Background: AIDS was the first recognized in U.S in summer of 1981 at centers for disease control and prevention reported the unexplained occurrence of pneumocystis jiroveci in previously healthy homosexual men in Los Angeles and Kaposi. First time in 1983 HIV syndrome was isolated from a patient of lymphadenopathy. Person with positive HIV serology who have ever had a CD4 lymphocyte count below 200celles/mcl and CD4 lymphocyte percentage below 14% are considered to have AIDS (CMDT 2017).Methods: This study was continuous longitudinal, prospective and retrospective, observational, at ART plus Centre, Kanpur K.P.S. institute of medicine (G.S.V.M. medical college) included the all patients on ART1 attending in Centre were screened for treatment failure based on clinical, immunological and virological criteria’s as decided by SACEP from 2016 to 2018.Results: Total numbers of patients are 118 among them 71 female and 47 males, age groups between 30-40 there are 54 patients. In study treatment with ART patients Hb levels more than 10%, Mean value before 10.85±1.31 and mean value after treatment was 10.5±1.31, TLC before 6970.94±6309.93 after treatment 6800.25±2522.99, Serum Bilirubin before and after treatment 0.69±0.49 and 0.95±1.13. Mean value of before and after treatment serum creatinin1.80±11.34 and 0.88±0.38.Conclusions: There is increased in serum creatinine and SGPT /SGOT and decrease in Hb levels in treatment of second line ART treatment so it should be monitored every monthly interval.

Abstract The text is a rewritten lecture on the staging of Mateja Bučar's dance installation On(the)line performed in Cankarjev dom on 20 October 2017. It draws parallels between a staged art situation and the philosophy of Gilles Deleuze whose book The Logic of Sense discusses the line of sense, the surface that separates bodies and events and represents the line of nonsense that produces sense and language. Conversely, the simple gesture of a line drawn along the lobby of Cankarjev dom in the performance On(the)line is one that establishes an extraordinary situation, a line of artistic nonsense that has, as Andrew Hewitt would say, the power to switch between the aesthetic and the political.

One of the core challenges in visual multi-target tracking is occlusion. This is especially important in applications such as video surveillance and sports analytics. While offline batch processing algorithms can utilise future measurements to handle occlusion effectively, online algorithms have to rely on current and past measurements only. As such, it is markedly more challenging to handle occlusion in online applications. To address this problem, we propagate information over time in a way that it generates a sense of déjà vu when similar visual and motion features are observed. To achieve this, we extend the Generalized Labeled Multi-Bernoulli (GLMB) filter, originally designed for tracking point-sized targets, to be used in visual multi-target tracking. The proposed algorithm includes a novel false alarm detection/removal and label recovery methods capable of reliably recovering tracks that are even lost for a substantial period of time. We compare the performance of the proposed method with the state-of-the-art methods in challenging datasets using standard visual tracking metrics. Our comparisons show that the proposed method performs favourably compared to the state-of-the-art methods, particularly in terms of ID switches and fragmentation metrics which signifies occlusion.

Background First-line antiretroviral treatment failure has become a public health concern in high, low and middle-income countries with high mortality and morbidity In Ethiopia, around 710,000 peoples were living with HIV and 420,000 of them were receiving ART in 2017. Little is known about the magnitude of first-line ART treatment failure and its associated factors in Ethiopia, particularly in the study area. Therefore, this study was aimed to find the magnitude of first-line ART treatment failure and its associated factors among adult patients attending ART clinic at Southwest shoa zone public hospitals. Methods Institutions based cross-sectional study was employed from February 1 to April 2, 2019. An interviewer administered questionnaire was used to collect data from 350 adult patients on ART using a systematic random sampling technique. The collected data were coded and entered into Epidata version 3 and exported to STATA SE version 14 for analysis. Bivariable and multivariable logistic regression was done to identify factors associated with first-line ART treatment failure. At 95% confidence level strength of association was measured using Odds ratio. Variables with a p-value of ≤ 0.25 in the bivariable analysis were considered as a candidate variable for multivariable analysis. To get the final variables step-wise backward selection procedure was used and those in the final model were selected at a p-value <0.05. Finally, texts, simple frequency tables, and figures were used to present the findings. Results In this study the magnitude of first-line ART treatment failure was 33.42%. Absence of baseline opportunistic infection AOR = 0.362 (95%CI0.178, 0.735), Staying on first-line ART for <5 years AOR = 0.47 (95%CI 0.252, 0.878), Nevirapine containing ART regimen AOR = 3.07 (95%CI 1.677, 5.63), Baseline CD4 count ≥100 cells/mm3 AOR = 0.299 (95%CI 0.152 0.591), absence of opportunistic infections after ART initiation AOR = 0.257 (95%CI 0.142, .467), time taking greater than an one-hour to reach health facility AOR 1.85 (95%CI 1.022 3.367) were significantly associated with first-line ART treatment failure. Conclusion The magnitude of first-line ART treatment failure was high in the study area. Base-line opportunistic infection, duration on first-line ART, NVP based ART, Baseline CD4 count level, OI after ART initiation, and time it takes to reach health facility were independent determinants of first-line ART treatment failure.

After Z test application the value is 13.316, p value 0.0001, MD 231.5, CI is 95%.Viral load value at time of start of second line ART mean value is 80683.85849±293449.2038, after 1 to 12 month of start of second line ART treatment mean value is 350.8559±128.1069, for this Z value is 2.972, p value 0.0033.Conclusions: Second line ART is most effective in treatment after failure with first line ART. CD 4 count increase and viral load decrease and clinical feature improve after treatment of ART 2.

The purpose of this paper is to evaluate the feasibility and clinical dosimetric benefit of an on-line, that is, with the patient in the treatment position, Adaptive Radiation Therapy (ART) system for prostate cancer treatment based on daily cone-beam CT imaging and fast volumetric reoptimization of treatment plans. A fast intensity-modulated radiotherapy (IMRT) plan reoptimization algorithm is implemented and evaluated with clinical cases. The quality of these adapted plans is compared to the corresponding new plans generated by an experienced planner using a commercial treatment planning system and also evaluated by an in-house developed tool estimating achievable dose-volume histograms (DVHs) based on a database of existing treatment plans. In addition, a clinical implementation scheme for ART is designed and evaluated using clinical cases for its dosimetric qualities and efficiency.

"Bird and Line" is an artistic inquiry into the relationship between a deep state of artistic consciousness and the act of drawing a line to arrive at the form of a bird. This inquiry further proposes that by using line as a mode of research, the artist begins to perceive the consciousness of a bird and the relationship it shares with its form. For me, the embodied and porous experiences of watching and knowing birds through the practice of working with "line" as an artistic element allow for an intimacy of experiencing and an unfolding of intersubjectivity. Artistic inquiry also acts as an investigation into self-awareness and self-realization in this space of making eco-art. These acts of being lead me into reflections on how perception and creativity are melded together during creative moments to allow for a porous consciousness to emerge and perform the act of drawing a bird. As an artist working with text, movement, and image, I embed questions on the ethics of creativity into how we evolve our lines of art, as well as encounter other beings. By unraveling the relationship between the inner and the outer through Indian aesthetic philosophy, I evolve methods for eco-art practice using line as an element. I emphasize the importance of artistic research with a framework of Indian aesthetics as a way of deepening our perception and relationships with the natural world. This article is written to make artistic processes visible through a reflective auto-ethnographic approach. I write in a non-linear reflective narrative to comprehend cultural ontologies that drive my practice, unfolding internal thought processes, directions of research, and moments of mystical experience.

Established antiretroviral therapy (ART) programs in sub-Saharan Africa have well-defined first-and second-line therapies but no standard third-line ART regimen. The impact of third-line ART on patients with multiclass-resistant HIV in resource-limited settings has not been well characterized. We conducted a retrospective review of patients on third-line ART at the University Teaching Hospital in Lusaka, Zambia. We assessed virologic and immunologic outcomes following 6 months of third-line therapy and found among those with a documented viral load, viral suppression (≤1000 copies/ml) at 24 weeks was 95% (63/66) with a mean increase in CD4 count of 116 cells/mm3 and viral suppression of 63% (63/100) by imputation of missing data. This study suggests that third-line therapy is clinically and virologically effective among patients with multiclass-resistance in a resource-limited setting in sub-Saharan Africa.

Grimaces are important facial expressions used in situated interactions for questioning social hierarchies and power relations. They counter the warmth of the smile and its invitation for shared foci of attention. In the photographs that surround us, the grimace is seldom to be found, whereas the smile is an almost ubiquitous expression in depictions in our “facial societies” (Macho, 2011). Interestingly, the need for depicted smiles provides us increasingly with images that depict not only smiles but, ever more, “as if” versions of smiles, facial expressions that we can identify with Goffman as “teeth grimaces”. While teeth grimaces reveal some aspects of how smiles are produced for depiction for our facial societies, the paper further suggests that we need more grimaces and fewer smiles in order to remember how social hierarchies and power relations, far from being abstract phenomena, must be constantly renegotiated in situated interactions. Background: ART failure is a growing public health problem and a major threat to the progress of HIV/AIDS control. In Uganda however, little is documented on treatment outcomes and their associated factors among individuals on second line ART regimen. The rapid scale-up of ART over the past has resulted in substantial reductions in morbidity and mortality. However, as millions of people must be maintained on ART for life, individuals with ART treatment failure are increasingly encountered and the numbers are expected to rise. This could be attributed to factors such as sub-standard regimens, limited access to routine viral load monitoring, treatment interruptions, suboptimal adherence, among others. The purpose of this study was to estimate 5-year cumulative treatment failure and the associated factors among individuals on second line ART regimen Eastern Uganda. Materials and methods: A retrospective analysis of 541 records of HIV positive individuals, switched to second line ART regimen from January 2012 to December 2017. Inferential statistics including the Chi square test and multivariable logistic regression analysis was applied to determine associations of treatment failure against of the selected demographic, laboratory and clinical factors was performed. Associations between treatment failure and the predictors was based on a P-value of less than 5% and confidence intervals level of 95%. Results: We reviewed 541 records of individuals on second line ART regimen, of which 350 (64.7%) were female, 226 (41.8%) were married, and 197 (36.4%) were older than 35 years. The mean age at ART initiation was 30 years (SD = 14.8), while the mean weight at ART initiation was 47 kg (SD = 18.6), (range 4-97 kg). The overall proportion of treatment failure was 23%. The cumulative mortality risk for 5 years was 12.4% and the mortality rate was 2.5 deaths per 100 individuals per year. The odds of developing treatment failure among individuals switched to ATV/r-based regimen were 44% lower as compared to individuals who were switched to LPV/r (ORadj0.56, 95% CI 0.35-0.90, P = .016). while the odds of experiencing treatment failure among individuals that used AZT at ART initiation were 43% lower as compared to individuals that used a TDF based regimen at ART initiation (ORadj0.57, 95% CI 0.33-0.98, P = .041). Conclusion: The 5 year cumulative incidence of treatment failure in a cohort of 541 individuals was 23%. The type of protease inhibitor (PI) used in second line regimen and use of AZT at ART initiation were significantly associated with treatment failure. Our study also shows that the cumulative mortality risk while on second line ART regimen was 12.4% while the mortality rate was 2.5 deaths per 100 individuals per year. Given the high level of treatment failure among individuals on second line ART regimen, yet the current ART protocols limits the use of third line ART regimens to only regional referral hospitals, the Ministry of Health should strengthen the surveillance systems for identifying individuals failing on second line ART regimen even at district hospitals and lower health facilities to facilitate timely switch to optimal regimen. The Ministry of health through the Quality Improvement Division should conduct routine onsite support supervision to sites offering ART to ensure that treatment guides and other standard of care like timely switch to appropriate regimens among others are being adhered to. Knowledge gaps identified can also be addressed through onsite Continuous Medical Educations.

HIV by ART2.Methods: It was Single centre hospital based clinic pathological study and continuous longitudinal, prospective and retrospective , observational, in ART PLUS Centre considering of all patient on 1st line ART who are screened for treatment failure based on clinical, immunological and virological criterias as decided by SACEP from 2016 to 2018.Results: This study shows increase in serum bilirubin from 5 to 13 in study subject and there was increase in no from 4 to 5 in subject of less than 9 Haemoglobin and in the group of 9 to 10 level Haemoglobin of subjects no. increase from 4 to 5 but more than 10 subject decrease from 110 to 108. Subject increase 6 to 10 of serum creatinine level >1.5mg /dl., This shows second line ART have adverse effect on liver , kidney and hematology. There is some adverse effect is seen as nausea, vomiting, headache, skin rash, abdominal pain and muscle pain, there was some serious adverse effect that required hospitalization.Conclusions: Study show that ART2 have adverse effect on liver and kidney function and anaemia. Some general adverse effect seen that also required treatment.

238 Background: We aimed to evaluate the treatment sequence for patients with metastatic castration-resistant prostate cancer (mCRPC) in real-world practice and compare overall survival in each sequential therapy. Methods: We retrospectively evaluated 146 patients with mCRPC who were initially treated with androgen deprivation therapy as metastatic hormone-naïve prostate cancer in 14 hospitals between January 2010 and March 2019. The agents for the sequential therapy included new androgen receptor-targeted agents (ART: abiraterone acetate or enzalutamide), docetaxel, and/or cabazitaxel. We evaluated the treatment sequence for mCRPC and the effect of sequence patterns on overall survival. Results: The median age was 71 years. A total of 35 patients received ART-ART, 33 received ART-docetaxel, 68 received docetaxel-ART, and 10 received docetaxel-cabazitaxel sequences. The most prescribed treatment sequence was docetaxel-ART (47%), followed by ART-ART (24%). Overall survival calculated from the initial diagnosis reached 83, 57, 79, 37 months in the ART-ART, ART-docetaxel, docetaxel-ART, and docetaxel-cabazitaxel, respectively. Multivariate Cox regression analyses showed no significant difference in overall survival between the first-line ART (n = 68) and first-line docetaxel (n = 78) therapies (hazard ratio: HR 0.84, P = 0.530), between the ART-ART (n = 35) and docetaxel-mixed (n = 111) sequences (HR 0.82, P = 0.650), and between the first-line abiraterone (n = 32) and first-line enzalutamide (n = 36) sequences (HR 1.58, P = 0.384). Conclusions: The most prescribed treatment sequence was docetaxel followed by ART. No significant difference was observed in overall survival among the treatment sequences in real-world practice.

Electron-off-axis holography has proven to be a most promising tool for the collection of the complete information about amplitude and phase modulation of the complex electron image wave in one single micrograph. Then amplitude and phase can be reconstructed numerically, offering almost any wave-optical possibility for the evaluation of the electron object wave at the exit surface, including the elimination of the influence of coherent aberrations of the objective lens in the image wave. Bottle-neck of the whole two step - registration and reconstruction - procedure has long been the need for highly accurate conversion of the holograms to numerical data as well as the available computational power, since the necessary Fast Fourier Transforms are time consuming numerical operations. Modern CCD slow-scan detectors and the permanently increasing computing power speed up this process and. combined with state of the art FEG microscopy, supply the reconstructed amplitude and phase of the image wave to the microscopist on-line.

A (non-exhaustive) survey of new and existing technologies for the monitoring of wastewater treatment plants is presented. Emphasis is given to the way these sensors can provide insight in the ongoing (bio-) processes. Three different uses for sensors can be found: for monitoring (operator support), in automatic control systems and as tools for plant auditing/optimization/modelling by consultants. From this, sensors have been classified in two basic types: (i) reliable, simple and low maintenance sensors for day-to-day monitoring and control and (ii) advanced, higher maintenance sensors that are used in auditing, model calibration and optimisation. The paper is organized according to the typical unit processes of biological wastewater treatment systems: anaerobic digestion, activated sludge, nutrient removal and sedimentation. Attention is drawn to a number of practical problems associated with the use of sophisticated sensors in the harsh (dirty) conditions of wastewater treatment processes. The use of autocalibration and built-in sensor checks, cleaning systems and reliable sample preparation units is illustrated. The paper ends with a discussion of the applicability of the different sensors.

Abstract This essay examines, from a position within Classics, different angles on critiques of historicism and the turn to anachronism in History, Art History, Medieval Studies, and Queer Theory before proposing the idea of ‘kairological history’, on the model of the artist Paul Chan’s ‘kairological art’. On this analysis, ‘kairological history’ engages the critical and creative resources of anachronic thinking alongside tools of historicism (e.g. empiricism, successionism, periodization, alterity) in making choices about ‘telling time’. These choices reflect a critical understanding of how temporality shapes the valuation of the past, particularly in relation to a ‘classical’ past; the negotiation of identity and difference between past and present; and the kinds of communities that history aims to support. The second half of the essay examines two instances of anachronism within the history of anatomy, one from Galen and one from the early twenty-first century. Both cases represent problems that historicism can correct. But the modality of correction, in itself, is anaemic and risks the very teleology that linear history is so often faulted for. The essay therefore explores what gets lost and what gets found when temporality is aligned with linearity, as well as non-linear modes of telling time.

Background: Antiretroviral therapy is one of the reasons for falling trend of HIV epidemic at present. The clinical efficacy, toxicity and reasons for failure of first line ART is understudied. This study aimed to determine the frequency rates and reasons for discontinuation of first line ART in a cohort of HIV positive adult patients.Methods: Cross sectional study was conducted on 11,968 patients of HIV registered at Victoria Hospital ART centre from 2011 to 2017. Using a structured proforma, relevant information was collected from patients taking first line ART. Descriptive statistics was used for analysing the results obtained.Results: Total 11,968 HIV patients were registered at ART centre during our study period of which only 4,008 patients were taking ART among them, 167 patients were referred for initiation of 2nd line ART. After evaluation 28 were continued on First line,1 opted out, 20 were transferred out,1 discontinued treatment, 17 died, 14 were lost to follow up, 5 were excluded from the study and only 81 patients were started on second line ART. Failure rate of first line ART in our study was 2.02%. Immunological failure followed by clinical failure were the most common reasons for changeover in this study. Tuberculosis was the most common comorbid disease in this study.Conclusions: First line ART is very effective and well tolerated and has a low failure rate. Low CD4 count, anaemia, raised ALP, low albumin were among the factors associated with treatment failure. WHO staging did not correlate with the treatment failure, recommended routine viral load monitoring for assessing treatment failure.

Background: ART introduction was a breakthrough step and a boon for HIV infected and AIDS patients. Since the launch of world’s second largest ART programme by Indian Government on 1st April 2004, duration and quality of patient’s life has improved significantly. But after years of treatment with first line ART drugs clinical deterioration is being observed in several patients. This study was done to find out clinical symptoms present in treatment failure cases and to look for any improvement after one year of second line ART.Methods: This is a Single tertiary care teaching hospital based clinical (assessment with investigation) continuous longitudinal, prospective and retrospective, observational study at ART plus Centre, Kanpur. All patients on 1st line ART treatment in the centre were screened for treatment failure as decided by SACEP from 2016 to 2018. Treatment failure was suspected in patients on first line ART treatment with deterioration or non-improvement of clinical symptoms which was later confirmed by immunological and virological test.Results: Study included 71 (61%) female and 47 (39%) male patients. Heterogeneous mode of transmission of disease was most common (97 (82%)) followed by Blood transfusion (11 (9%)). HIV infection prevalence in age group of 30 to 40 years was 54 (45%). Most common clinical features of the patients with first line ART treatment failure were weight loss (105 patients), chronic diarrhoea (56 patients), fever (34 patients), and cough (24 patients). After treatment for one year with 2nd line ART drugs above symptoms were reduced to 20, 5, 5 and 12 patients respectively.Conclusions: First line ART failure was most common among female patients and heterogeneous mode of transmission was the commonest. Most common clinical features of first line ART failure were weight loss, chronic diarrhoea, fever, and cough which were reduced significantly after treatment with 2ndline ART drugs for one year.

Many existing methods for video summarisation are not suitable for on-line applications, where computational and memory constraints mean that feature extraction and frame selection must be simple and efficient. Our proposed method uses RGB moments to represent frames, and a control-chart procedure to identify shots from which keyframes are then selected. The new method produces summaries of higher quality than two state-of-the-art on-line video summarisation methods identified as the best among nine such methods in our previous study. The summary quality is measured against an objective ideal for synthetic data sets, and compared to user-generated summaries of real videos.

I discuss errors in theory and in interpreting observations that are produced by the failure to consider resolution in space, time, and energy. I discuss convection in stellar model atmospheres and in stars. One dimensional convective models can never work well, but the errors in predicted diagnostics for temperature, gravity, and abundances can be calibrated. I discuss the variation of microturbulent velocity with depth, effective temperature, and gravity. These variations must be dealt with in computing models and grids and in any type of photometric calibration. I describe the state of the art in computing a model atmosphere and complete spectrum for stars with arbitrary abundances. I have made no significant progress on atomic or molecular line data for the last two years, but I will soon begin large scale production. I will continue to distribute the results on CD-ROMs, and I will make them available on the World Wide Web.

Background: Although HAART has successfully controlled the progression of HIV and helped to prevent the disease significantly, new challenges are heading up and ART failure despite of good adherence is becoming a major hurdle in the way of HIV control. Considering these facts present study was planned with an objective to study clinical and immunological profile of antiretroviral therapy failure HIV patients.Methods: This observational study was conducted at ART plus centre of one of the tertiary health care centre of India from February 2018 to September 2019. Patients satisfying inclusion and exclusion criteria were interviewed by using semi-structured questionnaire and were examined and investigated for any opportunistic infections and their CD4 counts were studied.Results: Among total 4098 patients alive on ART at present study centre 90.6% were responding well to the 1st line of ART, while 8.7% had failed to 1st line of ART but responding well to the 2nd line whereas 0.7% had failed to 2nd line and were initiated on 3rd line of ART. Failure of ART was the cause for change in regimen among 84.9% of patients. Median of rise in CD4 count at 6 months from switch to 2nd line was 137 cells/mm3.Conclusions: Antiretroviral therapy has significantly improved outcome of the disease. Failure of ART is the major cause for change in ART regimen. Majority of patients in failure had WHO clinical stage 1 and 2.
