This paper provides an analysis of the national Indigenous reform strategy – known as Closing the Gap – in the context of broader health system reforms underway to assess whether current attempts at addressing Indigenous disadvantage are likely to be successful. Drawing upon economic theory and empirical evidence, the paper analyses key structural features necessary for securing system performance gains capable of reducing health disparities. Conceptual and empirical attention is given to the features of comprehensive primary healthcare, which encompasses the social determinants impacting on Indigenous health. An important structural prerequisite for securing genuine improvements in health outcomes is the unifying of all funding and policy responsibilities for comprehensive primary healthcare for Indigenous Australians within a single jurisdictional framework. This would provide the basis for implementing several key mutually reinforcing components necessary for enhancing primary healthcare system performance. The announcement to introduce a long-term health equality plan in partnership with Aboriginal people represents a promising development and may provide the window of opportunity needed for implementing structural reforms to primary healthcare. What is known about the topic? Notwithstanding the intention of previous policies, considerable health disparity exists between Indigenous and non-Indigenous Australians. Australia has now embarked on its most ambitious national Indigenous health reform strategy, but there has been little academic analysis of whether such reforms are capable of eliminating health disadvantage for Aboriginal people. What does the paper add? This paper provides a critical analysis of Indigenous health reforms to assess whether such policy initiatives are likely to be successful and outlines key structural changes to primary healthcare system arrangements that are necessary to secure genuine system performance gains and improve health outcomes for Indigenous Australians. What are the implications for practitioners? For policymakers, the need to establish genuine partnership and engagement between Aboriginal people and the Australian government in pursuing a national Indigenous reform agenda is of critical importance. The establishment of the National Congress of Australia’s First Peoples provides the opportunity for policymakers to give special status to Indigenous Australians in health policy development and create the institutional breakthrough necessary for effecting primary healthcare system change.

Aboriginal community controlled PHC services have led the way in Australia in developing a model of PHC service that is able to address social issues and the underlying determinants of health alongside high quality medical care. This model is characterised by a comprehensive style rather than the selective PHC model that tends to be more common in mainstream services. Central to comprehensive PHC is community control, which is critical to the bottom up approach rather than the top down approach of selective PHC. The expansion of Aboriginal Community Controlled Health Services (ACCHSs) in Australia is a product of the colonial relationship that persists between Aboriginal and non-Aboriginal Australia. It is this relationship that explains why community control has been a feature of Aboriginal PHC services while similar attempts in the dominant society have tended to be incorporated into the mainstream. The mechanisms of control occur through community processes and should not be confused with day to day management processes, although the two are related. The Core Functions of PHC is a framework that reflects the experience of ACCHSs and allows for the development and assessment of comprehensive PHC. This framework is applied to a case study of the Central Australian Aboriginal Congress (Congress) which is the major Aboriginal health service in central Australia. The case study illustrates increasing utilisation of PHC services by Aboriginal people, and the capacity of community controlled organisations to respond to demographic and health pattern changes in their client populations.

The Strong Born Campaign (2022–2025) was launched by the National Aboriginal Community Controlled Health Organisation (NACCHO) in 2023. Strong Born is the first of its kind national Aboriginal and Torres Strait Islander health promotion campaign to address Fetal Alcohol Spectrum Disorder (FASD) within Australia. Strong Born was developed to address a longstanding, significant gap in health promotion and sector knowledge on FASD, a lifelong disability that can result from alcohol use during pregnancy. Utilizing a strengths-based and culturally sound approach, NACCHO worked closely with the Aboriginal Community Controlled Health Organisations (ACCHOs) to develop the campaign through co-design, as described in this paper. Since its inception, the ACCHOs have continually invested in driving change towards improvements in Aboriginal health determinants and health promotion. The Strong Born Campaign developed culturally safe health promotion tool kits designed for the community and health sector staff and also offered communities the opportunity to apply for FASD Communications and Engagement Grants to engage in local campaign promotion. The tool kits have been disseminated to 92 ACCHOs across Australia. This paper describes the development of the Strong Born Campaign and activities following its launch in February 2023 from an Indigenous context within Australia, as described by NACCHO.

The paper examines the presence of Aboriginal art, its contact with colonial and federation Australian art to prove that silencing of this art from the official identity narrative and art histories also served elimination of Aboriginal people from national and identity discourse. It posits then that the recently observed acceptance and popularity as well as incorporation of Aboriginal art into the national Australian art and art histories of Australian art may be interpreted as a sign of indigenizing state nationalism and multicultural national identity of Australia in compliance with the definition of identity according to Anthony B. Smith.

The last two decades have seen rapid changes in many facets of Aboriginal society, including morbidity and mortality. The same period has witnessed a dramatic increase in writing about and by Aborigines and this has necessitated a re-examination of the national “history” to include the indigenous people of Australia. Medical workers in Aboriginal Australia should be alert to the historical forces determining patterns of ill-health. Psychiatry in particular must develop this perspective if it is to participate with Aborigines in addressing emergent patterns of behavioural distress including suicide, parasuicide, ludic behaviour and self-mutilation. This paper demonstrates the importance of the socio-historical frame in the examination of these behaviours from one discrete region in isolated Aboriginal Australia: the Kimberley.

The Dampier Rock Art Precinct contains the largest and most ancient collection of Aboriginal rock art in Australia. The cultural landscape created by generations of Aboriginal people includes images of long-extinct fauna and demonstrates the response of peoples to a changing climate over thousands of years as well as the continuity of lived experience. Despite Australian national heritage listing in 2007, this cultural landscape continues to be threatened by industrial development. Rock art on the eastern side of the archipelago, on the Burrup Peninsula, was relocated following the discovery of adjacent off-shore gas reserves so that a major gas plant could be constructed. Work has now begun on the construction of a second major gas plant nearby. This article describes the rock art of the Dampier Archipelago and the troubled history of European-Aboriginal contact history, before examining the impact of industry on the region and its environment. The destruction of Aboriginal rock art to meet the needs of industry is an example of continuing indifference to Aboriginal culture. While the complex struggle to protect the cultural landscape of the Burrup, in particular, involving Indigenous people, archaeologists, historians, state and federal politicians, government bureaucrats and multi-national companies, eventually led to national heritage listing, it is not clear that the battle to save the Burrup has been won.

Many misconceptions about Aboriginal and Torres Strait Islanders stem from Australia’s period of colonisation in the 18th and 19th centuries when Indigenous people were believed to be inferior by European settlers. It is disturbing that after 200 years these negative ideas still exist and are often perpetuated through the mass media. Even though schools are well positioned to challenge these colonial values; unfortunately there are many factors which affect the depth and quality of teaching Aboriginal content, such as culture, history and contemporary issues. The government has aimed to disperse the inconsistencies associated with teaching Aboriginal perspectives by implementing a new Australian Curriculum with prescribed Aboriginal content. However, these changes will only have an impact if teachers are equipped and motivated to implement them. Therefore, understanding what influences teachers’ choice of content and approach to teaching Aboriginal Studies was a key aspect of this study. To explore these influences, non-Aboriginal teachers were interviewed at both government and independent schools. A desktop audit analysing the courses at Western Australian universities placed the participants’ responses into context of the pre-service and in-service learning available. An audit of the Australian Curriculum explored the expectations placed on teachers and the depth to which Aboriginal perspectives can be taught or avoided in secondary schools. This study identified four key issues that affected the teaching of Aboriginal curriculum content by non-Aboriginal teachers. These factors are: time management, school culture, teacher interest and preconceived ideas of both teachers and students. These issues must be addressed in order to improve the quality teaching of Aboriginal perspectives by standardising in-service learning and pre-service learning. Furthermore, improving cultural competency and acknowledging Aboriginal culture is critical at every school, regardless of the number of Aboriginal students. This study also revealed that the presence of an Aboriginal and Islander Education Officer could have a big impact in providing support, information and an Aboriginal presence at the school. Without these changes it will be difficult to disperse colonial values and challenge negative stereotypes

This thesis examines how Aboriginality, ‘Asianness’ and whiteness have been imagined from Federation in 1901 to the present. It recovers a rich but hitherto largely neglected history of twentieth century cross-cultural partnerships and alliances between Indigenous and Asian-Australians. Commercial and personal intercourse between these communities has existed in various forms on this continent since the pre-invasion era. These cross-cultural exchanges have often been based on close and long-term shared interests that have stemmed from a common sense of marginalisation from dominant Anglo-Australian society. At other times these cross-cultural relationships have ranged from indifference to hostility, reflecting the fact that migrants of Asian descent remain the beneficiaries of the dispossession of Aboriginal and Torres Strait Islander peoples. This thesis examines the material correlates of religious behaviour. Religion is an important part of every culture, but the impact religion has on structuring material culture is not well understood. Archaeologists are hampered in their reconstructions of the past because they lack comparative methods and universal conventions for identifying religious behaviour. The principal aim of this thesis is to construct an indicator model which can archaeologically identify religious behaviour. The basis for the proposed model stems directly from recurrent religious phenomena. Such phenomena, according to anthropological and cognitive research, relate to a series of spatio-temporally recurrent religious features which relate to a universal foundation for religious concepts. Patterns in material culture which strongly correlate with these recurrent phenomena indicate likely concentrations of religious behaviour. The variations between sacred and mundane places can be expected to yield information regarding the way people organise themselves in relation to how they perceive their cosmos. Using cognitive religious theory, stemming from research in neurophysiology and psychology, it is argued that recurrent religious phenomena owe their replication to the fact that certain physical stimuli and spatial concepts are most easily interpreted by humans in religious ideas. Humans live in a world governed by natural law, and it is logical that the concepts generated by humans will at least partially be similarly governed. Understanding the connection between concept and cause results in a model of behaviour applicable to cross-cultural analysis and strengthens the model’s assumption base. In order to test the model of religious behaviour developed in this thesis it is applied to a regional archaeological matrix from the Blue Mountains National Park in New South Wales, Australia. Archaeological research in the Blue Mountains has tentatively identified ceremonial sites based on untested generalised associations between select artefact types and distinctive geographic features. The method of analysis in this thesis creates a holistic matrix of archaeological and geographic data, encompassing both qualitative and quantitative measures, which generates a statistical norm for the region. Significant liminal deviations from this norm, which are characteristic indicators of religious behaviour are then identified. Confidence in these indicators’ ability to identify ceremonial sites is obtained by using a distance matrix and algorithms to examine the spatial patterns of association between significant variables. This thesis systematically tests the associations between objects and geography and finds that a selective array and formulaic spatiality of material correlates characteristic of religious behaviour does exist at special places within the Blue Mountains. The findings indicate a wide spread if more pocketed distribution of ceremonial sites than is suggested in previous models. The spatial/material relationships for identified religious sites indicates that these places represent specialised extensions of an interdependent socio-economic system where ceremonial activity and subsistence activity operated in balance and were not isolated entities.

AbstractPolitical representations of Aboriginal and Torres Strait Islander children and youth reflect the deep ambivalences Australian society continues to hold toward First Nations people. This chapter explores these ambivalences by considering two key representative fields concerning Aboriginal and Torres Strait Islander children in recent years, which serve to illustrate our thesis that children play a constitutive role as temporary outsiders who present both risk and renewal to the demos (Bray & Nakata, The Figure of the Child in Democratic Politics. Contemporary Political Theory, 19, 20. https://doi.org/10.1057/s41296-019-00319-x, 2020). The first focuses on the Northern Territory Don Dale Youth Detention Centre that became the site of political controversy in 2016 for its mistreatment of youth detainees. The second explores a 2020 campaign by the conservative Liberal National Party in a recent Queensland state election to implement a youth curfew in the cities of Townsville and Cairns, that have a high number of Aboriginal and Torres Strait Islander residents. As evidenced by these debates about youth crime and incarceration, we argue that Aboriginal and Torres Strait Islander children are often represented as a source of risk which lies in tension with and forecloses the transformative potential of representing Indigenous children as sources of renewal. These cases reveal the representative terrain in which Aboriginal and Torres Strait Islander young people must resist and speak back to a white national imaginary that works to limit the possible futures that Aboriginal and Torres Strait Islander peoples imagine for themselves.

Moreover, supporting these Aboriginal learners to have a sense of self-worth, self-awareness and personal identity that enables them to manage their emotional, mental, cultural, spiritual and physical wellbeing was identified as a key goal of the “Alice Springs (Mparntwe) Education Declaration”. This declaration sets out the national vision for education and the commitment of Australian Governments to improve educational outcomes for Aboriginal peoples across Australia (Commonwealth of Australia, 2019). This is a critical responsibility for the practices of Australian educators, policymakers and researchers alike. This chapter presents a unique on-Country approach to research with young Aboriginal people seeking to understand what a world worth living in means to them as individuals and for the communities they live in. The approach involved multimodal research methods that included poetry composition and photography, as media that revealed their Aboriginal youth voices, cultural sensitivities, identity and agency. For these young Aboriginal people, sitting on their own Country with sand from their Wiradjuri land sifting through their fingers, their words and images emerged as powerful resources for connecting to culture and to self as their Aboriginal identities flourished despite previously being demeaned by racism, ignorance, injustice and inequity. The poetry and photographs produced by these young Aboriginal males serve as a window into how cultural voice and vision expose ways identity and agency are socially-culturally-politically configured—both in their production and deployment. Their words and images demonstrate the kind of resilience needed for these Aboriginal youth to take their place in the world—one that they, too, see as worth living in.

AbstractMurujuga, as the Dampier Archipelago (including Burrup Peninsula) National Heritage Listed Place is known to its traditional custodians, is on the Pilbara coast of northern Western Australia. Murujuga’s scientific values are endorsed on Australia’s National Heritage List under a range of significance criteria. This chapter describes how an Australian local Aboriginal community’s contemporary connections and significance values have been framed through the lens of Outstanding Universal Value in a world heritage nomination—and the scaffolding required to translate local and national heritage values into the global purview. The World Heritage List (WHL) criteria distinguish between natural and cultural values: an anathema to Aboriginal custodians who see ngurra (country) as both a natural and cultural domain. We describe the disjunct between Aboriginal custodial connections to country and UNESCO’s framing of Outstanding Universal Values (OUV) for a world heritage nomination. The Ngarda-Ngarli are pursuing World Heritage by documenting outstanding universal cultural values under criteria i, iii, and v). For Aboriginal custodians this journey towards international recognition provides an opportunity to assert their local connection and control over this significant place, in the belief that global recognition will increase its protection. This chapter explores whether World Heritage recognition will help its traditional custodians to manage this extraordinary heritage estate, particularly in the face of the national economic value being placed on Industry in this same landscape.

AbstractA classroom can be a place to belong. Students become settled, ideas become familiar, relations become belongings. Teachers attentive to belonging can support critical conversations without fear that students will accidentally stumble onto alienating terrain. But the desire to settle, to make familiar, and to belong is not without its own ambivalence. For example, should non-Indigenous Australian students feel they ‘belong’ when engaging with the legacies of settler colonialism? Is learning about Aboriginal and Torres Strait Islander cultures and communities a desirable way for non-Indigenous students to feel settled in Australia? Aileen Moreton-Robinson’s work interrogates two impulses towards belonging among non-Indigenous Australians. On the one hand, she considers the erasure of Indigenous belonging through the legal fallacy of terra nullius and its subsequent variations in myths of British belonging to Australia. On the other hand, the essay questions non-Indigenous appeals to Indigenous communities as potential partners in national projects of collective belonging. Moreton-Robinson shows that non-Indigenous Australians ‘possess’ their symbolic home in the nation-state at the expense of Indigenous belonging. In what ways can non-Indigenous students be invited to question practices of belonging? What new classroom might this produce, and would everyone need to belong?

Abstract In this chapter on therapeutic residential care in Australia, the authors identify what is perhaps an overemphasis on “downsizing”—for example, reducing to units of four or fewer—without comparable efforts at “specialization” of programs and attention to their critical elements as inadequate to meet the complex needs of differing at-risk youth. More generally, they also question inadequate emphasis on group processes and structure, which they argue is central to the therapeutic milieu; inadequate emphasis on evidence-based practices, including model programs; inadequate national data gathering; and continuing disproportionality of Aboriginal children in out-of-home care as areas of continuing concern. They conclude that the idea of a “revitalized” residential care sector in Australia seems a long way off.

The politics of identity discussed here are still at the heart of current Indigenous Australian struggles for recognition. In the 1960s, for ethical and political reasons, the term Aboriginal became an ethnonym written with a capital ‘A’ to designate the descendants of the first inhabitants of Australia, some 500 groups speaking different languages. Aboriginal groups have not only different language names and cultural backgrounds, but different histories — massacres, forced sedentarisation in reserves, separation from their parents of children of mixed descent, discrimination, criminalisation — all of which broke the transmission of some peoples’ heritage. Yet many claim their ‘Aboriginality’ which gathers all under an Aboriginal flag (since 1972), even if not everyone agrees on the definition of a common identity. Some priviledge an identity of continuity, based on language, localised spirit-children and ritual links with the land, pre-contact modes of existence; others put forward an identity of resistance, rewriting colonial history, valorising a national Aboriginal identity that encompasses all mixed descendants, struggling for land-rights, against bad living conditions, exclusion and exploitation. Australia is in energy transition despite a national policy supportive of fossil fuels. Regional and remote areas, however, remain dependent on fossil fuels, including diesel. Renewable energy is becoming accessible for some regional communities, due to renewable energy incentives. This chapter considers the energy transition in Australia through the energy justice lens. It analyses the distribution of benefits and burdens of energy activities upon remote Indigenous communities, and examines energy price impacts and consumer protection reforms in liberalized electricity markets in the south. The analysis examines how social justice needs to inform the energy transition, also recognising that energy injustice cannot be separated from other social ills, such as poverty and discrimination based on factors including class, race, gender, or indigeneity. It concludes that there are significant protections emerging for energy consumers in the national electricity market, but an inequitable distribution of energy benefits and burdens in remote Aboriginal communities.

13 uranium mines operated in the South Alligator Valley of Australia’s Northern Territory between 1953 and 1963. At the end of operations the mines, and associated infrastructure, were simply abandoned. As this activity preceded environmental legislation by about 15 years there was neither any obligation, nor attempt, at remediation. In the 1980s it was decided that the whole area should become an extension of the adjacent World Heritage, Kakadu National Park. As a result the Commonwealth Government made an inventory of the abandoned mines and associated facilities in 1986. This established the size and scope of the liability and formed the framework for a possible future remediation project. The initial program for the reduction of physical and radiological hazards at each of the identified sites was formulated in 1989 and the works took place from 1990 to 1992. But even at this time, as throughout much of the valley’s history, little attention was being paid to the long term aspirations of traditional land owners. The traditional Aboriginal owners, the Gunlom Land Trust, were granted freehold Native Title to the area in 1996. They immediately leased the land back to the Commonwealth Government so it would remain a part of Kakadu National Park, but under joint management. One condition of the lease required that all evidence of former mining activity be remediated by 2015. The consultation, and subsequent planning processes, for a final remediation program began in 1997. A plan was agreed in 2003 and, after funding was granted in 2005, works implementation commenced in 2007. An earlier paper described the planning and consultation stages, experience involving the cleaning up of remant uranium mill tailings and other mining residues; and the successful implementation of the initial remediation works. This paper deals with the final planning and design processes to complete the remediation programme, which is due to occur in 2009. The issues of final containment design and long term stewardship are addressed in the paper as well as some comments on lessons learned through the life of the project.

From the late nineteenth century, ramblers, trampers and bushwalkers have been instrumental in the creation of national parks. Their advocacy combined interests in nature conservation with recreational pursuits, heralding the two competing and often contradictory purposes of national park estates. In Australia, protected wilderness areas were invariably repositories of sacred sites linked by networks of walking pads across landscapes shaped by millennia of Indigenous occupation. From the mid-twentieth century, new infrastructure was required in national parks to cater for the growth in tourism. In Australia, the state-based system of “national” parks resulted in an uneven approach to both the creation of protected areas and the design of infrastructure for the hosts and guests. This approach was in marked contrast to the United States, where the Mission 66 program – approved by Congress in 1955 – resulted in a decade-long programme of expenditure on infrastructure that established the reputation of their national park system, and ensured a systematic national approach. This paper examines the piecemeal history of planning for bushwalkers in Australian national parks through a comparison of competing interests – the minimal needs of the self-sufficient rambler with infrastructure that caters for diverse tourism experiences. Australian case studies illustrate a contested but changing approach to planning for pedestrians in protected areas, from the making of tracks by volunteers and depression-era work gangs to elevated walks through forest canopies. A historical analysis highlights the changing attitudes to tourism and conservation challenges, now informed by greater knowledge of ecology and the belated recognition of Indigenous ownership and pre-colonial land management regimes. Threats to the biodiversity in protected areas suggest that a planning approach, which combines multiple disciplines and interests, will increasingly elevate both the bushwalker and tourist in their experience of nature.

<p>Confidential Reporting on Structural Safety – Australasia (CROSS-AUS) is the confidential reporting system established in Australasia in 2018 to capture and share lessons learned from structural safety issues which might not otherwise get public recognition. It builds on the success of CROSS- UK, the unique UK based system which has been operating since 2005 and is part of a growing network of CROSS programmes internationally. There has been interest in CROSS in Australia for many years and CROSS-AUS would like to see this interest now extended into New Zealand. Ultimately the intention is to have a global freely available database to be used by the construction industry anywhere to improve the safety of their buildings, structures, and national infrastructure. The paper will describe the processes used by CROSS and provide examples of instances where beneficial changes have resulted.</p>

According to the World Energy Council (WEC) the estimated energy of the wave power in the world is in the range of 8,000 to 80,000 TWh/year to depths of 100 meters or higher and actually the utilization of wave energy resource it is possible because it has been implemented in countries like Australia, Indonesia, Nigeria, United Kingdom, Norway, Portugal and Colombia evaluating different types of marine technologies that take the advantage of the kinetic energy in the ocean waves. Mexico according to the National Institute of Statistics and Information (INEGI) has a land area of 1,972,550 km2 of which has a coastline of 11,150 km having potential for the use of their coasts. Baja California with a land area of 71,445 km2 (3.6% of the country) is located on a peninsula in northwest Mexico and has 720 km of coastline on the Pacific Ocean (6.4% nationally) with a range of depths of 25.6 m to 650 m at a distance of the coastline of 15 km, which makes it suitable to evaluate the use of wave energy at local sites. With the completion of this work will contribute to the characterization of the sites that will present the best technical and economic conditions for its implementation, considering the physical characteristics of the site as well as connection points on the transmission lines operated by the Federal Electricity Commission (CFE). For the preparation of this study was carried out in three stages: a) Site Selection, b) Evaluation of Wave Energy and c) Economic evaluation of sites using RETScreen. Based on the characteristics of the coast of Baja California the results obtained are the following: 1) 18 sites were selected with a sea depth averaged of 50 m, the annual density power was 7.5 kW/m, this represents a potential of 210 MW considering an average length of 2 km in each site, 2) The economic evaluation of this type of project was for a period of 30 years in RETScreen, considers an annual inflation rate of 5% and obtains an investment cost of 9,538 US $/kW for this type of generation. We conclude that this source of energy will reduce dependence on fossil fuels and contribute to the generation of electricity in the state of Baja California diversifying the energetic matrix state by the use of clean and renewable sources, which represents an investment opportunity between the public and private sector.

Professor Leslie R. Marchant was a Western Australian historian of international renown. Richly educated as a child in political philosophy and critical reason, Marchant’s understandings of western political philosophies were deepened in World War Two when serving with an international crew of the merchant navy. After the war’s end, Marchant was appointed as a Protector of Aborigines in Western Australia’s Depart of Native Affairs. His passionate belief in Enlightenment ideals, including the equality of all people, was challenged by his experiences as a Protector. Leaving that role, he commenced his studies at The University of Western Australia where, in 1952, his Honours thesis made an early case that genocide had been committed in the administration of Aboriginal people in Western Australia. In the years that followed, Marchant became an early researcher of modern China and its relationship with the West, and won respect for his archival research of French maritime history in the Asia-Pacific. This work, including the publication of France Australe in 1982, was later recognised with the award of a French knighthood, the Chevalier d’Ordre National du Mèrite, and his election as a fellow to the Royal Geographical Society. In this festschrift, scholars from The University of Notre Dame Australia appraise Marchant’s work in such areas as Aboriginal history and policy, Westminster traditions, political philosophy, Australia and China and French maritime history.

Background Lung cancer is the number one cause of cancer death worldwide.(1) It is the fifth most commonly diagnosed cancer in Australia (12,741 cases diagnosed in 2018) and the leading cause of cancer death.(2) The number of years of potential life lost to lung cancer in Australia is estimated to be 58,450, similar to that of colorectal and breast cancer combined.(3) While tobacco control strategies are most effective for disease prevention in the general population, early detection via low dose computed tomography (LDCT) screening in high-risk populations is a viable option for detecting asymptomatic disease in current (13%) and former (24%) Australian smokers.(4) The purpose of this Evidence Check review is to identify and analyse existing and emerging evidence for LDCT lung cancer screening in high-risk individuals to guide future program and policy planning. Evidence Check questions This review aimed to address the following questions: 1. What is the evidence for the effectiveness of lung cancer screening for higher-risk individuals? 2. What is the evidence of potential harms from lung cancer screening for higher-risk individuals? 3. What are the main components of recent major lung cancer screening programs or trials? 4. What is the cost-effectiveness of lung cancer screening programs (include studies of cost–utility)? Summary of methods The authors searched the peer-reviewed literature across three databases (MEDLINE, PsycINFO and Embase) for existing systematic reviews and original studies published between 1 January 2009 and 8 August 2019. Fifteen systematic reviews (of which 8 were contemporary) and 64 original publications met the inclusion criteria set across the four questions. Key findings Question 1: What is the evidence for the effectiveness of lung cancer screening for higher-risk individuals? There is sufficient evidence from systematic reviews and meta-analyses of combined (pooled) data from screening trials (of high-risk individuals) to indicate that LDCT examination is clinically effective in reducing lung cancer mortality. In 2011, the landmark National Lung Cancer Screening Trial (NLST, a large-scale randomised controlled trial [RCT] conducted in the US) reported a 20% (95% CI 6.8% – 26.7%; P=0.004) relative reduction in mortality among long-term heavy smokers over three rounds of annual screening. High-risk eligibility criteria was defined as people aged 55–74 years with a smoking history of ≥30 pack-years (years in which a smoker has consumed 20-plus cigarettes each day) and, for former smokers, ≥30 pack-years and have quit within the past 15 years.(5) All-cause mortality was reduced by 6.7% (95% CI, 1.2% – 13.6%; P=0.02). Initial data from the second landmark RCT, the NEderlands-Leuvens Longkanker Screenings ONderzoek (known as the NELSON trial), have found an even greater reduction of 26% (95% CI, 9% – 41%) in lung cancer mortality, with full trial results yet to be published.(6, 7) Pooled analyses, including several smaller-scale European LDCT screening trials insufficiently powered in their own right, collectively demonstrate a statistically significant reduction in lung cancer mortality (RR 0.82, 95% CI 0.73–0.91).(8) Despite the reduction in all-cause mortality found in the NLST, pooled analyses of seven trials found no statistically significant difference in all-cause mortality (RR 0.95, 95% CI 0.90–1.00).(8) However, cancer-specific mortality is currently the most relevant outcome in cancer screening trials. These seven trials demonstrated a significantly greater proportion of early stage cancers in LDCT groups compared with controls (RR 2.08, 95% CI 1.43–3.03). Thus, when considering results across mortality outcomes and early stage cancers diagnosed, LDCT screening is considered to be clinically effective. Question 2: What is the evidence of potential harms from lung cancer screening for higher-risk individuals? The harms of LDCT lung cancer screening include false positive tests and the consequences of unnecessary invasive follow-up procedures for conditions that are eventually diagnosed as benign. While LDCT screening leads to an increased frequency of invasive procedures, it does not result in greater mortality soon after an invasive procedure (in trial settings when compared with the control arm).(8) Overdiagnosis, exposure to radiation, psychological distress and an impact on quality of life are other known harms. Systematic review evidence indicates the benefits of LDCT screening are likely to outweigh the harms. The potential harms are likely to be reduced as refinements are made to LDCT screening protocols through: i) the application of risk predication models (e.g. the PLCOm2012), which enable a more accurate selection of the high-risk population through the use of specific criteria (beyond age and smoking history); ii) the use of nodule management algorithms (e.g. Lung-RADS, PanCan), which assist in the diagnostic evaluation of screen-detected nodules and cancers (e.g. more precise volumetric assessment of nodules); and, iii) more judicious selection of patients for invasive procedures. Recent evidence suggests a positive LDCT result may transiently increase psychological distress but does not have long-term adverse effects on psychological distress or health-related quality of life (HRQoL). With regards to smoking cessation, there is no evidence to suggest screening participation invokes a false sense of assurance in smokers, nor a reduction in motivation to quit. The NELSON and Danish trials found no difference in smoking cessation rates between LDCT screening and control groups. Higher net cessation rates, compared with general population, suggest those who participate in screening trials may already be motivated to quit. Question 3: What are the main components of recent major lung cancer screening programs or trials? There are no systematic reviews that capture the main components of recent major lung cancer screening trials and programs. We extracted evidence from original studies and clinical guidance documents and organised this into key groups to form a concise set of components for potential implementation of a national lung cancer screening program in Australia: 1. Identifying the high-risk population: recruitment, eligibility, selection and referral 2. Educating the public, people at high risk and healthcare providers; this includes creating awareness of lung cancer, the benefits and harms of LDCT screening, and shared decision-making 3. Components necessary for health services to deliver a screening program: a. Planning phase: e.g. human resources to coordinate the program, electronic data systems that integrate medical records information and link to an established national registry b. Implementation phase: e.g. human and technological resources required to conduct LDCT examinations, interpretation of reports and communication of results to participants c. Monitoring and evaluation phase: e.g. monitoring outcomes across patients, radiological reporting, compliance with established standards and a quality assurance program 4. Data reporting and research, e.g. audit and feedback to multidisciplinary teams, reporting outcomes to enhance international research into LDCT screening 5. Incorporation of smoking cessation interventions, e.g. specific programs designed for LDCT screening or referral to existing community or hospital-based services that deliver cessation interventions. Most original studies are single-institution evaluations that contain descriptive data about the processes required to establish and implement a high-risk population-based screening program. Across all studies there is a consistent message as to the challenges and complexities of establishing LDCT screening programs to attract people at high risk who will receive the greatest benefits from participation. With regards to smoking cessation, evidence from one systematic review indicates the optimal strategy for incorporating smoking cessation interventions into a LDCT screening program is unclear. There is widespread agreement that LDCT screening attendance presents a ‘teachable moment’ for cessation advice, especially among those people who receive a positive scan result. Smoking cessation is an area of significant research investment; for instance, eight US-based clinical trials are now underway that aim to address how best to design and deliver cessation programs within large-scale LDCT screening programs.(9) Question 4: What is the cost-effectiveness of lung cancer screening programs (include studies of cost–utility)? Assessing the value or cost-effectiveness of LDCT screening involves a complex interplay of factors including data on effectiveness and costs, and institutional context. A key input is data about the effectiveness of potential and current screening programs with respect to case detection, and the likely outcomes of treating those cases sooner (in the presence of LDCT screening) as opposed to later (in the absence of LDCT screening). Evidence about the cost-effectiveness of LDCT screening programs has been summarised in two systematic reviews. We identified a further 13 studies—five modelling studies, one discrete choice experiment and seven articles—that used a variety of methods to assess cost-effectiveness. Three modelling studies indicated LDCT screening was cost-effective in the settings of the US and Europe. Two studies—one from Australia and one from New Zealand—reported LDCT screening would not be cost-effective using NLST-like protocols. We anticipate that, following the full publication of the NELSON trial, cost-effectiveness studies will likely be updated with new data that reduce uncertainty about factors that influence modelling outcomes, including the findings of indeterminate nodules. Gaps in the evidence There is a large and accessible body of evidence as to the effectiveness (Q1) and harms (Q2) of LDCT screening for lung cancer. Nevertheless, there are significant gaps in the evidence about the program components that are required to implement an effective LDCT screening program (Q3). Questions about LDCT screening acceptability and feasibility were not explicitly included in the scope. However, as the evidence is based primarily on US programs and UK pilot studies, the relevance to the local setting requires careful consideration. The Queensland Lung Cancer Screening Study provides feasibility data about clinical aspects of LDCT screening but little about program design. The International Lung Screening Trial is still in the recruitment phase and findings are not yet available for inclusion in this Evidence Check. The Australian Population Based Screening Framework was developed to “inform decision-makers on the key issues to be considered when assessing potential screening programs in Australia”.(10) As the Framework is specific to population-based, rather than high-risk, screening programs, there is a lack of clarity about transferability of criteria. However, the Framework criteria do stipulate that a screening program must be acceptable to “important subgroups such as target participants who are from culturally and linguistically diverse backgrounds, Aboriginal and Torres Strait Islander people, people from disadvantaged groups and people with a disability”.(10) An extensive search of the literature highlighted that there is very little information about the acceptability of LDCT screening to these population groups in Australia. Yet they are part of the high-risk population.(10) There are also considerable gaps in the evidence about the cost-effectiveness of LDCT screening in different settings, including Australia. The evidence base in this area is rapidly evolving and is likely to include new data from the NELSON trial and incorporate data about the costs of targeted- and immuno-therapies as these treatments become more widely available in Australia.
