A nursing perspective following McIntyre and McDonald’s framework was used to unpack the complex issue of challenges faced by transgender people in the Canadian healthcare system, considering historical, ethical, legal, social, cultural, political, and economic perspectives. Transgender people have unique healthcare needs which are often misunderstood or unaddressed by healthcare professionals, leading to poorer outcomes and inequities. Issues concerning transgender people are becoming a focus and a higher priority for society. This literature review reveals the complexity of this issue as the roots in historical, ethical, legal, social, cultural, political, and economic contexts are explored. A variety of barriers and facilitators exist to addressing and resolving this issue, including transgender people avoiding healthcare, intolerance, lack of knowledge and understanding, lack of healthcare provider training, media representation, and economic costs. The analysis of this issue can be used to inform resolution strategies to utilize facilitators and overcome barriers, including increasing awareness and knowledge, improving education and healthcare provider competency, and utilizing nurse leaders as advocates, role models, and agents of change. Improving care of transgender people is a nursing leadership priority. By implementing the suggested resolution strategies, the healthcare system can begin to move towards a more inclusive, understanding, and holistic model of care to improve healthcare access and outcomes for transgender people.

Abstract Given the increased prominence of a border poll in Ireland, particularly following the outcome of the Brexit referendum, this paper provides an initial assessment of some of the issues that are likely to become central in any debate on this issue. We examine the relative income and growth position of Northern Ireland within a UK and Irish regional framework over time. We further compare, and contrast, in detail aspects of the structure of both economies on the island of Ireland in the areas of educational attainment, trade orientation and the role of Foreign Direct Investment. The paper goes on to analyse other relevant issues, such as the relative strength and weaknesses of the healthcare systems and the factors determining the potential economic cost of Irish unification. The objective of the research is to initiate an evidence-based approach to the question of a border poll and to provide an initial indication of the breadth of detail and analysis required for any debate to proceed in a meaningful manner.

The issue of health is seen as an accomplishment of harmonious integration of an individual with the entire universe, both spiritual and material. Thus understanding the issue of health and illness is predicted to hinge on a variety of diversified and distinctive factors such as cultural, geographical, social, political and economic dimensions. Due to this circumstance, many societies have a case to develop a healthcare system being acknowledged as an indispensable sphere to tackle a variety of health-and life-threatening ailments and diseases of their members. It is within this context that this paper reviews certain phenomena and factors galvanising the people in Nigeria to seek their healthcare needs in traditional healthcare which include its accessibility and acceptability, low cost treatment practitioners proximity, potency and efficacy, herbal formulations, enhancement of services and practices among others. The paper went further to highlight some emerging trends in this form of healthcare system.

The discipline of economics encompasses both theoretical concepts and empirical methods for the study of human behaviours, as well as competing schools of thought and ideologies that students need to engage with and reflect upon. The university economics lecturer has an important role in helping students accomplish this goal. However, as an individual, the lecturer also holds personal ideological positions that may emerge and elude L2 listeners. The aim of this paper is to propose a systematic and comprehensive procedure for detecting ideological stance in a university economics lecture. Using a case study approach, I apply Multimodal Critical Discourse Analysis to identify ideological stance and show how semiotic resources beyond verbal language contribute to its expression. This process was facilitated by the use of multimodal annotation software to analyse verbal expressions of ideological stance (e.g., evaluative language, rhetorical elements) and co-occurring non-verbal cues (e.g., prosodic features, gaze direction, gesturing). The method was able to shed light on how the lecturer’s contemporaneous use of multiple semiotic modes worked synergistically in the expression of ideological meanings in relation to a controversial issue (i.e., access to healthcare in the United States) in subtle but distinctive ways that were linked to the lecturer’s background and to the broader socio-political issues of the research context. The method could be leveraged to inform eSp settings to assist L2 learners in acquiring a better understanding of lecturers’ stance towards content, thus resulting in a more complete, effective, and satisfying lecture experience

The article looks at the state of the U.S. healthcare at the end of the pandemic which brought to light a range of systemic problems of the industry: low efficiency at extremely high cost, inequities to health care access and a strong divide in the American society on healthcare issues. The author examines the key factors that will shape the future of the industry. Among them are an uncertain economic situation and high inflation rates, the rollback of the Medicaid continuous enrollment provisions, the end of the public health emergency and the population aging. The divided Congress also won’t allow the democratic administration to realize large-scaled reforms announced by the President during the race. At the same time some provisions of the Inflation Reduction Act of 2022 will bring some changes to the industry. The analysis allowed the author to come to the following conclusions. American healthcare is experiencing a structural change. On the one hand, the system has to adapt to an endemic future. On the other hand, the difficult economic situation resulting from the COVID‑19 pandemic will be another stress test for the industry: shortages of medical personnel, unprofitable medical facilities, rising prices for medical care, and a likely increase of the uninsured. Many traditional players will need to review their operations. Non-traditional retail giants could change game rules and offer more efficient models for managing and health care delivery. Addressing the issue of equitable access to health care will require further intervention by the federal government. Healthcare will be one of the key topics in the 2024 presidential election.

Cross-border reproductive care is a complex issue of the modern world that also impacts the Russian Federation. The main reasons for engaging in cross-border reproductive care are various legal, social, cultural, economic and religious factors, as well as national healthcare quality. In many countries, reproduction involving third parties, i.e., their sperm, eggs and embryos, is prohibited by law. This is why gamete donation is one of the main causes of pursuing CBRC in a foreign country, with Russia holding leading positions in this industry. Current stage of healthcare development makes Russia a major surrogate tourism destination, due to its common European culture and improved public health standards. Besides, Russia, as a multiethnic state where all religions are represented, has the most liberal legislation concerning infertility treatment. Fertility tourists have the same rights as Russian citizens in terms of assisted reproduction procedures, including obtaining the birth certificate regardless of biological relation to the child.

This special issue aims to contribute to current knowledge held on mixed hospital systems from a historical perspective, as there is nowadays much debate on the sustainability and efficiency of public and private healthcare systems in the world in the COVID-19 pandemic. By focusing on the evolution of mixed hospital systems through the case study of the history of such systems in Catalonia in the last century, the authors of this special issue show that mixed hospital systems take a long period of time to be used, and trusted, by the population. It is also considered how public healthcare regulators can create a diversity of mechanisms that facilitate access by the population to healthcare services in times of external shocks such as pandemics. This introductory text begins with a section about the international context which explains the relevance of mixed hospital systems, which is followed by a summary of the main historical points regarding the Catalan model of mixed hospital provision since the 19th century. It also highlights the most significant contributions of the seven articles of this special issue, which consider how the Catalan society confronted social, economic, and political changes and how those actions led to configure a distinctive mixed model of hospital system. Finally, this text also sheds light on areas of research regarding the rich history of hospital healthcare that still need to be addressed.

For many decades, the Egyptian people have suffered from deplorable public healthcare service featured by indolence, malfunction and traditionalism. Although, healthcare is a distinctive service industry concerning various complicated responsibilities; but the consecutive government of Egypt had badly handled such issue. Then, the apathetic performance of the public healthcare service becomes a dilemma for the people of Egypt; especially the poor among them. Therefore, the foundation of an adequate public healthcare service system, that respect the dignity of the people and respond to their arising health care needs; was frequently on the agenda priorities of the Egyptian governments after the 2011 and 2013 uprisings. Hence recently, the government -after reaching political and economic stability- seeks to build an ambitious newly public health care system to meet the expectation of the people to acquire high standard inexpensive and hasty public healthcare services. Consequently, in order to realize such aim; the Egyptian government had established the public agency for accreditation and quality control according to law no.2 of comprehensive healthcare insurance system issued at 2018. Then, it urges a national campaign to reform the public healthcare sector and to develop the efficacy and quality of its services. Hence, this paper aims to propose how the public healthcare organizations in Egypt can tackle various challenges and enhance adequately its capabilities; in order to be able to adopt the proposed Lean Six Sigma (LSS) methodology effectively; which can then provide an adequate framework for creating organized improvement exertions in healthcare; necessary to bestow guidelines on how to manage a quality service system to patient satisfaction by decreasing waste, variation and work disparity in the service processes.

Poverty among women and girls remains a prevalent social justice and health issue that stunts the life potential and freedom of females throughout the globe. Through referencing four published articles, this text explores the incidence of poverty among women and girls due to gender discrimination, sexist ideologies and practices, and oppression on the basis of gender. Due to the presence of mechanisms that disproportionately generate poverty among females, many girls and women are automatically confined to a life that uniquely strips them of their inherent rights to dictate their future, and are instead forced into a life of perpetual suffering, violence, social exclusion, and ultimately, impoverishment. Examining this issue from a feminist lens is imperative in understanding the inner complexities of how women and girls in different areas of the world experience disadvantages on the basis of gender, especially from a social, political, cultural, and economic perspective. This can allow healthcare providers, such as nurses, to be able to examine such issues from a critical thinking lens, and become increasingly politically active and involved in female advocacy efforts and policy reform. Through nurses becoming increasingly involved in such efforts, dramatic positive change in the lives of women and girls throughout the globe can occur.

This thesis identifies and analyzes current problems in the healthcare market in China. Although many health indicators of China such as life expectancy and child mortality rates have improved significantly and suggest that the healthcare reform in China is successful, there have been many more reports of patient's violent attack towards the doctor, the sudden death of doctors, and decreasing doctor supplies that suggest otherwise. I observed that the relationship between doctors and patients are intense, doctors experience enormous working pressure, and many doctors are leaving the market. It makes me wonder how did government fail to improve its healthcare quality while health indicators suggest huge improvements. This thesis mainly focuses on how institutions contribute to the market inefficiencies. I hope this thesis will provide some insights for future reform policies.

This study offers insights into the impact of competition among Managed Care organizations (MCOs) on infant birthing charges and birth outcomes. Kentucky provides one of the nation’s first case studies to determine successes and failures of Medicaid MCOs, and by doing so, provides a prediction of the impact of Patient Protection Affordable Care Act (PPACA) competition on healthcare costs and birth outcomes. An analysis of a natural policy experiment in the state of Kentucky reveals that infants insured by a Medicaid MCO stay longer in hospitals, are less healthy, and cost more than those insured under Traditional Medicaid prior to a policy change. Utilizing a difference-in-difference-in-difference (DDD) estimation, this study found initial evidence in a competitive MCO environment of Traditional Medicaid average birth charges substantially more than births under a Medicaid MCO, while outcomes also revealed the incidence of normal delivery increased almost identical to that of private insurance. However, after a short time, average birth charges for infants born under Medicaid MCO climb higher than other payer-types and infant health begins to decline. Outcomes of this study signal that Managed Care infants are actually less healthy and cost substantially more than anticipated but it is possible that these outcomes can be attributed to insurance selection.

Two years after its signing into law, the Patient Protection and Affordable Care Act (PPACA), commonly called the Affordable Care Act (ACA), underwent a constitutional challenge at the Supreme Court. The Court’s decision to uphold the power of Congress in enacting most of the provisions of the ACA was regarded as a highly salient decision that is thought to have affected the public perception of the law. The focus of this thesis is to determine whether the Supreme Court was able to frame arguments used for or against the ACA in relation to the decision. By organizing and analyzing open-ended responses gathered from a panel survey both before and after the 2012 decision, I sought to determine how arguments used in discussion about the law and institutions regarding it varied after the decision. Findings show that the argument types used to explain individuals’ policy perceptions remained relatively stable throughout the waves. The study presented here offers an in-depth, micro-level effects of a real Court decision on individuals. By focusing on within-subject language change in a tight window around the decision, this approach helps clarify thinking about the relationship between the Court and public opinion.

Obesity has become a major issue in advanced societies having serious impacts on various social as well as economic levels. In addition to the personal costs of obesity leading to serious and chronic diseases, obesity projects additional burdens upon society including lack of productivity, often resulting in increased dependence on governmental benefits, as well as increasing health care costs, most of which are paid for by taxes. This phenomenon has become especially prevalent in the United States and the United Kingdom, with both countries attempting to reduce obesity levels with programs that utilize varying levels of paternalism. As the costs to these societies have become unsustainable, both have begun to experiment with behavioral insights to address the faults in their current programs. The resulting policy tool, known as a nudge, focuses on consumers' choice environments that contribute to poor health decisions and reframes the choice structures to intuitively encourage healthier choices. While both countries continue to study the use of nudges to reduce obesity, there are large differences in how each country has utilized nudges as a policy tool. For this large-scale problem and others like it, having the resources to develop policy solutions is not enough to resolve the problem: a country must be able to properly implement the solutions for them to be effective. This paper examines the development of nudging in each country and determines whether their differences in policy application reflect a broader problem of obesity-related policy intervention.

AbstractThe international movement of people through voluntary migration and forced displacement in the ASEAN is driven by economic inequalities within and between countries, and political and ethnic conflict. Using case examples of Malaysia and Thailand which espouse the neoliberal model of migration and development, and which are both guided by concerns of national security and economic efficiency in healthcare, this chapter appraises their contrasting models of health systems in the way healthcare access of migrant populations is conceptualised. Examining this issue from a comparative foundation and exploring the entitlement of a range of non-citizens to healthcare through the lens of documentation status, citizenship, and moral deservingness, this chapter demonstrates how borders move within national boundaries as neoliberal policies influence the normative frameworks underpinning health systems and the access of non-citizens to healthcare.

AbstractAn historic shift in focus on the quality and person-centeredness of health care has occurred in the last two decades. Accounts of results produced from reinvigorated attention to the measurement, management, and improvement of the outcomes of health care show that much has been learned, and much remains to be done. This article proposes that causes of the failure to replicate in health care the benefits of “lean” methods lie in persistent inattention to measurement fundamentals. These fundamentals must extend beyond mathematical and technical issues to the social, economic, and political processes involved in constituting trustworthy performance measurement systems. Successful “lean” implementations will follow only when duly diligent investments in these fundamentals are undertaken. Absent those investments, average people will not be able to leverage brilliant processes to produce exceptional outcomes, and we will remain stuck with broken processes in which even brilliant people can produce only flawed results. The methodological shift in policy and practice prescribed by the authors of the chapters in this book moves away from prioritizing the objectivity of data in centrally planned and executed statistical modeling, and toward scientific models that prioritize the objectivity of substantive and invariant unit quantities. The chapters in this book describe scientific modeling’s bottom-up, emergent and evolving standards for mass customized comparability. Though the technical aspects of the scientific modeling perspective are well established in health care outcomes measurement, operationalization of the social, economic, and political aspects required for creating new degrees of trust in health care institutions remains at a nascent stage of development. Potentials for extending everyday thinking in new directions offer hope for achieving previously unattained levels of efficacy in health care improvement efforts.

AbstractWho has – and who deserves – full or partial access to a state’s healthcare system? This question has become an issue of controversial political debate in many countries, not least in the context of recent crises such as the Covid-19 pandemic and, previously, the so-called ‘migration crisis’ of 2015–17. This chapter focuses on the latter crisis and its repercussions on the political regulation of healthcare access for a group at the very margins of society: asylum-seekers and refugees. By analysing recent asylum and health policy reforms in two main recipient countries in the EU, Germany and Sweden, the chapter sheds light on the intensifying level of politicisation of both health and incorporation policies in times of crisis.

AbstractLibya is a large country, ranking at fourth in terms of area both in the Arab world and the African continent (https://www.worldatlas.com/articles/which-are-the-10-largest-countries-of-africa-by-size.html). It is part of the World Health Organization–Eastern Mediterranean Regional Office (WHO–EMRO) region. Oil production is the main source of income which has transformed the country massively over the past 50 years and the healthcare system is one of the sectors that have improved significantly. The Health Act No (106), issued in 1973, guarantees free health services to all Libyans, with inevitable challenges regarding the delivery of adequate and sustainable services. The health system in Libya is a mix between the public sector and the private sector. The private sector is basically depending on funding through insurance companies and self-pay. It is not yet adequately developed but is striving and rapidly growing in the last two decades.The oncology services are accessible and available for all Libyans, most of the diagnostic and therapeutic facilities are of good standards and the modern treatments like immunotherapy and targeted therapies are also available. However, there were periods when the health care system—in general—was struggling to meet the increasing demand on health services and has seen considerable challenges, especially over the past few years due to the conflict, political, and economic instability of the country. This chapter covers the oncology care in Libya, describing the current state, challenges, and future directions.

AbstractHuman trafficking is a legal issue which can be found taught in standalone undergraduate modules and whose relevance reflects the volatility of today’s global world. Human trafficking is a criminal offence in most jurisdictions and consequently subject to judicial processes. However, it is also an example of a topic which is challenging to teach using legal frameworks alone. Drawing on a longstanding case study, this chapter argues that for students to make sense of the inherent complexity, law must take a backseat to the understanding of migration, criminology, policymaking, economics, employment and other key forces. That is the overconfidence that creating an Offence can solve an identified problem. The paper argues that there is a need to develop Learning and Teaching methods which cultivate adaptable and transferable skills which enable students to see the relationships between all of these elements, especially in light of recent trends (i.e. Covid-19 and the Fourth Industrial Revolution) that have markedly reshaped the socio-political landscape.

Since the late 1950s, Hong Kong's public health services have increased. They are mainly funded by taxes, supplemented by minimal user fees. In the late 1980s, the government recognized the limitations of this financing model and has subsequently proposed alternative methods of funding. Their proposals have been rejected by various stakeholders, who represent different, and even conflicting, values and interests. This chapter describes the development of health services and the debates that have surrounded health financing since the late 1980s. It shows that the health finance debate in Hong Kong is not a simple issue that can be tackled by rational planning; instead, it is a complex consequence of welfare politics in an increasingly mobilized society.

Clinical research evolved side-by-side with technology, leading to exponential data generation contributing to social and economic development. Nevertheless, data storage, integrity, and privacy concerns have emerged, raising trust issues regarding data sharing. This chapter will demonstrate how blockchain technology (BT) can address these problems and help optimizing processes, minimize costs, and monetize data. It will explain why these models are not fully explored and how cryptocurrencies are advantageous compared to traditional currency. Worldwide examples of companies developing network infrastructures that rely on private players will be provided, and European cases, where consortium models that count with different partners to build health blockchain infrastructures are being developed, will be discussed. Considering the business models to be addressed under the European Union (EU) jurisdiction, a hypothetical BT-based healthcare model with potential application in the EU scenario will also be highlighted.

The issue of patenting is a contentious issue. Different stakeholders hold opposite views. The pharmaceutical giants demand for stricter and longer Intellectual Property Protection for the promotion of research and development. They contend IP protection to be the “bedrock of their business”. While the health activists and governments of developing nations want greater flexibility in intellectual property protection and shorter patent period protection. Article 31 of the TRIPs agreement provides certain flexibilities to cater to certain situations. The countries are free to determine the grounds to issue provisions like compulsory licensing. But despite the provisions very few countries have used them. Pharmaceuticals giants with the backing of developed countries always try to denounce the practice of compulsory licensing with economic and political pressure. This chapter explains the concept and significance of Intellectual Property with reference to Pharmaceuticals. It also traces the little history of TRIPs agreement and explains the concept of Compulsory Licensing with trends in its use. It also discusses the few instances where compulsory License has been issued in different countries and tries to find the reasons of non issuance of Compulsory License.

В данной статье определены ведущие тенденции развития Российской Федерации в условиях введённых санкций, рассмотрен вопрос их влияния на различные сферы жизни общества и обозначены пути решения возникших проблем. Высказываются рекомендации по преодолению санкций и развитию в новых политических и экономических реалиях отдельных отраслей социально-экономической политики российского государства — здравоохранения, социального обеспечения, образования, туризма, экономики и др. Перечисляется перечень возможных последствий от введённых санкций, как положительных, так и отрицательных. Научная новизна работы заключается в том, что на Российскую Федерацию в настоящее время ежедневно накладываются всё новые и новые пакеты санкций со стороны ряда европейских государств и США. Целью исследования является изучение информации о сложившейся в настоящее время ситуации в нашем государстве и последующее определение влияния на обстановку в стране введённых санкций, последствиями которых выступают уходы с российского рынка многочисленных компаний. Проанализировав статью, можно сделать краткие выводы: 1. Тенденция импортозамещения позволит нашему государству выйти на новый экономический уровень, стать независимым от европейского рынка, а также снизить общий уровень цен на российском рынке. 2. Малый и средний бизнес при нынешних условиях лишается большей части конкурентов и получает стимул на развитие, в том числе в сфере туризма. 3. Политическая сфера также получает шанс на развитие посредством поднятия рейтинга доверия населения к действующей власти. 4. Санкции положили начало модернизации права в социальной сфере, так как осуществляется принятие новых законов по степени возникновения новых санкций. 5. Единственной отрицательной особенностью выступает психологическая атмосфера населения – повышается общая тревожность в обществе. Однако, при ускоренном развитии всех вышеперечисленных сфер, это явление быстро пойдёт на спад, так как в таком случае люди убедятся в незначительности последствий санкций. This article identifies the leading trends in the development of the Russian Federation under the imposed sanctions, examines the issue of their impact on various spheres of society and identifies ways to solve the problems that have arisen. Recommendations are made on overcoming sanctions and developing certain branches of the socio-economic policy of the Russian state in the new political and economic realities — healthcare, social security, education, tourism, economy, etc. The list of possible consequences from the imposed sanctions, both positive and negative, is listed. The scientific novelty of the work lies in the fact that new and new packages of sanctions from a number of European states and the United States are being imposed on the Russian Federation every day. The purpose of the study is to study information about the current situation in our country and the subsequent determination of the impact on the situation in the country of the sanctions imposed, the consequences of which are the departures from the Russian market of numerous companies. After analyzing the article, we can draw brief conclusions: 1. The trend of import substitution will allow our state to reach a new economic level, become independent from the European market, and also reduce the overall price level in the Russian market. 2. Under the current conditions, small and medium-sized businesses lose most of their competitors and receive an incentive to develop, including in the tourism sector. 3. The political sphere also gets a chance to develop by raising the rating of public confidence in the current government. 4. Sanctions marked the beginning of the modernization of law in the social sphere, as the adoption of new laws is carried out according to the degree of occurrence of new sanctions. 5. The only negative feature is the psychological atmosphere of the population – the general anxiety in society increases. However, with the accelerated development of all of the above areas, this phenomenon will quickly decline, since in this case people will be convinced of the insignificance of the consequences of sanctions.

Telehealth seems to be the new normal in this fast-changing environment. According to the European Commission eHealth was among the highest priorities before the COVID -19 pandemic. Transformation of health and care in the digital single market is among the EU`s six political priorities of the Commission 2019-2024 (2018 Communication on Digital Health and Care). The pandemic caused by COVID-19 just accelerates the necessity of the inclusion of digital health into the traditional healthcare systems. Telehealth services are among the biggest eHealth trends in EU. Therefore, one of the challenges is the national, regional and regulatory priorities regarding telehealth. There is lack of telehealth special legislative and governmental policies that needs to stimulate the developing and innovative solutions in medicine through technology and to envisage the upcoming innovation technology. Therefore, the government support and adequate policy making is important to support the development of the telehealth services. One of the main challenges is the electronic transactions of patient data among the telehealth providers and services and the cross-border patient data share. Another issue is the exchange of information among the national health institutions and providers and their interoperability. The Macedonian legislation does not have special legislation (policies, or laws) about telehealth. Telehealth is regulated as a term in the Law on health protection. Additionally, there is a lack of national acts, literature, and research in this subject matter. Thus, this paper will explore the telehealth from two main perspectives: scientific theories and legal practice and the users’ practice. Hence, this paper will analyze the legislation about the telehealth on the EU level and the EU Member States and the Macedonian legislation and the impact on the e-health that was made during COVID-19 pandemic. Furthermore, it will make comparative analyses among different countries into the EU zone compared with the EU aspirant country- the Republic of North Macedonia. A survey conducted among doctors in private and public healthcare institutions in the primary, secondary, and tertiary healthcare levels in the city of Stip and in the city of Skopje will provide data about the challenges, risks, and trends in telehealth before and during COVID -19.

An important aspect in terms of the health status of the population is the evaluation of quality of life by monitoring selected indicators, which can include health care. Interest in the health of the population is intensifying today. The main reasons include social, political, economic changes, but also the ongoing pandemic related to the spread of the COVID-19 virus. It is part of many values that we place in our quality of life. Improving the health level of the population is a very serious task. At the same time, healthcare is a very important factor in health, which is related to quality of life. The subject of our interest in the presented paper was the acquisition of data with a highly subjective character focused on the issue of health care in the city of Nitra. When choosing methods, we therefore preferred a questionnaire method based on our questionnaire. The questions were aimed at finding out the opinions, attitudes and evaluation of health care as an indicator of the quality of life of the population. We conducted the research in 2019 on a sample of 509 respondents the field and 261 online) in the city of Nitra. The aim of the paper was to analyze the subjective perception of health care in the city of Nitra by the method of perception. Based on the research, it can be stated that the provision of health care, access to health care, together with the length of the waiting time for admission is especially important for the inhabitants of Nitra. However, the most important factor in health care for the inhabitants of Nitra is satisfaction with the health care provided. The most negative respondents perceived the length of the waiting time for admission.

Human capital is the primary driver of change in the modern knowledge society. The migration of healthcare workers is a "brain drain" of highly qualified individuals. Healthcare workers are currently among the most sought-after and mobile occupations in the EU. The global trend of open borders and free movement of goods, people and capital is an important determinant of growth and development, which developed countries have already recognized at the expense of less developed ones. The aim of this paper is to highlight the importance of the volume of migration, which is not insignificant, and the long-term consequences are negative for society and the economy. There are numerous reasons for economically motivated migration, and there are three main groups of problems that need to be solved urgently in order to stop further emigration. The departure was primarily motivated by problems caused by general social and political circumstances, as well as dissatisfaction with the general state and socio-political climate in the country. Reasons for dissatisfaction are low wages and living standards, dissatisfaction with the organization and management of the state, hopelessness and deterioration of regions, cities, society and people, corruption and crime, religious intolerance and nationalism in the absence of positive changes in the state. An increasing number of young people are leaving their homelands in search of a "better life", and without them the progress of the country is impossible. What is worrying is the youthful pessimism that already prevails in the attitudes of young people towards issues of political and economic progress and social development. Croatia is currently one of the three EU countries with the largest number of emigrating healthcare workers. In its Report for Croatia for 2018, the European Commission stated that spending on healthcare in Croatia is stagnating, while spending across the EU is growing [2]. Healthcare expenditures in Croatia were among the lowest in the European Union; they amounted to only 760 euros per inhabitant in 2015 and are lower only in Romania, Latvia and Bulgaria. Furthermore, total healthcare spending in Croatia has remained stable since 2011, and the vast majority of EU member states have been spending more on healthcare in recent years, which is usually attributed to increasing costs for technological advances in medicine and an aging population. Healthcare is still underfunded and over-indebted. Since regular sources of financing are not sufficient to cover all costs, the system accumulates outstanding debts that are eventually settled with one-time transfers from the state budget. The scientific contribution of this work is to point out the shortcomings in the system and offer possible solutions.

The current COVID-19 epidemic is both a health and societal issue; therefore, groups historically excluded and marginalised in terms of healthcare will suffer if COVID-19 vaccines, tests, and treatments are to be delivered equitably. This rapid review is exploring the social and cultural challenges related to the roll-out, distribution, and access of COVID-19 vaccines, tests, and treatments. It highlights how these challenges impact certain marginalised groups. Case studies are taken from sub-Saharan Africa (the Democratic Republic of Congo, South Africa), with some focus on South East Asia (Indonesia, India) as they have different at-risk groups. Lessons on this issue can be learned from previous pandemics and vaccine roll-out in low- and mid-income countries (LMICs). Key points to highlight include successful COVID-19 vaccine roll-out will only be achieved by ensuring effective community engagement, building local vaccine acceptability and confidence, and overcoming cultural, socio-economic, and political barriers that lead to mistrust and hinder uptake of vaccines. However, the literature notes that a lot of lessons learned about roll-out involve communication - including that the government should under-promise what it can do and then over-deliver. Any campaign must aim to create trust, and involve local communities in planning processes.

Moldova’s politics, economy, justice system and media are increasingly dominated by a powerful group of elites, led by oligarchs - a new breed of businessmen-politicians who have emerged in the past decade - controlling strategic sectors of the economy and finance, hijacking the political system, taming the judiciary and acquiring monopolistic control of mass media, to promote and protect their vast business empires. Alongside traditional clan, kinship and patronage networks these elites exert influence through informal politics , shaping Moldova’s politics and economy, often hindering reforms for democratisation, rule of law, meritocracy and transparency. This helpdesk report looks at the nature and role of clans in Moldova in the country’s politics and economy. This literature review utilises academic as well as grey sources, research papers, media and blogs published mainly in the past ten years. The sources reveal a paucity of Moldova centric material, especially on the sub-issue of clans, but much more literature is available on the role of informal politics and state capture by elites, especially oligarchs, in Moldova. The evidence found did not address gender and disability issues.

The article examines the views of one of the founders of classical economics, David Ricardo, on the issues of investment, capital and profit. The need for this study is caused by the lack of a single definition of investment in the regulatory acts of investment legislation, as well as in the scientific community. Thus, there is a problem of regulatory regulation of one of the most important concepts of the economy. Given that the concepts of investment, capital, and capital investment are primarily economic categories, it seems quite reasonable to consider the emergence and development of these concepts in the retrospect of economic theories, in order to understand the essence of the phenomenon and finally solve the issue of its legal regulation. The scientific novelty of the study is that despite a large number of works on the work of David Ricardo «The Beginnings of Political Economy and Taxation», no special work was carried out on the contribution of the great economist to the theory of investment. Conclusions. The main achievements of Ricardo in the field of capital and investment include the author's definitions of capital, free capital, the creation of a theory of comparative advantages of trade, the division of capital into fixed and circulating capital depending on strength, as well as the description of the reasons that stimulate and hinder foreign investment.
