Abstract Denileukin diftitox, a genetically engineered fusion protein combining the enzymatically active domains of diphtheria toxin and the full-length sequence for interleukin-2 (IL-2), efficiently targets lymphoma cells expressing the high-affinity IL-2 receptor (IL-2R) consisting of the α/p55/CD25, β/p75/CD122, and γ/p64/CD132 chains. In vitro studies demonstrated that the retinoid X receptor (RXR) retinoid, bexarotene, at biologically relevant concentrations of 10–6M to 10–8 M, upregulated both the p55 and p75 subunits of the IL-2R and enhanced 5- to 10-fold the susceptibility of T-cell leukemia cells to denileukin diftitox. To determine whether this biomodulatory effect could be recapitulated in vivo, we treated 14 patients with relapsed or refractory cutaneous T-cell lymphoma with escalating doses of bexarotene (75 mg/day-300 mg/day) and denileukin diftitox (18 mcg/kg per day × 3 days every 21 days) in a phase 1 trial. Overall response was 67% (4 complete responses, 4 partial responses). Modulation of IL-2R expression was observed at or above a bexarotene dose of 150 mg/day. Four patients experienced grade 2 or 3 leukopenia, and 2 had grade 4 lymphopenia. Our results demonstrate that the combination of denileukin diftitox and bexarotene is well tolerated and that even low doses (150 mg/day) of bexarotene are capable of in vivo upregulation of CD25 expression on circulating leukemia cells.

AIMS OF THE STUDY: Data on the demographic changes over time for hip arthroplasty are rare in Switzerland. The aim of the study was to evaluate the influence of these changes on the distribution of age, gender, operated hip side, place of residence and caseload per surgeon over the last 50 years of hip arthroplasty at the Kantonsspital St. Gallen. METHODS: For this retrospective explorative study, data were collected from the operating theatre journals of hip replacements performed at Kantonsspital St. Gallen from 1969 to 2019. Every fifth year was included, which resulted in 5-year sampling rate over the observation period. The journals were handwritten until 1999 and digital from 2004 to 2019. The following data were obtained: age, sex, type of hip arthroplasty, side of operated hip, place of residence and name of main surgeon. Apart from overall descriptive statistics, we applied the Mann-Whitney U-test to test for differences in age and the binomial test for categorical variables. A linear regression model was applied to investigate the relationship between patients’ ages and historical data of life expectancy of the Swiss population. RESULTS: We included 2,963 patients, of whom 1,318 were men (median age = 67 yr., p25 = 59 yr., p75 = 74 yr.) and 1,608 women (median age = 72 yr., p25 = 63 yr., p75 = 79 yr.). Overall, women were significantly older than men, irrespective of whether they received primary total hip arthroplasty (median age = 70 yr. vs 66 yr., p25 = 61 yr. vs 58 yr., p75 = 77 yr. vs 73 yr., p <0.001), hemiarthroplasty (median age = 84 yr. vs 78.5 yr., p25 = 78 yr. vs 71 yr., p75 = 89 yr. vs 85 yr., p <0.001) or total revision arthroplasty (median age = 71 yr. vs 70 yr., p25 = 64.25 yr. vs 63 yr., p75 = 78 yr. vs 75 yr., p = 0.036). A trend toward rising median age is evident looking at the whole period observed from 1969 to 2019. Except for women in the total revision arthroplasty group (r = 0.226), a high correlation between increasing median age of patients undergoing hip replacement and life expectancy was found (r ≥0.663). Significantly more primary total hip prostheses (p = 0.003) and hip hemiprostheses (p <0.001) were implanted in women than in men between 1969 and 2019. Overall, no significant difference in side distribution was seen in the primary total hip arthroplasty (p = 0.061), total revision arthroplasty (p = 1.000) and hemiarthroplasty (p = 0.365) group. In contrast to earlier years, most patients in recent years are operated on by high-volume surgeons (>50 operations per surgeon per year). CONCLUSIONS: Demographic changes of patients undergoing total hip replacement reflect the overall demographic changes in the Swiss population. Over the last 50 years the indication for prosthetic hip replacements has not been extended to younger ages. The caseload in hip arthroplasty has changed over the last 50 years towards high-volume surgeons.

Background:Some autoimmune diseases, including rheumatoid arthritis (RA) and systemic lupus erythematosus (SLE), are considered to be independent risk factors for vascular morbidity and mortality. These pathologies present accelerated atherosclerosis, partly because of a chronic sustained inflammation, greater prevalence of cardiovascular risk factors (CVRFs) and pharmacological therapy. However, regarding primary Sjögren’s syndrome (pSS), available data are heterogeneous and proceed from small case series. For this reason, the aim of this study was to provide further information on the identification of atherosclerosis in pSS and its possible association with clinical and analytical parameters of the disease.Objectives:To assess presence of subclinical atherosclerosis by means of carotid ultrasound in patients with pSS and to analyze clinical, analytical and CVRF along with their potential association with the presence of subclinical cardiovascular affectation.Methods:This is a cross-sectional study of 38 patients with pSS (all patients met ACR/EULAR1classification criteria for pSS) and 38 age and sex matched controls. Demographic variables and classical CVRFs were collected (Hypertension, Diabetes mellitus, dyslipemia, Body Mass Index and smoking habit) and the presence of subclinical atherosclerosis was assessed by carotid ultrasound with carotid intima-media thickness (CIMT) measurement and determination of the presence of atheromatous plaques2, both in pSS patients and controls. Disease features were also collected in pSS patients (disease duration, disease activity measured by ESSDAI, glandular vs extraglandular involvement, serological features and treatments received).Statistical analysis: To evaluate differences between patients and controls, T-test or Wilcoxon test with continuity correction, were used for quantitative features and Fisher test for categorical variables. In order to test the presence of pSS as an independent risk factor for subclinical atherosclerosis, from other features as classic CVRFs or analytical data, first we adjusted logistic binomial regression in a bivariate analysis, to select possible predictors to be included in a multivariate analysis. For both groups, no differences between prevalence of classical CVRFs were found. Subclinical atherosclerosis presence was higher in patients with pSS than in controls [OR= 4.17, 95%CI (1.27- 16.54), p<0.001], as well as CIMT values (0.79± 0.43 mm vs. 0.66 ± 0.27 mm; p=0.02). An association of subclinical atherosclerosis with erythrocyte sedimentation rate [OR=1.18, 95%CI (1.05-1.37), p<0.05] and Rheumatoid Factor [OR=1.28, 95%CI (1.63-2.26), p<0.05].Conclusion:This cohort showed a greater prevalence of subclinical atherosclerosis in patients with pSS, indicating this disease as an independent risk factor for presence of early vascular damage.References:[1]Vitali C et al. Introdução: o aumento do número de casos de gestantes infectadas com o HIV é preocupante, entre as estratégias fomentadas para redução da transmissão vertical o aconselhamento pré-teste rápido de HIV apresenta-se como importante medida preventiva. Objetivo: Demonstrar, por meio de revisão da literatura, a importância do aconselhamento no teste rápido de HIV em gestantes. Método: estudo descritivo com pesquisa de revisão bibliográfica realizado por meio de artigos e publicações científicas encontradas nas bases de dados SCIELO, LILACS, CAPES e PUBMED com abrangência temporal de 2004 a 2018, utilizando-se os seguintes descritores: HIV, Aconselhamento, Teste rápido, Gestantes, Pré-Natal, em português e inglês. Resultados: obtiveram-se 18 artigos relacionados ao tema, utilizados após criteriosa seleção em revisão bibliográfica integrativa que foram utilizados como base teórica para a discussão acerca do tema do trabalho. Conclusão: observou-se que, além da necessidade em agilizar o diagnóstico, deve-se também fornecer as orientações necessárias pré e pós teste das gestantes atendidas para que o atendimento as esclareça de modo a priorizar medidas profiláticas na prevenção da doença e, no caso de positividade do resultado do exame, reduzir a transmissão vertical e estimular o seguimento do tratamento da condição patológica, além de estabelecer vínculos com o paciente visando à promoção de seu autocuidado.

BackgroundRheumatoid arthritis (RA) patients are at higher risk of developing cardiovascular (CV) diseases, both ischemic and non-ischemic, than the general population [1]. Inflammatory status and fibrotic changes at cardiac valves in these patients predispose them to a higher risk of valve diseases, which could be detected by echocardiography [2].ObjectivesTo determine the prevalence of valvular regurgitation, according to the time of disease evolution in RA patients.MethodsDescriptive, comparative, and cross-sectional study. We enrolled RA patients between 40 and 75 years old who fulfilled ACR/EULAR 2010 classification criteria and recruited in the Rheumatology service from tertiary care hospital. Patients were divided by time of disease evolution into quartiles. A transthoracic echocardiogram performed by a certified cardiologist blinded to clinical data. Normality was assesed by Kolmogorov-Smirnov test. Variables with normal and non-normal distribution were described by media and standard deviation, such as median and interquartile range (p25-p75), respectively. Differences between groups was analyzed by ANOVA, Kruskal-Wallis test or Chi-squared, accordingly.ResultsWe included 151 RA patients. Demographic characteristics are in Table 1. Overweight/obesity was found between 62-87% of patients, hypertension between 26-34%, and Type 2 Diabetes mellitus between 8-26%. The tricuspid valve was the most affected (73-84%). Abstract Tumor necrosis factor-α (TNF-α) binds to 2 distinct cell-surface receptors: TNF-α receptor-I (TNFR-I: p55) and TNF-α receptor-II (TNFR-II: p75). TNF-α induces leukocyte adhesion molecules on endothelial cells (ECs), which mediate 3 defined steps of the inflammatory response; namely, leukocyte rolling, firm adhesion, and transmigration. In this study, we have investigated the role of p75 in TNF-α–induced leukocyte adhesion molecules using cultured ECs derived from wild-type (WT), p75-null (p75−/−), or p55-null (p55−/−) mice. We observed that p75 was essential for TNF-α–induced E-selectin, vascular cell adhesion molecule 1 (VCAM-1), and intercellular adhesion molecule 1 (ICAM-1) expression. We also investigated the putative role of p75 in inflammation in vivo using an intravital microscopic approach with a mouse cremaster muscle model. TNF-α–stimulated leukocyte rolling, firm adhesion to ECs, and transmigration were dramatically reduced in p75−/− mice. Transplanted WT cremaster in p75−/− mice showed a robust leukocyte rolling and firm adhesion upon TNF-α activation, suggesting that the impairment in EC-leukocyte interaction in p75−/− mice is due to EC dysfunction. These results demonstrate, for the first time, that endothelial p75 is essential for TNF-α–induced leukocyte–endothelial-cell interaction. Our findings may contribute to the identification of novel p75-targeted therapeutic approaches for inflammatory diseases.

A 3-yr study (2002-2004) was conducted to evaluate the resistance of three Bt corn events to herbivory damage by the armyworm, Pseudaletia unipuncta (Haworth). Resistance was assessed by damage ratings and fresh mass measurements. Greenhouse studies were run in 2002 and 2004, and a field study was conducted in 2003. Hybrids expressing the Cry1A(b) insecticidal protein, Mon810 and Bt176 events, and their near isolines, as well as the TC1507 event, which expresses the Cry1F protein, were evaluated. Bt hybrids suffered less damage in all 3 yr and had higher fresh masses in 2002 and 2003 than the non-transgenic isolines. In 2002 and 2003, hybrids expressing the Cry1A(b) protein were significantly less damaged than the hybrid expressing Cry1F. In both 2002 and 2004, the Mon810 event suffered less damage than the Bt176 event, despite expressing the same Cry1A(b) protein. Bt corn, especially the Mon810 event may be used as an effective method of preventing P. unipuncta herbivory damage in corn. This paper aims to evaluate the behavior and longitudinal shear strength of the steel-concrete composite slab using the Steel Deck P75 through the partial shear connection method considering the contribution of the friction acting in the interface of the steel deck with the concrete at the region of supports. The method proposed by EN 1994-1-1:2004 for the design of composite slab systems is based in the analytical model similar to that of composite beams which allows to determine the degree shear connection between the steel sheeting and the concrete. This method allows to analyse the influence of friction at the support, in addition to the contribution of stud bolts on the mechanical resistance of composite slab. The composite slab was tested in the Federal University of Minas Gerais structure laboratory which used the semi-empirical "m-k" method to obtain its resistance. The behavior of the composite slab system was analysed through of the graphics deflections, end slips and strains, allowing the determination of its failure mode. Finally, the results obtained from the longitudinal shear resistance considering the friction at the support were applied in one practical example.

Poorly cohesive (PC) gastric cancer (GC) is extremely aggressive in progression, and there is an urgent need to identify the molecular pathways involved. We hypothesized the essential role of the RhoA–YAP axis in these mechanisms. The present observational multicenter retrospective study included 133 patients with PC GC treated at two dedicated European surgical centers between 2004 and 2014. YAP nuclear localization was measured by immunohistochemical (IHC) analysis of tissue biopsies. The complete absence of nuclear reactivity was coded as negative expression; we considered “any positive” as low nuclear expression (>0% but <10% of cells) and high nuclear expression (≥10% of cells). Women represented about half of the present series (52%), and the median age was 64 years (p25–p75 range: 53–75). Neoadjuvant and adjuvant treatments were administered to 10% and 54% of the cases, respectively. Extended systemic lymphadenectomy (D2) was the most common (54%). In nearly all cases, the number of retrieved nodes was ≥15, i.e., adequate for tumor staging (94%). An R0 resection was achieved in 80% of the cases. Most patients were pathological T stage 3 and 4 (pT3/pT4 = 79.0%) and pathological N stage 2, 3a, and 3b (pN2/pN3a/pN3b = 47.0%) at the pathological examination. Twenty patients (15%) presented distant metastases. Five-year overall survival (OS) was significantly higher (p = 0.029) in patients with negative YAP (46%, 95% CI 31.1–60.0%) than in the other patients (27%, 17.5–38.1%). Moreover, when controlling for sex, age, pT, pN, and percentage of signet ring cells in the multivariable analysis, YAP expression was a significant predictor of OS (HR 2.03, 95% CI: 1.18–3.51, p = 0.011). Our results provide new insights into the role of the YAP signaling cascade, as its activation was associated with a worse prognosis in PC GC.

A atriz foi vítima de violência sexual que culminou numa gravidez indesejada e posterior doação da criança. Healthcare-associated infections (HAIs) may have grave consequences for patients. In the case of sepsis, the 30-day mortality rate is about 25%. HAIs cost EU member states an estimated 7 billion Euros annually. Clinical decision support tools may be useful for infection monitoring, early warning, and alerts. MONI, a tool for monitoring nosocomial infections, is used at University Hospital Vienna, but needs to be clinically and technically revised and updated. A new, completely configurable pipeline-based system for defining and processing HAI definitions was developed and validated. A network of data access points, clinical rules, and explanatory output is arranged as an inference network, a clinical pipeline as it is called, and processed in a stepwise manner. Arden-Syntax-based medical logic modules were used to implement the respective rules. The system was validated by creating a pipeline for the ECDC PN5 pneumonia rule. It was tested on a set of patient data from intensive care medicine. The results were compared with previously obtained MONI output as a suitable reference, yielding a sensitivity of 93.8% and a specificity of 99.8%. Clinical pipelines show promise as an open and configurable approach to graphically-based, human-readable, machine-executable HAI definitions.

The periodization of fine and applied art after the industrial revolution and the response of alienated artists to industrial kitsch testifies to the utility and purpose of art—the artistic and social avantgardes and the crown of all their endeavors in the heroic period. From art and craft pastiche, to utopian efforts to reform society through design, a rich linear syntagm was structured that intrigued and burdened the creatives from this region, who sought to introduce the land of peasants and barbarogens into the currents of industrial and social progress, culture and art. One of those individuals, in whom Morris and Marinetti, Van Doesburg and Itten, Gropius and Meyer, Vassarelli and Dibiffe... Müller-Brockman and Rand conflicted, tirelessly pursued the affirmation and institutionalization of applied art and design through pedagogical, editorial, theoretical and research work, as well as visual and graphic practice. Miodrag Miša Nedeljković was a modernist with a small 'm', artist and designer, theoretician and practitioner, who nomadically moved through the currents of modern and postmodern fine and applied art and design with renaissance curiosity, driven by intrigue and logic, was primarily concerned with the emergence and establishment of circumstances, and environmental issues.

Under the United Nations Charter, the U.N. Security Council has several important functions and powers, not least with regard to taking binding actions to maintain international peace and security. The ten elected members have the opportunity to influence this area and others during their two-year terms on the Council. In this paper, we aim to illustrate some of these opportunities, identify potential guidance from prior elected members’ experiences, and outline the key procedures that incoming elected members should be aware of as they prepare to join the Council. In doing so, we seek in part to summarize the current state of scholarship and policy analysis in an effort to make this material more accessible to States and, particularly, to States’ legal advisers. We drafted this paper with a view towards States that have been elected and are preparing to join the Council, as well as for those States that are considering bidding for a seat on the Council. As a starting point, it may be warranted to dedicate resources for personnel at home in the capital and at the Mission in New York to become deeply familiar with the language, structure, and content of the relevant provisions of the U.N. Charter. That is because it is through those provisions that Council members engage in the diverse forms of political contestation and cooperation at the center of the Council’s work. In both the Charter itself and the Council’s practices and procedures, there are structural impediments that may hinder the influence of elected members on the Security Council. These include the permanent members’ veto power over decisions on matters not characterized as procedural and the short preparation time for newly elected members. Nevertheless, elected members have found creative ways to have an impact. Many of the Council’s “procedures” — such as the “penholder” system for drafting resolutions — are informal practices that can be navigated by resourceful and well-prepared elected members. Mechanisms through which elected members can exert influence include the following: Drafting resolutions; Drafting Presidential Statements, which might serve as a prelude to future resolutions; Drafting Notes by the President, which can be used, among other things, to change Council working methods; Chairing subsidiary bodies, such as sanctions committees; Chairing the Presidency; Introducing new substantive topics onto the Council’s agenda; and Undertaking “Arria-formula” meetings, which allow for broader participation from outside the Council. Case studies help illustrate the types and degrees of impact that elected members can have through their own initiative. Examples include the following undertakings: Canada’s emphasis in 1999–2000 on civilian protection, which led to numerous resolutions and the establishment of civilian protection as a topic on which the Council remains “seized” and continues to have regular debates; Belgium’s effort in 2007 to clarify the Council’s strategy around addressing natural resources and armed conflict, which resulted in a Presidential Statement; Australia’s efforts in 2014 resulting in the placing of the North Korean human rights situation on the Council’s agenda for the first time; and Brazil’s “Responsibility while Protecting” 2011 concept note, which helped shape debate around the Responsibility to Protect concept. Elected members have also influenced Council processes by working together in diverse coalitions. Examples include the following instances: Egypt, Japan, New Zealand, Spain, and Uruguay drafted a resolution that was adopted in 2016 on the protection of health-care workers in armed conflict; Cote d’Ivoire, Kuwait, the Netherlands, and Sweden drafted a resolution that was adopted in 2018 condemning the use of famine as an instrument of warfare; Malaysia, New Zealand, Senegal, and Venezuela tabled a 2016 resolution, which was ultimately adopted, condemning Israeli settlements in Palestinian territory; and A group of successive elected members helped reform the process around the imposition of sanctions against al-Qaeda and associated entities (later including the Islamic State of Iraq and the Levant), including by establishing an Ombudsperson. Past elected members’ experiences may offer some specific pieces of guidance for new members preparing to take their seats on the Council. For example, prospective, new, and current members might seek to take the following measures: Increase the size of and support for the staff of the Mission to the U.N., both in New York and in home capitals; Deploy high-level officials to help gain support for initiatives; Partner with members of the P5 who are the informal “penholder” on certain topics, as this may offer more opportunities to draft resolutions; Build support for initiatives from U.N. Member States that do not currently sit on the Council; and Leave enough time to see initiatives through to completion and continue to follow up after leaving the Council.
