Der dysfunktionale Konsum psychotroper Substanzen geht in der Regel mit einem Zwiespalt zwischen Abstinenzvorsatz bzw. der Absicht, den Konsum zu reduzieren, einerseits und dem Wunsch oder Zwang, den Konsum fortzusetzen, andererseits einher. Das von Miller und Rollnick (1991) beschriebene Motivational Interviewing (MI) ist ein zugleich klientenzentrierter und direktiver Behandlungsstil, der dieser Ambivalenz Rechnung tragen und somit Veränderungsbereitschaft erhöhen soll. Miller und Rollnick (1991) nennen fünf Prinzipien motivationaler Gesprächsführung: <OL><LI>Empathie, <LI>Widersprüche aufzeigen, <LI>Wortgefechte vermeiden, <LI>Nachgiebig auf Widerstand reagieren und <LI>Selbstwirksamkeit fördern.</OL> Diese Prinzipien stimmen mit den Annahmen (sozial-) psychologischer Modelle der Einstellungs- und Verhaltensänderung überein. Seit Ende der achtziger Jahre wurden vorwiegend in den angelsächsischen Ländern verschiedene motivationale Interventionen zur Sekundärprävention und Behandlung von Substanzabhängigkeit und -missbrauch entwickelt, die den von Miller und Rollnick (1991) formulierten Behandlungsprinzipien entsprechen (der Drinker’s Checkup, Motivational Enhancement Therapy, das Harm-Reduction-Programm BASICS, Brief Motivational Interviewing, Brief Negotiation sowie eine Reihe weiterer motivationaler Kurzinterventionen). Vor dem Hintergrund der bislang vorliegenden Literatur erscheint insbesondere die Durchführung standardisierter motivationaler Interventionen zur Reduktion dysfunktionalen Alkoholkonsums bzw. der negativen Konsequenzen eines fortgesetzten Alkoholmissbrauchs gerechtfertigt. Voraussetzungen einer Optimierung des Behandlungserfolgs sind neben der Identifikation zentraler Wirkmechanismen u.a. eine fortlaufende Prozess-Evaluation der Implementierung motivationaler Interventionen sowie eine evidenzbasierte Ausbildung.

The core principle of the approach is negotiation rather than conflict. In this article I review the historical development of motivational interviewing and give some of the theoretical underpinnings of the approach. I summarise the available evidence on its usefulness and discuss practical details of its implementation, using vignettes to illustrate particular techniques.

Objectives This review examines the effectiveness of motivational interviewing for physical activity self-management for adults diagnosed with diabetes mellitus type 2. Motivational interviewing is a patient centered individually tailored counseling intervention that aims to elicit a patient’s own motivation for health behavior change. Review questions include (a) How have motivational interviewing methods been applied to physical activity interventions for adults with diabetes mellitus type 2? (b) What motivational interviewing approaches are associated with successful physical activity outcomes with diabetes mellitus 2? Methods Database searches used PubMed, CINAHL, and PsycINFO for the years 2000 to 2016. Criteria for inclusion was motivational interviewing used as the principal intervention in the tradition of Miller and Rollnick, measurement of physical activity, statistical significance reported for physical activity outcomes, quantitative research, and articles written in English. Results A total of nine studies met review criteria and four included motivational interviewing interventions associated with significant physical activity outcomes. Discussion Findings suggest motivational interviewing sessions should target a minimal number of self-management behaviors, be delivered by counselors proficient in motivational interviewing, and use motivational interviewing protocols with an emphasis placed either on duration or frequency of sessions.

Introduction. Tuberculosis (TB) is an infectious disease that can attack various organs, especially the lung caused by Mycobacterium tuberculosis. The use of the Directly Observed Treatment Short-course (DOTS) strategy for tuberculosis treatment has not been successful, while cases of multidrug tuberculosis resistance have increased. Long-term treatment leads to decreased motivation and despair to recover. Motivational interviewing (MI) intervention is one way to overcome them. The purpose of this study was to determine the effect of motivational interviewing on hopelessness, motivation, and adherence to taking medication for tuberculosis patients. Methods. Pre experiment with the form of pretest - posttest group design was used in this study. The research sample amounted to 18 respondents by means of random sampling technique. This intervention was carried out for four weeks with two meetings per week. The independent variable is motivational interviewing, while the dependent variable is hopelessness, motivation, and compliance with taking medication. The instrument in this study uses a questionnaire that has been tested for validity and reliability. Statistical tests using the Wilcoxon signed rank test. Results. The results of statistical tests showed that motivational interviewing had an effect on decreased despair (p = 0,000) and increased motivation and compliance with medication (p = 0,000). Conclusion. Motivational interviewing interventions help tuberculosis sufferers identify, evaluate and respond to something they think, so that motivational interviewing has an effect on decreasing hopelessness, increasing motivation and adhering to medication in tuberculosis patients.

Abstract Introduction Automatic positive airway pressure (APAP) adherence to treat obstructive sleep apnea (OSA) is challenging to many patients. Motivational interviewing in one-on-one sessions has been shown to improve rates of APAP adherence. The goal of our improvement study was to improve APAP adherence and determine whether motivational interviewing is effective in a group setting. Methods Patients with a new diagnosis of OSA, regardless of severity, presented to our academic sleep disorders center for a group presentation on OSA and available treatments from November – December 2023. Groups of 5-10 patients received either a standard informative presentation or the same presentation with motivational interviewing techniques. Questionnaires with a Likert scale from 1 to 10 assessed pre- and post-presentation readiness to undergo treatment for their OSA and perceptions of importance to treat. Post-presentation questionnaires also included questions on likeliness to pursue treatment and confidence to pursue treatment. Results A total of 41 patients attended the group session with 53.6% (n=22) receiving the motivational interviewing component. For the motivational interviewing group, adherence data post-intervention revealed that the average total days of APAP use was 79%, average total hours used was 232.00 minutes, and average use greater than 4 hours was 51.6%. For the non-motivational interviewing group, adherence data post-intervention showed that the average total days of APAP use was 68.62%, average total hours used was 209.62 minutes, and average use greater than 4 hours was 46.54%. To date, 38% of non-motivational interviewing patients (n=13) were adherent by Medicare definition, compared to 40% of motivational interviewing patients (n=5). Questionnaires had equivocal scores in all categories pre- and post-presentation for the motivational interviewing and non-motivational interviewing groups and indicated a high level of motivation to pursue therapy. Conclusion Implementing motivational interviewing in a group setting was a feasible way to improve APAP adherence in our clinic. Patients are motivated to pursue APAP therapy in pre- and post-presentation questionnaires. This preliminary data is limited by ongoing intervention implementation yet promising for the use of motivational interviewing in the group setting. To the authors knowledge, no published improvement projects exist for evaluating the effectiveness of motivational interviewing in a group setting. Support (if any)
