The monograph includes studies of the last decades by a group of scientists engaged in solving problems of theoretical and methodological study of the problem of psychological manifestations of modern generations. The authors reflected the versatility of definitions of the concept of "generation" and revealed the multidimensional phenomenon of generations. The author's approach to the succession of generations deserves attention, which opens up prospects in the study of intergenerational relations. A level model of value-semantic mediation of intergenerational relations is presented, which allows a comprehensive look at the manifestations of generations. Reviews of generational studies are presented and empirical cross-sections are presented in the value-semantic perspective of the formation of generations. The problem of perception of generations by each other in age and gender aspects is considered in a special way. Attention is paid to the problems of spreading the belief in conspiracies in the modern youth environment and the harmful consequences of this. The empirical model of the conspirativist mentality is the beginning of research in this regard. It is addressed to philosophers, cultural scientists, sociologists, psychologists of various schools and anyone interested in new and recurring trends in human development in the world.

Feeding problems occur in around 50% of children with neurological disability. Needing help with feeds, choking, prolonged meal times, and poor weight gain are common. Gastro-oesophageal reflux and constipation may further complicate management, and obesity is a risk particularly in those with low energy requirements. Assessment should include a detailed feeding history and anthropometry. Oromotor skills and safety of swallow can be gauged by a speech and language therapist, and appropriate seating and eating tools by an occupational therapist. The dietician advises on nutritional requirements and how these can be met. Tube feeding is indicated for an unsafe swallow, recurrent aspiration pneumonia, and when an adequate energy intake cannot be achieved despite optimizing oral intake. On rare occasions, parenteral nutrition may be considered if gastrointestinal dysmotility progresses to the point that enteral feeding is no longer possible. This raises difficult ethical issues and early involvement of a palliative care team is highly recommended.

Cervical and lumbar spine pain are major causes of disability and distress. Careful assessment is needed of the nature and extent of the problem, for diagnosis and exclusion of important (treatable) differential diagnoses, and for the formulation and engagement of the patient in an appropriate treatment plan. Acute spinal pain frequently does not indicate underlying joint pathology. Chronic spinal pain is often associated with intervertebral disc disease or which is often classified together with facet joint osteoarthritis as spondylosis. Sciatica, brachalgia, or spinal claudication may each be a consequence of either spondylosis or intervertebral disc prolapse. Simple mechanical low back and neck pain may respond well to conservative management with analgesics and physiotherapy. Specific spinal problems, such as neuronal compromise, may require additional treatments. The roles of injections and surgery in the management of spinal pain continue to evolve. Although ongoing management is largely determined by the individual's clinical response, comprehensive health economic analyses inform healthcare policies which may limit treatment availability. Many people with spinal problems suffer long-term or recurrent pain and disability, with significant psychological and social impact. Multidisciplinary approaches are needed to facilitate pain management and enable people with spinal pain to lead fulfilling lives when the underlying condition cannot be cured.

Recovery from critical illness may not be smooth and uneventful for the patient. Complications and adverse events may beset the patient and lead to intensive care unit (ICU) re-admission. Problems upsetting patients after discharge may be a manifestation of post-intensive care syndrome, new or recurrent organ failure. Avoiding post-ICU complications may be prevented by ensuring a well-planned transition from ICU to the general ward. This may be achieved by minimizing the impact and duration of organ support, defining a structured rehabilitation programme prior to ICU discharge. After discharge a short period spent on an intermediate care unit with higher nursing staff ratios, together with follow-up on the general ward will help identify and treat any new problems. Unfortunately, re-admission may be required to effectively treat some complications. There are recognized risk factors for ICU re-admission and these should be used to warn of the increased likelihood of adverse events to aid appropriate planning avoiding re-admission to ICU.

A metabolic myopathy may be an overlooked etiology of recurrent exercised-induced weakness. This chapter discusses the clinical features and diagnostic considerations for adult patients with suspected metabolic myopathy. Adult patients with carbohydrate or fatty acid disorders typically have a history since childhood of exercise-induced myalgias, cramps, and fatigue. Episodes of myoglobinuria tend to present later, usually in the second decade. For any unexplained myopathy, especially if there is concern for a metabolic myopathy, there should also be consideration of a mitochondrial disease. Many mitochondrial disorders present with multiple neurological problems, such as deafness, seizures, encephalopathy, ptosis, or ophthalmoplegia. This chapter will discuss the complexities of genetic testing and muscle biopsy for the evaluation of a possible metabolic myopathy.

Stephen King’s The Shining (1977) is one of the most popular horror novels of all time. It tells the story of a family snowed in at a haunted hotel. The father, Jack Torrance, dreams of literary success and succumbs to the evil ghosts and supernatural forces of the hotel, eventually attacking his own family. This chapter argues that the central conflicts of the novel have their roots in human nature, reflecting evolutionarily recurrent adaptive problems of balancing conflicting evolved motives, most crucially motives for dominance versus motives for prosociality, and of surviving the hostile forces of nature. Moreover, the supernatural elements of the novel resonate with evolved cognitive dispositions for magical thinking and metaphysical dualism. The Shining effectively evokes and explores biologically salient conflicts and fears, offering a compellingly hard-nosed but ultimately optimistic perspective on those conflicts and fears.

Renal colic is the most common non-obstetric cause for abdominal pain and hospitalization during pregnancy. Ureteric stones occur in about 1 in 2,000 pregnancies, most (>80%) in the second and third trimesters. Primary management concerns are diagnostic foetal radiation exposure and the potential for adverse perinatal events arising either from the stone or from intervention. Indications for intervention are the same as for the non-pregnant patient, but are influenced by obstetric circumstances. Active treatment options may be temporizing (stent or nephrostomy) or definitive (ureteroscopic stone extraction). Historically, temporizing measures were the only recommended treatment option. However, potential problems associated with temporary drainage mechanisms include recurrent obstruction, infection, nephrostomy displacement, encrustation, infection, and pain. These factors may impact on pregnancy. In recent years, advances in surgical technology and technique have permitted definitive ureteroscopic management of stones during pregnancy.
