Transcranial magnetic stimulation was first introduced in the late 1980s. Numerous studies have used TMS as an investigational tool to elucidate cortical physiology and to probe cognitive processes. This article introduces TMS paradigms and presents information gathered on cortical neuronal connectivity. TMS paradigms that demonstrate intracortical inhibition include short-interval cortical inhibition (SICI), cortical silence period (cSP) and long interval cortical inhibition (LICI). There are two types of cortical inhibitions from the stimulation of other brain areas, interhemispheric inhibition and cerebellum inhibition. The inhibition of the motor cortex can also be induced through the stimulation of peripheral nerves. This article talks about studies that describe interaction between inhibitory and facilitatory paradigms, the results of which are discussed in terms of cortical physiology and connectivity. The study of the interactions among cortical inhibitory and excitatory circuits may help to elucidate pathophysiology of neurological and psychiatric diseases.

This chapter, “Lowly Origins,” examines the evolution of the nervous system and its implications for clinical neurology. Topics include peripheral nerve anatomy, extraocular muscles, and physiologic circuits related to respiration. Human neuroanatomy and neurologic disease carry a record of our vertebrate ancestors, and neurology is more understandable when the clinician is attuned to our ancient neurological circuits. The extraocular muscles are a prime example. Although the extraocular muscles have changed their orientation to the axis of the eye, and although not all of these muscles are as important as they once were, these muscles of the human eye have otherwise changed little from those of the shark. They remain similar in appearance and consistent in innervation. They are the best conserved muscles in all of vertebrate evolution. The development of limbs, loss of gills, assumption of bipedal locomotion, and development of a huge brain has had virtually no effect on them.

In addition to serving perception, gaze acts as a powerful social signal and mode of communication. Gaze is altered in several psychiatric diseases and impaired by a variety of central and peripheral lesions. Eye movements that serve to stabilize gaze include the vestibuloocular reflex (VOR) and fixation, whereas eye movements that shift gaze include saccades, cancellation of the VOR, and smooth pursuit. The pontine horizontal gaze center and midbrain vertical gaze center connect to extraocular motoneurons and mediate all eye movements. Neural circuits involved in generating the VOR, horizontal saccades and saccade modulation are described in detail. Nystagmus consequent to unilateral labyrinthine damage is explained. Other forms of nystagmus including the optokinetic response are introduced. The role of internuclear interneurons in coordinating horizontal saccades and their failure in internuclear ophthalmalplegia are detailed. Finally, the mechanisms involved in fixation and smooth pursuit are briefly presented.

This chapter discusses the use of somatosensory evoked potentials (SEPs) and pain evoked potentials for diagnostic purposes. The generators of SEPs following upper limb stimulation have been identified through intracranial recordings, permitting the analysis of somatosensory disorders caused by neurological diseases. Laser activation of fibers involved in thermal and pain sensation has extended the applications of evoked potentials to neuropathic pain disorders. Knowledge of the effects of motor programming, paired stimulations, and simultaneous stimulation of adjacent somatic territories has broadened SEP use in movement disorders. The recording of high-frequency cortical oscillations evoked by peripheral nerve stimulation gives access to the functioning of SI area neuronal circuitry. SEPs complement electro-neuro-myography in patients with neuropathies and radiculopathies, spinal cord and hemispheric lesions, and coma. Neuroimaging has overtaken SEPs in detecting and localizing central nervous system lesions, but SEPs still permit assessment of somatosensory and pain disorders that remain unexplained by anatomical investigations.

At a time when many sites were written about but only a few were represented in art, four distinct sets of artifacts circulated new information. Engraved silver cups list all the stops from Spanish Gades to Rome, while resembling the milestones that helped travelers track their progress. Vivid glass cups represent famous charioteers and gladiators competing in circuses and amphitheaters, and offered virtual experiences of spectacles that were new to many regions. Bronze bowls commemorate forts along Hadrian’s Wall with colorful enameling typical of Celtic craftsmanship. Glass bottles display labeled cityscapes of Baiae, a notorious resort, and Puteoli, a busy port, both in the Bay of Naples. These artifacts and their journeys reveal an empire divided not into center and periphery, but connected by roads that did not all lead to Rome.
