Deep space infections occur around the airway, including the epiglottis, the parotid gland, and the retropharyngeal spaces (retropharyngeal abscesses [RPA]) and parapharyngeal spaces. These infections can extend into the airway and mediastinum, and their extent can be difficult to appreciate without imaging. In adults, deep space infections most commonly result from trauma, irradiation, surgical procedures, and human or animal bites. In children, they more commonly result from cervical adenitis and thyroiditis caused by bacteria or viruses. RPA commonly presents with sore throat, fever, torticollis, dysphagia, neck pain, muffled “hot potato” voice, cervical lymphadenopathy, and respiratory distress. Epiglottitis symptoms classically include the triad of drooling, dysphagia, and distress but may include inspiratory stridor. Parotitis is recognized by a sudden firm, erythematous swelling of the preauricular and postauricular areas. Immediate airway management and otolaryngology consultation are required. Most patients will require transoral or transcervical incision and drainage in addition to IV fluid resuscitation and close observation.

This chapter extends the transparent epistemology of belief and knowledge given in Chapter 5, first to perception and then to sensation. It turns out that these two topics are very closely related. According to a widespread view, to have a bodily sensation—a headache, a tickle in one’s throat, an itch on one’s neck—is to perceive something occurring in one’s body. The chapter argues that this perceptual theory of sensation is correct. The epistemology of one’s bodily sensations thus becomes a special case of the epistemology of one’s perceptions, and the basic transparency account for perception applies. As one might expect, there are some complications.

Upper airway obstruction (UAO) from any cause should be considered a life-threatening emergency. In a conscious patient, UAO may present as respiratory distress, stridor, dyspnoea, altered voice, cyanosis, cough, decreased or absent breath sounds, wheezing, the hand-to-the-throat choking sign in the case of a foreign body, facial swelling, and distended neck veins. The cause of UAO should be identified and airway management devices must be immediately available prior to any airway manipulation CT scan, flexible bronchoscopy, and pulmonary function tests should be performed to evaluate the cause and the extent of the obstruction. Obstructive sleep apnoea (OSA) patients are at increased risk of developing UAO. Endotracheal intubation, insertion of a supraglottic device, laser therapy, and endotracheal stents maybe life-saving

‘Upon her neck and breast was blood, and upon her throat were the marks of teeth having opened the vein: – to this the men pointed, crying, simultaneously struck with horror, “a Vampyre, a Vampyre!”’ John Polidori’s classic tale of the vampyre was a product of the same ghost-story competition that produced Mary Shelley’s Frankenstein. Set in Italy, Greece, and London, Polidori’s tales is a reaction to the dominating presence of his employer Lord Byron, and transformed the figure of the vampire from the bestial ghoul of earlier mythologies into the glamorous aristocrat whose violence and sexual allure make him literally a ‘lady-killer’. Polidori’s tale introduced the vampire into English fiction, and launched a vampire craze that has never subsided. ‘The Vampyre’ was first published in 1819 in the London New Monthly Magazine. The present volume selects thirteen other tales of the macabre first published in the leading London and Dublin magazines between 1819 and 1838, including Edward Bulwer’s chilling account of the doppelganger, Letitia Landon’s elegant reworking of the Gothic romance, William Carleton’s terrifying description of an actual lynching, and James Hogg’s ghoulish exploitation of the cholera epidemic of 1831–2.
