Streszczenie:
The ability to produce and supply more food that is both nutritious and environmentally sustainable is a momentous challenge facing Africa. Where climate change is expected to negatively impact the agricultural resource of many parts of Southern Africa specifically. Climate-Smart Agriculture (CSA) has emerged as an approach considered capable of transforming and realigning agricultural systems to support food and nutritional security, and development under a changing climate. For sustainable food and nutrition security to be achieved, an effective policy environment is required that supports the widespread adoption of CSA application. In light of this context, this study aims to better understand nutrition’s current position within CSA-related policy at the national level by systematically reviewing all agriculture-related policy documents across Malawi, Tanzania, and Zambia, published between 2010 and 2019. The main findings show that efforts to address nutrition are being made within all countries and a sizeable number of policies, with crop-diversification and intensification presented as popular practices promoted as part of CSA. Nonetheless, the widespread adoption of these efforts remains weak and policies lack detail and instruction for the delivery of nutritional security. Cross-ministerial collaboration is recognised as essential for an improved policy environment, but few provide plans to strengthen such linkages or to include nutritional strategies. Clearer actions and policy outlines that promote nutrition as part of CSA are necessary if more effective action is to be achieved.

AbstractThe present study describes the main actions in Finnish nutrition policy during the past decades. The main actor is the National Nutrition Council, which provides nutritional recommendations and action programmes, and sets up expert groups to solve nutritional problems in the population. The main fortification programmes have been the iodization of table salt, supplementation of selenium to fertilizers and the vitamin D fortification programme. As an example of national legislation, labelling the salt content of foods is described. Finnish nutrition policy is based on a good monitoring system of nutrition and risk factors of chronic diseases, as well as active epidemiological research. However, the authorities have not often taken proposed fiscal measures seriously but have instead considered agricultural and economic policies more important than health policy.

Streszczenie:
Asia Pacific has undergone substantial economic growth and trade integration, yet food and nutrition security challenges persist. Different forms of malnutrition, including undernutrition, micronutrient deficiencies, and a rising prevalence of obesity and diet-related diseases, impose significant human and economic costs. This paper examines the opportunities to more strategically leverage regional agricultural trade policy to improve access to diverse, nutritious, and safe foods. Amid urbanization and dietary changes associated with income growth, demand for imported foods is rising across Asia. Meanwhile, pockets of food insecurity and nutritional inadequacy remain, especially among marginalized groups. Trade has potential to enhance year-round availability and affordability of fruits, vegetables, livestock products, and processed fortified foods to support nutritional diversity. However, proactive policy efforts are essential to build nutrition sensitivity. This analysis indicates that expanding regional trade networks, reducing transaction costs, upgrading value chains, utilizing food fortification, leveraging foreign investment strategically, and incorporating nutritional objectives within policy frameworks could significantly improve availability, affordability, diversity and safety of diets. But complementary investments in health systems, infrastructure, social protection, and women’s empowerment are equally critical given the multifaceted determinants of malnutrition. The present findings point to high potential nutrition and food security synergies from closer coordination between trade, health, agriculture and development policy realms. Mainstreaming nutrition across initiatives, assessing dietary impacts, and aligning incentives for food companies with public health goals are identified as priorities alongside value chain development for nutritious commodities. Further research should quantify expected nutrition outcomes of specific trade reforms. Overall, integrating nutrition sensitivity into Asia Pacific’s dynamic agricultural trade growth represents a promising opportunity to enable healthier, sustainable food systems across this populous region.

Background. School policies limiting the availability of sweetened beverages are often considered to be effective interventions for improving children’s diet and weight-related health. This study was designed to examine the effectiveness of the Rhode Island Healthier Beverage Policy in reducing consumption of unhealthy beverages and in producing changes in children’s weight status. Method. Students in 2 public middle schools in Rhode Island completed self-reported measures of dietary intake and were measured for height and weight prior to and 1 year following the implementation of a state-mandated healthier beverage policy. An inventory of beverages available in vending machines after the beverage policy was implemented provided a measure of adherence with the statewide policy. Results. Both surveyed schools demonstrated compliance with the beverage policy (ie, greater than 70% of available beverages complied). Self-reported consumption of sweetened beverages did not change significantly following policy implementation. Neither average BMI percentile for age and gender nor frequency of children in each weight category changed significantly 1 year after the policy was implemented. Conclusions. Although the healthier beverage policy was effectively implemented, it did not result in changes in self-reported sweetened beverage consumption or weight status 1 year later. Additional school policy and individual-level changes appear to be necessary to effect change in weight and dietary outcomes for children.

AbstractObjectiveThis paper examines socioeconomic, demographic, epidemiological and nutrition changes that have occurred in Chile in the last decades using concepts of epidemiological and nutrition transition, and discusses policies related to nutrition.Design and settingThis is a descriptive, population-based study to analyse changes in the Chilean diet and nutrition situation including some of the main demographic, socioeconomic and epidemiological variables. Data came from the FAO, the National Institute of Statistics, the Ministry of Health and national surveys. Results and policy implicationsIn Chile, the epidemiological and nutrition situation shifted from a pretransition stage with high rates of undernutrition to a post-transition stage with increasing rates of obesity in all groups aged less than 20 years. However, changes were not accompanied by modifications in nutrition policy, which had been successful in reducing undernutrition. Despite changes in diet to a ‘western’ diet and in nutritional status of the population from undernutrition to obesity, food and nutrition programmes have been maintained unaltered. Governmental and university organizations were created in 1994 and 1995 to address the current food and nutrition problems. The accomplishments of these institutions have been the elaboration of dietary guidelines, reformulating the food and nutrition programmes and the promulgation of the Food Sanitary Regulations for Chile.ConclusionsEducation for the prevention of nutrition excess problems should be a main food and nutrition policy in developing countries.

Streszczenie:
Ideas concerning relationships between diet and health in the UK are traced from the 1904 Comittee on Physical Deterioration to the outbreak of World War II. Archive material is used to describe the often conflicting views of the Medical Research Council and the Ministry of Health and Board of Education concerning the public health applications of nutrition science. In particular, the work of the Ministry of Health's first Advisory Comittee on Nutrition, which was appointed in 1931, is reviewed and evaluated. The debate among public healh practitioners over the nature, cause and extent of the 'nutrition problem' is documented and the role in this debate of official dietary guidelines which appeared during the 1930s, is assessed. The Impact of the Newer Knowledge of Nutrition on welfare feeding policy Is evaluated in the context of the official promotion of milk feeding in schools. In particular, Corry Mann's experimental evidence which was used to endorse this policy, is reconsidered, and it is shown that the NRC view that the trial was proof of the presence in milk of a "growth factor" which produced preferential growth efficiency in adequately fed children , was in error. From a re-evaluation of the evidence it is suggested that the experiment merely recorded catch-up growth in a group of poorly nourished children. The view that there existed an extensive nutritional problem due to poor quality diets is examined and challenged. Both dietary survey data and anthropometric evidence are used to present the case that there was throughout the period studied a widespread problem of underfeeding among the poor and that intervention strategies based on the Newer Knowledge were not an appropriate method of dealing with this problem. This casts doubts on the widely held view that there was a need for nutrition education and suggests that the problem was one of poverty rather than Ignorance. Disaggregated anthropometric data located by the author are analysed according to NCHS standards to assess the prevalence of underfeeding. Significantly higher prevalences of stunting than low weight-for-age exist in all data sets; this phenomenon is considered in detail and low weight-for-age is proposed as the preferred index of malnutrition in 20th Century historical studies. Attention is drawn to the relevance of these studies for the current nutrition and public health debate.

Streszczenie:
Higher food prices, dwindling purchasing power and socio-economic inequalities are crucial factors promoting under-nutrition in Nigeria. Previous efforts to enhance food consumption and nutrition achieved limited successes as most of the interventions focused largely on food supply with little detailed appraisal of drivers of food demand, especially at the household level. Consequently, this study examines the structure of food demand among households with emphasis on price and income as key variables on which interventions can be built to boost food consumption and nutrition. The study utilises the Nigeria Living Standard Survey and the food price data for 2003/2004 from the National Bureau of Statistics, Nigeria for analyses while employing the double-hurdle model, nutrient deficiency index and a pro-undernourished policy index as analytical tools. A Bayesian (Gibbs sampler) approach is used to estimate the almost ideal demand system (AIDS) model within the framework of the multivariate double-hurdle model. The ensuing demand elasticities indicate food demand patterns characterised by substitutability and complementarity relationships among food subgroups. Approximately 3.99 million people in Nigeria could have been pushed into hunger and calorific under-nutrition as a result of the recent global food price crises. Higher per capita food consumption and accelerated attainment of the Millennium Development Goals on nutrition can be achieved in Nigeria if future economic growth is accompanied by pro-poor income redistribution strategies. A universal transfer strategy or a targeting mechanism with broader scope than the child targeting mechanism being used under the existing conditional cash transfer scheme might be more efficient in reducing under-nutrition among poor households in Nigeria. Although the results suggest that a food stamp scheme would be more cost-effective in raising food consumption and nutrition among the neediest household groups in the country than a cash transfer intervention, its introduction should be considered with some circumspection.

Streszczenie:
In Ethiopia, 17% of pregnant women ages 18-49 are malnourished and have low awareness of prenatal nutrition, which may relate to increasingly high rates of maternal and infant mortality. The purpose of this mixed methods research study was to determine the effects of a community-based prenatal nutrition education intervention program on maternal nutrition knowledge and attitudes in the Alaje district of Ethiopia. The theoretical framework was Sen's capability theory of poverty, in which opportunities can lead to well-being and promote economic development. Research questions focused on the relationships among 8 independent variables-age, income source, degree of program implementation, marital status, education, number of pregnancies, number of children, and occupation-with respect to maternal nutrition knowledge and attitudes. Health workers recruited 135 pregnant and non-pregnant women in each of 2 villages: Dejen (control village) and Takha (experimental village), totaling 270 participants. The community intervention program was an add-on to the Ethiopian government's nutrition program and provided information on portion sizes, the importance of eating an extra meal each day, and obtaining adequate rest during pregnancy. Data from customized pretest and posttest focus groups and surveys were collected. Focus groups were analyzed manually and surveys were analyzed using 1-way ANOVAs and descriptive statistical analyses. The key findings were that the women in Takha had significantly greater knowledge of the importance of prenatal health requirements. The implications for positive social change include recommendations for policy makers about proper dietary practices in order to improve pregnancy outcomes related to maternal malnutrition.

Streszczenie:
Food insecurity (not having continuing access to nutritious food to maintain health) is common in the United States, especially in working poor households. The Supplemental Nutrition Assistance Program (SNAP) is a well-documented safety net for individuals and families struggling with food insecurity. Little is known about the effect of SNAP policy on food insecurity in working poor military households. The purpose of this qualitative phenomenological study was to investigate the lived experiences of food-insecure Army families and the perceived influence of SNAP policy on their food-insecurity. The theoretical framework was policy feedback theory. Data were collected from semistructured interviews with 13 Army heads of households. Data were coded and categorized to identify 3 themes: impact of Army culture, federal programs as stabilization, and limiting SNAP policy. Participants struggle with food insecurity due to unique aspects of military culture, such as transition, and the limitations of current SNAP policy. Findings may be used to inform policymakers of the influence of SNAP policy on food insecurity in the U.S. Army.

Streszczenie:
After the introduction of the Ontario School Food and Beverage Policy (PPM150), Healthy Schools 2020 was initiated to facilitate the policy’s implementation in the Champlain region. Few studies have evaluated teachers’ awareness of school nutrition policies. The purpose of this study was to explore teachers’ awareness of the current activities in their school nutrition environment. A cross-sectional online survey was sent to elementary school teachers in the Ottawa and Renfrew school boards. Descriptive statistics were used to calculate the frequency of nutrition-related activities at schools. Pearson chi-square was used to test for associations between awareness of policies and school food practices, and the presence of nutrition committees and changes to catered lunches and fundraising activities. A total of 243 elementary school teachers completed the survey. Most teachers (83%) were aware of the PPM150 and 25% were aware of the Healthy Schools 2020 initiative. Those who were aware of the PPM150 were more likely to attend school meetings, work at a school where the policy was posted, learned about the policy through the school website, and heard about the policy during professional development days (p<0.05). There was no difference between being aware of the PPM150 or Healthy Schools 2020 with the use of food as a reward. Furthermore, the presence of a school nutrition committee was not associated with any changes to catered lunches and was significantly associated with using food for fundraising (p<0.001). The results indicate that awareness of the PPM150 and the presence of a school nutrition committee did not result in changes in the school nutrition environment. There is a need for more communication about nutrition policies to school members, collaboration between partnerships and services, and training for teachers to improve school-based programs in the Champlain region.

The foundations of the state policy in the field of healthy nutrition and the policy of producers of healthy food products are the preservation and strengthening of public health, prevention of diseases associated with malnutrition of children and adults. The creation of safe and high-quality food products that improve the nutritional status of the population is an urgent task of the food industry. One of the key directions of its solution is connected with the development and introduction of various types of functional food products into the nutrition structure of the Russian population. The priority direction in the production of ice cream is the production of functional products, using food components and biologically active additives, not only contributing to an increase in the nutritional value of products, but also allowing them to give it the desired therapeutic and preventive properties.

Proper nutrition during pregnancy, lactation, and early childhood helps to prevent micronutrient deficiencies for mothers and children. Improved maternal nutrition during pregnancy has been associated with improved birth outcomes and minimized risk of pregnancy-related complications. Poor nutrition during pregnancy is associated with increased risk of preeclampsia, anemia, hemorrhage, and death in mothers. In addition, poor nutrition status of mothers leads to stillbirth, wasting, low birth weight, and delayed developmental milestones in children. Mothers are the source of many generations. Thus, their nutrition status predominantly influences the nutrition status of the forthcoming generations. Given the sensitivity of maternal, infant, and young children nutrition services, the government of Kenya, in partnership with international agencies such as UNICEF and WHO, has developed policies and guidelines to ensure the well-being of this key population. These guidelines and policies address malnutrition, exclusive breastfeeding, complementary feeding, health education and counseling, access to healthcare, food security, and policy coordination and implementation. Although Kenya has come a long way in implementing these policies, there appear to be several gaps in the promotion and application of these policies. Kenya is still struggling with potential gaps in maternal and infant nutrition policies in Kenya including limited coverage of nutrition counseling and support to pregnant women, inadequate focus on preconception nutrition, inadequate support for exclusive breastfeeding, poor integration of nutrition into maternal and child health programs, inadequate coverage of complementary feeding programs especially to children above 24 months of age, poor monitoring and evaluation of systems to examine the effectiveness of various maternal, infant and young children practices. Further, the COVID-19 pandemic increased the uncertainties in the application of nutrition guidelines in maternal nutrition. In particular, the impact of the COVID-19 pandemic exacerbated misperceptions in antenatal nutrition practices, breastfeeding practices, and complementary feeding. These misperceptions persist three years after the onset of the pandemic, thus adding to the existing gaps in the country’s maternal, infant, and young child nutrition policies. Within this context, the proposed study will explore the present gaps in Kenya’s maternal, young, and infant child feeding policies post-COVID-19 era and suggest sound recommendations to address these gaps effectively. Key Words: COVID-19 Pandemic, Maternal, Infant, and Young Child Nutrition Guidelines, Gaps in Maternal, Infant, and Young Child Nutrition Policies, Key policy recommendations, Evaluation of maternal and child nutrition programs

The purpose of this article is to examine the implications of population growth for global nutrition, and how nutrition interventions can support universal health coverage (UHC) and the sustainable development goals (SDGs). The article uses a systematic review of literature to analyze the relationships between population dynamics and nutrition outcomes across different regions and contexts. The findings show that population growth poses significant challenges for food systems, health systems and environmental sustainability, and that malnutrition in all its forms undermines human development and well-being. The article also highlights the opportunities and benefits of integrating nutrition into UHC and the SDGs and proposes a set of policy and programmatic actions to achieve nutrition equity. The article contributes to the existing knowledge on population-nutrition linkages and provides valuable insights for policymakers and practitioners working on health and development issues. The article also suggests some practical implications for improving nutrition service delivery, financing, governance and accountability within health systems and beyond.

Streszczenie:
A child’s health and survival are highly dependent on optimal maternal infant and young child nutrition practices. In Kenya, under-nutrition is a leading cause of death of children. For infants and children under the age of two, the consequences of under-nutrition are particularly severe, often irreversible, and far reaching in future. Rickets is a disease associated with bone deformity that is caused by inadequate mineralization in growing bones, mainly associated with deficiency in either vitamin D, calcium or both. If untreated it could lead to lifelong disability. Despite, the declines in the prevalence of nutritional rickets since the discovery of vitamin D and role of ultra violet light in prevention of the disease, the condition is still a concern in many affluent and developing countries. In Kenya, there is scanty data on the predisposing factors associated with the re-emergence of rickets, yet recent research shows increasing numbers of cases of rickets being reported at health facilities. A case control study of the children with rickets (cases) and those without rickets (controls) was conducted in 2019 at Mbagathi Hospital to establish the predisposing factors that influence the occurrence of rickets in children aged 6-59 months Nairobi County, Kenya. The results of the study show that large household size (p=0.04), low birth weight (p=0.000), birth order (p=0.03), use of family planning methods(p=0.000), malnutrition (wasting & underweight) (p=0.000), exclusive breastfeeding (p=0.008), lack of sunbathing (0.000), positioning children under the shade during sunbathing and attending day care (0.037) were significantly associated with the occurrence of rickets. The study recommends that caregivers should adopt sunbathing their children under direct sunlight, that further studies be conducted on the following factors household size, birth weight, birth order and attending day care in relation to occurrence of rickets. It also recommended that the Government of Kenya through the Ministry of Health and relevant bodies should formulate an elaborate policy on rickets, Keywords: optimal maternal infant, under-nutrition, Rickets, vitamin D, calcium.
