Streszczenie:
The currently relevant value-based healthcare model (Value-Based Healthcare) is more focused on ensuring long-term effectiveness and patient satisfaction, which is directly related to the implementation of the principles of patient-oriented and patient-centered activities [1]. In accordance with the Proposals (practical recommendations) on the organization of internal quality control and safety of medical activities in a medical organization (hospital) of the Federal State Budgetary Institution “National Institute of Quality” of Roszdravnadzor, compliance with the principles of patient-centered medical care provides for the development and implementation at the level of a medical organization of a patient safety strategy, including compliance with ethical standards and rules in the implementation of medical care activities. This also includes the procedure for organizing feedback with patients [15]. The use of information technologies in patient questionnaires and analysis of results allows for operational monitoring of patient satisfaction indicators and to create a basis for assessing most aspects of hospital activities from the perspective of patients: the quality of medical services and their results, interaction with staff, infrastructure and service components. The purpose of the study is to provide the results of the implementation of the author’s information and analytical system for collecting and processing information about patient satisfaction through an electronic web questionnaire and providing the calculation of satisfaction indicators in a multidisciplinary hospital. M a t e r i a l s a n d m e t h o d s . The basis of the study was a multidisciplinary hospital of the state budgetary healthcare institution “Regional Clinical Hospital No. 2” in Vladivostok. Within the framework of the study, the inpatient patients were surveyed using questionnaires in electronic form prepared by a medical organization, an information and analytical system was developed to conduct a mathematical analysis of the results of the survey according to five criteria for assessing the quality of medical care conditions. The study used a questionnaire including 20 questions, the answers to which express the opinion of patients about the quality of work of doctors and nursing staff, the comfort of the conditions of stay in the emergency department, as well as in the hospital during hospitalization. The questionnaire was developed in 2015, and for many years the survey was conducted using paper and manual calculations. But since 2021, patient questionnaires and analysis of the collected data have been transferred to an electronic format. In this article we present the results obtained with the help of an information system for collecting and analyzing questionnaires, in which adult patients of a round-the-clock hospital participated. R e s u l t s . The use of information technologies in patient questionnaires and analysis of results allows for operational monitoring of patient satisfaction indicators and to create a basis for assessing a number of aspects of hospital activity from the perspective of patients: the quality of medical services and their results, interaction with staff, infrastructure and service components make it possible to evaluate the effectiveness of measures to ensure comfortable conditions for the patient’s stay and can be used in practical healthcare to organize measures to eliminate defects and make decisions on the management of a medical organization. C o n c l u s i o n s . The introduction of additional methods for studying patient satisfaction using electronic forms, taking into account the specifics of the work of a medical organization to the requirements established by the Ministry of Health of the Russian Federation, makes it possible to increase the effectiveness of interaction with patients and can be one of the tools for assessing the social effectiveness of a medical organization (hospital).

SUMMARY: Professionals have been working in occupational safety and health (OS&H) in the United States since at least the advent of the Industrial Revolution. Formal educational in OS&H began in earnest with passage of the Occupational Safety and Health Act (OSHA) of 1970 that included provisions for funding training in OS&H disciplines. A study conducted by the National Institute for Occupational Safety and Health (NIOSH) in 2011 estimated that there were over 48,000 (2.3/10,000 working age adults) OS&H professionals working in the US. Employers surveyed in this study reported a need to hire over 25,000 OS&H professionals in the next 5 years. OS&H training programs were projected to graduate less than 13,000 students in that period. Core knowledge and skills in the various OS&H disciplines typically includes academic training in epidemiology, biostatistics, administration & management, behavioral health science, industrial hygiene, ergonomics, and foundation courses in occupational injury and disease prevention. Employers report interest in additional training in communication, technical writing, leadership, cross-disciplinary training, emerging hazards, and knowledge of regulations and compliance. Funding of OS&H programs has been a persistent challenge. A lack of student awareness of training and career opportunities poses additional challenges for recruitment of qualified applicants. While hiring demands and career prospects remain high for most OS&H disciplines, the support for training programs, and the supply of new graduates presents ongoing challenges for OS&H educators.

AbstractBackground:Out-of-hospital cardiac arrest (OHCA) is a life-threatening condition with an overall survival rate that generally does not exceed 10%. Several factors play essential roles in increasing survival among patients experiencing cardiac arrest outside the hospital. Previous studies have reported that implementing a dispatcher-assisted cardiopulmonary resuscitation (DA-CPR) program increases bystander CPR, quality of chest compressions, and patient survival. This study aimed to assess the effectiveness of a DA-CPR program developed by the Thailand National Institute for Emergency Medicine (NIEMS).Methods:This was an experimental study using a manikin model. The participants comprised both health care providers and non-health care providers aged 18 to 60 years. They were randomly assigned to either the DA-CPR group or the uninstructed CPR (U-CPR) group and performed chest compressions on a manikin model for two minutes. The sequentially numbered, opaque, sealed envelope method was used for randomization in blocks of four with a ratio of 1:1.Results:There were 100 participants in this study (49 in the DA-CPR group and 51 in the U-CPR group). Time to initiate chest compressions was statistically significantly longer in the DA-CPR group than in the U-CPR group (85.82 [SD = 32.54] seconds versus 23.94 [SD = 16.70] seconds; P <.001). However, the CPR instruction did not translate into better performance or quality of chest compressions for the overall sample or for health care or non-health care providers. Conclusion:Those in the CPR-trained group applied chest compressions (initiated CPR) more quickly than those who initiated CPR based upon dispatch-based CPR instructions.

B a c k g r o u n d . According to the genre characteristics, the article is a form of publicizing analytical conclusions from the experience of research in the field of the philosophical Chinese studies at the National Academy of Sciences of Ukraine from 1991 to the present day. The material for understanding was supplied from the environment of scientific professional activity of prominent figures of Ukrainian philosophical Sinology from the H. Skovoroda Institute of Philosophy of the NAS of Ukraine and the A. Krymskyi Institute of Oriental Studies of the NAS of Ukraine. The scientific achievements of academic Sinologists, who are leading researchers of Chinese philosophical education, science and culture from the archaic to modern times, are highlighted. They are, namely: V. Kiktenko, V. Pyrohiv, V. Hamianin, S. Kapranov, Y. Hobova and A. Usyk. M e t h o d s . The article uses an analytical approach based on the study of historical and philosophical sources. The methodological basis of the research is the main methods of historical and philosophical science: problem-categorical analysis, socio-cultural analysis and biographical analysis. At the same time, the methods of situational and contextual analysis of scientific texts are applied. R e s u l t s . Four conditional thematic vectors of the progress of academic philosophical Sinology in modern Ukraine are classified, namely the study of: 1. the history of ancient Chinese philosophy, especially Confucianism; 2. the history of Chinese philosophy in the 19th – early 21st centuries; 3) the history of Ukrainian Sinology in the 18th – early 21st centuries; 4. the history of modern foreign Sinology. The director of the A. Krymskyi Institute of Oriental Studies of the NAS of Ukraine and the head of the Ukrainian Association of Sinologists V. Kiktenko very effectively studies all these vectors, especially the last three of them. His colleagues at the institute V. Pyrohiv, V. Hamianin and Y. Hobova mainly research the first of these vectors, and primarily the history of Confucianism. Employees of the H. Skovoroda Institute of Philosophy of the NAS of Ukraine S. Kapranov and A. Usyk are also focused scientific attention on the history of Confucianism, in particular, the second of them studies social ethics and anthropology of Confucius. C o n c l u s i o n s . As a result it was established that modern domestic philosophical academic Sinology is a qualitatively new and innovative direction of Oriental studies in independent Ukraine. This discipline was formed as a result of the two-century progress of Ukrainian Chinese studies from the era of Hetmanshina until now. V. Kiktenko and his colleagues have made a significant and already internationally recognized contribution to the study of both the classical philosophical schools of Ancient China and their ideological heirs, in particular, the leaders of the PRC and the CCP from Deng Xiaoping to Xi Jinping during these thirty years. V. Kiktenko also laid the foundations for the study both of the history of Ukrainian Chinese studies and of modern foreign, primarily Western European and North American, Sinology.

Quantifying citations as a measure of academic achievement has been a disputed tool, not only within the Serbian academic community. Nature Magazine published “The Leiden Manifesto”, advocating harmonization between quantitative and qualitative criteria. As a contribution to such efforts, this paper examines the production of the researchers of the Institute for Contemporary History in Belgrade. The Institute has been chosen as the most productive institution in Serbia in terms of the number of publications. Proportionally to the number of published works in a certain language, the most frequently quoted papers have been written in German, then in French. The reason for that is the particular interest of some big academic communities for certain issues. For example, the German academic community’s curiosity for socialism derives from its interest in the German Democratic Republic, its history and its society. Papers dealing with foreign workers also address some German internal issues. Publications in French have been more often quoted if they addressed World War I topics. And last but not least, works on the Yugoslav wars of the 1990’s reflects the political and academic interest of the international community. Thus, the most quoted works often respond to the requirements of some foreign factors, such as the international community or some big national academic circles (German and French). These papers sometimes fail to address certain local educational and cultural needs. It is important to combine broader regional and international interests with internal cultural needs. Therefore, more papers should be written in foreign languages, especially in English, which is the primary language of international academic exchange.

A specially designed high resolution ICP-MS was combined with an ultrasonic nebulizer to analyze trace amounts of various inorganic contaminants contained in water samples. It was confirmed that the detection limits of most elements obtained by the proposed method were well under 1 ppt. The Standard Reference Material 1643b (trace elements in water) provided by NIST (National Institute of Standards and Technology, formerly U. S. National Bureau of Standards) was also analyzed. Concentration values obtained by this work were in good agreement with certified values for all the elements examined except for Cu. It can be concluded from the foregoing that the combination of the above two instruments materializes an extraordinarily sensitive method and paves the way for direct determination of a considerable number of trace and ultra-trace elements in water samples with acceptable speed and accuracy.

The institute of budgetary discipline breach plays a key function for the return of unjustly used funds to the state budget. There is a difference between the funds that have not been reimbursed from the European Union due to the application of the correction and the funds recovered at the national level, this difference is a burden for the state budget. The 24th International Towing Tank Conference (ITTC) has initiated an inter-laboratory test of a 5.720 m length model that is geometrically similar to David Taylor Model Basin (DTMB) model 5415, a pre-design version of the U. S. Navy surface ship combatant DDG-51. Resistance, sinkage, and trim measurements were completed during the last week of August 2009 on DTMB Carriage #1. Test results include an uncertainty analysis. Since the test program was initiated, the uncertainty analysis procedures were revised by the 25th ITTC for consistency with the ISO Guide to the Expression of Uncertainty in Measurement (ISO GUM 1995). This paper describes the analysis by the ISO GUM. All measurement results are traceable to NIST (National Institute of Standards and Technology). These results are for a test conducted on a single day over a Froude number, Fr, range of 0.10 to 0.41 and 9 different velocities. Only resistance results are reported here. A total of 12 repeat runs were performed at Fr = 0.10, 0.28, and 0.41. The variation in results is quite large in comparison to the uncertainty estimate at a single run condition. The uncertainty from Type A method as computed from the standard deviation of the time series was the largest uncertainty component in resistance from the drag block gages. For example at Fr = 0.41, the residuary resistance coefficient was CR = 0.003721 ±0.000041 (±1.11 %) with about ±0.000031 (±0.83 %) from the block gage measurement. A 4th-order polynomial approximates the experimental data quite well with a correlation of coefficient of 0.999. The fitted value at Fr = 0.41 is CR = 0.00366 ±0.00012 (±3.3 %). The curve fitted value is considered to be the best estimate of the uncertainty when combined with the measured value. For the most part, the resistance coefficients are in agreement with previous results within the uncertainty of the estimates.

Streszczenie:
Less than 15 years ago the National Heart, Lung and Blood Institute surveyed physicians in the United States in order to characterize the demographics of hemophilia. The average age of persons with hemophilia in the United States was found to be 11.5 years old. By 10 years later, the life expectancy was predicted to be normal, and indeed the average age of persons with hemophilia in the U.S. is now in the early twenties. Early, intensive and predictably efficacious control of hemorrhage has made this result possible, and the therapeutic product which has allowed such control is commercial clotting factor concentrate. We now know that starting in 1978, and with great frquency during 1982 and 1983, the majority of U.S. hemophiliacs were infected with human immunodeficiency virus (HIV). It is estimated that as of January, 1987, approximately two thirds of the 20,000' persons with hemophilia in the United States have been infected with HIV. Among those with severe factor VIII deficiency, more than 9056 are seropositive. As of 1/5/87, there were 288 cases of AIDS among U.S. hemophiliacs, for an AIDS rate of approximately 2.256 of those with HIV infection. This number included 185 with severe, 32 with moderate and 28 with mild hemophilia A; 12 with severe, 6 with moderate and 1 with mild hemophilia B; 9 with vWD, and 4 others. A disproportionate number were older patients: 55 were ages 1-19; 62 ages 20-29; 85 ages 30-39, and 86 age 40 or older. Although the AIDS attack rate is no longer climbing logarhythmically, new cases are certainly still occurring.A variety of other HIV-related syndromes have emerged. Of great concern is immune thrombocytopenia, which is now relatively common; among a group of 209 carefully followed HIV-positive patients at our center, 31 (1556) are or have been thrombocytopenic. Progressive failure to normally gain height and weight in children with hemophilia has recently been shown by our group to correlate with HIV antibody positivity, and also with decreased T4/T8 ratio, decreased T4 cell count, decreased skin test reactivity, and subsequent development of ARC or AIDS in some such children. Finally, a picture of progressive fall in T4 count associated with recurrent non-specific infections and increased likelihood of positive viral culture, may predict an increased risk of developing AIDS.We know that the immune dysfunction in hemophilia is complex, and not wholly explained by HIV infection. One important factor may be the many foreign proteins contained in commercial clotting factor concentrates, and their ability to stimulate T cells. It is known that latent HIV infection in cultured T4 lymphocytes can be induced to enter the proliferative, viral secretory phase by the addition of soluble foreign antigens to the cell culture. Recent data of Brettler and colleagues, to be presented at this meeting, suggest that the use of highly purified VI!I:C (specific activity >3000 u/mg) in place of the present extremely impure products, may improve the immune dysfunction in hemophilia. This observation offers a new hypothetical approach to the prevention of progressive T4 cell depletion in HIV infected hemophiliacs, and requires immediate and extensive further study. The psychosocial burden of HIV infection is immense. The need for extensive, formal education and support programs is largely unmet in most parts of the world. Such programs are best run out of hemophilia treatment centers in most cases, and must include an active program on prevention of sexual transmission, provision of HIV testing before and during pregnancies, provision for maintenance of confidentiality, etc. Education concerning HIV is like all other forms of education. It requires formal organization, a curriculum, active rather than passive learning in which there is interaction between the teacher and the pupil, time for planned repetition, reinforcement with written materials, and assessment of goals achieved. For all of these reasons it is inappropriate to assume that the physician at the hemophilia center will be able to provide an adequate education program. Adquate paramedical personnel will need to undertake this effort, under the directjon of the physician.

Currently the majority of care provided to a person living with Alzheimer’s disease or related dementia (ADRD) is from a family caregiver. There are approximately 55 million older adults in the world living with AD [1]. By 2030, there is estimated to be 8.4 million Americans living with Alzheimer’s [2]. Due to impaired memory and cognitive function, persons with ADRD often face pressing challenges such as loneliness, social isolation, anxiety, depression, and stress, which in turn accelerate cognitive and functional decline, increase premature mortality, and significantly reduce quality of life in these persons [3-6]. Our project aims to develop a scalable, personalized, accessible tool, named friendly robot to ease dementia (FRED), to engage with persons with ADRD and alleviate their challenges. Using state-of-the-art artificial intelligence (AI) and robotics techniques, FRED will assist with cognitive enrichment and physical activity to improve activities of daily living and quality of life for persons with ADRD and their care partners.By incorporating human centered design methods, our research engages the caregiving and Alzheimer and dementia community in the design of a non-drug intervention. Our paper will share results from our participatory focus group, which included people with ADRD and their caregivers (n12), in the assistance of the design and interactions of FRED.A thinking-aloud protocol was adopted once we had a prototype of FRED to share, where users were able to express freely any problems and/or concerns during interaction. Insights from the thinking-aloud results were used to improve the user interface design to enable the users and robot to interact and collaborate in an effective, natural way. Additional focus groups with our improved design are forthcoming. From our first stakeholder focus group, we have identified that ADRD patients need structure, organization, and routine. Initial reactions of the social robot were positive and supportive. Santiago de Chile (33°27’S and 70°42’W), capital city of the country, is placed in the central valley. It has a Mediterranean climate with a long dry season (between 7 and 8 months). Its annual average temperature is 12,2°C, whereas the thermal oscillation is considerable: there is almost 13°C between January and July average temperatures (hottest and coldest months, respectively) and the difference between maximum and minimum temperatures ranges between 10°C and 16°C during all the year. According to the National Statistics Institute, 37.4% and 36.0% of new housing during 2006 were built using mainly brick masonry and concrete in their walls, respectively. In both cases, thermal insulation was not generally considered. On the contrary for the heating period, a favorable thermal performance in summer should be expected (low thermal insulation in combination to high thermal mass). However, some recent studies show the completely opposite scenario, since an important percentage of users declare overheating in their own dwellings. This apparent contradiction could be understood from a database limitation, due to these official data do not reflect the impact of the current thermal regulation, which is in force since January 2007. Notwithstanding the required standards are weak in comparison to the international state-of-art (e.g. 1,9 W/m2K as maximum U-value for walls in Santiago), nowadays apartment buildings in Santiago are including at least 20 mm of thermal insulation in their walls to give compliance to the code. This paper proposes a series of dynamic thermal simulations to apartment buildings in Santiago, with the aim of establish the impact of different constructive solutions by means of thermal behavior, both in winter and summer. These digital models are statistically based on the typologies offered in the private real estate market during both periods 2001-2002 and 2006-2007, according to a database from Portalinmobiliario.com. These were determined using a multivariate analysis of their attributes – producing homogeneous market niches - through the hierarchical clustering technique. These homogeneous niches were identified in the real estate private submarkets, assessing different attributes. Thermal simulations were made using the TAS software, a dynamic-state digital tool. According to the results, the implementation of the thermal regulation – intended mainly to reduce heating consumption – have produced unfavorable comfort conditions in all the studied market niches, in comparison with the business as usual scenario. These conclusions allow establishing challenges and opportunities for the private real estate market, in order to integrate new thermal regulations with the private market reality.
