The aim of this study is to identify the ways in which nurses can be effective in helping anxious patients to feel calmer or more secure. This subject is important to all nurses who work in close day-to-day contact with people under stress. The study uses the Inferential Model of Communication as its main theoretical foundation, emphasising the value of identifying the intentions of the communicator and the inferences made by the respondent. It establishes a Nursing definition of the verb "to reassure" as "an attempt by nurses to communicate with patients who are anxious, worried or distressed with the intention of inducing them to predict that they are safe or safer than they presently believe or fear". The literature review reveals few research-based studies which explicitly refer to "reassurance", but many experimental studies of interventions designed to calm anxious patients. The inferential model helps to highlight the theoretical inadequacies of interventions based on "information-giving", and demonstrates the importance of the distinction between prediction and control in aversive situations. Grounded Theory methods were used to collect and analyse a total of 351 Critical Incidents reported in writing by 202 nurses, and in tape-recorded interviews by a further fifty-one nurses and fifty-one patients. The incidents were drawn from the experience of nurses and patients in a wide variety of clinical settings, including general hospital, community, psychiatric and mental handicap settings. A set of descriptive categories was developed from this database to code all the incidents collected. The classification scheme was tested for inter-rater coding reliability, yielding agreement levels of ninety per cent or higher in most categories. The results show that the nurses used five helping strategies - prediction, support, patient control, distraction and direct action. Of these, only the first two are always forms of "reassurance" as defined above. It appears that rational choice of a helping strategy requires nurses to compare their views of the aversiveness of patients' situations with the views of the patients themselves. Out of this comparative assessment, the study suggests that it is possible to predict which helping strategies are most likely to be effective in inducing patients to feel calmer, and which ones may have undesirable side-effects. The study concludes by offering some suggestions for further research, arguing that the inferential model of communication has demonstrated its potential as a powerful tool for the analysis of nurse-patient communication.

The thesis explores the focus of district nursing practice in order to identify areas of study for inclusion in the district nurse curriculum. The "focus" is defined as the point of greatest activity (Chambers, 1991) and so evolves from nursing actions. It is assumed that some nursing actions are dependent upon the patients'needs/problems identified in the assessment process. As all district nurse students are Registered General Nurses they are familiar with nursing needs in hospital and the knowledge that underpins them. The problems experienced frequently by patients at home were explored and compared with those that occur frequently in hospital. The differences in the problems between the two settings enabled knowledge specific to district nursing to be identified. Data was collected from hospitals and the community in one outer London Health Authority. The research was structured through Facet theory which allowed the focus and parameters of the study to be made explicit. The data was analysed using a multiple sorting task, multidimensional scaling procedures and statistical tests. The results demonstrated that a relationship exists between some problems and illustrated those that are likely to occur together. A difference was found between some common problems experienced by patients at home from those experienced in hospital. However, other problems occur with equal frequency and severity in both settings. This suggests that the focus of care is different at home from hospital but that a family resemblance exists. It was argued that the focus of district nursing is on health teaching, which is facilitated by the giving of physical care. Concepts fundamental to district nursing, which underpin the focus of care at home, were identified. Areas of study for inclusion in the district nurse curriculum were suggested and questions for further research raised.

The current rationalisation of health care in the Western Cape may result in a decrease in the number of patients attending the Trauma Unit at Groote Schuur Hospital (GSH), one of the two large tertiary care hospitals in the Western Cape. This in turn may result in cuts in staff allocations to this unit. The nursing staff need to be proactive in preventing potential cuts which may compromise the services that they offer. Current statistics collected by nursing managers in the trauma unit at GSH provide an indication of the volume of work handled, but do not necessarily capture the intensity of that work. The purpose of this research project will determine the extent to which nursing care required by patients attending the trauma unit at GSH has increased and to establish appropriate staff workload scheduling. The existing classification systems available for assessing patient acuity levels are no longer suitable as they use patient numbers to describe workload. By using a classification system specifically developed for the use by nurse managers in high care units (trauma units), the appropriate staffing norms based on the acuity level of patients can be determined. Finally, this research project will determine a suitable model for measuring the intensity of workload specific to a trauma unit environment for the effective and efficient allocation of staff.

Despite the large scale utilisation of nursing auxiliaries (NAs) within the health service, there is a paucity of research evaluating their effectiveness. This study aimed to compare the contribution to patient care of NAs with that of qualified nurses (QNs) using a qualitative indicator, nurse-patient verbal interaction. Different grades were also compared in terms of activities performed and perceptions of their work environment. The organisation of nursing work also has major implications for the roles of QNs and NAs. The study therefore also sought to evaluate the effect of three organisational modes, primary, team and functional nursing, on the work and work perceptions of both grades. A questionnaire was developed which discriminated between organisational modes. This was used to select three wards from each mode (nine in total) for participation in the study. Within each ward, four QNs and four NAs were chosen randomly for inclusion. Data were collected by direct observation and semi-structured interviews. Each subject also completed a Work Environment Scale. The most important differences were found across organisational mode, with QNs and NAs within modes engaging in similar patterns of work, verbal interactions with patients and regarding their work environment similarly. This suggests a culture exists within each organisational mode which permeates the work of both grades of staff. Primary wards were generally found to differ from team and functional wards, with both QNs and NAs regarding their work more positively and working in more therapeutic ways. The study has important implications for the debate about which grade of staff is most suited to caring for elderly patients. It is argued NAs are capable of providing therapeutic care for elderly patients within a pattern of ward organisation which facilitates sustained nursing staff-patient allocation and appropriate supervision and direction in the form of QNs working with NAs.

Throughout the twentieth century, nursing services gradually moved from being located within the community to being concentrated in institutions, such as hospitals. The aim of this thesis is to identify those nursing services that existed within the Rockhampton region from 1911 to 1957; to document the evolution of the services; and to explore those factors that influenced this evolution. In particular, an emphasis is placed on social and political factors. The nursing services explored in this thesis include private duty nursing, private hospitals, church and charity facilities, public hospitals and public community services. These services represent most nursing opportunities during the first half of the twentieth century. However, this thesis takes a unique position by exploring all services in detail within a limited location. In order to accomplish this, an empirical historical method is utilised, based on a wide range of documentary primary sources drawn from archival collections relating to Rockhampton and the nursing profession. By examining a limited geographical area, this thesis highlights the complexity of nursing in regards to who nursed, how nursing was practiced and what factors influenced nursing. A particular feature that emerges within this thesis is the important role untrained nurses played within nursing services throughout the period under review. This group dominated private duty nursing and lying-hospitals in the Rockhampton region, although were gradually restricted to facilities for the aged and chronically ill. Trained nurses also became more institutionalised throughout the period, gradually losing former levels of autonomy as they gained more controlled working conditions, wages and career structures. Finally, this thesis highlights variations in nursing services between metropolitan and regional areas of Queensland.

This thesis reports a study that aimed to create a better understanding of nursing models. It set out to explore nursing models from the qualified nurse's perspective. Such perspective is of interest to the debate about nursing models which has often been conducted at the rhetorical rather than evidence based level. The methodological approach is that of grounded theory (Glaser and Strauss, 1967) with a three stage interview process as the data collection method. The sample comprised qualified nurses in Scotland who were undertaking educational programmes which included input on nursing models. The central themes from the interview stages were: first interview: operationalising the model; second interview: contextualising the model; and the third interview: nursing models and the reality of practice. These central themes were the foundation for the development of a three model typology distinguishing between: the theoretical model which is the conceptual model of the theorist, is abstract, general and developed through inductive and deductive approaches and presented as a potential picture of nursing; the mental model which is the personal pattern or schema of the individual nurse and represents the way nursing is described by the individual; and the Surrogate model which is a functional representation used by nurses to collect data, communicate and through which the organisation can standardise and audit practice. It is concluded that nursing models should not be seen as pertaining to a single entity but be described in the typologies described above. In this way some of the confusion about the way nursing models have been introduced and taught can be addressed. This is especially viewed in the context of models as forms of truth, external objects, adaptable, tools for use or tools for thought, as having individual or collective value and requiring evaluation from the individual's perspective.

A prática do gerenciamento de enfermagem necessita romper barreiras da influência do método funcional e direcionar-se às práticas contemporâneas do gerenciamento do cuidado de enfermagem, pautado pela ideia da gerência participativa, tendo como centralidade o usuário, criando espaços coletivos, envolvendo todos os agentes do processo de trabalho, com a articulação da gerência ao cuidado de enfermagem, na perspectiva de que, a partir dele, emergem elementos para a organização do trabalho de enfermagem. Entretanto, essa concepção de gerenciamento ainda é pouco difundida e sua prática não está consolidada. Esta pesquisa teve como objetivo caracterizar, segundo a ótica de enfermeiros que atuam na enfermagem brasileira na área de gerência, o gerenciamento de enfermagem hospitalar centrado no usuário, bem como descrever sua concepção, identificar as prioridades e sistematizar tendências futuras. Metodologicamente, foi realizado estudo exploratório quantitativo, de mensuração de opinião, utilizando a técnica Delphi, com participação de enfermeiros experts em gerenciamento de enfermagem hospitalar, atuantes em hospitais e em docência, em três rodadas de opiniões para estabelecimento de consenso sobre o assunto. Os dados foram apresentados segundo estatística descritiva e analisados a partir do referencial teórico sobre o tema. O projeto foi aprovado pelo Comitê de Ética em Pesquisa. Os resultados evidenciaram que o cenário atual no qual, o gerenciamento de enfermagem está imerso, é influenciado pelo método funcional, sendo explícita a necessidade de mudança de paradigma, sem, contudo, desconsiderar seus aspectos positivos, sendo o gerenciamento do cuidado de enfermagem tendência potencial, por ter o usuário como foco das ações e por considerar a lógica das instituições, dos profissionais e usuário. Na atualidade, já é possível identificar o enfoque do gerenciamento do cuidado, porém, sem a concretização na prática que se encontra distanciada dos princípios que a norteiam. Identificou-se que há lacuna entre o panorama atual e a perspectiva futura, na qual aspectos que ainda não fazem parte da realidade atual têm perspectiva de concretização. A concepção versou sobre a articulação da gerência no cuidado de enfermagem, sendo que, na ocorrência da cisão entre essas dimensões, são geradas deficiências na assistência. Estabeleceu-se consenso quanto à necessidade de priorizar o cuidado como núcleo do processo de trabalho e as ações gerenciais, tendo como finalidade a assistência. Para a implementação do gerenciamento de enfermagem com foco no usuário, evidenciou-se a necessidade de desenvolver competências pautadas no trabalho coletivo. Este estudo possibilitou identificar conhecimentos referentes ao gerenciamento de enfermagem, na perspectiva da atualidade e do futuro. Entende-se que o gerenciamento de enfermagem, com foco no usuário, articula gerência e assistência, sofre influência e influencia as especificidades das diferentes clínicas. Identificar e discutir os pontos fracos e fortes, os fatores facilitadores e dificultadores, bem como as características essenciais para a prática do gerenciamento, favorece a reflexão sobre a estruturação dos processos e o direcionamento das ações para que, de fato, as mudanças necessárias comecem a ocorrer. Trata-se de alternativa contemporânea e inovadora para modificar moldes de gerenciamento ainda praticados que não têm obtido êxito ao contemplar a integralidade da assistência ao usuário.
The nursing management practice needs to break barriers of the influence of the functional method and direct itself to the contemporary practices of nursing care management, guided by the idea of participative management, focused on users, creating collective spaces, involving all stakeholders of the work process, jointly with nursing care managers, in the perspective that elements for the organization of nursing work emerge from it. However, this conception of management is still not widespread and its practice is not consolidated. This study aimed to characterize, according to the viewpoint of nurses working in management of Brazilian nursing, the management of user-centered hospital nursing, and to describe its design, identify the priorities and systematize future trends. It is an exploratory and quantitative study, with measurement of opinion, using the Delphi technique, and with the participation of nurses expert in hospital nursing management, who works in hospitals and in teaching, in three rounds of opinions to establish consensus on the subject. Data were presented according to descriptive statistics and analyzed from the theoretical framework on the subject. The project was approved by the Research Ethics Board. Results showed that the current scenario, in which nursing management is inserted, is influenced by the functional method, and there is explicit need for a paradigm shift, without, however, ignoring its positive aspects. Nursing care management is a potential trend, as it focuses its actions on users and considers the logic of the institutions, professionals and users. Currently, it is possible to identify the focus of care management, however, this is not found in practice, which is detached from its guiding principles. Gaps were identified between the current situation and future perspective, in which aspects that are not yet part of the current reality have a prospect of accomplishment. The design addressed the joint management in nursing care, and, in the case of splitting between these dimensions, deficiencies in care occur. Consensus was established regarding the need to prioritize care as the core of the work process and management actions, aiming at care. For the implementation of nursing management focused on users, there is need to develop skills based in collective work. This study identified knowledge related to nursing management, in the perspective of present and future. It is understood that nursing management, focused on users, articulates management and care, is influenced and influences the specificities of the different areas. Identifying and discussing the strengths, weaknesses, facilitating factors and difficulties, as well as the essential characteristics of the management practice, encourages reflection on the structuring of processes and guides actions so that, in fact, the needed changes start to happen. It is a contemporary and innovative alternative to modify management molds still practiced and that have not been successful in providing comprehensive care to users.

The United States has experienced historically low graduation rates in public and private 2-year, degree-granting institutions. Many of these institutions are community colleges, which account for 60% of all student enrollment. This study was conducted to explore supports and services that may be helpful to working adult students over the age of 25 enrolled in a 2-year associate degree nursing program in a community college. Tinto's interactionalist theory of student persistence and retention and constructivist theory were the conceptual frameworks for this qualitative case study. The two guiding questions were focused on the types of support that would be helpful for degree completion and service improvements that would most effectively assist students to graduate. Data were collected using semistructured interviews and observations with 10 participants who volunteered from a bound system. Requirement for participation included being over the age of 25 and enrolled in the 2-year associate degree nursing program. Data were analyzed using a phenomenological reduction process and cross-sectional analysis to identify convergent and divergent themes in the data. The findings of this study highlight 5 overarching themes as described by the participants: support system, barriers to education, effect of work, engagement in school services, and recommendations for college improvement. The findings of this study could be helpful to administrators and policy makers in developing supports and services that promote retention and degree completion of students in the 2-year associate degree nursing programs. Completion of a 2-year associate-degree nursing program promotes financial viability and meets the workforce needs of the community.

This study is about the progress of a cohort of students who commenced a Project 2000 pre-registration nurse education course in April 1991. The research was undertaken entirely within one setting - in a College of Healthcare which was one of the 13 Demonstration colleges in England - and was conducted over a period of three years and ten months. The aim of the study was to evaluate what was considered by the nursing profession, government officials and educationalists, to be a radically new and innovative nursing course. A case study research approach of illuminative evaluation was used, within which a three stage framework of observation, further enquiry and evaluation was followed. The emphasis of the research was on the illumination of the processes, perceptions and contingencies which resulted in particular outcomes for the participants. This has enabled a comprehensive understanding of the complex realities which surrounded this innovative course, and the effects that a number of contemporaneous events have had on the findings. A description of these events has been presented, as has a literature review, overviews of both the history which preceded the introduction of Project 2000 and the professional concerns regarding the rapidity with which it was implemented. Multiple data collection methods were used to obtain the required information from the April 1991 cohort, the teachers involved with these students, the practitioners from three health authorities and other members of the College staff. The analyses of the data and the progressive focusing on the findings, have resulted in the identification of numerous positive and negative aspects and outcomes, together with problem areas and particular issues associated with the rapidity of the implementation of the course and the management of change. The placing of the negative and positive findings in a broader explanatory context, demonstrates how they relate to each other and with the research literature, and how the resulting conclusions have been reached. The research demonstrated that there were some very positive aspects and desirable outcomes which arose from the P.2000 programme. However, the concerns are the detrimental effects which arose from the introduction of numerous innovations into a constantly changing environment. The nursing profession had placed great importance on the introduction of P.2000, but insufficient time had been allowed for the application of management of change theories which would have enabled a more successful implementation to take place.

This thesis provides the rationale for the necessity of a redefining of the holistic back into nursing after the consequences of technological restructuring. This study revealed that the impact of modern technology-based, prescriptive changes on professional nursing practice in Canada has resulted in an increasing alienation of labour for nurses, including direct interference with patient-based nursing care, authority, necessity for broader knowledge systems, stability and fragility in job security. The implications of this examination have demonstrated that this shift has not been the result of mechanical technologies alone, but the science-based management philosophies and communicative nature of technologies as well.
This project has verified that more importantly than the advancing technological shift itself the danger for professional nursing has been in what these systems are actively replacing. Namely, prescriptive technologies work to establish a managerial or 'expert' presence and authority within the practice of nursing serving to change professional understandings for nurses as well as to decrease value in the judgement and holistic care skills of registered nurses.

This thesis is based on a journey towards developing team working within a primary healthcare setting which enabled the NHS agenda for primary care service delivery to keep pace with the government modernisation agenda. (Department of Health, 1997). Initially the focus was on the development of an integrated nursing team which enabled all disciplines of nurses to work towards a patient focussed healthcare service, but it soon became evident that all staff involved in delivering the primary healthcare service were essential to the process and developments of the enquiry if the objective was to be achieved. An action enquiry approach based on collaborative and participative action research (Carr and Kemmis, 1986; Lincoln and Guba, 1989; Cayer, 1997) was discussed and was the prime method of enabling changes to occur in the healthcare practice. This is represented by the interrelated four main cycles of enquiry that have emerged from the data, and discussed in this thesis. Key to the success of the developments was an understanding of team working and leadership as it applied within a healthcare setting and also the underlying dynamics, which are evident when different professional groups from different traditions and knowledge base work together. (Schon, 1983) This was explored within the context of a systems approach to organisational development and through reflective dialogue along the principles advocated for creating a learning organisation. (Senge, 1990) This thesis will demonstrate how confidence developed in myself and the practitioners, especially those from marginalised groups, and how the wider healthcare system made an impact on the developments within the practice. The area of leadership will be discussed from multiple perspectives and recognition that as a concept all stakeholders had a poor understanding of leadership. The key finding from this study identifies the need for a holistic approach to manage and sustain change, and indeed everyday productive working relationships. This especially identifies the importance of giving attention to the preparation of future healthcare workers, the appropriateness of organisational structures in which services are delivered and support structures available to those in team leadership positions.

This descriptive correlational study describes the relationships among 30 home care clients' perceptions of nursing invasiveness, territorial control, and satisfaction with nursing care. The self-report data were collected using a Nursing Invasiveness Scale (NIS), Index of Patient Territorial Control Perceptions (IPTCP), and Patient Satisfaction Instrument (PSI). The subjects were males and females, aged 23 to 93 years, who were receiving home care nursing. Analysis of the data suggests that the subjects perceived a low level of invasiveness by home care nurses, "much control" within their homes (their primary territories), and were highly satisfied with their nursing care. There was a significant negative correlation (r = -0.79) between perceptions of nursing invasiveness and satisfaction with nursing care. The relationships between perceptions of nursing invasiveness and territorial control (r = -0.02) and between perceived territorial control and satisfaction with nursing care (r = 0.14) were not significant.

Graduate nurses' transition from school to the work place is challenging and often leads to burnout. There was no graduate nurse residency program in women's services at the practicum facility. As a result, this facility had been unable to recruit or hire graduate nurses in the women's services unit. The purpose of this project was to develop a nurse residency program in women's services to address the lack of formal orientation for new graduate nurses at this facility. A graduate nurse residency program will provide further training for nurses to care for a more complicated population of pregnant women. Theoretical support for this project was Duchscher's, theory of transition, which suggests that allowing graduates time to adjust within a context of support allows them to develop their thinking and practice and helps them move through the stages of professional role transition. The project included a review of literature, development of a nurse residency plan, all materials needed to operationalize the program in the institution, and plans for implementing and evaluating the program over time within the context of institutional challenges, goals, and strengths. Collaboration with institutional stakeholders helped to ensure the contextual relevance of the program and ongoing administrative ownership to provide momentum for the program to move forward following delivery of the products of the DNP project to the institution. In sum, the products of this project comprise a turn-key solution to the institutional need for a graduate nurse residency program in women's services. Social change implications include possible improvement in the recruitment and retention of graduate nurses as well as the consistent development of competent and safe practitioners who will improve maternal and newborn outcomes at the facility.
