The research is to analyse the immunohistochemical reaction of orthodontic force on the periodontium reformed by nanobiphasic calcium phosphate ceramics (nBCP). Two third incisors were selected randomly and operated as experimental groups in 2 Beagle dogs. In the labial aspects of the third incisors, alveolar bone defects were surgically made and implanted with NBCP. The contralateral teeth in the same jaw did not receive any treatment as control. After 24 weeks, all the third incisors were moved labially. The dogs were euthanized 4 weeks later. The expression levels of osteocalcin were detected by immunohistochemical staining. Positive osteocalcin expressions in regenerated periodontium were observed and compared with the normal periodontium in the control groups. There were no significant differences within and between them. It means the periodontium regenerated by nBCP can bear orthodontic forces with a normal function. Based on these findings, we concluded that nBCP may offer a new bone graft choice for periodontic disease patients who have demands for orthodontic treatment.

Orthodontics and periodontics have a sophisticated elaborate interdependence as every orthodontic intervention has a periodontal extent. Although orthodontic alignment promotes adequate plaque control, many detrimental effects on the periodontium may be observed during the treatment. Periodontal intervention is required at all stages of orthodontic therapy starting from orthodontist diagnosing a case to mid-treatment analysis of periodontium and also posttreatment investigation of the case. Many a time, successful orthodontic treatment is dependent on the overall maintenance of the health of the periodontium. On the other hand, orthodontic tooth alignment may be used as an adjunctive treatment in periodontally compromised patients. This review will help in better understanding of the orthodontic–periodontal interrelationship for the better framework of the treatment approach to bring out the optimum results in patients.

Introduction: Orthodontics and periodontics are two closely related disciplines, as they both affect the periodontium. An upstream analysis can reduce the risk of periodontal pathologies appearing or worsening. The aim of our study was to determine the risks of orthodontic forces on pathological periodontium. Methodology: This is a systematic review of articles evaluating the need for pre-orthodontic periodontal treatment using the PRISMA method. A total of 11 recent articles were selected. The study was carried out from January to March 2023. Results: The articles asserted a high risk of aggravation of pre-existing periodontal pathologies (54.5%). The 27.2% mentioned risks of gingivopathy, periodontitis, plaque accumulation, increased inflammation and loss of attachment. Nine percent of articles predicted a risk of increased bone dehiscence, gingival recession, alveolysis and root resorption. Discussion: Orthodontic therapy can be considered a risk factor for the periodontium through two phenomena: difficulty in meticulous hygiene practice; and osteoclasia caused by the infectious agents responsible for periodontal disease. One study showed that orthodontic treatment can transform gingivitis into periodontitis and/or aggravate pre-existing periodontitis. Conclusion: Periodontal treatment is essential before orthodontic treatment to optimize periodontal support and improve orthodontic therapy.

AbstractDiseases of the periodontium occurs in the childhood, adolescence and early adulthood, but the prevalence of periodontal diseases and tissue destruction increases with age. The increase may be caused by the cumulative effect of the number of bursts of periodontal destruction, deterioration in plaque removal efficiency, or an increase in the number of teeth retained in old age and therefore affected by plaque-indltced disease. But with appropriate periodontal therapy and self-performed plaque control, older patients can be expected to have as favorable treatment outcomes as young patients.

Discussion is presented on the treatment of the periodontium through each phase of orthodontic therapy. Current oral hygiene procedures are outlined and refined according to special orthodontic needs.

O biótipo gengival, definido como a espessura da gengiva no sentido vestíbulo-lingual, desempenha importante papel na homeostasia dos tecidos periodontais, podendo ser considerado um preditor no sucesso em longo prazo das terapias periodontais e periimplantares. Assim sendo, é de suma importância reconhecer as dimensões do tecido gengival e as diferentes formas de qualificá-lo e principalmente quantificá-lo. Apesar de haver descrito na literatura inúmeros métodos para este fim, existem poucos estudos comparando a efetividade de um método em relação a outro. Desta maneira, este estudo buscou avaliar se há concordância entre avaliações clínicas e tomográficas na classificação do biótipo gengival, se existe correlação entre o biótipo gengival e a espessura óssea subjacente, além de descrever um novo método de tomada tomográfica que permita a análise quantitativa do biótipo gengival. Foram avaliados 12 indivíduos os quais realizaram tomografias computadorizadas de feixe cônico como exame imaginológico de diagnóstico ou planejamento pré-cirúrgico. Em cada paciente foram realizados quatro diferentes métodos de avaliação qualitativa da espessura gengival (transparência a sondagem, transgengival, visual através de fotografia e tomográfico), dois métodos de avaliação quantitativa (transgengival e tomográfico) da espessura gengival e avaliação da espessura óssea através da tomografia computadorizada de feixe cônico. Os resultados foram avaliados estatisticamente através do teste KAPPA, Teste t pareado e coeficiente de correlação de Pearson (pM0.05). O novo método de tomada tomográfica descrito neste estudo é eficaz para avaliação do biótipo gengival, havendo grande concordância (86,1% Kappa 0,51) e forte correlação (r=0,824) entre ele e o método transgengival (padrão ouro). A correlação entre a espessura óssea e a espessura gengival foi moderada quando utilizado o método transgengival e tomográfico (r=0,567 e r=0,653 respectivamente).
The gingival biotype, defined as the thickness of the gum in the bucco-lingual direction, plays an important role in the homeostasis of the periodontal tissues and can be considered a predictor of long-term success of periodontal and peri-implant therapies. It is therefore of paramount importance to recognize the dimensions of the gingival tissue and the different ways to qualify it and especially quantify it. Although there are numerous methods described in the literature for this purpose, there are few studies comparing the effectiveness of one method over another. Thus, this study sought to evaluate whether there is concordance between clinical assessment and CT in the classification of gingival biotype, if there is a correlation between the gingival biotype and the underlying bone thickness, and describe a new method of tomographic tapping for the quantitative analysis of biotype gingival. They evaluated 12 individuals who underwent computed tomography cone beam as imaginologic diagnostic test or pre-surgical planning. In each patient were performed four different methods of qualitative evaluation of the gingival thickness (transparency of periodontal probe, transgengival, look through photo and tomographic), two quantitative assessment methods (transgingival and tomographic) gingival thickness and assessment of bone thickness by CT Cone beam computed. The results were statistically evaluated by Kappa test, paired t test and Pearson\'s correlation coefficient (pM0.05). The new method of making tomographic described in this study is effective for evaluating gingival biotype, there is great agreement (86.1% Kappa 0.51) and strong correlation (r = 0.824) between it and the transgingival method (gold standard). The correlation between the bone thickness and moderate gingival thickness was used as the transgingival and tomographic method (r = 0.567 and r = 0.653 respectively).

Resumo: Tem sido relatado que o consumo de cigarros influencia negativamente a cicatrização periodontal após procedimentos terapêuticos. Entretanto, existe pouca informação em relação à influência da inalação da fumaça de cigarro e da administração de nicotina sobre a regeneração periodontal espontânea. Sendo assim, o objetivo deste estudo foi avaliar histologicamente em ratos, o impacto da inalação da fumaça de cigarro e da administração de nicotina num modelo de reparo periodontal espontâneo. Foram utilizados neste estudo, 39 ratos adultos (Rattus Norvegicus, Albinus, Wistar), todos originários do centro de bioterismo da UNICAMP. Os animais foram divididos aleatoriamente em 3 grupos experimentais: Grupo 1 ¿ controle (n=13), Grupo 2 ¿ administração subcutânea de nicotina (3mg/kg), 3 vezes ao dia (n=13) e Grupo 3 ¿ exposição à fumaça de cigarro, 3 vezes ao dia (n=13). Trinta dias após o início dos regimes, sob anestesia geral, foi criado um defeito do tipo fenestração (King et al., 1997 modificado), tendo como dimensões 4mm de comprimento, 3mm de altura e 1mm de profundidade, com o objetivo de expor a região vestibular da raiz distal dos primeiros molares inferiores, e proporcionar a remoção do cemento dental. Os animais foram sacrificados 21 dias após a criação dos defeitos. Os resultados foram avaliados histometricamente para os seguintes parâmetros: extensão do defeito remanescente, porcentagem de preenchimento do defeito, densidade do novo osso e extensão de novo cemento. A análise intergrupo demonstrou que os defeitos tinham tamanho similar inicialmente, e que a exposição a fumaça de cigarro reduziu significativamente (P=0.05) a densidade do novo osso (80.07+3.45%, 76.37+5.27%, 72.74+6.24%, grupos 1, 2 e 3; respectivamente) e o preenchimento do defeito (95.97+4.64%, 90.62+4.64%, 85.34+7.70%, grupos 1, 2 e 3; respectivamente). Não ocorreu a formação de novo cemento em nenhum dos grupos experimentais. Dentro dos limites deste estudo, a análise dos dados nos permite concluir que a fumaça de cigarro pode influenciar negativamente a regeneração periodontal espontânea em ratos
Abstract: Cigarette smoking has been shown to negatively influence healing following periodontal therapeutic procedures. However, limited information is available on the effect of cigarette smoking on periodontal self-healing capacity in sites not previously exposed to contaminants. Therefore, the aim of this study was to histologically evaluate in rats, the impact of cigarette smoke inhalation and nicotine administration on a spontaneous periodontal healing model. Thirty nine male adult Wistar rats were used, and randomly assigned to one of the following groups: Group 1- control (n=13), Group 2- subcutaneous nicotine administration (NA) (3mg/Kg) (n=13), and Group 3- cigarette smoke inhalation (CSI) (n=13). Thirty days after the beginning of CSI and NA regimens; periodontal fenestration defects (4x3x1mm) were created at the buccal aspect of the distal root of first mandibular molars, and the animals sacrificed 21 days after surgery. The percentage of bone fill and density of new formed bone, new cementum formation, and extension of the remaining defect were histometrically obtained. Intergroup analysis demonstrated that the defects were initially similar in size, and that CSI significantly reduced (P=0.05) density of the newly formed bone (80.07+3.45%, 76.37+5.27%, 72.74+6.24%, for group 1, 2 and 3; respectively) and bone filling (95.97+4.64%, 90.62+4.64%, 85.34+7.70%, for group 1, 2 and 3; respectively). No new cementum was formed along the root surface in the above groups. Within the limits of the present study, data analysis suggests that smoking may influence the self-healing capacity of periodontal tissues regardless previous exposure to dental biofilm

Resumo: O objetivo deste trabalho foi investigar a influência do fator idade sobre o comportamento e expressão gênica de células do ligamento periodontal (CLPD), e quais alterações o tratamento com fator de crescimento básico de fibroblastos (bFGF) pode induzir nestas populações celulares. Culturas primárias de CLPD foram obtidas e divididas nos seguintes grupos experimentais: A ¿ inclusos (células obtidas de terceiros molares inclusos de voluntários entre 18 e 30 anos de idade), B ¿ jovens (células obtidas de pré-molares erupcionados de voluntários entre 15 e 20 anos de idade), C ¿ idosos (células obtidas de dentes erupcionados de voluntários acima de 60 anos de idade), D ¿ grupo A submetido ao tratamento com 10?g/ml de bFGF, E ¿ grupo B submetido ao tratamento com 10?g/ml de bFGF e F ¿ grupo C submetido ao tratamento com 10?g/ml de bFGF. Foram realizados ensaios de proliferação e viabilidade celular, mineralização, quantificação dos níveis de proteína total e do padrão de expressão dos seguintes genes: Colágeno tipo I e III, metaloproteinase (MMP)-2 e -8, inibidor tecidual de metaloproteinase (TIMP)-1 e ¿2, fator de crescimento derivado de plaquetas (PDGF)-1, proteína morfogenética óssea (BMP)-3, bFGF, osteoprotegerina (OPG), receptor ativador do fator nuclear kß ligante (RANKL), interleucina (IL)-1ß, IL-4, IL-6, IL-8 e receptor de fator de crescimento de fibroblasto (FGFR) 1 por meio da técnica de ¿real-time¿ PCR (PCRq). A análise dos resultados demonstrou que a idade diminuiu a taxa de proliferação celular e a formação de nódulos minerais (P<0,05), enquanto que não se observou efeito significativo da idade sobre a viabilidade celular e quantidade total de proteína secretada (P>0,05). Já na análise de expressão gênica observou-se que a idade diminuiu significativamente os níveis de RNAm para colágeno tipo I e IL-4, enquanto que para os genes FGFR1, MMP-8, TIMP-1, OPG e IL-1ß e 6, observou-se um aumento nos níveis de RNAm (P<0,05). O tratamento com 10?g/ml de bFGF aumentou a taxa proliferativa em todos os grupos (P<0,05), entretanto com menor intensidade nas células de idosos (P<0,05). Já a formação de nódulos minerais foi inibida pelo fator crescimento em todos os grupos experimentais (P<0,05), enquanto que a viabilidade celular e quantidade total de proteína secretada não foram afetadas (P>0,05). Na avaliação do padrão de expressão gênica, os resultados demostraram que o tratamento com bFGF, diminuiu nos níveis de RNAm para todos os genes (P<0,05), com excessão da TIMP-1 e IL-8 que aumentaram (P<0,05), BMP-3 que aumentou nas células de jovens e diminui nas células de idosos (P<0,05), e PDGF-1 que não foi afetada (P>0,05). Dentro dos limites do estudo, podemos concluir que: i) foi possível estabelecer culturas primárias de células do ligamento periodontal obtidas de pacientes jovens e idosos, ii) a idade pode modular propriedades importantes das CLPD, diminuindo seu potencial de regeneração de estruturas minerais além de exacerbar características pró-inflamatórias e de degradação da matriz extracelular e iii) o tratamento com 10?g/ml de bFGF induz alterações similares no comportarmento de CLPD independentemente da idade do doador
Abstract: The aim of this study was to investigate the effect of aging on periodontal ligament cells (PDLC), modulated by the treatment with bFGF. Primary periodontal ligament cell cultures were obtained and divided into the following experimental groups: A ¿ impacted (cells from impacted molars from subjects aged 18 to 30 years), B ¿ young (cells from erupted pre-molars from subjects aged 15 to 20 years), C ¿ aged (cells from erupted teeth from patients aged more than 60 years), D ¿ group A treated with 10?g/ml of bFGF, E ¿ group B treated with 10?g/ml of bFGF and F ¿ group C treated with 10?g/ml of bFGF. For all groups, we performed proliferation and cell viability assays, mineralization assay, total protein quantification and assessed mRNA levels of: type I and III collagen, MMP-2 e -8, TIMP-1 e ¿2, PDGF-1, BMP-3, bFGF, OPG, RANKL, IL-1, IL-4, IL-6, IL-8 and FGFR 1 utlizing the ¿real-time¿ PCR technique. Data analysis demonstrated that aging negatively influenced cell proliferation and mineral nodule formation (P<0.05), while cell viability and total protein secretion were not affected (P>0.05). Gene expression analysis revealed that aging decreased mRNA levels of collagen type I and IL-4, and increased mRNA levels of FGFR1, MMP-8, TIMP-1, OPG and IL-1 and 6 (P<0.05). Treatment with 10?g/ml of bFGF significantly increased cell proliferation for all groups (P<0.05), however with a less impact on aged cells (P<0.05). In addition, cell viability and total protein secretion were not affected by bFGF (P>0.05), while mineral nodule formation was significantly inhibited (P<0.05). Finally, gene expression analysis demonstrated that bFGF decreased mRNA levels of all genes (P<0.05), except for TIMP-1 and IL-8 that were increased (P<0.05), PDGF-1 that was not affected (P>0.05) and BMP-3 that was increased in young cells and decreased in aged cells (P<0.05). Within the limits of our study, we may conclude that: i) it was possible to esblish primary cultures of PDLC obtained from young and aged donors, ii) aging modulates important features of periodontal ligament cells (PDLC), decreasing the potential for regeneration of mineralized tissues and favoring a pro-inflammatory and degenerative profile and iii) treatment with 10?g/ml of bFGF leads to similar PDLC behavioral alterations despite of the donor age

Total bacterial disinfection and elimination from the human root canal system are crucial in clinical endodontic procedures [Card et al., 2002; Kakoli et al., 2009]. The current approaches relying on mechanical instrumentation and root canal irrigation and medicaments have demonstrated that eradication of bacteria occurs when the bacteria are in direct contact with the medicaments. However, persistent infection following routine treatments has suggested that bacteria may harbor in the root canal anatomical irregularities and/or deep dentinal tubules, therefore, surface irrigation of medicaments may not be able to reach those regions. Heat treatment has been used for obturation of the root canal in endodontic practice. In this study we hypothesize that as an alternative, surface heating using a System B heating catheter through the root canal surface would be effective for bacterial elimination in the deep dentin. The heat-induced cytotoxic response kills bacteria in the root dentin via heat conduction from the thermal energy incident on the root canal wall. In principle, a high power setting and/or a long heating duration can always achieve sufficient temperature elevations in deep dentin. Yet, the detailed temperature distribution inside the dentin and possible thermal damage to the supporting periodontium are unknown. Therefore, it is of clinical importance to perform and investigate temperature elevations in dentin to provide clinicians with an optimized and effective treatment protocol to minimize unnecessary thermal damage to the surrounding structure.

Review question / Objective: The aim of this systematic review and meta-analysis of case-control studies is to find out the association of IL-1 RN intron 2 (VNTR) rs2234663 Gene Polymorphism and the occurrence and progression of periodontitis(including chronic periodontitis, aggressive periodontitis and early-onset periodontitis). Condition being studied: Periodontitis is one of the most common ailments affecting the teeth, leading to the destruction of the supporting and surrounding tooth structure. Periodontitis is originally a disease originating from the gingival tissue which if left untreated results in penetration of inflammation to the deeper tissues, altering the bone homeostasis causing tooth loss. Periodontal disease has a multifactorial origin. The main culprit identified in periodontitis is the bacterial biofilm growing on the tooth surfaces. The deleterious effects of periodontopathogens are not limited to the periodontium, but they also exude their ill effects on the systemic health of the patients. While the host response determines the progression of the disease, genetics, environmental factors, systemic health of the patient, lifestyle habits and various social determinants also play a role. Interleukin-1 receptor antagonist encoded by this gene IL-1RN is a member of the interleukin 1 cytokine family. This protein inhibits the activities of interleukin 1, alpha (IL1A) and interleukin 1, beta (IL1B), and modulates a variety of interleukin 1 related immune and inflammatory responses, particularly in the acute phase of infection and inflammation. We aim to study their association by conducting a meta-analysis.
