Background. Few charitable overseas surgical missions produce cost-effectiveness analyses of their work. Methods. We compared the pre- and postoperative health status for 157 total hip arthroplasty (THA) patients operated on from 2007 to 2011 attended by an annual Canadian orthopedic mission to Ecuador to determine the quality-adjusted life years (QALYs) gained. The costs of each mission are known. The cost per surgery was divided by the average lifetime QALYs gained to estimate an incremental cost-effectiveness ratio (ICER) in Canadian dollars per QALY. Results. The average lifetime QALYs (95% CI) gained were 1.46 (1.4–1.5), 2.5 (2.4–2.6), and 2.9 (2.7–3.1) for unilateral, bilateral, and staged (two THAs in different years) operations, respectively. The ICERs were $4,442 for unilateral, $2,939 for bilateral, and $4392 for staged procedures. Seventy percent of the mission budget was spent on the transport and accommodation of volunteers. Conclusion. THA by a Canadian short-stay surgical team was highly cost-effective, according to criteria from the National Institute for Health and Care Excellence and the World Health Organization. We encourage other international missions to provide similar cost-effectiveness data to enable better comparison between mission types and between mission and nonmission care.

Objective To address whether a multidisciplinary team of pediatric otolaryngologists, anesthesiologists, pediatric intensivists, speech-language pathologists, and nurses can achieve safe and sustainable surgical outcomes in low-resourced settings when conducting a pediatric airway surgical teaching mission that features a program of progressive autonomy. Study Design Consecutive case series with chart review. Setting This study reviews 14 consecutive missions from 2010 to 2019 in Ecuador, El Salvador, and the Dominican Republic. Methods Demographic data, diagnostic and operative details, and operative outcomes were collected. A country’s program met graduation criteria if its multidisciplinary team developed the ability to autonomously manage the preoperative huddle, operating room discussion and setup, operative procedure, and postoperative multidisciplinary pediatric intensive care unit and floor care decision making. This was assessed by direct observation and assessment of surgical outcomes. Results A total of 135 procedures were performed on 90 patients in Ecuador (n = 24), the Dominican Republic (n = 51), and El Salvador (n = 39). Five patients required transport to the United States to receive quaternary-level care. Thirty-six laryngotracheal reconstructions were completed: 6 single-stage, 12 one-and-a-half-stage, and 18 double-stage cases. We achieved a decannulation rate of 82%. Two programs (Ecuador and the Dominican Republic) met graduation criteria and have become self-sufficient. No mortalities were recorded. Conclusion This is the largest longitudinal description of an airway reconstruction teaching mission in low- and middle-income countries. Airway reconstruction can be safe and effective in low-resourced settings with a thoughtful multidisciplinary team led by local champions.

Since 1892, seven versions of the national geological map of the Republic of the Ecuador have been published. In 1892, Dr. Theodor Wolf, of the German Scientific Mission, published the 1st map which included a preliminary geomorphological description. Later, Dr. Walther Sauer with the support of the Universidad Central del Ecuador (Central University of Ecuador), published a 2nd version in 1950, and then an update in 1970, both included oil wells information. In 1969, the Servicio Nacional de Geología y Minería del Ecuador (National Service of Geology and Mining of Ecuador), and Institut Français du Pétrole (French Petroleum Institute), published the 3rd version which included a mineralogical index. The 4th version was developed by the Dirección General de Geología y Minas del Ecuador (General Directorate of Geology and Mines of Ecuador) and Institute of Geological Sciences & Natural Environment Research Council, and was published in 1982, it included a geological model from a plate tectonics approach. Later, in 1993, the Corporación de Desarrollo e Investigación Geológico Minero y Metalúrgica del Ecuador (Geological, Mining and Metallurgical Research and Development Corporation of Ecuador) and the British Geological Survey published a 5th version, which included an overview of mineral occurrences on the Ecuadorian Andes. On the 21st. century, in 2001 was published the 6th version, that consisted of a reviewed national geological map, which included data transferring to the geographical information system ArcGis. Ultimately in 2017, the Instituto Nacional de Investigación Geológico Minero Metalúrgico (National Institute of Geological-Mineral-Metallurgical Research), published the 7th version, incorporating the last updated geological results of previous campaigns. Every version contributed to the understanding of the regional geodynamic framework. Nevertheless, in the last two decades, informal coding of units for the geological cartography, thrived, involving a lack of characterization and descriptions of representative outcrops, along with uncertainties on geochronological control for newly mappable defined units, reduce reliability on the national geological cartography, increasing the uncertainties for the understanding the regional geodynamic framework.

Introduction: Based on Duffy’s (2009) Quality Caring Model and Hill and Watson’s Caring Science Curriculum Model (2011), the School of Nursing embraces the philosophy that caring collaborative relationships are the center of a culture of quality caring in nursing. Culture and cultural competence are integral to caring. Immersion experiences have been identified as impacting cultural competence (Bentley & Ellison, 2007; Kohlbry, 2016; Larsen & Reif, 2011; Long, 2012). Methodology: This paper describes the planning, implementation and lessons learned from an immersion experience as part of a mission focused study abroad course to Quito, Ecuador. Discussion: Twelve students, three faculty, and two nurse practitioners cared for approximately 100 patients per day for 10 days. Lessons learned related to faculty, students and logistics are discussed. Results: Students demonstrated positive outcomes related to critical thinking and clinical reasoning, holistic care of diverse populations, communication and collaboration, professional accountability, and effective teaching strategies.

PurposeThe purpose of this qualitative study is to explore the role of spirituality, understood as a personal relationship with God, in missionary intercultural experience.Design/methodology/approachWe conducted narrative interviews with eight Polish consecrated missionaries in Argentina, Bolivia, Ecuador, Peru and Paraguay. We used thematic analysis to establish spirituality in missionary experience and narrative analysis to examine sensemaking processes.FindingsMissionary spirituality was defined by a personal relationship with God as a source of consolation, psychological comfort, strength to cope with distressing experiences, and Grace promoting self-improvement. It compensated for the lack of family and psychological support and enhanced psychological adjustment to the environment perceived as dangerous. Spirituality helped missionaries deal with cultural challenges, traumatic and life-threatening events. Traumatic experiences furthered their understanding of the mission and triggered a spiritual transition that entailed a change in their life, attitudes and behavior.Research limitations/implicationsComparative research into religious vs nonreligious individual spirituality in the experience across various types of expats in various locations could capture the professional and cultural specificity of individual spirituality. Research is also needed to link spirituality with expat failure.Practical implicationsCatholic agencies and institutions that dispatch missionaries to dangerous locations should consider providing professional psychological assistance. Narrative interviewing could be used to enhance missionaries' cultural and professional self-awareness, to better serve the local community. Their stories of intercultural encounters could be incorporated into cross-cultural training and the ethical and spiritual formation of students and future expats.Originality/valueThis study captures a spiritual aspect of intercultural experience of under-researched expats. It offers a model of the involvement of individual spirituality in coping in mission.

In the era of space travel, ecology has continued to gain relevance as the science of the ‘spaceship Earth’. In this context, it can be said to have a mission, which is to understand the complex network of the life-support systems that keep biospheric processes operating in a way suitable to sustaining living organisms and their environment (Odum & Sarmiento 1997). With the realization that ecology, in the broad sense, provides the means to understand the mechanics of nature, scholars are using ecological understanding at the interfaces of disciplines to: (1) prevent and reverse the demise of biodiversity in marine and terrestrial ecoregions (conservation biology), (2) reduce impacts of population pressure on the resource base of people (ecological anthropology), (3) establish more parsimonious economic activities to ensure optimum yields for the long term (ecological economics), (4) plan for an appropriately-equitable and socially-integrative sustainable development (environmental design), (5) restore degraded ecosystems and landscapes (restoration ecology), and (6) model hypothetical future scenarios where predictions from ecological theory may prove valuable for the future of mankind (environmental planning).

Abstract: This paper analysis of the System of Human Resource Management offered by the Industrial Engineering Portoviejo-Ecuador to boost productivity of business. So that the graduate will perform effectively and allows the academy to deliver effective specialist industrial sector, which would contribute to improving the living standards of the National Development. Study developed in two parts. The first part is about the competition for university students that implement principles of human talent that stimulate productivity of business. The second part related the curricula used according to the management of human talent used in business, considering the competitive aspects of the work, mission and vision pursued in the industry. The methodology involves the technique personal interview executives of the division of human talent of business in the city of Portoviejo-Ecuador to determine accurate information on the reality that crosses this area of human knowledge. The main results of the research, product-processing fieldwork include establishing qualitative principles to optimize the system for management of human talent competition for Industrial Engineering at the Universidad Técnica de Manabí (UTM) -2015 Index Terms: academy, talent, industry, productivity.

Dessa samfund eftersom de alla delar samma teologiska rötter. Covenant Church och Pactokyrkan är båda direkt och indirekt frukt av Svenska Missionskyrkan. Metoden som använts är en jämförande kvalitativ studie där samfundens dokument kring nattvarden varit centrala. Resultatet visar att kyrkorna delar synen på att Jesus instiftade nattvarden åt sina lärjungar kvällen innan Han korsfästes. De delar också syn på att nattvarden är ett tecken på försoningen i Honom. Covenant Church och Svenska Missionskyrkan har båda öppna nattvardsbord och Pactokyrkan har ett stängt nattvardsbord.

Chapter 5 takes a closer look at three of the most important sets of actors involved at a national level in defining the agenda for migrant integration in Ecuador: the Ecuadorian state (especially the Ministry of Foreign Relations and Plan Ecuador), the United Nations system (especially UNHCR and UNDP) in Ecuador, and the Catholic Church. By tracing the institutional development and interaction of these three sets of organizations, the chapter seeks to understand how the differing political incentives and institutional missions and structures of each component of the migration governance network have impeded or promoted human security and peacebuilding. The chapter identifies trust, mission, capacity, and transnational linkages as key factors influencing outcomes. Recognizing the importance of change over time, the chapter traces three distinct phases within the time frame of 2000–2017: Absence, Coordination, and Regression.

This chapter deals with early criticisms of SIL’s work among the Waorani from outsiders, as well as dissatisfaction among some Waorani in Tewæno with the atmosphere created by Dayomæ and Rachel Saint. The 1971 Barbados Declaration insisted that Christian missions were hostile to indigenous cultures. It provided justification for other critical voices, including German-born Erwin Patzelt in Ecuador and writer Laurie Hart in the US. In Tewæno, after a power struggle with Dayomæ, a Wao woman named Zoila Wiñame and her followers left Tewæno to establish Dayono, a community with no ties to SIL. SIL sought to curb the influence of Saint and Dayomæ, in part by adding more staff: medical worker Rosi Jung and literacy instructor Pat Kelley. Evangelicals in the US continued to celebrate Saint as a great missionary, and they flocked to a series of “Auca Update Rallies,” featuring Saint and three Wao Christians.

This chapter explores the tension and competition between various missionaries and missions agencies in Ecuador as each group tried to position itself to contact the Waorani. Rachel Saint and Elisabeth Elliot reconciled for the first of many times before Rachel and Dayomæ traveled to the US for This Is Your Life and speaking engagements, a trip that lasted a year. Dayomæ converted to Christianity and was baptized by V. Raymond Edman, president of Wheaton College, where three of the missionaries killed by the Waorani had attended. In Ecuador the missions first involved in the Waorani project continued efforts to make contact. Mission Aviation Fellowship resumed flights over Wao clearings. Brethren missionary Wilfred Tidmarsh ventured near Waorani territory. In November 1957 two Waorani women, Mintaca and Mæncamo, came out of the rainforest. Elisabeth Elliot befriended them, brought them to live with her, and sought to learn the Wao language.

Abstract This study draws upon two sources of primary data: self-administered questionnaires completed by 129 Mission members and open-ended interviews conducted with 107 Mission members. These offer complementary ways of looking at the origins of evangelical missionaries. The questionnaire survey, on the one hand, provides uniform background information including sociodemographic characteristics, the occurrence of “separating” events in childhood (such as migration, going away to boarding school, parental death), and religious milestone experiences (such as conversion and calling). Responses were given by 75 women and 54 men between the ages of 24 and 64 who began working as HCJB missionaries in Ecuador between the years 1941 and 1983. When summarized statistically, they yield one sort of answer to the question of where the people that I studied “came from.”

Building an argument from legal documents and interviews with activists, the chapter advances a definition of drone technicity. The case study is the convergence of technologies and conservation in a 2017 Sea Shepherd Conservation Society mission to use drones to collect evidence of an illegal slaughter and transshipment of sharks from the waters surrounding Timor-Leste and the Galápagos National Park, Ecuador, to China. While the drone assisted this conservation, ultimately the marine poachers escaped with impunity because of a bribe. In the process, this chapter argues that the capacities of conservation technologies are moderated by politics, in this instance, corruption.

Abstract The chief obstacle to most studies about Latin American Jewry, particularly of the smaller communities, is the absence of reliable information. Obtaining data on small Jewish minorities in Latin America is an almost impossible mission. For instance, the Jewish community in Quito, Ecuador, is not even mentioned in the important recent survey on Jews in Latin America as presented in the American Jewish Year Book (1985).Our purpose is to present and interpret recent information on the demographic and economic structure of the Jewish households in Quito.The community of Quito was selected for two reasons: first, it is small and relatively accessible for investigation; second, it is still, to a large extent, a homogeneous community in which many of the methodological hazards of misinterpretation can be avoided.

Reading understanding is an indispensable ability for the educational training of every citizen. Designates a higher level than the mere fact of reading (decoding lingülistic signs). While reading already implies a great cognitive advance for anyone, reading comprehension requires more skills. Understanding a text always requires three previous areas. Two of them refer to basic functions: memory and attention. The other to training: is the knowledge of the world.However, even if there are these three elements, if there is not sufficient motivation and training approach to various types of texts, the degree of reading comprehension will be poor.In Ecuador, reading comprehension levels are still low. The standardized tests that the Ecuadorian State has applied to students for admission to public universities for 10 years are evidence of the serious delay in this skill. This situation would also explain the low levels of performance in other subjects, as well as the little cultural and educational development of the general population.The causes that contribute to this low reading comprehension of the Ecuadorian student population are multiple. They range from the high cost of the book, low access to technology, the absence of a network of public libraries throughout the country, an educational system with reluctant teachers to apply innovative methodologies for reading stimulation, etc.The insufficient application of innovative methodologies for reading stimulation has built the myth that reading is boring. Young people do not read because nothing in the social environment invites them to do so demonstrating that it is an authentically attractive and useful activity. In this context, this research set out to build a gamified application for the development of students reading students from 12 to 16 years of age.For the creation of this application, the IPLUS methodology and the Videujeos Roblox Studio Development tool were used. This video game allows to train students reading skills, through the use of exercises resolution in multiple choice questions and the Cloze test. Both modalities are based on understanding a text and selecting the correct answer from a set of options. While multiple choice questions are based on the text, for the Cloze test the words omitted systematically in a written text that contain empty spaces must be located. This anima was implemented in a 3D world in Roblox so that the student can strengthen his ability to reading his reading comprehension.Iplus is a methodology created for the design of serious games for educational purposes. This methodology uses a participatory approach focused on the user where requirements are taken with experts in pedagogy and the subject. The main phases of this methodology are identification, pedagogical objectives, game script, gameplay, and refinement.Regarding the implementation of the serious game, the gamified application contains challenges to strengthen reading comprehension in the player in different themes or missions: Costa, Sierra, and East, the three regions that make up the Ecuadorian territory. Each of the scenarios contains instruction signs, a Start button, a blackboard to enter the correct answer, and response options.This gamified application in Roblox is aimed at being a tool for learning and strengthening reading comprehension in school students close to paying the transform exam, the standardized access test to public universities. This can be done independently by students or being a support tool for teachers and their students since reading comprehension skills are applicable for a lifetime.With this work, the IPLUS methodology and the Roblox Studio software create a thematic environment where reading comprehension becomes a skill resulting from the player's enjoyment has been used. Stimulation and reading shake hands thanks to this application.The results obtained suggest that the gameplay can be improved to offer greater feedback to the user about the search for the correct answer to be such an extensive scenario. Finally, this application can expand its scope by using its playability dynamics to other areas of knowledge such as mathematics, natural sciences, and social sciences.Regardless of the extension of the readings, in the design, the extension of the texts can be adapted as well as their complexity so that the reading skill can be graduated. The application works very well with short texts such as those required by the Cloze test.

This report explores what (if any) diversification strategies national oil and gas companies (NOGCs) are employing to engage in an energy transition, with a focus on four South American countries: Argentina, Brazil, Colombia and Ecuador.The authors identify how four South American NOGCs are preparing to transform in the face of climate change and the energy transition. They do so by looking at these companies’ publicly stated ambitions regarding diversification: they focus in particular on whether and how these companies are leaving fossil fuels behind, as a sign of transforming their core business and moving towards a more structural transformation overall in society. The four South American NOGCs are Ecopetrol (Colombia), Petrobras (Brazil), Yacimientos Petrolíferos Fiscales (YPF, Argentina) and EP Petroecuador (Ecuador). While all four companies are controlled by their national governments, three are also public companies that are traded on domestic and international stock markets. This report can serve as a resource for comparative analysis and to inform transition strategies in oil and gas–producing countries, for energy and finance researchers, professionals working within NOGCs, and policymakers shaping NOGCs’ missions, strategies and investments. Key messages National oil and gas companies, or NOGCs, must transform to survive and fit into a new global dynamic for mitigating the impacts of climate change. While navigating their roles in generating public revenue and domestic employment, enabling public services, and other characteristics, they will also have to overcome institutional barriers in order to accelerate their diversification into non-fossil fuel businesses. Diversification to new low-carbon businesses is a part of but not the priority for decarbonization for four of South America’s NOGCs. These four South American NOGCs have limited capacity to expand into new low-carbon businesses. More research is needed to assess and strengthen South American NOGCs’ preparedness for a transition to a low-carbon future, including factors such as financial, technical and managerial capabilities, and their role in national and global political economies.
