Streszczenie:
With increasing numbers of children being placed in out-of-home care it is vital that we have a better understanding of the factors that influence the placement journey, specifically placement stability and placement movement. This convergent mixed method research investigated factors influencing placement trajectory from the perspective of children and carers in the out-of-home care system. Child-carer engagement at the levels of individual child or carer, family-care context and child protection system were found to be critical. A key outcome was the recommendation to structure out-of-home care policy and practice around the placement trajectory concept, placing relationship at the centre, and using child-focused outcomes to evaluate out of home care.

Chan, Christine Mei Sheung. "Obesity in preschool children in Hong Kong : a mixed method study". Thesis, University College London (University of London), 2008. http://discovery.ucl.ac.uk/1444169/.

Pełny tekst źródła
Streszczenie:
Background: Obesity is the world's fastest growing epidemic, especially in areas of rapid social and economic transition such as South East Asia. The prevalence of childhood obesity in Hong Kong and China has been rapidly increasing for a decade. The Medline database lists 100,000 articles on obesity, with hundreds of confirmed and putative "causes" and hypothesized (though rarely proven) preventive strategies. Contemporary scholars have called for obesity research to stop focusing on single issues (at the level of the genome, the individual, or society as a whole) and embrace multi-level theories that allow the interplay between micro-and macro to be explored and followed over time. Since little information of Hong Kong childhood obesity has been available, this study used Glass and McAttec (2006) and Sallis et al (2005) to study the aetiology of obesity in Hong Kong preschool children. The work described in this thesis aimed at investigating the social and cultural factors of childhood obesity in Hong Kong. Methods Mixed method research, including quantitative and qualitative tools was employed for this study. Ten in-depth case studies were undertaken of six obese and four normal weight preschool children recruited from four kindergartens with different socio-economic characteristics throughout Hong Kong. These case studies were built up from ethnographic observation at home, in kindergartens, in restaurants/cafe's semi-structured interviews with child, parents, grandparents, live-in foreign domestic helpers and teachers construction of genogram analysis of dietary diary and questionnaire to primary caregiver were used. The case studies were nested in a wider quantitative study of body image perceptions and attitudes to food and child rearing in 119 primary caregivers of preschool children. SPSS software was used to analyse quantitative data for descriptive statistics, correlations and factor analysis. Ritchie and Spencer's framework was first employed to sort out the qualitative data. Subsequently a further, multi-level theoretical analysis was used to link quantitative and qualitative data, first to generate individual case descriptions, second to draw themes across cases, and third to place the findings in wider historical and policy context. Findings: The findings affirm previous research that the onset of obesity is multifactorial and involves a highly complex interplay between inherited predisposition, physiology, and individual behaviour (especially dietary choices, active play, and sleep) which is itself influenced by personality, attitudes and preferences. All of these appear to be shaped and constrained by opportunities (e.g. availability and affordability of food), and the wider social and economic context of Hong Kong as a modem, rapidly developing free-trade society. The data suggest that the knowledge, attitudes and confidence of the primary caregiver (typically the mother), which are strongly influenced by social networks and level of acculturation, are critically important in determining behaviour predisposing to obesity in the very young. Family structure, whether the family eat together regularly, and norms about mealtime behaviour, also seem critical. Conclusions: There are neither simple causes nor simple solutions to the obesity problem. Mixed method research driven by multi-level theories can help generate hypothesis about how multiple antecedents interact to cause obesity in an individual. Based on the findings of the work described here, a small-scale intervention, the Adopted Healthy Family Scheme, has been developed and is at the preliminary pilot stage.

Streszczenie:

This study explores the role of interracial parents in the development of racial identity in their mixed race children by examining how conversations surrounding race in a mixed race family relate to the ways children in that family racially identify. In addition, the study explores how parents’ understandings and perceptions of their own racial identity and their child’s racial identity affect the way their child feels about race. Semi-structured interviews were conducted on self-identified interracial parents and their mixed race children between the ages of 4 to 9 years old based on themes regarding mixed race identity including family’s identity, racial awareness of the child, dual socialization, and sociocultural factors. This study found that parents early experiences growing up, phenotypic expression of parent and child, current political climate, stereotypes and influence of schools had related to the ways in which parents discussed race with their children. More research must be done on mixed race identity, particularly outside of Black/White dichotomies.

Streszczenie:
Autism Spectrum Disorder (ASD) is a neurodevelopmental disorder affecting 0.74% of children under the age of 7 in Australia. ASD is characterised by persistent impairments in social interaction and communication across different contexts, as well as restricted, repetitive behaviour, activities, or interests. Symptoms are usually present in the early developmental stages of life and cause significant impairment in social and other important areas of functioning. Parents of children with ASD are often highly stressed and experience poorer quality of life which has been associated with the additional challenges arising from parenting a child with ASD. Given that parental well-being has been shown to have a reciprocal relationship with child behaviour and child well-being, identifying potentially modifiable factors which can reduce parental stress and improve quality of life may have broader clinical benefit. One potential factor that has been relatively unexplored is self-compassion. Self-compassion generally refers to the nonjudgemental awareness of suffering whilst meeting this suffering with acts of self-kindness. This thesis investigated self-compassion using a mixed methods approach examining whether self-compassion could extend from a theoretical contribution to practical implications in mothers of children with ASD, using mixed methods. The aim of the first study was to test an extended ABCX model of adaptation by including an additional internal resource variable, self-compassion, to examine the potential contribution to the model. A self-report questionnaire was administered to 139 parents of children with ASD. Findings from this study suggest the negative dimension of self-compassion was related to greater parental stress, whereas the positive dimension of self-compassion was related to greater quality of life. The aim of the second study was to examine the daily encounters of parents with stress and self-compassion using semi structured interviews from 19 mothers. This study provided evidence relating to the impact and causes of stress, the benefits of self-compassion, the barriers to self-compassion, and the aids to self-compassion. The second part of the interviews gathered information regarding parents’ needs, goals, and preferences that subsequently informed a socially valid intervention for parents. Lastly, the third study of this project served as a proof-of-concept study, where three parents participated in a 4-week self-compassion-based intervention. The intervention was informed by the data collected in studies 1 and 2 as well as the Centre of Clinical Intervention Self-Compassion modules. Three mothers ages 30-48 years attended four, 2-hour sessions. This study found a self-compassion intervention to be socially valid and effective in improving parent outcomes, including an increase in self-compassion, decrease in parenting stress, depression, anxiety, and stress, and better quality of life. Understanding potential modifiable factors that may improve parent well-being is particularly important in parents of children with ASD, who tend to report higher stress and poorer quality of life.

Streszczenie:
Change in adaptive outcomes of children who are treated for brain tumors is examined using longitudinal data. The children received different types of treatment from none to any combinations of three treatments, which are surgery, radiation and chemotherapy. In this thesis, we use mixed model to find the significant variables that predict change in outcomes of communication skill, daily living skills and socialization skill. Fractional polynomial transformation method and Gompertz method are applied to build non-linear longitudinal curves. We use PRESS as the criterion to compare these two methods. Comparison analysis shows the effect of each significant variable on adaptive behaviors over time. In most cases, model with Gompertz method is better than that with Transformation method. Significant predictors of change in adaptive outcomes include Time, Gender, Surgery, SES classes, interaction between Time and Radiation, interaction between Time and Gender, interaction between Age and Gender.

Streszczenie:
Although social anxiety disorder (SAD) in children is increasing in western societies, there is a paucity of preventative psycho-educational programmes available. This mixed method research is aimed at evaluating and refining a newly developed intervention programme for a non-clinical child population. The objective is to evaluate the role of emotion regulation (ER) in social anxiety (SA) and identify what could aid the possible prevention of this disorder. The first study employed a video task with 64 children, aged 8-11 years to explore key behavioural measures associated with high versus low (non-clinical) SA. The negative consequences of heightened SA levels and significant relationship between SA and ER informed the focus of the Emotion and Social Skills Programme for Children (ESST-C). Study Two tested the efficacy of the ESST-C in a community sample of 144 children, aged 8-11 years of which 85 children participated in the ESST-C and 59 made up the control group (CG). Participants’ levels of SA and ER were assessed pre- and post- programme, and at three-month follow-up, whereas participants of the CG were only assessed at baseline and at three-month follow-up. The findings did not show a significant decrease in levels of SA and higher levels of ER in children who participated in the programme as opposed to children in the CG, except on levels of anxiety, only partly supporting the efficacy of the ESST-C. Furthermore, the results of the first two studies identified the importance of ER skills. The aim of the third study was therefore to understand ER from the child’s perspective. A qualitative study was conducted on a non-clinical group of 7-12 year olds whose parents were also interviewed on their child’s reactions to socially and emotionally challenging situations. The main findings of this study identified distinct ER styles related to SA, three of which are illustrated to highlight what needs to be assessed and addressed in the development of future prevention programmes for SAD with this age group. Overall, the current thesis is a step forward in the prevention of SAD in children.

This thesis investigates the phenomenon known as a Maori ethnic identity. The topic is investigated using personal interviews and the findings are reported by way of personal narrative. Eight informants were interviewed. All presently identify as Maori and have arrived at this point following a diverse range of experiences. The thesis documents these experiences and those things that are important to them on a personal level. As such, this thesis investigates the topic of Maori ethnicity as it pertains to a group of individuals, not to Maoridom as a whole. It was found that no single paradigm could be applied to my informants conception of identity. Each constructed their identity in a unique way. Integral to all identities, however, was the use of both cultural and biological factors. In constructing and maintaining their identities as Maori my informants looked firstly to the presence of ancestry and, following from this cultural practices were employed. The use of ancestry as a basis of identity, and the causal attributes associated with it (such as natural leanings towards the use of Maori language), represent essentialist tendencies on the part of many of my informants. Also of great interest was the perception, by many of my informants, that cultural traits were innate. This is described as a Lamarckian way of viewing ethnicity.

Streszczenie:
Memory difficulties are prevalent in patients with neurological disorders across lifespan. However, little is known about how memory difficulties are experienced in everyday life, and their functional impacts. This thesis aimed to improve the assessment of children’s everyday memory and explore patients’ lived experience and coping with an under-researched memory disorder—accelerated long-term forgetting (ALF). Chapter 1 presented a general introduction to everyday memory difficulties in patients with neurological disorders. In Chapter 2, a systematic review evaluated currently available paediatric memory questionnaires and found that all, except one memory questionnaire, lacked evidence base to be used as assessment tools. Hence, exploratory factor analyses (EFA) were conducted to establish factorial structures of two commonly administered questionnaires, the Parent Memory Questionnaire (PMQ) and Child Memory Questionnaire (Child MQ), in a neurotypical paediatric sample (Chapter 3) and a clinical sample (Chapter 4). The two EFAs successfully established a comparable three-factor structure for the PMQ, but not the Child MQ. Next, Chapter 5 and 6 provided the first evidence of the lived experience of ALF of adults with epilepsy using a mixed method approach. Quantitative and qualitative findings from Chapter 5 revealed that ALF was associated with profound losses across personal, relationship and functional domains, and multiple psychosocial challenges. Chapter 6 documented findings from semi-structured interviews with patients, their family members, and health professionals on coping strategies for ALF and ALF-inflicted psychosocial difficulties. These coping strategies were categorised into avoidance, emotion-focused, memory-focused, seizure control. Overall, findings informed on the measurement of everyday memory problems and urged for the development of comprehensive intervention for patients with neurological disorders who also experience memory difficulties.

s self-identifications regarding religion and their experiences in a predominantly Muslim country, Turkey. By adopting a qualitative research design, in-depth interviews were conducted with 9 Jewish-Muslim mixed couples and 3 children in Istanbul and in Izmir. In addition, an official from the Turkish Jewish Community and an active member of the community were interviewed to determine the general perception of the Turkish Jewish Community regarding mixed marriage. The Jewish-Muslim mixed couples&rsquo
self-identifications with their Jewish and Muslim cultures and heritages reveal the importance of understanding individuals&rsquo
self-perceptions about their own identities since their self-perceptions cannot be understood by primordial ascriptions. The children of these mixed couples&rsquo
self-identifications provide important insights into the issue of the relative attractiveness of Jewish and Muslim identities in Turkey. The Jewish-Muslim mixed couples&rsquo

Chow, Maria. "The psychological and social impact of influenza-like-illness in children on their families". Thesis, The University of Sydney, 2013. http://hdl.handle.net/2123/9263.

Pełny tekst źródła
Streszczenie:
Influenza and influenza-like-illness (ILI) can cause excess paediatric morbidity. There are around 20 million and 34 million cases of influenza and respiratory syncytial virus infections occurring worldwide in children younger than 5 years of age respectively. Research has shown that influenza and ILI in children pose significant medical and economic impacts. Parents of children with ILI had up to 4.3 days work days lost per episode for self-reported influenza. However, the psychological and social impacts of ILI have not been well explored. Understanding these impacts aids in the comprehensive assessment of disease burden, and assists in evaluation of health interventions targeting ILI. This thesis has two aims. The first aim is to explore the psychological and social impact of ILI in children on their families, through measuring the quality of life (QoL) of parents of children affected by ILI. The second aim is to develop an ILI-specific QoL questionnaire for measuring parental QoL. A systematic review was conducted to identify and evaluate the psychometric properties of condition-specific QoL questionnaires for caregivers of children with paediatric conditions. Twenty-five condition-specific caregiver QoL questionnaires covering 16 conditions were identified. Conditions included atopic dermatitis, asthma, diabetes, oro-facial disorders, and two acute illnesses. Questionnaires were developed predominantly in high-income countries. Questionnaires were high in content validity and rigor of hypothesis testing; satisfactory for criterion validity; fair in reliability and responsiveness; but poor in internal consistency and structural validity. There is a paucity of QoL questionnaires for caregivers of otherwise healthy children suffering from physical injuries and acute conditions associated with significant caregiver burden. No ILI-specific QoL questionnaires were identified. The present study employed a mixed methods approach, which consisted of a quantitative survey and qualitative interviews. Healthy children aged 6-36 months were recruited from 90 childcare centres and 2 paediatric-focused general practices in Sydney, Australia, during 2010. For the quantitative survey, QoL of parents of children aged 6-36 months prior to the 2010 influenza season using the PAR-ENT-QoL questionnaire (ear-nose-throat-specific) was measured, then subsequently for parents of children with ILI (designated as the ILI group) using the SF-12v2 Acute Form (generic) and PAR-ENT-QoL, and contemporaneously for parents of aged-matched children without ILI (designated as the non-ILI group). Of 381 children enrolled from 90 childcare centres, 105 developed ILI. Parents had significantly lower QoL during the period of their child’s ILI, compared with the period before ILI and with parents of children without ILI. Two factors were significantly associated with parental QoL: total time spent caring for their child during ILI and whether the child had severe or non-severe ILI. Correlations of PAR-ENT-QoL and SF-12v2 scores were satisfactory. Qualitative semi-structured interviews were then conducted with parents of children with severe ILI among the same sample group. The aim was to explore parents’ experiences and the impact on their lives while their child had ILI. A total of 21 interviews were conducted with 23 parents. Interviews were recorded, transcribed, and analysed using a modified grounded theory approach. Five main themes emerged: ―sudden changes in daily life‖, ―emotional impact‖, ―social isolation and relationship changes‖, ―importance of family and friend support‖, and ―interaction with the medical system‖. Perceived practical and emotional support from family and/or friends and consultations with the treating doctor appeared to moderate the impact. Based on the systematic review (described in Chapter 2), as well as findings from the quantitative survey and qualitative interviews, Care-ILI-QoL (a new ILI-specific parental QoL questionnaire) was drafted and developed. Healthy children aged 6-48 months were recruited from 48 childcare centres in Sydney, Australia during 2011. The Care-ILI-QoL and SF-12v2 Acute form instruments were administered in the 2011 influenza season to parents of a sick child 2 weeks after the onset of ILI, and again 2 weeks after the child had recovered. Out of the 125 children enrolled, 55 children had one or more episodes of ILI (total 75 ILI episodes). After undergoing item reduction procedures and exploratory factor analysis, Care-ILI-QoL was reduced from 25 to 16 items covering 4 factors: Daily Activities; Perceived Support; Social Life; and Emotions. Care-ILI-QoL has satisfactory concurrent and discriminant validity, good internal consistency, and excellent responsiveness. This thesis promotes a more holistic and profound understanding of the impact of ILI in children on their families. Care-ILI-QoL is the first ILI-specific QoL instrument for parents and is demonstrated to be valid and reliable in a developed country setting where the child is affected by ILI. It can provide a quantitative indicator for psychological and social impacts of ILI, has the potential to be applied in clinical and research settings to assist measurement of disease burden, or as a needs assessment vii tool for resources such as social support; to inform policy changes in domains such as the workplace or childcare centres, or prioritisation of vaccine development. It also has the potential to be derived as a preference-based single index measure for economic evaluation. It is hoped that the present research will contribute to a long-term outcome whereby parents experience only minimal impact in their QoL when their child has ILI.

This study examines the ways in which the education of children of undocumented immigrants is impacted by their parents’ undocumented status as well as their personal documentation status. In this thesis, I shed light on the experiences of fifteen college students and alumni who are variously-documented, including: undocumented students, DACA students, and documented students with undocumented family members. Drawing on these experiences, I argue that students’ education is impacted by legal and social exclusion, family stress and psychosocial impacts, personal legal status, and a lack of services for undocumented students and children of undocumented immigrants in educational institutions. I also argue that mothers and other family members lessen these challenges by providing students with the support and encouragement to complete a higher education. I conclude this study by making specific recommendations for what K- higher education institutions can do to better serve undocumented and documented students within mixed-status families.

Streszczenie:
For several decades, there has been debate over whether white parents can help adopted minority ethnic children to develop a "positive" ethnic identity and cope with racism. Such debates are particularly complex for mixed ethnicity children, as there are particular practical and conceptual difficulties involved in finding them ethnic "matched" adoptive placements. Underpinned by the sociology of childhood and children's rights-based perspectives, this study addressed three research aims: a) how mixed ethnicity adopted children viewed their ethnic identities, particularly in relation to their adoptive identities; b) the children's experiences of discrimination; and c) their adoptive parents' ethnic socialisation approaches. Semi-structured interviews were carried out with adoptive parents (n=18) and children (aged 6-16, n=11). The children's adoptive identity appeared to be a more salient feature of their lives than their ethnicity. Despite most of the children reporting that they had suffered from racism and bullyi ng, the majority of adoptive parents (BME and white) had not prepared their children to cope with discrimination. However, most of the adoptive parents had actively tried to teach their children about their cultural heritage. Adoptive parents had used three different approaches to cope with the ethnic differences in their families: the "colourblind approach", the "finding similarities and acknowledging differences" approach, and the "Iegitimising differences" approach. The findings suggest that adopters need particular pre-adoption training and support to help them to prepare their children for discrimination. However, the salience of adoption in the families' lives suggest that practitioners should not focus on issues relating to children's ethnic identities and side line issues in relation to the children's adoptive identities when selecting, assessing, preparing and supporting adoptive parents.

Streszczenie:
This dissertation investigated the roles of early childhood fundamental motor skills (FMS) competence on physical, cognitive, and psychosocial health outcomes among underserved preschoolers in Head Start, and examined parental influence on their children's FMS competence. An explanatory sequential mixed methodology was used to examine the predictive strength of FMS competence on physical, cognitive, and psychosocial health outcomes among 216 underserved preschoolers from six Head Start centers (Mage = 4.32, SD = 0.63; girls 56.5%). This methodology allowed for a follow-up qualitative aspect to explore the influence of parents' perceptions and behaviors on their child's FMS competence and health outcomes in a subsample of eight parent–child dyads who demonstrate high or low FMS competence in the quantitative data. The results of this dissertation suggest that preschoolers' FMS competence, especially locomotor skills, were associated with and predicted various health outcomes in sedentary behavior (β = -0.21), light physical activity (β = 0.23), executive function (β = -0.21), and perceived motor competence (β = 0.34). No significant influences of FMS competence on moderate-to-vigorous, body fatness, HRQoL were found (p > 0.05). We also found that positive parental influences (role modeling, support, and facilitation) were observed more often among preschoolers in the high FMS competence group. In addition, preschoolers with single, divorced, or young parents (age range 18–24) tended to show low FMS competence. The analysis of children's drawing pictures also indicated differences in images related to the moments when they played games or sports with parents based on their FMS competence levels. Practical implications and future research direction are discussed.

Human trafficking is a global problem and a multi-billion dollar industry. Most victims are women and girls and more than half are children. In the United States, many at risk youth continue to attend school with school nurses on the frontlines. Using the Peace and Power Conceptual Model, a mixed methods study was conducted to explore their awareness, attitudes, and role perceptions in prevention of commercial sexual exploitation of children (CSEC). Two factors related to increased awareness, and positive attitudes and role perceptions to prevent of CSEC included prior exposure to working with vulnerable students, and prior education about CSEC. Two factors that inhibited identification of CSEC included an uncertainty in identifying CSEC, and a lack of collaboration with colleagues in schools. Four sub-themes were identified; ‘exposure/knowledge, ‘collaboration’, ‘role boundaries’, and ‘creating respite space’. Future research should target the multidisciplinary school team. Simultaneous policy efforts should focus on improving practice conditions for school nurses to support their role in identification and intervention to prevent CSEC among at risk youth.

Since 2015, a U.S. Agency for International Development and Virginia Tech Education and Research in Agriculture collaboration has introduced and tested mung bean as a potential crop to alleviate malnutrition and food insecurity in Senegal. This MS thesis describes a study conducted to assess the impact of mung bean on dietary diversity of Senegalese women and children in the Kaolack, Matam and Bakel localities of Senegal. A mixed-methods research approach included individual surveys to determine dietary diversity scores (DDS) and focus groups to assess the perceived impacts of mung bean. The dietary diversity survey was conducted with 194 participants including adult women, ages 15 to 70 years (n=109) and children, ages 0-10 years (n=85). Half (52%) of the population were mung bean consumers. The dietary diversity surveys revealed an average DDS of 5.73 on a scale of one to 10, with 5.83 and 5.62 for mung bean and non-mung bean consuming groups, respectively. There was a statistically significant difference in DDS between mung-bean consuming women and both mung bean and non-mung bean children, and between mung bean and non-mung bean consumers in Bakel; however, there was no significant difference between overall mung bean and non-mung bean groups DDS. Focus groups (n=11) with mung bean consuming women identified perceived agricultural, health, and financial benefits associated with mung bean consumption. These results can increase our understanding of how mung bean may influence policy-relevant issues for the Senegalese population, including agricultural, health and financial outcomes that are not reflected in dietary diversity surveys.

This paper explores photography in children’s picturebooks and its ability to extend image-making and reading by creating a hybrid genre that merges real and non-real worlds. In analysing the use of photography in such a hybrid genre, the work of Lauren Child (2006, 2001a, 2001b, 2000), Polly Borland (2006), Shaun Tan (2007, 2000, 1998) and Dave McKean (2004a, 2004b, 1995) is deconstructed. These artists utilise photography in contemporary picturebooks that are fictional. In addition, David Doubilet’s images (1990, 1989, 1984, 1980) are discussed, which fuse underwater photojournalism with art, for factual outputs. This research uncovers a gap in picturebook literature and creates a new hybrid by merging genres to produce a work that is both factual and fictional. The research methodology in this study includes a brief overview of photography and notions of truth, contemporary picturebook trend theory, use of a student focus group, industry collaborations and workshops, and environmental education pedagogy. This thesis outlines summaries of research outcomes, not the least of which is the capacity for photography to enrich narrative accounts by providing multilayered information, character perspectives and/ or a metafictive experience. These research outcomes are then applied to the process of creating such a hybrid children’s picturebook.

Pełny tekst źródła
Streszczenie:
Oral narrative ability has received increasing attention over the past three decades, and the importance of children's oral narrative skills to academic achievement has been well established. Children with reading disabilities are known to demonstrate difficulties in the ability to produce and comprehend oral narratives (Roth & Spekman, 1986; Snyder & Downey, 1991). However, the nature of the relationship between oral narrative ability and reading comprehension performance in children with reading disability is not clear. The experiments reported in this thesis aim to address this issue. The following questions are asked: 1) Do deficits in oral narrative ability contribute to reading comprehension difficulties? and 2) What is the likely direction of the relationship between aspects of oral narrative ability and reading comprehension performance in children with reading disabilities? Fourteen children (aged between 6;4 and 7;8 at the initial assessment) with mixed reading disability (MRD: i.e., children who demonstrate both word recognition and listening comprehension deficits) participated in the study. Their oral narrative skills were compared to those of their chronological age-matched peers with typical development (TD) and their reading-age-matched peers with typical development (RMTD). The study consisted of three phases: 1) A longitudinal phase in which the children's oral narrative performance was assessed on three occasions over a two-year period; 2) An intervention phase (using a nonequivalent pretest-posttest control group design) in which 10 children participated in an oral narrative intervention program that focused on enhancing children's story structure knowledge; and 3) A follow-up assessment phase conducted eight months post-intervention. Oral narratives were elicited in a personal narrative context and in a story retelling context. Oral narrative production ability was analysed at macrostructure (story quality) and microstructure (morpho-syntax and vocabulary) levels. Oral narrative comprehension was assessed in a fictional story context through questions relating to story structure elements. Reading comprehension performance was assessed using a standardised test of reading ability. The results from the longitudinal study showed that the children with MRD demonstrated inferior oral narrative production and oral narrative comprehension performance compared to their peers with typical reading development at each assessment occasion. When comparing the poor readers' performance to the RMTD group at the third assessment trial, the results suggested that the children with MRD demonstrated a specific deficit in oral narrative comprehension. In contrast, a pattern of delay was observed on the microstructure measures of oral narrative performance. The results from the intervention indicated significant treatment effects with large effect sizes for oral narrative comprehension performance. Despite this improvement in oral narrative comprehension, there was little change in oral narrative production ability, and transfer to reading comprehension was not evident. Although the follow-up assessment indicated sustained improvement in oral narrative comprehension for the children with MRD, accelerated reading comprehension progress was not evident. The findings from the longitudinal case study highlighted the benefits of oral narrative intervention for a child considered at high risk of continuing academic difficulties. This thesis provides evidence of the persistent oral narrative difficulties in children with MRD. The findings also provide support for the importance of narrative structure knowledge to these poor readers' oral narrative comprehension performance. The results demonstrate that oral narrative comprehension ability explains only a small amount of the variance in reading comprehension performance. Rather, the persistent word recognition difficulties of the children with MRD exert the biggest influence on their reading comprehension performance. These results are discussed in terms of current models of reading and language development. Implications for clinical practice are also addressed.

Streszczenie:
Introduction: Despite the importance of pharmacokinetic (PK) information for patient management there are low numbers of paediatric PK studies and little guidance available on optimum study design and conduct. Method: Drawing on Implementation Science, a mixed-methods study was conducted, including a scoping review (SR) (PK literature: 1990-2015) and quantitative and qualitative inquiry (stakeholders: lay population, service users and health-care professionals). Aim: to explore the feasibility and acceptability of paediatric PK research. Results: The SR (203 papers) highlighted significant problems with participant recruitment, retention and sampling. Stakeholders (n=240) added insight into these phenomenon, with lack of research staff, additional blood-sampling and appointments highlighted as significant barriers to recruitment and conduct. Facilitators included sensitivity and timeliness of approach, communication, involvement of child/young person (CYP) in decision-making, engagement between research and clinical teams, reassurance of safety, pain minimisation, and avoidance/reduction of burden to the CYP and family. Dedicated research support was viewed as critical to success. Discussion: PK research was viewed as feasible and acceptable by service users and health professionals, even in the context of critical illness. Novel, evidence-based, patient-centred, recommendations for future PK study conduct and design have been generated which are applicable for those designing, approving and implementing PK research.

Pełny tekst źródła
Streszczenie:
Studies comparing musically trained and untrained children and adults provide evidence of structural, functional and behavioural changes associated with experience-specific adaptation within the cortical and subcortical sensory-motor neural networks. Researchers have suggested that changes associated with musical learning may transfer to near domains (e.g. fine motor ability) and/or far domains, such as general intelligence. However, few studies have considered the concomitant development of a range of cognitive, behavioural and socio-emotional measures reflecting emerging musicianship. No other study has attempted to situate these findings within the context of adult musicians’ experience. Two studies are presented here; firstly a quantitative longitudinal quasi-experimental investigation of multiple measures of musicianship. 19 children received only statutory school music group lessons over one academic year, and another 19 children received additional extracurricular musical instrument lessons for the first time during that year. A battery of tests included measures of aptitude, intelligence, memory, motor abilities and parental and teacher reports of socio-emotional behaviours. Results showed musical training enhanced hand-eye coordination and fluid intelligence, replicating and extending previous studies. The second study is a qualitative grounded theory investigation of a range of 28 adult musicians reflecting contemporary working musicians in the U.K. This includes nonconformist and popular musicians as well as conductors and music producers. They reflected upon what it is to be a musician, and what qualities they were aware their experiences had brought to their lives. A musicians’ model of musicianship emerged which challenges assumptions relating to the concept of transfer effects. The data generates new hypotheses that musical learning supports and encourages flexible cognitive and behavioural skills and creativity that are further enhanced by the concomitant experience of nonverbal communications encompassing music and socialisation.

This study examined the relationship between child rearing practices and ethnic identity in interracial and Yoruba children in Nigeria. An in-depth study of 20 interracial and 20 Yoruba families was conducted. Three methods of measuring ethnic identity (a doll choice technique, Draw-A-Person and 'Tell me about yourself') were administered to 20 interracial and 20 Yoruba children (aged 6-9 years) in the home environment. A questionnaire was administered to the mother in each family to measure the extent of use of 'elite' Yoruba child rearing practices. A separate questionnaire was'also administered to fathers. The mothers' questionnaire responses were correlated with the children's test-results. The doll choice technique and Draw-A-Person were found to be more successful than 'Tell me about yourself'. However, an additional 'food preference' measure was extracted from 'Tell me about yourself'. The results indicated that Yoruba children showed a stronger Yoruba identity than interracial children and that the majority of children in both groups showed correct owngroup identification. As expected, Yoruba mothers were found to use more Yoruba child rearing, practices than interracial mothers. A significant relationship between child rearing practices and ethnic identity was found in the interracial group but not in the Yoruba group. Correlations between doll play, Draw-A-Person and the food preference measure were generally low. Race of experimenter (white and Yoruba) did not affect children's test results. Results were interpreted within a family interactions framework and with considerations given to the social and cultural background of the subjects. It was suggested that socialization may be important for the development of ethnic identity in the 'minority' interracial children but not in the 'majority' Yoruba children. The implications of this finding for interracial children in other societies and for other ethnic minority groups was discussed.

Streszczenie:
This research is a qualitative, heuristic study involving in-depth interviews with eight white, professional heterosexual women in interracial relationships. The women were found through an opportunistic or snowball approach. The participant women were in the age range 25-60. Six were married and two were in long term relationships. All women had children, seven having mixed-race children between 18 months and 23 years of age. Four women had partners of African-Caribbean heritage, three had partners of African heritage and one had a partner of Nepalese heritage. The women shared their reflections on having to confront the realities of racism, coming to terms with their own ambiguous racial position, facing the notion of whiteness and considering their social position as white women. The research was conducted using a heuristic methodology to explore white women's experiences, using creative images and personal reflective and reflexive narratives integrated throughout the text. The research offers insight into how the social experiences of being in an interracial relationship impacts on white women; as individuals, partners and in their role of mother. Implications for themselves as mothers and parenting their children in a racist context are explored and discussed. The findings suggest the women can feel caught between the known (whiteness) and the unknown (blackness). Having crossed a 'socially unaccepted racialised boundary' and challenging explicit dominant social, gendered and racialised beliefs, the women stepped into the unknown involving experiences of changes in status, challenges to assumptions of their maternal competence and living in a world which involved a continuous process of deconstruction and reconstruction of a new, unforeseen racialised identity. The white women moved from being an 'insider' within their own dominant social experiences, to becoming an 'outsider' within another cultural context, sometimes experiencing uncertainty about where they belonged. The white women experienced a shift of reference group orientation, with a new experience of continuous external scrutiny unfolding. These newly encountered social and personal events challenged the white women to review how they previously saw themselves, with this all impacting on their previously taken for granted social status. These experiences impacted at emotional and cognitive levels. As a consequence, the white women often found themselves occupying a liminal or unknown space where a process occurs of attempting to come to terms with the new experiences, new learning and adopting alternative strategies to deal with these different experiences. Implications for counsellors working with white women in interracial relationships are considered and suggestions for therapeutic engagement are made.

Streszczenie:
The overall objective of this study was to understand the use of services by parents of children with an autism spectrum disorder in an inner London area. Children for this study were aged between four to eight years. A concurrent mixed method design was adopted. The primary investigation involved a quantitative approach to identify factors that were associated with and predictive of service use. Child factors (of neuropsychiatric symptoms, behavioural disturbances, autism symptomatology, and IQ), parental factors (ofparenting stress and depression), and socioeconomic factors were included in the quantitative investigation. Five support types from different services were included. Individual interviews with a small number of purposiveJy selected parents also took place to explore their views and beliefs that might be related to service use decisions. As predicted, ASD children of this sample were reported to have a high level of neuropsychiatric symptoms and behavioural disturbances. The level c;f parenting stress and risk of depression were clinically significant in a majority of this parent sample. IQ was not related to any child and parental factors. As also expected, a high level of service use was found in this parent sample, in that a majority of parents reported use of multiple services. Users and non-users of CAMHS and Social Care were analysed. Of all associated factors, the level of behavioural disturbances was the only factor found to be significantly predictive of CAi\1HS use. Predictive factors for Social Care use included parenting stress, intellectual impairment, and speaking English at home. Information from individual interviews with parents revealed high level of demands and challenges faced by parents. Issues related to service use decision were highlighted, including I 1 1 1 I j USE OF SERV1CES BY PARENTS OF ASD CHlLDREN J J difficulties finding out services thal were available and equality of access. Implications for clinical practice, future research directions and service development were discussed.

Streszczenie:
This intersectional study explored how mothers of mixed race children negotiate racialised and classed bordering practice in two neighbourhoods within a provincial city in the UK. The research was influenced by Critical Race Theory. It employed a critical interpretation of Bourdieusian symbolic capital to look at how localised spatial dynamics impact on the social geographies of mothers . of mixed race children. Grounded in ethnographic principles, the study explored spatial practice within the two localities as research participants produced a visual representation of the places they took their children in the neighbourhood. Some families participated in a 'go-along', a type of participant observation, in which they were accompanied on a routine trip with their children. This allowed participants to visualise spatial dynamics in real space and time. The study found that classed habitus affects the neighbourhood geographies of families with mixed race children; with those ascribed as middle class having more spatial mobility and ascribed cosmopolitan habitus, than those assigned as lower class. However, working class parents of mixed race children also shared a cosmopolitan habitus and them, and their children, whilst having much less mobility, had more ethnically diverse friendship groups than middle class participants. The study concluded that families with mixed race children make choices about household residence based on an assessment of the social geographies of different localities. Whilst they remain interested in the level and composition of minority ethnic diversity, they are influenced by the intersection of a range of other social differences such as socio-economic differences, symbolic capital (habitus) and geography. New data emerged about potential working class 'edge areas'. These neighbourhoods where families with mixed race children had settled had average numbers of ethnic minority population and residents ascribed as more 'tolerant' towards those racially different, than in other working class urban areas.
