Abstract Motivation Reconciliation between a host and its symbiont phylogenies or between a species and a gene phylogenies is a prevalent approach in evolution, however no simple generic tool (i.e. virtually usable by all reconciliation software, from host/symbiont to species/gene comparisons) is available to visualize reconciliation results. Moreover there is no tool to visualize 3-levels reconciliations, i.e. to visualize 2 nested reconciliations as for example in a host/symbiont/gene complex. Results Thirdkind is a light and easy to install command line software producing svg files displaying reconciliations, including 3-levels reconciliations. It takes a standard format recPhyloXML as input, and is thus usable with most reconciliation software. Availability and implementation https://github.com/simonpenel/thirdkind/wiki. IntroductionElectronic medication reconciliation systems are known to reduce medication errors. We hypothesised that refinement of the electronic medical record (EMR) and provider education could improve adherence to completion of admission medication reconciliation, thereby potentially limiting prescribing errors. Our goal was to improve the percentage of patients with medication reconciliation completed within 24 hours of admission to at least 90%.MethodsA prospective interventional study was conducted at a university-affiliated community hospital between 1 January 2017 and 30 September 2018. We determined the baseline percentage of medication reconciliations performed within 24 hours of admission, and those completed at any time prior to discharge from the hospital. Three plan-do-study-act cycles were then performed, with interventions including live and email reminders to complete medication reconciliation and addition of a column to EMR patient lists indicating whether reconciliation had been completed.ResultsThe percentage of medication reconciliations completed within 24 hours of admission was lowest for the pre-intervention cycle (62.4%) and was highest for Cycle 3 (80.9%). The percentage of reconciliations completed any time prior to discharge was higher and increased in a similar stepwise fashion from 71.1% to 88.4% through Cycle 3. There was a post-intervention trend toward a higher rate of reconciliation completion for patients aged 18–40. Male patients were also more likely to have their admission medication reconciliations completed prior to discharge.ConclusionOur interventions resulted in a statistically significant 18.5% increase in the rate of admission reconciliation completion. Though this increase fell short of our goal, this study demonstrates that provider education and optimisation of the EMR can sustainably improve adherence with medication reconciliation, thereby fostering improved patient care. Further improvement could be achieved by focusing on the medication lists of our older patients and female patients.

Comparative genomic studies are often conducted by reconciliation analyses comparing gene and species trees. One of the issues with reconciliation approaches is that an exponential number of optimal scenarios is possible. The resulting complexity is masked by the fact that a majority of reconciliation software pick up a random optimal solution that is returned to the end-user. However, the alternative solutions should not be ignored since they tell different stories that parsimony considers as viable as the output solution. In this paper, we describe a polynomial space and time algorithm to build a minimum reconciliation graph — a graph that summarizes the set of all most parsimonious reconciliations. Amongst numerous applications, it is shown how this graph allows counting the number of non-equivalent most parsimonious reconciliations.

Streszczenie:
Duplication-Transfer-Loss (DTL) reconciliation is a widely used computational technique for understanding gene family evolution and inferring horizontal gene transfer (transfer for short) in microbes. However, most existing models and implementations of DTL reconciliation cannot account for the effect of unsampled or extinct species lineages on the evolution of gene families, likely affecting their accuracy. Accounting for the presence and possible impact of any unsampled species lineages, including those that are extinct, is especially important for inferring and studying horizontal transfer since many genes in the species lineages represented in the reconciliation analysis are likely to have been acquired through horizontal transfer from unsampled lineages. While models of DTL reconciliation that account for transfer from unsampled lineages have already been proposed, they use a relatively simple framework for transfer from unsampled lineages and cannot explicitly infer the location on the species tree of each unsampled or extinct lineage associated with an identified transfer event. Furthermore, there does not yet exist any systematic studies to assess the impact of accounting for unsampled lineages on the accuracy of DTL reconciliation. In this work, we address these deficiencies by (i) introducing an extended DTL reconciliation model, called the DTLx reconciliation model, that accounts for unsampled and extinct species lineages in a new, more functional manner compared to existing models, (ii) showing that optimal reconciliations under the new DTLx reconciliation model can be computed just as efficiently as under the fastest DTL reconciliation model, (iii) providing an efficient algorithm for sampling optimal DTLx reconciliations uniformly at random, (iv) performing the first systematic simulation study to assess the impact of accounting for unsampled lineages on the accuracy of DTL reconciliation, and (v) comparing the accuracies of inferring transfers from unsampled lineages under our new model and the only other previously proposed parsimony-based model for this problem.

ABSTRACTThe reconciliations in PKPU (Debt Payment Obligation Postponement) offers full or half of the debt payment methods in order to avoid bankruptcy, the reconciliation are legally applied and binding the parties needed for reconciliation validation from the Commercial court. Yet in practice, there found indications of reconciliation’s approval delay which has been approved by the parties as the result of reports that has yet to submitted to the committee in the time of PKPU meanwhile by the judge with the verdict of time extention in the form of PKPU still deviate from the legislation, so that it leads to the validity of the reconciliation. This articles is decriptive analytical, with the purpose to see the the delay in impelementation validation in PKPU by the judge in Commercial Court and the legal consequences of the said reconciliation’s aproval delay and its relation to the principle of legal certainty. This research found that the implementation of the ratification reconciliation in PKPU begins with a vote of the creditors and obtains permanent legal force through the decision of ratification reconciliation by the Commercial Court. According to the UUKPKPU, the postponement of the ratification of the reconciliation of the Commercial Court may postpone and set a trial date no later than 14 (fourteen) days after the postponement. The provisions governing this period of time are formal substance, the procedural law of Bankruptcy and PKPU which cannot be overruled by judges, bind, and provide legal certainty for the parties. Delaying the ratification of the reconciliation in the PKPU resulted in the settlement of debtor’s debt to creditors to be delayed, because the reconciliation had not been legally enforce and binding on the parties. The ratification reconciliation that violates Article 284 paragraph (3) of the UUKPKPU, results in the reconciliation being invalid and the debtor being declared bankrupt. Keywords: legal certainty; PKPU; reconciliation; validation.

OBJECTIVE To evaluate the clinical effect and estimate cost avoidance attributed to a pharmacist-led admission medication reconciliation service at a children's hospital. METHODS This was a prospective observational cohort study that measured pharmacist interventions for pediatric patients over a 90-day period. Pharmacists logged all interventions identified during medication reconciliation in real time. Patient demographic data were collected retrospectively. Cost avoidance from prevented adverse drug events (ADEs) was estimated based on previously published literature. RESULTS Pharmacists completed 283 admission medication reconciliations during the study period. Of those, 69% of medication reconciliations required intervention. Interventions affected care during the hospital admission in 21.9% of patients and 8 medication reconciliations resulted in prevention of a major ADE. This pharmacist-led service resulted in an estimated cost avoidance of $46,746.65 in the 3-month period. CONCLUSIONS Implementation of a pharmacist-led admission medication reconciliation service for pediatric patients improved medication safety and resulted in significant cost avoidance, which justifies investment in these pharmacist resources.

This paper investigates whether the increases and decreases to earnings and stockholders' equity presented in 20-F reconciliations influence perceptions of the risk of investing, the quality of the accounting principles, and the financial performance of the reporting firm. The research results indicate that subjects perceive the risk of a hypothetical firm filing a 20-F reconciliation with reconciliation decreases to be higher, and the quality of accounting principles lower, than a hypothetical firm either complying with U.S. GAAP or filing a 20-F reconciliation with reconciliation increases. Additional analysis suggests that these significant effects are due to a negative effect from the reconciliation decrease consistent with the effects predicted by attribute framing theory. The findings have implications for the SEC as they consider whether foreign registrants on U.S. exchanges should be allowed to comply with International Accounting Standards (IAS) without reconciliation to U.S. GAAP. These findings also have implications for foreign registrants on U.S. exchanges. The results indicate that firms that have higher non-U.S. GAAP earnings (and a consequent reconciliation decrease) should comply with U.S. GAAP as the reporting of a reconciliation decrease creates a negative perception of the firm.

This paper introduces the concept of micro-reconciliation as a pragmatic action to support cultural safety and humility work. Similar to cultural safety and humility, micro-reconciliation practices aim to challenge and diminish racism, inequality and inequity experienced by Indigenous peoples. In arguing for changes to the human service sector, micro-reconciliation exists at the intersections between entrenched structural racism and the psychological and emotional roots of discrimination that play out in every day service delivery. Three organizing practices are discussed; acknowledment, witnessing and moral courage, as the basis of micro-reconciliaion work and the advancement of cultural safety and reconciliaton.

Highly cooperative social species are expected to engage in frequent reconciliation following conflicts in order to maintain pack cohesiveness and preserve future cooperation. By contrast, in social species with low reliance on cooperation, reconciliation is expected to be less frequent. Here, we investigate the pattern of reconciliation in four captive wolf packs and four captive dog packs. We provide evidence for reconciliation in captive wolves, which are highly dependent on cooperation between pack members, while domestic dogs, which rely on conspecific cooperation less than wolves, avoided interacting with their partners after conflicts. Occurrence, intensity, latency, duration and initiation of wolf reconciliations appeared to vary as a consequence of a compromise between the costs (e.g. risk of further aggression) and the benefits (e.g. restoring relationship with opponents) of such interactions. Our results are in line with previous findings on various wolf packs living under different social and ecological conditions, suggesting that reconciliation is an important strategy for maintaining functional relationships and pack cohesiveness. However, current results on dogs are in contrast to the only other study showing that reconciliation can occur also in this species. Therefore, the occurrence of reconciliation in dogs may be influenced by social and environmental conditions more than in wolves. Which factors promote and modulate reconciliation in dogs needs to be further investigated.

Abstract Background Penicillin allergy reconciliation is an important aspect of antimicrobial stewardship with ~10% of the population reporting a penicillin allergy. Our facility utilizes a Penicillin Allergy Reconciliation Program (PARP) led by an Infectious Diseases (ID) Pharmacist and pharmacy students to identify patients with penicillin allergies to reconcile and intervene when necessary. Information is collected by interview, electronic medical record (EMR) review, prescription outpatient fill history. This study evaluated reconciliations with and without a PARP in patients in a community health system. Methods This was a retrospective study that compared reconciliations performed on adult patients admitted at least once in 2019 with a self-reported penicillin allergy and ID physician consult at a hospital with a PARP (Institution 1) and one without a formal evaluation and intervention program (Institution 2) within the same community health system with same ID physicians. The primary outcome was documented reconciliation of a patient’s penicillin allergy during an inpatient visit in 2019. Reconciliation was defined as an edit or clarification (updating the severity, reaction, or comments section, as well as deleting) to a patient’s penicillin allergy in the EMR. The secondary outcome evaluated the percentage of total and ID consult patients with a penicillin allergy. Results There were 245 patients who met criteria and were included in the study, 113 from Institution 1 and 132 from Institution 2. For the primary outcome, there were 82 (72.6%) reconciliations at Institution 1 and 15 (11.4%) reconciliations at Institution 2 (p < 0.001). Interventions at Institution 1 and 2 resulted in 74 EMR updates and 8 removals and 14 EMR updates and 1 removal, respectively. Reconciliation was performed on the same visit as the ID consult in 59/82 patients (72%) at Institution 1 and 11/15 patients (73.3%) at Institution 2. All reconciliations at Institution 2 were made by pharmacist (10) or nurses (5). For the secondary outcome, 10.9% of patients with an ID consult and 12.6% of all patients admitted in 2019 had a penicillin allergy (p=0.027). Conclusion A PARP led by an ID pharmacist and students was an effective method to perform penicillin allergy reconciliations, even in the presence of active ID consultation. Disclosures Bruce M. Introduction: How to improve medication reconciliation has been an ongoing discussion in hospitals across the nation. This study was designed to identify areas for potential improvement in the medication reconciliation process for an 80-bed, inpatient psychiatric hospital. A previous evaluation conducted at the site indicated that 45% of medication reconciliations were correct. Subsequently, a new process was developed to improve this area of patient care. This process included an update to existing medication-reconciliation forms, staff education, and the standardization of all protocols involved. The investigators examined the updated process to identify gaps in patient care during the admission medication-reconciliation process. Methods: The primary outcome of the study was an assessment of the accuracy of the updated medication-reconciliation protocols. Data, including medication, dosage, route, and frequency, were collected from randomly selected patients (13 years and older) admitted during the 2-month study period. These data were collected at least 24 hours after admission. Patients were interviewed and their home pharmacy was contacted to determine whether the information collected during the initial medication-reconciliation process was correct. Results: The investigator identified 44 patients during the collection period and compared the results to the previous study, before the enhancements in the medication-reconciliation process. The accuracy of medication reconciliation in this analysis was 80%, compared with 45% from the previous study (P = 0.0011). Discussion: Personnel training and protocol updates led to a statistically significant increase in the accuracy of the hospitals medication-reconciliation process. Ongoing staff education and assessment of the improved protocol may further increase accuracy in the medication-reconciliation process at this hospital.

Background: Medication changes are common after hospitalizations, and medication reconciliations are one tool to help identify potential medication discrepancies. Objective: To determine the impact of a pharmacy-driven medication reconciliation service on number of medication discrepancies identified. Methods: This was a retrospective cohort, chart-review study conducted at an internal medicine outpatient clinic. Patients at least 18 years of age were eligible for inclusion if they presented for a hospital follow-up appointment within 14 days of discharge between September 1, 2015, and May 31, 2016, from a system hospital. The 2 cohorts were patients with a pharmacist-completed medication reconciliation note written in the electronic health record on the date of their hospital follow-up appointment and those without. The primary outcome was number of medication discrepancies identified during medication reconciliation. Secondary outcomes included types of discrepancies, 30-day hospital readmission, and 30-day emergency department visits. This study was approved by the facility institutional review board. Results: Seventy-nine patients were included, and 38 patients had a pharmacist-completed medication reconciliation (48%). A total of 64 medication discrepancies were identified in 26 patients; of these, 49 discrepancies were resolved during the appointment (77%). There was an average of 2.46 medication discrepancies (±2.34) per patient. The most common discrepancy was missing medications. Thirty-day readmission rate was 5.3% in the intervention group and 19.5% in the control group ( P = .054). Conclusions: A pharmacist-completed medication reconciliation identified many medication discrepancies that were then resolved. From this study, pharmacist-led medication reconciliations following hospital discharge appear valuable.

Abstract The article sets out to stir up the debate on reconciliation project in the post colonial Africa. As we strategise on ways and means of delivering the promise of reconstruction, there is need to pay more attention on the reconciliation for individual and society. In other words, does reconciliation mean blanket forgiveness or reparation? How can we ensure that those who looted Africa account for their misdeeds without further complicating the situation? The article is set on the premise that even though there are many paradigms in African theology of the twenty-first century, minor paradigms (refer to reconciliation, liberation, inculturation, market-theology and charismatic among others) and the dominant paradigm (refer to reconstruction) are both critical in the holistic rebuilding of the post colonial Africa. This said; it is imperative to critically assess reconciliation as an important paradigm – as it runs concurrently with other paradigms in Africa today. In particular, are the Truth, Justice and Reconciliation Commissions taking place in various countries of the tropical Africa, since Tutu’s South African sample of 1995, rooted in African cultural and religious heritage, and hence authentic? How can Africa go about her reconciliative phase?

Reconciliation has been a topic of major importance in both the German and South-African theological and political contexts. Political events involving the Christian Churches in these countries have prompted a renewed consideration of what is understood by the concept. This article examines some aspects of reconciliation in the contexts of Christian ethics and argues that, from a theological and historical perspective, the nature and practice of what was called reconciliatio is thoroughly religious, although what exactly is involved has been perceived differently over the course of Church history. Reference is also made to the New Testament understanding of the concept and in an attempt to actualise the biblical kerugma, several propositions regarding the significance of a religious view of reconciliation in an eschatological context are provided as a conclusion.

AbstractChristian public theology extends reconciliation beyond its principal sacramental concern for relationships between God and penitent to the construction of ‘socially just’ public relationships for the settlement of intra-national conflict. In theological terms, reconciliation brings public relationships into what Hally calls ‘the Christ narrative of passion, death and resurrection’ in which the perpetrators of injustice repent and seek forgiveness. This article introduces the conflicts that these discourses aim to resolve in Australia, Fiji and New Zealand and explains and contrasts reconciliation’s relative importance in each of these jurisdictions. Moreover, the article’s cross-jurisdictional comparison shows reconciliation’s limits and possibilities as public theology, and argues that in Australia and New Zealand it has helped to create political environments willing to admit indigenous perspectives on a range of policy issues. On the contrary, however, the article also shows that the Fijian churches have distorted the concept of reconciliation to support political imperatives that are difficult to rationalize theologically, even though they are presented by the churches as being concerned with religious goals.

SYNOPSIS: On November 15, 2007, the U.S. Securities and Exchange Commission (SEC) eliminated the requirement that foreign private issuers reporting under International Financial Reporting Standards (IFRS) include a reconciliation to U.S. GAAP in their 20-F filing. To the extent that the reconciliations had information content, it is possible that the information environment of IFRS filers deteriorated in the post-reconciliation period, unless they voluntarily improved disclosure quality. Using difference-in-differences tests, we examine whether there was any change in the persistence of earnings and analyst forecast dispersion after the new regulation. We find that earnings persistence increased (did not increase) and analyst uncertainty measured by the forecast dispersion did not increase (increased) for firms domiciled in weaker (stronger) investor protection countries. These results suggest that firms from a weaker investor protection environment had a greater incentive to “signal” the quality by voluntarily improving the disclosure quality in the post-reconciliation period to compensate for any possible information loss from no longer providing the reconciliation. Our findings also suggest that the elimination of the reconciliation requirement did not have a uniform effect on IFRS filers and that the effect varies with the firm's home country reporting environment. AbstractIn this article, the late Professor Efemie Ikenga-Metuh reflects on the basic message that Pope John Paul II delivered on the occasion of his 1998 visit to Nigeria. That message, says Metuh, could be summarized in two words: Reconciliation and Dialogue. It will only be when Nigerians begin to dialogue frankly and respectfully with one another that they will be able to come to reconciliation. And in reconciliation is the key to deliverance from the situation of corruption and injustice in which Nigeria is still so sadly held hostage.

AimPatients are more likely to experience a ‘medicines-related safety incident’ when medicines reconciliation happens more than 24 hours after admission to an acute setting,1 according to the National Institute for Health and Care Excellence (NICE). The study aimed to assess the impact on medicine reconciliations following the introduction of a dedicated Paediatric Medicines Management Pharmacy Technician to the paediatric wards at a District General Hospital (DGH).MethodsData has been routinely collected by the pharmacy department over of a number of years showing the time of medicines reconciliations compared with the time of hospital admission. This data shows the number of medicine reconciliations that were completed within 24 hours of hospital admission and the number that were not completed within 24 hours. The data is routinely collected on the Thursday of the first full week of every month. All patients that were admitted to the paediatric wards were included in this data. The service is only funded Monday to Friday through the Child Health Department of the DGH. This data excludes neonates admitted to the Neonatal Intensive Care Unit. Data was collected from 83 paediatric patients in March/April/May 2017 and 78 paediatric patients in March/April/May 2019.ResultsData collected for the paediatric patients over March/April/May 2017 showed that around 21.7% of all paediatric patients admitted to the wards had a completed medicines reconciliation within 24 hours. The data collected over the same period in 2019 showed that 85% paediatric patients admitted to the wards had a completed medicines reconciliation within 24 hours.ConclusionThis study was useful in demonstrating the effectiveness of introducing a dedicated Paediatric Medicine Management Pharmacy Technician to the paediatric wards in a DGH. It showed that the proportion of medicine reconciliations within 24 hours prior to the change was very low, but after the change it was very high with nearly all patients having a completed medicines reconciliation within 24 hours. Prior to the introduction of a dedicated Paediatric Medicines Management Pharmacy Technician, the paediatric wards at this DGH were not meeting the standards set by NICE regarding medicines reconciliations within 24 hours of being admitted to an acute setting. After the introduction the paediatric wards were meeting these standards. By meeting NICE guideline QS120 Medicines Optimisation, the DGH has reduced the likelihood of medicines-related safety incidents. With the introduction of a dedicated Paediatric Medicines Management Pharmacy Technician there have been many other benefits. These include counselling to parents/children on the use of their medicines; checking of patients’ own medicines to see if they are still fit for purpose; advice to parents about unlicensed medicines and why they are used; where to obtain further supplies when new medicines have been started; and assisting parents and GP surgeries with any supply issues.ReferenceNational Institute for Health and Care Excellence. SYNOPSIS: This study evaluates the extent to which the FASB and IASB convergence projects and the EU-wide adoption of IFRS have impacted the differences between firms’ financial results under U.S. GAAP and IFRS. Using 2004 to 2006 reconciliation disclosures of 75 EU cross-listed firms, we find that the average gap between U.S. GAAP and IFRS income and between U.S. GAAP and IFRS shareholders’ equity declined from 2004 to 2006, consistent with convergence, though the net income gap remains significant. Although both pensions and goodwill are included in the convergence projects, these adjustments appear to be the dominant reconciliation items. Across the EU, net income and shareholders’ equity reconciliation amounts differ significantly by industry and by legal origin of the firm’s home country, raising questions about the homogeneity of IFRS as implemented. Furthermore, most firms report IFRS net income (shareholders’ equity) higher (lower) than U.S. GAAP net income (shareholders’ equity), with the result that 28 percent of the sample firms’ 2006 ROE under IFRS is more than 5 percentage points higher than under U.S. GAAP, whereas fewer than 10 percent of the sample report ROE more than 5 percentage points lower. Finally, shareholders’ equity reconciliations and income reconciliations appear value-relevant, although results are somewhat sensitive to model specification. Overall, our findings indicate that significant numerical differences still exist between results under IFRS and U.S. GAAP, despite convergence. In light of the SEC’s elimination of the requirement for reconciliations between the two sets of standards and the potential adoption of provisions allowing U.S. firms to choose between the standards, investors and other financial statement users should be aware of the significant numerical differences.

Medication reconciliation is an important component to the care of hospitalised patients and their safe transition to the ambulatory setting. In our Family Medicine Hospitalist Service, patient care is frequently transferred between the various physicians, residents, nurses and eventually to a separate group of providers who provide ambulatory management. Due to frequent transitions of care, there was no clear ownership of the medication reconciliation process. To improve the medication reconciliation process, a Transition of Care Team composed of registered nurses was created to oversee the entire reconciliation process. The team engaged the patient and their family, when needed, contacted patients’ pharmacies and their providers, reconciled the patients’ hospital medication list with the ambulatory list at hospital admission and within 24 hours of discharge, and attended the hospital follow-up visit to verify medications and provide continuity of care. Implementation of the team allowed for additional investigative resources, redundancy in preventing errors and early recovery should an error occur. The percent of medications with error after implementation of the Transition of Care Team was reduced from 131/386 (33.9%) to 147/787 (18.7%) at hospital admission, 81/354 (22.9%) to 42/834 (5.0%) at discharge and 43/337 (12.8%) to 6/809 (0.7%) at follow-up visit (two proportion tests, p<0.001). In addition, the percent of charts without any errors improved at hospital discharge from 8/31 (25.8%) to 46/70 (65.7%) and at hospital follow-up visit from 16/31 (51.6%) to 64/70 (91.4%) (two-proportion test, p<0.001). Previously viewed as three separate reconciliations occurring at admission, discharge and hospital follow-up, the approach to medication reconciliation was reframed as a continuous process occurring throughout the hospitalisation and hospital follow-up resulting in improved reconciliation accuracy and safer transitions to the ambulatory setting.

ABSTRACT This paper examines shareholder wealth effects in U.S. and home-country markets relating to the Securities and Exchange Commission's (SEC) decision to eliminate the Form 20-F reconciliation. During the period of examined events, we find positive cumulative abnormal returns for the treatment sample of U.S. cross-listed firms that prepare financial statements under International Financial Reporting Standards (IFRS), but no such effects for our control sample comprising cross-listed non-IFRS, U.S. domestic, or home-country firms. We find the stock market impact for our treatment sample to be positively related to our proxy for cost savings and negatively related to the pre-adoption reconciliation magnitude from IFRS to U.S. GAAP. This suggests shareholders place some value on reconciliation information, but the costs of preparing and auditing reconciliations generally outweigh concern about information loss. Moreover, we find that information loss is less pronounced for firms having used IFRS for a longer period, suggesting the learning effect mitigates information loss. Objectives: To integrate fourth-year student pharmacists on advanced pharmacy practice experience (APPE) rotations within several different ambulatory clinics to perform medication reconciliations and enhance interdisciplinary practice. Methods: The study design was a descriptive, prospective multisite study among a variety of ambulatory care outpatient clinics. Student pharmacists were partnered with physicians to conduct medication reconciliations during clinic visits for 4 hours per week, with data collection from January 2016 to September 2016. This program originated from physician requests for additional pharmacy involvement and led to the development of a successful implementation strategy to involve student pharmacists in the medication reconciliation process. Results: Student pharmacists identified 537 medication discrepancies among 491 patients, including commission of medications (36%), documentation of previously omitted medications (27%), and incomplete patient allergy information (11%). Students spent an average of 10 minutes on each encounter. Conclusion: Documentation from this innovative program suggests improvement in medication reconciliation and enhanced patient care with limited time required of student pharmacists. A similar program could be developed and utilized at other clinical sites.

While SFAS No. 131 is intended to increase the transparency of financial reporting using a management approach, it may reduce shareholders ability to interpret segment disclosures relative to the industry approach employed under SFAS No.14. This study investigates whether segment reconciliation differences affect stock prices and whether abnormal returns can be earned using information about two components of earnings: aggregated segment earnings and segment earnings reconciliations. We compute reconciliations as the difference between firm-level consolidated earnings and aggregated segment-level earnings. Firms that report negative SERs have greater sales and profitability, greater return on equity, as well as more operating cash flows and firm growth. This suggests that firms that report aggregated segment earnings greater than firm-level consolidated earnings may be better off financially. Our findings show that mispricing does occur when firms report positive SERs by the market, underestimating the segment earnings reconciliation component of earnings persistence. Investors can also earn positive abnormal returns when investors take a long (short) position with the portfolio with the highest (lowest) absolute SERs. On the contrary, we find investors earn negative abnormal returns when firms report negative SERs. Collectively, this study provides evidence that mispricing occurs and that investors over/underestimate the importance and/or persistence of segment earnings reconciliations.

Streszczenie:
The rooting of phylogenetic trees permits important inferences about ancestral states and the polarity of evolutionary events. Recently, methods that reconcile discordance between gene-trees and species-trees—tree reconciliation methods—are becoming increasingly popular for rooting species trees. Rooting via reconciliation requires values for a particular parameter, the gene transfer to gene duplication ratio (T:D), which in current practice is estimated on the fly from discordances observed in the trees. To date, the accuracy of T:D estimates obtained by reconciliation analyses has not been compared to T:D estimates obtained by independent means, hence the effect of T:D upon inferences of species tree roots is altogether unexplored. Here we investigated the issue in detail by performing tree reconciliations of more than 10,000 gene trees under a variety of T:D ratios for two phylogenetic cases: a bacterial (prokaryotic) tree with 265 species and a fungal-metazoan (eukaryotic) tree with 31 species. We show that the T:D ratios automatically estimated by a current tree reconciliation method, ALE, generate virtually identical T:D ratios across bacterial genes and fungal-metazoan genes. The T:D ratios estimated by ALE differ 10- to 100-fold from robust, ALE-independent estimates from real data. More important is our finding that the root inferences using ALE in both datasets are strongly dependent upon T:D. Using more realistic T:D ratios, the number of roots inferred by ALE consistently increases and, in some cases, clearly incorrect roots are inferred. Furthermore, our analyses reveal that gene duplications have a far greater impact on ALE’s preferences for phylogenetic root placement than gene transfers or gene losses do. Overall, we show that obtaining reliable species tree roots with ALE is only possible when gene duplications are abundant in the data and the number of falsely inferred gene duplications is low. Finding a sufficient sample of true gene duplications for rooting species trees critically depends on the T:D ratios used in the analyses. T:D ratios, while being important parameters of genome evolution in their own right, affect the root inferences with tree reconciliations to an unanticipated degree.

Streszczenie:
Information reconciliation is a crucial procedure in the classical post-processing of quantum key distribution (QKD). Poor reconciliation efficiency, revealing more information than strictly needed, may compromise the maximum attainable distance, while poor performance of the algorithm limits the practical throughput in a QKD device. Historically, reconciliation has been mainly done using close to minimal information disclosure but heavily interactive procedures, like \textit{Cascade}, or using less efficient but also less interactive ---just one message is exchanged--- procedures, like the ones based in low-density parity-check (LDPC) codes. The price to pay in the LDPC case is that good efficiency is only attained for very long codes and in a very narrow range centered around the quantum bit error rate (QBER) that the code was designed to reconcile, thus forcing to have several codes if a broad range of QBER needs to be catered for. Real world implementations of these methods are thus very demanding, either on computational or communication resources or both, to the extent that the last generation of GHz clocked QKD systems are finding a bottleneck in the classical part. In order to produce compact, high performance and reliable QKD systems it would be highly desirable to remove these problems. Here we analyse the use of short-length LDPC codes in the information reconciliation context using a low interactivity, \textit{blind}, protocol that avoids an a priori error rate estimation. We demonstrate that $2 \times 10^3$ bits length LDPC codes are suitable for blind reconciliation. Such codes are of high interest in practice, since they can be used for hardware implementations with very high throughput.

A large portion of the recent performance increase in the High Performance Computing (HPC) and Machine Learning (ML) domains is fueled by accelerator cards. Many popular ML frameworks support accelerators by organizing computations as a computational graph over a set of highly optimized, batched general-purpose kernels. While this approach simplifies the kernels’ implementation for each individual accelerator, the increasing heterogeneity among accelerator architectures for HPC complicates the creation of portable and extensible libraries of such kernels. Therefore, using a generalization of the CUDA community’s warp register cache programming idiom, we propose a new programming idiom (CoRe) and a virtual architecture model (PIRCH), abstracting over SIMD and SIMT paradigms. We define and automate the mapping process from a single source to PIRCH’s intermediate representation and develop backends that issue code for three different architectures: Intel AVX512, NVIDIA GPUs, and NEC SX-Aurora. Code generated by our source-to-source compiler for batched kernels, borG, competes favorably with vendor-tuned libraries and is up to 2× faster than hand-tuned kernels across architectures.
