A descriptive study was conducted to examine the relationship between family support and elder well-being of a group of ambulatory elders living in the community. Information regarding the elder parent's reporting of the overall support received by adult children and its affect on well-being was sought. Forty five female subjects, aged 65 to 91 years, participated in this study. The subjects completed a questionnaire which included the Demographic Questionnaire, Family Support Exchange Questionnaire, Family APGAR, and Philadelphia Geriatric Center Morale Scale. The sample population was homogeneous and no significant correlations were demonstrated between quantity or quality measures of family support and elder well-being. The study findings were consistent with previous studies in that elder parents generally live in close proximity to at least one adult child and have frequent contact with them. Elderly parents were satisfied with the support they received from children and would confide in their children.

This study investigated the relationship between frequency of musical activity participation, Sense of Coherence (SOC), and subjective report of health in older adults. From a sample of 197 older adults from Muncie and Anderson, Indiana, 129 (65.5%) individuals participated in this study. The participants completed an information sheet, the Musical Activity Participation Report, SOC, and Perceived Health questionnaires. Pearson r correlation coefficients found frequency of musical activity participation was not correlated with SOC but was correlated with subjective reports of health. A frequency table and chi-square analysis revealed that cohort and education were related to musical style preferences. A one-way analysis of variance (ANOVA) revealed that musical activities enhanced perceived quality of life to a lesser degree than other activities. However, many participants commented on the positive impact of music on well-being. A one-way ANOVA and t-tests found gender and musical upbringing were related to frequency of musical activity participation.

The purpose of this study was to examine the relationship between health promotion and quality of life in noninstitutionalized older adults. The study was conducted within the framework of Nola Pender's Health Promotion Model.Approval for this study was obtained from the Institutional Review Board of Ball State University for human protection of the participants. Permission from the five churches' boards was obtained. Written assurance of anonymity of subjects was given.This study was descriptive and correlational. Quality oflife was the dependent variable and the independent variables were health promoting behaviors, importance of health, perceived health status, the number of chronic health conditions and any consequent disruption to life, prior involvement in a senior citizens' wellness group or with a health advisor, and demographic variables including age, gender, marital status, living alone or with a companion or family, and education. Volunteer subjects aged 65 and older from five church groups were asked to complete the following questionnaires: Health Promoting Lifestyle Profile, Quality of Life Index, Value Survey, and a demographic sheet. The demographic questionnaire included an item regarding self-perceived health status, whether or not the participant had been involved in a wellness group or with a health advisor for older adults, and a checklist of chronic health conditions and whether or not these had had debilitating consequences for the participant.Hypotheses included the following: (1) There is no correlation between health promoting behavior and quality of life among older adults. (2) There is no correlation between importance of health and quality of life among older adults. (3) There is no correlation between perceived health status and quality of life among older adults. (4) There is no correlation between the number of chronic health conditions and quality of life among older adults. (5) There is no correlation between disruption brought about by chronic health conditions and quality of life among older adults. (6) There is no significant difference between those older adults who had participated in a senior citizens' wellness group or with a health advisor and those who had not. (7) There are no significant differences in older adult males and females and those who live alone or with family or a companion regarding quality of life. (8) There is no correlation between age and quality of life among older adults. (9) There is no correlation between number of years of education and quality of life among older adults.Statistical significance was found between Quality of Life Index and Health Promoting Lifestyle Profile scores (r=0.24, p<0.05) and Quality of Life Index and self-perceived health status scores (r=0.33, p<0.01). Other correlations, t-test, and analyis of variance did not achieve statistical significance.

Introduction: Resilience is an important trait and mechanism for older adults facing adversity in their later life. Physical activity has been established as an important determinant of mental health among older adults, in which relationship of resilience may play a mediating role. In order to examine the relationships among resilience, physical activity, and mental health in older adults, the current project is designed to conduct three studies. Study-1 aims to use qualitative method to identify the characteristics that contribute to resilience in a group of community-dwelling older adults in Hong Kong. Study-2 aims to develop and validate an age-specific resilience scale, named as "Resilience Scale for Chinese Older Adults (RSCOA)". Study-3 aims to test a model in which physical activity is hypothesized to influence mental health in path mediated by resilience. Methods: Twenty-five Chinese older adults (2 males, 23 females) aged 69 to 100 years (M=80.00, SD=39.08) were included in Study-1. Interviews were conducted to collect information about the adverse events in their lives, as well as their attitudes towards adversity and the beliefs underlying their approaches to overcoming adversity. The transcripts were analysed using qualitative content analysis. In Study-2, a sequential approach was used to develop the RSCOA in three stages. In stage 1, scale candidate items were generated and initial content validity of the RSCOA were explored. In stage 2, the factorial structure of the RSCOA was extracted using exploratory factor analysis. In stage 3, exploratory structural equation modeling was conducted to test the structure. The reliability and validity of the RSCOA were also examined. In Study-3, cross-sectional data was collected from a sample of 293 Chinese older adults [60 males, 233 females; age from 65 to 95 years old (M=76.58, SD=7.02)] from eight community senior centres located in three districts in Hong Kong. Correlation analyses were performed and path analysis was employed to assess the associations among physical activity, resilience and mental health variables. Results: In Study-1, 7 themes were emerged from the interviews: 1). Equanimity- the realisation that life has both joys and sorrows. 2). Positive attitudes towards life. 3). Meaningfulness- the realisation that life has a purpose and is full of hope. 4). Self-reliance- a belief in one's capabilities and the realisation that each person's life path is unique. 5). Social support- which comes from family and friends, health care professionals, staff in the senior centres and others' appraisal. 6). Environmental support- which includes the support from neighbours, community senior centres and government. 7). Spirituality and faith- which can take the form of a belief in destiny, gratitude or religious faith. For the scale development and validation, a pool of 70 candidate items for RSCOA was established with good content validity in stage 1. In stage 2, seven factors, namely perseverance, self-reliance, spirituality, social support, living in the moment, environmental support, and meaningfulness, reflecting internal, external and existential dimensions, were identified and reflected in 27 items in the construct of resilience. In stage 3, the 27-item RSCOA was further examined, by which 3 items were deleted to achieve acceptable internal consistency (α= 0.87) and good concurrent as well as construct validities, and finally a 24-item RSCOA was confirmed. The measurement model displayed satisfactory model fit and proven invariant across gender. Building on the three path models, it was found that significant mediation effects of resilience on the relations from physical activity to depression (β =-.54, p=.002), from physical activity to loneliness (β=-.25, p=.005), as well as from physical activity to psychological well-being (β=.37, p=.008). In sum, the results confirmed that resilience mediates relationship from physical activity to mental health among the older adults. Conclusion: The qualitative analysis found that resilience of the Hong Kong older adults is characterised by internal, external and existential factors. A reliable and valid age-specific instrument for measurement of resilience in Chinese older people was developed. Older adults who are more physically active reported greater psychological well-being and less mental health problems, and the effects were mediated by resilience. The results may be useful for developing interventions that aim at assisting older adults to enhance the positive experiences and mental health in their lives.

Moving into an institutional environment may cause feelings of anxiety, depression, grief or loss in the increasing number of elderly residents in long term care facilities. The lived experience of pet visitation was explored as a possible remedy. This study is significant because findings provide information about intervention strategies to assist residents in comfort and adjustment.Five themes, identified in ten interviews of residents in three midwest long term care facilities, illustrated that pet interaction: (a) contributed a sense of responsibility, environmental control, (b) afforded unconditional caring, companionship, (c) provided a connection with family and home. (d) furnished a sense of identity in the facility community, and (e) provided a sense of being protected within the facility. Evidence supports that pet visitation is a beneficial experience to the residents in long term care facilities. Study results are available for facility administrators as encouragement and justification to establish pet visitation programs.

Findings were that caregivers considered all items on the Information Needs and Patient Care Needs subscales to be important but most of the unmet needs were from the Patient Care subscale. The needs less satisfied in relation to importance were (a) ways to improve patient appearance, (b) activities that will make patient feel purposeful, (c) information on how to give medications, (d) ways to reassure patient, (e) ways of coping with patient's diagnosis, (f) ways to dress patient comfortably, (g) ways to deal with patient's decreased energy, and (h) importance of not leaving patient alone.The implications for nursing are to assess and anticipate the needs of the caregiver of the stroke survivor so that his or her needs can be met. Preparing caregivers for their new role by meeting their needs will help the nurse met the primary goal of helping the patient.

The purpose of this dissertation was to understand how genetics, socioeconomic status (SES), and lifestyle factors influence the development of age-related macular degeneration (AMD), glaucoma, and diabetic retinopathy in an adult population in Dallas County. Two hundred fifty-three older adults participated in this study as the sample. Crosstabulation and binary logistic regression were utilized to analyze the data. Results indicated a disparity among participants' test scores, visual health status, and perceptions of their visual impairment and highlighted the fact that many seniors are not educated about age-related retinal disorders. Furthermore, variables reaching statistical significance were consistent with the literature included race/ethnicity, age, having a family history of both AMD and diabetes, frequency of eye exams, and level of education. The results not consistent with the literature as affecting visual health included health insurance, access to health care, body weight, and smoking status. Recommendations for future study included applied research focusing on determining risk factors, raising awareness, educating, and providing early detection of these diseases among low to middle income Caucasian, African American, and Hispanic older adults.

This study explores the health beliefs and preventive health behaviours of older Hong Kong Chinese people resident in Australia. Participation in physical activity was used as the case study. There were two frameworks used to shape the research. Because of their perceived influence on the health beliefs and practices of Hong Kong Chinese people, the traditional Chinese philosophies of Confucianism, Taoism, and Buddhism provided the philosophical framework. The Theory of Planned Behaviour provided a theoretical framework for understanding the target group's preventive health behaviour. Data was collected by means of in-depth interviews, participant observation and case study. Twenty-two informants were interviewed, their transcripts analysed, summarised and typologised, identifying six states of physical activity participation. Findings demonstrated that this target group possessed a holistic view of health, with food taking a special role in preventive care and self-treatment at times of illness. The Cantonese terms used to denote “physical activity” caused confusion among the target group. Most interpreted it as meaning deliberate planned body movement, strength-enhancing activities or exercise, although some did see it as including mundane daily activities and chores. Lack of time, no interest and laziness were reported as the main reasons for low participation in deliberate planned physical activity. Cultural, social and environmental determinants were the intrinsic and extrinsic factors influencing attitudes toward physical activity, as well as perceived social supports and perceived control over physical activity participation barriers. To a large extent, these interactive determinants of health were rooted in the three traditional Chinese philosophies mentioned above. The thesis concludes by arguing that rather than simply advocating activities designed for other populations, health promotion strategies and education need to create links to the traditions of this target group and also clarify their conception of physical activity.

The lifespan developmental framework views aging as a time of continued personal change and development. Death is perhaps the most salient developmental challenge older persons face. The relationship of self-transcendence, spirituality, and hope to personal death perspectives was the focus of this research. A descriptive correlational approach was used to study the relationships between these variables. Interviews were conducted with forty healthy adults aged sixty-five and older. Findings revealed that self-transcendence and spirituality were significantly correlated with positive death perspectives in this sample. Findings also revealed significant relationships between age and death perspectives, gender and self-transcendence, and gender and spirituality. No significant relationships were found between hope and death perspectives and hopelessness and death perspectives.

Objective: To evaluate the effectiveness of exercises on fall prevention among community-dwelling elderly. Design: Systematic review of randomized controlled trials. Method: The literature search of articles was conducted through the electronic databases of PubMed, Medline and EMBASE and manual search, and was confined to articles in English language with full text and publications from 2002 to 2012. Randomized controlled trials with exercise as the only intervention which aimed at reducing falls in older people aged 60 or above in community i.e. community-dwelling elderly were included. The primary outcomes were number of fallers, fall rate, time to the first and subsequent falls. Data Extraction: A total of 145 articles were retrieved through the electronic data bases (137 articles) and manual search (8 articles) of which 10 were selected after applying the inclusion criteria. According to the checklist developed by National Institute for Health and Clinical Excellence (NICE) for randomized controlled trials, the overall methodological quality of the 10 studies was rated as good as they had fulfilled 79%-93% of the assessment criteria in the NICE checklist. Results: The studies involved a sample size of 3,138 at the median age of 69 to 83. All the subjects were ambulatory and able to mobilize independently. The duration of exercise interventions ranged from 1.5 months to 12 months at a total of 11-156 hours. Eight studies showed that exercises were effective in fall prevention whereas two studies found no evidence of such effectiveness. The effect of exercises on fall prevention was multifactorial including the duration, frequency and continuity of the exercise programs, the health status of the target population and the individuals’ adherence to the exercise programs. Shorter exercise programs and make-up class arrangement resulted in higher adherence rate. Tailor-made (based on the functional capabilities of the subjects) and progressive (gradual increase of the intensity and challenge of the exercises during the intervention period) nature of the exercise programs also enhanced their effectiveness on the elderly. For the studies in which exercise did not have any positive effect on fall prevention, the subjects were generally older (median age at 81) and frailer (with 3-7 frail attributes). Conclusions: This review suggests exercise may be an effective intervention for fall prevention among community-dwelling elderly. Different types of exercise including strengthening, balance, endurance and weight-bearing exercises as well as Tai Chi may prevent falls. The elderly’s age and health status must be taken into consideration when designing exercise intervention programs for the elderly. Further researches are recommended to determine the optimal type, intensity, frequency and duration of exercises in fall prevention. Observations from the studies provide insights for future researches, such as the fall prevention effect of group and home exercises, tailor-made and progressive exercise programs. In Hong Kong, no randomized controlled trial studies have been conducted to examine different intervention programs on fall prevention. To achieve generalizabilty of the studies in the review, further local researches, particularly well designed and powered randomized controlled trials have to be conducted to assess the effect of different kinds of exercise interventions on fall prevention among the community-dwelling elderly.

This descriptive correlational study was designed to examine the relationship between situational control, and both psychological and physical well-being, in the institutionalized elderly. A convenience sample of 52 elderly institutionalized subjects was selected from two intermediate care facilities. The subjects completed the Perceived Weil-Being Scale, the Situational Control Of Daily Activities Scale, and the Subject Information Sheet. All residents were found to have an overall perception of situational control. However, residents perceived a lack of control for the daily activities of eating and grooming. The majority of residents obtained a moderately high score on the psychological well-being and physical well-being scales. No significant relationship was found between situational control and psychological well-being, or situational control and physical well-being. A significant positive relationship was found between psychological well-being and physical well-being.

ENGLISH ABSTRACT: Global aging is occurring at an unprecedented rate. South Africa has the highest proportion of older adults in Southern Africa, with nearly 7% of the population over the age of 60 years in 1997. However, although people are living longer, statistics show that they are not necessarily living healthier. The majority of women who outlive men have to deal with more chronic diseases as well as a poorer functional status than the latter. The purpose of the present study was to implement a movement competence programme suited to the needs of South African older adult women; requiring inexpensive apparatus and that can be performed in any environment. A time-series design was used which included follow up testing 9 months after the cessation of the movement competence programme. The intervention group consisted out of 21 (76.14±5.44 years) older adult women, who were randomly selected from a retirement village. The movement competence programme was broad based in nature and was performed in two one hourly sessions a week for 12 weeks. After pre-tests of functional capacity, selfperception and resilience, the older adult women were tested using the Physical Self-Perception Profile (Fox & Corbin, 1989) and the Resilience Scale (Connor & Davidson, 2003) respectively. Significant improvements (p<0.05) were observed in the Berg Balance Scale, 8-Foot Up-and-Go and the Physical Self-Perception’s results of the older adult women. No significant (p>0.05) difference was noted in the Barthel Index and Resilience Scale after the 12-week movement competence programme. Follow up testing indicated a significant improvement in the resilience of the older adult women who continued to exercise, compared to those that chose a sedentary lifestyle after the movement competence programme.
AFRIKAANSE OPSOMMING: Globale veroudering vind teen ’n ongekende tempo plaas. Suid-Afrika beskik oor die hoogste verhouding ouer volwassenes in Suidelike Afrika met amper 7% van die populasie in 1997 ouer as 60 jaar. Hoewel mense egter langer lewe toon statistiek dat hulle nie noodwendig gesonder lewe nie. Die meerderheid dames wat langer lewe as mans het te kampe met meer chroniese siektes asook ’n swakker funksionele status as dié van laasgenoemde. Die doel van die huidige studie was om ’n bewegingsbevoegdheidsprogram te implementeer wat aan die behoeftes van ouer Suid-Afrikaanse volwasse dames voldoen, waar goedkoop toerusting benodig word en in enige omgewing uitgevoer kan word. ’n “time-series” ontwerp was gebruik wat opvolgtoetse ingesluit het nege maande ná die beëindiging van die bewegingsbevoegdheidsprogram. Die intervensiegroep het bestaan uit 21 (76.14±5.44 jaar) ouer volwasse dames wat lukraak geselekteer is by ’n aftree-oord. Die bewegingsbevoegdheidsprogram was breed in fokus en was uitgevoer in twee eenuurlikse sessies per week vir 12 weke. Ná voortoetse oor funksionele kapasiteit, selfpersepsie en veerkrag, is die ouer volwasse dames getoets deur respektiewelik gebruik te maak van die Fisieke Selfpersepsie Profiel (Fox & Corbin, 1989) en die Veerkragskaal (Connor & Davidson, 2003). Beduidende verbeterings (p<0.05) van die ouer volwasse dames is waargeneem in die Berg Balansskaal, “8-Foot Up-and-Go” en die resultate op die Fisieke Selfpersepsie. Geen beduidende (p>0.05) verskil is waargeneem in die Barthel Indeks en Veerkragskaal ná die 12 weke aanbieding van die bewegingsbevoegdheidsprogram nie. Opvolgtoetse het ’n beduidende verbetering aangedui in die veerkrag van die ouer volwasse dames wat aangehou het met oefening in teenstelling met dié wat gekies het om ’n sedentêre leefstyl te volg nadat die bewegingsbevoegdheidsprogram voltooi is.

This research project aims to clarify the factors that impact successful aging in Assisted Living facilities (ALFs) in Denton County, Texas. We hypothesize that social disengagement decreases physical and mental components of quality of life. This exploratory research project employed standardized questionnaires to assess residents in the following domains; HRQOL, social engagement status, level of cognition, depression, and the level of functioning. This study collected data from 75 participants living in five ALFs. The average of Physical Component Scale (PCS) and Mental Component Scale (MCS) was 35.33, and 53.62 respectively. None of the participants had five or more social contacts out of facilities, and two-third of them had two or less social contacts. On average, those participants who were more socially engaged had higher score of MCS compared with disengaged counterparts. The level of physical function significantly affects social engagement, when people with more disabilities are more likely to be socially disengaged. Social engagement and depression significantly impact MCS, when depression is a mediating factor between social engagement and mental component of quality of life. Considering the expansion in aging population in the United States within the next three decades, the demand for high quality long-term care will skyrocket consequently. This study reveals that external social engagement can sustain HRQOL of residents in assisted living facilities.

Differences in Mexican American ethnicity, family and friend social support, and importance of diabetes self-management as related to diabetes management in the older adult population were evaluated with the University of Michigan Health and Retirement Study (HRS) 2003 Diabetes Study. Comparisons were made between Mexican Americans with Type II diabetes and similar non-Hispanic Caucasian and African American individuals with Type II diabetes. Neither family/friend social support nor importance of diabetes self-management were significant predictors of HbA1c levels. Results did not support the idea that perception of receiving support from family/friends or placing importance on diabetes self-management covaried with lower HbAlc level (family/friend: beta = -.13, t = -1.47, p = .143; self management: beta = .08, t = .55, p = .584).

This study was designed to measure differences between elderly and young adults in terms of their expression of positive and negative emotion. It was hypothesized that that there would be a difference between young and aged subjects in intensity of positive and negative emotional experience as measured by response to cartoons and the Survey of the Quality of Life of Adult Men and Women (SQLAMW, Malatesta & Kalnok, 1984). Sex differences as well as the effect of the sequence of cartoons were also tested.Emotions have been widely studied, yet researchers have rarely focused on the elderly population and those studies that do exist have methodological problems. Early research suggests a drift towards negative emotion with age, whereas more recent studies challenge this position.Volunteer subjects were 61 noninstitutionalized adults (24 males, 37 females) age 60 and over and 93 college undergraduates (36 males, 57 females) age 18-25. They met in groups of 2 to 10 to complete the research instruments.Positive and negative emotional reactions were obtained from ten point Likert scale ratings of 38 cartoons for funniness, pain, and hostility. Cartoons were presented in two sequences. Additional measurements of emotion were obtained from the SQLAMW.Four mean scores were calculated for each subject and served as dependent variables. Positive and negative affect on the SQLAMW generated two mean scores. A mean score for positive emotion on cartoons was calculated from ratings of funniness and for negative affect by combining ratings of pain and hostility. A 2 X 2 X 2 (Age x Sex x Cartoon Sequence) multivariate analysis of variance was conducted.Analysis for negative emotion resulted in a significant main effect for age on the SQLAMW and an age by sex interaction on cartoons. The young reported higher levels of negative emotion on the SQLAMW. However, cartoon ratings showed aged males higher than young males on negative emotion. For positive emotion, males scored higher than females on cartoons. All other effects were nonsignificant.

Social contact is known to be vital for older adults' mental and physical health, but few studies of social interactions have taken place in long-term care settings. The current study investigated whether the psychological well-being of assisted living residents was influenced by factors associated with residents' social interactions involving humor. Specific aims of the present study were to develop and test a measure related to humor-related social exchanges, to examine how humor-related social exchanges affect residents' mental health, and to explore whether humor-related social exchanges mediated the effects of resident and facility characteristics on indices of mental health. One hundred and forty older adults residing in 14 assisted living facilities in the Portland, Oregon metropolitan area were interviewed about the frequency and types of social interactions they experienced with members of their facility-based social networks, as well as depression, mood, loneliness, self-esteem, and self-rated health. A 12-item, two-factor model of humor-related social exchanges was identified through confirmatory factor analysis, including both positive and negative humor-related social exchange factors. The newly developed scale displayed evidence of adequate reliability and validly in the current sample. Results indicated that both positive and negative humor-related exchanges were associated with various aspects of mental health, although negative humor-related exchanges appeared to be a stronger predictor of mental health than positive humor-related exchanges. Both positive and negative humor-related exchanges also served as mediators between resident and facility characteristics and indicators of mental health. Cultivating a better understanding of the relationships between humor-related social exchanges and mental health may be beneficial for researchers interested in the way humor impacts older adults' ability to cope with stress. This research may also be of value to long-term care providers who create interventions designed at improving residents' mental health and overall quality of life.

This thesis aims to describe health inequalities among older people in Britain in the 1980s and 1990s and to assess whether various personal circumstances and experiences contribute towards this variation. Three sources of data are used: the Longitudinal Study; the first Whitehall cohort of male civil servants; and baseline quality of life information from the MRC Trial of the Assessment and Management of Older People in the Community (MRC Study). Housing tenure, car availability, and employment grade are the main socioeconomic measures used, but also social class and income. Findings: People disadvantaged in mid-life socioeconomic circumstances continue to experience increased risks of mortality, insitutionalisation, poor self-reported health and functioning 20-30 years later. Smoking and cardio-respiratory factors in middle age partially accounted for the differentials found in the Whitehall Study. The MRC Study revealed worse prospects for five dimensions of health-related quality of life among people in rented homes compared to owner-occupied ones, even among those who were deemed independent. Symptoms of ill health, and health behaviours accounted for over 40% of the housing tenure differentials in quality of life among these independent people. Being in a deprived or densely-populated area was not as strong a discriminator of quality of life as personal housing-tenure. Finally, people whose socioeconomic circumstances become worse in late middle age have greater risks of poor health outcomes than those who stay advantaged. The findings on benefits of improvements in socioeconomic circumstances are more mixed and complicated by ill health leading to apparent upward socioeconomic mobility. Conclusions: The three studies provide evidence of both long-term implications of socioeconomic position in mid-life and continuing relevance of socioeconomic position in old age. Although personal factors and health symptoms contribute to health inequalities in old age they are also seen as a possible product of socioeconomic position.

The care of older people in the hospital setting has been the subject of increasing scrutiny from policymakers, researchers, philosophers and age campaigners over the last twenty years. However, in spite of continued attention in organisational training, dignity campaigns and government policy, reports of undignified care continue to surface through notable incidents such as the Mid Staffordshire Public Inquiry. This study examined the role that the cultural representation of older people may have on the attitudes of NHS staff towards them, hypothesising that this influence can be as meaningful as that gathered in the work or social settings. The research adopted a mixed methods approach incorporating a national survey of NHS staff cultural consumption habits, multi-modal discourse analysis of cultural texts typically consumed by that population, and semi-structured interviews with NHS staff. The findings show that older people are generally underrepresented from large swathes of culture, and when represented there is a tendency towards either negative or positive stereotypes. Market trends and operational processes within the cultural industries help to explain the variation of representation on offer, with audience segmentation being a key factor in cases where older people were absent or negatively stereotyped. These representations were found to inform the interviewees’ perceptions of the older population, although this was also mediated by experiences gathered on the job, in training or through social engagement. While efforts are ongoing to foster intergenerational understanding in NHS organisational culture and local communities, the unique position of the cultural industries makes similar interventions in this field more challenging.

Australia and many other developed communities are ageing rapidly, placing a strain on the delivery of health services. This thesis examines the use of innovative health services management coupled with information and communication technology (ICT) to more efficiently deliver services to disabled older people in the hospital, community and residential care. The hypothesis explored is that ICT can provide clinical services to older people in poorly serviced communities and groups, thus extending the influence and capabilities of specialist health care professionals. The relevance of these studies is predominantly for those people who live outside the metropolitan regions, particularly in remote and rural communities, and also for those frail older people, who because of disability, are unable to travel to specialist health services. There are a series of studies presented in this thesis which have all been published. They have demonstrated that in a community and rural setting, ICT use in the assessment and management of geriatric syndromes such as dementia is valid and practical. This included the validation of commonly used assessment tools via Telehealth. A Telehealth protocol for assessment of Alzheimer's Dementia (AD) was developed and published. The use of ICT to link health services clinical and administrative data for determining stroke outcomes and disability has been evaluated and a resource utilization prediction model developed. Finally, in residential care a survey and a qualitative study of poor uptake of ICT services in hostels and nursing homes revealed insights into ICT perception by the older people in care facilities and their professional staff. The implications and future development of these studies have been discussed, especially barriers to increased uptake of ICT, cost comparisons and the potential of future technologies such as video conferencing mobile phones.
