potential strengths.

The research reported in this thesis explored the perceived value of transition support programs for newly registered nurses in New South Wales and the health care facilities offering such programs. Although transition support programs have been designed and implemented in various forms since the transfer of nurse education to the tertiary sector, there remains little evaluative evidence of the value of such programs. Two groups of registered nurses formed the participants in this study. The first were new graduate nurses who completed a transition support program within the past 12 months. The second comprised experienced nurses who worked with new graduate nurses during their transition support program. The study was carried out in seven hospitals in area health services across and around Sydney, representing both small and large facilities with bed numbers ranging from 195 to 530. Data were collected from four sources including the printed materials made available by sample hospitals. Questionnaires, interviews and observations were used to determine the purposes, outcomes and strengths and weaknesses of transition support programs. The data were analysed using descriptive statistics and theme extraction. The themes described the ways in which the transition support programs were used to facilitate the transition of the newly registered nurse to confident beginning practitioner. There was widespread belief from the study participants that some aspects of nurse education at university were inadequate. As a result, various structures and policies were required to support the new graduate nurses as they entered the workplace. The transition support programs were used to increase nursing staff for the study hospitals and to provide new learning opportunities for new graduate nurses to enable them to develop the clinical and professional skills required of competent registered nurses. The rotational aspect of the transition support programs were used to provide staff for the less popular areas of the hospital as well as a variety of experiences and skill development opportunities that were considered lacking in the current undergraduate education of nurses. The work environment where the programs operate were described as difficult with nurses exposed to violence and bullying practices from fellow staff and frequently required to work with a less than ideal number or appropriate skill mix of nursing staff. The hospitals also used the transition support program to exert a controlling influence over the new graduate nurse by way of roster management, assessment of skills and the expectation that each nurse would complete a transition support program before being offered full time work. The thesis concludes with recommendations and future research avenues. It would be useful for hospitals to conduct formal evaluations of the transition support program they offer to provide the most effective program possible. One source of information could be sought from the new graduate nurses regarding their needs during the first six months of employment. It is also suggested that a study be commenced that investigates the reasons behind the perception that nurse education at universities in New South Wales is inadequate.

This research explores how registered nurses constructed their leadership role during the provision of health care services in acute care, adult hospitals in Brisbane, Queensland, Australia. As health care organizations change to meet the demands of the twenty first century, nurses in Australia are coming to realize there is a dissonance between what they perceive to be the relevance of their work and the perception of the relevance of nurses’ work by others in the health care system. Consequently, nurses’ contributions to health care services are not recognized. The literature highlights that one way to address this problem is to articulate the various leadership roles contemporary nurses are asked to undertake. This is the aim of this thesis. This research seeks to illuminate the role of the nurse within changing health care systems by making clear the nature of their work through the perspectives of leadership. Consequently, the purpose of this study is to explore how nurses have undertaken leadership initiatives in their role as health care providers within contemporary health care organisations. The literature review generated following research questions: 1. How do nurses describe leadership within their health care organisations? 2. How do nurses experience leadership within their health care team? 3. How do nurses construct their leadership role whilst providing health careservices? In order to legitimate its findings this study aimed to provide a clear theoretical framework. In order to gain a clear understanding of the personal experiences and meanings of the participants, the theoretical framework for this study was underpinned by the interpretive philosophies the epistemological framework of constructionism and the theoretical perspective of symbolic interactionism. The methodology of case study enabled an empirical investigation of a contemporary nursing phenomenon, leadership wherein the researcher was able to pose questions to those nurses from whom most could be learned. Data were collected through two stages. In stage one, the exploratory stage data was collected through three focus group interviews. Stage two aided deeper exploration of the nurses’ leadership constructs with data obtained through one-to-one interviews. Analysis of the data enabled the development of a model of nurse leadership. Participants identified that their leadership was constructed through three perspectives of Self as Leader, Self and Others and Self in Action. The findings contrast the nurses’ unique leadership constructs to those of health care organisations, highlight the lack of acknowledgment for nurse leadership within health care teams, and demonstrate how the nurses’ leadership constructs influence their decision to act in the provision of patient care. This study concludes that as the nurses come to realise traditional leadership models are incompatible with their goal of achieving patient centred care, they have developed a different style of leadership to achieve their vision of patient centred care. Finally this study offers recommendations in the areas of nursing practice, nursing education and research.

Registered nurses entering a Nurse Prescriber programme participated in a mixed methods case study to explore the extent of their bioscience knowledge and the confidence with which that knowledge was held. Forty two Nurse Prescriber students, aged 26 – 55 years, from a range of job roles were recruited. Using questionnaires and interviews, both quantitative and qualitative data were obtained. An examination of the Nurse Prescribers’ views of pre-registration nursing demonstrated that the knowledge gained had been related to practice but had been both superficial and lacking in breadth. The bioscience in pre-registration programmes had not sufficiently prepared the participants for their roles as registered nurses. The importance of experiences gained as a registered nurse in the practice setting in the learning of bioscience was strongly emphasised. Participants reported greater learning of bioscience by informal methods such as work experience, use of books and the Internet and discussion with colleagues than from experiences in the classroom. Interviewees placed particularly strong emphasis on the importance of learning from medical colleagues. The role of post-registration programmes emerged as important in learning bioscience because it related to the job role. Post-registration courses also emerged as significant in giving confidence to the registered nurse. Confidence increased not just in terms of the knowledge held, but also in terms of nurses’ ability to communicate with patients, relatives, and doctors, their ability to understand nursing skills, and their willingness to admit when something was not known.

The recent impetus to increase the number of baccalaureate-prepared nurses comes from within and outside the profession, prompting increased numbers of registered nurses (RN-BSN) to return to college. Yet little is known about what these adult, non-traditional students do to prepare for the challenges ahead. Therefore, the purpose of this mixed-methods case study was to learn how RN-BSNs prepare for their return to college, why they go about doing so, and what resources they utilize. Secondly, what are the gaps, how do they overcome them, and what is their advice to potential RN-BSN students so they could be better prepared. The Transtheoretical Model and the Epidemiological Model were used to guide the study. The eight participants interviewed ranged in age (32-57). Each had contemplated the return to school since their initial nursing program. Six were enrolled at a rural satellite site at their place of employment. While participants prepared in a variety of ways, findings showed they also did not know what to expect and thus felt unprepared in many respects. The most common type of preparation was mental; whereas the most frequent gap in preparation was academic, including writing papers, presentation skills, computer literacy, citations, obtaining books, information literacy, and study skills. Overall, five themes summarized their advice to future students: (a) obtain academic skills early on, (b) find balance, (c) make connections, (d) embrace the difference, and (e) don't wait; make the decision. Based on the findings, examples of recommendations that are both practical and affordable are outlined.

The nursing home industry has low registered nurse retention rates. Low retention rates can be related to job satisfaction. The nursing home industry needs qualified registered nurses (RNs) who are satisfied with all aspects of their jobs. The purpose of this study was to examine factors affecting job satisfaction of RNs currently employed in nursing homes using Herzberg's (1968) Dual Factor Theory.Herzberg (1968) has identified internal factors that motivate employees on the job. If intrinsic factors are met, the employee is satisfied. Extrinsic factors, if present, will not satisfy the employee, but will prevent dissatisfaction (Herzberg, 1968). The Minnesota Satisfaction Questionnaire - Short Form, was used to determine the intrinsic, extrinsic and general satisfaction scores of 48 (480) RNs currently employed in ten nursing homes from the midwest. Confidentiality and anonymity of the subjects were maintained as questionnaires contained no identifying information.Findings supported previous studies on job satisfaction of RNs employed in nursing homes. Participants did not identify intrinsic factors as being fully satisfactory, but responses were closer to satisfied than dissatisfied. Extrinsic factors were not sources of dissatisfaction, but responses were closer to dissatisfied than satisfied. General satisfaction scores determined respondents were not fully satisfied, but were closer to neutral scores. Responses were closer to being satisfied than not. Conclusions were that extrinsic and intrinsic factors influence job satisfaction.Implications call for examination of factors effecting job satisfaction. Managers should increase job security, social services, activites, independence and variety. Human relations, company policies and technical supervision should be open to input from staff as to how the extrinsic motivational factors can be improved. Recommendations include replication of this study with a large sample size; assuring nurses under 40 years of age participate in further studies and the need for nursing home administrators to address intrinsic and extrinsic factors that affect job satisfaction.

ENGLISH ABSTRACT: Managing human resources is crucial to the efficient and effective delivery of quality health care. However, turnover of nurses constitutes a major factor in the shortages of staff which are being experienced by the nursing profession in the Republic of South Africa. Shortages of trained nurses with experience, particularly in public sector hospitals, have adverse effects on the provision of efficient and effective quality health care to the consumers of this service. The aim of the study was to provide an understanding of and insight into those inherent problems in the health sector that propel nurses to leave public sector institutions. The overall objective was to investigate and identify some of the factors which exercised an influence on the turnover of registered nurses in public sector hospitals, so as to be able to provide suggestions to hospital managers on the more effective management of human resources, in order to retain nursing personnel. Theresearch was based on a model of nursing turnover which regarded voluntary withdrawal as a process in which feelings of satisfaction with pay, on the one hand, and the opportunity of obtaining alternative jobs in the labour market, on the other, were proposed as the primary causative factors of turnover behaviour. In addition the propositions, made in literature reviewed, that age, tenure, kinship responsibility, general training, education, professionalism, marital status, lintent to stay', job satisfaction, routinization, job autonomy and responsibility, instrumental communication, promotional opportunity, integration, supervisory relationships, distributive justice, work-load, and local kin acted as predictors of turnover, were also investigated. The research was conducted with a sample of 123 nurses in one hospital, Groote Schuur, situated in the Western Cape Province of the Republic of South Africa. The sample included registered nurses, senior professional (registered) nurses, and chief professional (registered) nurses. The data was gathered from respondents by means of a selfadministered questionnaire. In addition, data was gathered by means of semi-structured, open-ended discussions with nursing management. The hypothesised interactions between variables influencing nursing turnover were explored by means of basic statistics, which made it possible to assess the effects of both independent and dependent variables. The results of data analysis provided some support for the proposition contained in the hypothesis. The determinants whose increase produced a greater degree of turnover were firstly, the many jobs available outside the hospital and secondly, professionalism. The determinants whose increase resulted in reductions in turnover were "intent to stay" (which the researcher views as a dimension of commitment), the existence of local kin (kinship responsibilities), participation in making job-related decisions (job autonomy), the receipt of sufficient work-related information (instrumental communication and good supervisory relationships), and tenure. The determinants whose decreaseresulted in increased turnover werepromotional opportunities, distributive justice, pay satisfaction, job satisfaction, integration, opportunity for self-development,age and tenure. Turnover of nurses has serious ramifications for employers, patients, and the nursing profession itself. Effective management of employee turnover is of critical importance to health care providers, employees, and patients. Better control of turnover can improve the quality of patient care, reduce labour costs, and improve employee morale.
AFRIKAANSE OPSOMMING: Die bestuur van menslike hulpbronne is die deurslaggewende omstandigheid ten einde die lewering van effektiewe en doeltreffende gesondheidsorg van gehalte. Nietemin, dra die omset van verpleegkundiges grotendeels by tot die personeeltekort wat tans deur die verpleegprofessie in Suid-Afrika ondervind word. Die tekort aan ervare, opgeleide verpleegkundiges, veral in die openbare sektor staatshospitale, het 'n nadelige uitwerking op die voorsiening van effektiewe en doeltreffende gesondheidsorg van gehalte aan die verbruikers van hierdie diens. The Baccalaureate Nursing program in San Antonio, Texas experienced a decrease in National Council Licensure Examination for Registered Nurses (NCLEX-RN) on the first attempt for students graduating between 2009 and 2014 without a clear explanation for the decline. The purpose of this quantitative non-experimental correlational study was to analyze retrospective data from the school of nursing in San Antonio to determine the extent to which multiple variables (age, gender, race/ethnicity, cumulative pre-nursing GPA, cumulative GPA of nursing courses, remediation, and the Assessment Technologies Institute (ATI) examination predicted NCLEX-RN performance. The research question was: Is the ATI comprehensive examination a significant predictor of the NCLEX-RN performance of graduating nursing students in the San Antonio, Texas nursing program, either (a) on its own; or (b) in combination with other independent variables. The statistical problem was directed toward identifying the significant variables that predicted the NCLEX-RN performance of graduating nursing students between 2009 and 2014 using binary logistic regression analysis. The proportion of N = 334, nurses who passed the NCLEX-RN was n = 232, 69.5%. The answer to the research question, based on odds ratios (OR) was that NCLEX-RN performance could not be predicted solely by using the ATI predictor examinations. The ATI examination score was the strongest predictor of passing the NCLEX-RN (OR = 1.59) Ethnicity (OR = 1.38) and the combined pre-nursing and nursing GPA (OR = 1.28) were also found to be predictors of NCLEX-RN performance. The proportions of NCLEX-RN failures and need for remediation were highest among the African-American students. The gender and age of the students were not significant predictors of NCLEX-RN performance. The results of this research can be utilized by the San Antonio nursing program, as well as other nursing programs, to identify and address the factors identity of those graduating nursing students who are at risk of failing the NCLEX-RN. The researcher has shown that the predictor variables of the ATI predictor examination, cumulative college and nursing GPA’s, and ethnicity have a statistically significant correlation and therefore have impact on first time NCLEX-RN test takers passing the exam. It is recommended that Bachelor Degree Programs in Nursing focus on specific strategies within their institutions that would have a direct impact on these variables.

Mellan 1990 - 2010, sjönk antalet Hivinfekteradepersoner från 18 till 6 %, till följd av ökad medvetenhet och mer säkert sex. Idag har Ugandaen av de högsta nivåerna av nyligen infekterade människor. Unga vuxna i åldrarna 15–24 år ärden grupp med högst risk att bli infekterade med HIV i Uganda. Det är vanligt attsjuksköterskor runt om i världen har rådgivande samtal. Även om hur rådgivning utförs kanvariera så är målet att minska riskbeteenden och motivera till att ha säkert sex.Syfte: Syftet med den här kandidatuppsatsen är att beskriva legitimerade sjuksköterskorserfarenheter av att ha rådgivande samtal med unga vuxna om HIV.Metod: En kvalitativ studie med semistrukturerade intervjuer på Lubaga sjukhus i Kampala,Uganda. 12 legitimerade sjuksköterskor intervjuades från tre olika avdelningar, materialettranskriberades och analyserades med en induktiv kvalitativ analysmetod, inspirerat avBurnard.Resultat: Fyra teman identifierades: en komplex miljö att ha rådgivande samtal med ungavuxna, unga vuxnas försök till självhjälp, Socioekonomisk utsatthet kopplat till HIV och ungavuxnas känslor relaterat till HIV. Ur dessa fyra teman upptäcktes 11 kategorier.Konklusion: Legitimerade sjuksköterskor upplever att det finns både hjälpande och stjälpandefaktorer som påverkar deras rådgivande Sammanfattningsvis så orsakar unga vuxnas rädslaför HIV, stigma relaterat till HIV och socioekonomiska förhållanden hindrar för samtal ochhur resursfulla unga vuxna är underlättar för rådgivande samtal.
Background: In the early 1990’s Uganda suffered from a HIV-epidemic and was one of themost effected countries in Sub-Sahara Africa. However, from 1990 - 2010, the level of HIVinfected people dropped from 18 to 6 % due to awareness raising and less high-risk behaviorssuch as unsafe sex. Today, Uganda has one of the highest levels of newly infected people.Young adults in the ages 15 - 24 years old are the most exposed group to get infected withHIV in Uganda. Counseling is a common task in nursing all around the world. Although thespecifics of how counseling is performed could differ, the main goal is to reduce the numberof high-risk behaviors and motivate the person to engage in safe sex, including sexual contactand use of condoms.Aim: The purpose of this study is to describe nurses’ experiences of counseling young adultsabout HIV.Method: A qualitative study with semi-structured interviews at Lubaga hospital in Kampala,Uganda. 12 registered nurses were interviewed from three wards at the hospital, the materialwas transcript and then analyzed with an inductive qualitative content analysis approachinspired by Burnard.Results: Four themes were identified: a complex environment to counsel young adults in,young adult’s efforts to self-care, socio-economic challenges linked to HIV and young adult’sfeelings associated with HIV. Moreover, 11 categories emerged through the themes.Conclusion: Registered nurses experience both factors that burdens and facilitate counseling.In conclusion, young adults fear of HIV, HIV-related stigma and socioeconomic factors posean obstacle when counseling young adults about HIV and young adults’ resources makescounseling easier.

The health care field has become increasingly more complex, requiring new nurses to be prepared upon graduation to respond to a variety of complex situations. Unfortunately, many graduates from associate degree nursing (ADN) programs are not able to think critically upon entering the work force. This presents a major problem for the nurse and for the employer. The purpose of the study, therefore, was to gain a deeper understanding of the graduates' perceptions of their ability to critically think during their first year of clinical practice, and if they believed their program prepared them to be critical thinkers. The key research questions focused on how the novice nurses reconciled their performance on a critical thinking, online assessment, the Health Sciences Reasoning Test (HSRT), with their perception of their critical thinking skills, and if they felt prepared, during their first year of clinical practice, to critically think. The conceptual framework applied was Bloom's Taxonomy and Tanner's clinical judgment model. A purposeful sampling of 7 novice nurses from 3 ADN programs was chosen. After completing the HSRT, audio-taped phone interviews were conducted. The data indicated that the participants felt unprepared to respond to emergent patient situations, thus undermining their self-worth and clinical competency. The participants agreed there was a need for a critical thinking course in ADN curriculum. A project was created for a 9-week critical thinking course, incorporating theory, clinical practice, and simulation exercises. Social change is expected to occur when student nurses are able to critically think upon graduation, resulting in positive patient outcomes, both of which will benefit patients, their families, and their communities.

Acquired Immunodeficiency Syndrome (AIDS) is primarily identified as a metropolitan disease. However, it has suggested that the Centers for Disease Control may underestimate the prevalence of AIDS in the population of higher socioeconomic status, overstate the relative prevalence of AIDS in the minorities, and understate the prevalence of the disease in the Midwest (Laumann, Gagnon, Michaels, Michael, & Coleman, 1989). The problem addressed in this study was to determine whether groups of urban and rural perioperative registered nurses differ in their attitudes of tolerance toward AIDS patients. The attitudes of rural and urban perioperative nurses were examined in a comparative descriptive design. It is important to identify nurses' attitudes toward AIDS patients because nurses must interact with AIDS patients on an increasing basis.Lazarus and Folkman's (1984) Theory of Cognitive Emotion was used for the framework. A convenience sample of 77 perioperative registered nurses was obtained for the study. Five midwestern hospitals were used to collect the data. Two urban hospitals and three rural hospitals were used as collection sites. The AIDS Attitudes Scale (AAS) was used as the tool to collect the data (Shrum, Turner, and Bruce, 1989). The AAS consists of a fifty-four item questionnaire designed to measure attitudinal tolerance towards the AIDS patient. Validity and reliability of the tool were established with a resulting reliability score of .94.Findings revealed significant differences among urban and rural perioperative registered nurses in attitudes toward AIDS patients (p=.0387), with urban perioperative nurses being more tolerant of AIDS patients. Item-by-item analysis indicated that although urban perioperative nurses were more tolerant, an urban perioperative nurse would be more uncomfortable around a patient with AIDS (p=.0082). However, more rural perioperative nurses indicated that they would move out if a roommate had AIDS (p=.0030). Rural perioperative nurses indicated more often that no one deserved to have a disease like AIDS (p=.0057). Demographic profiles of registered perioperative nurses demonstrated similar backgrounds in relation to age, educational level, and gender.Conclusions of this study indicated urban perioperative registered nurses hold more tolerant attitudes toward HIV/AIDS patients than do rural perioperative registered nurses.

The purpose of this study is to determine the level of job satisfaction of Registered nurses in Muscat and relationships to Herzberg's motivation and hygiene factors. The theoretical underpinning of this study is based on Herzberg's concepts, in which an individual's motivation and hygiene factors are met within an organization, to the extent that the individual employee will express in job satisfaction. A descriptive, nonexperimental research design was used. The Work Quality Index and a demographic survey were given to 202 registered nurses at Sultan Qaboos University Hospital (SQUH) and Royal Hospital (RH) in Muscat. The data was collected over three weeks and was analyzed, using descriptive statistic, t-test, ANOVA for independent mean, and correlation statistics as appropriate. The results indicate that nurses in the two hospitals had a moderate level of job satisfaction. It also proves that motivation factors and hygiene factors are significantly correlated with job satisfaction, meaning that the fulfillment of both types of factors determines the overall job satisfaction of the nurses.

Background and Purpose: Registered nurses (RN’s) are well positioned to provide physical activity counseling (PA counseling) to their hospital patients. RNs educate during ‘teachable moments” as hospital patients often find themselves in a state of readiness to make lifestyle changes such as increases in regular physical activity. Although the health benefits of physical activity are well documented, PA counseling has not been well studied among the nursing population or in the hospital environment. The main purpose of this study was to identify the percentage of RNs who provide PA counseling to their patients and to explore the factors which contribute to their decisions to provide PA counseling. Methods: A web-based survey was used to gather information from 323 hospital-based RNs employed at five hospitals in four states. The survey gathered information including current physical activity levels, thirteen perceived barriers to physical activity based on the Exercise Benefits and Barriers Scale, and information regarding whether RNs provide PA counseling to their hospital-based patients. Results: RNs reported an average of 3.3 barriers to being physically active. One hundred and eighty- seven RNs provided PA counseling to their patients (57.8%) and 133 did not provide PA counseling (41.1%). The presence of barriers to being physically active was related to PA counseling for nine of the thirteen barriers. The following hypotheses were supported: 1) RNs who report lower levels of perceived barriers to being physical active were more likely to provide PA counseling than those who report higher levels of perceived vigorous physical activity were more likely to provide PA counseling than those who reported lower levels of light, moderate, and vigorous physical activity. Conclusion: The findings from this study revealed RNs who are physically active and report fewer barriers to physical activity were more likely to provide PA counseling to their hospital-based patients. Future research needs to address types and quality of PA counseling as well as utilize theory driven intervention designs.

The following study explored the experiential eustress accounts of nurses within a hospital work environment. Previous literature found some level of connection between eustress and hope, self-efficacy, collaboration, resilience, and self-perceptions of health. However, the previous literature was attempting to explore and explain a cognitively appraised construct, eustress, without first gathering experiential accounts of the construct from participants. This may have altered the discoveries previously made and necessitates the following research. The current study produced data that helped to address the gap in the previous literature relevant to the guiding research question: What is the eustress experience of RNs in organizational contexts? The constructivist grounded theory method was used to answer the research question. The current study explored the experiential accounts of 12 Registered Nurses (RN) from a hospital within the Western US. All participants were RNs, female, aged 25-65 and having various experience levels (5 to 43 years). Various levels of grounded theory coding were used on the data to produce three models and a theory. The current study found that the participants explained eustress differently than the terminology used in previous literature. All accounts of eustress were directly connected to intense situations (highly stressful experience) that had positive outcomes. These eustress experiences altered the RNs in self-reported beneficial ways, ranging from enhancing confidence, resilience, and professional growth to changing motivation, altering professional perceptions, and changing how they felt about stress in general. The presence of a collaborative, open-minded, and trustworthy team was also noted as having a profound effect on participant recollections of eustress.

The increasing complexity of the healthcare environment calls for increasing emotional intelligence (EI) competence in nurses so as to establish effective relationships that facilitate positive patient outcomes. Because nurses need to be competent in EI, it is important to determine if professional nursing programs prepare graduates in emotional intelligence and whether years of professional experience contribute to development of emotional intelligence. This study assessed the emotional intelligence competence of 164 baccalaureate nursing alumni who graduated during the years 2007-2010 from three Benedictine institutions in the Midwest to see if there was growth of EI with experience as a registered nurse, and to determine if age, gender, grade point average (GPA), and years of total healthcare work experience prior to graduation predicted EI. Participants completed the Mayer Salovey Caruso Emotional Intelligence Test (MSCEIT) and a demographic survey. Findings from this study indicated 79.4% of participants were competent or higher on the MSCEIT total EI score. Percentages of nurses scoring in the competent or higher range on each of the four branch scores of perceiving, using, understanding and managing emotions were 80.6%, 72.7%, 84.2%, and 84.9% respectively. An independent-sample t-test found no significant differences on EI scores between graduates with 1-2 years compared to 3-5 years of experience as a registered nurse. Results of a linear stepwise regression for determining the usefulness of age, GPA, gender, years of total healthcare experience prior to graduation and years of experience as an RN to predict the total EI score on the MSCEIT revealed being female as a significant predictor (p = .015). Being female was a significant predictor for the using emotions branch (p = .047). Significant predictors of EI on the understanding emotions branch were GPA (p < .001) and being female (p = .023). There were no significant findings regarding the perceiving and managing branches of the MSCEIT. The findings of this study indicate there is work to be done to improve the EI competence of nursing graduates. Continued research on the topic of EI and nursing is needed to build the knowledge base on how to promote positive patient outcomes.

Lifelong learning is strongly encouraged within healthcare, through government and professional initiatives. The emphasis of this learning is to positively influence practice thereby enhancing the patient experience and patient care. However, within these initiatives there is little consideration of the impact of this lifelong learning on the individual, nor their ability to engage with it. This study employed a hermeneutic approach which explored the feelings, perceptions and experiences of a group of registered nurses relating to their learning about practice. The study provides insight into how nurses view themselves professionally and academically. Data were collected in three ways. Demographic data was collected to situate the sample group. Written and verbal data were collected through the use of meta-planned focus groups (Davies et al 2001). Linguistic metaphor analysis was carried out on the written and verbal data which was coded, categorized and organised into themes. The study revealed a number of attributes in relation to learning about practice. Whilst many participants were intrinsically motivated to develop new skills and ways of working, they were also extrinsically motivated through the relationships they had with colleagues. It was recognised that a supportive environment and collaborative partnership led to a successful learning experience. However, whilst many of the participants recognised their own need to develop skills of assertiveness, they identified that professional inequalities and the inappropriate use of power within their relationship with colleagues adversely affected their learning. The findings from the study resulted in the development of a model of support for learning in practice. This is based on the individual nature of learning, the social environment of learning and cultural aspects of learning. These three areas encompassed the feelings, perceptions and experiences of registered nurses related to their learning about practice.

Increased breach occurrences in healthcare cause concern for health information as reported by the Federal Government. Significant effort, regulations, and safeguards are in place to protect the systems used in the healthcare industry. Employee handling of security remains an area of vulnerability related to security protocols. The unified theory of acceptance and usage of technology (UTAUT) served as the model for this qualitative exploratory study with the purpose of understanding registered nurse (RN) perceptions and lived experiences related to IT security. Face-to-face interviews were conducted with 20 participants from the Three Rivers Chapter American Association Critical Care Nurses. Transcribed data were analyzed with a priori codes aligned to the constructs of UTAUT and emergent themes. The emergent themes from the RNs' lived experiences revealed perceptions of IT security mishandling, including walking away from the computer without log-off, and sharing of accounts through single sign on authenticated badges. Strategic planning for the organizational IT security may be strengthened due to the insight about the RNs' workflow related to IT security. Understanding employee perceptions, expressed intentions, and self-reported behaviors to IT security allows for the ability as managers to apply that knowledge to IT security systems, access methods, and implement procedures that will provide for increased organizational IT security and increased patient confidence. The social change from this work may provide contributions to the development of IT infrastructure systems for healthcare helping to create and maintain continued access to and availability of electronic medical records and data for increasing numbers of people who need health maintenance and care.

Many entry-level nurses are not prepared to handle medical emergencies. Although responsible for managing the care of individuals with complex medical conditions, many of these nurses compromise the safety of patients due to a lack of experience and an inability to apply clinical judgment. The purpose of this study was to explore the perceptions of registered nurses about their transition from entry-level to advanced beginner. Bandura's social cognitive theory, along with Colaizzi's descriptive method of data analysis, provided a basis for this phenomenological study. Research questions focused on challenges that entry-level nurses have experienced with problem-solving and complex patient care that requires advanced critical thinking and the application of clinical judgment. Criterion sampling facilitated recruitment of advanced-beginner RNs, with data collected through semistructured, one-on-one interviews. Data analysis occurred in a series of steps, including extracting and developing meanings from interview transcripts, clustering meanings into description lists, and eliminating outliers. Data analysis revealed 12 major themes aligned with behavior, clinical environment, and personal/cognitive factors. Among the findings were that nurses often felt unsupported, unable to manage conflict, unprepared, unseasoned, inefficient, and unable to lead others effectively. This study was necessary because its findings may provide insights leaders in health services can use to develop strategies to better prepare entry-level nurses to care for individuals with complex medical conditions. Among the implications for positive social change are developing a better tool for the training and advancement of entry-level nurses, consequently improving patient safety and reducing health care costs.

This purpose of this quality improvement project was to develop an educational training curriculum designed for staff nurses to use the teach-back method with patients with low health literacy during discharge instructions. Health literacy is associated with the ability to read, comprehend, and make appropriate healthcare decisions; the teach-back method asks patients to restate or explain health information in their own words, aiding retention and literacy. Effective staff education plays a critical role in the education of patients during discharge teaching: Nurses must help to ensure that patients and their families understand healthcare information and apply this information in their daily lives. The program was planned in an ambulatory care setting and used conceptual frameworks including the Iowa model and the Plan-Do-Study-Act cycle for quality improvement. The program was informed by evidence in the peer-reviewed literature. The curriculum was contextualized by needs of the care setting. The evidence was organized and analyzed using a review matrix to identify common findings among major studies that were applicable to the context. As an aid to operationalization of the program, an implementation plan and an evaluation plan were developed for use by the institution to move the program forward without additional planning. This project may effect positive social change by addressing a health care need that exists throughout the population and is prevalent in those of lower socioeconomic status. Increasing health literacy among patients is likely to promote improved health outcomes among those who are most vulnerable to illness.

A descriptive quantitative study was conducted to determine what factors nurses considered when discharge planning, and how these factors compared with factors identified in the literature as being effective in planning for discharge. This study was undertaken because with the intended Prospective Payment System (PPS) of funding to hospitals, it is essential that Western Australian Clinical Registered Nurses are able to prepare patients for discharge effectively to prevent the financial burden of cost outliers and re-admissions. Anderson and Steinberg ( 1984) in their studies of factors that influence the cost of hospital care for the elderly, found that the results of inappropriate and premature discharges resulted in a 22% readmission rate within 60 days of discharge for all Medicare hospitalisation. Their (1988) study of readmission rates of Medicare beneficiaries between 1974 and 1977 showed that the added costs associated readmissions cost the U.S. government more than US$2.5 billion per annum. It would seem probable that such a system introduced into the Australian health:care system will have the potential to produce similar effects for Western Australian patients, nurses and nursing.

Objective: To determine the readiness of registered nurses (RNs) who provide care for patients with diabetes in a primary care setting for RN prescribing. Setting: Canadian provinces and territories. Participants: Canadian RNs working in primary care settings who provide care to patients with diabetes. Methods: A cross-sectional survey design was used to examine the readiness of RNs by measuring the attributes of readiness (value, confidence and willingness). The survey included questions on the demographic profile of participants including their age, education level and whether or not they had obtained additional certifications. This data was used to describe the sample and to determine whether there were differences between RNs who were ready to prescribe and RNs who were not ready to prescribe. Results: Almost 75% of the respondents were "ready" for RN prescribing. There were high positive correlations between "value" and "confidence", "value" and "willingness", and "confidence" and "willingness". There were no statistically significant differences in age, level of education and certifications for respondents who were ready to prescribe compared to those who were not ready to prescribe. Conclusions: The results of the present study indicate that RNs who provide care for patients with diabetes in a primary care setting are ready for prescribing. This practice has the potential to make more effective use of health human resources, allowing RNs to work to their full potential and increase accessibility to care for patients with diabetes.

The focus of this research study is the phenomenon of the registered nurses' role with students in clinical placement experience. With the transfer of nursing education from the hospital sector to higher education, the role of the student has changed from employee to supernumerary status. The role of the registered nurse with students has also changed, but very little is known about the way registered nurses view and understand their role with students in clinical placement experience. The research investigates the variation in understanding that registered nurses have of their role with students and identifies the dimensions of this experience of the role. While this study is located in Queensland, Australia, the role of the registered nurse with students is recognised as an area which has been the subject of little formal study in other states and countries. Students have frequently expressed satisfaction and dissatisfaction about their clinical placement experience, an essential component of the preparation for undergraduate nursing programs. The research approach represents an innovative way to the study of the understanding registered nurses have of their role with students in clinical placement experience. The approach was selected with the intention of exploring the issue from the perspective of registered nurses, rather than students or evaluation of programs which appears to be the traditional approach to studying clinical placement experience. Registered nurses are acknowledged as major participants in clinical placement experience for students but their perspective of the role has rarely been considered. The selection of phenomenography adopted for this study is based on the appropriateness of the research approach to explore the object of inquiry. The aim of phenomenography is to describe, analyse and understand the meaning people ascribe to their world and how they construe different phenomena. The central concern is not the phenomena being investigated, nor the people who experience the phenomena, but the relation between the two: the relation between the RNs and their role with students. The findings from this study result from 28 individual interviews with registered nurses in order to identify the variation of understanding of the experience of the role with students. The registered nurses who participated in the research were from fifteen public and private hospitals throughout regional and metropolitan, Central and South Eastern Queensland. The major outcomes that emerged from this study show that registered nurses have different ways of understanding their role with students in clinical placement experience and that the number of different variations is limited. Eight distinct categories of description were identified, denoting the understanding that registered nurses have of their role. These have been labelled: Facilitator role, Teacher/Coach role, Overseer/Supervisor role, Peer Support and Role Model role, Instructor role, Manager/Foreman role, Authority role and Resister/Dissenter role in recognition of the meaning identified for each category of description. The outcome space mapped a four level developmental relationship between the categories, which denote the variation of ways registered nurses understand their role with students. These ranged from minimal involvement to total commitment and involvement with students. These findings support the need for open acknowledgment of the informal but complex role registered nurses have with students in clinical placement experience, and the need for adequate preparation for this role. The findings also provide insight into the concerns of employers and recent graduate registered nurses of the adequacy of clinical placement experience as preparation for the role of registered nurse. Further research and recommendations are proposed using these findings as the basis to improve the quality of the experiences of registered nurses and students in clinical placement experience.
